Changes of insulin sensitivity in obese rats and the effects of glucocorticoid

Abstract:
Objective: To observe the changes of insulin sensitivity in high-fat-induced obese rats and the effects of glucocorticoid.Methods: The adult rats were fed with high fat food for 80 days and the obese ones were selected.The fasting blood glucose,the fasting insulin and the degree of insulin resistance of the obese rats were compared with that of the normal ones.Both the obese rats(n=40) and the normal rats(n=38) were intraperitoneally injected normal saline and different doses of hydrocortisone succinate,and the serum levels of fasting blood glucose,fasting insulin and the degree of insulin resistance were observed.Results: The obeses rats had higher serum levels of fasting blood glucose,fasting insulin and insulin resistance than the normal ones(P<0.001).Compared with the normal saline group,the serum levels of fasting blood glucose,fasting insulin and the degree of insulin resistance were increased in the hydrocortisone succinate group(P<0.01).Conclusions: Obesity may cause hyperinsulinemia and insulin resistance,and glucocorticoid can induce insulin resistance.
Detection of Staphylococcus aureus toxigenes and its clinical application

Abstract:
Objective: To study genic feature of Staphylococcal enterotoxins(SEs),exfoliative toxins(ETs),toxic shock syndrome toxin 1(TSST-1) and Panton-Valentine leukociclin(PVL) carried by both hospital-acquired Staphylococcus aureus(HA-SA) and community-acquired Staphylococcus aureus(CA-SA).Methods: The genes of SEA-SEE,ETA,ETB,TSST-1 and PVL were detected by PCR.Results: Out of 116 SA strains,85 strains were HA-SA.SEA was detected in 6 strains,SEC in 1 strain,SED in 1 strain,and PVL in 2 strains.Thirty one strains were CA-SA.SEA was detected in 1 strain,SEB in 1 strain,SED in 1 strain,and PVL in 7 strains.SEE,ETA,ETB and TSST-1 were negative in all SA.PVL positive rate detected in CA-SA was much higher than that in HA-SA(P<0.005),while positive rates of SEs,ETs,TSST-1 were not significant different between CA-SA and HA-SA(P>0.05).Conclusions: Staphylococcus aureus may secrete various toxins,detecting should pay attention to whose toxins while separating Staphylococcus aureus.
Expression of TRAIL receptors in human endometrial carcinoma cell line

Abstract:
Objective: To investigate the expression of death receptors(DR4,DR5) and decoy receptors(DcR1,DcR2) of TNF-related apoptosis inducing ligand(TRAIL) in normal endometrium tissues,carcinoma endometrium tissues and the adjacent tissues.Methods: Reverse transcriptase polymerase chain reaction(RT-PCR) was used to detect mRNA positive expressions of the receptors of TRAIL in 20 cases of normal endometrium tissues,20 cases of carcinoma endometrium tissues and the adjacent tissues.Results: DR4 and DR5 were expressed in normal endometrium tissues,carcinoma endometrium tissues and the adjacent tissues.DcR1 expressed differently in the tissues of endometrium,but with no statistical significance(P>0.05);the expression of DcR2 was much lower in the carcinoma endometrium tissues than in normal or tumor adjacent tissues(P<0.005);the expression of DcR2 was similar in normal and tumor adjacent tissues(P>0.05).Conclusions: The low expression of DcR2 in the carcinoma of endometrium may be concerned with its pathogenesis,which might play a role in preventing endometrium from carcinomatous change.
Influence of arsenic trioxide on breast cancer cell and telomerase activity

Abstract:
Objective: To investigate the inhibitary effect of different concentrations of arsenic trioxide(As2O3) on breast cancer cells MDA-MB-231 and on the activity of telomerase hTERT-mRNA so as to provide theoretical and experimental basis for the treatment of breast cancer.Methods: MDA-MB-231 cells were cultured with As2O3 diflerint cokcenfrations.The growth inhibition was analyzed by MTT assay;the telomerase activity was determined by TRAP-PAGE-SILVER staining;the hTERT-mRNA expression was examined by RT-PCR assay.Results: As2O3 could obviously inhibit the growth of MDA-MB-231 cells,which was related with the concentrations of As2O3 and time of culture.Its 24-hour inhibitory concentration(IC50) was 24.73 μmol/L and 48-hour inhibitory concentration(IC50) was 6.99 μmol/L.Treated with As2O3(10-40 μmol/L) for 24-48 h,the telomerase activity and hTERT-mRNA expression of MDA-MB-231 cells were greatly inhibited.Conclusions: As2O3 can inhibit the growth of MDA-MB-231 cells significantly.Treated with As2O3,the activity of telomerase and telomerase hTERT-mRNA can be inhibited remarkably,and the effects are dependent on the concentrations and time of culture.
Primary culture and identification of neurons from cerebral cortex of newborn rats

