Study on the paclitaxel long-circulating nanomicelles combined with NT21MP to reverse drug resistance in breast cancer MCF-7/PR cells

Abstract:
ObjectiveTo investigate the effects of paclitaxel long-circulating nanomicelles combined with vMIP-Ⅱ-N-terminal peptide(NT21MP) on drug resistance of paclitaxel-resistant brest cancer MCF-7/PR cells.MethodsSulforhodamine B(SRB) staining was used to detect the survival rate of breast cancer paclitaxel-resistant cell line MCF-7/PR.Cell scratch and Transwell assay were used to detect the effects of paclitaxel nanomicelles combined with NT21MP on the migration and invasion of paclitaxel-resistant cells.Flow cytometry was used to analyze the apoptosis and cell cycle.Western blotting was used to detect the expression of apoptosis, drug resistance and EMT-related proteins.ResultsCompared with paclitaxel group, the paclitaxel nanomicelles could effectively inhibit the cell proliferation, migration and invasion, the expression levels of apoptotic proteins Bax and caspase3 were up-regulated, the expression levels of anti-apoptotic proteins Bcl-2 were down-regulated, the expression levels of MRP, P-GP and EMT-related proteins Vimentin and Slug were down-regulated, and the expression levels of E-cadherin were up-regulated(P < 0.01).Compared with the paclitaxel nanomicelles group, the paclitaxel nanomicelles combined with NT21MP group had more significant effects on the inhibition of cell proliferation, migration, invasion, cycle, anti-apoptosis and reversal paclitaxel resistance.ConclusionsThe paclitaxel nanomicelles can effectively reverse the drug resistance of paclitaxel-resistant breast cancer cells, and the effects of the combination of paclitaxel manomicelles and NT21MP is more significant.
Study on the effects of 3-bromopyruvate on the sensitivity of human nasopharyngeal carcinoma cells to cisplatin and its mechanism

Abstract:
ObjectiveTo investigate the effect of 3-bromopyruvate (3-BrPA) on enhancing the sensitivity of human nasopharyngeal carcinoma cells to cisplatin and its mechanism.MethodsThe effects of 3-BrPA and cisplatin on the proliferation of nasopharyngeal carcinoma HNE1 cells were detected by MTT assay.The effects of 3-BrPA and cisplatin on the colony forming ability of HNE1 cells were observed using colony formation assay.The early apoptosis was analyzed by mitochondrial membrane potential detection kit.The nuclear morphological changes were detected by DAPI fluorescence staining.The apoptosis rate was detected by Annexin Ⅴ-FITC/PI double staining.The changes of intracellular ATP level were measured by ATP detection kit.The protein expressions of Bcl-2, Bax, Mcl-1 and Bak were analyzed by Western blotting.Results3-BrPA (20, 40, 80, 160 and 320 μmol/L), cisplatin (2, 4, 8, 16 and 32 μmol/L) and 80 μmol/L 3-BrPA combined with different concentrations of cisplatin significantly inhibited the proliferation of HNE1 cells in vitro, which was statistically significant compared with control group (P < 0.01).The combination of 8 μmol/L 3-BrPA and 0.8 μmol/L cisplatin significantly decreased the colony formation of HNE1 cells compared with that of 3-BrPA or cisplatin alone and control group (P < 0.01).The transformation of red fluorescence to green fluorescence was obvious, and nuclear fragmentation and nuclear pyknosis increased significantly in 80 μmol/L 3-BrPA combined with 8 μmol/L cisplatin group.The apoptosis rate and intracellular ATP level in 80 μmol/L 3-BrPA combined with 8 μmol/L cisplatin group were significantly higher and lower than those in 3-BrPA or cisplatin alone group and control group, respectively (P < 0.01).The expression levels of Bcl-2 and Mcl-1 protein were significantly downregulated, and the expression levels of Bax and Bak protein were significantly upregulated in 80 μmol/L 3-BrPA combined with 8 μmol/L cisplatin group, which was statistically significant compared with 3-BrPA or cisplatin alone group and control group (P < 0.05 to P < 0.01).Conclusions3-BrPA can induce apoptosis in HNE1 cells and enhance the sensitivity of HNE1 cells to cisplatin.The mechanism of which may be related to the decrease of intracellular ATP level, down-regulation of Mcl-1 and Bcl-2 protein, and up-regulation of Bak and Bax protein.
Role of long non-coding RNA LINC00173 in paclitaxel-resistant breast cancer cells

Abstract:
ObjectiveTo investigate the effects of long non-coding RNA LINC00173 on the proliferation, invasion and migration of paclitaxel-resistant breast cancer cells, and its possible mechanism of action.MethodsThe expression levels of LINC00173 in breast cancer cell lines(MCF-7, MDA-MB-231 and SKBR-3) and normal breast cancer epithelial cell lines(MCF-10A) were detected using qRT-PCR.The LINC00173 overexpression vector and interference fragment were constructed, and transfected into breast cancer cell lines, respectively.The qRT-PCR was used to detect the expression level of transfected LINC00173.The proliferation ability of breast cancer cells was detected using CCK8 assay.The transwell invasion and scratch assay were used to detect the invasion and migration of cells.The expression levels of CyclinD1, β-catenin and P-gp proteins were detected using Western blotting.ResultsCompared with MCF-10A cells, the expression level of LINC00173 in breast cancer cell line was low.After MCF-7 cells transfected with interference fragments and MCF-7/PR resistant cell lines transfected with overexpression vector of si-LINC00173, the expression level of si-LINC00173 decreased compared with the negative control group(si-NC).Compared with the LV-NC group, the overexpression of LV-LINC00173 increased, and the difference of which was statistically significant(P < 0.01).After interfering the expression of LINC00173, the proliferation and migration of MCF-7 cells enhanced(P < 0.01).After overexpression of LINC00173, the proliferation, invasion and migration of MCF-7/PR cells decreased(P < 0.01).After the interfering with LINC00173, the expression levels of CyclinD1, β-catenin and P-gp in MCF-7 cells significantly increased(P < 0.01).After the overexpression of LINC00173, the expression levels of CyclinD1, β-catenin and P-gp in MCF-7/PR decreased significantly(P < 0.01).ConclusionsThe decreasing expression level of LINC00173 in breast cancer cells may participate in the process of paclitaxel-resistance by regulating β-catenin expression and its occurrence, development and invasion.LINC00173 is a new molecule involved in the development of breast cancer, and which can provide a new theoretical basis for clinical diagnosis and treatment of breast cancer.
Effect of cinobufacin on the proliferation and expression of PTEN/AKT/mTOR signaling pathway in non-small cell lung cancer cell line A549

Abstract:
ObjectiveTo observe the expression changes of PTEN/AKT/mTOR signaling pathway protein after the lung cancer cells A549 treated with cinobufacin.MethodsDifferent concentrations of cinobufacin were added to the cultured lung cancer cell line NCI-A549, and the cell proliferation activity was measured using cell counting kit-8 after 24, 48 and 72 h.Ki67 and EdU were used to detect the proliferation of cells after 72 h of treatment with cinobufacin.Western blotting was used to detect the expression levels of protein kinase B/mammalian rapamycin target protein pathway protein(AKT/mTOR) and phosphorylated-phosphatase and tensin homolog deleted on chromosome ten(P-PTEN).ResultsThe proliferation of A549 cell line inhibited after treatment with 0.2 μg/mL cinobufacin for 48 h(P < 0.01), and the proliferation of A549 cell line was inhibited after treatment with 0.8 μg/mL cinobufacin for 12 h(P < 0.05 to P < 0.01).Compared with the control group, the expression levels of p-PI3K, p-AKT, p-mTOR, p-S6 and p-PTEN in experimential group after 72 h of treatment with different concentrations of cinobufacin decreased(P < 0.05 to P < 0.01), while the differences of the expression levels of PI3K, AKT, mTOR, S6 and PTEN were not statistically significant between the control group and erperimantial group(P>0.05).ConclusionsCinobufacin may inhibit the proliferation of lung cancer cells A549 by inhibiting the phosphorylation of PI3K/AKT/mTOR signaling pathway protein.
Application value of 18F-FDG PET/CT in the detection of metastasis or recurrence of ovarian cancer patients with CA125 increasing after operation

