A method for detection of monocyte/macrophage phagocytosis of fluorescent labeled Mycobacterium tuberculosis by flow cytometry

Abstract:
Objective: To establish a method for detection of monocyte/macrophage phagocytosis of fluorescent labeled Mycobacterium tuberculosis(Mtb) by flow cytometry.Methods: Mtb was labeled with fluorescein isothiocyanate(FITC) at different concentrations.The efficient labeled rate was detected by flow cytometry.Whole blood taken from healthy subjects were incubated with FITC labeled Mtb at 37℃ from 10 min to 120 min,followed by lysing red blood cells,washings,and then quenched the fluorescein of Mtb that not phagocytosed with trypan blue.The amount of monocytes with positive FITC were measured as phagocytosis rate by flow cytometry.FITC labeled Mtb and phorbol myristrate-differentiated THP-1 cells as macrophages were incubated from 1 h to 6 h with the ratio of 10:1,or incubated for 1 h and 2 h at the ratios from 1:1 to 100:1,and the phagocytosis rates were detected by flow cytometry as same method above.Results: The efficient labeled rate for Mtb was 92% when incubated with FITC at the concentration of 50 μg/ml.The percentages of monocyte phagocytosis of Mtb were increased from 34.68% to 79.90% in coculture time from 10 min to 120 min.The percentages of THP-1 macrophage phagocytosis of Mtb were increased from 30% to 81% for 1 h and 6 h of coculture.The THP-1 cell phagocytosis of Mtb were reached platform when FITC labeled Mtb incubated with THP-1 cells with at the ratio of 100:1 for 1 h(81%),or at the ratio of 50:1 for 2 h(82%).After quenched with trypan blue,the percentages of monocyte phagocytosis of Mtb decreased by 29% in 10 min and by 6% in 120 min,whereas the percentages of THP-1 cell phagocytosis of Mtb decreased by 1% in 1 h and 7% in 6 h.Conclusions: The application of flow cytometry to measure monocyte/macrophage phagocytosis of FITC labeled Mtb is simple,rapid,and reproducible method.
Isolating culture and characteristics of rat-marrow-derived endothelial progenitor cells

Abstract:
Objective: To investigate the isolating culture and characteristics of endothelial progenitor cells(EPCs) derived from rat bone marrow.Methods: The EPCs of rat bone marrow were cultured by second adherent amplification in vitro.The capacity of the cells' proliferation was observed daily.These cells were performed immunofluorescence staining to detect the expression of CD133,CD34,Flk1 and CD31.The capability of forming blood vessels was observed through three-dimensional cultured cells.Results: Cell colony-forming units(CFUs) appeared at the 4th day.At the center of CFUs,cells were round,but around the CFUs,cells were fusiform shaped.Up to the 10th day,cells covered the bottom of the bottle like cobblestones.The proto-passage EPCs expressed CD133,CD34 and Flk1.And the 2nd passage cells expressed CD34,CD31 and Flk1.The 2nd passage EPCs by three-dimensional culture manifested sprouted EPCs and formed tube-like structures in collagen.Conclusions: EPCs can be gained from bone marrow through proliferation of cells ex vivo and these cells has the capability of forming blood vessel.
Expression of DPC4 gene in esophageal carcinoma and its significance

Abstract:
Objective: To investigate the correlationship between the expressions of DPC4 and vascular endothelial growth factor(VEGF) in esophageal carcinoma and their clinicopathological behavior.Methods: The expressions of DPC4 and VEGF proteins were examined by immunohistochemical method in 174 cases of esophageal carcinoma tissues.Results: The positive rates of DPC4 in normal esophageal mucosa.and esophageal carcinoma were 80% and 40.8%,respectively(P<0.005).The expression of DPC4 was higher in adenocarcinomas than in squamous carcinomas(P<0.05).With the depression of differentiation,the positive rate of DPC4 descended,but was not significant(P>0.05).The DPC4 expression of the group with lymph node metastasis was lower than the group without that(P<0.005).But the positive rates of VEGF in normal esophageal mucosa.and esophageal carcinoma were 23.3% and 43.1%,respectively(P<0.005).With the depression of differentiation,the positive rate of VEGF ascend.The VEGF expression of the group with lymph node metastasis was higher than the group without that(P<0.05).The expression of DPC4 was nagtive correlated with the expression of VEGF(P<0.001).Conclusions: The low expression of DPC4 is an important factor of esophageal carcinoma occurrence;The low expression of DPC4 may accelerate creating blood vessels and lymph node metastasis of esophageal carcinoma.
Expression of claudin-1 and claudin-4 in endometrial carcinomas and its significance

