Influence of ureaplasma urealyticum infection on the structure of testicular tissue and the level of testosterone in rats

Abstract:
Objective:To study the influences of ureaplasma urealyticum(UU) infection on the structure of testicular tissue and level of testosterone in rats.Methods:Forty clean grade male Sprague dawley(SD) rats were randomly divided into the experiment group A(at 7 d after surgery), experiment group B(at 14 d after surgery, group B), control group C(at 7 d after surgery) and control group D(at 14 d after surgery)10 rats each group.The group A and B were injected with 0.6 ml UU4(1×107CCU/ml) through punctio vesicae, and the group C and D were injected with the same volume of UU liquid medium.At day 7 and 14 after injection, the structures of testicular tissue of all rats were observed by HE staining, the levels of testosterone in the testis interstitial fluid and plasma of all rats were detected by chemiluminescence method.Results:The changes of inflammatory pathology(including the layer and amount of seminiferous tubule spermatogenic cell decreasing, basement membrane lesion, inflammatory cell infiltrating and mature sperms decreasing) in the testis of group A and B were found by HE staining, the inflammatory changes in group B were lighter than those in group A.The structures of testicular tissue in group C and D were normal.The levels of plasma testosterone in group A and B were significantly lower than that in group C and D(P<0.01), the difference of plasma testosterone level between group A and B was not statistically significant(P>0.05).The testosterone level in testis interstitial fluid in group A was significantly lower than that in group B, C and D(P<0.01), the differences of the testosterone levels in testis interstitial fluid between group B, C and D were not statistical significant(P>0.05).Conclusions:UU infection can lead to the pathologic change of testicular tissue and testosterone secretion decrease.With the extension of time, the testicular tissue structure and testosterone level can gradually recover.
Imaging anatomy of the branch of brain cingulate culcus and its significance

Abstract:
Objective:To observe the vertical branches and probability of occurrence in cingulate sulcus for providing the anatomical basis in studying the development pattern and relationship between function and structure of cingulate sulcus.Methods:The brain sagittal MRI(7 mm each layer) of 30 volunteers were obtained.The median sagittal MRI imaging of brain were transported into eFilm 1.5, the vertical branches and appearance frequency of cingulate sulcus were observed.Results:The four types(including zero, one, two and three vertical branches) in cingulate sulcus were found according to the paracingulate sulcus appearance.There were side and individual differences in each type.Conclusions:There is obviously side difference in the branch occurrence of cingulate sulcus.Compared with right side, less branch in left side is found, which indicates that the folding degree in fight cingulate sulcus cortex is high than that in left side.
Construction and packaging of the recombination lentivirus vector with RNA interference gene targeting human Fas

Abstract:
Objective:To construct the recombination lentivirus vector with short hairpin RNA gene targeting human factor associated suicide(Fas), and silence the Fas gene expression in umbilical cord mesenchymal stem cells.Methods:According to Fas gene mRNA sequence, four groups of the short hairpin RNA gene sequence targeting Fas were designed, synthesised and annealed to form double-stranded DNA fragments, which was inserted the LV3 vector digested by BamHⅠ and EcoRⅠ enzyme, then it was transfected into the competent cells.The positive clones were identified by PCR, and analyzed by sequencing.The packaging and titer of lentivirus were determined after transfecting 293T cells.Results:Lentiviral vector was constructed successfully by the identification of restriction enzyme digestion and gene sequencing.The lentivirus vector was transfected into 293 T cells using the fluorescence microscope observation, the infection efficiency of which was more than 90%.The high titer of Lentiviral vector was obtained, which was 3×108 TU/ml.Conclusions:The recombination lentivirus vector with interference gene targeting human Fas is successfully constructed, which can provide the basis of transfecting umbilical cord mesenchymal stem cells.
The effect of estrogen on retinal ganglion cell apoptosis and Bcl-2 expression in retinal ischemia-reperfusion injury of rabbit

Abstract:
Objective:To explore the effects of estrogen on retinal ganglion cell apoptosis and Bcl-2 expression in retinal ischemia-reperfusion injury of rabbit.Methods:The animals were randomly divided into the sham operation group, ischemia group and estrogen treatment group, the ischemia group and estrogen treatment group were subdivided into 3, 6, 12, 24 and 72 h time-point after reperfusion.The retinal ischemia-reperfusion injury model of rabbit was established.The retinal ganglion cell apoptosis was detected using TUNEL method, the bcl-2 expressions in retinal tissue at different time-points were detected by immunohistochemistry SABC method.Results:The number of TUNEL-positive retinal ganglion cells in estrogen treatment group at 12, 24 and 72 h after reperfusion were less than that in ischemia group(P<0.05 to P<0.01).Compared with the ischemia group, the number of bcl-2 positive cells in estrogen treatment group significantly increased at 12, 24 and 72 h after reperfusion(P<0.01).Conclusions:Estradiol benzoate can increase the bcl-2 anti-apoptosis gene expression in retinal ischemia-reperfusion injury, and play a therapeutic effect on retinal ischemia reperfusion injury through inhibiting the cell apoptosis.
The correlation of the polymorphism of cytotoxic T lymphocyte associated antigen-4 gene and recurrence of Graves' disease