Abstract:
Objective: To establish an ideal method for primary culture of neurons from cerebral cortex of neonate rats.Methods: The cerebral cortex of newborn rats was digested with trypsin to obtain single cell suspension,after the cell viability was estimated by trypan blue staining,the cells were planted;when the cells were cultured for 3-4 d,Ara-c was added to purify the neurons;the cultured cells were finally,identified by toluidine blue stain for Nissl bodies.Results: The survival rate of the conducted cells was high;under the culture condition in vitro,the cells grew well.When the cells grew for 6-7 d,the cell body was big,the prominences were thick and there were many branches;the prominences interlaced to net,forming typical nerve fiber net.When the cells grew for 10-14 d,cells with Nissl bodies were above 90%.Conclusions: It is an ideal technique for culture of cortical neurons from cerebral cortex of neonate rats in vitro.
Microanatomy observation of sigmoid sinus in temporal bone

Abstract:
Objective: To study the relationship between sigmoid sinus and its adjacent structure in order to provide an anatomic basis for otochirurgia.Methods: The distance between sigmoid sinus and its adjacent structure was measured respectively based on the simulation of transmastoid approach.Results: The distance between the precingular plale of sigmoid sinus and the external auditory superameateal spine was(13.28±1.87) mm.The distance between sigmoid sinus and the surface of mastoid was(5.16±0.73) mm.The distance between sigmoid sinus and external auditory superameatal spine was(17.85±1.62) mm.The distance between sigmoid sinus inferior genu and external auditory superameatal spine was(19.68±2.11) mm.The distance between sigmoid sinus and the short process of incus was(17.37±1.54) mm.The distance between sigmoid sinus and the horizontal semicircular canal was(14.61±1.37) mm.Conclusions: It has guidance significance to measure the distance between sigmoid and the surface of mastoid,external auditory superameatal spine,and the other adjacent structure for the transmastoid approach.
The survivin expression in brain glioma tissues and effection of survivin antisense oligodexynucleotides on U251 cells

Abstract:
Objective: To observe the expression of survivin in brain glioma tissues and effects of survivin antisense oligodexynucleodides(ASODN) on the proliferation and apoptosis of U251 cells.Methods: RT-PCR method was performed for detecting survivin mRNA expression in specimens.The glioma cells U251 transferred with survivin ASODN for 48 h,the proliferation and apoptosis were measured by flow cytometry(FCM) and cell morphology.Results: Survivin mRNA was expressed in brain glioma tissues,and the positive rate was 76.7%.When U251 cells were treated with different concentration of ASODN at 100 nmol/L,300 nmol/L,and 500 nmol/L,the apotosis rates were (19.46±3.85)%,(38.60±1.85)%,and(55.34±3.16)%,respectively;and the proliferation index(PI) were(52.24±6.59)%,(36.14±4.01)%,and(24.14±5.57)%,respectively,the difference was distinct compared with control and groups of different concentration(P<0.05-P<0.01).Conclusions: Survivin mRNA was expressed in the brain glioma tissues,survivin ASODN efficiently inhibits the proliferation and induce apotosis of U251 cells in a dose-dependent manner.
Clinical significance of plasma protein Z level in patients with taking orally warfarin

Abstract:
Objective: To observe the alteration of protein Z(PZ) in patients with taking orally warfarin.Research the reason of this alteration and the monitor function of the PZ in pauients with taking orally warfarin.Methods: Thirty patients with taking orally warfarin and 23 healthy adults as the control group,and detect the level of the plasma PZ and prothrombin time(PT),PT:INR.PZ was measured by ELISA and PT,PT:INR were measured by stago auto-analyor.Results: In taking orally warfarin group PZ levels was(240.87±192.72) ng/ml.It compared with health group's PZ level(1 812.6±347.16) ng/ml has dropped(P<0.001).Taking orally warfarin group's PT(24.18±4.52) s compared with health group's PT(12.67±0.42) s has raised(P<0.001).Taking orally warfarin group's PT:INR(2.448±0.63) compared with health group's PT:INR(0.943±0.04) has raised(P<0.001).Conclusions: PZ level of the patients who take orally warfarin is obrions drop.Fingdings shows that taking orally warfarin could make the PZ drop.The plasma protein Z level can be used as an indicator to monitor the amonnt of warfarin.
Pharmacokinetics and bioequivalent of azithromycin dispersible tablets in healthy volunteers

