Effect of lncRNA EGOT on LPS-induced inflammation and apoptosis of alveolar epithelial cells by targeting miR-320a

Abstract:
ObjectiveTo explore the effect of long-chain non-coding RNA eosinophilic transcription factor (EGOT) on lipopolysaccharide (LPS) -induced apoptosis and inflammation of alveolar epithelial A549 cells and its possible mechanism.MethodsA549 cells were divided into control (Con) group, LPS group, LPS+pcDNA group, LPS+pcDNA-EGOT group, LPS+anti-miR-NC group, LPS+anti-miR-320a group, LPS+pcDNA-EGOT+miR-NC group, and LPS+pcDNA-EGOT+miR-320a group.Real-time quantitative PCR (RT-qPCR) was used to detect the expression levels of EGOT and miR-320a, flow cytometry was applied to detect apoptosis, and the kit was used to detect the levels of interleukin (IL) -6 and IL-1β.Dual luciferase reporter gene assay and RT-qPCR was employed to confirm the targeting relationship between EGOT and miR-320a.ResultsCompared with the Con group, the apoptosis rate, IL-6 and IL-1β levels of A549 cells in the LPS group were increased(P < 0.01), the EGOT expression level was reduced(P < 0.05), and the miR-320a expression level was significantly increased (P < 0.01).Compared with the LPS+pcDNA group, the apoptosis rate, Bax protein expression, IL-6 and IL-1β levels of A549 cells in the LPS+pcDNA-EGOT group were reduced(P < 0.05), and the Bcl-2 protein expression were increased(P < 0.05).Compared with the LPS+anti-miR-NC group, the apoptosis rate, Bax protein expression, IL-6 and IL-1β levels of A549 cells in the LPS+anti-miR-320a group were significantly reduced(P < 0.01), and the Bcl-2 protein expression were significantly increased(P < 0.01).Compared with the LPS+pcDNA-EGOT+miR-NC group, the apoptosis rate, Bax protein expression, IL-6 and IL-1β levels of A549 cells in the LPS+pcDNA-EGOT+miR-320a group were significantly increased (P < 0.01), and the Bcl-2 protein expression were significantly reduced (P < 0.01).The results of dual luciferase reporter gene assay showed that EGOT targeted and negatively regulated the miR-320a expression.ConclusionsLncRNA EGOT can alleviate LPS-induced inflammation and apoptosis of alveolar epithelial cells by targeting miR-320a.
Optimization of ultrasonic sterilization parameters for Pseudomonas aeruginosa by neural network and its application in chronic osteomyelitis

Abstract:
ObjectiveTo explore the effect of ultrasound in adjuvant treatment of chronic osteomyelitis from the aspect of antibacterial effect.MethodsThe neural network model was used to optimize the ultrasonic sterilization parameters in vitro, and the optimized parameters were applied to treat rabbit femoral chronic osteomyelitis model.The number of microbial infection and the Norden score were used to evaluate the effect of ultrasound treatment on chronic osteomyelitis.ResultsThrough fitting the neural network model, it was found that the fitting effect was the best in the case of hidden node 4, and the best parameters were as follows: 40 W/cm3 as the ultrasound intensity, 60 kHz as the ultrasound frequency, and 15 min as the ultrasound time, which resulted in the best sterilization effect.These parameters were applied to treat rabbit femoral chronic osteomyelitis model, and it was found that the antibiotic azithromycin had the best synergistic effect, which had good effects on microbial infection and Norden score.ConclusionsUltrasound can assist the treatment of chronic osteomyelitis through bacteriostasis, and its synergistic effect with antibiotics has the best effect.
Study on the expression and mechanism of miRNA-155 in epileptic patients

Abstract:
ObjectiveTo detect the expression levels of miR-155 in brain tissue samples of epileptic patient, and analyze the association between miR-155 and pathogenesis of epilepsy.MethodsThe expression levels of miR-155 and GABRA1 in 55 brain tissue samples of epilepsy patients and 55 brain tissue samples of cerebral hemorrhage patients were detected using qRT-PCR, and the correlation between the expression of miR-155 and GABRA1 was analyzed.The rat epilepsy model was established, the mRNA expression of miR-155 and GABRA1 was determined using qRT-PCR, and the correlation between the expression of miR-155 and GABRA1 was analyzed.Targetscan was used to analyze whether GABRA1 was a target gene of miR-155, 3'-UTR of GABRA1, the mutant 3'-UTR was inserted into psicheck2S vector, and then transfected into HepG2 cells.Luciferase assay was used to determine whether GABRA1 combined with miR-155 in HepG2 cells treated with miR-155 mimic.ResultsThe expression levels of miR-155 and GABRA1 in brain tissue samples of epilepsy patients significantly increased(P < 0.01), and the expression levels of miR-155 were positively correlated with GABRA1(P < 0.01).The expression levels of miR-155 and GABRA1 in the brain tissue of rat epilepsy model significantly increased(P < 0.01), and the expression levels of miR-155 were positively correlated with GABRA1(P < 0.01).The luciferase activity of HepG2 cells transfected with 3'-UTR of GABRA1 was significantly higher than that of HepG2 cells transfected with mutation 3'-UTR(P < 0.01).ConclusionsThe miR-155 level significantly increases in brain samples of epilepsy patients, which may be involved in the pathological and physiological process of epilepsy by regulating the expression of GABRA1.
Study on the apoptosis induced by dihydromyricetin in human ovarian cancer HO-8910 cells and its mechanism

Abstract:
ObjectiveTo investigate the effects of dihydromyricetin on the growth and apoptosis of human ovarian cancer HO-8910 cells, and its mechanism.MethodsThe HO-8910 cells were cultured in vitro, and treated with dihydromyricetin at different concentrations(10, 20 and 40 μmol/L).The proliferation of cells was detected using MTT and CCK-8 method, the apoptosis of cells was detected using the Annexin V-FITC/PI flow cytometry and Hoechst 33258 fluorescence staining, and the expression levels of Caspase-3, Bcl-2, Bax, ERK and p-ERK protein were detected using Western blotting.ResultsThe results of MTT and CCK-8 was showed that dihydromyricetin could inhibit the growth of HO-8910 cells in a concentration-dependent manner.The results of flow cytometry showed that dihydromyricetin could promote the HO-8910 cell apoptosis in a concentration-dependent manner.The typical morphological changes of apoptosis in HO-8910 cells treated with dihydromyrmectin were found using Hoechst 33258 fluorescence staining.The results of Western blotting showed that dihydromyricetin could up-regulate the levels of Caspase-3 and Bax, and down-regulate the levels of Bcl-2, ERK and p-ERK in HO-8910 cells(P < 0.05).ConclusionsDihydromyricetin can inhibit the activity of human ovarian cancer HO-8910 cells and induce their apoptosis, and the mechanism of which is related to the regulation of ERK/MAPK signaling pathway.
Effect of siRNA silencing MUC16 on the proliferation and migration of gallbladder carcinoma cells

Abstract:
ObjectiveTo explore the effects of siRNA silencing mucin 16(MUC16) on the proliferation and migration of gallbladder carcinoma GBC-SD cells, and its molecular mechanisms.MethodsThe GBC-SD+MUC16-siRNA group was constructed using the lentivirus infection and siRNA interference technique to silence MUC16 gene in GBC-SD cells, and the negative control group(GBC-SD+NC group) and blank control group(GBC-SD group) were set up simultaneously.The RT-PCR was used to detect the expression of MUC16 in GBC-SD cells, the MTT and scratch assays were used to detect the proliferation and migration ability of GBC-SD cells, and the Western blotting was used to detect the protein expressions of CD44 and PCNA in GBC-SD cells.ResultsThe cell proliferation rate and cell migration number in GBC-SD+MUC16-SiRNA group were lower than those in GBC-SD group(P < 0.05).The expression levels of MUC16 and CD44 protein in GBC-SD+MUC16-SIRNA group were lower than those in GBC-SD group and GBC-SD+NC group(P < 0.05 to P < 0.01), and the expression level of PCNA protein in GBC-SD+MUC16-SIRNA group was also lower than that in GBC-SD+NC group(P < 0.05).ConclusionsSiRNA silencing MUC16 can inhibit the proliferation and migration of gallbladder cancer cells, which may provide the new experimental evidence for the treatment of gallbladder cancer.
Correlation between Cdc42 and YAP in the diagnosis of nonvalvular atrial fibrillation

