Abstract:
ObjectiveTo study the effect of circFOXK2 targeting miR-4677-3p on the malignant behavior of oral squamous cell carcinoma (OSCC) cells.MethodsThe expression of circFOXK2 and miR-4677-3p in OSCC tissues and adjacent tissues was calculated by RT-qPCR.Si-circFOXK2, si-NC, miR-NC, miR-4677-3p mimic, pcDNA, pcDNA-circFOXK2, si-circFOXK2+anti-miR-NC, and si-circFOXK2+anti-miR-4677-3p were respectively transfected into OSCC cells HSC3.The effects of circFOXK2 and miR-4677-3p expression on the malignant behavior of HSC3 cells were detected by scratch healing, CCK-8, Transwell and colony formation assays.The targeting relationship between circFOXK2 and miR-4677-3p was determined by dual luciferase method.ResultsThe expression level of circFOXK2 in OSCC tissue was significantly higher than that in adjacent tissues (P < 0.01), while the expression level of miR-4677-3p was significantly lower than that in adjacent tissues (P < 0.01).After inhibiting the expression of circFOXK2, the HSC3 cell optical density (OD) value, scratch healing rate, clone formation number and invasion number were significantly reduced (P < 0.01), and the expression level of miR-4677-3p was significantly increased (P < 0.01).After overexpression of miR-4677-3p, the HSC3 cell OD value, scratch healing rate, clone formation number and invasion number were significantly reduced (P < 0.01).MiR-4677-3p was a target gene of circFOXK2.MiR-4677-3p down-regulation significantly reduced the effect of inhibiting circFOXK2 on cell OD value, scratch healing rate, clone formation number, and number of invasion of HSC3 cells (P < 0.01).ConclusionsInhibition of circFOXK2 can suppress the proliferation, migration and invasion of OSCC cells by promoting miR-4677-3p expression.

Abstract:
ObjectiveTo explore the effects of Y-box protein 5 (SOX5) on rat osteoblasts (OB) proliferation and nuclear factor κB (NF-κB) signaling pathway.MethodsThe newborn 24 h SPF grade SD suckling rats were taken, and the OB of the rat skull was separated by enzyme digestion method.Morphological observation and ALP staining were used to identify osteoblasts.The fourth generation OBs with good growth were divided into blank control group, pcDNA3.1 group, pcDNA-SOX5 group, si-NC group, and siRNA-SOX5 group.After 48 h of transfection, qRT-PCR was used to detect the expression of SOX5 mRNA in the cells; MTT method was used to detect cell proliferation viability; ALP activity detection kit was used to measure ALP activity; alizarin red staining was used to observe the formation of calcium nodules; Western blotting was used to detect OB differentiation and the expression of NF-κB signaling pathway-related proteins [Runt-related transcription factor 2 (Runx2), type Ⅰ collagen (collagen Ⅰ), nuclear factor κB p65 (NF-κB p65) and its phosphorylated protein, KB inhibits protein kinase α (IκBα), tumor necrosis factor α (TNF-α)].ResultsCompared with the blank control group, the SOX5 mRNA level, p-NF-κB p65/NF-κB p65, and TNF-α proteins expression in the OB of rats in the pcDNA-SOX5 group were significantly increased (P < 0.05), the OB proliferation activity, ALP activity, the number and area of calcified nodules, Runx2, collagen Ⅰ, and IκBα expression were significantly reduced (P < 0.05);the SOX5 mRNA level, p-NF-κB p65/NF-κB p65, and TNF-α proteins expression in the OB of rats in the siRNA-SOX5 group were significantly reduced (P < 0.05), the OB proliferation activity, ALP activity, the number and area of calcified nodules, Runx2, collagen Ⅰ, and IκBα expression were significantly increased (P < 0.05).ConclusionsSOX5 can regulate the proliferation and differentiation of OB, and its mechanism may be related to the regulation of NF-κB signaling pathway.

Abstract:
ObjectiveTo investigate the expression and subcellular localization of sirtuins 2(SIRT2) in the mouse 1-cell stage zygote.MethodsThe mRNA and protein expression profiles of SIRT2 in the development of mouse 1-cell stage zygote were detected by qRT-PCR and Western blotting.The co-localization of SIRT2 and α-tubulin was observed by cell immunofluorescence method.ResultsIn the mouse 1-cell stage zygote, the expressions of SIRT2 at mRNA level had consistency with protein level.The expression level was increased during the transition from G1 phase to G2 phase, and decreased during the transition from G2 phase to M phase.During the transition from G2 phase to M phase, the localization of SIRT2 was gradually shifted from the cell cortex to the nucleus and localized in the spindle in the mid-M phase, when in the late M phase, it relocated to the cell cortex.ConclusionsThe expression of SIRT2 protein in mouse 1-cell stage zygote is cell cycle phase-dependent.The expression of SIRT2 protein increases in the cytoplasm of mouse 1-cell stage zygote mitotic interphase (G1, S, G2), and the accumulated SIRT2 regulates the dynamics of spindle microtubules under some certain condition and plays a role in the G2/M transition.
Correlation analysis of serum IL-33, TNF-α activity and NF-kB level in peripheral blood lymphocytes with different degrees of coronary heart disease

