TAICE术后不同转归患者血清VEGF、OPN及肿瘤标志物水平分析

摘要: 目的:分析经动脉免疫化疗栓塞(TAICE)术后不同转归患者血清血管内皮生长因子(VEGF)、骨保护素(OPN)及肿瘤标志物水平,明确其对术后疗效的判断意义。方法:选择中晚期肝癌患者41例,接受TAICE治疗后分别对临床疗效、VEGF及OPN、肿瘤标志物水平进行评估。结果:术后完全缓解(CR)患者4例(9.8%),部分缓解(PR)31例(75.6%),稳定(SD)4例(9.8%),进展(PD)2例(4.9%),总有效率85.4%。PR患者OPN及VEGF均明显高于CR患者(P<0.01),SD患者OPN及VEGF均较CR及PR显著升高(P<0.01),PD患者OPN及VEGF均较CR、PR及SD显著升高(P<0.01)。PR患者甲胎蛋白(AFP)、癌胚抗原(CEA)、α-L-岩藻糖苷酶(AFU)均较CR患者升高(P<0.05~P<0.01),SD患者AFP、AFU均较CR及PR显著升高(P<0.01),CEA亦较CR患者升高(P<0.05),PD患者AFP、CEA、AFU均较CR、PR及SD升高(P<0.05~P<0.01)。结论:TAICE治疗原发性肝癌具有较好的临床疗效,VEGF及OPN可作为评价其临床疗效的重要指标。Abstract: Objective:To analyze the serum levels of vascular endothelial growth factor(VEGF),osteoprotegerin(OPN) and tumor markers in patients with different outcomes after transcatheter arterial immune chemoembolization(TAICE),and its significance in evaluating postoperative effects.Methods:The clinical efficacy,and the levels of VEGF,OPN and tumor marker in 41 patients with moderate or advanced liver cancer were evaluated after TAICE.Results:The complete remission(CR) in 4 cases(9.8%),partial remission(PR) in 31 cases (75.6%),SD in 4 cases(9.8%) and PD in 2 cases(4.9%) were identified after operation,the total effective rate was 85.4%.The levels of OPN and VEGF in PR patients were significantly higher than those in CR patients(P<0.01).Compared with the CR and PR patients,the levels of OPN and VEGF in SD patients increased significantly(P<0.01).Compared with the CR,PR and SD patients,the levels of OPN and VEGF in PD patients increased significantly(P<0.01).Compared with the CR patients,the levels of AFP,CEA and AFU in PR patients increased significantly(P<0.05 to P<0.01).Compared with the CR and PR patients,the levels of AFP and AFU in SD patients increased significantly(P<0.01).Compared with the CR patients,the level of CEA in SD patients increased significantly(P<0.05).Compared with the CR,PR and SD patients,the levels of AFP,CEA and AFU in PD patients increased significantly(P<0.01).Conclusions:The treatment of hepatocellular carcinoma with TAICE has better clinical effects.The VEGF and OPN involve in development of hepatocellular carcinoma,which are the important indicators in evaluating the clinical efficacy.