Abstract:
Objective: To evaluate the bioequivalence of two domestic preparations of azithromycin in healthy volunteers.Methods: A single oral dose of 500 mg test or reference formulations were given to 20 male healthy volunteers in a randomized cross-over design.The serum concentration of azithromycin was determined by liquid chromatography-tandem mass spectrometry method.Pharmacokinetic parameters and relative bioavailability were calculated by DAS program.Results: Pharmacokinetic parameters of test and reference were not statistically different(P>0.05).The relative bioavailability of test was(119.6±52.9)%.Conclusions: The test and reference formulations were bioequivalent.

Abstract:
Objective: To explore the curative curative effects of regimens containing levofloxacin with varied courses on coal workers'pneumoconiosis-tuberculosis.Methods: The curative effects were compared between 51 patients receiving the treatment scheme of 6SHRZV/12HRE and 50 patients receiving the treatment scheme of 3SHRZV/9HRE.The comparision included the sputum negative conversion rates of tuberculosis-bacillus at the end of the 6-month treatment and the whole treatment,as welll as the chest X-ray improvement after the course of treatment.Results: There was significant difference between the two groups(the treatment scheme of 6SHRZV/12HRE and 3SHRZV/9HRE) in the sputum negative conversion rates of tuberculosis-bacillus(92.2%,78%,respectively) at the end of the whole treatment(P<0.05).There was significant difference between the two groups in the chest x-ray improvement after the course of treatment(P<0.05).Conclusions: According to the evaluation for the curative effects of regimens including levofloxacin,the treatment scheme of 6SHRZV/12HRE is appropriate to coal workers'pneumoconiosis-tuberculosis.

Abstract:
Objective: To explore the clincopathological features of acquired immunodeficiency syndrome(AIDS) cases in North Anhui Province.Methods: The clinical and pathological material of 2 cases were analyzed retrospectively and the associated literature was reviewed.Results: The HIV infection was confirmed by Anhui CDC.One case featured continuous fever,wasting,lymphadenopathy and herpes zoster,and the other case manifested respiratory symptoms.Conclusions: In the area of high incidence of HIV infection,the clinical and pathological doctors should be familiar with the presentations and diagnostic criteria of AIDS,and when meeting with patients with diseases of skin,mucosa,lymph nodes and respiratory tract with unknown causes AIDS should be considered and further examinations made.
Transurethral electro resection for prostatic hyperplasia with small volume: A report 30 cases

Abstract:
Objective: To investigate the curative effects of transurethral electro resection of the prostate plus bladder neck incision(TURP+BNI) for the treatment of benign prostatic hyperplasia with small volume.Methods: The data of endoscopic treatment relevant to small-size benign prostatic hyperplasia with small volume was analyzed and reported.Results: Thirty patients were followed-up for 6 to 38 months with a mean of 22 months.The success rate was 93.3%.No patients suffered from the serious complications.Conclusions: The transurethral resection of the prostate plus bladder neck incision is effective and safe to treat the benign prostatic hyperplasia with small volume.
Change of platelet parameters and its clinical significance in neonates with hypoxic-ischemic

Abstract:
Objective: To investigate the changes of platalet parameters and its clinical significance in neonates with hypoxic-ischemic encephalopathy(HIE).Methods: Forty-six neonates with HIE were included in the study.Their blood platelets count(BPC),mean platelet volume(MPV) and platelet distribution width(PDW) were detected at the acute stage(under three days after birth) and the recovery stage(seven days after birth).The data were compared with those from 35 healthy controls born whthin 3 days.The 46 cases of HIE were then divided into three growps according to the severity of their disease,and the data from these three groups were compared.Results: At the actue stage,BPC in the neonates with HIE was obviously lower than that in the control group,and MPV and PDW were obviously higher.There was remarkable difference(P<0.001).At the recovery stage,BPC in HIE group increased obviously to the level of control group(P<0.001),and MPV and PDW descended to the level of control group(P<0.001).BPC in the HIE group reduced along with the aggravation of the condition gradually,while MPV and PDW increased.Conclusions: The change of platecet parameter is of great value in judging HIE and its severity.
Clinical application of recombinant human epidermal growth factor in repair of cleft palate