Abstract:
ObjectiveTo investigate the serum expression levels of cell division cyclin 42(Cdc42) and transcription-assisted activator Yes-associated protein(YAP) in patients with non-valvular atrial fibrillation, and its clinical significance.MethodsEighty-one patients with non-valvular atrial fibrillation(including 38 patients with paroxysmal atrial fibrillation and 43 patients with persistent atrial fibrillation) were selected, and 40 patients with non-atrial fibrillation at the same time were set as the control group.The serum levels of Cdc42 and YAP of patients with non-valvular atrial fibrillation were detected using ELISA, and statistically analyzed in combination with related serological indexes.The relationship between the levels of Cdc42 and YAP and diagnosis of non-valvular atrial fibrillation were investigated.ResultsCompared with the control group, the levels of CRP, national standardized ratio, D-dimer and LAD increased, the LVEF level decreased, and the differences of which were statistically significant(P < 0.01).Compared with patients with paroxysmal atrial fibrillation, the level of LAD increased, the level of left ventricular ejection fraction(LVEF) decreased in patients with persistent atrial fibrillation, and the differences of which were statistically significant(P < 0.01).Compared with the control group, the serum levels of Cdc42 and YAP increased in patients with atrial fibrillation, and the differences of which were statistically significant(P < 0.01).Compared with patients with paroxysmal atrial fibrillation, the serum levels of Cdc42 and YAP increased in patients with persistent atrial fibrillation, and the differences of which were statistically significant(P < 0.01).The results of Pearson correlation analysis showed that the serum levels of Cdc42 and YAP were positively correlated with the levels of D-dimer and LAD in patients with atrial fibrillation(P < 0.05 to P < 0.01), and negatively correlated with LVEF(P < 0.01).The area under ROC curve of the combined diagnosis of atrial fibrillation with Cdc42 and YAP was significantly higher than that of the single diagnosis(P < 0.05).The expression levels of D-dimer, LAD, Cdc42 and YAP increasing and LVEF level dereasing were the risk factors of arial fibrillation(P < 0.05 to P < 0.01).ConclusionsAbnormal expression of serum Cdc42 and YAP in patients with atrial fibrillation may be involved in the occurrence and development of atrial remodeling in atrial fibrillation, and which can be used as a potential indicator for the early diagnosis of atrial fibrillation.
Early warning value analysis of early detection indicators of COVID-19 in critical disease

Abstract:
ObjectiveTo analyze the early warning value of early detection indicators of corona virus disease 2019(COVID-19) in critical disease.MethodsThe patients with COVID-19 were divided into the critical group and non-critical group, and diabetic group and non-diabetic group.The levels of early blood glucose, neutrophils, C reactive protein, serum amyloid A, sialic acid, globulin, cystatin C, creatine kinase, creatine kinase isoenzyme, α-hydroxybutyrate dehydrogenase, lactate dehydrogenase, lactate dehydrogenation enzyme isozyme, serum albumin and other indicators were compared between the critical group and non-critical group, and between diabetic group and non-diabetic group.ResultsThe differences of the gender and age between critical group and non-critical group were not statistically significant(P>0.05).The differences of the levels of neutrophils, serum amyloid A, sialic acid, creatine kinase isoenzyme, α-hydroxybutyrate dehydrogenase, lactate dehydrogenase, lactate dehydrogenase isoenzyme and serum albumin between critical group and non-critical group were statistically significant(P < 0.05 to P < 0.01), and there was no statistical significance in the levels of blood glucose, C reactive protein, globulin, cystatin C and creatine kinase between two groups(P>0.05).There was no statistical significance in the gender and age between the diabetic group and non-diabetic group(P>0.05), the differences of the levels of blood glucose and serum albumin between two groups were statistically significant(P < 0.01 and P < 0.05), and the differences of the levels of neutrophils, C-reactive protein, serum amyloid A, sialic acid, globulin, Cystatin C, creatine kinase, creatine kinase isoenzyme, α-hydroxybutyrate dehydrogenase, lactate dehydrogenase and lactate dehydrogenase isoenzyme between two groups were not statistically significant(P>0.05).ConclusionsThe inerease of neutrophils, serum amyloid A, sialic acid of inflammatory indexes, creatine kinase isoenzyme, α-hydroxybutyrate dehydrogenase, lactate dehydrogenase, lactate dehydrogenase isoenzyme and decrease of the albumin are the early predictors of critical disease.The decrease of nutritional status caused by hyperglycemia has an important impact on the severity of COVID-19.

Abstract:
ObjectiveTo observe the clinical efficacy of electroacupuncture at "Intestine Three Needles" in the treatment of intestinal dysfunction in sepsis.MethodsA total of 60 sepsis patients with intestinal dysfunction were randomly divided into the control group and observation group(30 cases in each group).The clinical efficacy, serum inflammatory markers, plasma motilin level, gastrointestinal function index, APACHE-Ⅱ score and gastrointestinal function AGI score in two groups were observed before and after 7 days of treatment.ResultsThere was no statistical significance in each index between two groups before treatment(P>0.05).After treatment, the serum inflammation index, measuring points of defecating, intra-abdominal pressure, abdomen circumference, APACHE-Ⅱscore and gastrointestinal AGI score decreased, the motilin level and number of bowel sounds per minute increased, and the above indexes in observation group were better than those in control group(P < 0.05 to P < 0.01).The total effective rate in observation group was better than that in control group after 7 days of treatment(P < 0.05).ConclusionsElectroacupuncture at "Intestine Three Needles" can significantly enhance the gastrointestinal motility, reduce the gastrointestinal inflammation and improve the gastrointestinal function.
Expression of high mobility group protein B1 in breast invasive ductal carcinoma tissues and its relationship with prognosis

Abstract:
ObjectiveTo analyze the expression level of high mobility group protein B1(HMGB1) in breast invasive ductal carcinoma tissues, and its relationship with prognosis.MethodsThe clinicopathological data of 139 female patients with breast invasive ductal carcinoma were collected.Immunohistochemistry was used to detect the expression levels of HMGB1 in breast invasive ductal carcinoma tissue and adjacent normal tissue.The relationship between HMGB1 expression level and clinicopathological characteristics in breast invasive ductal carcinoma was analyzed, and the relationship between HMGB1 expression level and prognosis in patients with breast invasive ductal carcinoma was evaluated.ResultsThe nuclear high expression rate and cytoplasmic positive rate of HMGB1 in breast invasive ductal carcinoma tissues were 80.58% and 16.55%, respectively, which were significantly higher than those in adjacent normal tissues(46.04% and 0.00%) (P < 0.01).The cytoplasmic positive expression rates of HMGB1 in high histological grade, negative estrogen receptor and negative progesterone receptor of breast invasive ductal carcinoma tissues were higher(P < 0.05), and the nuclear expression rate in high histological grade tissue was significantly higher(P < 0.05).The tumor histological grade was an independent factor associated with high nuclear expression of HMGB1 in breast invasive ductal carcinoma(OR=2.197, P < 0.05).The tumor histological grade(OR=3.028, P < 0.01), estrogen receptor expression(OR=0.133, P < 0.01), and TNM stage(OR=3.817, P < 0.05) were the independent correlated factors for cytoplasmic positive expression of HMGB1 in breast invasive ductal carcinoma tissues.One hundred and thirty-nine breast cancer patients were followed up for 66.5(18-75) months, the overall 5-year survival rate and 5-year relapse-free survival rate were 88.49%(123/139) and 77.70%(108/139), respectively.The 5-year survival rate, 5-year relapse-free survival rate and survival curve among different nuclear and cytoplasmic expression groups were analyzed using Kaplan-Meier method and Log-rank it indicated that high expression of HMGB1 nucleus affected the 5-year relapse-free survival rate(P < 0.05).ConclusionsThe high expression of HMGB1 in cell nucleus and cytoplasm is closely related to a number of poor prognostic factors in patients with breast invasive ductal carcinoma, and the high expression in nucleus is of guiding significance in predicting postoperative recurrence of breast invasive ductal carcinoma.The positive expression in cytoplasm is related to tumor histological grade, estrogen receptor and TNM stage.
Analysis of the frailty state and its influencing factors in elderly patients with atrial fibrillation

