Effect of NCAPH on migration and invasion of breast cancer cells

Abstract:
ObjectiveTo investigate the expression of non-SMC condensin Ⅰ complex subunit H (NCAPH) in breast cancer and its impact on the migration and invasion of breast cancer cells.MethodsThe TCGA database was used to obtain the original data, which was then analyzed to compare the expression of NCAPH in breast cancer tissue and normal breast tissue.The study also utilized the GEO database for verification purposes.The study involved an analysis of NCAPH protein expression in the HPA database.The role of NCAPH in the prognosis of breast cancer patients was analyzed using the Kaplan-Meier database.Interference fragments of the NACPH gene were screened and the effects of knocking down the NCAPH gene on the migration and invasion ability of breast cancer cells were evaluated using scratch and Transwell assays.ResultsBoth TCGA and GEO database indicated that NCAPH was highly expressed in breast cancer tissues (P < 0.01).In contrast, the HPA database revealed that NCAPH protein levels were low in normal breast tissues, but significantly elevated in breast cancer tissues.Additionally, Kaplan-Meier database demonstrated that high expression of NCAPH was associated with a reduce survival rate in breast cancer patients(P < 0.01).There were significant differences in NCAPH mRNA expression levels between breast cancer cell lines SKBR-3, MDA-MB-231, and MCF-7 and human normal breast epithelial cell line MCF-10A (P < 0.05).The results of both scratch and Transwell assays demonstrated that interference with NCAPH significantly inhibited the migration and invasion ability of breast cancer cells (P < 0.01).ConclusionsNCAPH is highly expressed in breast cancer tissues, and correlated with the prognosis of breast cancer patients.Knocking down NCAPH can significantly inhibit the migration and invasion of breast cancer.
Effects of miR-141 targeting ZEB2 on the proliferation, migration, and invasion of glioma cells

Abstract:
ObjectiveTo investigate the effects of miR-141 targeting zinc finger E-box binding homeobox 2 (ZEB2) on the proliferation, migration, and invasion of glioma cells.MethodsHuman glioma cell line U87 was cultured in vitro, transfected, and then divided into blank control (NG) group, negative transfection (mimics-NC) group, and overexpression miR-141 (miR-141-mimics) group, co-transfection (miR-141-mimics+pc-ZEB2) group.Real-time fluorescence quantitative PCR was used to detect the levels of miR-141 and ZEB2 mRNA; the proliferation, migration and invasion of U87 cells in each group were detected; Western blotting was used to detect the expression of proliferation cell nuclear antigen (PCNA), invasion and migration-related proteins (E-cadherin, N-Cadherin, Vimentin); the targeting relationship between miR-141 and ZEB2 was predicted by TargetScan database and verified by dual luciferase reporter gene experiment.ResultsU87 cells were successfully transfected with mimics-NC, miR-141-mimics, and pc-ZEB2;compared with the NG group and the mimics-NC group, the U87 cell proliferation inhibition rate and E-Cadherin protein expression in the miR-141-mimics group were significantly increased (P < 0.05), and the clone formation rate, scratch healing rate, number of invasive cells, the expression of ZEB2 mRNA and protein, and the expression of PCNA, N-Cadherin, and Vimentin protein were reduced (P < 0.05);compared with the miR-141-mimics group, the U87 cell proliferation inhibition rate and E-Cadherin protein expression in the miR-141-mimics+pc-ZEB2 group were significantly reduced (P < 0.05), and the clone formation rate, scratch healing rate, number of invasive cells, the expression of ZEB2 mRNA and protein, and the expression of PCNA, N-Cadherin, and Vimentin protein were increased (P < 0.05).ConclusionsOverexpression of miR-141 may target and down-regulate ZEB2 to inhibit the proliferation, invasion, and migration of glioma U87 cells.
Study on the relationship between lncRNA SNHG5 and miR-132-3p expression in placenta tissue of preeclampsia patients

Abstract:
ObjectiveTo observe the expression of long non-coding RNA small nucleolar RNA host gene 5 (lncRNA SNHG5) and miR-132-3p in placental tissue of preeclampsia (PE) patients, and to analyze the correlation between them, and their diagnostic value for PE.MethodsA total of 112 patients with PE were selected as the study subjects, and divided into severe group (n=58) and mild group (n=54) according to the severity of PE.Sixty healthy pregnant women in the same period were selected as the control group.Real-time fluorescence quantitative PCR was used to detect the expression of lncRNA SNHG5and miR-132-3p in placental tissue.Pearson correlation analysis was applied to observe the correlation between lncRNA SNHG5 and miR-132-3p in placental tissue of PE patients, and the correlation between lncRNA SNHG5, miR-132-3p and clinical indicators.ROC curve was employed to analyze the diagnostic value of lncRNA SNHG5 and miR-132-3p levels for PE.ResultsThe level of 24 h urinary protein, systolic blood pressure and diastolic blood pressure of PE patients in the mild group and severe group was higher than that in the control group (P < 0.05), and the level of systolic blood pressure and diastolic blood pressure in the severe group was higher than that in the mild group (P < 0.05).The level of lncRNA SNHG5 in the mild group and severe group was lower than that in the control group (P < 0.05), and the level of miR-132-3p was higher than that in the control group (P < 0.05);the level of lncRNA SNHG5 in the severe group was lower than that in the mild group (P < 0.05).The expression of lncRNA SNHG5 and miR-132-3p in placental tissue was correlated with 24 h urine protein, systolic blood pressure and diastolic blood pressure of PE patients (P < 0.05 to P < 0.01), but was not correlated with age, gestational week and body mass index (P>0.05).There was a negative correlation between lncRNA SNHG5 and miR-132-3p expression in placental tissue of PE patients (r=-0.509, P < 0.05).The results of ROC curve analysis showed that the area under the curve of lncRNA SNHG5 and miR-132-3p in placental tissue in the diagnosis of PE was 0.865 and 0.883, the cutoff value was 0.93 and 1.44, the sensitivity was 87.9% and 79.3%, and the specificity was 71.7% and 83.3%, respectively; the area under the curve of the combination of lncRNA SNHG5 and miR-132-3p in the diagnosis of PE was 0.921, the sensitivity and specificity were 89.7% and 81.7%, respectively.ConclusionsLncRNA SNHG5 is lowly expressed and miR-132-3p is highly expressed in the placenta tissue of PE patients, and they are negatively correlated, which provides new ideas for the diagnosis and targeted therapy of PE.
Effect of circ_AFF2 on the proliferation, migration and invasion of rheumatoid arthritis synovial fibroblasts by regulating PI3K/AKT pathway

Abstract:
ObjectiveTo explore the mechanism of circ_AFF2 regulating the proliferation, migration, and invasion of rheumatoid arthritis (rheumatoid arthritis, RA) synovial fibroblasts by regulating the phosphatidylinositol 3-kinase (PI3K)/serine/protein kinase B (AKT) pathway.MethodsqRT-PCR was used to detect the relative expression of circ_AFF2 in healthy control synovial tissue and RA synovial tissue.By culturing RA synovial fibroblast MH7A cells in vitro, the cells were divided into si-NC group (transfected with si-NC), si-circ_AFF2 group (transfected with si-circ_AFF2), and si-circ_AFF2+IGF-1 Group (after transfection of si-circ_AFF2, treated with 50 ng/mL PI3K/AKT pathway activator IGF-1).Then qRT-PCR was used to detect the relative expression of circ_AFF2 in each group of cells; MTT assay was used to detect cell viability in each group; Western blotting was used to detect the expression of Ki-67, PCNA, MMP-2, E-cadherin, N-cadherin, and PI3K/AKT pathway-related proteins in each group of cells; Transwell assay detects cell migration and invasion ability of each group.ResultsCompared with healthy control synovial group, the relative expression of circ_AFF2 was significantly increased in RA synovial tissue (P < 0.01).Compared with the si-NC group, the relative expression of circ_AFF2, cell viability, Ki-67 and PCNA protein expressions in the si-circ_AFF2 group were significantly decreased (P < 0.01).Compared with the si-NC group, the number of migrating cells, the number of invasive cells, the expression of MMP-2 and N-cadherin protein in the si-circ_AFF2 group were significantly decreased (P < 0.01), and the protein expression of E-cadherin was significantly increased (P < 0.01).The protein expressions of p-PI3K and p-AKT in the three groups were significantly decreased (P < 0.01), and the protein expressions of p-PI3K and p-AKT in the si-circ_AFF2 group and the si-circ_AFF2+IGF-1 group were lower than those in the si-NC group (P < 0.05), while there was no change in the expression of PI3K and AKT among three groups (P>0.05).Compared with si-circ_AFF2 group, the relative expression of circ_AFF2, cell viability, number of migrating cells, number of invasive cells, Ki-67, PCNA, MMP-2 and N-cadherin protein expressions in si-circ_AFF2+IGF-1 group were significantly increased (P < 0.01), and E-cadherin protein expression was significantly decreased (P < 0.01).ConclusionsCirc_AFF2 can promote the proliferation, migration and invasion of RA synovial fibroblasts by activating the PI3K/AKT pathway.
Expression of Ninjurin2 in breast cancer and its relationship with prognosis