Abstract:
Objective:To explore the correlation of the polymorphism of cytotoxic T lymphocyte associated antigen-4(CTLA-4) gene and recurrence of Graves' disease(GD).Methods:The A/G polymorphisms of CTLA-4 gene exon seventeenth codon 49 locus in 102 patients with GD recurrence, who included 60 recurrence cases within 3 years of withdrawal drug(A group) and 42 recurrence cases after 3 years of withdrawal drug(B group), 40 primary GD patients(primary group) and 80 healthy people(normal control group) were detected using polymerase chain reaction restriction fragment length polymorphism.Results:The frequency distribution of genotype and allele in CTLA-4 gene exon 1 of 49 locus showed that the frequency of GG genotype and G allele in primary GD patients were higher than those in normal control group(P<0.01), the frequency of GG genotype in A group was higher than that in B group and primary group(P<0.01), and the frequency of GG genotype in B group was higher than that in primary group(P<0.01).Conclusions:The A/G polymorphism of CTLA-4 gene exon seventeenth codon 49 locus is related to the GD occurrence, the GG genotype of which may be the reason of the poor effect of antithyroid drugs and early recurrence in GD patients.
MicroRNA-221 expression affects invasion potential of human prostate carcinoma cell lines by regulating disheveled-2

Abstract:
Objective:To evaluate the effect of microRNA-221(miR-221) on the neuroendocrine(NE) differentiation and invasive function of prostate cancer cells.Methods:The expressions of 7 miRNAs in the cell lines of androgen-dependent carcinoma of prostate(LNCaP) and androgen-independent carcinoma of prostate(LNCaP-AI) were detected by Northern blotting.The LNCaP and LNCaP-AI cells cultured in androgen-depleted medium were transfected with different synthetic miR-221, and their invasive abilities were evaluated by a matrigel invasion assay;the cell growth was assessed by using the cell counting kit-8 cell proliferation assay at different time points;the expression of neuron-specific enolase and dishevelled-2 (DVL2)during NE differentiation and migration was respectively measured by quantificational real-time polymerase chain reaction and Western blot.Results:The miR-221 level was increased significantly in LNCaP-AI cell line when compared with LNCaP cell line.Overexpression of miR-221 in LNCaP cells significantly promoted neuron-specific enolase expression and accelerated NE differentiation;suppression of miR-221 expression improved the abilities of migration and invasion of LNCaP-AI cells;meanwhile, the DVL2 mRNA and protein levels were upregulated after the transfection of anti-miR-221 in LNCaP-AI cells(P<0.01).Conclusions:There is a significant difference in miR-221 expression between androgen-dependent prostate cancer and androgen-independent prostate cancer cells.MiR-221 contributes to the NE differentiation of LNCaP cells, which may be the reason of androgen-independence.MiR-221 may regulate the migration of LNCaP-AI cells through DVL2 in advanced prostate cancer.
Analysis of intensity-modulated radiation therapy in 45 patients with local recurrence of esophageal cancer after radiotherapy

Abstract:
Objective:To evaluate the feasibility, short-term curative effect, adverse reaction and its related factors of intensity-modulated radiation therapy(IMRT) in patients with local recurrence of esophageal cancer after radiotherapy.Methods:Forty-five patients with local recurrence of esophageal cancer after radiotherapy were treated with 1.8 to 2.0 Gy of IMRT 5 times a week, a median total dose for 50.4 Gy(range from 45 to 60 Gy).Results:Among 45 patients, the re-irradiation therapy in 42 patients were completed, complete response in 8 cases, partial response in 27 cases and stable disease in 7 cases were found.Grade 2 and grade 3 acute radiation esophagitis occurred in 18 cases and 2 cases, respectively.Grade 2 and grade 3 acute radiation pneumonitis occurred in 7 cases and 1 case, respectively, and the patients relieved after active treatment.Five patients occurred the perforation of esophagus in 3 months after radiotherapy, and died within 6 months.Three patients terminated the radiation because of the general deterioration, 1 patient died because of pneumorrhagia in six months after radiotherapy.The total survival rates of 1 and 2-year were 40.0% and 17.8%, respectively.The patients with complete remission survived over 2 years.Conclusions:The treatment of patients with local recurrence of esophageal cancer after radiotherapy with IMRT is effective, which has certain remission rate and high complication.The perforation of esophagus and more than grade 3 radiation pneumonitis should be alerted.
The expression of survivin protein in lung and esophageal squamous cell carcinoma