Abstract:
ObjectiveTo explore the value of 18F-FDG PET/CT in detecting the metastasis or recurrence of ovarian cancer patients with serum CA125 increasing after surgery.MethodsThe clinical data of 32 suspected metastasis or recurrence ovarian cancer patients with postoperative CA125 level increasing were retrospectively analyzed.The results of 18F-FDG PET/CT were compared with traditional imaging(CT or ultrasound).The diagnosis results of patients were confirmed by the pathological examination of reoperation or puncture biopsy, and following-up data of clinical and imaging data.ResultsThe results of 18F-FDG PET/CT showed that the common sites of metastasis or recurrence were pelvis, abdominal cavity and thorax in turn, and the metastasis organs included the lymph node, lung, liver, spleen and bone.The SUVmax value of metastatic or recurrent lesions was higher than the normal reference value.The sensitivity and accuracy of 18F-FDG PET/CT were higher than those of traditional imaging(P < 0.05 and P < 0.01), but the difference of the specificity between two methods was not statistically significant(P>0.05).Conclusions18F-FDG PET/CT has a high diagnostic efficacy in the metastasis or recurrence of ovarian cancer after operation.
Application value of percutaneous transhepatic gallbladder drainage guided by ultrasound in the treatment of acute biliary pancreatitis

Abstract:
ObjectiveTo explore the therapeutic value of percutaneous transhepatic gallbladder drainage(PTDG) guided by ultrasound in acute biliary pancreatitis.MethodsA total of 128 patients with acute biliary pancreatitis were divided into the observation group(64 cases treatment with PTGD) and control group(64 cases treatment without PTGD), and two groups were treated with the same routine method.After treatment, the indicators(including the abdominal distension relief time, exhaust time, hospital stay, white cells count, glutamic amino transferase, total bilirubin, procalcitonin, lactic acid and Bahhazar CT score), and complications(including peripancreatic effusion, peripancreatic infection, pancreatic fistula, ARDS, MODS incidence of effusion, peripancreatic infection, pancreatic fistula and mortality) were compared between two groups.ResultsCompared with the control group, the diarrhea remission time, exhaust time, peripancreatic effusion rate, peripancreatic infection rate and hospital stay in the observation group were low, and the differences of whose between two groups were statistically significant(P < 0.05 to P < 0.01).The differences of the pancreatic fistula rate, ARDS rate, MODS rate and mortality between two groups were not statistically significant(P>0.05).The differences of the WBC, ALT, bilirubin, PCT, lactic acid and Balthazar CT score between two groups on admission were not statistically significant(P>0.05), the levels of WBC, ALT, bilirubin, PCT and lactic acid in observation group were lower than those in control group after 3 d and 6 d of admission, and the Balthazar CT score was lower than that in observation group after 7 d of admission(P < 0.05 to P < 0.01).ConclusionsPTGD is effective and safe, and can significantly improve the therapeutic effect of acute biliary pancreatitis.
Value of endoscopic submucosal dissection and endoscopic full-thickness resection in the treatment of upper gastrointestinal submucosal tumors:197 cases analysis

Abstract:
ObjectiveTo assess the efficacy and safety of endoscopic submucosal dissection(ESD) and endoscopic full-thickness resection(EFR) in the treatment of the upper gastrointestinal submucosal tumors(SMT).MethodsThe clinical data of 197 patients with upper gastrointestinal SMT were retrospectively analyzed.All patients were treated with ESD or EFR.The tumor resection rate, operation time, intraoperative perforation rate, postoperative bleeding rate and histopathological results in the patients were evaluated.ResultsAmong the 197 patients, 154 cases originated from submucosa, were treated with ESD, and the lesions were completely stripped.The average operative time(from lesion labeling to complete dissection) of ESD was (24.0±3.6)min, no postoperative bleeding occurred, the perforation occurred during the operation, and the wound was successfully closed by titanium clips.Forty-three cases originated from submucosa, the tumors tightly adhered to the serous membrane layer, and were treated with EFR.Complete dissections in 42 cases were successfully performed, and the wound was successfully closed by titanium clips and nylon pocketbook suture under gastroscope.One case was changed to surgical operation due to the large tumor body and difficulty in adhesion and dissection.The mean operation time of EFR was (52.0±9.7)min, no hemorrhage occurred after operation, and the aortic perforation in the patients were found during EFR.The results of pathological and immunohistostaining examination identified the leiomyoma in 75 cases, ectopic pancreas in 43 cases, stromal tumor in 39 cases, lipoma in 26 cases, cyst in 4 cases, neuroendocrine tumor in 3 cases, schwannoma in 3 cases, adenomyoma in 2 cases, hemangioma in 1 case and lymphangioma in 1 case.All patients were followed up for 3 to 18 months, the median follow-up time was 7 months, and there was no recurrence case.ConclusionsESD and EFR technology can safely and completely remove the upper gastrointestinal SMT, and provide a complete and reliable pathological diagnosis.With the advantages of small trauma and quick recovery, it is worthy of further clinical promotion.
Clinical and CT findings of coronavirus disease 2019 in the different age groups

Abstract:
ObjectiveTo analyze the differences of the clinical and CT imaging features of coronavirus disease 2019 (COVID-19) in the different age groups.MethodsThe clinical and CT data of 107 patients with clinically confirmed covid-19 were retrospectively analyzed and divided into three groups (< 30 years old group, 30-60 years old group, and >60 years old group) according to their ages.The CT findings and clinical data were analyzed in three groups.ResultsThe typical clinical symptoms of COVID-19 in 107 patients showed fever (98.1%, 105/107) and cough (77.6%, 83/107).Laboratory examination showed increased C-reactive protein (63.6%, 68/107), and decreased white blood cells and lymphocytes (42.1%, 45/107;43.9%, 47/107).The typical CT findings displayed scattered subpleural ground-glass opacity in two lungs (94.4%, 101/107), most commonly accompanied with vasodilation (89.7%, 96/107) and halo sign (78.5%, 84/107).Dyspnea and clinical type were statistically significant between < 30 years old group, >60 years old group and 30-60 years old group, >60 years old group (P < 0.05).The difference of distribution of lung lobes in three groups was significant (P < 0.05).There were significant differences in paving stone sign and solid components between >60 years old group, < 30 years old group and 30-60 years old group (P < 0.05).ConclusionsThe clinical manifestations, laboratory examination and CT findings of COVID-19 have some characteristics, but different age groups possess their own characteristics.Grasping characteristics of these clinical and CT imaging features is helpful for early definite diagnosis.
Analysis of influencing factors of secondary osteoarthritis after operation of tibial plateau fracture

Abstract:
ObjectiveTo explore the influencing factors of secondary osteoarthritis (OA) after operation of tibial plateau fracture.MethodsSeventy nine patients with tibial plateau fracture treated with operation were selected and divided into secondary OA group (n=17) and non-secondary OA group(n=62).The risk factors of secondary OA to surgical treatment of tibial plateau fracture were analyzed by single factor and logistic regression analysis.ResultsThere were no significant difference in gender, age, fracture classification (Schatzker type), degree of injury violence, smoking and drinking history, operation time, bleeding volume, bone grafting and total hospitalization days (P>0.05)between the two groups, but there was significant difference in perioperative days (P < 0.05).Regression analysis showed that perioperative period of more than 7 days was a risk factor for secondary OA.ConclusionsThe duration of perioperative period can indirectly reflect the energy absorption intensity of fracture, the destruction of soft tissue, and the effect of perioperative symptomatic treatment.It is an important influencing factor of postoperative secondary OA.
Comparison of the analgesic effect between ultrasound-guided popliteal fossa superior and lower lateral approaches of the greater trochanter for sciatic nerve block in the elderly after total knee arthroplasty