Abstract:
ObjectiveTo explore the expression of nerve injury-induced protein 2(Ninjurin2) in breast cancer and its relationship with prognosis.MethodsImmunohistochemical staining was used to detect the expression of Ninjurin2 in 52 cases of breast cancer tissues and paired adjacent tissues.The relationship between the expression of Ninjurin2 and the clinicopathological characteristics and prognosis of breast cancer was analyzed.The Kaplan-Meier method was used for survival analysis.The expression of Ninjurin2 in breast cancer cell lines(T47D, SKBR3, HCC1954 and MDA-MB231) and normal breast epithelial cell lines(MCF-10A) was detected by Western blotting.Ninjurin2 siRNA sequence(silencing group) and negative control sequence(control group) were transferred into MDA-MB231 cells with lipofectamineTM 2000 kit, and then cell proliferation was detected by clone formation assay and CCK8 method.The invasive ability of MDA-MB231 cells was detected by Transwell assay, and cell migration was detected by cell scratch test.ResultsThe high expression rate of Ninjurin2 in breast cancer tissues was 69.23%(36/52), which was higher than 30.77%(16/52) in adjacent tissues, and the difference was statistically significant(P < 0.05).Compared with MCF-10A cells, the relative expression of Ninjurin2 protein in T47D, SKBR3, HCC1954 and MDA-MB231 cells was higher(P < 0.05).Ninjurin2 was related to breast cancer stage(P < 0.05), but not related to age, menopausal status, tumor size, and lymph node metastasis(P>0.05).The results of survival curve analysis showed that the disease-free survival rate(DFS) of patients with high expression of Ninjurin2 was lower than that of patients with low expression(P < 0.05).Compared with the control group, the relative expression of Ninjurin2 protein, the number of cloned cells, the proliferation activity at 72 h and 96 h of cell culture, the number of invasive cells and cell migration rate in the silence group were lower(P < 0.05).ConclusionsNinjurin2 is highly expressed in breast cancer cells and tissues, and is related to the clinical prognosis of breast cancer.
Effects of long non-coding RNA C5orf66-AS1 on the proliferation and apoptosis of gastric cancer cells by targeting miR-637

Abstract:
ObjectiveTo investigate the effects of long non-coding RNA C5orf66-AS1 on the proliferation and apoptosis of gastric cancer (GC) cells by targeting miR-637, so as to provide a basis for the clinical treatment of GC.MethodsThe cancer tissues and adjacent tissues of 32 GC patients were collected.Human gastric adenocarcinoma cell lines MKN28, MKN451, BGC823, MGC803, AGS and normal gastric epithelial cell line GES-1 were cultured in vitro.The expression of C5orf66-AS1 and miR-637 was detected by qRT-PCR.AGS cells in logarithmic growth phase were transfected with liposome transfection reagent and divided into control group, GV144 group, GV144-C5orf66-AS1 group, miR-NC group, miR-637 mimics group, GV144-C5orf66-AS1 + miR-NC group, and GV144-C5orf66-AS1 + miR-637 mimics group.CCK-8 method was used to detect cell proliferation ability, flow cytometry was used to analyze the apoptosis, Western blotting was used to determine the expression levels of CyclinD1, B-cell lymphoma-2 gene (Bcl-2) and its homologous protein Bax of cells.RNA immunoprecipitation experiment was used to verify the relationship between C5orf66-AS1 and miR-637.ResultsCompared with adjacent tissues or normal gastric epithelial cell line GES-1, the expression levels of C5orf66-AS1 and miR-637 in GC tissues and cell lines MKN28, MKN451, BGC823, MGC803, AGS were reduced (P < 0.05).The AGS cells with the lowest expression level of C5orf66-AS1 were selected for transfection experiments.After transfection of GV144-C5orf66-AS1, the C5orf66-AS1, miR-637 expression levels, apoptosis rate and Bax protein expression in AGS cells were significantly increased, and cell viability (48 h, 72 h, 96 h), CyclinD1 and Bcl-2 proteins were reduced (P < 0.05).There was an interaction between C5orf66-AS1 and miR-637.Moreover, the overexpression of miR-637 could make the overexpression of C5orf66-AS1 more effective in inhibiting AGS cell proliferation and promoting apoptosis (P < 0.05).ConclusionsC5orf66-AS1 and miR-637 are lowly expressed in GC tissues and cells.Overexpression of C5orf66-AS1 and up-regulation of miR-637 can synergistically inhibit the proliferation of GC cells and promote apoptosis.
Effects of dapagliflozin on cardiac function in patients with type 2 diabetes mellitus complicated with heart failure with preserved ejection fraction

Abstract:
ObjectiveTo study the effect of dapagliflozin on left ventricular ejection function and quality of life in patients with type 2 diabetes mellitus(T2DM) complicated with heart failure with preserved ejection fraction(HFpEF).MethodsSeventy T2DM patients with HFpEF treated with insulin to reduce blood glucose were selected and randomly divided into observation group(n=38) and control group(n=32).The patients in the observation group were additionally treated with dapagliflozin to control the blood glucose, and patients in the control group were additionally treated with acarbose to control the blood glucose.The 6-minute walk test(6MWT) was used to evaluate the cardiac function of the patients, and the KCCQ questionnaire was applied to evaluate the quality of life of the patients.The 6MWT score, KCCQ score, left ventricular ejection fraction(LVEF), ventricular septal thickness, monocyte/high-density lipoprotein cholesterol ratio(MHR), N-terminal pro-B-type natriuretic peptide(NT-proBNP) and C-reactive protein(CRP) level were compared between the two groups before and after treatment.ResultsAfter two months of treatment, the 6MWT score and MHR in the observation group were lower than those in the control group(P < 0.01 and P < 0.05), and there were no significant differences in the KCCQ score, ventricular septal thickness, LVEF, NT-proBNP and CRP levels between the two groups(P>0.05).After four months of treatment, the 6MWT score, MHR and CRP level in the observation group were lower than those in the control group(P < 0.05 to P < 0.01), the KCCQ score was higher than that in the control group(P < 0.05), and there were no significant differences in the ventricular septal thickness, LVEF and NT-proBNP level between the two groups(P>0.05).After six months of treatment, the KCCQ score and LVEF level in the observation group were higher than those in the control group(P < 0.05), the 6MWT score, MHR and CRP level were lower than those in the control group(P < 0.05 to P < 0.01), and there were no significant differences in the ventricular septal thickness and NT-proBNP level between the two groups(P>0.05).ConclusionsDapagliflozin can improve the clinical symptoms and quality of life of T2DM patients with HFpEF.