Abstract:
Objective:To investigate the expressions of survivin protein in esophageal and lung squamous cell carcinoma.Methods:The expressions of survivin protein in esophageal and lung squamous cell carcinoma sample were detected using immunohistochemical staining.Results:The expressions of survivin in lung and esophageal squamous cell carcinoma were high.Survivin positive staining was located mainly in cytoplasm, partly in nuclear, and partly in nuclear and cytoplasm.The differences of survivin expression in cytoplasm in patients age, cell differentiation and lymph node metastasis of tumor were not statistically significant(P>0.05).The survival rate of patients with lung squamous cell carcinoma complicated with survivin expression in cytoplasm was lower than that in patients with esophageal squamous cell carcinoma(P<0.05).Conclusions:The expression of survivin in cytoplasm is not related to the tumor type.The expression of survivin in cytoplasm is not related to the characteristics of lung and esophageal squamous cell carcinoma.The expression of survivin in cytoplasm is related to the prognosis of lung and esophageal squamous cell carcinoma.
Reasons for misdiagnosis of palpation-negative breast cancer:an analysis of 23 cases

Abstract:
Objective:To improve the diagnosis of breast cancer by analyzing the reasons for misdiagnosis of palpation-negative breast cancer.Methods:Twenty-three cases of palpation-negative breast cancer confirmed by pathology were analyzed retrospectively, and the reasons for misdiagnosis were summarized.Results:Of the 23 cases misdiagnosed, 16 cases were proved to have mammary gland ill hyperplasia, 4 cases mammary gland hyperplasia with tumor formation trend, 1 case had postoperative scarring nipple eczema, 1 case Nipple eczema and 1 case breast intraductal papilloma.The misdiagnosis occurred 3 months to 1 year before confirmation.Conclusions:The features of palpation-negative breast cancer are usually not typical.Clinicians should heighten vigilance and use timely or jointly proper inspection methods to improve the diagnosis accuracy of breast cancer.
The diagnosis and treatment of 32 cases with upper gastrointestinal tract perforation

Abstract:
Objective:To investigate the clinical diagnosis and treatment of upper digestive tract perforation.Methods:Select 32 cases of upper digestive tract perforation, among them 31cases employ emergency operation treatment, 29 cases employ simple repair, 2 cases employ a majority of gastrectomy, 1 case employ expectant treatment.Results:Thirty-one cases cured, and 1 case died.Conclusions:Selecting reasonable operation type, striving early operation, strengthening perioperative management, and active preventing and treating complications and basic disease for the patients with upper digestive tract perforation according to their pollution level of abdominal cavity, lesion site, nature of lesion, and patient's age and basic diseases can improve curative effects.
The effect of total hip replacement in the treatment of developmental dysplasia of the hip in 24 adults

Abstract:
Objective:To explore the near- and middle-term curative effects of total hip replacement in the treatment of developmental dysplasia of the hip(DDH) inadults.Methods:Twenty-four patients with DDH were treated with total hip replacement.The preoperative Harris scores, postoperative X-ray, operation time and bleeding volume of all cases were analyzed.Results:All patients were followed-up for 24 to 36 months.The preoperative and postoperative Harris scores were 28 to 65 and 72 to 92, respectively.The sciatic nerve palsy, deep vein thrombosis, wound infection and artificial dislocation were not be found in all patients.X-ray showed the prosthesis location was good, and no aseptic loosening and sinking of prosthesis appeared.Conclusions:The near-term curative effects of total hip replacement in the treatment of DDH of adults are good.The sufficient preoperative assessment, choosing suitable prosthesis and reconstruction of acetabulum center are the key of successful operation.
The expression of microRNA-100 in gastric cancer and its clinical significance

Abstract:
Objective:To detect the expression of microRNA-100(miRNA-100) in gastric cancer and its relation with clinicopathologic features.Methods:The total RNA in 50 gastric cancer tissue specimens and non-tumor adjacent tissue specimens were extracted, the expression levels of miRNA-100 were detected by TaqMan real-time quantitative PCR assay.The correlations of miRNA-100 expression and differentiation and lymph node metastasis of gastric cancer were analyzed.Results:The expression levels of miRNA-100 in gastric carcinoma tissue and normal gastric tissue were 1.31±0.69 and 0.74±0.36, respectively, the difference of which was statistical significance(P<0.01).The differences of miRNA-100 expressions in age, gender, tumor location, tumor size, degree of differentiation, lymph node metastasis and TNM staging were not statistically significant(P>0.05).Conclusions:The abnormal expression of miRNA-100 may be associated with the occurrence of gastric cancer.
Significance of serum lactate level and lactate clearance rate in predicting the prognosis of critically ill patients

Abstract:
Objective:To observe the significance of the lactic acid level, lactic acid clearance rate, and the acute physiology and chronic health evaluation Ⅱ(APACHEⅡ) score in evaluating the clinical efficacy and prognosis of critically ill patients.Methods:The serum lactic acid level and the clearance rate were monitored, and the APACHE Ⅱ score was determined in the first 24 hours in 68 critically ill patients in the intensive care unit(ICU).The patients were divided into survival group, death group, high lactic acid clearance group and low lactic acid clearance group.Results:The 6 h lactic acid clearance rate of the survival group was significantly higher than that of the death group(P<0.01);the APACHEⅡ score and mortality rate of the patients with high lactic acid clearance was significantly lower than those with low lactic acid clearance(P<0.01).Conclusions:The lactic acid clearance rate in critically ill patients may indicate the severity of the illness and are worth clinical concerning.
Depression in patients with acute stroke and the related factors

