侧卧位和仰卧位PFNA治疗超高龄股骨粗隆间骨折的疗效比较
Effect comparison of the PNFA between lateral decubitus and supine position in the treatment of intertrochanteric femur fracturesin the advanced age patients

摘要: 目的:对比侧卧位和仰卧位防旋型股骨近端髓内钉(PFNA)治疗超高龄股骨粗隆间骨折患者的疗效。方法:对20例超高龄股骨粗隆间骨折患者进行侧卧位和仰卧位PFNA治疗。记录手术时间、观察测量术后尖顶距及调查计算随访患者Harris髋关节功能评分,对比其疗效。结果:侧卧位组平均手术时间为(42.6±7.4)min,仰卧位组为(97.5±38.82)min,侧卧位组手术时间明显短于仰卧位组(P<0.01)。患者随访时间为8~21个月,其中18例患者随访时间超过1年。根据Harris髋关节功能评分表评分,结果显示侧卧位组和仰卧位组分别为(84.9±3.45)分、(87.2±5.6)分,2组尖顶距分别为(20.2±4.89)mm、(21.5±4.86)mm,差异均无统计学意义(P>0.05)。结论:相对于仰卧位,侧卧位PFNA治疗超高龄股骨粗隆间骨折患者,简化了手术操作,长期疗效令人满意。

Abstract: Objective: To compare the effects of the proximal femoral nail antirotation(PNFA) between lateral decubitus and supine position in the treatment of intertrochanteric femur fractures in the advanced age patients.Methods: Twenty advanced age patients with intertrochanteric femur fractures were treated with PFNA in lateral decubitus and supine position.The operating time,postoperative tip-apex distance(TAD) and Harris hip score were observed,and the curative effects between two positions were compared.Results: The average operation time in patients with the lateral decubitus position (42.6±7.4) mins was shorter than that in patients with supine position (97.5±38.82) mins(P<0.01).All patients were followed up for 8 to 21 months,and the following up time in 18 cases were more than 1 year.The Harris hip score and TAD in patients with the latral decubitus position and supine position were (84.9±3.45)point & (87.2±5.6)point and (20.2±4.89)mm & (21.5±4.86)mm,respectively,the difference of which was not statistically significant(P>0.05).Conclusions: Compare with the supine position,the PNFA in latral decubitus position in treating the advanced age patients can simplify the operative process,and the long-term effect of which is satisfactory. -