Abstract:
ObjectiveTo explore the value of capillary leakage index (CLI) in predicting the condition and evaluating the prognosis of patients with severe acute pancreatitis (SAP) complicated with acute lung injury (ALI).MethodsA total of 78 SAP patients were selected as the study subjects, and divided into non-ALI group (n=40) and ALI group (n=38) based on whether acute lung injury occured.According to the 28-day survival status of the patients, they were divided into survival group (n=45) and death group (n=33).The clinical baseline data of SAP patients were analyzed; the correlation between CLI and acute physiology and chronic health evaluation scoring system (APACHEⅡ), modified CT severity index (MCTSI), and lactate was evaluated using Pearson correlation analysis; the prediction of CLI on the occurrence of ALI and risk of 28-day mortality was analyzed by ROC curve; the cumulative survival rate under different risk stratification levels of CLI was analyzed with Kaplan-Meier survival curve.ResultsThe C-reactive protein (CRP), CLI, lactate level, MCTSI, and APACHEⅡ scores of patients in the ALI group were higher than those in the non-ALI group (P < 0.05 to P < 0.01);the serum albumin (ALB) level of patients in the ALI group was significantly lower than that in the non-ALI group (P < 0.01).There was no statistically significant difference in gender between the survival group and the death group (P>0.05);the age, CRP, CLI, lactate level, MCTSI, and APACHEⅡ scores of patients in the death group were higher than those in the survival group (P < 0.05 to P < 0.01);the ALB level of patients in the death group was significantly lower than that in the survival group (P < 0.01).There was a significant positive correlation between CLI and APACHEⅡ score, MCTSI, and lactate level of SAP patients at admission (P < 0.01).The area under the ROC curve of CLI for predicting SAP complicated with ALI and 28-day mortality was 0.802 (95%CI: 0.705-0.899) and 0.758 (95%CI: 0.651-0.865), respectively; the optimal cutoff values were 0.517 and 0.394, with corresponding CLI values of 9.335 and 9.770, sensitivity of 84.20% and 72.70%, and specificity of 67.50% and 67.70%, respectively.Based on the latest optimal cutoff value of 9.552, Kaplan-Meier survival curve analysis showed that the 28-day cumulative survival rate in the CLI>9.552 group was significantly lower than that in the CLI≤9.552 group (P < 0.01).ConclusionsCLI can predict the occurrence of SAP complicated with ALI in the early stage and plays an important role in evaluating the prognosis of patient, especially for SAP patients with CLI>9.552, who have a higher risk of short-term mortality, which needs special attention from clinical doctors.
Effect comparison between hyperextension traction elastic compression combined with percutaneous pedicle screw in the treatment of single level thoracolumbar fracture

Abstract:
ObjectiveTo compare the effects of hyperextension traction elastic compression combined with percutaneous pedicle screw with traditional posterior incision pedicle screw internal fixation in the treatment of single level thoracolumbar fracture.MethodsThe clinical data of 46 patients with thoracolumbar fracture were retrospectively analyzed.According to different treatment methods, the patients were divided into the hyperextension traction elastic compression combined with percutaneous screw placement group(24 cases) and traditional incision group(22 cases).The anterior vertebral edge height change, Cobb angle change, incision length, intraoperative blood loss, operative time, postoperative hospital stay, preoperative and postoperative visual analogue scale(VAS) scores, postoperative Japanese Orthopaedic Association (JOA) score and Oswestry disability index(ODI) score were compared between two groups.ResultsCompared with the traditional incision group, the incision length, operation time, intraoperative blood loss and postoperative hospital stay in the percutaneous screw placement group reduced (P < 0.05).The Cobb angle of sagittal kyphosis in two groups decreased after surgery (P < 0.05).The anterior margin height of injured vertebrae was significantly higher than that before surgery (P < 0.05).The VAS scores at 1 week and 6 months after surgery were significantly lower than those before surgery (P < 0.05).After 6 months of surgery, the VAS and ODI scores in the percutaneous screw placement group were lower than those in the traditional incision group (P < 0.05), and the postoperative JOA score in the percutaneous screw placement group was higher than those in the traditional incision group (P < 0.05).ConclusionsHyperextension traction elastic compression combined with percutaneous pedicle screw internal fixation in the treatment of thoracolumbar fracture has definite clinical efficacy, which has the advantages of less trauma, less bleeding, quick postoperative functional recovery, etc., and can provide a stable and reliable treatment for thoracolumbar fracture patients.
Application of biofeedback therapy in low anterior rectum resection syndrome

Abstract:
ObjectiveTo investigate the clinical effect of biofeedback therapy (BFT) in low anterior rectal resection syndrome(LARS).MethodsThe data of 165 patients who met the diagnostic criteria of LARS after laparoscopic low anterior rectal resection were analyzed retrospectively.According to the different treatment methods of LARS, they were divided into BFT group(86 cases) and control group(79 cases).The BFT group received two courses of BFT regularly from the 3rd week after discharge, and the control group received routine symptomatic treatment after operation.The changes of LARS scores before and after treatment were compared between the two groups.ResultsThere was no significant difference in LARS scores between the two groups at the time of enrollment and after the first course of treatment(P>0.05).After the second course of treatment, the scores of patients in the two groups were lower than those at the time of enrollment(P < 0.05), and the scores in the BFT group were significantly lower than those in the control group(P < 0.01).Two weeks after the end of the second course of treatment, the scores in the two groups were lower than those at the end of first and second courses of treatment(P < 0.05), and the scores in the BFT group were significantly lower than that in the control group(P < 0.01).ConclusionsBFT can reduce the LARS scores faster than conventional treatment, and accelerate the recovery of LARS patients.

ObjectiveTo analyze the clinical outcomes and related factors of meconium aspiration syndrome (MAS) in neonates.MethodsThe clinical data of 80 neonates with MAS were retrospectively analyzed.According to the prognosis, they were divided into mild group and severe group.The clinical data were compared between the two groups, the risk factors that affected the poor prognosis of MAS were analyzed by multivariate logistic regression, and the incidence of complications and risk factors of major complications in MAS neonates were analyzed.ResultsAmong the 80 neonates, 14 gave up treatment or died, with a poor prognosis rate of 17.50%.The proportion of fetal age>40 weeks, asphyxia history, amniotic fluid Ⅲ ° pollution, placental abnormalities, Apgar score≤7 points at 5 min of birth, cesarean section, chest X-ray grading in the severe group was higher than those in the mild group (P < 0.05 to P < 0.01), and the proportion of Apgar score at 1 min of birth, meconium aspiration through respiratory tract immediately after birth, mechanical ventilation, probiotics supplementation in the severe group was lower than those in the mild group (P < 0.05 to P < 0.01).Multivariate logistic regression analysis showed that gestational age>40 weeks, asphyxia history, amniotic fluid Ⅲ ° pollution, Apgar score < 7 points at 5 min of birth were the risk factors for poor prognosis in MAS neonates (P < 0.05 to P < 0.01), and immediately clearing meconium through respiratory tract after birth, mechanical ventilation, probiotics supplementation were the protective factors (P < 0.05 to P < 0.01).The main complications of MAS neonates were hypoxic-ischemic encephalopathy (43/80, 53.75%), respiratory failure (38/80, 48.75%), pulmonary arterial hypertension (32/80, 40.00%) and metabolic acidosis (28/80, 35.00%).Univariate and multivariate logistic regression analysis showed that gestational age>40 weeks, elderly age of pregnant women, amniotic fluid Ⅲ ° pollution, and history of fetal asphyxia were the main risk factors for MAS neonates complicated with hypoxic-ischemic encephalopathy (P < 0.05 to P < 0.01), while immediately clearing meconium through respiratory tract after birth was the protective factor (P < 0.05).ConclusionsMAS neonates may abandon treatment or die in hospital due to the influence of gestational age, amniotic fluid pollution, asphyxia history, Apgar score and so on.Immediately clearing meconium through respiratory tract after birth, assisted mechanical ventilation and probiotic supplementation can help to improve clinical outcomes and reduce the risk of complications.
Diagnostic value of serum miR-21-3p detection combined with lymph core needle biopsy in cervical lymph node metastasis of thyroid cancer

Abstract:
ObjectiveTo investigate the value of serum miR-21-3p detection combined with ultrasound-guided lymph core needle biopsy (LCNB) in the diagnosis of cervical lymph node metastasis of thyroid cancer.MethodsA total of 80 patients with thyroid cancer with cervical lymph node enlargement received serum miR-21-3p detection and LCNB were collected.The histopathologic results were taken as the gold standard to compare the diagnostic value of serum miR-21-3p, ultrasound-guided LCNB and their combination in thyroid cancer with cervical lymph node metastasis.ResultsThe histopathologic results showed cervical lymph node metastasis in 49 patients and no cervical lymph node metastasis in 31 patients.The accuracy and sensitivity of serum miR-21-3p detection combined with ultrasound-guided LCNB in the diagnosis of thyroid cancer with cervical lymph node metastasis were 91.8% and 87.6%, respectively, which were higher than the accuracy of 76.2%, 84.7% and sensitivity of 82.3% and 85.7% of the two independent examinations (P < 0.05).The area under the receiver operator characteristic curve of the combined examination was 0.907 1, which was higher than that of the two independent examinations (0.776 5 and 0.829 2) (P < 0.05).ConclusionsCompared with single detection, serum miR-21-3p combined with ultrasound-guided LCNB has better accuracy, sensitivity and diagnostic efficiency in the diagnosis of lymph node metastasis of thyroid cancer.
Detection and clinical significance of serum HBV pgRNA in patients with chronic hepatitis B treated with nucleoside/nucleotide analogues