Abstract:
Objective:To investigate the prevalence of post stroke depression(PSD) at stroke acute phase and the related risk factors, and to provide clinical evidence for early diagnosis and treatment of it.Methods:Case-control study was used.102 patients of stroke were collected.The necessary conventional data including socio-demographic, medical history, blood, computed tomography(CT) or magnetic resonance imaging(MRI) scan examination were obtained.The patients were assessed 2 to 4weeks after the onset of stroke and divided into PSD group or control group according to the diagnostic criteria of depression in CCMD-3 of Chinese and the Hamilton Depression Rating Scale.The neurological deficit was estimated by National Institutes of Health Stroke Scale(NIHSS).The daily life skills were assessed by activities of daily living scales(ADL).Results:Of the 102 patients investigated, depressive disorder occurred in 45.09% of the cases(n=46).It was not related to the nature or location of the stroke, and there was no statistically significant difference between patients with diabetes, hypertension, coronary heart disease or hyperlipidemia(P>0.05), but patients with ≥2 complications were more susceptible to PSD(P<0.05).The score of NIHSS and ADL was consistently associated with the incidence of depression(P<0.01) and the score of ADL protection factors of PSD.Conclusions:Depression is a frequent and common complication among patients with stroke.The incident of PSD is not related with the nature or location of stroke, but obviously related to ADL and NIHSS.PSD may be associated with the risk factors for cerebrovascular diseases and complications.
Analysis of the related factors of the carotid atherosclerotic plaque stability

Abstract:
Objective:To investigate the risk factors of the carotid atherosclerotic plaque stability.Methods:Eighty-four patients with carotid plaque were divided into the unstable plaque group(31 cases) and stable plaque group(53 cases) according to the results of carotid artery ultrasound.The relationships between the risk factors of C-reactive protein(CRP), low density lipoprotein cholesterol(LDL-C), homocysteine(Hcy) and Cystatin C(CyC), and plaque stability in two groups were compared.Results:The levels of CRP, LDC-C, Hcy and CyC in unstable plaque group were higher than that in stable plaque group(P<0.05 to P<0.01).Logistic regression analysis showed that the CRP, LDC-C, Hcy and CyC were the risk factors of the carotid atherosclerotic plaque stability(P<0.05).Conclusions:The CRP, LDC-C, Hcy and CyC were the important risk factors of the carotid atherosclerotic plaque stability.
The relationship between meteorological factors and stroke incidence in Chaohu city

Abstract:
Objective:To investigate the effects of meteorological factors on the incidence of stroke for providing the basis of the stroke prevention.Methods:The data of 11100 cases with stroke from 2008 to 2012 were collected in Chaohu city.The relationships between the onset time of stroke and temperature, air pressure and relative humidity in Chaohu were analyzed using univariate correlation analysis, and the central tendency of disease was analyzed using a round distribution analysis.Results:The incidence of cerebral hemorrhage was negatively correlated with air temperature, positively correlated with air pressure(P<0.01), and not correlated with relative humidity(P>0.05).The cerebral infarction was positively correlated with air temperature(P=0.01), and not correlated with air pressure and relative humidity(P>0.05).The annual incidence of cerebral hemorrhage showed significantly central tendency from 2008 to 2012(P<0.05), and the total incidence also showed a central tendency, the average angle of which was 12.222 8°, equivalent to 13 January.The differences of the average angle of cerebral infarction in 2008, 2010 and 2012, equivalent to 8 July, 11 August, 20 September, were statistically significant(P<0.05), but the difference of five-year total average angle was not statistically significant(P>0.05).Conclusions:The incidence of cerebral hemorrhage in Chaohu city has a clear seasonality, major in December to February of winter and minor in summer.The onset of cerebral infarction trend tends to be higher in summer and autumn, lower in the spring and winter.
Effect of dexmedetomidine in attenuating the hypotension induced by epinephrine-lidocaine:a report of 100 cases

Abstract:
Objective:To evaluate the effect of dexmedetomidine(DEX) on the hemodynamic changes induced by scalp infiltration with epinephrine-lidocaine mixture during craniotomy under general anesthesia.Methods:One hundred patients with intracranial tumor were randomly and equally divided into five groups, and each group was infiltrated 16 ml of epinephrine-lidocaine mixture of different concentrations:group A 2.5 μg/ml, group B 5 μg/ml, group C 7.5 μg/ml, group D 10 μg/ml and group E 7.5 mg/ml received intravenous pumping of DEX 1 μg/kg before scalp infiltration.The hemodynamic changes were recorded before anesthetic induction, infiltration, using DEX and 5 min after infiltration.Results:The number of the patients with declined mean arterial pressure(MAP) over 20% was greater in group C than that in group D(P<0.05).The MAP and systolic blood pressure(SBP) had no statistically significant difference among the five groups in Tbase values and T0(P>0.05).The SBP in group E on T1, T2, and T7~T10 was higher than that on T0(P<0.05 to P<0.01);and compared with group C, the SBP on T1~T5, T8~T9, the diastolic blood pressure(DBP) on T1 and T3~T10, and the MAP on T1~T10 had significant differences(P<0.05);the DBP, MAP and heart rate of group E had no significant differences at different time-points(P>0.05).Conclusions:The epinephrine-lidocaine mixture used for scalp infiltration could reduce the arterial pressure of the patients undergoing craniotomy;intravenous pumping of DEX could attenuate the hypotension degree induced by scalp infiltration of epinephrine-lidocaine mixture.
Study on the application of clinical pathway in patients with acute ST elevated myocardial infarction