Abstract:
ObjectiveTo investigate the effects of ultrasound-guided sciatic nerve block in total knee replacement.MethodsOne hundred and twenty patients with unilateral total knee arthroplasty were divided into the group A and group B according to the random number table method(60 cases each group).The group A were treated with sciatic nerve block through the greater trochanteric lateral approach, and the group B were treated with sciatic nerve block through upper lateral popliteal approach.The sciatic nerve block index, sensory and r motor block onset time, maintenance time and block effects were compared between two groups.ResultsThe operation time, number of punctures and puncture depth in group B were significantly less than those in group A(P < 0.01).The puncture success rate, block success rate and patient satisfaction in group B were significantly higher than those in group A(P < 0.01).There was no statistical significance in the onset and duration of block between two groups(P>0.05), and the block effect scores of sensory and motor in group B were significantly higher than those in group A(P < 0.01).There was no statistical significance in the VAS scores after 8 h of surgery between two groups(P>0.05).The muscle strength of the posterior femoris in the group B after 8h of operation achieved Ⅲ level and above, and which was significantly better than that in group A(P < 0.01).ConclusionsThe ultrasound-guided upper lateral popliteal approach to sciatic nerve block after total knee arthroplasty is short puncture time, shallow penetration depth, high success rates of puncture and block, and good patient satisfaction and muscle strength of the posterior femoral muscle, which is worthy of clinical promotion.
Analysis of the influencing factors of ischemic stroke complicated with cerebrovascular atherosclerotic stenosis

Abstract:
ObjectiveTo analyze the influencing factors of ischemic stroke complicated with cerebrovascular atherosclerotic stenosis.MethodsOne hundred and forty-two patients with acute ischemic cerebral infarction(including 110 cases with cerebrovascular atherosclerotic stenosis and 32 without) were selected.The clinical data of the patients were collected, and the logistic risk factor regression model was used to analyze the influencing factors of ischemic stroke complicated with cerebrovascular atherosclerotic stenosis.ResultsThe results of logistic risk factor regression analysis showed that the age, hypertension, diabetes mellitus, D-D, Cystatin C, Hcy and apoA were the independent influencing factors of atherosclerotic stenosis in patients with ischemic stroke(P < 0.01).ConclusionsThe occurrence of cerebral atherosclerotic stenosis in ischemic stroke patients can be influenced by the interaction of many factors.By predicting the risk of the occurrence of atherosclerotic stenosis, it is helpful to take necessary preventive measures and affect the prognosis of patients.
Effect of acupuncture at Du meridian and Jiaji acupoint on balance function of patients with cerebral infarction

Abstract:
ObjectiveTo observe the effects of acupuncture at Du meridian and Jiaji acupoint on balance function of patients with cerebral infarction.MethodsSixty patients with balance dysfunction after cerebral infarction were randomly divided into the control group and treatment group(30 cases in each group).Two groups was treated with conventional drug and rehabilitation, and the treatment group was additionally treated with acupuncture at Du meridian and Jiaji acupoint for 4 weeks.The Sheikh trunk control ability score, Berg balance function score and improved Barthel index were observed before and after the treatment.ResultsAfter treatment, the Sheikh's trunk control ability score, Berg balance function score and improved Barthel index in two groups were higher than those before treatment, and the improvement degree in treatment group was significantly higher than that in control group(P < 0.01), and the differences of the three indexes between two group0s were statistically significant(P < 0.01).ConclusionsAcupuncture at Du meridian and Jiaji acupoint has good effects on the balance function of patients with cerebral infarction.
Analysis of the efficacy of high dose tigecycline in the treatment of lower respiratory tract infection with ineffective carbapenem treatment

Abstract:
ObjectiveTo investigate the effects of high dose tiagocycline in the treatment of lower respiratory tract infection with ineffective carbapenem treatment.MethodsThe clinical data of 85 lower respiratory tract infection patients with ineffective carbapenem treatment were analyzed retrospectively.According to the etiology of disease before administration, the patients were divided into the empirical treatment group(26 cases) and target treatment group(59 cases), and the target treatment group was subdivided into the CRAB group(36 cases) and CRKP group(23 cases).The clinical characteristics, clinical efficiency and inflammatory markers between the empirical treatment group and target treatment group were compared, and the risk factors of clinical efficacy was analyzed.ResultsThe clinical effective rate of high-dose tigecycline in CRAB group and CRKP group were 63.89% and 73.91%, respectively, which was significantly higher than that in empirical treatment group(30.77%)(P < 0.05 and P < 0.01).There was no statistical significance in clinical efficacy of high-dose tigecycline between CRAB group and CRKP group(P>0.05).After treatment with high-dose tigecyclne, the WBC, CRP and PCT in CRAB group and CRKP group decreased significantly(P < 0.05), while only CRP decreased significantly in empirical treatment group(P < 0.05), the differences of WBC and PCT were not statistically significant between before and after treatment(P>0.05).The results of logistic regression analysis showed that the severe disease had a significant effect on clinical efficiency(P < 0.05).ConclusionsFor lower respiratory tract infection patients with ineffective carbapenem treatment, the combination therapy based on high-dose tigecycline can achieve good anti-infective effect on the patients, and the target treatment is more effective than empirical treatment.
Study on the influencing factors and mechanism of Hirschsprung-associated enterocolitis

ObjectiveTo investigate the influencing factors and mechanism of Hirschsprung-associated enterocolitis(HAEC).MethodsA total of 114 cases of HAEC patients treated with radical megacolon were collected.The influencing factors of HAEC were analyzed using logistic regression analysis method.The expression levels of Lipopolysaccharide(LPS) and tight junction protein(ZO-1) in the dilated segment tissue of children with Hirschsprung's disease(HSCR) were detected using ELISA assay, and the correlations between which and HAEC were analyzed using the Pearson correlation analysis method.ResultsAmong 114 children patients, 69 cases without HAEC and 45 cases with HAEC(37.82%) were identified, and the preoperative HAEC in 23 patients(20.17%), postoperative HAEC in 10 patients(8.77%) and preoperative and postoperative HAEC in 12 patients(10.53%) with HAEC were found.The differences of the gestational week and type of HSCR were not statistically significant(P>0.05), while the exclusive breastfeeding was a protective factor for HAEC and preoperative HAEC(P < 0.01 and P < 0.05).The expression level of LPS in the dilated segment tissue of preoperative HAEC group were higher than that of non-HAEC group(P < 0.01), the expression level of ZO-1 was low, and the LPS expression level was negatively correlated with ZO-1(r=0.361, P < 0.01).The expression level of LPS in dilated segment tissue of the exclusive breastfeeding group was lower than that of the non-proprietary breastfeeding group(P < 0.01).ConclusionsThe exclusive breastfeeding is a protective factor for the pathogenesis of HAEC.Breast milk may reduce the risk of HAEC by decreasing the expression level of LPS to further affect the expression of related tight junction proteins.
Comparison of short-term efficacy of laparoscopy therapy in elderly patients with advanced gastric cancer