Abstract:
Objective: To observe the effect of recombinant human epidermal growth factor in repair of cleft palate.Methods: Sixty patients were divided into two groups,one with 30 patients using recombinant human epidermal growth factor as experimental group and the other 30 patients as control group.All the patients were observed on several aspects such as healing situation of wound surface,persistence time of ache,recovery of normal diet,the changes in temperature and the weight.Results: Compared with control group,experimental group had the shorter persistence time of red swelling of the raw surface and of ache(P<0.001),the faster recovery of normal diet and the lower of decrease in weigh(P<0.001).But the changes in temperature were not different in the two groups(P>0.05).Conclusions: The recombinant human epidermal growth factor can facilitate the wound healing.The application of recombinant human epidermal growth factor in palatoplasty is beneficial to the patients.
Clinical analysis of 16 cases of placenta accreta

Abstract:
Objective: To discuss the etiology,diagnosis and treatment of placenta accreta.Methods: Sixteen placenta accreta cases were analyzed retrospectively and the relative literature was reviewed.Seven of the sixteen cases received emergency hysterectomy,four cases were treated with sature and compression hemostasis,one case was performed intrauterine pack with gauze for hemostasis,three cases received uterine artery perfusion plus embolism treatment and one case failed in conservative treatment.Results: Of the 4802 delivery cases,placenta accreta occurred in 16 cases,with an incidence rate of 1:300.Among the 16 cases,placenta previa was observed in 10 cases and previous cesarean delivery in 4 cases;4 cases were found to have more than 3 times of induced abortion and 11 cases had 2 times of induced abortion.No one died.Conclusions: The related risk factors include placenta previa,previous cesarean delivery and multiple induced abortion.Early prenatal diagnosis of placenta accreta is important for the prognosis of the disease.Emergency hysterectomy is usually inevitable for cases with serious hemorrhage;conservative treatment is usually adopted to cases of non-emergency and women with the desire of procreation.

Abstract:
Objective: To investigate the causes of sudden cardiac death(SCD) in the teenagers and to provide effective preventive measures.Methods: The autopsy pathological materials of two cases of SCD were observed and analyzed.Results: One case suffered from dilated cardiomyopathy and the other had myocarditis complicated with congenital heart disease(atrial septal defect).Conclusions: Transient and dangerous SCD is scarce in the youth,but tends to increas in recent years,which should arouse our attention.And early treatment of the primary disease is proposed.
Expression and its significance of transcription factor Ets-1 in esophageal squamous cell carcinomas

Abstract:
Objective: To analyze the expression and its significance of Ets-1(E26 transformation-specific) in esophageal squamous cell carcinomas(ESCC).Methods: Tissue samples were collected from 68 patients who underwent surgery in our hospital from 1999 to 2005.The expression of Ets-1 in these samples by immunohistochemistry was detected.Results: Ets-1 protein expressed in esophageal carcinoma tissues is significantly higher than in pericancerous tissues(P<0.005).The expression of Ets-1 is positively correlated with the pathology grading,lymph node metastasis,and TNM stage of ESCC(P<0.05).The pathological type were no association with the Ets-1 protein expression positive rates in ESCC.Conclusions: The results indicated that Ets-1 was over expressed in ESCC tissues and correlated with its pathology grading,lymph node metastasis and TNM stage.

Abstract:
Objective: To explore the diagnosis and treatment value of visual evoked potential(VEP) for patients with optic nerve contusion following ocular damage.Methods: VEP detection was performed in 36 patients with optic nerve contusion,and the result was compared with that of the contralateral healthy eyes and the normal control group.Results: VEP was determined in 40 eyes of 36 patients.Simple decrease of amplitude was found in 30 eyes of 28 patients and decrease of amplitude accompanied by the prolongation of latency in 8 eyes of 7 patients.There was significant difference in amplitudes of P100 among the damaged eyes,the contralateral healthy eyes and the normal group(P<0.001 and P<0.01).But the prolongation of latency of P100 was found in 7 patients.The difference was significant as compared with the healthy eyes(P<0.001),but no significant difference was noted in the latency of P100 as compared with the normal control group(P>0.05).Conclusions: VEP is sensitive in the detection of optic nerve contusion following ocular damage.It is of value in early diagnosis and prognosis of optic nerve contusion following ocular damage.
Intensive insulin treatment of newly diagnosed type 2 diabetes mellitus