Objective:To evaluate the practicability of application of clinical pathway in patients with acute ST-elevation myocardial infarction(STEMI).Methods:Sixty-one cases of STEMI within 12 h of symptom onset were received primary percutaneous coronary intervention.All patients were randomly divided into two groups, 35 cases were brought directly to catheterization laboratory(rapid group), and 26 cases were admitted to the coronary care unit or cardiac ward firstly, then transferred to the catheterization laboratory(slow group).Hospitalization expenses, hospitalization time, the psychological changes of the subjects, patients' satisfaction degree were recorded.The psychological changes of the subjects were determined by self-rating anxiety scale(SAS) and self-rating depression scale(SDS).Results:In rapid group, the total hospitalization expenses, the western medicine expenses, and the hospitalization time were significantly shorter than in slow group(P<0.01);but the hospitalization expenses per day was higher than in slow group (P<0.01).SAS and SDS indices were decreased significantly in rapid group compared with those in slow group(P<0.01 and P<0.05).Patients were more satisfied with their treatments in rapid group (P<0.01).Conclusions:Application of clinical pathway in the STEMI patients can obviously decrease expenses, hospitalization time and psychological problems.And it can increase the satisfaction degree of patients.It is a new and effective management mode.
The effect of two kinds of anesthesia on the cognitive function and analgesia in elderly patients after hip arthroplasty

Abstract:
Objective:To investigate the effects of the sufentanil and flurbiprofen anesthesia on the cognitive function and analgesia in elderly patients after hip arthroplasty.Methods:One hundred and twenty patients treated with hip arthroplasty were divided into the flurbiprofen combined with sufentanil analgesia group(group A) and sufentanil analgesia group(group B) according to random number table(60 cases each group).Group A were treated with 100 mg of flurbiprofen combined with 2.0 mg of sufentanil, diluting to 200 ml, patient-controlled analgesia(PCIA) for 2 ml/30 min.Group B were treated with 2.0 mg of sufentanil, diluting to 200 ml, PCIA for 2 ml/30 min.The analgesic effects in two groups were scored using visual analog scale(VAS) after 3 h, 6 h, 12 h and 24 h of operation.The cognitive function in two groups were assessed using mini-mental state examination(MMSE) before operation and after 1 d, 3 d and 5 d of operation.The sufentanil dosage, PCIA times and complications of two groups were recorded after 1 h, 24 h and 48 h of operation.Results:The VAS scores in group A after 3 h, 6 h, 12 h and 24 h of operation were significantly lower than those in group B(P<0.01).The MMSE scores in group A after 1 d, 3 d and 5 d of operation were higher than those in group B(P<0.05 to P<0.01).The sufentanil dosage and PCIA times in group A after 1 h and 24 h of operation were less than those in group B(P<0.01).The incidence rates of nausea and vomiting, restlessness, respiratory depression and cognitive dysfunction in group A were significantly lower than those in group B(P<0.05).Conclusions:The flurbiprofen analgesia can effectively reduce the incidence of cognitive dysfunction and postoperative PCIA sufentanil usage, and improve sedation analgesia.

Abstract:
Objective:To observe the effects of different doses of candesartan in the treatment of the diabetic nephropathy(DN) complicated with hypertension and its safety.Methods:Sixty patients with DN complicated with hypertension were randomly divided into the observation group and control group(30 cases each group), which were treated with 8mg and 16 mg of candesartan by oral once a day, respectively.The levels of 24h urine albumin, serum albumin and blood pressure in two groups before and after treatment were observed.Results:Compared with before treatment, the 24 h urine albumin and blood pressure in two groups after treatment decreased significantly(P<0.01).The levels of plasma albumin in two groups after treatment increased, and its rising degree in observation group was significantly higher than that in control group(P<0.05).Conclusions:The treatment of the DN complicated with hypertension with candesartan can significantly reduce the levels of 24 h urinary albumin and blood pressure of patient, and which can play a good protection on the kidneys.