Abstract:
ObjectiveTo analyze the clinical efficacy and safety of total laparoscopic radical gastrectomy and laparoscope-assisted radical gastrectomy in elderly patients with advanced gastric cancer, to provide reference for the surgical treatment of elderly patients with gastric cancer.MethodsA total of 54 elderly patients with advanced gastric cancer were selected and divided into total laparoscopic radical gastrectomy group (n=24) and laparoscope-assisted radical gastrectomy group (n=30) according to random number table method.The blood loss, number of lymph node dissection, postoperative pain score, exhaust time, eating time, hospitalization time, complication rate, C-reactive protein (CRP), CD4/CD8 and patient's satisfaction were compared between the two groups.ResultsThere were significant differences in postoperative pain score, exhaust time and hospitalization time between the two groups (P < 0.01).The level of CRP at 3 d and 7 d after operation in total laparoscopic radical gastrectomy group was significantly lower than that in laparoscope-assisted radical gastrectomy group (P < 0.01), and the level of CD4/CD8 at 1 d and 7 d after operation was higher than that in laparoscope-assisted radical gastrectomy group (P < 0.01 and P < 0.05), which at 1 d after operation was significantly higher than that before operation (P < 0.01).ConclusionsThe total laparoscopic radical gastrectomy has obvious advantages in short-term clinical efficacy for elderly patients with advanced gastric cancer.
Feasibility study on ultrasound-guided vena cava filter implantation and its clinical application

Abstract:
ObjectiveTo explore the feasibility, safety and clinical value of vena cava filter (VCF) implantation under the guidance of color ultrasound.MethodsThe data of 203 patients undergoing vena cava filter (VCF) implantation were collected.Among them, twenty-one patients were placed VCF under the guidance of color ultrasound (ultrasound-guided group), and 182 patients were placed VCF under the guidance of DSA (DSA-guided group).The hospitalization time, hospitalization cost, operation time, filter tilt rate, operation success rate, and VCF-related complications during follow-up were compared between the two groups.ResultsThere was no significant difference in the success rate and filter tilt rate between the two groups (P>0.05).The hospitalization time, hospitalization cost and operation time in ultrasound-guided group were lower than those in DSA-guided group (P < 0.05 to P < 0.01).There was no VCF-related complications during the follow-up of 3 to 6 months.ConclusionsBoth ultrasound-guided and DSA-guided VCF implantation have good safety and feasibility.Color Doppler ultrasound guidance is more suitable for patients with contrast medium allergy, renal insufficiency and difficulty in moving, which is worthy of clinical promotion.
Effect of dorsal thoracic artery perforator flap in the repair of dorsal hand soft tissue defect

Abstract:
ObjectiveTo study the repair effects of dorsal thoracic artery perforator flap in dorsal hand soft tissue defect.MethodsEighty-eight patients with soft tissue defects were investigated.The hand function assessment scales of patients between before and after 6 months of treatment were compared.The patient's operation time, intraoperative blood loss, hospital stay, donor site wound healing time, recipient site wound healing time, flap survival rate and postoperative complications were analyzed.ResultsThe scores of opposable fingers, opposable palms, finger adduction and abduction, and total score of hand function of 88 patients after 6 months of treatment were significantly higher than those before treatment(P < 0.05 to P < 0.01).The operation time, intraoperative blood loss, hospital stay, donor site wound healing time and recipient site wound healing time in 88 patients were (95.32±10.59)min, (305.21±26.87)mL, (10.53±1.65)d, (7.26±0.95)d and (8.47±1.16)d, respectively.Among 88 patients, 85 skin flaps survived, and the survival rate of which was 96.59%.Two skin flaps turned purple, and improved after hemoperitoneum drainage and vasodilator treatment.One case of skin flap was partial necrosis, and survived after secondary debridement and retransplantation of the flap.There were 3 cases of blood circulation disorder, one case of hematoma, one case of infection and 2 cases of severe scar hyperplasia after operation.The incidence rate of complication was 7.95%.ConclusionsThe repair of dorsal hand soft tissue defect with dorsal thoracic artery perforator flap can significantly improve the hand function of the patient, the survival rate of flap is high, which is suitable for further study and use in clinic.

Abstract:
ObjectiveTo explore the early effect of intra-articular injection of ropivacaine on postoperative analgesia on the first day after total knee replacement.MethodsEighty patients treated with total knee arthroplasty were randomly divided into the observation group and control group.The observation group and control group were treated with and without intra-articular injection of ropivacaine after the drainage tube was removed on the first day after operation, respectively.The postoperative pain visual simulation score(rest and exercise state), postoperative hospital stay, postoperative first time to the ground, analgesic drug dosage and incision complications were evaluated in two groups.ResultsAt rest, the area under the visual analogue score curve in observation group for 5 consecutive days after surgery was significantly lower than that in control group(424±32 vs 515±39, P < 0.01).The visual analogue scores of pain at various time points in observation group were significantly lower than those in control group on the first and second day after operation(P < 0.05 to P < 0.01).The length of hospital stay in observation group was significantly lower than that in control group(P < 0.01).The first landing time in observation group was significantly earlier than that in control group(P < 0.01).ConclusionsThe intra-articular injection of ropivacaine on the first day after total knee arthroplasty is a good analgesic effect on patients, and conducive to recovery.

Abstract:
ObjectiveTo study the correlation between ambulatory blood pressure parameters and severity of coronary lesion.MethodsThe 24 h ambulatory blood pressure and coronary angiography in 322 patients suspected by coronary heart disease were detected.According to the results of coronary angiography, the patients were divided into the normal coronary artery group(85 cases) and single vessel lesion group(91 cases), two branches lesion group(77 cases) and three branches lesion group(69 cases).The ambulatory blood pressure parameters were compared among 4 groups.ResultsThe 24-h ASBP, 24-h APP, dAPP, nASBP and nAPP in patients with single, double and three-vessel lesions were significantly higher than those in patients with normal coronary artery(P < 0.05 to P < 0.01).The number of lesion branches(double and three branches) in non-dipper, reverse dipper and super-dipper blood pressure patterns patients were higher than those in dipper blood pressure patterns patients(P < 0.01).Under the arytenoid blood pressure pattern, the proportional distribution of normal, single, double and three branches lesion gradually decreased(P < 0.01).Under the inverting arytenoid blood pressure pattern, the proportional distribution of three, double, single and normal branches lesion gradually decreased(P < 0.01).The results of multiple logistic regression analysis showed that the smoking, diabetes, 24 hASBP, dASBP, 24 hAPP, dAPP, and nAPP were the risk factors for coronary heart disease(P < 0.05 to P < 0.01).ConclusionsThe ambulatory blood pressure pattern is significantly correlated with the severity of hypertensive target organ damage.

Abstract:
ObjectiveTo explore the effects of various mobilization schemes, age, chemotherapy cycle, disease state and other factors on the autologous peripheral blood stem cell mobilization and collection.MethodsThe clinical data of 31 patients with acute leukemia(AL), Non-Hodgkin's lymphoma(NHL) and multiple myeloma(MM) were retrospectively analyzed.The patients with Granulocyte Colony-Stimulating Factor(G-CSF) mobilization scheme, large dose VP-16(Etoposide) combined with G-CSF mobilization scheme and Cyclophosphamide(CTX) combined with G-CSF mobilization scheme were divdied into the the G-CSF group, VP-16 group and CTX group, respectively.The number of cells, changes of hematological indexes and adverse reactions among three groups were compared.The effects of age, gender, disease status, chemotherapy cycle, treatment to mobilization time on stem cell collection were analyzed.ResultsA total of 59 times were collected from 31 people, and the (5.7±1.6)×108/kg mononuclear cells and (2.1±0.7)×106/kg CD34+ cells were obtained.The number of median day from mobilizating to collecting in G-CSF group was 4 days, but the CD34+ cell production in G-CSF group was lower than that in other two groups(P < 0.05).There was no statistical significance in the number of CD34+ cells between the VP-16 group and CTX group(P>0.05), and the number of median day from mobilizating to collecting in VP-16 group and CTX group were 12 days.The platelet level in CTX group was lower than that in VP-16 group(P < 0.05).Five cases with ranulocytopenia complicaed with fever(>38 ℃) in VP-17 group were found after chemotherapy, while 3 patients in the CTX group were identified.The number of CD34+ cells collection in male patients was higher than that in female patients, which in patients with ≤55 years old was higher than that in patients with >55 years old, which in patients with ≤8 chemotherapy courses was higher than that in patients with >8 chemotherapy courses, and which in CR patients was better than that in PR patients(P < 0.05).ConclusionsChemotherapy combined with G-CSF can mobilize more CD34+ cells, and the platelet level in the high-dose VP-16 + G-CSF Mobilization scheme is higher, and the safety of collection is more guaranteed.The success rates in male, age ≤55 years old and course of chemotherapy ≤8 courses patients are higher.
Clinical study on anlotinib combined with docetaxel in the third-line treatment of advanced squamous cell lung carcinoma