Abstract:
ObjectiveTo analyze the correlation of serum interleukin 33 (IL-33), tumor necrosis factor α (TNF-α) levels and nuclear factor κB (NF-κB) protein expression in peripheral blood lymphocytes with the severity of coronary heart disease (CHD), and explore its role and value in the development of CHD.MethodsA total of 167 patients (CHD group) admitted to Department of Cardiology with coronary angiography and confirmed as CHD were selected, and during the same period, 27 cases with normal coronary angiography were confirmed as control group(CON group).According to Gensini score, 167 CHD patients were divided into mild lesion group(< 30 points, LS group, 84 cases), moderate lesion group(≥30 points to < 60 points, MS group, 54 cases), and severe lesion group(≥60 points, HS group, 29 cases).Serum IL-33 and TNF-α levels were detected by ELISA, the differences of IL-33 and TNF-α levels in different degrees of coronary artery disease were analyzed, and the correlation between IL-33, TNF-α levels and CHD was analyzed.Meanwhile, the peripheral blood lymphocytes in CHD group and CON group were collected for protein extraction, and NF-κB protein expression was detected by Western blotting.ResultsSerum total cholesterol(TC) level in HS group was increased compared with that in CON group, LS group and MS group(P < 0.05), serum IL-33 and TNF-α levels in HS group were increased compared with those in MS group, LS group and CON group, and these parameters were gradually increased with the aggravation of coronary artery disease(P < 0.01).There was a positive correlation between serum IL-33, TNF-α levels and coronary artery disease(r=0.663, P < 0.01).The NF-κB protein expression in CHD group was significantly higher than that in CON group(P < 0.01).ConclusionsWith the increasing degree of coronary artery disease, serum IL-3 and TNF-α levels were gradually increased, and there was a certain degree of linear correlation.NF-κB protein expression in CHD group was significantly increased compared with that in CON group.

Abstract:
ObjectiveTo investigate the application value of transcutaneous oximeter and distal perfusion catheter (DPC) in acute limb ischemia (ALI) complications of venoarterial extracorporeal membrane oxygenation (VA-ECMO).MethodsA total of 50 patients who received VA-ECMO treatment were selected as the study subjects.All patients were treated with ultrasound-guided peripheral femoral arteriovenous catheter and indwelling DPC.The patients were divided into ischemia group (n=15) and non-ischemia group (n=35) according to the occurrence of ALI.Transcutaneous oxygen pressure (PtcO2) and transcutaneous carbon dioxide pressure (PtcCO2) of lower extremity on the side of arterial catheterization were monitored, and oxygen challenge test (OCT) was performed.The general clinical data, PtcO2 and PtcCO2 before and after OCT, 10 min-OCT, and PtcO2 and PtcCO2 before and after DPC access were compared between the two groups.The predictive value of PtcO2 and PtcCO2 was evaluated by receiver operating characteristic (ROC) curve.ResultsPtcO2 was significantly increased after OCT in patients without ischemia, resulting in statistically significant differences in PtcO2 and 10 min-OCT between the two groups (P < 0.01), while there were no significant changes in PtcO2 and PtcCO2 after OCT in patients with ischemia (P>0.05).There were statistically significant differences in PtcO2 and PtcCO2 between the two groups before DPC access (P < 0.01), but PtcO2 was significantly increased and PtcCO2 was significantly decreased in the ischemic group after DPC access, resulting in no statistically significant difference between PtcO2 and PtcCO2 between the two groups after DPC access (P>0.05).ROC curve analysis showed that 10 min-OCT, PtcO2 after OCT, PtcO2 and PtcCO2 before DPC access had good predictive value on the occurrence of ALI, and the area under ROC curve was 0.945 (95%CI: 0.934 -1.000), 0.904 (95%CI: 0.821-0.987), 0.939 (95%CI: 0.870-1.000), 0.874 (95%CI: 0.766-0.983), and pairwise comparison of the ROC curves of the above four indicators showed no statistical significance (P>0.05).ConclusionsTranscutaneous oximeter can be used as an effective means to monitor ALI in VA-ECMO patients, and the occurrence of ALI can be identified earlier by OCT.DPC can significantly improve the lower extremity blood supply of VA-ECMO patients, and it is recommended to be routinely placed during VA-ECMO catheter placement.

Abstract:
ObjectiveTo evaluate the incidence and clinical characteristics in primary Sjögren's syndrome(pSS) with secondary immune thrombocytopenia (ITP).MethodsFour hundred and ninety-eight pSS patients were retrospectively analyzed.Clinical manifestations, laboratory results and immunological index characteristics were compared between pSS patients with or without ITP.ResultsAmong the 498 pSS patients, 17.86%(89/498) had the secondary ITP, and the ITP group showed the higher disease activity and lower C3 levels(P < 0.01) compared with the control group (P < 0.01).In addition, pSS patients with secondary ITP had less arthritis(15.8% vs 5.6%, P < 0.05) and interstitial lung disease(16.5% vs 4.5%, P < 0.05).ConclusionsSecondary ITP is a common complication in patients with pSS.The main clinical characteristics include the decreased complement C3 levels and higher disease activity score.However, the development of interstitial lung diseases and arthritis were independent protective factors for secondary ITP in pSS patients.

Abstract:
ObjectiveTo analyze the clinical data of patients with acute on chronic liver failure (ACLF) and determine the risk factors affecting the short-term prognosis.MethodsThe relevant data of 95 patients with ACLF were collected and divided into the death group (38 cases) and survival group (57 cases) according to the prognosis 30 days after admission.Then the relevant risk factors affecting the short-term prognosis of ACLF patients were analyzed.ResultsThere were significant differences in white blood cell count, neutrophil count, triglyceride, apolipoprotein B, prothrombin time, international normalized ratio, prothrombin activity, cystatin C (CysC), complication with acute kidney injury and the score of model for end-stage liver disease (MELD) between the two groups at admission (P < 0.05 to P < 0.01).Multivariate logistic analysis suggested that CysC was a risk factor for short-term prognosis of patients with ACLF.ConclusionsCysC is an earlier warning factor for ACLF patients complicated with AKI than Cr, which is an independent risk factor for the short-term prognosis of ACLF patients.