Abstract:
Objective: To evaluate the effect of short-term intensive insulin treatment on newly-diagnosed type 2 diabetes millitus.Methods: Forty-one newly-diagnosed type 2 diabetes millitus were randomly divided into group A and group B.Group A were given regular oral hypoglycemic,and group B received insulin intensive treatment.Fasting plasma glucose(FPG),2 hour postprandial glucose(PG2h),fructosamine(FA),triglyceride(TG) and total cholesterol(TC) were observed and compared 2 and 4 weeks before and after treatment.Results: After short-term intensive insulin treatment,the FPG,PG2h,FA of group B reduced significantly,compared with that of group A.The difference was significant(P<0.05).Conclusions: Intensive insulin treatment can effectively control glucose,TG and TC in newly diagnosed type 2 diabetes millitus.
Analysis of clinical manifestation and MRI of 42 patients with multiple sclerosis

Abstract:
Objective: To summarize the clinical and MRI characteristics of multiple sclerosis(MS) and to improve the diagnosis of the disease.Methods: The clinical manifestation and MRI appearance of 42 cases of MS were analyzed retrospectively.Results: MS was commonly seen in young and middle aged females and the onsets were acute or subacute.Twenty-six of the cases had evoking factors before onset,which were mainly relevant to infections.Weakness and visual deterioration were the most common initial symptoms.Thirty-two of the cases had relapses.The lesion was observed in the spinal cord in 29 cases,in the optic nerve in 28 cases,in the cerebral hemisphere in 14 cases,in the brain stem in 8 cases and in the cerebellum in 4 cases.Fourteen cases had paroxysmal symptoms.As shown by MRI,the intra-brain focus was mainly located around the lateral ventricles and corpus callosum.Oval lesion was the most common form.Most of the lesions were closely adjacent to the lateral ventricle or their long-axis was perpendicular with the tangent line of the lateral ventricle.Intra-spinal foci were in the cervical and thoracic cord,which were long-strip or multiple noncontinuous fusiform,and located posteriolaterally in the spinal cord.After treatment with corticosteroids,most of the focal lesions were reduced or disappeared.Conclusions: Diagnosis of MS could be improved by combining the clinical manifestation with the MRI of brain and spinal cord.
Clinical analysis of 74 elderly patients with severe bradyarrhythmia

Abstract:
Objective: To study the clinical features of severe bradyarrhythmias in elderly patients.Methods: The clinical data,electrocardiogram,static and dynamic electrocardiogram,classification of arrhythmia and therapy of 74 elderly patients with severe bradyarrhythmias were retrospectively analyzed.Results: Of the 74 cases,39 cases presented sick sinus syndrom,26 cases(35.14%) high degree atrioventricular block and 9 cases atrial fibrillation with long RR interval.Syncope occurred in 32 cases,near syncope in 17 cases and dizziness in 15 cases.Thirty-five cases were observed >3 seconds long RR intervals by dynamic and static electrocardiogram and 31 cases had a heart rate of <40 times/minute on daytime.Parmanent cardiac pacing therapy was applied to all the cases and some patients received antiarrhythmial therapy.Conclusions: Severe bradyarrhythmias usually leads to severe symptoms in elderly patients.Static and dynamic electrocardiogram is of great value in distinguishing the cause of syncope and obtaining the change of heart rate and rhythm.Parmanent cardiac pacing combined with antiarrhythmial therapy is efficient in treatment of bradycardia with tachyarrhyarrhythmias.