Abstract:
ObjectiveTo observe the effectiveness and safety of anlotinib combined with docetaxel in the third-line treatment of advanced squamous cell lung carcinoma.MethodsOne hundred patients with advanced squamous cell lung carcinoma were selected and divided into control group (n=44) and observation group (n=56) according to the random number table method.The control group was treated with dexamethasone and docetaxel, and the observation group was treated with anlotinib additionally on the basis of control group.The clinical efficacy, adverse reactions, survival and quality of life were compared between the two groups.ResultsAfter treatment, there was no statistical difference in the objective remission rate, disease control rate, incidence of adverse reactions and mortality between the two groups (P>0.05).The overall survival time of control group and observation group was 10.21 months (95%CI:7.290-13.139) and 12.65 months (95%CI:10.588-14.712), respectively.After treatment, the scores of functional area and general health status of the two groups were higher than those before treatment (P < 0.05), and the scores of symptom area and cancer symptom were lower than those before treatment (P < 0.05);the scores of functional area and general health status in the observation group were significantly higher than those in control group (P < 0.01), and the scores of symptom area and cancer symptom in observation group were significantly lower than those in control group (P < 0.01).ConclusionsThe combination of amlotinib hydrochloride and docetaxel in the third-line treatment of advanced squamous cell lung carcinoma can prolong survival time, improve quality of life, and not significantly increase adverse reactions of patients.
Comparison of the therapeutic effects between IMRT and 3D-CRT in the treatment of postoperative breast cancer

Abstract:
ObjectiveTo compare the effects between intensity modulated radiotherapy(IMRT) and three-dimensional conformal radiotherapy(3D-CRT) in the treatment of postoperative breast cancer.MethodsOne hundred and twenty-two postoperative breast cancer patients were divided into the IMRT group and 3D-CRT group according to the random number table method(61 cases each group).The recurrence and metastasis after 1 year of operation were compared between two groups.The cosmetic effects and occurrence of acute radiation skin injury were compared between two groups.The levels of serum hormones[estradiol(E2) and progesterone(P)] and tumor markers[carcinoembryonic antigen(CEA) and carbohydrate antigen 153(CA153)] were compared between two groups before and after 5 weeks of radiotherapy.ResultsAfter 1 year of operation, no recurrence or metastasis case occurred in two groups.The excellent rate of cosmetic effects in IMRT group was significantly higher than that in 3D-CRT group, and the incidence rate of acute radiation skin injury in IMRT group was lower than that in 3D-CRT group(P < 0.05).There was no statistical significance in the serum levels of E2 and P in two groups between before and after radiotherapy(P>0.05).After 5 weeks of radiotherapy, the serum levels of CA153 and CEA in two groups were lower than those before radiotherapy(P < 0.05), but the difference of which was not statistically signficant between two groups(P>0.05).ConclusionsBoth IMRT and 3D-CRT have good effects on postoperative radiotherapy of breast cancer, but IMRT has more advantages in reducing skin damage and cosmetic effect, and positive significance in improving patients' quality of life.

ObjectiveTo evaluate the effects of sublingual immunotherapy(SLIT) on clinical efficacy and quality of life of patients with allergic rhinitis(AR) complicated with atopic cough(AC).MethodsThe clinical data of 96 AR patients complicated with AC, aged 6-55 years, were retrospectively analyzed, and divided into the SLIT group(52 cases) and control group(44 cases).The control group was treated with simple anti-allergic drug, and the SLIT group were additionally treated with SLIT on the basis of control group.The TNSS, DCSS, NCSS, TMS, VAS and RQLQ scores were compared between two groups before treatment and after 1 year and 2 years of treatment.ResultsCompared with before treatment, the TNSS, DCSS, NCSS, TMS, VAS and RQLQ scores in two groups significantly decreased after 1 year and 2 years of treatment(P < 0.01), and the differences of those in two groups between before and after 1 year and 2 years of treatment were statistically significant(P < 0.01).Compared with after 1 year of treatment, the VAS and RQLQ scores in SLIT group were further improved after 2 years of treatment(P < 0.01 and P < 0.05).The differences of the TNSS, DCSS, NCSS, TMS, VAS and RQLQ scores between two groups before treatment were not statistically significant(P>0.05).After 1 year and 2 years of treatment, the TSS, DCSS, NCSS, TMS, VAS and RQLQ scores in SLIT group were lower than those in control group, and the difference of which were statistically significant(P < 0.05 to P < 0.01).ConclusionsFor AR patients complicated with AC, the treatment effects of SLIT combined with drugs is more significant than that of drug therapy alone, the improvement of quality of life is more significant.
Analysis on the status and influencing factors of repeated wheezing after bronchiolitis in infant

Abstract:
ObjectiveTo investigate the occurrence of repeated wheezing after bronchiolitis in infants, and explore its related factors.MethodsThe asthmatic attack in 102 infants with bronchiolitis were analyzed during the following-up period.The infant patients were grouped according to the wheezing.The general information, family history of asthma, pet contact history, history of allergic disease and bronchiolitis were compared between two groups.ResultsAmong 102 children, 3 cases lost.During 3, 6, 9 and 12 months of following-up, at least one time wheeze occurred in 16 cases(16.16%), 34 cases(34.34%), 39 cases(39.39%) and 43 cases(43.43%), respectively, During 12 months of following-up one wheeze occurred in 8 cases, two wheezes occurred in 11 cases, and more than three wheezes occurred in 24 cases.The results of single factor analysis showed that the differences of the breast feeding less than 3 months, passive smoking history, eczema history, repeated lower respiratory tract infection history, family allergic disease history, severe bronchiolitis, respiratory syncytial virus infection and food allergy between repeated and non-repeated wheezing groups were statistically significant(P < 0.05 to P < 0.01).The results of logistic analysis showed that the severe bronchiolitis, eczema, family allergic disease and recurrent lower respiratory tract infection were the risk factors of repeated wheezing(P < 0.05), while breast-feeding for more than 3 months was the protective factor of repeated wheezing after bronchiolitis(P < 0.01).ConclusionsThe severe bronchiolitis, family history of allergic diseases, eczema and repeated lower respiratory tract infection are the high risk factors of repeated wheezing after bronchiolitis.Clinical prevention and treatment should be done to reduce the incidence of wheezing.
The relationship between the interleukin-1 gene polymorphism and Helicobacter pylori infection, and atherosclerosis in healthy middle-aged population