Abstract:
Objective: To explore the relations between the expression of claudin-1 and claudin-4 in endomentrial carcinoma and the clinicopathologic parameters.Methods: The expressions of claudin-1 and claudin-4 were determined in 52 cases of endometrial carcinomas,24 cases of dysplasias and 19 cases of proliferative endometriums by S-P immunohistochemistry,and their correlations to the clinicopathologic parameters were analyzed.Results: The positive expression rates of claudin-1 in proliferative endometrium,endometrial dysplasia and endometrial carcinoma were 89.5%,66.7% and 63.5%,respectively,with a trend of decreasing.The difference was significant in the three groups(P<0.005).The positive expression rates of claudin-4 in the above groups were 36.8%,70.8% and 90.4% respectively,with a trend of increasing.There difference was significant in the three groups(P<0.005).The expression of claudin-1 had no correlation to the histological grade,pathologic stage,myometrial invasion,lymph node metastasis or histological type;while the expression of claudin-4 was related to the myometrial invasion,but had no relation to the above parameters.Conclusions: Hypoexpression of claudin-1 and overexpression of claudin-4 may be involved in the carcinogenesis and development of endometrial carcinoma,and may contribute to the diagnosis,treatment and prognosis of endometrial carcinoma.
Analysis of death causes of heart failure patients received cardiac resynchronization therapy

Abstract:
Objective: To explore the causes of death in patients with heart failure who have received cardiac resynchronization therapy(CRT).Methods: Fifty-three patients with NYHA class Ⅲ or Ⅳ heart failure who had failed to respond to standard pharmacologic therapy were treated by CRT.The routine medicine was applied after the operation.The follow-up lasted from 1 to 80 months,and the outcome was observed.Results: Seven patients died during the follow-up,5 of whom were cardiac death and 2 non-cardiac death.Cardiac death was mainly induced by deterioration of heart function and cardiac arrhythmia.Deterioration of heart function was related to primary heart disease and occurrence of atrial fibrillation.Conclusions: Causes of death in patients with CRT are varied and cardiac death is associated with the primary heart disease and the occurrence of atrial fibrillation.
Diagnosis and treatment of pulmonary sequestration: Analysis of 10 cases

Abstract:
Objective: To explore the diagnosis and surgical treatment of pulmonary sequestration.Methods: The clinical data of 10 cases of pulmonary sequestration were retrospectively studied.Results: The preoperative misdiagnosis was 100% and all the 10 cases were identified by exploration in operation.No hemorrhage or other complications occurred and all the cases were discharged after full recovery.Conclusions: Cognition to pulmonary sequestration should be elevated.The exploration operation shows that the features of pulmonary sequestration are adhesion and clausura of pectus,pink lung tissues with no locus niger and abnormal impulse of vasa which can be touched near the ligament of lower lobe.Correct diagnosis is key to the treatment of pulmonary sequestration.
Primary tracheobronchial amyloidosis: Report of 2 cases and review of literature

Abstract:
Objective: To study the epidemiology,manifestation,diagnosis and treatment of primary tracheobronchial and pulmonary amyloidosis.Methods: The clinical features of 2 cases of primary tracheobronchial amyloidosis were summarized,and the analysis was performed by reviewing the literature.Micowave,prednisone and alkeran were adopted in the treatment.Results: The most common symptoms of primary tracheobronchial and pulmonary amyloidosis were cough,sputum,dyspnea and emptysis.Biopsy's specimens showed that Congo red stain was all positive.One patient improved markedly and the other went to another hospital.Conclusions: Primary tracheobronchial and pulmonary amyloidosis is rarely seen in clinic and its diagnosis relies on pathology.No specific management has been found up till now.
Analysis of the circadian rhythm of hypertensive cerebral hemorrhage attacks