Abstract:
Objective:To estimate the association of plasma prolactin(PRL) concentration with the risks of breast cancer among premenopausal women.Methods:The concentration of plasma PRL was measured in 65 premenopausal women with breast cancer and 65 matched control subjects by radioimmunoassay.The relations between the plasma PRL concentration and the breast cancer in premenopausal women were observed using conditional logistic regression analysis.All the cases were grouped according to the pathological features;the association of the level of PRL and the risk of breast cancer for different sub-groups were evaluated.Results:The plasma PRL concentration of the case group was significantly higher than that of the control group.Increased risk of breast cancer was positively related with the elevated plasma concentration of PRL(adjusted odds ratio [OR] for the highest versus the lowest quartile=1.54, 95% confidence interval [95%CI]=0.84-4.07, Ptrend=0.031).The concentration of plasma PRL increased modestly in estrogen receptor positive carcinomas but did not change with the tumor size, grade or lymph node status.Conclusions:The results suggest that high plasma concentration of PRL is positively related with the breast cancer, especially estrogen receptor positive carcinomas among premenopausal women.

Abstract:
Objective:To investigate the sedative and analgesic effects of dexmedetomidine(Dex) in patients with paroxysmal supraventricular tachycardia(PSVT) undergoing radiofrequency catheter ablation(RFCA).Methods:Thirty patients with PSVT undergoing RFCA were randomly assigned to Dex group(n=15) and NS group(n=15).All patients were performed with local infiltration anesthesia at the puncture site.In Dex group, the patients were received Dex on maintenance dose of 0.05 μg·kg-1·h-1 to the end of RFCA procedure followed by loading dose of 1 μg/kg within a 10 min infusion, the patients in NS group were received 0.9% saline with same volume.The parameters including mean arterial pressure(MAP), heart rate(HR), visual analogue scale(VAS), and the observer's assessment of alertness/sedation scale were assessed at different time points.Additionally, the times of stopping procedure because of intolerant pain, peri-operative adverse events and patients' satisfaction were recorded.Results:Compared with NS group, MAP, HR and VAS were significantly decreased in Dex group(P<0.01).Patients in Dex group presented a favorite sedation without troubles in trigging tachycardia and delayed recovery, meanwhile, the times of stopping procedure because of intolerant pain was significantly decreased(P<0.01), and the satisfaction was higher than in NS group(P<0.05).Conclusions:Application of Dex in patients with PSVT undergoing RFCA presents a good sedation and analgesia, alleviates the changes of hemodynamics induced by procedural stress without any delayed recovery and respiratory inhibition, and achieves high satisfaction.However, the occurance of bradycardia should be noticed during loading dose infusion.

Abstract:
Objective:To study the relationship between the level of plasma homocysteine(Hcy) and subcortical small vessel disease(SVD), and the clinical significance.Methods:The levels of plasma Hcy in 90 cases with cerebral SVD(including lacunar infarction and white matter lesion) diagnosed by MRI(SVD group) and 90 hospitalized patients without SVD(control group) were compared.Results:The levels of plasma Hcy in SVD group were significantly higher than those in control group(P<0.01).Conclusions:The level of plasma Hcy increasing is an independent risk factor of cerebral SVD.

Abstract:
Objective:To observe the anesthetic effects of fiberoptic bronchoscopy positioning supreme laryngeal mask airway(SLMA) in kidney operation with lateral position.Methods:Sixty patients with ASA Ⅰto Ⅱ treated with lateral position kidney operation were randomly divided into SLMA group(30 cases) and tube intubation group(TT group, 30 cases).The levels of MAP, HR and PETCO2 were observed after intubation tube under anesthesia.The success rates of the first and second intubation, anesthetic complications and extubation response were recorded.Results:The success rates of the first and second intubation in SLMA group were 90.0% and 100.0%, respectively.The changes of MAP and HR levels at all time-points were not significant in SLMA group(P>0.05), the levels of MAP and HR at T2 to T5 time-points were significantly higher than those in T1 time-point in TT group(P<0.01).The increasing of MAP in T2 to T5 time-points and HR in T2 to T4 time-points in TT group were higher than those in SLMA group(P<0.05 to P<0.01).The cough and airway complications in 24 hours in SLMA group were less than those in TT group(P<0.05).Conclusions:The fiberoptic bronchoscopy positioning SLMA can be safely used in the anesthetic management of kidney operation with lateral position.
Monosegmental fixation for the treatment of single thoracolumbar vertebral fractures:a report of 18 cases

Abstract:
Objective:To evaluate the efficacy of monosegment fixation in the treatment of thoracolumbar fractures.Methods:A total of 18 patients with thoracolumbar fractures were treated with monosegmental pedicle instrumentation and bone graft fusion.The Cobb angle of the injured vertebral segment, percentage of the vertebral compression and the spinal canal sagittal diameter stenosis rate were compared before and after the operation.Neurological function was evaluated by the Frankel scale.Results:All the 18 patients were followed up.All the cases achieved bone fusion with no significant lose of the vertebrae body height or implant failure.X-ray film showed that the cobb angle of the injured vertebral segment, percentage of the vertebral compression and the spinal canal sagittal diameter stenosis rate improved significantly after operation(P<0.01).As to the Frankel scale, there were 2 patients at Grade B, 5 at Grade C, 7 at Grade D and 4 at Grade D before operation;and there were 3 patients at Grade C, 8 at Grade D and 7 at Grade E after operation.Conclusions:The transpedicular fixation through posterior approach combined with bony graft is an effective and reliable procedure for specific types of thoracolumbar vertebral fractures, but strict indication is the key to this procedure.
Nasal mucosa morphology change of chronic rhinosinusitis before and after operation and its correlation analysis