Abstract:
ObjectiveTo investigate the effects of the gene polymorphism of IL-1-511 or IL-1RN VNTR on atherosclerosis in Helicobacter pylori (HP) infection aggravation healthy middle-aged people.MethodsThe genotypes loci polymorphism of IL-1 511 or IL-1RN VNTR in 227 healthy middle-aged people were determined using polymerase chain reaction(PCR)-restriction digestion analysis.The HP IgG, and CagA and VacA antibody were detected using ELISA, and the degree of atherosclerosis was assessed using the ankle-brachial index(ABI).ResultsThe main polymorphism of IL-1β-511 were CT(64.8%) and TT(22.3%), respectively.The allele frequency of four-repeat(IL-1RN*1) was 68.7%, and two repeat(IL-1RN*2) alleles was 29.2% in IL-1RN VNTR.The difference of ABI in subjects with different Il-1 gene polymorphisms was not statistically significant(P>0.05).The ABI in positive HP subjects was significantly lower than that in negative HP subjects(P < 0.01).After adjusting the age, sex, hypertension, diabetes, smoking status, BMI, triglyceride and interleukin-1 Gene polymorphism, the difference of the ABI between positive and negative HP subjects was not statistically significant(P>0.05), and the positive HP CagA antibody was a low risk factor of ABI(P < 0.01).ConclusionsThe HP infection, especially CagA antibody-positive HP infection, may be a risk factor of atherosclerosis in the middle-aged Han population in southern China, and there is no significant correlation between it and polymorphisms of two IL-1 gene sites.
Effect of massage combined with horse chestnut cream imported by ultrasound in the treatment of chronic nonspecific low back pain

Abstract:
ObjectiveTo observe the efficacy of massage combined with horse chestnut cream imported by ultrasound in the treatment of chronic nonspecific low back pain.MethodsEighty patients with chronic nonspecific low back pain were divided into the control group and observation group using random number table method(40 cases in each group).The control group was treated with massage, and the observation group was treated with massage combined with horse chestnut cream imported by ultrasound, one time every day, for 4 weeks.The total effective rate, visual analogue scale(VAS) score, lumbago disturbance index score(ODI) and quality of life rating scale(SF-36) score in two groups were evaluated before and after treatment.ResultsAfter 4 weeks of treatment, the total effective rates of the control group and observation group were 67.5% and 90.0%, respectively, and the difference of which was statistically significant(P < 0.01).The differences of the VAS score, ODI score and SF-36 score between two groups were statistically significant after 4 weeks of treatment(P < 0.01).ConclusionsThe massage combined with horse chestnut cream imported by ultrasound can reduce the pain degree, and significantly improve the lumbar function and overall quality of life of patients with chronic nonspecific low back pain.
Application of modified uncut tubular stomach in radical resection of esophageal carcinoma

Abstract:
ObjectiveTo investigate the feasibility, safety and short-term clinical effects of the application of modified uncut tubular stomach and traditional cut tubular stomach in radical surgery of esophageal carcinoma.MethodsThe clinical data of 47 patients treated with radical surgery of esophageal carcinoma were retrospectively analyzed.All patients with esophageal cancer were confirmed by gastroscopy before surgery.The patients were divided into the modified uncut tubular stomach group(24 cases) and traditional cut tubular stomach group(23 cases).The operation time, intraoperative bleeding, length of tubular stomach, material usage, postoperative observation index and short-term follow-up after discharge in two groups were analyzed.ResultsAll operations were successfully completed, no intraoperative bleeding and other accidents occurred.The operation time, length of tubular stomach and material usage in modified uncut tubular stomach group were less than those in traditional cut tubular stomach group(P < 0.05 to P < 0.01).The differences of the intraoperative hemorrhage, postoperative 2 d thoracic drainage amount, length of hospital stay, incidence rates of complications during perioperative period and short-term follow-up between two groups were not statistically significant(P>0.05).There was no serious complication or death.ConclusionsThe modified uncut tubular stomach can shorten the operation time and reduce the material usage, and does not increase the anastomotic fistula and stenosis, pulmonary infection, postoperative acid reflux, and chest and stomach dilation.

ObjectiveTo explore the clinical effect of rapid rehabilitation therapy on severe acute pancreatitis under intensive care.MethodsThe clinical data of 96 patients with severe acute pancreatitis were analyzed retrospectively.Forty-six cases from January 2010 to March 2016 were set as the control group, which was treated with traditional regimen; fifty cases from April 2016 to December 2019 were set as the observation group, which was treated with rapid rehabilitation therapy such as sufficient analgesia, early (24-72 h) oral or nasogastric tube feeding, controlled fluid resuscitation, early removal of nasogastric tube, reduction of use or early withdrawal of deep vein catheter and catheter, and early activity on the basis of control group.Pain score, body mass loss, incidence of systemic and local complication, ICU hospitalization time and hospitalization expense were compared between the two groups.ResultsCompared with the control group, the pain, overall complications, infusion time, treatment cost and ICU hospitalization time in the observation group were significantly improved (P < 0.05 to P < 0.01), and the mortality was not statistically significant (P>0.05).ConclusionsApplication of rapid rehabilitation therapy under the intensive care can reduce complication, save hospitalization expense, shorten ICU hospitalization time, improve the quality of life, and accelerate the recovery of patients with severe acute pancreatitis, which can be expected to become an alternative method for the treatment of patients with severe acute pancreatitis.
Effectiveness and safety of oxycodone combined with intraspinal anesthesia for femoral neck fracture surgery in elderly patients

Abstract:
ObjectiveTo analyze the effectiveness and safety of oxycodone combined with intraspinal anesthesia for femoral neck fracture surgery in elderly patients. MethodsForty patients with femoral neck fracture were divided into the general anesthesia group(group C, n=21), and oxycodone combined with subarachnoid anesthesia group(group O, n=19) according to the anesthesia program.The hemodynamic parameters[heart rate(HR), mean arterial pressure(MAP), blood oxygen saturation(SpO2)], stress indexes[cortisol(Cor), norepinephrine(NE) and angiotensin Ⅱ(Ang Ⅱ)] before anesthesia(T0), after anesthesia(T1) and during suture(T3) were compared between two groups.The visual analogue score(VAS) before and after operation were compared between two groups, and the occurrence of complications were recorded. ResultsAt T0, there was no statistical significance in the levels of HR, MAP and SpO2 between two groups(P>0.05).At T1 and T2, the levels of HR and MAP in two groups were significantly lower than those at T0(P < 0.01), while the levels of HR and MAP at T1 and MAP level at T1 in group O were higher than those in group C(P < 0.05 to P < 0.01).At T0, there was no statistical significance in the levels of Cor, NE and Ang Ⅱ between two groups(P>0.05).At T1 and T2, the serum levels of Cor, NE and Ang Ⅱ in two groups significantly increased compared with at T0, while the levels of Cor, NE and Ang Ⅱ in group O at T1 and T2 were significantly lower than those in group C(P < 0.01).There was no statistical significance in VAS score between two groups before operation(P>0.05).After 6 h of surgery, the VAS scores in two groups were significantly lower than those before operation(P < 0.01), and the VAS score in group O was significantly lower than that in group C(P < 0.01).The incidence rates of nausea and vomiting in group O were lower than those in group C(P < 0.05), and there was no statistical significance in the incidence rates of pulmonary infection, incision infection and lower limb venous thrombosis between two groups(P>0.05). ConclusionsCompared with conventional intravenous general anesthesia, the oxycodone combined with subarachnoid anesthesia is more advantageous in reducing circulatory fluctuation and general stress injury, enhancing postoperative analgesia, and reducing postoperative complications in elderly patients with femoral neck fracture surgery.

ObjectiveTo investigate the differences of carotid ultrasound indexes between advanced age and middle-and-low age patients with acute cerebral infarction.MethodsA total of 134 patients with acute cerebral infarction(≥65 years old) were divided into the advanced age group(≥80 years old) and middle-and-low age group(65-79 years old).The differences of the ultrasound indexes[including intima-media thickening(IMT), number of plaques, plaque position, plaque property and degree of vascular stenosis] were compared between two groups.ResultsThe differences of the number of plaques and IMT between two groups were not statistically significant(P>0.05), the detection rate of plaques at the bifurcation of right carotid artery in advanced age group was significantly higher than that in middle-and-low age group(P < 0.05), and the differences of the plaque property and degree of vascular stenosis between two groups were statistically significant(P < 0.05).ConclusionsAmong the elderly patients with acute cerebral infarction, the plaque at right carotid artery bifurcation is more common, the unstable plaque is less common, and the degree of vascular stenosis is relatively mild.