Abstract:
Objective: To investigate the circadian rhythm of the hypertensive cerebral hemorrhage attacks for better prevention of the disease.Methods: One hundred twenty nine patients hypertensive cerebral hemorrhage were classified on the attack time at 6-hour interval from 0:00 o'clock during the 24 hours of the day,and the occurrence of the attacks were analyzed based on the time segments. Results: The peak time of occurrence of the acute cerebral hemorrhage attacks was from 6 am to 12 o'clock at noon.Conclusions: The occurrence of cerebral hemorrhage attacks is obviously a circadian tendency,high risk period is on the morning.
Postoperative palinesthesia and analgesia effect of flurbiprofen axetil on patient receiving general anesthesia

Abstract:
Objective: To observe the postoperative palinesthesia and analgesia effect of flurbiprofen axetil on patients with general anesthesia.Methods: One hundred and twenty patients(ASAⅠ-Ⅱ) receiving general anesthesia were randomly divided into flurbiprof axetil group(group F),morphine group(group M),control group(group C).Before the abdominal membrane was closed up,group F were given intravenous injection of flurbiprofen axetil 50 mg,group M intravenous injection of morphine 4 mg and group C not any drugs.The time of palinesthesia,time of extubation,detention time in recovery room,postoperative palinesthesia,incidence rate of complication in eight hours,postoperative analgesia degree score and the number of patients needing to take pain-killing drugs because of pain in 2,4,6 and 8 h after operation were observed.Results: The time of palinesthesia,time of extubation and detention time in recovery room in group F were shorter than that in group M(P<0.01);there was no significant difference between group F and group C in the time of palinesthesia(P>0.05),but the defference was significant in time of extubation and detention time in recovery room(P<0.01);the incidence of complication was lowered in the time of palinesthesia;VAS score had no statistical difference at 2 h after operation among the 3 groups(P>0.05).The pain degree in group F and group M was signifantly lowered compared with that of group C(P<0.01),but the difference was not significant between group F and group M at 4 h,6 h and 8 h after operation(P>0.05).The number of patients needing to take pain-killing drugs in the three groups had no statistical difference at 2 h,4 h,6 h and 8 h after operation.Conclusions: Administration of flurbiprofen axetil before the end of operation has analgesia effect on patients receiving general anesthesia,and may decrease the incidence of complication at the time of palinesthesia without affecting the course of palinesthesia.
Value of endoscopy in diagnosis of hiatus hernia

Abstract:
Objective: To assess the value of endoscopy in diagnosing and differentiating hiatus hernia(HH).Methods: The endoscopic characters of HH of 112 cases were analyzed and the results of the X-ray barium meal and the occurrence of esophagitis were calculated.Results: The major endoscopic features of HH were as follows:the GE conjunction was moved up;a portion of stomach could be seen to slide up with the loose,enlarged hiatus of diaphragm and was companied by high morbility of esophagitis.Conclusions: It is direct,clear and convenient to diagnose HH with endoscope.
Analysis of antibiotic resistance of extended-spectrum β-lactamases-producing Escherichia coli strains

Abstract:
Objective: To study the resistance of the extended-spectrum β-lactamases(ESBLs) -producing Escherichia coli(E.coli) strains and to provide basis for clinical therapy.Methods: Agar dilution method was used to determine MICs of 13 antibiotics against 63 strains of ESBLs-producing E.coli which were collected between October 2005 and June 2006 in our hospital.Results: ESBLs-producing E.coli was generally sensitive to carbopenems and piperacillin/tazobatan,while most strains were highly resistant to piperacillin and quinolones.The rate of resistance to cefepime,ceforazone/sulbactam,ceftazidime,cefoxitin,ceftaxime and ceftriaxone were 34.92%,36.51%,44.45%,55.56%,82.54% and 84.13%,respectively.Conclusions: ESBLs-producing E.coli strains are highly resistant to antibiotics in our hospital.
Edaravone for treatment of cerebral hemorrhage: Report of 30 cases

