脑卒中目前是全球老年人群主要的死亡和致残原因,全球每年新增病例超过1 300万例,在我国,缺血性脑卒中发病率持续上升,而出血性卒中发病率呈缓慢下降趋势[1-3]。阿司匹林等抗血小板药物作为心脑血管疾病二级预防的基石,已被证实在降低心脑血管事件的早期风险中具有重要作用[4-5]。而从21世纪初,在国内外患有高血压、糖尿病和血脂异常等血管危险因素的50岁以上的中老年人,已经开始服用阿司匹林用来预防缺血性卒中,对缺血性心脑血管疾病的发生起到了有效的防治作用。但是,值得注意的是,一些中老年人服用阿司匹林长达5年或10年以上,但仍有部分人群出现复发性卒中。长期服用阿司匹林还有预防脑卒中的作用吗?因为阿司匹林存在抵抗作用,所以长期服用阿司匹林对脑卒中的预防作用可能极小,而阿司匹林的不良反应较为突出,如消化道出血、脑出血等。这些问题虽然国内外已有一些研究,但随访时间都较短,3年以上的随访研究很少,没有强有力的证据证明阿司匹林的利弊作用[6-7]。所以需要探讨5年以上长期服用阿司匹林对脑卒中的防治作用,阿司匹林的不良反应是否增加了老年人的死亡率和胃肠道出血等问题。本研究通过前瞻性临床研究探讨长期服用阿司匹林预防缺血性脑卒中的有效性及安全性。

入组时纳入2 381人,随访丢失263人,其中87人拒绝进一步参与,176人搬离原住所,随访结束时共2 118人被纳入基线分析,其中阿司匹林组共1 015人,非阿司匹林组共1 103人,平均年龄71.7岁,男1 083人(51.1%),女1 035人(49.2%)。在2 118名研究对象中,5年随访结束时,阿司匹林组381人、非阿司匹林组491人发生缺血性脑卒中。在血管危险因素中,血脂异常(39.8%)、高血压(31.2%)和2型糖尿病(27.8%)是最常见的疾病。在基线特征方面,阿司匹林组与非阿司匹林组差异均无统计学意义(P>0.05)(见表 1)。

阿司匹林组381人、非阿司匹林组491人发生了缺血性脑卒中,主要结果急性脑梗死和TIA在阿司匹林组与非阿司匹林组之间差异有统计学意义(急性脑梗死:调整后HR=0.879,95%CI:0.763~0.977,P < 0.05;TIA:调整后HR=0.862,95%CI:0.781~0.958,P < 0.05)。次要结果中,心绞痛(调整后HR=0.924,95%CI,0.786~0.992,P < 0.05)、心肌梗死(调整后HR=0.728,95%CI:0.612~0.969,P < 0.05)在2组之间差异有统计学意义;心血管死亡(调整后HR=0.976,95%CI:0.718~1.474,P>0.05)、全因死亡(调整后HR=1.043,95%CI: 0.806~1.346,P>0.05)在2组差异无统计学意义。在服用阿司匹林组与未服用阿司匹林组间比较,30 d NIHSS评分、90 d mRS评分差异有统计学意义(P < 0.05)(见表 2)。

在阿司匹林组与非阿司匹林组中,进一步分析了阿司匹林服用时间与缺血性卒中发生的关系。阿司匹林组中服用阿司匹林 < 13个月、13~24个月、25~36个月和37~48个月者比非阿司匹林组发生缺血性脑卒中的例数显著降低(P < 0.05~P < 0.01),而服用阿司匹林49~60个月者与非阿司匹林组发生例数差异无统计学意义(P>0.05)(见表 3)。

阿司匹林组与非阿司匹林组比较,颅内出血、其他部位出血和胃肠道肿瘤发生情况差异无统计学意义(P>0.05)。但2组在胃肠道出血(4.2% VS 2.4%)和胃肠道不良反应(16.1% VS 8.3%)方面差异有统计学意义(P < 0.05)(见表 4)。