Abstract:
ObjectiveTo investigate the brain protective effect of dexmedetomidine combined with ulinastatin on patients undergoing valve replacement under cardiopulmonary bypass.MethodsEighty patients undergoing elective cardiac valve replacement were randomly divided into control group (group N), dexmedetomidine group (group D), ulinastatin group (group U), and dexmedetomidine combined with ulinastatin group (group U+D).Group D was pumped 1.0 μg/kg for 15 minutes before induction, and then 0.4 μg·kg-1·h-1 until the end of the operation.Group U was given 20 000 U/kg intravenously immediately after induction.The usage of dexmetomidine and ulinastatin in group U+D was the same as above, and group N had the same amount of 0.9% sodium chloride solution.Before cardiopulmonary bypass (T1), ascending aorta opening (T2), shutdown (T3), and 6 hours after surgery (T4), blood gas analysis was performed to detect internal jugular vein oxygen saturation (SjvO2), arterial internal jugular vein oxygen content difference (Da-jvO2), and cerebral oxygen uptake rate (CERO2) in four groups of patients, and local cerebral oxygen saturation (rSO2) was recorded.Serum interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), S-100 β, and neuron specific enolase (NSE) were measured by ELISA at the time point T1 to T4 and 24 hours after surgery (T5).Finally, postoperative neurocognition was assessed on the first day before surgery (T0) and the seventh day after surgery (T6), and the incidence of delirium within one week after surgery was statistically analyzed.ResultsAt T2-T3, compared with group N, the levels of SjvO2 were remarkably raised in other three groups, while the leves of Da-jvO2 and CERO2 were remarkably descend; compared with group D and group U, the levels of SjvO2 were remarkably raised in group D+U, while the levels of Da-jvO2 and CERO2 was remarkably descend (P < 0.05 to P < 0.01).At T2, compared with group N, the levels of rSO2 was remarkably raised in other three groups; compared with group D and group U, the levels of rSO2 were remarkably raised in group D+U (P < 0.05 to P < 0.01).At T2-T4, compared with group N, the concentration of NSE, S100β, IL-6 and TNF-α was lower in group D, group U and group D+U; compared with group D and group U, the concentration of NSE, S100β, IL-6 and TNF-α was lower in Group D+U (P < 0.05 to P < 0.01).At T6, compared with group N, the MMSE score of others three groups raised; compared with group D, the MMSE score of group D+U increased (P < 0.01), compared with group U, the MMSE score of group D+U increased, there was significant difference (P < 0.01).At T6, compared with group N, there was no significant difference in the incidence of delirium in group D and group U, but the incidence of delirium in group D+U was decreased within one week after operation (P < 0.01), and the incidence of delirium in group D+U was lower than that in group D (P < 0.05).ConclusionsDexmetomidine combined with ulinastatin can further inhibit the sympathetic activity and inflammatory reaction of patients with rheumatic valvular disease during CPB, significantly improve perioperative cerebral oxygen metabolism, reduce brain injury, and have a certain brain protective effect.

Abstract:
ObjectiveTo investigate the correlation between bone mineral density(BMD) of different positions, fracture risk calculated by fracture risk assessment tool(FRAX®) and body mass index(BMI) in postmenopausal women.MethodsThe demographic information and BMD-related information of 313 postmenopausal women detected by BMD examination in the First Affiliated Hospital of Bengbu Medical University from September 2020 to August 2021 were investigated.The probability of major osteoporotic fractures(including spine, forearm, hip or shoulder) and probability of hip fracture(PHF) over the next 10 years were calculated using the FRAX® tool.The correlation of predicted values of FRAX® and BMD with BMI were analyzed, and it was analyzed whether there was a difference between the FRAX® predicted values of the combined and uncombined femoral neck T value.ResultsThe BMD of lumbar spine, femoral neck and total hip increased with the increase of BMI(P < 0.01).In combination with the femoral neck T value, the risk of fracture decreased with the increase of BMI(P < 0.01).At the BMI < 28 kg/m2 of the predicted value of FRAX® without the T value of the femoral neck, the fracture risk decreased gradually with the increase of BMI(P < 0.05), and the fracture risk began to increase at BMI≥28 kg/m2.Among people with reduced bone mass, it was difficult to screen out those at high risk of fracture, and the FRAX® significantly underestimated the risk of major osteoporotic fractures.ConclusionsThe BMD in people with low body mass is generally low, and the risk of fracture calculated by FRAX® is relatively high.

Abstract:
ObjectiveTo observe the clinical efficacy of platelet-rich fibrin(PRF) combined with Bio-Oss bone substitute in the repair of postoperative bone defect of jaw cyst.MethodsA total of 36 patients with bone defects in a certain range after maxillary cyst surgery were randomly divided into the control group and observation group(18 cases in each group).The control group was treated with Bio-Oss filling to repair bone defects, and the observation group was treated with PRF combined with Bio-Oss filling to repair the cavity of bone defect caused by jaw cyst.All patients were regularly followed up for 3, 6 and 12 months after surgery, and the CBCT imaging was used to measure bone mineral density to evaluate the clinical efficacy.ResultsThe wounds of jawbone cyst in the observation group healed well after surgery, 1 case with mild erosion and redness in the control group was identified after 7 days of surgery, and no rejection reaction of filling material was be observed.The results of imaging examination showed that compared with the control group, the filling materials in the cavity of jaw defect in the observation group grew well with the new bone and surrounding bone tissue over time, and the bone tissue repair effect could achieve clinical satisfactory.The bone mineral density in observation group was higher than that in control group at 3, 6 and 12 months after surgery(P < 0.05 to P < 0.01).ConclusionsThe PRF combined with Bio-Oss bone substitute can effectively promote the osteogenic effect of bone defect after maxillary cyst surgery.

Abstract:
ObjectiveTo explore the application value of hysteroscopic cold knife technique in patients with uterine submucosal myoma of uterus and fertility requirements.MethodsA total of 90 patients with submucosal myoma of the uterus were selected as the study subjects, with 45 patients treated with hysteroscopic cold knife technique as the observation group and 45 patients treated with hysteroscopic electrocautery as the control group.The differences in intraoperative bleeding volume, surgical time, complications, inflammatory indicators (interleukin-6, C reactive protein) before and on the first day after surgery, intrauterine adhesions and endometrial thickness at 3 months after surgery, and pregnancy at 12 months after surgery between the two groups of patients were compared.ResultsCompared with the control group, the observation group had shorter surgical time, lower incidence of complications, lower postoperative inflammatory indicators, good endometrial repair status and higher pregnancy rate (P < 0.05 to P < 0.01).ConclusionsThe hysteroscopic cold knife technique has the advantages of short surgical time, fewer complications, low incidence of uterine adhesions, minimal damage to the endometrium and high postoperative pregnancy rate in the treatment of submucosal myoma of uterus in patients with fertility requirements, which is a minimally invasive surgery worthy of popularization.