Objective: To explore the relationship between cerebral infarction acompanied by type 2 diabetes mellitus and carotid atherosclerosis.Methods: The intima-media thickness of the carotid artery,the area and quality of carotid plaque and the level of stenosis of artery were measured in 37 cerebral infarction patients combined with type 2 diabetes mellitus using carotid artery ultrasound.Results: Cerebral infarction combined with type 2 diabetes mellitus was obviously correlated with carotid atherosclerosis plaques.Atherosclerosis plaques was found in 32 cases in the diabetes group,and only 19 cases in the contral group;and the plaques rarely caused severe stenosis of carotid artery(P<0.005).The infarction position was obviously homolateral to the carotid atherosclerosis.As the time of diabetes meuitus prolonged,the appearance of carotid atherosclerosis plaques increased,which might be related with blood glucose,blood lipids and fibrinogen(P<0.05-P<0.001).Conclusions: Most patients with cerebral infarction combined with type 2 diabetes mellitus had carotid atherosclerosis plaques.It is of clinical value to prevent cerebral vascular disease in patients with diabetes mellitus and provide proper treatment for patients with cerebral infarction complicated with type 2 diabetes mellitus.

Abstract:
Objective: To explore a new method to repair large cicatricial alopecia and summarize the methods to reduce the common complications.Methods: Nine patients with large area of cicatricial alopecia were treated by axial scalp flap expansion.Results: All the 9 patients were satisfied with the defect repair.None of important complications occurred.Conclusions: With careful plan and meticulous strategies against to complications,axial scalp flap expansion is an ideal method for reconstruction of the large cicatricial alopecia.

Abstract:
Objective: To summarize the learning from experience of diagnosis and treatment of bronchopulmonary cyst.Methods: The clinical data of diagnosis and treatment of 20 patients with bronchopulmonary cyst were retrospectively analyzed.The single pulmonary lobectomy was performed in 14,double pulmonary lobectomy in 2,the right middle plus upper or lower lobe pneumonletomy in 1 respectively,the left lung resection in 2 patients,the tongue section plus the left lower lobe resection in 1 patient,and excision of.Results: There was no surgical mortality in this group.The postoperative chest drainage was 100 to 150 ml every day in 3.20 ml of 1% lidocaine and erythromycin 1.0 g were injected into the chest cavity once on the 7th or 8th day to reduce pleural effusion.The drainage was removed 10 to 12 days after operation.No other postoperative complications occurred.All of patients were cured and discharged from hospital,and were followed up for 6 months to 3 years.The patients recovered well and no recurrence occurred.Conclusions: The surgery is the preferred treatment for bronchopulmonary cyst,because it can confirm the diagnosis and avoids misdiagnosis.

Abstract:
Objective: To study the clinical effects and side effects of low dose of heparin and Vitamin K1 intervention on serious bronchopneumonia in infants.Methods: Forty-two infants were randomly divided into therapeutic group and control group.In addition to the routine therapy,the theraneutycgroup was administered heparin 0.3-0.5 mg/kg,vitamin K1 0.3-0.5 mg/kg twice a day for three days,then once a day for two days;the control group received only the routine therapy.The curative effects and side reactions were observed.Results: The total effective rates of theraneutycgroup and control group were 95.0% and 86.4%,respectively.The theraneutycgroup was obviously superior to the control group(P<0.05),and no obvious side reactions were observed.Conclusions: Treatment of serious bronchopneumonia in infants with the aid of low dose of Hepair and Vitamin K1 is effective and presents little side reaction.It is worth popularizing.

Abstract:
Objective: To explore the changes of serum COX-2,IL-8 and IgE levels in asthmatic children and their clinical significance.Methods: The serum COX-2,IL-8 and IgE levels of 121 asthmatic children,including 62 cases in the acute attack group and 59 cases in the remission group,and 60 healthy controls were detected by sandwich ELISA.Results: The serum COX-2,IL-8 and IgE levels in the asthmatic groups increased(P<0.05).The serum COX-2,IL-8 and IgE levels in the acute attack group were higher than those in the remission group(P<0.01);the serum COX-2,IL-8 and IgE levels in both acute attack group and remission group were higher than those in control group(P<0.01).Conclusions: COX-2,IL-8 may involve in the pathologeic process of asthmatic children,which may accelerate secretion of IgE from positive feedback regulation.Inflammatory reaction may still exist during the remission stage of asthma.The imbalance of cytokine network may probably be important reasons as a molecular basis in the asthma pathogenesis.The therapy of anti-COX-2 or cytokine network may relieve the condition of the asthma children.