Abstract:
ObjectiveTo analyze the frailty state and its influencing factors in elderly patients with atrial fibrillation(AF).MethodsThe clinical data of 124 elderly patients with AF were collected by random sampling survey.The frailty state of all patients was evaluated using FRAIL scale, and its relevant influencing factors were analyzed.ResultsThe age, exercise status, eating habits, nutritional status, anxiety symptoms, depression symptoms, sleep status, cognitive function and other factors had a significant impact on the frailty state of patients(P < 0.05 to P < 0.01).The results of logistic multiple regression analysis showed that the age, nutritional status, ability of daily life, sleep status and cognitive function were positively correlated with the frailty state of patients(P < 0.05 to P < 0.01).ConclusionsThe age, sleep status and cognitive function are the risk factors of the frailty state of patients with AF.The older the patients, the lower the ability of daily life, the worse the sleep status, and the worse the cognitive function, and the more serious the frailty are.
Analysis of bronchodilation test in 212 patients with chronic obstructive pulmonary disease

Abstract:
ObjectiveTo investigate the changes and significance of pulmonary function indexes in patients with chronic obstructive pulmonary disease(COPD) after bronchodilation test.MethodsThe clinical data of 212 patients with COPD were retrospectively analyzed.The pulmonary function was assessed using pulmonary function meter, the lung ventilation was assessed using bronchodilation test, and the severity of dyspnea was assessed using modified British Medical Research Council dyspnea scale(mMRC).ResultsWith the increasing of COPD grade, the mMRC value of patients significantly increased(P < 0.01).The age of patients in severe and extremely severe groups were significantly higher than that in mild and moderate groups(P < 0.01).After bronchodilation test, the forced expiratory volume in one second(FEV1) and forced vital capacity(FVC) in COPD patients increased to some extent, the FVC change value in severe and extremely severe groups were significantly greater than that in mild and moderate groups(P < 0.01).The change rates of FEV1 and FVC increased with the exacerbation of COPD(P < 0.05 to P < 0.01), but there was no statistical significance in the change of FEV1 among patients with different degrees of COPD(P>0.05).The mMRC in COPD patients was positively correlated with the age, FVC change value and FVC change rate(r=0.267, 0.825, 0.330, P < 0.01), and negatively correlated with FEV1(r=-0.804, P < 0.01).ConclusionsThe FEV1 is an indicator of airflow limitation in patients with COPD, and the changes of FVC between before and after bronchodilation test can be used to evaluate gas entrapment, and the gas entrapment is associated with symptoms of dyspnea.
Analysis of early death factors after surgical treatment for Stanford type A aortic dissection

Abstract:
Objective To analyze the treatment process and outcome of patients with Stanford A aortic dissection and summarize the experience of surgical treatment. Methods Retrospective analysis was performed on the surgical data, treatment plan and prognosis of 51 cases with Stanford type A aortic dissection.Patients were divided into death group and nondeath group according to whethe they died or not. Results Compared with the non-death group, patients in the death group were significantly involved in the aortic root, the aorta occlusion time was longer, the intraoperative blood transfusion volume was higher, more postoperative adjuvant therapy was required with CRRT and ECMO, and the stay time in ICU and hospitalization time were longer(P < 0.05 to P < 0.01).Multiple regression analysis of causes of death suggested that aortic root involvement, postoperative CRRT and ECMO were correlated with death(OR=9.714, 8.831 and 10.997, P < 0.05). Conclusions Emergency surgery is the most effective treatment for Stanford A aortic dissection.Root "sandwich" technique is an effective method for root bleeding and aortic valve insufficiency.The hemostasis of distal and proximal segments of aorta is the key to the success of the operation.

Abstract:
Objective To explore the effects of Solitaire AB stent thrombus removal combined with butylphthalide sodium chloride injection on serum HCY, PLGF, NIHSS score and mRS score in patients with acute anterior circulation large vessel occlusive stroke. Methods A total of 120 patients with acute anterior circulation cerebral infarction were selected as the experimental subjects of the study.The patients were divided into the control group and observation group by random number table method, with 60 cases in each group.Patients in the control group were treated with Solitaire AB stent thrombus removal.Patients in the observation group were given butylphthalide sodium chloride injection on the basis of the control group.Serum HCY, PLGF and NIHSS score and mRS score were compared between the two groups. Results Before treatment, there was no significant difference in the clinical pathological data of the two groups of patients and the comparison of various indicators in this study.After treatment, the serum HCY level in the observation group immediately after surgery was(17.35±5.01) μmol/L, the level of HCY on the 14th day after operation was (13.79±3.13) μmol/L, the immediate level of HCY in the control group was(21.54±4.95) μmol/L and the level of HCY on the 14th day after operation was(16.65±4.23) μmol/L.The improvement of the observation group was significantly better than that of the control group (P < 0.01).The PLGF level of patients in the observation group was(79.28±9.13) ng/L at 14 days after operation.The PLGF level in the observation group was(62.35±7.26) ng/L, and the PLGF level in the observation group was significantly better than that of the control group.The NIHSS score and mRS score in the two groups were decreased(P < 0.01).After 14 days of treatment, the NIHSS score in the control group was(17.52±3.37), which was higher than that in the observation group(13.28±3.19) (P < 0.01).The proportion of patients with mRS score ≤ 2 in the control group was 28.33%, which was lower than 68.33% in the observation group(P < 0.01). Conclusions Solitaire AB stent thrombus removal combined with butylphthalide sodium chloride injection can effectively increase the serum HCY expression level and reduce the PLGF level, the patient's NIHSS and mRS scores in patients with acute anterior circulation large vessel occlusive stroke, which can improve the clinical efficacy.
Effect of hyperbaric oxygen combined with r-globulin on the rehabilitation of viral encephalitis children

Abstract:
Objective To explore the effects of hyperbaric oxygen combined with r-globulin on the rehabilitation and levels of β-endorphin(β-EP), myelin basic protein(MBP) and myocardial enzyme spectrum in children with viral encephalitis(VE). Methods One hundred and ten children with VE were divided into the observation group and control group using random number table method(55 cases in each group).The control group was treated with r-globulin, while the observation group was treated with hyperbaric oxygen combined with r-globulin.The clinical efficacy, clinical symptom, relief time, levels of β-EP, MBP and myocardial enzyme spectrum, and incidence rate of sequelae were observed in two groups. Results The therapeutic efficiency in observation group(90.91%) was significantly higher than that in control group(72.73%) (P < 0.01).The remission time of fever, headache, nausea and vomiting and consciousness in observation group were significantly lower than those in control group(P < 0.01).After treatment, the serum levels of β-EP and MBP in two groups significantly decreased(P < 0.01), and which in observation group were significantly lower than those in control group(P < 0.01).After treatment, the serum levels of LDH、CK、CK-MB and AST in two groups significantly decreased(P < 0.01), and which in observation group were significantly lower than those in control group(P < 0.01).The incidence rate of sequelae in observation group(9.01%) was significantly lower than that in control group(23.64%) (P < 0.05). Conclusions Hyperbaric oxygen combined with r-globulin can effectively treat the VE.The neurologic function can be improved, and the incidence rate of sequelae can be reduced by decreasing the levels of β-EP, MBP and myocardial enzyme in VE children.It is worthy of clinical promotion.
Clinical effects of the preoperative measurement combined with intraoperative precise puncture percutaneous kyphoplasty in the treatment of osteoporotic vertebral compression fractures