Abstract:
ObjectiveTo explore the effectiveness of CT angiography(CTA) in the risk assessment of intracranial aneurysm(IA), and systematically analyze its morphological features.MethodsThe clinical data of 103 patients with IA from January 2017 to may 2019 were retrospectively analyzed.The CTA image morphology between ruptured and unruptured patients was compared, and its correlation with tumor rupture was analyzed.ResultsAmong 103 patients, 78 cases(84 tumors) ruptured, and 25 cases(28 tumors) did not rupture.The rupture of traffic artery was mainly after IA, followed by anterior communicating artery, middle cerebral artery, anterior cerebral artery and posterior circulation, and the difference between the anterior and posterior cerebral arteries was statistically significant(P < 0.05).The differences of the tumor size, tumor location, tumor height, tumor neck width ratio(AR), tumor depth to tumor carrier artery diameter ratio(SR), area ratio(ratio of aneurysm area to tumor neck carrier artery area ratio) and number of sac between the ruptured group and non ruptured group were statistically significant(P < 0.05 to P < 01).The results of logistic regression analysis showed that the tumor size, AR, SR and area ratio were the independent risk factors of aneurysm rupture(P < 0.05).ConclusionsCTA can clearly and accurately reflect the morphological features of intracranial aneurysms.The morphological findings such as tumor size, AR, SR and area ratio can be used as the independent evaluation indicators of IA rupture risk.
Clinical value of serum cystatin C level in the screening, diagnosis and postoperative evaluation of patients with breast cancer

Abstract:
ObjectiveTo investigate the expression levels of cystatin C(Cys C) in serum and cancerous tissue of patients with breast cancer, and its clinical value in the screening, diagnosis and disease evaluation of patients with breast cancer.MethodsFifty-three patients with primary breast cancer, 48 patients with benign breast tumor and 40 healthy females were divided into the breast cancer group, benign group and control group, respectively.For the three groups, the serum Cys C levels were detected using latex enhances transmission immunity turbidimetry, and the diagnostic efficacy(including sensitivity, specificity, and accuracy rating) of serum Cys C in breast cancer were evaluated using ROC curve.The expression levels of Cys C protein in breast benign tumor tissues, breast cancerous tissues and para-cancerous tissues were detected using Western blotting.ResultsThe serum level of Cys C in breast cancer group was significantly higher than that in control group and benign group(P < 0.01), and the level of Cys C in benign group was higher than that in control group(P < 0.01).The serum level of Cys C in breast cancer patients after operation was lower than that before operation(P < 0.01).When the Cys C≤1.015 mg/L was negative, and Cys C>1.015 mg/L was positive, the sensitivity, specificity and accuracy of Cys C in diagnosing breast cancer were 67.92%, 80.00% and 73.12%, respectively.The serum levels of Cys C in breast cancer patients with age >50 years old, tumor size ≥2 cm, infiltrating ductal carcinoma, lymph node metastasis and clinical classification for Ⅲ type were higher than those in breast cancer patients with age ≤50 years old, tumor size < 2 cm, infiltrating lobular carcinoma, no lymph node metastasis and clinical classification for Ⅰ-Ⅱ type, respectively, and the differences of which were statistically significant(P < 0.01).The results of Western blotting showed that the expression level of Cys C protein in breast cancer tissues was lower than that in para-cancerous tissues, and the expression level of Cys C protein in para-cancerous tissues was higher than that in benign breast tissues(P < 0.01).ConclusionsDetecting the serum level of Cys C has good sensitivity, specificity and accuracy in clinical screening and diagnosis of breast cancer.The heterogeneity of Cys C expression in cancer tissues and serum may be closely related to its role in breast cancer progression.

Abstract:
ObjectiveTo evaluate the combination diagnostic efficacy of five serum tumor markers in lung cancer based on propensity score-matched method.MethodsSixty-five lung cancer patients and 129 pneumonia patients from respiratory medicine clinic were divided into the lung cancer group and control group, respectively.The serum levels of CEA, NSE, CYFRA21-1, Pro-GRP and SCC were detected using electrochemical luminescence method.The confounding factors were removed by propensity score-matched method.The receiver operating characteristic curves were drawn to compare the diagnostic efficacy of five serum tumor markers, either alone or in combination.ResultsFifty-seven cases were matched with lung cancer group and control group by 1:1 propensity score matched method.The serum levels of CEA, CYFRA21-1 and NSE in lung cancer group were significantly higher than those in control group(P < 0.01), and the differences of the serum levels of Pro-GRP and SCC between two groups were not statistically significant(P>0.05).According to the pathological types, the matched lung cancer group was divided into the squamous cell carcinoma group(20 cases) and adenocarcinoma group(37 cases), and the results showed that the levels of CYFRA21-1, CEA and NSE in adenocarcinoma group and NSE level in squamous cell carcinoma group were significantly higher than those in control group(P < 0.01).The serum CEA, CYFRA21-1 and NSE in lung cancer group showed better diagnostic efficacy(P < 0.05).The diagnostic efficiency of multi-indicator combination was higher than that of single indicator, and there was no statistical significance in diagnostic efficacy among different combinged strategies(P>0.05).The Youden index of multi-indicator combination in the diagnosis of adenocarcinoma was higher than that of squamous cell carcinoma.ConclusionsThe combination of multiple indicators can improve the diagnostic efficiency of lung cancer, especially for adenocarcinoma.

ObjectiveTo explore the correlation between self-management behavior and quality of life in elderly patients with hypertension, so as to provide reference for improving the quality of life of elderly patients.MethodsA total of 150 elderly patients with hypertension were selected by convenient sampling.The general information questionnaire, hypertension patients self-management behavior rating scale and generic quality of life inventory-74 were used to investigate.ResultsThe total scores of self-management of elderly hypertensive patients with different ages, educational levels and economic status were statistically significant (P < 0.05 to P < 0.01).The total scores of quality of life of elderly hypertensive patients with different ages, educational levels and economic status were statistically significant (P < 0.05).The total scores of self-management and the scores of each dimension were positively correlated with the total scores of quality of life (P < 0.01).Multiple linear regression analysis showed that emotion management, medication management and exercise management were the influencing factors of quality of life in elderly patients with hypertension (P < 0.05 to P < 0.01).ConclusionsImproving self-management of elderly patients with hypertension, especially emotion management, medication management and exercise management, can help to improve the quality of life of patients.

ObjectiveTo explore the construction method and practice effects of training system for specialized anesthesia nurses.MethodsFrom January 2018 to December 2018, eighty nurses from department of anesthesiology, core rotating nurses and department of anesthesiology in other hospitals were selected, and the training system of specialized anesthesiology nurses was established and trained.Before and after training, the core competence, mastery of anesthesia risk related knowledge and competence of nursing staff were assessed.ResultsAfter the training of specialized anesthesia nurses, the scores of each item in the assessment of core competence, anesthesia risk and competency were significantly higher than those before the training(P < 0.01).ConclusionsSystematic training for nurses specialized in anesthesia can significantly improve their core competence, risk awareness and competence, which is of great significance in improving the medical quality of anesthesia specialty.

ObjectiveTo explore the effects of ear point pressing on relieving pain in the first stage of labor in primipara.MethodsForty-nine primiparas with vaginal delivery were randomly divided into the control group, E1 group, E2 group and E3 group using random number table method.The control group was nursed using routine care.The E1, E2 and E3 groups were nursed with ear point pressing intervention during latent period, active period and from latent period to active period, respectively.The pain and fear/anxiety among four groups before and after intervention were compared.ResultsThe pain and fear/anxiety in E1 and E3 groups after latency intervention were significantly lower than those before intervention and control group(P < 0.05 to P < 0.01).There was no statistical significance in the analgesic effects between E2 group and E3 group after active intervention, and that before intervention and control group(P>0.05).ConclusionThe analgesic effects of ear point pressing during the latent period of vaginal delivery is obvious, and can reduce the maternal fear/anxiety, which is worthy of clinical promotion.

