据统计,全球每年大约有1 500万人发生脑血管意外,其中,高达40%的脑卒中病人伴有痉挛的发生,卒中后3个月左右是痉挛发生高峰期[1]。痉挛使病人的主、被动运动受限,严重影响其日常生活和活动,且治疗时间较长,所需医疗费用至少是无痉挛发生病人的4倍[2]。此外,痉挛可能导致病人肌肉及关节疼痛或不自主运动,影响其平衡、协调及步态,因此,适当的康复措施是降低病人住院费用和减轻残疾的重要手段[3]。近年来,MOTOmed智能训练仪作为新型设备广泛应用于临床康复治疗,它通过视觉反馈及不同运动模式改善病人肌力、肌张力、平衡及协调功能等[4];痉挛肌电刺激是针对卒中后痉挛病人设计的低频治疗仪,在牵张反射和交互性抑制的原理上通过刺激使痉挛肌松弛,拮抗肌兴奋,调节肌张力[5]。本研究按随机对照方法在常规康复训练的基础上增加MOTOmed智能训练及痉挛肌电刺激治疗脑卒中后下肢痉挛病人93例,取得了满意的疗效。现作报道。

选取2015年10月至2016年10月连云港市第一人民医院康复科、蚌埠医学院第一附属医院康复科脑卒中住院病人93例,诊断均符合第四届全国脑血管疾病学术会议制定的脑卒中诊断标准[6]。纳入标准:(1)均经CT或MRI证实该诊断且为首次发病;(2)病程 < 3个月,年龄20~70岁;(3)有一侧肢体功能障碍,采用改良Ashworth量表评级,肌张力评定1~3级;(4)生命体征平稳,无认知障碍。排除标准:肢体感觉障碍、严重心肺功能不全、严重言语功能及认知功能障碍;骨折、外伤等引起的髋、膝、踝关节活动受限。93例中男48例,女45例;脑出血46例,脑梗死47例;左侧偏瘫49例,右侧偏瘫44例;病程7~90 d;按电脑抽取随机分为3组,3组一般资料差异均无统计学意义(P>0.05)(见表 1),具有可比性。

常规组采用康复科常规治疗[7]。(1)良肢位摆放;(2)神经肌肉促进技术:床上翻身、桥式运动、PNF技术的下肢屈肌训练、关节负重挤压等训练促进病人分离运动出现;(3)转移训练:床-轮椅转移、重心转移;(4)主动、被动牵伸训练:通过自身重量或治疗师帮助下进行躯体长轴牵引,牵伸痉挛肌等训练;(5)坐位、站立平衡训练、坐站训练;(6)步态训练:上下楼梯、平衡杠内行走、侧方行走、向前行走、转身训练;(7)作业训练。以上训练每次40 min,每周6次。
电刺激组在常规组基础上应用痉挛肌电刺激。采用北京金豪公司设备厂生产的SD-608型痉挛肌电刺激治疗仪,仪器采用低频双通道,分A、B两组输出单相方波,频率为0.1~5.0 Hz,脉宽0.1~2.0 ms,病人坐位或仰卧位:一组电极片放在腓肠肌(痉挛肌)两端肌腱,另一组放置在其拮抗肌胫前肌肌腹,两路电流交替出现,强度以引起肌肉收缩为适,每次20 min,每天1次,每周6 d。
MOTOmed组在常规组基础上采用德国生产的viva2 MOTOmed智能训练仪,早期偏瘫病人下肢肌力为0~2级时(此时肌张力仍可能较高),可使用床边MOTOmed智能训练仪,取仰卧位,设置被动训练模式,系统带动患肢进行训练,促进肢体功能恢复。当病人肌力为3级及以上时,据病人运动功能情况,设置主动训练模式及转速;当病人可进行主动训练时,根据病人实际肌力设定训练阻力,阻力设定范围1~30 N,设定阻力以病人可承受劳累程度为准。每次20 min,每天1次,每周6 d。

分别于治疗前及治疗8周后对3组病人进行康复评定,采用改良Ashworth评定量表(MAS)评定病人肌张力情况,其评定结果共分为0、1、1+、2、3和4级,分别设为0、1、2、3、4、5分,分值越高表示肌张力越高,采用Fugl-Meyer运动功能评定量表(FMA)下肢功能部分对病人运动功能进行评定,改良Barthel指数评定量表(MBI)对病人日常生活能力进行评定。

采用χ2检验、t检验、单因素ANOVA分析、q检验和配对t检验。

3组病人治疗前MAS、FMA及MBI评分差异均无统计学意义(P>0.05),治疗后与各自治疗前MAS、FMA及MBI评分比较差异均有统计学意义(P < 0.01)。治疗后MOTOmed组与常规组及电刺激组MAS、FMA及MBI评分比较差异均有统计学意义(P < 0.01),且MOTOmed组病人疗效明显优于常规组及电刺激组(P < 0.01)(见表 2)。

Clinical effect of MOTOmed intelligent training combined with spastic myoelectric stimulation in the treatment of lower limb muscle spasticity in stroke patients

目的观察MOTOmed智能训练联合痉挛肌电刺激对脑卒中病人下肢肌痉挛的临床疗效。 方法将93例脑卒中后下肢痉挛病人按电脑抽取随机分为常规组、电刺激组、MOTOmed组,常规组采用常规康复训练,电刺激组采用常规康复及痉挛肌电刺激疗法,MOTOmed组在常规康复上增加痉挛肌电刺激及MOTOmed智能训练,每组各31例。采用改良Ashworth评定量表(MAS)、Fugl-Meyer运动功能评定量表(FMA)及改良Barthel指数评定量表(MBI)在治疗前、治疗8周后分别对病人下肢肌张力、运动功能及日常生活活动能力进行评定比较。 结果3组病人治疗后与各自治疗前MAS、FMA及MBI评分比较差异均有统计学意义(P < 0.01)。治疗后MOTOmed组与常规组及电刺激组MAS、FMA及MBI评分比较差异有统计学意义(P < 0.01),且MOTOmed组病人疗效明显优于常规组及电刺激组(P < 0.01)。 结论MOTOmed智能训练联合痉挛肌电刺激可明显降低卒中后痉挛病人下肢肌张力,改善病人运动功能及日常生活活动能力。 -

Abstract:ObjectiveTo investigate the clinical effects of MOTOmed intelligent training combined with spastic myoelectric stimulation in the treatment of lower limb muscle spasticity in stroke patients. MethodsNinety-three stroke patients with posterior lower limb spasm were randomly divided into the conventional group (treated with conventional rehabilitation), electrical stimulation group (treated with conventional rehabilitation combined with spastic myoelectric stimulation) and MOTOmed group (treated with conventional rehabilitation, spastic myoelectric stimulation combined with MOTOmed intelligent training) (31 cases each group).The lower limb muscle tension, motor function and activities of daily living in three groups were evaluated using the modified Ashworth scale (MAS), Fugl-Meyer assessment (FMA) and modified Barthel index (MBI) before treatment and after 8 weeks of treatment, and which among three groups was compared. ResultsThe differences of the MAS, FMA and MBI scores in three groups between before and after treatment were statistically significant (P < 0.01).After treatment, the differences of the scores of MAS, FMA and MBI between MOTOmed group, and conventional and electrical stimulation groups were statistically significant (P < 0.01), and the treatment effect in MOTOmed group was significantly better than that in conventional and electrical stimulation groups (P < 0.01). ConclusionsThe application of MOTOmed intelligent training combined with spastic myoelectric stimulation can significantly decrease the lower limb muscle tension, improve motor function and activities of daily living in stroke patients with muscle spasticity. -