Abstract:
ObjectiveTo observe the changes of serum hepatitis B virus (HBV) pregenomic RNA (pgRNA) levels in patients with chronic hepatitis B (CHB) after antiviral therapy with entecavir (ETV) or tenofovir disoproxil fumarate (TDF), and its relationship with HBsAg and HBV DNA, so as to explore the clinical significance of HBV pgRNA in antiviral therapy.MethodsA total of 89 newly diagnosed CHB patients were selected, including 44 patients receiving ETV treatment (ETV group) and 45 patients receiving TDF treatment (TDF group).The patients were followed up before treatment, at 24 weeks and 48 weeks of treatment, and the serum levels of HBsAg, HBV DNA, HBV pgRNA were measured, the changes and differences in serum HBV pgRNA levels between the two groups were analyzed, as well as the correlation between HBV pgRNA and HBsAg, HBV DNA.ResultsWith the prolongation of treatment time, the HBV pgRNA levels of patients in the two groups significantly decreased (P < 0.01).At 24 weeks of treatment, the HBV pgRNA levels of patients in the two groups were lower than those before treatment (P < 0.05), and at 48 weeks of treatment, the HBV pgRNA levels were lower than those at 24 weeks of treatment (P < 0.05).Before antiviral treatment and at 24 weeks and 48 weeks of treatment, the HBV pgRNA levels were significantly positively correlated with HBsAg levels in the ETV group (P < 0.01), and HBV pgRNA levels were also significantly positively correlated with HBV DNA (P < 0.01).Before antiviral treatment and at 24 weeks and 48 weeks of treatment, the HBV pgRNA levels were significantly positively correlated with HBsAg levels in the TDF group (P < 0.01);before antiviral treatment and at 24 weeks of treatment, the HBV pgRNA levels were significantly positively correlated with HBV DNA in the TDF group (P < 0.01), but the HBV pgRNA levels were not significantly correlated with HBV DNA in the TDF group at 48 weeks of treatment (P>0.05).The negative conversion rate of HBV DNA of patients in the TDF group was higher than that in the ETV group at the same time point, and at 48 weeks of treatment, there was a statistically significant difference between the two groups (P < 0.05).ConclusionsThe serum HBV pgRNA levels of CHB patients show a significant downward trend during the process of antiviral treatment, and serum HBV pgRNA has certain clinical significance in evaluating and monitoring the antiviral efficacy of nucleos(t)ide analogues.

Abstract:
ObjectiveTo investigate the correlation between the expression and methylation of long-chain 3-hydroxyacyl-CoA dehydrogenase(LCHAD) gene and pathological pregnancy state.MethodsEighteen cases with early-onset severe preeclampsia were selected as group A, 15 cases with hemolysis, elevated liver enzymes and low platelets(HELLP) syndrome as group B, 17 cases with antiphospholipid syndrome as group C, and 20 normal pregnant women as the control group.The general data, methylation of the promoter region of LCHAD gene and the expression level of LCHAD mRNA in each group were compared, and the correlation between the methylation level of promoter region of LCHAD gene and mRNA expression was analyzed.ResultsThe gestational age in group A and group B was lower than that in control group(P < 0.05), which in group C was higher than that in group B(P < 0.05);the systolic blood pressure, diastolic blood pressure and 24 h urine protein level in group A and group B were higher than those in control group(P < 0.05);the levels of alanine aminotransferase and serum creatinine in group B were higher than those in control group(P < 0.05).The methylation level at -899, -853, -615, -984, -774, -727 and -579 site of LCHAD gene in group A was higher than that in control group, which at -853 site was higher than that in group B, and which at -899, -853 and -615 site higher than that in group C(P < 0.05).The methylation level at -899, -853, -774 and -615 site of LCHAD gene in group B was higher than that in control group, which at -899, -853band -615 site was higher than that in group C(P < 0.05).The expression of LCHAD mRNA in group A, group B and group C was lower than that in control group(P < 0.05).The methylation level at -899, -853, -727, -615 and -579 site of LCHAD gene in group A was negatively correlated with the mRNA expression level(P < 0.05);the methylation level at -899, -853 and -615 site of LCHAD gene in group B was negatively correlated with the mRNA expression level(P < 0.05);the methylation level at all sites of LCHAD gene in group C had no significant correlation with the mRNA expression level(P > 0.05).ConclusionsThe methylation of LCHAD gene in pregnant women with early-onset severe preeclampsia and HELLP syndrome is at a high level, and it is closely related to mRNA expression.
Application of piezosurgery combined with contra-angle high speed turbines in extraction of impacted mandibular teeth

Abstract:
ObjectiveTo investigate the application in minimally invasive extraction of impacted mandibular teeth by using piezosurgery combined with contra-angle high speed turbines.MethodsEighty-four eligible patients were selected and randomly divided into piezosurgery, contra-angle high speed turbines, and the combined groups, with 28 cases in each group.The impacted teeth of the three groups were extracted by contra-angle high speed turbines, piezosurgery, and piezosurgery combined with contra-angle high speed turbines, respectively.The operation time, postoperative pain degree within 24 hours, mouth opening degree, and postoperative swelling degree of patients were compared among the three groups.ResultsThe operation time of patients in the combined group was shorter than that in contra-angle high speed turbines and piezosurgery groups (P < 0.05).Twenty-four hours after the operation, the VAS score of the combined group was lower than that of the contra-angle high speed turbines group (P < 0.05).There were statistically significant differences in postoperative pain degree, mouth opening degree, and postoperative swelling degree among the 3 groups (P < 0.05), and the combined group was better than the other two groups.ConclusionsPiezosurgery combined with contra-angle high speed turbines can shorten the operation time of teeth extraction, improve teeth extraction efficiency, reduce the postoperative reaction, and improve the comfort of patients.
Clinical effects of CO2 fractional laser preconditioning combined with photodynamic therapy in the treatment of actinic keratosis

Abstract:
ObjectiveTo evaluate the clinical efficacy and safety of CO2 fractional laser preconditioning combined with photodynamic therapy in the treatment of actinic keratosis.MethodsA total of 30 patients with actinic keratosis were randomly divided into the control group and observation group (15 cases in each group).The control group was treated with photodynamic therapy, while the observation group was pretreated with CO2 fractional laser, then treated with photodynamic therapy.Two groups were treated once every 1 to 2 weeks.If the skin lesions did not completely subside after one treatment, the treatment could be repeated, and the total treatment times of one course of treatment should not exceed 6 times.Two groups were followed up after 4 weeks and 6 months of treatment.ResultsAfter 4 weeks of treatment, the effective rate of the observation group(100.00%) was higher than that of control group(60.00%) (P < 0.05).After 6 months of treatment, the recurrence rate of the control group(46.67%) was higher than that of the observation group(6.67%) (P < 0.05).There was no statistical significance in the pain score during treatment between two groups (P>0.05), and no serious adverse events occurred in two groups during treatment and follow-up.ConclusionsCO2 fractional laser pretreatment can better remove the solar keratosis skin rash, effectively reduce the recurrence of skin rash, and is safe without obvious adverse reactions.
Relationship between cholecystolithiasis complicated with non-alcoholic fatty liver disease and metabolic syndrome

Abstract:
ObjectiveTo explore the relationship between cholecystolithiasis complicated with non-alcoholic fatty liver disease (NAFLD) and metabolic syndrome (MS).MethodsAccording to the principle of sex and age match, we conducted a case-control study with 60 patients with cholecystolithiasis complicated with NAFLD in observation group (1 patient withdrew voluntarily, 59 patients were actually included) and 60 patients with cholecystolithiasis in control group.The general data and related indexes of MS were collected, including waistline, body mass index(BMI), triglyceride (TG), total cholesterol (TC), high density lipoprotein (HDL-C), fasting blood glucose (FPG), fasting insulin (FINS), insulin resistance (IR), alanine aminotransferase (ALT), alanine aminotransferase (AST), tumour necrosis factor-α(TNF-α), interleukin-6 (IL-6), etc.The differences between two groups were analyzed and multivariate conditions logistic regression analysis was used to screen risk factors.ResultsThe prevalence of MS in the observation group was higher than that in the control group (P < 0.05).There was significant difference in waist circumference between the observation group and the control group (P < 0.05).The levels of BMI, MS, TG, TC, ALT, FINS, IR, TNF-α and IL-6 in the observation group were significantly higher than those in the control groups (P < 0.05).Multivariate condition logistic regression analysis showed that BMI, TG, ALT, IR, TNF-α, IL-6 were independent risk factors for cholecystolithiasis complicated with NAFLD (P < 0.05).ConclusionsBMI, TG, ALT, IR, TNF-α, IL-6 are independent risk factors for cholecystolithiasis complicated with NAFLD.Cholecystolithiasis with NAFLD complicated is closely associated with MS.