Abstract:
Objective:To explore the nasal mucosal morphology changes of chronic rhinosinusitis after operation and its correlation.Methods:Sixty patients with chronic rhinosinusitis and 30 healthy examination people were divided into the observation group and control group, respectively.The observation group were subdivided into the infection group and allergy group according to the clinical symptoms and pathological types.The observation group were treated with conventional Messerklinger endoscopic sinus surgery.The postoperative curative effects and nasal mucosa morphology in observation group were observed, and the correlative factors of influencing nasal mucosa recovery were analyzed.Results:The treatment effective rate in infection group(96.67%) was significantly higher than that in allergy group(63.33%)(P<0.01).The difference of nasal mucosa morphology between the infection group and allergy group before treatment was not statistically significant(P>0.05), the nasal mucosa morphology in infection group after treatment was better than that in allergy group(P<0.05 to P<0.01).Conclusions:The nasal mucosal morphology after endoscopic sinus surgery improves significantly, and its recovery is correlated to the pathological type.

Objective:To investigate the effects of the cognitive status of doctor-patient relationship on professional study in medical students.Methods:The medical students from the clinical medicine of Bengbu Medical College were investigated using self-made questionnaire.Results:Compared with junior students, the cognitions of tense doctor-patient relationship, risky professional doctor, heavy work pressure and more worry about doctor-patient relationship were found in senior students(P<0.01).Compared with junior students, the cognitions of more optimistic doctor-patient relationship and two sides responsibilities of doctor-patient dispute were found in senior students in the future(P<0.01).Compared with senior students, the professional ethics in junior students was paid more attention to(P<0.01).The news media was the main way of medical students understanding doctor-patient relationship, the teachers', words and deeds were important factors of affecting the cognitions of doctor-patient relationship in medical students(P<0.05).The professional satisfaction degree of senior medical students was lower than that in junior medical students(P<0.01).The learning enthusiasm of medical students with tense doctor-patient relationship was higher than that in students without tense doctor-patient relationship(P<0.01).Conclusions:The undesirable doctor-patient relationship has negative effects on professional satisfaction degree, learning enthusiasm and job-choosing view, which can be reduce by the effects of society, media, school and teacher.

Abstract:
Objective:To observe the effects of community comprehensive intervention guided by general medical theory on preventing diabetes.Methods:One hundred and three patients with type 2 diabetes were treated with community comprehensive interventions guided by general medical theory.The awareness rate of diabetes-related knowledge, movement and physical indicators in patients with type 2 diabetes were evaluated before and after intervention.Results:The awareness rate of diabetes-related knowledge after the intervention was significantly higher than before the intervention(P < 0.05);after the intervention, patients with diabetes significantly improved movement, the number of patients with movement was significantly higher than before the intervention, the number of patients without exercise and lack of exercise were significantly lower than before the intervention;after the intervention, the physical indicators were significantly better than before the intervention(P < 0.05).Conclusions:The community comprehensive intervention guided by general medical theory is good effect on diabetics, and can overcome the shortcomings of traditional medical model, which is worthy of promotion.

Objective:To provide theoretical basis for the intervention of social anxiety in medical students by analyzing the relationship between social anxiety and emotional intelligence.Methods:The Social Avoidance and Distress Scale(SAD) and Emotions Intelligence Scale(EIS) were employed to survey the medical students in Anhui Province by random sampling, and 948 of the 1 040 questionnaires were valid.Results:Emotional intelligence had significant predictive effects on social anxiety(P<0.01).emotional perception and management of emotions in others had significant negative prediction on social anxiety(P<0.01), and use of emotion had a significant positive prediction on social anxiety(P<0.01).Conclusions:The medical students with lower level of emotional intelligence have a higher level of social anxiety.Emotional intelligence is closely related to the social anxiety in the medical students, so improving the emotional intelligence may help to reduce the level of social anxiety.

Objective:To identify the correlations of between the levels of plasma glucose, blood cholesterol, tumor necrosis factor-α(TNF-α) and adiponectin(APN) and traditional chinese medicine syndromes in prediabetes.Methods:The levels of fasting plasma glucose(FPG), 2-hour plasma glucose(2 hPG), triglyceride(TG), total cholesterol(TCH), high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), TNF-α and adiponectin in 90 prediabetes patients with phlegm of Qi stagnation, dampness of spleen deficiency and Qi stagnation of Yin deficiency syndrome(30 cases each syndrome) and 20 healthy people(the control group) were detected.Results:Compared with the normal control group, the levels of FPG, TG and TCH and TNF-α increased, and the level of APN decreased in three syndromes patients, the differences of the levels of FPG and 2 hPG between three syndromes patients were not statistically significant(P>0.05).The levels of TG and TCH were the most significant(P<0.05), but the difference between and LDL-C was not statistically significant in phlegm of Qi stagnation patients(P>0.05).The differences of the levels of TNF-α and APN between three syndromes patients were statistically significant(P<0.05 to P<0.01).Conclusions:There are certain correlations between the levels of TCH, TG, TNF-α and APN and traditional chinese medicine syndromes in prediabetes, especial for in phlegm of Qi stagnation syndrome patients, which can provide a reference in identifying different syndromes in prediabetes.
Analysis of the irregular antibodies detection in 8 884 patients