Abstract:
ObjectiveTo detect the expression levels of serum matrix metalloproteinase-9 (MMP9) and high-sensitivity C reactive protein (hs-CRP) in patients with middle cerebral artery atherosclerotic stenosis and analyze the value of MMP9 and hs-CRP combined with high-resolution magnetic resonance imaging (HRMRI) parameters in predicting the occurrence of acute stroke.MethodsA total of 183 patients with middle cerebral artery atherosclerotic stenosis were selected and divided into the acute stroke group (89 cases) and non-acute stroke group (94 cases) according to whether acute stroke occurred during hospitalization.Predictive values of stenosis degree, serum MMP9 and hs-CRP levels in patients with middle cerebral artery atherosclerotic stenosis were analyzed.ResultsVulnerable plaque, degree of stenosis, Essen stroke risk score (ESRS) and levels of MMP9, hs-CRP in the acute stroke group were higher than those in non-acute stroke group (P < 0.05).The area under curve (AUC) of degree of stenosis and the levels of MMP9 and hs-CRP in the predictive of acute stroke in patients with atherosclerotic stenosis were 0.886, 0.857 and 0.634, respectively, and AUC of their combination was 0.923.ConclusionsSerum MMP9 and hs-CRP combined with HRMRI parameters can greatly improve the accuracy of prediction in acute stroke.

ObjectiveTo construct a risk nomogram model for predicting the severity of periodontal disease in elderly patients, and to verify the predictive efficacy of the model.MethodsA total of 279 elderly patients with periodontal disease were selected as the research objects, and the relevant data of the patients were collected.Univariate logistic regression was used to analyze the related factors affecting the progression of the elderly patients with periodontal disease to moderate to serve level, and the nomogram prediction model was established.ResultsAmong the 279 cases, 83 cases (29.7%) were mild, 196 cases (70.3%) were moderate and severe.Age ≥70 years old, smoking, diabetes mellitus, unilateral chewing, brushing frequency < 2 times/d were the independent risk factors for the progression of moderate to severe periodontal disease in elderly patients (P < 0.05 to P < 0.01).Based on the above five risk factors, the relevant nomogram model was established and verified.The results showed that the C-index of training set and validation set were 0.734 (95%CI: 0.712-0.758) and 0.717 (95%CI: 0.698-0.738), respectively.The calibration curve and ideal curve fitting of the two sets were well reflected, and the AUC of the two sets were 0.751 and 0.748, respectively.ConclusionsThere are many risk factors for the progression of periodontal disease in elderly patients.The nomogram model based on risk factors has good predictive ability, which can provide reference for early clinical control of periodontal disease.

Abstract:
ObjectiveTo explore the expression of miR-595 and miR-1246 in serum of ulcerative colitis and its correlation with intestinal flora.MethodsA total of 160 patients with ulcerative colitis were selected as the observation group.According to the disease activity, the patients were divided into active phase (aUC group) and remission phase (rUC group), with 80 cases in each group.During the same period, 158 healthy people for physical examination were selected as the control group.Real-time fluorescent quantitative PCR (RT-qPCR) was used to detect the expression levels of serum miR-595 and miR-1246.Enzyme-linked immunosorbent assay (ELISA) was used to detect serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-22 (IL-22) levels.Fresh feces of patients and healthy people were collected, and gradient dilution method was used to culture and analyze the flora.ResultsThe expression levels of miR-595, miR-1246, TNF-α, IL-6 and IL-22 in the aUC and rUC group were significantly higher than those in the control group (P < 0.05), and which in the aUC group were higher than those in the rUC group (P < 0.05).The number of lactobacillus and bifidobacteria in the aUC group and rUC group were lower than those in the control group (P < 0.05), and which in the aUC group were lower than those in the rUC group (P < 0.05).The number of enterococcus and Escherichia coli in aUC group was significantly higher than that in control group and rUC group (P < 0.05).Pearson correlation analysis showed that the expression levels of miR-595, miR-1246, TNF-α, IL-6 and IL-22 were negatively correlated with the number of lactobacilli and bifidobacteria (P < 0.05), and positively correlated with the number of enterococci and Escherichia coli (P < 0.05).ConclusionsThe expression of miR-595 and miR-1246 in the serum of ulcerative colitis is significantly up-regulated, and closely related to the number intestinal flora.

Abstract:
ObjectiveTo explore the application value of combination of prenatal serological quadruple screening and non-invasive prenatal testing (NIPT) birth screening mode in fetal chromosome screening.MethodsA total of 11 796 pregnant women undergoing maternity check-ups were selected as the observation subjects.According to the prenatal serological quadruple screening, the pregnant women were divided into high-risk group and borderline-risk group.The pregnant women in the high-risk group and borderline-risk group were tested for NIPT, meanwhile the pregnant women with a high-risk NIPT result were subjected to routine karyotyping and/or high-throughput sequencing tests, and all pregnant women were followed up.ResultsDown's screening screened out 721 high-risk pregnant women and 1 455 borderline-risk pregnant women.Among the 721 high-risk pregnant women, 19 were high-risk by NIPT, that was 2.64% of those who required invasive prenatal diagnosis.Among the 1 455 pregnant women who showed a borderline-risk in Down's screening, 5 cases were high-risk by NIPT, that was 0.34% of those who needed invasive prenatal diagnosis.The 24 pregnant women with high-risk NIPT were all subjected to karyotype analysis, which showed that 20 pregnant women showed chromosomal abnormalities.The high-risk pregnant women in Down's screening confirmed 19 cases, including 13 cases of 21-trisomy syndrome, 4 case of 18-trisomy syndrome, 1 case of 13-trisomy syndrome, and 1 case of other chromosomal abnormalities.A borderline-risk pregnant woman was diagnosed as 21-trisomy syndrome.A total of 20 cases of 24 pregnant women were consistent with the results of the karyotype analysis.Combined with the pregnancy outcome, the high-risk rate of Down's screening was 6.11%, the false positive rate of chromosome after combined NIPT screening was 3.29%, and the detection rate was 83.33%.ConclusionsCombination of prenatal serological quadruple screening and NIPT birth screening mode can reduce the proportion of invasive examinations in high-risk pregnant women, it has certain clinical significance for the screening of 21-trisomy syndrome and 18-trisomy syndrome.