Abstract:
ObjectiveTo observe the feasibility and clinical effects of preoperative measurement combined with intraoperative precise puncture percutaneous kyphoplasty(PKP) in the treatment of osteoporotic vertebral compression fractures. MethodsThe clinical data of 98 single-segment osteoporotic vertebral compression fractures patients treated with PKP were retrospective analyzed.Eighty-five patients were followed up for more than 1 year.According to the puncture method during operation, the patients were divided into the observation group(47 cases) and control group(38 cases).The observation group and control group were treated with preoperative image measurement combined with precise puncture PKP and conventional PKP, respectively.The operative time, number of intraoperative fluoroscopy, recovery of injured vertebral height, postoperative bone cement distribution and leakage were compared between two groups.The pain relief and quality of life improvement in two groups were evaluated using visual analog scale(VAS) and Oswestry disability index(ODI). ResultsThe operation time and intraoperative fluoroscopy times in observation group were lower than those in control group, and the proportion of bone cement diffusion to the midline in observation group was higher than that in control group(P < 0.05 to P < 0.01).The difference of the amount of intraoperative bone cement injection between two groups was not statistically significant(P>0.05).After surgery, the VAS score and ODI in two groups decreased, the mean height of injured vertebrae and local Cobb angle were significantly recovered compared with those before surgery, and the difference of which was statistically significant(P < 0.01).The differences of the VAS score, ODI, average height of injured vertebrae and local Cobb angle between two groups at different time points were not statistically significant(P>0.05).The results of postoperative CT scan showed that the incidence rates of bone cement leakage in the observation group and control group were 8.5%(4/47) and 21.0%(10/38), respectively, and the difference of which was statistically significant(P < 0.05). ConclusionsThe preoperative image measurement combined with precise puncture during the PKP can improve the accuracy of vertebral puncture, reduce the operative time and fluoroscopy times.Moreover, it makes the bone cement better diffuse over the midline, and leakage rate of bone cement decrease.
Relationship of serum uric acid and ambulatory blood pressure in elderly patients with primary hypertension

Abstract:
Objective To explore the relationship of serum uric acid and ambulatory blood pressure in patients with primary hypertension. Methods The 24-hour ambulatory blood pressure in 61 elderly patients with primary hypertension was monitored, and the serum levels of uric acid were detected.According to the median level of serum uric acid, the patients were divided into the normal uric acid group(NUA group) and high uric acid group(HUA group).The 24 h average systolic and diastolic blood pressure(24hSBP and 24hDBP), daytime average systolic and diastolic blood pressure(dSBP and dDBP), nighttime average systolic and diastolic blood pressure(nSBP and nDBP) and blood pressure variability(BPV)[including 24-hour systolic blood pressure standard deviation(24hSBPSD) and 24-hour diastolic blood pressure standard deviation(24hDBPSD)] were compared between two groups.The correlation between serum uric acid level and blood pressure indexes was analyzed.According to 24hSBP, the patients were devided into the group with good blood pressure control and the group with poor blood pressure control, and the differences of serum uric acid level and ambulatory blood pressure parameters between the two groups were compared. Results The 24hSBP, dSBP and SBPSD in HUA group were higher than those in NUA group(P < 0.05), and significantly positively correlated with the serum uric acid level(P < 0.05 to P < 0.01).The uric acid level in the group with good blood pressure control was lower than that in the group with poor blood pressure control(P < 0.01).The levels of SBP, dSBP and nSBP in the group with good blood pressure control were lower than those in the group with poor blood pressure control(P < 0.01). Conclusions With the increasing of the level of serum uric acid in patients with primary hypertension, the systolic blood pressure and BPV increase.Monitoring the serum uric acid has important clinical significance in the prevention of hypertension.
Effect of the hip function and walking ability on quality of life and their correlation in elderly patients with hip fracture after operation

Abstract:
ObjectiveTo investigate the effects of the hip function and walking ability on quality of life and their correlation in elderly patients with hip fracture after operation. MethodsOne hundred and sixty elderly hip fracture patients treated with operation were selected as the research objects.The patients were investigated using the general information questionnaire, hip function score questionnaire, walking ability test and quality of life scale. ResultsThe total scores of hip function, walking ability and quality of life were(56.95±5.09) points, (15.88±3.03) points and(62.12±11.84) points, respectively.The effects of different ages, gender, educational levels, surgical methods and complicated basic diseases on the postoperative quality of life were statistically significant(P < 0.05 to P < 0.01).The hip function and walking ability of patients were positively correlated with quality of life(P < 0.05). ConclusionsThe hip joint function and walking ability are positively correlated with the quality of life after operation.Clinicians should take reasonable measures to improve the quality of life of patients by enhancing their hip function and walking ability.

Abstract:
Objective To investigate the effects of dexmedetomidine(Dex) on the postoperative cognitive function and plasma inflammatory cytokines in elderly patients with cirrhosis. Methods Fifty-two type ASA Ⅱ-Ⅲ elderly patients with cirrhosis scheduled by liver resection were divided into the control group and Dex group(26 cases in each group).In the control group, the routine general anesthesia was used, while in the DEX group, on the basis of routine general anesthesia, the Dex(0.5 g/kg) was pumped for 10 min before anesthesia induction, and 0.4 μg·kg-1·h-1 infusion until the peritoneum was suture.The serum levels of S-100β protein, inflammatory cytokines and other indexes in two groups were detected before 1 day of surgery, immediately after surgery, and after 24 h and 48 h of surgery, and the cognitive function of patients was evaluated using MMSE. Results The serum levels of S-100β protein, IL-6 and TNF-α in two groups after operation significantly increased compared with before operation(P < 0.05 to P < 0.01).The serum levels of S-100β protein, IL-6 and TNF-α in Dex group at the end of operation, 24 h and 48 h after operation were significantly lower than those in control group(P < 0.05 to P < 0.01).The incidence rate of postoperative cognitive dysfunction(POCD) in Dex group was significantly lower than that in control group(P < 0.05). Conclusions Dex can decrease the incidence rate of POCD in the elderly cirrhosis patients treated with liver resection, which might be mediated by the inhibition of perioperative inflammatory response.
Analysis of the relationship between BRCA1/2 gene mutation and clinicopathological characteristics of epithelial ovarian cancer

Abstract:
ObjectiveTo explore the relationship between BRCA1/2 mutation and clinicopathological features of epithelial ovarian cancer. MethodsThe BRCA gene in 70 patients with epithelial ovarian cancer diagnosed by pathology was detected, the BRCA1/2 mutation was analyzed, and the relationship between BRCA1/2 mutation and clinicopathogical characterloistics of ovarian cancer patients(including age, clinical stage, pathological type, familial inheritance and platinum drug sensitivity) was analyzed. ResultsAmong 70 patients with epithelial ovarian cancer, 20 cases with BRCA1/2 gene mutation were found(accounting for 28.5%), which included 16 cases with BRCA1/2 gene mutation and 4 cases with BRCA2 gene mutation.The incidence rate of high-grade papillary carcinoma, susceptibility to platinum therapy and family history in BRCA1/2 mutation group were higher than those in non-mutation group(P < 0.05 to P < 0.01), but there was no statistical significance in the age and clinical stage between two groups(P>0.05). ConclusionsThe epithelial ovarian cancer with BRCA1/2 mutation is prone to recurrence, high grade malignancy, and sensitive to platinum drugs.The BRCA1/2 gene mutation in epithelial ovarian cancer patients is related to the family genetic history of the patients.
Effect of IVF-ET unilateral salpingectomy on the serum levels of AMH and ovarian function