Abstract:
Objective:To analyze the blood transfusion results of the patients with irregular antibodies, and provide the basis for avoiding hemolytic reaction.Methods:The antibodies of 8 884 patients were screened using the micro-column gel method, and the specificity of the positive samples were identified with panel cell.Results:Among 8 884 patients, 85 cases with positive irregular antibodies were found, the positive rate of which was 0.96%.The positive rates in male and female were 0.75% (19/2530) and 1.04% (66/6354), respectively.The positive rates in the patients with transfusion history or pregnancy history and without transfusion history and without pregnancy history were 1.17%(70/5980) and 0.52% (15/2904), respectively, the difference of which was statistically significant (P<0.01).Conclusions:The positives rates of irregular antibodies in female and the patients with transfusion history or pregnancy history are higher than that in male and the patients without transfusion history and pregnancy history.The screening of irregular antibody before transfusion can ensure its security.

Objective:To establish an optimal extraction process for Shiwei Guiqi Zhenwu granules.Methods:The yield of dry extract and the extraction rates of astragaloside A and paeoniflorin were used as the indices;the orthogonal design was used to investigate the influence of the solvent volume extraction time and extraction frequency on the extraction results.Results:The best extraction process was adding 6 times of water and decocting three tiwes, 1 hour each time.Conclusions:The optimal extraction process is feasible and reliable.

Objective:To investigate the antibiotics use in a 3A grade hospital, and to provide reference for rational clinical antibiotics use and scientifically establish procurement plan.Methods:Original antibiotics drug's data from the Hospital Information System of this hospital from 2009 to 2010 were collected, defined daily dose, sales amount sort and defined daily dose were used to analyze the data.Results:Comparing 2010 to 2009, the species of antibiotics drug were decreased by 13.64%, sale amounts were increased by 6.01%;Oral administration clarithromycin sustained release tablet, cefdinir capsule, and cefetamet pivoxil tablet were the top three sales amount in 2 consecutive years.Cefotiam for injection, aztreonam, cefathiamidine were the top three sales amount in 2 consecutive years.Conclusions:In the past two years, the varity number of antibiotics was decreased notably, and the ratio of antibiotics to total drugs was reasonable.The authors advise that continue to strengthen propagand of rational antibiotics use, strictly follow “Guiding Principles for Clinical Application of Antibacterial”, and reduce irrationally drug use phenomenon.
Perioperative care of severe heart failure patients treated with cardiac resynchronization therapy

Objective:To investigate the perioperative nursing experience of severe chronic heart failure patients treated with cardiac resynchronization therapy(CRT) or cardiac resynchronization therapy-cardioverter defibrillator(CRT-D).Methods:The data of the preoperative, intraoperative and postoperative nursing in 8 chronic heart failure patients treated with CRT or CRT-D were retrospectively analyzed.Results:The operations in 5 patients treated with CRT, 2 patients treated with CRT-D and 1 patient treated with CRT-D were unsuccessful, and then treated with implantable cardioverter defibrillator ICD were successful.Compared with before treatment, the cardiac function and ejection fraction of all cases were improved after treatment(P<0.01).The QRS wave becoming narrow and no cardiac tamponade, electrode dislocation and incision non-healing in all patients were found, the acute left heart failure in 1 case and incision blood effusion in 1 case were found.Conclusions:The CRT or CRT-D for treating chronic intractable heart failure is an effective means, the perioperative good care is an important means to get better effects.
Effect of family nursing intervention on the quality of life of liver cirrhosis patients

Objective:To investigate the effects of family nursing intervention on the quality of life of liver cirrhosis patients.Methods:The effects of family nursing intervention on the quality of life and compliance of liver cirrhosis patients were analyzed by QLQ-C30 and self-designed questionnaire.The experimental group and control group were treated with family nursing intervention and routine nursing, respectively.Results:The factors levels of the quality of life of liver cirrhosis patients were improved after family nursing intervention, the overall quality scores of life in experimental group after 3 months and 6 months of discharge were significantly higher than those in control group(P<0.01).The differences of the functional areas, symptoms areas and individual symptom factor between two groups were statistically significant(P<0.05 to P<0.01).The right medicine, daily routine, reasonable diet, moderate exercise, preventing infection and psychological adjustment compliance rate in experimental group after 3 months and 6 months of discharge were higher than those in control group(P<0.05 to P<0.01).Conclusions:Family nursing intervention can improve the quality of life and compliance of liver cirrhosis patients, which is worth of popularization in practice.