Abstract:
Objective: To evaluate the safety and efficacy of transurethral plasmakinetic resection of prostate(PKRP).Methods: The data of benign prostatic hyperplasia(BPH) patients with PKRP from February 2006 to March 2007 were reviewed.The weight of the prostate was between 30 g and 140 g.Results: The operation lasted for 30 to 180 min.The resected tissues weighed 22-102 g.During the operation,no transurethral prostatic resection syndrome occurred.All patients were followed up for 2-12 months.Average maximum flow-rate increasedfrom 6.2 ml/s preoperatively to 18.9 m/s postoperatively.International prostatic symptom score(IPSS) decreased from average 26.1 preoperatively to average 7.3 postoperatively.Conclusions: PKRP is effective with high safety and less complications.

Abstract:
Objective: To study CT characteristic appearances of benign and malignant thyroid nodules,and evaluate the value of CT in differential diagnosis of thyroid nodules.Methods: Thirteen patients with benign thyroid nodule and fourteen patients with malignant thyroid nodules proved by operation and pathology were retrospectively analyzed.All patients were examined by CT plain scan.Among them,13 patients with thyroid carcinoma and 9 patients with benign thyroid nodules underwent thin slice enhanced CT scanning.Results: Thirteen of the 14 patients with thyroid cancer were correctly diagnosed by thyroid CT,and the diagnostic accuracy of CT was 92.9%.In 13 patients with thyroid carcinoma,vague contour of malignant nodules and discontinuing contour of the lesion thyroid outline and vague fatty space around thyroid were shown.Trachea of 4 patients with thyroid carcinoma and carotid sheath of 6 patients and tracheo-asophageal colpus of 4 patients and neck muscles and connective tissue of 9 cases were invaded by malignant lesions.The ipsilateral enlarged lymph nodules were found in 7 patients with thyroid carcinoma.The CT diagnostic accuracy of benign thyroid nodules was 84.6%.In 12 patients with benign thyroid nodule,clear contour and intact thyroid outline were shown and clear fatty space could also be seen.The total CT diagnostic accuracy of all 27 patients with benign and malignant thyroid nodules was 88.9%.Conclusions: Thin slice enhanced CT scan can play an important role in differential diagnosis of thyroid nodules.

Abstract:
Objective: To investigate treatment and therapeutic effect of CT guided percutaneous puncture and interventional therapy in renal and heptic cysts.Methods: Thirty-three heptic cysts in 30 patients and 48 renal cysts in 45 patients were treated with CT guided percutaneous puncture and interventional therapy.The amount of anhydrous alcohool was 25% of apirated amount of cysts.Gauge 18-21 aspiration needles were used for all patients.All patients were followed-up by CT or ulthasonograpy for three mouths to one year.Results: Twenty-one heptic cysts disappeared;9 heptic cysts were reduced in size and there was no change in 3 cysts.The effective rate was 90.9% and the rate of complete regression was 63.6%.Thirty renal cysts disapeared;12 renal cysts were reduced,and there was no change in 6 cysts.The effective rate was 87.5% and the rate of the complete regression was 62.5%.There were no severe complications.Conclusions: CT guided percutaneous cysts puncture and interventional therapy is a useful and safe procedure for the treatment of renal cysts and heptic cysts.

Abstract:
Objective: To study the characteristics of antibiotic susceptibility of Acinetobacter baumannii so as to provide basis for clinical application.Methods: The distribution of One hundred thirty-five stains Acinetobacter baumannii isolated from clinic and their drug resistance were analyzed retrospectively.Results: The incidence of Acinetobacter baumannii was the highest in the intensive care units(48.1%),and the department of orthopaedics(14.8%).Respiratory tract and surgical wounds exhibited as the major infected sites,and the incidences were 59.3% and 28.9%,respectively.The rate of resistance to imipenem,piperacillin/tazobac,ceftazidime,Amikacin,Gentamicin,ampicillin,nitrofuranto and cefazolin was 32.6%,44.4%,66.7%,69.6%,83.7%,90.4%,96.3% and 100.0%,respectively.They were also resistant to other antibiotics to certain degree,126 stains showed multiple-resistance.Conclusions: Acinetobacter baumannii is the major opportunistic pathogenic bacteria in hospital infection which shows high rate of resistance to routine antimicrobial agents and even multiple drug resistance with important clinical impact.Antimicrobial agents should be used rationally to decrease multi-drug resistance.