Abstract:
Objective: To observe the clinical effect of edaravone on intracerebral hemorrhage(ICH).Methods: Sixty patients with ICH were randomly assigned to two groups.Thirty patients in the treatment group were given edaravone in addition to the routine therapy for 14 days and the other 30 patients were treated routinely.The degree of neurologic impairment before treatment and at 14th and 28th day after treatment was compared between the two groups by the National Institute of Health Stroke Scale(NIHSS).The prothrombin time(PT),activated partial thromboplastin time(APTT) and fibrinogen(FG) were recorded before and after edaravone therapy in the treatment group.Results: The NIHSS of the treatment group was obviously lower than that of the control group(P<0.01).The PT,APTT and FG had no significant change in the treatment group(P>0.05).The treatment group had no more incidence of adverse drug reaction than the control group(P>0.05).Conclusions: Edaravone is an efficacious and safe therapeutic agent for ICH.
Treatment of advanced non-small cell lung cancer with combination therapy of docetaxel and oxaliplatin: A report of 34 cases

Abstract:
Objective: To observe the effect and side effect of docetaxel combined with oxaliplatin in the treatment of non-small cell lung cancer(NSCLC).Methods: Thirty-four patients with advanced NSCLC(Karnofsky score ≥ 60,expectancy life span ≥ 3 months) were included in the sstudy.Docetaxel 75 mg/m2 was administrated in intravenous infusion at day 1 and oxaliplatin 130 mg/m2 was administrated at day 2.One cycle consisted of 21 days and the clinical response was assessed after two cycles.Results: Of the 34 patients,CR was achieved in 0,PR in 8,SD in 19 and PD in 7.The response rate was 23.5%.The main side effects were peripheral neuritis and inhibition of bone marrow.Conclusions: Combination therapy of docetaxel and oxaliplatin is effective on advanced NSCLC with acceptable toxicity and especially suitable for elderly and drug resistant patients.
Clinical observation of 125I interstitial brachytherapy for abdominal malignant tumor

Abstract:
Objective: To assess the clinical observation of radioactive 125I seeds interstitial brachytherapy for abdominal malignant tumors.Methods: Five patients with primary and metastatic liver cancer received 125I seeds interstitial brachytherapy under CT.Four cases of pancreatic cancer and 1 case of metastatic liver cancer were treated with 125I interstitial brachytherapy during laparotomy under outrasound guidance,and the follow-up lasted for 1-12 months.Results: The implanting was successfully in all the 10 cases.The pains were relieved obviously in 5 caes and no severe complications were observed.CT showed that the volume of the malignant tumors decreased to some extent during the 2-month follow-up,with an overall response rate of 90%.Conclusions: Radioactive seeds interstitial brachytherapy may relieve the symptoms of patients with abdominal malignant tumors and improve their life quality.
Microendoscopic tubular retractor system for lumbar intervertebral disc herniation: Report of 95 cases

Abstract:
Objective: To investigate the efficacy of microendoscopic tubular retractor system(METRx) in treatment of lumbar intervertebral disc herniation.Methods: Microendoscopic tubular retractor system was applied to excise the nucleus pulpous through the rear route.Results: All the cases were followed up for 4 months to 3 years.According to the Nakai scale,the outcome was excellent in 71 cases,good in 15 cases and fair in 9 cases.No symptom aggravation cases were observed.The fine rate was 90.5%.Conclusions: METRx has a satisfactory short-term efficacy in treatment of lumbar disc herniation.
Effect of positive end-expiratory pressure on central venous pressure of the patients undergoing mechanical ventilation

Abstract:
Objective: To observe the effect of positive end-expiratory pressure(PEEP) on central venous pressure(CVP) of patients undergoing mechanical ventilation.Methods: CVP of 10 patients with respiratory failure undergoing mechanical ventilation at different levels of PEEP(0~14 cmH2O) were monitored.Results: There were increases of CVP accompanied with PEEP increment,and there was a significant positive correlation between CVP and PEEP.Conclusions: PEEP and CVP is significantly correlated.
Clinical and pathological analysis of cellular leiomyoma arising from different parts of the uterus

Abstract:
Objective: To investigate the clinical features of cellular leiomyoma arising from different parts of the uterus and its prognosis.Methods: The cases of uterine leiomyoma treated in our hospital from 2000 to 2005 were reviewed retrospectively,and the incidence rate,clinical manifestation and age of onset of cellular leiomyoma arising from different parts of the uterus were analyzed.Results: Of the 1538 cases of cellular leiomyoma,the incidence of cellular leiomyoma arising from the intervallum accounted for 16.09%,from below the mucous membrane 30.69%,and from the subserosa 9.18%.The difference was significant(P<0.01﹚.The median onset-age of cellular leiomyoma was 41,while the median onset-age of common leiomyoma was 47.The prognosis of both groups had no significant difference during the follow-up.Conclusions: The cellular leiomyoma of uterus prone to arise from below the mucous membrane of the uterus and the prognosis is good.
Diagnosis and treatment of endometrial atypical proliferation: Analysis of 27 cases

