胸段硬膜外镇痛(TEA)已被证明是最有效的术后镇痛方法,可促进胸部或腹部手术后的康复。单用布比卡因或联合舒芬太尼行TEA可能显著抑制逼尿肌功能,从而导致临床相关的残余尿量(PVR)增加,因此需要监测或先行导尿[1]。研究[2]表明罗哌卡因在剖宫产腰硬膜外间隙麻醉中对下肢运动阻滞的影响较布比卡因小,2种局部麻醉药对膀胱功能的影响可能不同。罗哌卡因的镇痛效力约为布比卡因的60%[3],本研究比较了TEA中使用罗哌卡因与等量布比卡因对膀胱功能影响。现作报道。

本研究为单中心、随机、双盲、平行、对照的前瞻性研究。选取2017年9月至2018年5月于我院择期行开放胸腹部手术病人50例;ASA分级Ⅱ~Ⅲ级;不存在下尿路梗阻症状(超声显示PVR < 100 mL)。排除标准:任何存在TEA禁忌以及妊娠的病人。将50例病人采用随机数字表法分为A组(布比卡因镇痛组)和B组(罗哌卡因镇痛组),每组25例。2组性别、年龄、身高、手术时间、硬膜外阻滞平面以及疼痛评分差异均无统计学意义(P>0.05)(见表 1), 具有可比性。

2组病人麻醉诱导前均在T7~8或T8~9间隙放置胸段硬膜外导管。采用经典定位确定插入部位,坐位与肩胛骨下端相交的线上识别T7脊髓。硬膜外穿刺针采用中位或副正中位入路,硬膜外间隙采用阻力丧失技术确定。试验剂量为1.5 mL利多卡因20 mg/mL和0.005 mg/mL肾上腺素,以排除蛛网膜下腔或血管内放置。2组均于手术前20 min用布比卡因2.5 mg/mL激活TEA,速率为6~10 mL/h。术中未采用阿片类药物。全麻诱导采用咪达唑仑、舒芬太尼、依托咪脂,顺阿曲库铵、异丙酚持续泵入维持麻醉深度。诱导后插入尿道导管,留置至下一次尿动力学检查。术后2组TEA用药分别为布比卡因1.25 mg/L和罗哌卡因2 mg/L,初始输注速度为8 mL/h,加药量为5 mL,锁定时间1 h。由于罗哌卡因的镇痛效力约为布比卡因的60%,所以使用高浓度罗哌卡因达到等效化镇痛。分别在术前1天和术后使用TEA期间对病人进行2次尿量监测并采集数据。

PVR差(△ PVR=术后TEA期间PVR-术前基础PVR)。记录初始尿意期膀胱容积、强烈尿意期膀胱容积、最大膀胱容积、膀胱顺应性、最大逼尿肌压力、最大流量逼尿肌压、不同时间点PVR(术后TEA期与基线时)。膀胱收缩指数(反映逼尿肌收缩强度)=最大流量逼尿肌压力+5×最大尿流率。膀胱排尿效率=(排尿量/膀胱容量)×100%。记录罗哌卡因和布比卡因可能引起的相关不良反应。

TEA期间2组PVR均较基础值增加(P < 0.05),A组PVR高于B组(P < 0.01);TEA期间排尿量A组大于B组(P < 0.01);TEA期间2组最大尿流率和膀胱收缩指数均较基础值减少(P < 0.05),且A组低于B组(P < 0.01)(见表 2)。

目的通过对残余尿量以及最大尿流量的监测,比较罗哌卡因与布比卡因对胸段硬膜外镇痛(TEA)病人尿路功能的影响。 方法选取择期行开放胸腹部手术病人50例,采用随机数字表法分为A组(布比卡因镇痛组)和B组(罗哌卡因镇痛组),每组25例。分别在术前及TEA时行尿动力学检查。主要观察指标是术前与术后TEA术后残余尿量(PVR)的差异。次要观察指标是各组之间和组内最大尿流量的变化。 结果TEA期间2组PVR均较基础值增加(P < 0.05),A组PVR高于B组(P < 0.01);TEA期间排尿量A组大于B组(P < 0.01);TEA期间2组最大尿流率和膀胱收缩指数均较基础值减少(P < 0.05),且A组低于B组(P < 0.01)。2组基础值和TEA期间初感尿意、强烈尿意、膀胱最大容量、膀胱顺应性参数比较差异均无统计学意义(P>0.05)。 结论罗哌卡因用于TEA的残余尿量明显低于布比卡因,罗哌卡因对逼尿肌功能的影响较小。 Abstract:ObjectiveTo compare the effects of ropivacaine and bupivacaine on urinary tract function in patients with thoracic epidural analgesia(TEA) by monitoring residual urine volume and maximum urine flow. MethodsFifty patients undergoing elective open thoracic and abdominal surgery were selected and divided into group A(bupivacaine analgesic group) and group B(ropivacaine analgesic group) by random number table method, with 25 patients in each group.Urodynamic examination was performed in preoperative and TEA.The primary observation index was the difference of postoperative residual urine volume(PVR) before and after TEA, and the secondary observation index was the change of maximum urine flow between groups and within groups. ResultsDuring TEA, PVR in both groups increased compared with the baseline value(P < 0.05), and PVR in group A was higher than that in group B(P < 0.01).The urine output in group A was higher than that in group B during TEA(P < 0.01).During TEA, the maximum urine flow and bladder contraction index in the two groups were decreased compared with the baseline value(P < 0.05), and which in group A was lower than that in group B(P < 0.01).There were no significant differences in baseline value, initial urine intention, strong urine intention, maximum bladder capacity and bladder compliance parameters between the two groups during TEA(P>0.05). ConclusionsThe residual urine volume of ropivacaine for TEA is significantly lower than that of bupivacaine, which reflects that ropivacaine has little effect on detrusor function. -
