工作能力强化训练对手外伤术后上肢功能的影响
Effect of the intensive training of work ability on upper extremity function after operation of hand trauma

摘要: 目的:观察工作能力强化训练对手外伤术后上肢功能的影响。方法:将70例手外伤术后病人随机分为2组,对照组病人只进行常规康复治疗。观察组病人在对照组常规康复治疗基础上进行工作能力强化训练,内容包括建筑工作站、电工工作站、维修工作站、运输工作站、文秘工作站和驾驶工作站训练等,每个工作站训练30 min,每天需完成全部6个工作站训练各1次,训练持续12周。2组病人在康复训练前后进行相关康复评估,其中运动功能评定用Fugl-Meyer运动功能评定法,情绪功能评定用汉密顿抑郁量表,日常生活活动能力评定用改良Barthel指数或独立功能评定法。结果:2组病人治疗前各项评分差异均无统计学意义(P>0.05),2组病人治疗后各项评分均较治疗前明显改善(P< 0.01),而观察组病人治疗后各项评分改善程度均显著优于对照组(P< 0.01)。结论:手外伤术后上肢功能障碍早期行更多的职业康复干预有重要的临床意义。Abstract: Objective: To observe the effects of the intensive training of work ability on upper extremity function after operation of hand trauma.Methods: Seventy patients with hand injury after operation were randomly divided into the control group and observation group.The control group was treated with routine rehabilitation.The observation group was additionally trained with the intensive training of work ability on the basis of routine rehabilitation therapy.The contents included building,electrician,repair,transportation,secretary and driving workstations training.The training time in each workstation was 30 min,six workstations training was completed every day,one time every workstation for 12 weeks.The related rehabilitation in two groups was evaluated before and after rehabilitation training.The motor function,emotional function and activity of daily living(ADL) were evaluated using Fugl-Meyer assessment(FMA),Hamilton Depression Scale(HAMD) and modified Barthel index(MBI) or FIM,respectively.Results: The difference between the scores of all items in the two groups before treatment was not statistically significant(P>0.05),the scores of all items in the two groups were significantly improved after treatment(P<0.01),and the scores of all items of patients in the observation group after treatment were significantly better than those in the control group(P<0.01).Conclusions: The early postoperative occupation rehabilitation intervention has important clinical significance in the recovery of upper limb dysfunction caused by hand injury.