ObjectiveTo evaluate influencing factors of liver fibrosis in HBeAg negative hepatitis B with alanine aminotransferase (ALT) less than 2 upper limit of normal(ULN)and establish the non-invasive prediction model to assess the severity of liver fibrosis.MethodsThe clinical data of 295 patients in HBeAg negative CHB patients with ALT≤2ULN were retrospectively analyzed.The degree of liver fibrosis S≥2 was taken as the discriminant criterion for significant liver fibrosis according to the pathological results of liver puncture.There were 94 cases in the mild group of liver fibrosis (S≤1) and 201 cases in the significant group (S≥2).The independent predictors of liver fibrosis were screened by multivariate logistic regression analysis and non-invasive model was constructed.Finally, the model was evaluated by area under the receiver operating characteristic curve to identify the severity of liver fibrosis.ResultsMultivariate logistic regression analysis showed that aspartate aminotransferase and hepatitis B core antibody were the independent predictors of liver fibrosis (P < 0.01).The AUC of this model was 0.721(95%CI: 0.660-0.782, P < 0.01).The sensitivity and specificity for the diagnosis of significant liver fibrosis were 60.0% and 74.5%.ConclusionsThe non-invasive prediction model based on the two indicators of aspartate aminotransferase and hepatitis B core antibody has high diagnostic value for evaluating the severity of liver fibrosis in CHB.

Abstract:
ObjectiveTo investigate the predictive value of interleukin-10 (IL-10) and catalase (CAT) in post-stroke depression (PSD) in patients with acute ischemic stroke.MethodsSerum IL-10 and CAT levels in 151 patients with acute ischemic stroke within 24 h of hospital admission were measured.Depressive symptoms were assessed using the 17-item Hamilton depression scale (HAMD-17), and PSD was defined as a HAMD score ≥8.ResultsAt 1-month follow-up after stroke, 51 (33.8%) patients were diagnosed with PSD.Compared with non-PSD patients, the PSD group had higher NIHSS scores at admission and discharge, larger cerebral infarction volume, lower BI score, higher mRS score, and lower serum CAT and IL-10 levels (P < 0.05).After adjusting for potential confounders (P < 0.05 in univariate logistic regression analysis), IL-10 (OR=0.615, 95%CI: 0.410-0.923) and CAT (OR=0.757, 95%CI: 0.652-0.914) were independent predictors of PSD.IL-10 levels (r=0.394, P < 0.01) and CAT levels (r=0.306, P < 0.01) were negatively correlated with HAMD scores in PSD patients.ROC analysis showed that the cutoff point for serum IL-10 levels was 2.06 pg/mL, and the AUC for predicting PSD was 0.739.Meanwhile, the cutoff point for the CAT level was 1.07 U/L, and the AUC for predicting PSD was 0.630.Patients with lower IL-10 levels (< 2.06 pg/mL) were more likely to develop PSD than their counterparts (≥2.06 pg/mL) (OR=9.750, 95%CI=2.671-35.534, P < 0.01).Meanwhile, patients with lower CAT levels (< 1.07 U/L) were more likely to develop PSD than their counterparts (≥1.07 U/L) (OR=5.052, 95%CI=1.256-20.322, P < 0.05).ConclusionsSerum IL-10 and CAT can be used as independent protective predictors of PSD in patients with acute ischemic stroke.Patients with low levels of IL-10 and CAT are more likely to develop PSD 1 month after stroke.

ObjectiveTo analyze the clinical significance of thrombocytosis in peripheral blood of patients with neutrophil cytoplasmic antibody-associated vasculitis (AAV).MethodsClinical and relevant laboratory data of 110 AAV patients were analyzed.They were divided into a normal platelet group (n=67) and an increased platelet group (n=43) based on the first peripheral platelet count results after admission.The clinical and relevant experimental examination data of the two groups were compared, and the survival status of the patients was regularly followed up.ResultsUnivariate analysis found that there were statistically significant differences in gender, age, peripheral white blood cell count, neutrophil count, lymphocyte count, serum complement C3 level, erythrocyte sedimentation rate, and serum C-reactive protein between the two groups (P < 0.05 to P < 0.01).Logistic regression analysis found that gender, peripheral white blood cell count, and neutrophil count were independent risk factors for AAV patients with thrombocytosis (P < 0.05).The usage rate of intensive therapy in the thrombocytopenia group was significantly higher than that in the normal platelet group (P < 0.05).There was a statistically significant difference in mortality rates between the two groups of patients (P < 0.01).ConclusionsThe condition of AAV patients with increased peripheral platelets is severe and the prognosis is poor, active intervention should be taken to improve the prognosis through early diagnosis and treatment.

