小剂量米非司酮联合GnRHa治疗卵巢巧克力囊肿术后疗效观察
Efficacy of ovarian chocolate cysts after a low dose of mifepristone with GnRHa therapy

摘要: 目的：探讨术后小剂量米非司酮联合促性腺激素释放激素（GnRHa）治疗卵巢巧克力囊肿的临床疗效。方法：选取行腹腔镜巧克力囊肿剥除术的单侧卵巢巧克力囊肿97例，采用数字表法随机分为观察组49例和对照组48例。观察组术后第2天开始小剂量口服米非司酮10 mg/d，持续服用3个月；并在术后第1次月经来潮第2天肌内注射GnRHa，每次3.75 mg，每28 d 1次，连续注射3次。对照组仅注射GnRHa，用药方法同观察组。1年后，比较2组的治疗效果，并观察记录出现的不良反应。结果：用药1年后，观察组临床总有效率和复发率分别为100.00%和4.08%；对照组临床总有效率和复发率分别为87.50%和14.58%，2组的总有效率和复发率差异均无统计学意义（P>0.05）。治疗前和治疗后2组血清促卵泡激素（FSH）、促黄体生成素（LH）、雌二醇（E2）和孕酮（P）差异均无统计学意义（P>0.05）；2组治疗后血清FSH和LH水平均较治疗前显著提高，E2和P水平均较治疗前显著降低（P<0.01）。观察组不良反应发生率为38.78%，对照组不良反应发生率为35.42%，2组不良反应发生率差异无统计学意义（P>0.05）。结论：卵巢巧克力囊肿术后小剂量米非司酮联合GnRHa治疗效果好，复发率低，并利于改善卵巢功能。Abstract: Objective: To investigate the clinical effect of low ovarian chocolate cysts after mifepristone with GnRHa therapy. Methods: Ninety-seven cases that they were underwent laparoscopic cystectomy chocolate unilateral ovarian chocolate cysts were choosed, by the use of digital they were randomly divided into observation group 49 cases and control group 48 cases.Second day after operation observation group started low doses of oral mifepristone,10 mg/d, continued to take 3 months; and after the first menstrual cramps second day intramuscular GnRHa,3.75 mg every time,every 28 days,continuous injection of three times.Control group was injected GnRHa, treatment methods with the observation group.After 1 year,the treatment of two groups were compared,and the recording of adverse reactions observed appears.Results: After 1 year, the total effective rate and the recurrence rate of the observation group were 100.00% and 4.08% respectively;the total effective rate and the recurrence rate of the control group were 87.50% and 14.58%,respectively.The total efficiency and recurrence rate between the two groups were not significantly different(P>0.05).Before and after treatment between the two groups in serum FSH,LH,E2,P was not statistically significant (P>0.05);former levels of serum FSH,LH,E2,P level of treatment and treatment after comparison,FSH was significantly improved water,LH,E2,P levels were significantly reduced,and the differences were statistically significant(P<0.01).The incidence of adverse reaction in the observation group and the control group were 38.78% and 35.42%,was not statistically significant(P>0.05).Conclusions: Effect of low dose mifepristone combined with GnRHa treatment in chocolate cyst of ovary well after operation,the recurrence rate is low,and will help to improve ovarian function.