Abstract:
Objective: To evaluate diagnosis and treatment methods of endometrial atypical proliferation.Methods: The clinical data of 28 cases of endometrial atypical proliferation were retrospectively reviewed.Results: Three cases received drug therapy,1 case was under treatment and 2 cases were proved to have benign lesion by diagnostic curettage.Seventeen cases received surgical therapy and no relapse was detected in the follow-up,excepting for 4 patients who were not in the city and were not followed up.Conclusions: Precancerous lesion and early endometrial carcinoma can be detected through mass screening,which will enable the patients to receive treatment in the early stage and avoid endometrial carcinoma.
Influence of remifentanil combined with propofol for controlled hypotension on gastric intramucosal pH

Abstract:
Objective: To discuss the influence of remifentanil combined with propofol for controlling hypotension on gastric intramucosal pH(pHi).Methods: Twenty patients undergoing elective vertebral fracture internal fixation,femoral shaft fracture internal fixation or hip replacement patients were randomly divided into two groups.The test group(n=10) received remifentanil and propofol for anesthesia and lowering blood pressure,and control group(n=10) received fentanyl and propofol for anesthesia and sodium nitroprusside(SNP) for lowering blood pressure.The MAP,SBP,DBP,HR,CVP,PgCO2,arterial blood gas and pHi at the time point T0(before the blood pressure lowering),T1(immediate relief and stability),T2(blood pressure dropping 10 min),T3(blood pressure dropping 30 min),T4(hypotension 1 h),T5(stopping blood pressure dropping 10 min) and T6(stopping blood pressure dropping 30 min) were determined and compared between the both groups.Results: In the test group,the value of PgCO2 at the time point T2,T3,T4 were obviously higher than that at T0 and T1(P<0.05);pHi at T3 and T4 were lower than that at T0 and T1(P<0.05).The value of PgCO2 increased from 46.5 mmHg(T0) to 70.8 mmHg(T4),and recovered at T6.In control group,the value of PgCO2 at T2,T3,T4 and T5 were higher than that at T0 and T1(P<0.05);the value of pHi at T2,T3,T4 and T5 were obviously lower than that at T0(P<0.01).The value of PgCO2 increased from 47 mmHg(T0) to 79 mmHg(T4),and recovered at T6.The value of PgCO2 at T2,T3 and T4 in the test group were obviously lower than that in control group(P<0.01),whereas the pHi at T3 and T5 were higher than that in control group(P<0.01).Conclusions: The transient gastrointestinal microcirculation disturbance can be found in controlling hypotension by remifentanil and propofol,but it is obvious smaller than by the traditional medicine SNP,and can be recovered in 10 min after stopping the medicine.
Transrectal closed repair and rectal submucosal injection of sclerosing agent for treatment of rectocele: Report of 20 cases

Abstract:
Objective: To explore the surgical efficacy of transrectal closed repair plus rectal submucosal injection of sclerosing agent on patients with rectocele.Methods: Twenty cases of rectocele were treated with combined therapy of transrectal repair and rectal submucosal injection of sclerosing agent.Results: Eighteen of the 20 cases were cured and 2 cases responded well.No remarkable sequelae were observed in the follow-up of 0.5-2.0 years.Conclusions: Combination therapy of transrectal closed repair and rectal submucosal injection of sclerosing agent is safe and effective for treatment of rectocele.
Enteral and parenteral nutrition support for criticall illness patients

Abstract:
Objective: To investigate the remedial effect of enteral nutrition(EN) on ICU patients.Methods: Fifty ICU patients were randomly assigned to EN group and parenteral nutritional(PN) group as control,with 25 patients in each.Both EN and PN groups were given preparations of the same amount of colorie and nitrogen.Before and 9 days after nutritional support,their serum albumin,serum forealbumin and hemoglobin levels,as well as the thickness of skin fold at musculus triceps brachii and muscular perimeter of the arm,were determined.Results: The serum forealbumin and serum albumin in the patients with EN support were markedly improved(P<0.01),compared with those in the patients with PN support(P<0.01).However,the hemoglobin and the thickness of skin fold at musculus triceps brachii and muscular perimeter of the arm were not statistically different in the two groups.Conclusions: Compared with PN support,EN support might significantly ameliorate the nutritional state of ICU patients.
Clinical analysis of three procedures for hysterectomy