Abstract:
ObjectiveTo compare the impact of combined therapy with dapagliflozin and metformin versus metformin monotherapy on liver fat content (LFC) and cystatin C (Cys C) levels in patients with type 2 diabetes mellitus (T2DM).MethodsA total of 121 inpatients with T2DM were enrolled.Patients were divided into metformin monotherapy group (group A, n=51) and dapagliflozin combined with metformin therapy group (group B, n=70) based on their treatment regimen.All patients had been continuously and stably using their respective treatment regimens for more than six months, the levels of LFC were measured using 1H-magnetic resonance spectroscopy, and serum Cys C levels were assessed using the immunoturbidimetric method.The detailed information of basic demographic and laboratory indexes were collected.The correlations between different laboratory parameters and LFC were analyzed, and the effects of different treatment modalities and other laboratory indicators on LFC and Cys C were explored.ResultsThere were no statistically significant differences in age, gender composition, disease duration, and body mass index (BMI) between the two groups (P>0.05).Compared to the group A, patients in the group B showed a significant reduction in LFC, serum Cys C levels and total cholesterol (TC) (P < 0.05 to P < 0.01), but had the increases of triglycerides (TG) and very low-density lipoprotein (VLDL) (P < 0.05).The proportion of nonalcoholic fatty liver disease was higher in the group A (90.2%) than that in the group B (30.0%) (P < 0.01).Further correlation analysis revealed that BMI, fasting blood glucose (FBG), and Cys C were associated with LFC (P < 0.05 to P < 0.01).Using LFC as the dependent variable and BMI, FBG, Cys C, disease duration, TC, TG, and medication as independent variables in multiple linear regression analysis, it was found that compared to metformin monotherapy, combined dapagliflozin and metformin therapy reduced LFC by 0.440 units (P < 0.01).Similarly, when using Cys C as the dependent variable and BMI, FBG, LFC, disease duration, TC, TG, and medication as independent variables in multiple linear regression analysis, it was found that combined metformin and dapagliflozin therapy reduced serum Cys C by 0.689 units compared to metformin monotherapy (P < 0.01).ConclusionsThe combined therapy with metformin and dapagliflozin can effectively reduce the levels of LFC and serum Cys C in T2DM patients, suggesting that such treatment pattern is an important factor influencing LFC and serum Cys C.

ObjectiveTo investigate the correlation between serum Kisspeptin, Betatrophin levels, insulin resistance in patients with gestational diabetes mellitus (GDM).MethodsA total of 86 GDM patients (GDM group) and 60 normal pregnant women (NGT group) were selected.Serum Kisspeptin and Betatrophin levels were detected.The correlation between serum Kisspeptin, Betatrophin levels and insulin resistance in GDM was analyzed.ResultsThe levels of fasting plasma glucose (FPG), fasting insulin (FINS), homeostasis model assessment-insulin resistance (HOMA-IR), hemoglobin A1c (HbA1c), total cholesterol (TC), triglycerides (TG), Kisspeptin and Betatrophin in the GDM group were higher than those in the NGT group (P < 0.01).Pearson correlation analysis showed that serum Kisspeptin and Betatrophin levels were positively correlated with TC, TG, PFG, FINS and HOMA-IR (r=0.367-0.576, P < 0.01), and Kisspeptin was positively correlated with Betatrophin (r=0.326, P < 0.05).Multiple linear regression analysis results showed that Kisspeptin (B=0.671) and Betatrophin (B=0.886) were independently correlated with HOMA-IR in patients with GDM (P < 0.05 to P < 0.01).ConclusionsSerum Kisspeptin and Betatrophin levels are significantly increased in GDM patients, and there is a positive linear correlation between Kisspeptin, Betatrophin levels and insulin resistance in GDM patients.

Abstract:
ObjectiveTo explore the risk factors for postpartum hemorrhage (PPH) in pregnant women during spontaneous delivery, and to establish a risk nomogram prediction model.MethodsThe clinical information of 3 067 parturients who underwent an obstetric examination and eventually delivered vaginally in the department of obstetrics was collected.According to the occurence of PPH within 24 hours after delivery, the parturients were divided into PPH group (n=229) and non-PPH group (n=2 838).The clinical information of the two groups was compared, and logistic regression analysis was used to screen the risk factors for PPH in spontaneous delivery and a prediction model was established.The area under the ROC curve, sensitivity and specificity were used to evaluate the performance of the model, and Bootstrap self-sampling method was used to verify the model internally.ResultsThere were significant differences between PPH group and non-PPH group in age, multiple pregnancy, pregnancy induced hypertension, prenatal D-dimer, prenatal Fib, the third stage of labor, placental adhesion, and fetal body mass (P < 0.01).Multivariate logistic regression analysis showed that high age, multiple births, pregnancy induced hypertension, pre-partum D-dimer elevation, long third stage of labor, placenta adhesion, and large fetal body mass were all risk factors for PPH in spontaneous delivery of pregnant women (P < 0.01), and pre-partum Fib elevation was a protective factor for PPH in spontaneousl delivery of pregnant women (P < 0.01).By using the above 8 indicators to construct and verify the nomogram prediction model, it was found that the AUC of the model was 0.824, the sensitivity was 0.819, and the specificity was 0.715 for predicting PPH in spontaneous delivery of pregnant women.After 1 000 samples of Bootstrap method, the MAE of the calibration curve was 0.004, which showed that the model had a good consistency in predicting the risk of spontaneous delivery PPH in pregnant women and the actual occurrence.ConclusionsThe clinical integration of pregnant women's age, number of fetuses, pregnancy-induced hypertension, pre-partum D-dimer, pre-partum Fib, the third stage of labor, placental adhesions, and fetal weight indicators are used to construct a prediction model that can improve the accuracy of evaluating pregnant women's spontaneous delivery PPH.

Abstract:
ObjectiveTo optimize the detection condition of amino acids in Pholidota cantonensis Rolfe.from different producing areas of Fujian province by solvent reflux extraction and ninhydrin colorimetric method.MethodsThe content of total amino acids was determined by ninhydrin colorimetric method with glycine as reference; the effect of different influence factors on detection were studied to determine the optimal detection conditions; on the basis of single factor experiment, L9 (34) orthogonal test was used to investigate the effects of solvent dosage, solvent concentration, extraction time and extraction times on the extraction process of total amino acids from Pholidota cantonensis.The optimal extraction process was optimized based on these indexes.ResultsThe optimum detection conditions were: wavelength 568 nm, ninhydrin (2%) volume 3 mL, pH 6, buffer solution 2 mL, water bath heating temperature 80 ℃ and heating time 40 min.The optimal ultrasonic extraction conditions were as follows: alcohol concentration 85%, alcohol amount 35 mL, extraction time 60 min, and twice extraction.The contents of total amino acids in four different producing areas in Fujian province were as follows: Yongtai county, Shanghang county, Pingnan county and Yongchun county.ConclusionsThe ninhydrin colorimetric method is simple, accurate, reliable and reproducible.The optional extraction is stable and feasible, which can provide a basis for quality evaluation control, the determination and further development of Pholidota cantonensis.In the collected samples, the best quality of Pholidota cantonensis from Yongtai county is obtained.