Abstract:
ObjectiveTo investigate the effectiveness analysis of acupoint application combined with kidney tonification and blood activation method on patients with recurrent abortion.MethodsA total of 90 patients with recurrent abortion were selected and randomly divided into western medicine group (group A), western medicine + triditional Chinese medicine group (group B) and western medicine + Chinese medicine comprehensive treatment group (group C), with 30 patients in each group.Group A: oral aspirin; group B: oral aspirin and triditional Chinese medicine for kidney tonification and blood activation; group C: acupoint application on the basis of group B; all until 12 weeks of pregnancy.The clinical efficacy, serum human chorionic gonadotropin (hCG), progesterone (P), anti-cardiolipin antibody (ACA), anti-β2-glycoprotein Ⅰ antibody (anti-β2-GPⅠ) and ultrasound were observed before and after treatment in the three groups.ResultsAfter treatment, the differences were statistically significant in the efficacy of the three groups (P < 0.05), and the efficacy of group C was higher than that of group A and group B (P < 0.05 to P < 0.01).The hCG and P were significantly higher in patients of the three groups compared with the same group before treatment (P < 0.01), and group C was significantly higher than group A and group B (P < 0.01).The positive rates of ACA and anti-β2-GPⅠ in group B and group C were lower than those before treatment in the same group (P < 0.05), and the difference between the degree of reduction in group A and before treatment was not statistically significant (P>0.05).The positive rates of ACA and anti-β2-GPⅠ between the three groups were statistically significant (P>0.05), among which, group C was lower than group A and group B (P < 0.05).The platelet function improved in all groups, the (D-D) levels were significantly lower than before treatment (P < 0.01), the D-D levels in group C were significantly lower than those in group A and group B (P < 0.01), and group B was significantly lower than group A (P < 0.01).ConclusionsAcupoint application combined with kidney tonification and blood activation method has significant efficacy in the treatment of patients with recurrent abortion, which can effectively improve the hormone level and platelet function of patients and increase the cure rate of patients, and is worthy of clinical application.
Effects of dexamethasone combined with ropivacaine incision infiltration on pain and wound healing after complex anal fistula surgery

Abstract:
Objective: To explore the effects of dexamethasone combined with ropivacaine incision infiltration on pain and wound healing after complex anal fistula surgery.Methods: A total of 120 patients with complex anal fistula surgery were collected and randomly divided into control group,ropivacaine group,and combination group,with 40 patients in each group.All patients were treated by directional thread-drawing procedure.Before surgery,the patients were anesthetized by ropivacaine via spinal anesthesia.During or post-surgery,the incisions were infiltration with dexamethasone combined with ropivacaine in the combination group,the incisions were infiltration with ropivacaine in the ropivacaine group,and the incision of the control group was not treated.The pain,exudation and wound healing time after operation were compared among the three groups.Besides,the rats with anal fistula were randomly divided into control group,ropivacaine group,and combination group.The medication and treatment during and postoperation were the same as those of the patients.The wound tissues were collected 7 days after surgery.Western blotting was used to measure the protein expressions of VEGF and bFGF.Histopathology was used to analyze the inflammatory cell infiltration.qPCR was used to detect the mRNA expressions of IL-1β,IL-6 and TNF-α.Results: At 3,7 and 14 day after surgery,the pain and seepage grades in patients of the ropivacaine group and combination group were significantly better than those of the control group,and the patients in the combination group were also significantly better than those in the ropivacaine group (P<0.01).Animal experiment results showed that,compared to the control group,the protein expressions of VEGF and bFGF in wound tissues in the ropivacaine group and combination group increased significantly,and the inflammatory cells and the mRNA expressions of IL-1β,IL-6 and TNF-α in the ropivacaine group and combination group decreased significantly (P<0.01).Meanwhile,the wound healing rate and protein expression of VEGF and bFGF in the combination group were higher than those in the ropivacaine group,the IL-1β,IL-6,TNF-α MRNA expression was significantly lower than that of ropivacaine group (P<0.01).Conclusions: Dexamethasone combined with ropivacaine incision infiltration significantly attenuates pain and promotes wound healing in patients after complex anal fistula surgery.Animal experiments have reached the same conclusion,which provides a basis for clinical application.

Abstract:
ObjectiveTo investigate the changes of hematocrit (HCT) and pH before and after blood transfusion in patients with traumatic hemorrhage and their relationship with coagulation function.MethodsA total of 102 patients wilh traumatic blood loss were enrolled for the prospective clinical trial.The HCT and pH indexes of each group were detected using coagulometer; the coagulation reaction time (R), coagulation angle (a-Angle), and maximum magnitude of thrombus (MA) were detected using the thromboelastography; the logistic regression analysis was used to analyze the correlation of coagulation function before and after blood transfusion with HCT, pH, R, a-Angle, and MA values.ResultsBefore blood transfusion, HCT, pH, fibrinogen (Fib), a-Angle value, and MA value of group A, B and C were compared, and the corresponding result was group A < group B < group C(P < 0.05);prothrombin time(PT), thrombin time(TT), activated partial thrombopaltin time(APTT), R value were compared, and the result was group A >group B>group C(P < 0.05); after blood transfusion, there was no significant difference in coagulation indexes among groups A, B and C (P>0.05), and all indexes were improved compared with those before blood transfusion(P < 0.05).Before blood transfusion, pH, a-Angle value, MA value of group D, E and F were compared, and the corresponding result was group D < group E < group F (P < 0.05).PT, TT, APTT, R value of group D, E and F were compared, and the corresponding result was group D>group E>group F.pH value of group F and G were compared, and the corresponding result was group F < Group G.R value of group F and G were compared, and the corresponding result was group F>group G (P < 0.05 to P < 0.01).There was no significant difference in a-Angle value, MA value, PT.TT.APTT, and R value between group F and group G(P>0.05);there was no significant difference in the Fib value among group D, E, F(P>0.01) and the Fib values among group D, E, F were smaller than those in group G(P < 0.05 to P < 0.01).There was no significant difference in TT, APTT, R values, a-Angle values, and MA values of group D, E, F and group G after transfusion (P>0.05), and all indexes were significanlly improved compared wilh those before blood transfusion, and the difference within the group was statistically significant (P < 0.05).The higher the levels of HCT and pH, the shorter the ICU stay and bleeding time of traumatic blood loss patients(P < 0.05 to P < 0.01).Pearson correlation analysis found that HCT and pH(< 7.45) were negatively correlated PT, TT, APTT, R value, hospitalization time, bleeding time and other coagulation indexes (P < 0.01).There was a positive correlation between HCT and coagulation indexes such as Fib, a-Angle value, and MA value (P < 0.01).HCT and pH were also positively correlated with the coagulation indexes of traumatic hemorrhage(P < 0.01).ConclusionsThe severity of coagulation dysfunction in patients with traumatic blood loss is associated with the decreases of HCT and pH.The lower the HCT level, the higher the incidence of acidosis and the worse the prognosis.Thus, the detection of HCT and pH before and after blood transfusion is inevitable for the clinical diagnosis and treatment of traumatic hemorrhage, which are also important indicators affecting the coagulation function.
Comparison of the application effects of different anesthesia blocks in elderly patients treated with laparoscopic cholecystectomy