ObjectiveTo explore the risk factors of urinary irritation in patients with indwelling double J-stent after treatment with flexible ureteroscope combined with holmium laser lithotripsy.MethodsA total of 275 patients with ureteral calculi were indwelled with double J-stent after treatment with ureteral flexible ureteroscope combined with holmium laser lithotripsy.Among them, 210 patients with urinary irritation and 65 cases without urinary irritation were divided into the observation group and control group, respectively.The clinical data of two groups were collected, and the influencing factors of urinary irritation in patients with inwelling double J-stent were analyzed.ResultsThe male, inwelling double J-stent time > 2 weeks, F6 type, abnormal ureter and diabetes were the independent risk factors of urinary irritation(P < 0.01).The preoperative controlling urinary tract infection and systematic nursing were protective factors of postoperative urinary irritation(P < 0.01).ConclusionThe influencing factors of urinary irritation at postoperative indwelling double J-stent after ureterolithiasis are more.The appropriate type and length of the double J-stent should be selected according to the actual length of ureter, the extubation is as early as possible, the primary disease and occurrence of urinary tract infection before surgery should be controlled.
Influence of empowerment education on self-efficacy and quality of life in patients with hypertension

Abstract:
ObjectiveTo explore the relationship between empowerment education model and self-efficacy of patients with hypertension, and to explore their impact on the quality of life of patients.MethodsHypertension patients from May 2018 to September 2019 were randomly selected and divided into control group and observation group according to the admission time (30 cases in each group).The control group received conventional oral education, while the observation group received empowerment education on the basis of traditional methods.The blood pressure control effect, life compliance, self-efficacy, SF-36 score and so on were investigated in the two groups.ResultsAfter the implementation of empowerment education, the excellent rate of blood pressure control, nursing satisfaction, self-efficacy score and SF-36 total score in observation group were significantly higher than those in control group(P < 0.01), and all the indicators after intervention were better than those before intervention(P < 0.01).ConclusionThe application of empowerment education model in hypertension patients after discharge can significantly regulate blood pressure indicators, increase treatment compliance and improve self-efficacy of patients, which is worthy of clinical promotion.
Investigation on the health empowerment of elderly patients with diabetes mellitus and its influencing factors

ObjectiveTo investigate the health empowerment of elderly patients with diabetes mellitus, and analyze the influencing factors.MethodsA total of 225 elderly patients with diabetes mellitus were investigated using the health empowerment scale and general self-efficacy Scale, and the data were statistically analyzed using multiple linear regression analysis.ResultsThe health empowerment score of elderly patients with diabetes was(94.08±23.03).The dimension of responsibility belief was the highest[(4.09±1.07)], and the participation in the treatment dimension was the lowest[(2.94±0.95)].The results of multiple regression analysis showed that the health empowerment score in the women, young age, high educational level, long course of disease and high self-efficacy score patients were high(P < 0.05 to P < 0.01).ConclusionMedical staff should reasonably assess the health empowerment capacity of elderly patients with diabetes, select the best teaching methods and content for the health status of patients to improve their quality of life.
Analysis of the status quo and influencing factors of perioperative nursing demand of patients with hemorrhoids

ObjectiveTo investigate the status quo of perioperative nursing demand of hemorrhoids patients, and analyze the possible influencing factors. MethodsThe level of nursing demand of patients with hemorrhoids were investigated using self-made questionnaire.The differences among different gender, age, education level and occupation were compared, and the main influencing factors were determined using multiple linear stepwise regression method.ResultsThe valid investigation in 243 hemorrhoids patients were completed.The nursing demand rate was 99.13%, and the total score was(22.16±2.76) points.The nursing demand of postoperative complication prevention was the highest[(1.92±0.27) points], the demand of attitude towards nurses was next highest[(1.92±0.28) points], and the demand of continuous nursing was the lowest[(1.29±0.50) points].The gender, age, education level, occupation, long-term residence and course of disease had significant influence on the nursing demand score of hemorrhoids patients(P < 0.05 to P < 0.01).The results of multiple stepwise regression showed that the education level was the main factor affecting the degree of nursing demand, the lower the education level was, the higher the degree of nursing demand was(P < 0.01).ConclusionThe general nursing demand of patients with hemorrhoids during perioperative period is relatively high, which is mainly related to their personal education level.The concept of humanized nursing service should be followed.

ObjectiveTo explore the effects of social support combined with perioperative nursing path on postoperative recovery of patients with hysteromyoma.MethodsThe method of discontinuous analysis was used in this study.Forty patients treated with hysteromyoma surgery from January 2018 to December 2018 and 40 patients treated with hysteromyoma surgery from January 2019 to October 2019 were were divided into the control group and observation group, respectively.The control group and observation group were nursed with routine care and social support combined with perioperative nursing path, respectively.The postoperative recovery index, psychological status and social support level before and after nursing were compared between two groups.ResultsThe postoperative time of getting out of bed, exhaust time and hospitalization time in observation group were significantly shorter than those in control group(P < 0.01), and the hospitalization cost in observation group was significantly lower than that in the control group(P < 0.01).After nursing, the scores of self-rating anxiety scale(SAS) [(38.76±5.41)] and self-rating depression scale(SDS)[(36.48±5.22)] in observation group were significantly lower than the scores of SAS[(49.87±6.26)] and SDS[(46.75±6.38)] in control group, respectively(P < 0.01).The social support rating scale score[(52.49±5.87)] was significantly higher than that in control group[(41.75±6.12)](P < 0.01).ConclusionThe social support combined with perioperative nursing path can promote the recovery of uterine fibroids patients as soon as possible after surgery, reduce anxiety and depression of patients and improve the level of social support.It has high clinical value, and is worthy of promotion.

ObjectiveTo improve the quality of Yangyin Qingyan Granule.MethodsIn Yangyin Qingyan Granule, the Honeysucklein, figwort, licorice, oldenlandia and balloon flower root were iidentided using the thin layer chromatography, and the chlorogenic acid content was measured using HPLC.ResultsThe results of chromatographic identification showed that chromatographic position of the test sample was consistent with that of the control sample, and the strong specificity and no negative interference were found.The results of high performance liquid chromatography analysis showed that the chlorogenic acid was well linear in the range of 0.236 to 2.360μg, the average recovery rate was 97.79%, and the RSD% was 1.17%.ConclusionThe method is simple, reliable, good specificity and high reproducibility, which can fully reflect the inner quality of Yangyin Qingyan Granule, and be used to improve the quality control of the preparation.

ObjectiveTo establish the quality evaluation method of Wuwei GuJu Badu powder.MethodsThe microscopical identification was used to observe the microscopical characteristics of Dioscoreae Rhizoma and unprocessed Arisaematis Rhizoma.Thin layer chromatography (TLC) was applied to qualitatively identify Borneolum Syntheticum and Dioscoreae Rhizoma.Residual titration was employed to determine the content of Alumen.ResultsThe microscopical characteristics of needle-like calcium oxalate crystals in Dioscoreae Rhizoma and unprocessed Arisaematis Rhizoma were obvious.The TLC spots of Borneolum Syntheticum and Dioscoreae Rhizoma were clear and separated well, and the negative control had no interference.The method evaluation of residual titration for Alumen content met the requirements of the standards, with average recovery rate of 98.90% and relative standard deviation of 1.29%(n=6).ConclusionThe established method of microscopical identification, TLC and residual titration is simple, accurate and reproducible, which can be used for the quality control of Wuwei GuJu Badu powder.
