  • ObjectiveTo explore the value of tumor-stroma ratio(TSR) in the assessment of the prognosis of breast cancer, and its correlation with clinicopathology.MethodsOne hundred and twenty tumor tissue samples of breast cancer patients were stained using HE, the tumor cells in three visual fields with obvious tumor infiltration were calculated to obtain the highest value under the microscope.All cases were followed up, and the relationship between TSR, and clinicopathology and prognosis of breast cancer were analyzed.ResultsThe TSR was correlated with the lymph node metastasis and histological grade(P < 0.01).The single and multivariate analysis of prognosis showed that the TSR was an independent prognostic factor of breast cancer(P < 0.01).ConclusionsThe TSR is an independent prognostic factor of breast cancer, which has high repeatability and simple operation, and is worthy of clinical promotion.

  • 乳腺癌是一种常见的恶性肿瘤, 近年来发病率明显增高, 已经跃居女性肿瘤的首位, 严重危害女性的健康[1], 对于乳腺癌的预后判断目前仍无较好的指标。肿瘤间质比(tumor-stroma ratio, TSR)的概念最早是由MESKER等提出的, 指的是肿瘤组织内肿瘤间质与实质的比值, 其最先应用于大肠癌的预后评估[2], 在乳腺癌方面的研究较少。本文旨在探讨TSR在乳腺癌预后的评估作用及其与临床病理关系, 现作报道。

1.   资料与方法
  • 收集我院肿瘤外科2015年1月至2018年1月收治的120例乳腺癌病人的临床资料, 年龄36~69岁, 肿瘤大小1.0~4.0 cm, 病理均为浸润性导管癌, 其中淋巴结转移阳性为48例, 术前均未行新辅助化疗、放疗, 且为初次治疗, 手术方式为乳腺癌改良根治术, 术后辅助治疗遵照中国抗癌协会乳腺癌诊疗规范进行。

  • 将新鲜手术切除标本中肿瘤浸润最多的组织制成5 μm厚的HE染色玻片, 由2名病理科副主任以上职称医师独立读片, 先在40倍镜下寻找肿瘤浸润明显的区域, 这个区域周围应均有肿瘤组织包围, 然后用100倍镜下寻找肿瘤细胞, 计算出肿瘤细胞的百分比, 剩余部分即为间质比。至少选择3个视野进行比较评估, 取其中间质比最高值为最终TSR。通过分层研究发现, TSR取50%截断值时统计学意义最大, 故将50%作为截断值。其中>50%为高TSR, ≤ 50%为低TSR。所有病人均通过门诊或电话随访等方式进行随访, 随访至2018年7月或病人死亡, 研究过程中无失访病人。

  • 采用单因素logistic回归分析、相关性分析和COX比例风险模型多因素分析。

2.   结果
  • 120例乳腺癌病人中高TSR 50例, 占41.67%, 低TSR 70例, 占58.33%。

  • 相关性分析显示, 乳腺癌TSR与组织学分级、肿瘤淋巴结阳性呈正相关(P < 0.01), 与其他病理因素相关性无统计学意义(P>0.05)(见表 1)。

  • logistic单因素分析发现, 乳腺癌预后与TSR、组织学分级、肿瘤淋巴结情况相关性有统计学意义(P < 0.05~P < 0.01), 与其他病理因素相关性无统计学意义(P>0.05)(见表 2)。

  • 120例乳腺癌病人随访至2018年7月或病人死亡时间, 其中随访最短时间为7个月, 高TSR病人死亡5例, 低TSR病人死亡10例, 通过COX多因素分析显示病人总生存期与TSR有密切相关性(P < 0.01), 与其他病理因素相关性无统计学意义(P>0.05)(见表 3)。

3.   讨论
  • 肿瘤微环境是肿瘤组织中的各种细胞和结构构成, 它直接参与了肿瘤的发生[3]。肿瘤的间质成分可以营养实质部分, 对于肿瘤的增殖、分化、黏附、迁移等生物学行为有密切的关系, 是肿瘤生长转移必须的[4]。肿瘤间质又受肿瘤细胞分泌的细胞因子的调节, 在肿瘤的发生与转移的过程中具有重要的作用, 越来越受到关注[5]。TSR最先由MESKER教授提出并首先应用于结直肠肿瘤预后的评估, 有研究[6]显示710例结肠癌病人中, TSR高组总体生存率及无瘤生存时间均明显低于低TSR组。不仅在结直肠癌中, 在其他实体肿瘤如食管癌[7]、宫颈癌[8]、肺癌[9]中均提示低TSR是预后较好的一个评价指标, 且不受术后治疗的影响。多项研究发现TSR在50%为临床预后统计学差异最大, 故本研究中也将其定为截断值。目前TSR在乳腺癌中的研究较少, 我们研究发现120例乳腺癌病人中对于肿瘤组织中高TSR组其总体生存时间低于肿瘤组织中低TSR组, 进一步分析TSR与其他临床病理因素相关性发现HSR表达与淋巴结情况、组织学分级的相关性有统计学意义, 与其他病理因素无明显相关性。

    本研究中单因素分析显示乳腺癌预后与TSR、组织学分级及肿瘤淋巴结情况相关性有统计学意义。120例乳腺癌病人随访共死亡15例, 通过COX多因素分析乳腺癌总生存率与乳腺癌组织中TSR密切相关(P < 0.01)。多项研究[10-11]亦显示TSR是乳腺癌独立的预后因素, 不受病人手术方式及术后治疗影响。

    乳腺癌目前相对的预后指标包括肿瘤大小、淋巴结情况、组织学分级以及免疫化的雌激素受体、孕激素受体、Her-2表达等, 通过本研究发现TSR与传统的预后指标并不完全一致, 与肿瘤的淋巴结转移情况和组织学分级关系较紧密, 考虑与肿瘤间质细胞与肿瘤的恶性程度及其促进了淋巴结转移等情况有关, 但目前其机制并不完全清楚[12], 有报道[13]在肿瘤的生长及转移过程中肿瘤细胞分泌一些细胞因子包括转化生长因子β、基质金属蛋白酶等在其中起到了关键性作用, 目前本研究随访时间较短仍需进一步研究。但TSR的监测重复性强, 操作简单, 适合临床推广。