Abstract:
ObjectiveTo compare the application effects of different anesthesia blocks in elderly patients treated with laparoscopic cholecystectomy(LC).MethodsA total of 89 elderly patients udergoing LC were divided into two groups: bilateral transversal plane block (TAPB) combined with bilateral rectus sheath block (RSB) group (48 cases) and bilateral TAPB group (41 cases) according to the way of anesthesia.The intraoperative hemodynamics, stress indexes, postoperative recovery and adverse reactions were compared between two groups.ResultsAt the beginning of surgery, after 30 minutes of surgery and at extubation, the heart rate, systolic blood pressure, diastolic blood pressure, blood glucose and cortisol in the combination group were lower than those in TAPB group(P < 0.05 to P < 0.01).After surgery, there was no statistical significance in the breathing recovery time, eye opening time or orientation recovery time between two groups(P>0.05).The differences of the incidence rates of adverse reactions(including headache, nausea and vomiting, dysuria and low back pain) were not statistically significant between two groups (P>0.05).ConclusionsThe application of TAPB combined with RSB anesthesia in elderly patients treated with LC can better ensure the stability of intraoperative circulation and relieve intraoperative stress, which dose not increase the risk of related adverse reactions and is high safety.

ObjectiveTo investigate the curative effects of Tongluo Kaibi tablet combined with meridian acupoint massage in patients with lumbar disc herniation(LDH) and its influence on lumbar function and pain degree.MethodsA total of 118 LDH patients were randomly dividied into the control group and observation group(59 cases in each group).The routine symptomatic treatment and nursing were given into two groups, the control group was treated with the meridian acupoint massage, and the observation group were additionally treated with Tongluo Kaibi tablet orally on the basis of the control group for 6 months.The curative effects and its influence on lumbar function and pain degree were compared between two groups.ResultsThe total effective rate in the observation group(94.92%) was higher than that in control group(81.36%) (P < 0.05).There was no statistical significance in the visual analogue scale(VAS) scores between two groups before treatment(P>0.05).After 1, 3 and 6 months of treatment, the VAS scores in two groups decreased, and which in the observation group was lower than that in control group(P < 0.05 to P < 0.01).There was no statistical significance in the lumbar function score and quality of life score between two groups before treatment(P>0.05).After treatment, the motion range of lumbar joints(flexion/extension), Japanese Orthopaedic Association scores and quality of life scores in two groups increased compared with before treatment, and which in the obserevation group were higher than those in control group(P < 0.05 to P < 0.01).After treatment, the Oswestry disability index scores in two groups decreased, and which in the observation group was lower than that in control group(P < 0.05).There was no statistical significance in the incidence of total adverse reaction between two groups(P>0.05).ConclusionsTongluo Kaibi tablet combined with meridian acupoint massage can effectively improve the lumbar function and relieve pain in patients with LDH, which is beneficial to improve the prognosis of patients and has good safety.

Abstract:
ObjectiveTo analyze the correlation between visceral adiposity index (VAI), lipid accumulation index (LAP) and diabetic cardiac autonomic neuropathy (DCAN).MethodsTwo hundred and fifty patients with type 2 diabetes mellitus were selected and divided into DCAN group and non-DCAN group according to whether DCAN occurred.The VAI, LAP, Ewing test parameters and related biochemical indexes were compared between the two groups.According to the quartile level of VAI and LAP, the two groups were subdivided into 4 groups (V1-V4 group, L1-L4 group), and the distribution of DCAN in each group was compared.Spearman correlation analysis was used to evaluate the correlation between VAI, LAP levels and each Ewing test parameter of DCAN patients, logistic regression analysis was used to calculate the risk of DCAN in patients with different VAI and LAP levels, and ROC curve analysis was used to evaluate the predictive diagnostic value of each body fat indicator on DCAN.ResultsAmong the 250 patients with type 2 diabetes mellitus, 158 had DCAN and 92 did not have DCAN, and the incidence of DCAN was 63.20%.The age, duration of diabetes mellitus, waist circumference, body mass index, waist to body ratio, waist to hip ratio, VAI, LAP, total cholesterol, triacylglycerol, and low-density lipoprotein cholesterol levels of patients in the DCAN group were significantly higher than those in the non-DCAN group (P < 0.01), while the level of high-density lipoprotein cholesterol was significantly lower than that in the non-DCAN group (P < 0.01).With the increase of VAI and LAP quartiles, the proportion of DCAN patients increased continuously, and the difference of which was statistically significant (P < 0.01).Logistic regression analysis showed that after adjusting the risk factors, the risk of DCAN in the V4 group was about 7.439 times of that in the V1 group (P < 0.05), and the risk of DCAN in the L4 group was about 53.241 times of that in the L1 group (P < 0.01).Spearman correlation analysis showed that VAI and LAP were negatively correlated with Valsalva R-R ratio, heart rate difference of deep breathing, and heart rate difference in upright position (P < 0.05 to P < 0.01), and positively correlated with systolic pressure difference in upright position and diastolic pressure difference in upright position (P < 0.05 to P < 0.01).ROC curve analysis results showed that the area under the curve predicted by VAI and LAP for DCAN in male and female patients was higher than other indicators, and the area under the curve predicted by VAI and LAP in male patients was higher than that in female patients (P < 0.05).ConclusionsThe risk of DCAN is positively correlated with VAI and LAP levels.With the increase of VAI and LAP levels, the risk of DCAN increases.Compared with other body fat indicators, VAI and LAP have better predictive ability for DCAN, and both have higher predictive value in male.

Abstract:
ObjectiveTo explore the effect of Tongyuan method acupuncture on the clinical efficacy of patients with nervous tinnitus of damage of kidney-essence type.MethodsEighty patients with nervous tinnitus were randomly divided into control group and study group, with 40 cases in each group.The control group was treated with conventional treatment, and the study group was treated with conventional treatment combined with Tongyuan method acupuncture.The curative effect, TCM syndrome score, tinnitus handicap index (THI) score, and sleep quality were compared between the two groups.ResultsThe total effective rate in the study group was 97.50%, which was higher than 82.50% in the control group (P < 0.05).Before treatment, there was no significant difference in TCM syndrome scores, THI and Pittsburgh sleep quality index (PSQI) scores between the two groups (P>0.05).After treatment, the TCM syndrome scores, THI and PSQI scores of the two groups decreased (P < 0.05), and which in the study group were significantly lower than those in the control group (P < 0.05 to P < 0.01).ConclusionsTongyuan method acupuncture has a good effect on patients with nervous tinnitus of damage of kidney-essence type, which can significantly improve the clinical symptoms and sleep quality of patients, and reduce the degree of tinnitus disorders.
Analysis on orange risk factors in early pregnancy of delivering women in Anhui province, 2021

Abstract:
ObjectiveTo identify the prevalence of orange (higher) risk referred to as orange risk) and distribution of orange risk factors among early pregnant of delivering women in Anhui province during 2021, in order to provide reference for strengthening pre-pregnancy and early pregnancy care and treatment.MethodsAccording to geographic location, 4 of 16 cities in Anhui province were chosen.Pregnancy risk assessment and relevant information on delivering women in 2021 were collected from Anhui Maternal and Child Health Information System.ResultsThere were 42 321 delivering women in 2021 with risk assessment results during early pregnancy.Among them, 5 092 were assessed as orange risk (12.03%).The prevalence of orange risk was 17.92% in the east, 14.26% in the south, 13.61% in the middle part and 6.21% in the north (P < 0.01).The women of first delivery were different from those with second and more delivery in terms of age, occupation, citizenship, location (P < 0.05 to P < 0.01).The distribution of the 3 types of orange risk factors among women of first delivery was different from those of second and upper delivery (P < 0.01).Among the 35 orange risk factors, 29 were detected (82.86%).The factors comprising more than 10% were, in order of prevalence: BMI≥28 kg/m2 (47.05%), drug-treated diabetes mellitus, thyroid disease and prolactinoma (20.35%), uterus surgery ≥2 times (11.66%), and age ≥40 year-old (10.25%).ConclusionsIn Anhui province, the detection rate of orange risk among early pregnancy of delivering women in 2021 is high, with significant regional differences, and the categories of orange risk factors in early pregnancy are inverted, with a high proportion of category 1 and category 2 factors and nearly half of obesity.Pre-pregnancy and early pregnancy health care and medical services should be strengthened to continuously reduce the number of women with category 1 and 2 orange risk factors in early pregnancy and effectively reduce the incidence of orange risk in early pregnancy.
Construction of a risk prediction model for malnutrition in patients after traumatic fracture surgery