Objective: To investigate the resistance of extended spectrum β-lactamase(ESBLs) producing Escherichia coli to aminoglycoside antibiotics and genotyping of aminoglycoside inactive enzymes.Methods: Kirby-Bauer(K-B) method was employed to detect the resistance of 38 strains of ESBLs producing Escherichia coli to 4 aminoglycoside antibiotics(amikacin netihmicin,gentamicin and tobramycin).And 6 kinds of aminoglycoside inactive enzymes[saac(3) -Ⅰ,aac(3) -Ⅱ,aac(6') -Ⅱ,aac(6') -Ⅰb-cr,ant(2'') -Ⅰ and ant(3'') -Ⅰ] were determined by polymerase chain reaction(PCR).Results: The resistance rates of the 38 ESBLs producing Escherichia coil to the 4 kinds of aminoglycoside antibiotics were 28.9%,39.5%,76.3% and 76.3% respectively;the positive rates of aminoglycoside inactive enzyme genes of aac(3) -Ⅱ,aac(6') -Ib-cr,ant(3'') -Ⅰ and ant(2'') -Ⅰ were 63.2%,36.8%,10.5% and 2.6%,respectively;aac(3) -Ⅰ,aac(6') -Ⅱ were not found in these strains.Conclusions: The aminoglycoside inactive enzyme genes producing Escherichia coil are predominantly aac(3) -Ⅱ and aac(6') -Ib-cr AMEs genes.The aminoglycoside resistance profiles are related to AMEs gene patterns.More attention should be paid to drug resistance mechanism in management of clinical infections.
The distribution of pathogens and their drug resistance in senile patients with lower respiratory tract infection: A clinical investigation

Abstract:
Objective: To investigate the distribution and drug resistance of bacteria causing lower respiratory tract infection in senile patients in department of respiratory diseases,and study the prevalence of the bacteria producing extended-spectrum β-lactamases(ESBLs) in Escherichia coli and Klebsiella pneumoniae.Methods: The specimen of sputum was tested by culture to inspect pathogens.Kirby-Bauer agar diffusion method was used to judge drug sensitivity.Phenotypic confirmatory test was used to detect ESBLS.Results: A total of 521 strains were obtained from specimen of 2 177 sputum.Among which 389 strains were Gram-negative bacilli,81 strains were Gram-positive coccus,51 strains were fungus.The predominant pathogens in Gram-negative bacilli were Acinetobacter baumanii and Klebsiella pneumoniae,and showed high resistance to common antibiotics.The total percentage of ESBLsproducing was 23.3%,in which Escherichia coli was 19.1%,and Klebsiella pneumoniae was 26.0%.Conclusions: The Gram-nagative bacilli was the major pathogens isolated from senile patients with lower respiratory tract infection in department of respiratory diseases,and it has markedly increased resistance to common antibiotics.The monitoring for pathogens and drug sensitivity should be enhanced to give clinicians suggestion for rational use of antibiotics.

Abstract:
Objective: To explore the nursing measures for patients with esophageal stenosis or esophageal fistula treated by esophageal stent implantation.Methods: Esophageal stent placement was performed on 23 cases of esophageal stenosis or esophagus fistula.Before operation,comprehensive and meticulous preparations were made;during operation,accurate and swift support was provided;after operation,the patients were rightly positioned and given proper instructions about diet,prevention and handling of complications and matters after being discharged.Results: The dysphagia of 11 patients with esophageal stenosis were obviously improved and could take food by mouth.The esophagus fistulas in other 11 cases were sealed up and they could take food without choking in the procedure.Conclusions: The operation of esophageal stent placement can eliminate the dysphagia of patients with esophageal carcinoma complicated with esophageal stenosis or fistula and improve their life quality.Proper nursing may decrease the complication and enhance the therapeutic efficacy.

Abstract:
Objective: To study the effect of comfortable nursing on cancer patients treated with chemotherapy.Methods: One hundred cancer patients treated with chemotherapy were randomly divided into two groups:Fifty-two in the observation group and forty-eight in the control group.The observation group recived comfortable nursing based on comfortable nursing model,while the control group rexceived only traditional nursing.The satisfaction in the two groups was surveyed by self-command questionnaire.Results: The satisfaction in the observation group was 96.2%,which was higher than that in the control group(81.3%) (P<0.05).The incidence of adverse reaction in the observation group was distinctly lower than that in the control group(P<0.05 to P<0.005).Conclusions: Comfortable nursing for cancer patients receiving chemotherapy may make the patients feel satisfactory and safe in physiology and psychology,and their life quality is greatly improved.