Abstract:
Objective To study the effects of in vitro fertilization-embryo transfer(IVF-ET) unilateral salpingectomy on the serum levels of anti-Müllerian hormone(AMH) and ovarian function. Methods A total of 200 patients with IVF-ET were divided into group A(70 cases treated with bilateral resection), group B(70 cases treated with unilateral resection), and group C(60 cases without resection) according to the method of salpingectomy.The AMH level and ovarian function on injection day, bilateral ovarian sinus follicles(AFC), use time and dosage of gonadotropin(Gn), number of eggs taken, effective embryos, high-quality embryos, total number of pregnancy, abortion, premature delivery and term delivery were compared among the three groups. Results The levels of AMH, luteinizing hormone and estradiol in group A were significantly lower than those in group B and group C, the levels of follicle stimulating hormone and progesterone in group A were significantly higher than those in group B and group C, and the AMH level in group B was significantly lower than that in group C(P < 0.05 to P < 0.01).The bilateral AFC in group A was significantly lower than that in group B and group C, the Gn dosage in group A was significantly higher than that in group B and group C, and the Gn dosage in group B was significantly higher than that in group C(P < 0.05 to P < 0.01).There was no statistical significance in the number of eggs taken among the three groups(P>0.05).The number of effective embryos and high-quality embryos in group A were significantly lower than that in group B and group C (P < 0.05).The abortion rate in group A was significantly higher than that in group C, and the rate of full-term delivery in group A was significantly lower than that in group B and group C(P < 0.05 to P < 0.01). Conclusions The IVF-ET unilateral salpingectomy can have a certain impact on the AMH level and ovarian function, but there was no significant change in pregnancy outcome.
Efficacy analysis of the Zista channel in the treatment of multi-segment lumbar spinal stenosis

Abstract:
ObjectiveTo explore the clinical efficacy of minimally invasive decompression and internal fixation under the Zista channel in the treatment of multi-segment lumbar spinal stenosis(LSS). MethodsThe medical records of 64 patients with multi-segment LSS were collected.Thirty patients treated with traditional open decompression and internal fixation, and 34 patients treated with minimally invasive decompression and internal fixation under the Zista channel were divided into the open group and channel group, respectively.The surgical indicators, complications, preoperative and postoperative 3 months and two years pain visual simulation(VAS) score, Japanese Orthopaedic Association(JOA) score, postoperative Oswestry disability index(ODI), MacNab surgery satisfaction evaluation and intervertebral fusion were compared between two groups. ResultsThe intraoperative blood loss and length of hospital stay in channel group were lower than those in open group(P < 0.01), and the differences of the operation time and complications between two groups were not statistically significant(P>0.05).After 3 months and 2 years of surgery, the VAS score and ODI were significantly lower than those before surgery, and the JOA score was higher than that before surgery in two groups(P < 0.05 to P < 0.01).However, the difference of each index during the follow-up period between two groups was not statistically significant(P>0.05).The differences of the surgical satisfaction and intervertebral fusion rate between two groups was not statistically significant(P>0.05). ConclusionsThe minimally invasive decompression and internal fixation under the Zista channel in the treatment of LSS can improve the recent pain and limited function of LSS, and has less trauma and faster recovery compared with traditional open surgery.

Abstract:
Objective To detect the expression levels of microRNA-34a(miR-34a) in breast cancer and corresponding normal tissues, and study the correlation between miR-34a and clinicopathology of breast cancer. Methods The expression levels of miR-34a in 67 breast cancer specimens were detected using qRT-PCR technology, and analyze the correlation between it and clinicopathological factors. Results The expression level of miR-34a in breast cancer tissue was significantly lower than that in matched adjacent tissue(P < 0.01).The differences of the expression levels of miR-34a in cancer tissues among the patients with different ages, sex, menstrual status, progesterone receptor levels, Ki67 expression and human epidermal growth factor receptor levels were not statistically significant(P>0.05).The differences of the expression levels of miR-34a in cancer tissues tumor among the patients with different tumor diametec lymph node metastasis, pathological type, clinical stage, estrogen receptor(ER) level and triple negative were statistically significant(P < 0.05 to P < 0.01).The expression level of miR-34a in tumor diameter ≤ 2 cm group was higher than that in tumor diameter >2 cm group(P < 0.05), the expression level of miR-34a in lymph node without metastasis group was higher than that in lymph node with metastasis group(P < 0.05), and the expression level of miR-34a in non-invasive group was higher than that in invasive group(P < 0.05).The expression level of miR-34a in stage Ⅰ and Ⅱ groups were higher than that in Ⅲ+Ⅳ group(P < 0.01).The expression level of miR-34a in positive ER group was higher than that in negative ER group(P < 0.01), and the expression level of miR-34a in triple negative group was lower than that in non-triple negative group(P < 0.01). Conclusions The expression level of miR-34a in breast cancer is low, and the analysis results of various clinical and pathological factors indicates that it may play an important role in the development and prognosis of breast cancer.
The significance of the phenylalanine hydroxylase gene mutation in the diagnosis of phenylketonuria

Abstract:
ObjectiveTo explore the diagnostic significance of phenylalanine hydroxylase(PAH) gene mmutation in phenylketonuria(PKU), and its influence on children with PKU.MethodsOne hundred and thirty-five children with PKU were investigated.The mutations of PAH gene in all cases were detected using polymerase chain reaction and Sanger sequencing.The large number of deletion and duplication of PAH gene were investigated using multiplex ligation-dependent probe amplification(MLPA), and the data of detection was treated with SPSS 21.0 software to analyze the mutation of PAH and its effects on intelligence quotient of KPU children.ResultsAmong the 135 PKU children(270 pairs of PAH alleles), the PAH gene mutations in 12 children(accounted for 8.89%) were found, and 12 mutation sites were identified, the mutation frequency of R243Q, Y204C, R241C, IVS4-1, R413P, R111X, R261Q, W326X and Y356X were 5.19%, 5.19%, 3.70%, 2.96%, 2.22%, 1.48%, 1.48%, 0.74% and 0.74% of the total PAH alleles, respectively, while the mutation frequency of E56D, F161S and A345T was not obvious.The results of PAH gene mutations detected by SNaPShot, Sanger and MLPA in 12 children with PKU showed that the homozygous mutations accounted for 16.67%, and the compound and heterozygous mutations accounted for 83.33%.The PA and FIQ scores in 12 children with PKU after intervention were lower and higher than those before intervention, respectively(P < 0.05), and the differences of the VIQ and PIQ between before and after intervention were not statistically significant(P>0.05).The results of Pearson analysis showed that the mean controlled concentration of PAH in 12 PAH-mutated PKU children during treatment was negatively correlated with the intelligence quotient test value at the first year of life(r=-0.924, P < 0.01).ConclusionsIn the process of clinical treatment of PKU children, the scientific and reasonable detection of PAH gene mutation and strict control of children's PAH blood concentration have better clinical application and promotion value, and it is also one of the key factors to improve children's intelligence quotient.