Abstract:
ObjectiveTo explore the nutritional status and risk factors of malnutrition in patients after traumatic fracture surgery, and to construct a nomograph model to predict malnutrition in patients after traumatic fracture surgery.MethodsA total of 400 patients undergoing traumatic fracture surgery were selected as the study subjects, and their nutritional status was evaluated using the mini-nutritional assessment (MNA).The patients were divided into malnutrition group (n=110) and non-malnutrition group (n=290) according to the MVA scores.Univariate analysis and logistic regression analysis were applied to evaluate the risk factors of malnutrition in patients after traumatic fracture surgery, and a nomograph model was constructed to predict the incidence of malnutrition in patients after traumatic fracture surgery.ResultsAmong the 400 patients undergoing trauma fracture surgery, 110 cases (27.50%) had malnutrition.The results of univariate analysis showed that there were significant differences in age, body mass index, smoking history, hypertension, diabetes mellitus, chronic gastritis, fracture type, hemoglobin, albumin, and blood loss between the two groups (P < 0.05 to P < 0.01).Multivariate logistic regression analysis showed that age, diabetes mellitus, chronic gastritis, fracture type, hemoglobin and albumin were independent influencing factors for malnutrition in patients after traumatic fracture surgery (P < 0.05 to P < 0.01).Using the above indicators to construct a nomograph model, the area under the curve for predicting malnutrition after traumatic fracture surgery was 0.865 (95%CI: 0.825-0.905), with a sensitivity of 81.51% and a specificity of 79.82%.ConclusionsThe incidence of malnutrition or malnutrition risk in patients after traumatic fracture surgery is high.Age, diabetes mellitus, chronic gastritis, fracture type, hemoglobin and albumin are independent influencing factors of malnutrition after traumatic fracture surgery.The nomograph model can directly and concisely provide personalized malnutrition risk prediction for patients after traumatic fracture surgery.
Analysis of the current status of recurrence fear in patients with thyroid cancer after surgery and its influencing factors

Abstract:
ObjectiveTo investigate the current status of cancer recurrence fear in patients with thyroid cancer after surgery and its influencing factors.MethodsA total of 93 patients with thyroid cancer who underwent surgical treatment were evaluated by the cancer recurrence fear scale (FCRI-SF), disease perception scale, cognitive emotion regulation questionnaire, social support scale and social restriction scale.The current situation and influencing factors of cancer recurrence fear were analyzed.ResultsThe FCRA-SF score of 93 patients was (13.87±4.04), and 61 patients (65.59%) had more than 13 points.Univariate analysis showed that there were significant differences in the score of FCRI-SF among different gender, age, education, lymph node metastasis and place of residence (P < 0.05 to P < 0.01);There was no significant difference in the score of FCRI-SF among different marital status, occupational status, medical insurance, operation mode and smoking status (P < 0.05).Correlation analysis showed that cancer recurrence fear was positively correlated with disease perception, non-adaptive cognitive strategies, and social limitations (P < 0.01), significantly negatively correlated with social support (P < 0.01), and was not associated with adaptive cognitive strategies (P>0.05).Multiple linear regression analysis showed that age, disease perception, non-adaptive cognitive strategies, social support and social restriction entered the model (F=59.54, P < 0.01), together explained 61.20% of the cancer recurrence fear.ConclusionsAge, disease perception, non-adaptive cognitive strategies, social support and social restriction are important influencing factors of the cancer recurrence fear.Clinical nursing should pay more attention to young thyroid cancer patients, improve social support, and reduce social restriction, disease perception and non-adaptive cognitive strategies.
Effects of metacognition-based mindfulness based cognitive therapy on anxiety and blood glucose in prediabetic patients

Abstract:
ObjectiveTo investigate the effect of mindfulness-based cognitive therapy(MBCT) based on metacognition on anxiety, meta-emotional ability and blood glucose of prediabetic patients.MethodsA total of 126 cases of prediabetic patients with psychological needs diagnosed in the endocrine clinic were selected and randomly divided into control group and study group, with 63 cases in each group.The control group was given the traditional education model.On the basis of this, the study group was given metacognition-based MBCT.The effects of self-rating anxiety scale (SAS), trait meta-mood scale(TMMS), disease knowledge and behavior scores, and blood glucose control were compared before and after intervention 3 months.ResultsBefore intervention, there was no significant difference in related indexes between the two groups(P>0.05).After intervention, SAS, TMMS, disease knowledge and behavior scores, and blood glucose control levels of the study group were all better than those of the control group(P < 0.01).ConclusionsMetacognition-based MBCT can improve patients' metacognitive ability, effectively relieve anxiety, improve disease knowledge score, and effectively control blood glucose level.

Abstract:
ObjectiveTo observe the clinical effects of moxibustion combined with traditional Chinese medicine encapsulation in the treatment of cold-dampness obstruction type lumbar disc herniation(LDH).MethodsA total of 80 patients with cold-dampness obstruction type LDH were randomly divided into the control group and combined treatment group according to random number table method(40 cases each group).The control group were treated with the routine treatment and nursing of cold-dampness obstruction type LDH.The combined treatment group were treated with the moxibustion combined with traditional Chinese medicine encapsulation on the basis of the control group.The clinical efficacy, VAS, ODI and JOA scores in two groups were observed before and after treatment.ResultsAfter treatment, the effective rate in the combined treatment group(95%) was better than that in control group(80%, P < 0.05).The VAS and ODI scores in two groups after treatment were lower than those before treatment, and the scores of combined treatment group were lower(P < 0.01).The JOA scores in two groups after treatment were higher than those before treatment, the score of combined treatment group was higher, and the difference of which between two groups was statistically significant(P < 0.05 to P < 0.01).ConclusionsThe moxibustion combined with traditional Chinese medicine encapsulation can improve the lumbar function and relieve pain of cold-damp-obstruction lumbar disc herniation, and the clinical effect is remarkable and worthy of promoting.
Study on the status of cognitive frailty in elderly patients with type 2 diabetes mellitus and its influencing factors

Abstract:
ObjectiveTo investigate and analyze the status quo of cognitive frailty(CF) in elderly patients with type 2 diabetes mellitus and its influencing factors for providing the direction of nursing intervention to delay the occurrence of CF.MethodsThe status quo of CF in 117 elderly patients with type 2 diabetes mellitus were investigated using the FRAIL frailty scale, montreal cognitive assessment scale and clinical dementia rating scale.The logistic regression analysis was used to analyze the related factors of CF occurrence.ResultsSixty cases with CF(51.28%) included 22 potentially reversible patients(18.80%) and 38 reversible patients (32.48%), and 57 cases (48.72%) with normal cognization were found.The results of univariate analysis showed that the proportions of older age, lower education, body mass index (BMI)≥24 kg/m2, disease course >10 years, disabled living ability, poor sleep and depression in CF group were higher than those in normal congnization group(P < 0.05 to P < 0.01).The results of multivariate analysis showed that the age≥80 years old, BMI≥24 kg/m2, disease duration >10 years, disability of living ability and depression were the risk factors of CF(P < 0.05 to P < 0.01).ConclusionsElderly type 2 diabetes mellitus is more prone to CF, so early screening, clear classification and early intervention should be carried out according to its occurrence factors.
A longitudinal study of disability levels after reperfusion in patients with acute cerebral infarction