Abstract:
ObjectiveTo establish a method for the determination of blood concentration of risperidone (RIS), and to explore the correlation between blood concentration of RIS and thyroid function indexes in patients with schizophrenia.MethodsThe plasma samples were precipitated with acetonitrile, and then determined by ultra performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) with clomipramine as the internal standard.The chromatographic column was BEH C18, and the mobile phase was acetonitrile-water (containing 5 mmol/L ammonium acetate solution and 0.01% ammonia) with gradient elution.The flow rate was 0.5 mL/min, the column temperature was 40 ℃, and the injection volume was 2 μL.The methodology of UPLC-MS/MS was investigated by multiple reaction monitoring scanning, and it was used to determine the plasma RIS concentration in 116 patients with schizophrenia.The hospitalization information of related cases was collected, and the correlation between RIS blood concentration and thyroid function indexes was explored.ResultsRIS and 9-OHRIS had a good linear relationship in the concentration range of 0.4-120.0 ng/mL.The intra-day and inter-day accuracy, extraction recovery, matrix effect and stability met the requirements.The mean steady-state plasma concentration was (49.76±18.07)ng/mL.There was no significant difference in the steady-state plasma concentration of patients at each time point of treatment (P>0.05).Except for TSH (P>0.05), the other four indicators showed a fluctuating upward trend over time after treatment (P < 0.05), but the RIS plasma concentration of female patients was positively correlated with T4, FT3 and FT4 (r=0.480, 0.524, 0.454, P < 0.05).ConclusionsThe method is accurate, simple, rapid, sensitive and reproducible, and can be used to determine the plasma concentration of RIS in patients with schizophrenia.Higher blood drug concentration in female patients may affect the thyroid function index, and then affect the thyroid function of patients, which needs clinical attention.

ObjectiveTo explore the value of shear wave elastography (SWE) combined with smart three-dimensional superb microvascular imaging (Smart 3D SMI) in distinguishing benign and malignant breast nodules.MethodsA total of 108 patients (118 nodules) who underwent breast ultrasound examination were selected as the study subjects.All patients underwent two-dimensional ultrasound examination, SWE examination, and Smart 3D SMI examination, and the pathological results were used as the gold standard to compare the diagnostic efficacy of SWE, Smart 3D SMI, and their combination examination.ResultsThe consistency between SWE and Smart 3D SMI detection alone and pathological results was average (P < 0.01) The consistency between SWE combined with Smart 3D SMI and pathological results was good (P < 0.01).The sensitivity, specificity, and accuracy of SWE combined with Smart 3D SMI in diagnosing the nature of breast tumors were higher than those of SWE and Smart 3D SMI alone, and the difference was statistically significant (P < 0.05 to P < 0.01).ConclusionsThe combination of SWE and Smart 3D SMI has good application value in the differential diagnosis of benign and malignant breast nodules.

ObjectiveTo understand the willingness to engage in geriatric care among nursing students and analyze its factors under the background of the implementation of "1+X geriatric care" training in higher vocational college.MethodsA total of 428 nursing students in higher vocational college were randomly selected as study subjects, and general information questionnaire, questionnaire on geriatric care training and willingness to engage in geriatric care, aging knowledge scale and elderly attitude scale were used to conduct the questionnaire survey.ResultsA total of 302 (70.56 %) nursing students were willing to participate in geriatric care training, 172 (40.19 %) students were willing to engage in geriatric care.Nursing students had poor aging knowledge (13.84±6.43), and tended to appreciate the elderly (114.03±15.12).Nursing students who were from poor families, interested in nursing, willing to participate in geriatric care training, with higher scores of the elderly knowledge and with positive attitude towards the elderly had higher willingness to engage in geriatric care (P < 0.05 to P < 0.01).Family income, willingness to participate in geriatric care training, scores of prejudice dimension and the preference for nursing profession were the major factors that affected the willingness to engage in geriatric care(P < 0.05 to P < 0.01).ConclusionsThe willingness to participate in geriatric care training among nursing students in higher vocational college is positive but the willingness to engage in geriatric care still needs to be improved.By improving the mechanism of geriatric care industry and changing the employment concept of nursing students, the attraction to talents can be improved and more nursing students will be encouraged to devote themselves to the geriatric care industry.
Effect evaluation of finger movements on preventing radial artery occlusion after transradial coronary intervention

Abstract:
ObjectiveTo explore the effect of finger movements on preventing radial artery occlusion (RAO), the changes of the postoperative hemodynamic parameters of the radial artery in patients were observed dynamically, and the effects of the finger dance exercise on the early and late RAO occurrence were analyzed.MethodsA total of 171 patients undergoing elective coronary angiography and interventional therapy were randomly divided into control group (89 patients) and observation group (82 patients).The observation group carried out finger movements for 3 h after surgery, and none was performed in the control group.The thickness, diameter and peak flow velocity of the radial artery were measured before puncture, 3 days and 1 month after surgery.The changes of the above indicators were dynamically observed, and the early and late RAO occurrence were analyzed.ResultsThe age, gender, smoking, combined disease and other clinical characteristics of patients in the two groups were not significantly different (P>0.05).The radial artery diameter and intimal thickness in the observation group had no statistical significance compared with the control group in the same period before surgery, 3 days and 1 month after surgery (P>0.05).The peak flow velocity of radial artery in the observation group had no statistical significance compared with the control group before surgery (P>0.05), but the peak flow velocity of radial artery in the observation group was higher than that in the control group 3 days and 1 month after surgery (P < 0.05).On the third day after surgery, the early RAO rate in the observation group was 5.7% (4/70), lower than the 16.3% (13/80) in the control group (P < 0.05).At 1 month after surgery, no RAO occurred in the observation group, and the occlusion rate was 0% (0/68), which was lower than 14.9% (11/74) in the control group (P < 0.01).The incidence of adverse reactions such as forearm swelling and puncture point blood infiltration was not significantly different between the two groups (P>0.05).ConclusionsFinger movements helps to improve the early radial artery blood flow velocity, can significantly reduce the occurrence of early and late RAO after coronary angiography, and does not significantly affect the incidence of adverse reactions.Postoperative finger dance exercise is a simple, safe and effective prevention strategy for RAO.
Correlation of self-management ability with family function and coping style in patients with chronic obstructive pulmonary disease