ObjectiveTo observe the clinical effects of sodium hyaluronate combined with recombinant human epidermal growth factor(rhEGF) in the treatment of xerophthalmia after senile cataract surgery.MethodsNinety xerophthalmia patients after phacoemulsification were randomly divided into the observation group and control group(45 cases in each group).The control group was treated with sodium hyaluronate eye drops on the basis of anti-inflammatory drugs, 1 drop/time, 3 times/day.The observation group was additionally treated with eye drops, 1 drop/time, 3 times/day based on the control group.The treatment time in two groups was 28 days.The tear film break-up time(BUT) test, Schirmer I test(SITI), corneal fluorescent staining(FL) and symptom score in two groups were performed before treatment, after 14 and 28 days of treatment.The levels of interleukin-6(IL-6) and hypersensitive C reactive protein(hs-CRP) in tears of two groups were detected.ResultsWith the progress of treatment time, the BUT and SIT values in two groups significantly increased(P < 0.05), and the FL, symptom score, levels of IL-6 and hs-CRP significantly decreased(P < 0.05).The BUT and SIT values in observation group after 14 days and 28 days of treatment were significantly higher than those in control group(P < 0.01).The FL value, symptom score, levels of IL-6 and hsCRP in observation group after 14 and 28 days of treatment were significantly lower than those in control group(P < 0.01).ConclusionsThe combination of sodium hyaluronate and rhEGF in the treatment of xerophthalmia after senile cataract surgery is effective.
Study on the correlation of obstructive sleep apnea syndrome with functional constipation

ObjectiveTo investigate the correlation between obstructive sleep apnea syndrome(OSAS) and functional constipation(FC).MethodsOne hundred and eighty-five patients with OSAS were divided into the observation group(82 OSAS cases complicated with FC) and control group(103 cases).The apnea-hypopnea index(AHI), sleep parameters, sleep structure and body mass index(BMI) were comapred between two groups.ResultsThe AHI and BMI values in observation group were significantly higher than those in control group[(56.9±17.34) vs(32.3±11.09)](P < 0.01).The AD in moderate and severe FC+OSAS group was significantly higher than that in control group (P < 0.01), while minSaO2% was significantly lower than that in control group (P < 0.01).In the moderate(FC+OSAS) group, the proportion of sleep stage 1+2 and respiration-related microarousal index were(79.6±11.7) and (30.1±10.4), respectively, which were significantly higher than those in control group(P < 0.01).In the severe(FC+OSAS) group, the proportion of sleep stage 1+2 and respiration related microarousal index were(82.3±6.7) and (35.3±17.6), respectively, which were significantly higher than those in control group(P < 0.01).The proportions of slow-wave sleep and REM sleep in the moderate and severe (FC+OSAS) groups were[(8.2±7.3) and(9.6±2.5)] and[(6.1±5.2) and(8.7±1.8)], respectively, and which were significantly lower than those in control group(P < 0.01).ConclusionsFC is closely related to OSAS.
Study on the expression levels of tissue plasminogen activator-inhibitor complex in patients with coronary heart disease and its correlation with prognosis

ObjectiveTo investigate the expression level change of tissue plasminogen activator-inhibitor complex(t-PAIC) in patients with coronary heart disease(CHD), and its correlation with prognosis.MethodsOne hundeed and eighty patients with CHD, which included 107 cases with unstable angina pectoris and 73 cases with stable angina pectoris, were set as the CHD group, and 60 patients without CHD were as the control group.The serum levels of creatine kinase isoenzyme(CKMB), total cholesterol(TC), C reactive protein(CRP), fasting blood glucose(FBG), plasminogen activator inhibitor 1(PAI-1) and tissue plasminogen activators(t-PA) were observed in two groups.The three, double and single-vessel lesions in CHD group, and levels change of PAI-1 and t-PA in control group were analyzed.ResultsThe serum levels of TC, CRP, FBG, CKMB and PAI-1 in unstable angina pectoris patients of CHD group were significantly higher than those in stable angina pectoris pateints of CHD group and control group(P < 0.05), and the serum level of t-PA in unstable angina pectoris of CHD group was lower than that in stable angina pectoris pateints of CHD group and control group(P < 0.05 to P < 0.01).The serum levels of TC, CRP, FBG, CKMB and PAI-1 in stable angina pectoris patients of CHD group were higher than those in control group(P < 0.05 to P < 0.01).The serum levels of PAI-1 and t-PA in the three-vessel lesion group, double-vessel lesion group, single-vessel lesion group and control group decreased in turn, respectively, and the differences of which were statistically significant(P < 0.01).ConclusionsThe t-PA level is correlated with the severity of CHA and number of coronary artery stenosis.The more severe the disease and more coronary stenosis are, the higher the level of PAI-1 and lower the level of t-PA are.The disease development of patients with CHD can be predicted through the detection of t-PA.

Abstract:
ObjectiveTo study the clinical effect of acoustic power target drug delivery combined with HRZES regimen on the retreatment of cavitary pulmonary tuberculosis.MethodsEighty patients with recurrent cavitary pulmonary tuberculosis were divided into two groups by the random number table method, with 40 cases in each group.The control group was treated with HRZES.The observation group was treated with acoustic power target drug delivery based on HRZES.Lesion absorption, cavity closure, sputum negative conversion and complications at 2, 4 and 8 months after treatment were compared.ResultsThe lesion absorption rate, cavity closure efficiency and sputum conversion rate in the observation group were significantly higher than those in the control group at 2, 4, and 8 months(P < 0.05).There was no significant difference in adverse reaction rate between the two groups(P>0.05).ConclusionsAcoustic power target drug delivery combined with HRZES regimen for retreatment of cavitary pulmonary tuberculosis is safe and effective, which can promote the cavity shrinkage, sputum negative conversion and lesion absorption without increasing adverse reactions.
Clinical analysis of small-dose bone cement prophylactic strengthening of sandwich vertebra in the treatment of "sandwich" osteoporotic vertebral compression fracture

Abstract:
ObjectiveTo investigate the necessities of the small-dose bone cement prophylactic strengthening of sandwich vertebra in the treatment of "sandwich" osteoporotic vertebral compression fracture during percutaneous kyphoplasty.MethodsA total of 50 "sandwich" osteoporotic vertebral compression fracture patients treated with percutaneous kyphoplasty were selected, 29 cases were treated with the small-dose bone cement prophylactic strengthening of sandwich vertebra based on the conventional percutaneous kyphoplasty(observation group) and 20 cases were treated without sandwich vertebrula strengthening(control group).The incidence of sandwich vertebral fractures was followed up.ResultsThe differences of the preoperative bone mineral density, age and body mass index were not statistically significant between two groups(P>0.05).One case with fracture in the observation group and 8 cases with fractures in the control group were identified, and the difference of postoperative fracture rate between two groups was statistically significant(χ2=4.73, P < 0.05).ConclusionsIn the conventional percutaneous kyphoplasty of "sandwich" osteoporotic vertebral compression fracture, the small-dose bone cement prophylactic strengthening of sandwich vertebra can reduce the risk of postoperative sandwich vertebral fractures.
The application value of USCOM in volume therapy of patients treated with hip replacement and its effect on early prognosis