Abstract:
ObjectiveTo explore the dynamic change of limb disability, self-care ability and related influencing factors in patients with acute cerebral infarction after reperfusion therapy for 6 months.MethodsA total of 112 patients with acute cerebral infarction after reperfusion therapy in the department of neurology were selected using purposive sampling method.The clinical data of patients were collected.And the modified Rankin scale (mRS score) and Barthel index were used to evaluate the level of disability and the ability of daily living of the patients.Patients with mRS score 2-5 points were defined as disability, and the level of disability was correspondingly calculated.One-way analysis of variance was performed to analyze the influencing factors of patients' daily living ability.ResultsThe results of patients' self-care ability 6 months after treatment showed that 61 patients (57.0%) were completely self-care, and the rest of the patients had varying degrees of self-care impairment.There were statistically significant differences in the total score of self-care ability among patients with or without atrial fibrillation, different onset ages, disease treatment methods, disease severity, and limb rehabilitation training (P < 0.05 to P < 0.01).The disability rates of patients with acute cerebral infarction at 1, 3, and 6 months after reperfusion treatment were 44.86%, 40.19%, and 35.51%, respectively.The mRS results of 1 month, 3 months, and 6 months after treatment showed that the level of patients' physical disability could gradually decrease over time(P < 0.05).ConclusionsThe level of disability in patients with acute cerebral infarction after reperfusion therapy can gradually recover over time, and motor function rehabilitation is an essential factor in the limb function recovery of patients.
Application value of nurse-led graded dietary management in stroke patients with dysphagia

Abstract:
ObjectiveTo evaluate the application effects of nurse-led graded dietary management in stroke patients with dysphagia.MethodsOne hundred stroke patients with dysphagia were randomly divided into the control group and observation group(50 cases in each group).The doctor-led dietary management was given to the control group, and the nurse-led graded dietary management was given to the observation group.The swallowing function in two groups were evaluated using water swallow test.The clinical efficacy and incidence rate of aspiration were compared between two groups.ResultsAfter intervention, the swallowing function of the observation group(grade Ⅰ 19 cases, grade Ⅱ 18 cases, grade Ⅲ 10 cases, grade Ⅳ 2 cases and grade Ⅴ 1 case) were significantly improved compared with the control group(grade Ⅰ 8 cases, grade Ⅱ 16 cases, grade Ⅲ 12 cases, grade Ⅳ 11 cases and grade Ⅴ 3 cases)(P < 0.01).The clinical efficacy in observation group was significantly better than that in control group(P < 0.05).The aspiration rate in observation group was significantly lower than that in control group(P < 0.01).ConclusionsThe nurse-led graded dietary management can effectively improve the swallowing function of stroke patients with dysphagia, improve clinical efficacy and reduce the incidence rate of aspiration.
Analysis of the mediating effect of nutrition and fatigue on social support and self-care ability of patients with chronic heart failure

Abstract:
ObjectiveTo explore the status, relationship and mechanism of self-care ability, social support, nutrition status and fatigue level in patients with chronic heart failure(CHF).MethodsA cross-sectional study was conducted to investigate the CHF patients using the self-care ability assessment scale, social support rating scale, heart failure-specific micro-nutrition evaluation tool and Chinese multi-dimensional fatigue scale.On the basis of understanding the current situation and correlation analysis, a structural equation model was built to verify the mutual relationship, and Bootstrap method was used to test and verify the intermediary role.ResultsThe total scores of self-care ability, social support, nutrition and fatigue in patients with CHF were (19.740±11.656) points, (24.288±5.050), (17.266±4.703) points and (76.152±7.611) points, respectively.There were significant correlations among all variables(P < 0.05).Among the CHF patients, the direct effect of social support on self-care ability was significant(Z=3.138, P < 0.05), the total indirect effect was significant(Z=1.915, P < 0.05), the mediating pathway of fatigue was significant(Z=1.423, P < 0.05), and the chain mediating pathway between nutrition and fatigue was significant(Z=1.567, P < 0.05).ConclusionsMedical staff should pay more attention to the self-care ability of patients with CHF.Through early identification, monitoring of individual social support, effective assessment and management of fatigue level, and taking the improvement of fatigue as the starting point, the role of social support in improving self-care ability should be given full play to improve individual health.At the same time, based on the chain mediation effect, it is still necessary to pay attention to the positive impact of nutrition in the mechanism of social support on self-care ability.
Application value of the rehabilitation program based on mirror therapy in patients with hemodialysis after autologous arteriovenous fistula

ObjectiveTo investigate the effects of rehabilitation program based on mirror therapy on the quality of internal fistula and self-management ability of hemodialysis patients after observation autogenous arteriovenous fistula(AVF).MethodsThirty patients treated with AVF for the first time from December 2019 to September 2022 and 30 patients treated with AVF for the first time from January 2018 to November 2019 were divided into the observation group and control group, respectively.The control group and intervention group were guided by the routine rehabilitation exercise and rehabilitation exercise program based on mirror therapy for 2 months, respectively.The effects of upper limb mobility, internal fistula vessel diameter, anastomotic blood flow, first use time of internal fistula and self-management ability between two groups were compared before intervention, after 1 month and 2 months of intervention.ResultsAfter 1 and 2 months of intervention, the scores of Fugl-Meyer assessment upper extremity scale (FMA-UE), internal fistula vessel diameter, anastomotic blood flow and AVF self-management ability in the observation group were higher than those in control group(P < 0.05).There were statistically significant in all indexes between two groups before and after intervention(P < 0.01).With the passage of intervention time, the scores of FMA-UE, internal fistula vessel diameter, anastomotic blood flow and AVF self-management ability showed an increasing trend, which after 1 month and 2 months of intervention were higher than those before intervention(P < 0.05 to P < 0.01), and which after 2 months of intervention was higher than those after 1 month of intervention(P < 0.05 to P < 0.01).The maturation time of fistula in the observation group was significantly lower than that that in control group(P < 0.01).ConclusionsThe rehabilitation program based on mirror therapy can significantly improve the mobility of the upper limbs in the operative side, increase the blood vessel diameter and anastomotic blood flow, promote the maturity of internal fistula and improve the self-management ability of the patients with AVF.

ObjectiveTo explore the effect of spousal synchronous empowerment education based on binary coping model on self-management and blood glucose control in patients with diabetes mellitus.MethodsA total of 120 diabetic patients were selected as the research objects and randomly divided into a control group(n=60) and an intervention group(n=60).Patients of the control group were subject only to empowerment education model and that of the intervention group received the binary coping model for spousal synchronous empowerment education.The data of all subjects in the two groups, such as fasting blood glucose (FBG), 2-hour postprandial plasms glucose (2hPBG), glycosylated hemoglobin (HbA1c), self-management behavior scale scores and total scores, were collected at the baseline and 3 and 6 months after the intervention.ResultsThere were no significant difference in FBG, 2hPBG, HbA1c and self-management behaviors and total scores between the two groups of patients at the baseline (P>0.05);the data of FBG, 2hPBG and HbA1c of the intervention group at 3 months and 6 months after intervention were better than that of the control group(P < 0.05 to P < 0.01).FBG, 2hPBG, and HbA1c showed a decreasing trend with time in both groups before and after the intervention (P < 0.05 to P < 0.01).The self-management behavior scale scores and total scores in the intervention group were higher than in the control group after the intervention (P < 0.01).Scores of all indicators in the two groups after the intervention were higher than before the intervention (P < 0.05 to P < 0.01).ConclusionsSpousal synchronous empowerment education based on the binary coping model can effectively improve the ability of self-management and glucose control, which should be advocated for application in nursing health education.
Study on the mediating role of psychological resilience between frailty and quality of life in elderly patients with maintenance hemodialysis

Abstract:
ObjectiveTo understand the psychological resilience, weakness and quality of life of elderly patients with maintenance hemodialysis(MHD), analyze the correlations among three, and study the mediating role of psychological resilience between frailty and quality of life in elderly patients with MHD.MethodsA cross-sectional study was conducted in 50 patients with MHD aged ≥60 years in the department of nephrology.The general data questionnaire, fateful scale, quality of life scale and mental resilience scale were used for measurement and investigation, and the statistical analysis and mediation effect test were conducted.ResultsThe frailty score, psychological resilience score and quality of life score in elderly patients with MHD were (6.28±2.53)(22.05±6.86) and (53.55±17.24) points, respectively.The results of correlation analysis showed that the frailty was negatively correlated with the life quality and psychological resilience(P < 0.01), and the psychological resiliencewas positively correlated with life quality(P < 0.01).The results of mediation effect test showed that the psychological resilience partially mediated the relationship between frailty and quality of life, and the mediation effect accounted for 27.0% of the total effect.ConclusionsAmong the elderly patients with MHD, the frailty is significantly negative correlation with the life quality and psychological resilience, the psychological resilience is significantly postive correlation with life quality, and the psychological resilience partially mediates the relationship between frailty and quality of life.