长期服用阿司匹林预防缺血性脑卒中的有效性及安全性分析

目的探讨长期服用阿司匹林对缺血性脑卒中的有效性及安全性。 方法纳入2015年1月至2016年5月在重庆市陆军特色医学中心体检中心的老年人群,入组时纳入2 381人,随访丢失263人,5年随访结束时共2 118人被纳入基线分析,平均年龄71.7岁,51.1%是男性。根据阿司匹林的服用情况进行分组,其中阿司匹林组被定义为根据处方记录入组时服用阿司匹林者,非阿司匹林组被定义为入组后观察期间未服用阿司匹林者。研究中观察的主要结果为缺血性脑卒中,包括缺血性脑梗死和短暂性脑缺血(TIA);次要结果为心绞痛、心肌梗死、心血管死亡和全因死亡。安全性结果包括胃肠道出血、颅内出血和其他部位的出血,采用Cox比例风险模型估计阿司匹林与缺血性脑卒中的关系。 结果阿司匹林组共1 015人,非阿司匹林组1 103人。5年随访结束时,阿司匹林组381人、非阿司匹林组491人发生缺血性脑卒中。主要结果和次要结果(心绞痛、心肌梗死)在2组间差异有统计学意义(P < 0.05),心血管死亡与全因死亡差异无统计学意义(P>0.05)。阿司匹林组胃肠道出血(4.2% VS 2.4%)和胃肠道不良反应(16.1% VS 8.3%)高于非阿司匹林组(P < 0.05)。阿司匹林组与非阿司匹林组比较,服用阿司匹林 < 13个月、13~24个月、25~36个月和37~48个月缺血性脑卒中的风险显著降低(P < 0.05),而服用阿司匹林49~60个月缺血性脑卒中发生率差异无统计学意义(P>0.05)。 结论长期服用阿司匹林降低缺血性脑卒中的发生有效性逐渐降低,并增加胃肠道出血的风险。 Abstract:ObjectiveTo investigate the efficacy and safety of long-term aspirin in the treatment of ischemic stroke. MethodsA total of 2 381 elderly people in the Physical Examination Center of Chongqing Army Characteristic Medical Center from January 2015 to May 2016 were included.A total of 2 118 people were included in the baseline analysis at the end of 5-year follow-up.The mean age was 71.7 years, and 51.1% were male.The patients were divided into two groups according to the use of aspirin, in which the aspirin group was defined as those who took aspirin when they were recorded according to the prescription, and the non-aspirin group was defined as those who did not take aspirin during the observation period.The primary results observed in the study were ischemic stroke, including ischemic cerebral infarction and transient cerebral ischemia(TIA), and the secondary results were angina pectoris, myocardial infarction, cardiovascular death and all-cause death.Safety results included gastrointestinal hemorrhage, intracranial hemorrhage and other bleeding.Cox proportional hazard model was used to estimate the relationship between aspirin and ischemic stroke. ResultsThere were 1 015 patients in aspirin group and 1 103 patients in non-aspirin group.At the end of 5-year follow-up, ischemic stroke occurred in 381 patients in the aspirin group and 491 in the non-aspirin group.The primary and secondary results(angina pectoris, myocardial infarction) were significantly different between the two groups(P < 0.05), but there was no significant difference between cardiovascular death and all-cause death(P>0.05).Gastrointestinal bleeding(4.2% vs 2.4%) and gastrointestinal adverse reactions(16.1% vs 8.3%) in the aspirin group were higher than those in the non-aspirin group(P < 0.05).The risk of ischemic stroke was significantly reduced after taking aspirin for < 13 months, 13-24 months, 25-36 months and 37-48 months(P < 0.05), but there was no significant difference in the incidence of ischemic stroke after taking aspirin for 49-60 months(P>0.05). ConclusionsLong-term use of aspirin gradually reduces the effectiveness of preventing ischemic stroke and increases the risk of gastrointestinal bleeding. -