Abstract:
ObjectiveTo understand the self-management ability of patients with chronic obstructive pulmonary disease(COPD), analyze the correlation between family function, coping style and self-management ability, to provide scientific theoretical guidance for clinical implementation of effective nursing intervention for COPD patients.MethodsA total of 115 patients with COPD were selected as study subjects.They were surveyed by the general condition questionnaire, chronic disease self-management behavior scale(CDSMS), family care index scale (APGAR) and medical coping style scale (MCMQ).Pearson analysis was used to analyze the correlation between self-management ability and family function and coping style of COPD patients.ResultsCOPD patients with age ≤70 years old, religious belief, education level >high school, monthly family income >5 000 yuan and living with spouse or parents or children had higher scores of self-management ability, and the differences were statistically significant(P < 0.05 to P < 0.01).The self-management score of 110 COPD patients was(38.06±6.48) points, the family function score was(7.12±1.01) points, and the coping style score was(47.39±6.23)points.Pearson analysis showed that self-management ability was positively correlated with family function and coping style(P < 0.05 to P < 0.01).ConclusionsThe scores of self-management ability, family function and coping style in COPD patients are all at medium level, and self-management ability is positively correlated with family function and coping style.It is suggested that clinical medical staff should pay more attention to family function and coping style of COPD patients and take targeted measures to enhance self-management ability.

ObjectiveTo explore the application effect of self-made "6-minute walking training rehabilitation package" in the community cardiac rehabilitation on patients undergoing coronary heart disease after percutaneous coronary intervention (PCI).MethodsEighty patients with coronary heart disease at 6 months to 3 years after PCI were randomly divided into a control group and an observation group, with 40 cases in each group.The control group received routine post-interventional nursing guidance and community conventional follow-up, while the observation group was additionally provided with a "6-minute walk training rehabilitation package" on the basis of the routine guidance to guide patients in home-based exercise for rehabilitation training.The 6-minute walking test (6MWT) and living quality of the two groups were compared after a follow-up survey lasted for one year.ResultsAfter one year of intervention, the 6MWT distance in the observation group was longer than that in the control group (P < 0.01).There was statistically significant differences in the scores of the physiological, psychological, and social fields between the observation group and the control group (P < 0.01), while there was no statistically significant difference in scores in the environmental field (P>0.05).ConclusionsThe application of self-made walking training rehabilitation package to stable stage patients with coronary heart disease, 6 months to 3 years after PCI to guide home rehabilitation exercise significantly increases their 6-minute walking distances, enhances exercise tolerance, and improves the quality of life.

ObjectiveTo explore the efficacy of multidisciplinary collaborative emergency mode combined with early injury control resuscitation in the treatment of hemodynamically unstable pelvic fractures.MethodsA total of 30 patients with hemodynamically unstable pelvic fractures who implemented multidisciplinary collaborative emergency mode combined with early injury control resuscitation were selected as the observation group, and 30 patients with hemodynamically unstable pelvic fractures who implemented traditional active fluid resuscitation before the observation group mode were selected as the control group.The infusion volume of colloidal solution, crystal solution, plasma, and concentrated red blood cells within 24 hours, recovery of body temperature, coagulation function, and lactate clearance time, as well as the incidence and mortality rate of disseminated intravascular coagulation (DIC) of patients in the two groups were statistically analyzed.ResultsThere was no colloidal solution supplementation in the observation group.The infusion volume of crystal fluid and concentrated red blood cells in the observation group were lower than those in the control group, while the plasma infusion volume was higher than that in the control group, the recovery time of body temperature, coagulation function, and lactate clearance were shorter than those in the control group, and the differences were statistically significant (P < 0.01).The incidence of DIC in the observation group was 3.33%, and the mortality rate was 6.67%, which was not statistically significant compared to 10.00% and 16.67% in the control group (P>0.05).ConclusionsThe multidisciplinary collaborative emergency mode combined with early injury control resuscitation applied to hemodynamically unstable pelvic fractures can quickly and effectively restore the patient's hemodynamically stable state.
Study on the best prevention mechanism for sudden delirium in ICU patients

Abstract:
ObjectiveTo analyze the main risk factors of sudden delirium in ICU patients, construct logistic regression model and decision-tree model through a prospective verification to establish the best prevention mechanism for early identification of delirium.MethodsA total of 198 ICU patients from January 2018 to January 2020 were retrospectively summarized as the model group.According to the diagnostic criteria of delirium within 24 hours, they were divided into delirium group (n=88) and non-delirium group (n=110).The risk factors were screened by multivariate logistic regression analysis.In addition, 87 patients from February 2020 to February 2021 were choosed as the validation group.Receiver operating characteristic curve was used to compare the predictive efficacy of the two models.ResultsCompared with the patients without delirium, the age, acute physiology and chronic health (APACHE Ⅱ) score and complications of the patients with delirium were increased, sedation time and mechanical ventilation time were prolonged, serum neuro-specific enolase(NSE) and arterial blood lactic acid were increased, and oxygenation index was decreased (P < 0.01).Logistic regression analysis showed that APACHE Ⅱ score, serum NSE and arterial blood lactate level were independent risk factors for delirium (P < 0.01).The area under the curve of the decision-tree model was higher than that of logistic regression model (P < 0.01).ConclusionsThe incidence of sudden delirium within 24 hours in ICU patients is high, and multiple risk factors may be involved in the occurrence of delirium, including APACHE Ⅱ score, serum NSE and arterial blood lactic acid level.The decision-tree model may have a higher predictive efficacy than traditional logistic regression model, which provides a better evaluation method for guiding clinical medical staff to correctly identify high-risk groups of delirium in early stage.