Abstract:
ObjectiveTo investigate the clinical application value of ultrasonic cardiac output monitor(USCOM) in volume therapy of patients treated with hip replacement and its effect on early prognosis.MethodsOne hundred and twenty-six volume therapy patients treated with hip replacement were randomly divided into the observation group(USCOM monitoring fluid replacement, 63 cases) and control group(routine fluid replacement, 63 cases).The general information, clinical indicators, arterial blood lactic acid levels at different time points and occurrence of postoperative complications between two groups were compared.ResultsThe differences of the baseline data of age, sex, body mass index, operation time, anesthesia time, ASA grade and complications between two groups were not statistically significant(P>0.05).The intraoperative bleeding volume, urine volume, total crystalloid volume, total transfusion volume and total hospitalization time in observation group were obviously lower than those in control group, and the colloid liquid volume in observation group was obviously higher than that in control group(P < 0.01).The differences of the vasoactive drugs usage between two groups was not statistically significant(P>0.05).The arterial blood lactate level in observation group was obviously lower than that in control group at T1(P < 0.01), and the differences of the arterial blood lactate levels at other time-points between two groups were not statistically significant(P>0.05).The incidence rates of nausea, vomiting and nemia of postoperative complications in observation group were obviously lower than those in control group(P < 0.01).ConclusionsThe clinical effects of USCOM in volume therapy of patients treated with hip replacement is significant, which can reduce the occurrence of postoperative complications and improve the early prognosis of patients.
Clinical study of double small incision ACDF in the treatment of the continuous 4-level cervical spondylotic myelopathy

Abstract:
ObjectiveTo explore the clinical efficacy of double small incision anterior cervical discectomy and fusion(ACDF) in the treatment of continuous 4-level cervical spondylotic myelopathy(CSM).MethodsSeventy patients with CSM were divided into the double small incision group and single incision group according to the random number table method, and the double small incision group and single incision group were treated with the double small incision and traditional single incision ACDF, respectively.The surgical completion and follow-up complication rate in two groups were recorded, and the scores of the Japanese Orthopaedic Association(JOA), cervical total range of motion(ROM), cervical functional disability index(NDI) and visual analogue scale(VAS) were compared between two groups before surgery, after 3 months of surgery and at the last follow-up.ResultsThere was no statistical significance in the operation time, incision length and bleeding volume between two groups(P>0.05).The incidence rate of complications in double small incision group was lower than that in single incision group(P < 0.01).There was no statistical significance in the VAS score, cervical ROM, NDI and JOA score between two groups at each observation time(P>0.05).The VAS score, cervical ROM and NDI in two groups gradually decreased before surgery, after 3 months of surgery and at the last follow-up(P < 0.01), and there was no statistical significance in the JOA score between two groups(P>0.05).ConclusionsThe double small incision ACDF can achieve the same short-term efficacy as traditional single incision in the treatment of continuous 4-level CSM, but the double small incision ACDF can reduce the complications of anterior cervical surgery such as hoarse voice and dysphagia, which has certain application value.
Study on the short-term effects of modified frontal sinus endoscopic surgery in the treatment of refractory rhinosinusitis

Abstract:
ObjectiveTo investigate the short-term efficacy of modified frontal sinus endoscopic surgery in the treatment of refractory rhinosinusitis.MethodsSeventy-eight patients with refractory rhinosinusitis were divided into the control group and observation group according to the random number table method(39 cases in each group).The control group was treated with sinus open surgery, and the observation group was treated with the modified frontal sinus endoscopic surgery.Two groups were followed up for 3 months.The incidence rate of complication, recurrence rate, olfactory function before and after operation, nasal mucosa ciliary transport rate and quality of life were compared between two groups.ResultsThe total effective rate in observation group was significantly higher than that in control group(P < 0.01).The smell level in observation group was significantly better than that in control group(P < 0.05).After 3 months of operation, the nasal mucosa ciliary transport rates in two groups increased compared with before operation(P < 0.01), and which in observation group was significantly higher than that in control group(P < 0.01).The incidence rate of postoperative complications and recurrence rate in observation group(7.69% and 2.56%) were lower than those in control group(25.64% and 20.51%), respectively(P < 0.05).The symptoms of mental function and physical function in two groups after operation were significantly improved compared with those before operation(P < 0.01), and the improvement in observation group was more obvious than that in the control group(P < 0.01).ConclusionsThe modified frontal sinus endoscopic surgery in the treatment of refractory rhinosinusitis can effectively improve the nasal mucociliary transmission rate, significantly improve the patient's olfactory function and quality of life, and is high safety and not easy to relapse.

ObjectiveTo evaluate the effects of clinical pharmacist intervention to optimize insulin use in patients with type 2 diabetes mellitus.MethodsA total of 136 type 2 diabetes mellitus(T2DM) patients treated with subcutaneous insulin were divided into the control group(68 cases) and observation group(68 cases) according to the random number table method.The control group was treated with the traditional health education, and the observation group was treated with the clinical pharmacists to participate in the guidance of insulin medication for 3 months.The differences of the cognitive level of insulin use, insulin preservation and compliance, blood glucose level and problems in the course of medication were compared between two groups before and after intervention.ResultsThe name, type and preservation method of insulin, needle specification, precautions before injection, injection method, precautions during injection, needle use, mealtime, score of hypoglycemia self-help, MMAS-8 score and good compliance rate of insulin use in observation group were higher than those in control group(P < 0.01).After intervention, the levels of fasting blood glucose, 2 h postprandial blood glucose and HbA1c in observation group were significantly lower than those in control group(P < 0.01), and the C-peptide level in observation group was significantly higher than that in control group(P < 0.01).During the study period, the incidence rates of fear of insulin injection, hypoglycemia, abnormal skin at the injection site, pain during injection and tip leakage in observation group were lower than those in control group(P < 0.05 to P < 0.01).ConclusionsThe participation of clinical pharmacists in the guidance of insulin use in elderly diabetic patients can improve the cognitive level of insulin use and medication compliance, maintain the stability of blood glucose level, and improve medication safety.

Abstract:
ObjectiveTo evaluate the application value of the assessment system based on competency in the training of general surgical residents.MethodsThe assessment system was designed and developed based on the job competency(including 4 first-level indicators and 20 second-level indicators), and applied in the standardized training for general surgeons in The First Affiliated Hospital of Bengbu Medical College in 2020.Among 221 trainees, 124 professional postgraduate students and 97 standardized training physicians were divided into the group A and group B, respectively.The group A and group B were subdivided into the A1 and B1(training duration for 1 year), and A2 and B2 groups(training duration for 2 years) according to the training time.After the training, the trainees' satisfaction with the evaluation system was investigated.ResultsThe overall satisfaction with the assessment system in trainees was 97.94%.The differences of the scores of the first-level indicators of medical ethics, clinical practice ability, clinical comprehensive ability, examination and attendance, and total performance between group A and group B were not statistically significant(P>0.05).The scores of six second-level indicators(including doctor-patient relationship, medical history quality, clinical skills, surgical assistant, ability to deal with complex patients and clinical thinking ability) in group A2 and group B2 were significantly higher than those in group A1 and group B1(P < 0.05 to P < 0.01).The scores of the physical examination in group A2 were lower than that in group A1(P < 0.05).ConclusionsThe evaluation system based on job competency plays a good guiding role in the standardized training and assessment of general surgeons, and has certain guiding value for the training of students with different seniority.

ObjectiveTo deepen the young doctors' understanding of anatomical structures and increase their interest in arthroscopic surgery through 3D printing of full-scale physical models of knee diseases.MethodsFifty-six medical undergraduate clinical internship students were randomly divided into the observation group and control group(28 cases in each group).The observation group was taught using the 3D printing of full-scale physical models of knee joint, and the control group was taught using the traditional teaching model.After completing the training tasks according to the teaching requirements, the teaching effectiveness in two groups were evaluated according to the results of students' theoretical assessment and questionnaire investigation.ResultsAmong the objective evaluation indexes, the scores of reading film test and arthroscopic operation evaluation in observation group were higher than those in control group(P < 0.01).Among the subjective evaluation indexes, the scores of the teaching interesting, teaching participation, learning effectiveness, understanding of teaching, learning attention, arthroscopic acceptability and arthroscopic skill improvement in observation group were higher than those in control group(P < 0.05 to P < 0.01).ConclusionsThe 3D printed model of knee joint can significantly improve the clinical teaching effects of arthroscopy, and has a great advantage compared with the traditional teaching mode.The 3D printed model of knee joint has good practicability and clinical value in the teaching of arthroscopic surgery.