Abstract:
Objective: To explore the clinical effect of three procedures for hysterectomy.Methods: The clinical data of 185 cases of hysterectomy were reviewed retrospectively.Seventy-eight cases received total abdominal hysterectomy,56 cases total vaginal hysterectomy and 51 cases laparoscopy-assisted vaginal hysterectomy.The effect and outcome were compared.Results: There was no significant difference in surgery time and anus exhaust time among the 3 groups(P>0.05).Total abdominal hysterectomy group were obviously supior to total vaginal hysterectomy group and laparoscopy-assisted vaginal hysterectomy group in average bleeding amount and time of hospital stay(P<0.01).Conclusions: Each of the 3 surgical procedures has its own advantages,so the choice of operation shoud be based on the overall evaluation of the patients' condition,the doctors' skill and the hospitals' condition.

Objective: To investigate the efficacy of open reduction and internal plate fixation in treating intra-articular calcaneal fracture.Methods: Thirty-two cases of intra-articular calcaneal fracture were treated with open reduction and internal plate fixation.All the cases were followed up and Kerr assessment score was used to evaluate the therapeutic effect.Results: The follow-up lasted for 12-24 months.All the fractures healed 4 months after the operation.The Kerr assessment score showed that the results were excellent in 12 cases,fair in 16 cases,poor in 3 cases and bad in 1 case.The fine rate was 87.5%.Conclusions: Open reduction and internal plate fixation is a good method for treatment of displaced intra-articular calcaneal fractures.Open reduction can also restore the articular congruency,the shape and alignment of the calcaneus.

Abstract:
Objective: To explore the value of hysteroscopy combined with B ultrasonics for patients with difficult intrauterine devices(IUD) removal.Methods: One hundred and twenty-five cases with difficult IUD removal by traditional methods were performed hysteroscopy and B ultrasonics to verify the position of IUD.After precised location,the IUD were taken out through vagina or abdominal cavity successfully.Results: The hysteroscopy and B ultrasonics showed that 49 IUD were in normal position,41 IUD were partly incarcerated,28 IUD were fragment and residual,6 IUD were distorted and 1 IUD was in abdominal cavity.One IUD was taken out through abdominal cavity and the other 124 IUD were taken out through vagina.Conclusions: It is effective and safe to take out IUD with the aid of hysteroscopy and B ultrasonics.

Abstract:
Objective: To evaluate the effects of different concentrations of lidocaine on the hemodynamics of patients radical resection of esophageal cancer.Methods: Fifty-four ASAⅠ-Ⅱpatients were randomly divided into epidural normal saline(NS) group,epidural 1% lidocaine(Lido) group and epidural 2% Lido group,with 18 cases in each group.Anesthesia was induced and maintained with epidural infusion of Lido or NS at 7 ml/h.The baseline value,blood pressure(BP),heart rate(HR) of the patients were recorded before and during the operation.Results: During the operation,NS group showed a higher BT and HR(P<0.01),2% Lido group an over-decrease in BP and lower HR(P>0.05),and 1% Lido group a comparatively stable BP and HR.Conclusions: General anesthesia combined with local anesthesia is effective and safe in radical resection of esophaged cancer.

Abstract:
Objective: To discuss the experience in diagnosis and treatment of the major arterial injury in limbs.Methods: The clinical data of 23 patients with the major arterial injury in limbs between January 1996 and July 2006 were retrospectively analyzed.The injured arteries were repaired by arterioplasty,ligation,end-to-end anastomosis,great saphenous vein graft,artificial blood vessel graft,embolectomy with Fogarty catheter.Results: Three patients were amputated in limbs.Twelve patients were followed up from 3 months to 5 years.One patient suffered from low extremity ischemia.One patient had ischemic muscular atrophy in low extremity.Conclusions: Early diagnosis and improvement of recognition of arterial injury are the key to increase the survival of the limbs and avoid the occurrence of false aneurysm.The method of operation,embolectomy with Fogarty catheter and early fasciectomy can increase salvage.