ObjectiveTo discuss the clinical application value of pulse indicator continuous cardiac output(PICCO) monitoring based on guidance of early goal-directed fluid therapy(EGDT) in patients with severe multiple injuries.MethodsSixty patients with severe multiple injuries were divided into the control group and observation group by random number table method(30 cases in each group).The control group was given the capacity management under central venous pressure(CVP) monitoring, and the observation group was given the capacity management under PICCO.The critical degree score, changes of various physiological indexes and prognosis were compared between two groups, and the hemodynamic parameters changes were compared in the observation group between before and after treatment.ResultsThe APACHE Ⅱ scores after 6, 24 and 48 h of treatment and ISS scores after 24 and 48 h of treatment in observation group were significantly lower than those in control group(P < 0.01).The differences of the heart rate, mean arterial pressure, serum lactic acid level, urine volume and PaO2/FiO2 after 6, 24 and 48 h of treatment between two groups were statistically significant(P < 0.05 to P < 0.01).The mechanical ventilation time, duration of ICU and 72 h total fluid resuscitation in observation group were significantly less than those in control group(P < 0.01), and the standard rate of 6 h fluid resuscitation in observation group was significantly higher than that in control group(P < 0.05), and there was no statistical significance in the 28 d mortality between two groups(P>0.05).The results of comparison within the observation group showed that the cardiac output index, intrathoracic blood volume index, total end-diastolic volume index and peripheral vascular resistance index increased after 6, 24 and 48 h of treatment increased(P < 0.05 to P < 0.01), and the extracascular pulmonary fluid index decreased significantly compared with before treatment(P < 0.05 to P < 0.01).ConclusionsThe PICCO monitoring based on EGDT can be used in volume management of patients with severe multiple injuries, which can improve the severity, physiological indicators and hemodynamic status of patients, and has a certain effect on improving clinical treatment effect and prognosis.

Abstract:
ObjectiveTo investigate the effects of modified prone position on patients treated with lateral inter vertebral foramen surgery.MethodsOne hundred and thirty patients treated with lateral intervertebral foramen surgery were randomly divided into the control group and observation group(65 cases in each group).The control group were treated with lateral intervertebral foramen surgery in the conventional prone position, while the observation group were treated with lateral intervertebral foramen surgery in the modified prone position.The operative index, pain level, recovery and clinical satisfaction were compared between two groups.ResultsThe operation time in observation group was shorter than that in control group(P < 0.01), the number of C-arm fluoroscopy in observation group was less than that in control group(P < 0.01), and the difference of the intraoperative blood loss was not statistically significant(P>0.05).The differences of the VAS scores and ODI between two groups at 1 month and 2 months after operation were not statistically significant(P>0.05).The clinical satisfaction in observation group was higher than that in control group(P < 0.01).ConclusionsThe modified prone position in lateral intervertebral foramen surgery can effectively improve the surgical index, and improve the clinical satisfaction of patients.

Abstract:
ObjectiveTo investigate the effects of family function and mental resilience on self-management behavior in patients with gestational diabetes mellitus(GDM).MethodsA total of 204 GDM pregnant women were investigated using the general information questionnaire, family care index, mental resilience scale and self-management behavior scale.ResultsThe total scores of family function, mental resilience and self-management behavior in GDM pregnant women were(8.59±1.79) points, (68.41±9.31) points and(114.39±12.32) points, respectively, and the score index was 68.09%.The each dimension and total scores of family function and mental resilience were positively correlated with the each dimension and total scores of self-management behavior in GDM pregnant women(P < 0.05 to P < 0.01).ConclusionsThe family function and mental resilience levels are positively correlated with self-management level in GDM pregnant women.The family function and mental resiliencey improvement of GDM pregnant women should be focused on, so as to improve their level of self-management.
Study on the reliability and validity of modified four-grade functional activity score in the evaluation of postoperative mobility ability of patients in thoracic surgery

Abstract:
ObjectiveTo explore the applicability of modified four-grade functional activity score in evaluating postoperative mobility ability of patients in thoracic surgery.MethodsSeventy patients who underwent thoracic surgery were randomly selected as the research objects, and given modified four-grade functional activity score after surgery.The evaluation results were analyzed by item analysis (including item discrimination test and item homogeneity test), reliability analysis and validity analysis.In the item discrimination test, the scores of 70 patients were sorted in descending order.The scores of the first 25% of patients and the last 25% of patients were used as the cut-off points of high value and low value, respectively.The first 25% of patients were set as high score group and the last 25% of patients as low score group, and the differences of each item between the two groups were compared.The homogeneity of items was tested by Pearson correlation coefficient between the scores of each item and the total score of the modified four-grade functional activity score.The reliability of the scale was analyzed using Cronbach's α coefficient.The content validity of the scale was evaluated by content validity index (CVI).ResultsThe results of item analysis showed that there were significant differences in each item between high score group and low score group (P < 0.01), and the score of each item was positively correlated with the total score of the scale (P < 0.05).The results of reliability analysis indicated that the Cronbach's α coefficient of the scale was 0.934.The results of validity analysis displayed that the CVI of each item at item level and the overall consistency CVI of each item at scale level were 1.0.ConclusionsThe modified four-grade functional activity score has good applicability in the evaluation of postoperative mobility ability of patients in thoracic surgery.
Analysis of the quality of life and its influencing factors in type 2 diabetes mellitus patients treated with insulin

Abstract:
ObjectiveTo investigate the quality of life in type 2 diabetes mellitus(T2DM) patients treated with insulin, and its influencing factors.MethodsA total of 200 T2DM patients treated with insulin were selected from a tertiary hospital in Hu'nan province using convenience sampling method.The patients were investigated using insulin therapy related quality of life(ITR-QOL), insulin treatment attitude scale(ITAS) and insulin treatment adherence questionnaire(ITAQ).ResultsThe score of the ITR-QOL was(91.50±11.99) points.The results of multiple linear regression analysis showed that the residence status, working status, daily injection number, treatment attitude and compliance were the influencing factors of the quality of life in T2DM patients treated with insulin(P < 0.05 to P < 0.01), and which explained 38.3% of all variation.ConclusionsThe quality of life in T2DM patients treated with insulin was at the middle level.The medical personal should pay attention the T2DM patients treated with insulin, provide targeted support and interventions to improve the quality of life.

Abstract:
ObjectiveTo investigate the status of knowledge, belief and practice of pediatric nurses on medical adhesive related skin injury(MARSI), and analyze the influencing factors.MethodsA total of 352 pediatrics nursing staff from 11 grade A comprehensive hospitals and 94 nursing staff from 2 grade A children specialized hospitals were selected as the research objects.The self-designed "questionnaire on knowledge, belief and practice of pediatric nurses on MARSI" was used to conduct an online questionnaire survey from September 1 to 8, 2018 to understand the knowledge, belief and practice of pediatric nurses on MARSI.The descriptive analysis, LSD test and rank sum test were used for statistical analysis.ResultsThe overall score of MARSI in paediatric nurses was moderate.The score of MARSI knowledge dimension was(23.17±5.82) points, and which was equivalent to 64 points in percentage system.The score of attitude dimension was(43.36±8.26) points, which was equivalent to 87 points in percentage system.The score of behavior dimension was(48.14±10.76) points, which was equivalent to 74 points in percentage system.The MARSI score and three dimensions scores of nurses in training group were higher than those in non-training group(P < 0.01).ConclusionsThe knowledge level and behavior level of pediatric nurses on MARSI should be improved urgently, and the training of pediatric nurse on MARSI should be further improved.