Objective: To understand the pathogeny of and treatment for atrial fibrillation(AF).Methods: The pathogeny,type,artery embolization complications,treatment and prognosis of 150 hospitalized patients with atrial fibrillation were analyzed retrospectively.Results: Coronary heart disease ranked the first,followed by hypertension in pathogeny.Amiodarone was safe and effective for treatment of cardioversion of paroxysmal atrial fibrillation and maintenance of sinus rhythm.Conclusions: Coronary disease and hypertension are the major causes of atrial fibrillation.Analysis of the causes of atrial fibrillation is of great value for the prevention and treatment of the disease.

Abstract:
Objective: To evaluate the effect of partial parenteral nutrition(PPN) plus enteral feeding on premature infants.Methods: Forty premature infants were randomly divided into control group and study group.The study group were administered parenteral nutrition in addition to enteral feeding,while the control group received 5%-10% glucose,fluid and electrolytes simultaneously.The relevant indices were measured in the 2 groups.Results: There were no significant differences in blood biochemistry and glucose between the two groups before and after PPN(P>0.05).The time of hospital stay and birth-weight regain was significantly shorter in the study group than in the control group(P<0.05-P<0.01).Conclusions: Premature infants could tolerate small volume of PPN during the first day of life.A combination of intravenous and small amount of oral feeding has no significant influence on the blood biochemistry and glucose of the infants,which may shorten the hospital stay and help the weight gain.

Abstract:
Objective: To investigate the change of the mast cells(MCs) and its relationship with the expression of Th1 cytokines(IL-2,IFN-γ),and to elucidate their possible roles in visceral hypersensitivity in patients with irritable bowel syndrome(IBS).Methods: The mucosa specimens were taken from the sigmoid colon of 10 normal controls and 30 patients with post-infective IBS.The MCs,IL-2 and IFN-γ were stained by histochemical and immunohisto-chemical methods respectively,and color image analyzer was applied to investigate the results qualitatively and quantitatively.The ultrastructure of cells in colic mucosa and the changes of the MCs were studied through an ultra-microscopy using situs embedding technique and a light microscopic study in serial sections.The results were analyzed by image analysis software.Results: The number of MCs in the sigmoid colon of patients with IBS was significantly elevated(P<0.05),and MCs in IBS presented prominent variations.Significant increase of the IL-2 and IFN-γ was found in the intestinal mucosa of IBS compared to the control.Conclusions: The Expressions of MCs,IL-2 and IFN-γ are significantly increased in patients with post-infective-IBS,which indicates that the activation of colonic MCs is involved in the visceral sensitization.The MCs may be the middle link of immunity mechanism and nerve mechanism.High expressions of IL-2 and IFN-γ may partly explain the sensitization of the internal organs.Infection is not the only factor leading to the changes of mast cells in patients with IBS.

Abstract:
Objective: To investigate the reasons of unsuccessful interventional treatment for congenital heart disease and the method of surgical treatment.Methods: The interventional treatment failed in 23 patients with congenital heart disease,and in 4 patients the occluder was dislocated after interventional treatment.All of the 27 patients underwent surgical treatment including removal of the displaced occluder and/or the congenital heart disease repair on cardiopulmonary bypass on the same day of interventional treatment or three weeks later.Results: The results of surgical treatment were satisfying.All patients recovered,and the heart function was in NYHA class Ⅰ.Conclusions: The indication for the interventional treatment should be strict and the size of occluder should be correct.Occluder displacement is a severe complication after interventional treatment with congenital heart disease.Once the occluder is found dislocated a more aggressive strategy with emergent earlier surgical treatment is advocated before deterioration of heart function.

Objective: To evaluate the efficacy of two procedures for management of pelvic flour dysfunction(PFD).Methods: Of the 70 cases of PFD,25 were performed vaginal hysterectomy plus colporrhaphia anterior-posterior(tradition group) and 45 pelvic flour reconstruction surgery plus transobturator suburethral tape ventrofixation(reconstruction group).The clinical parameters were observed and analyzed.Results: The operation time,blood loss,recovery time of postoperative gastrointestinal function,recovery time of urination and the rate of postoperative diseases were(95.4±20.5) min,(133.2±17.7) ml,(24.5±6.5) min,(34.6±9.4) min,2.22% in the reconstruction group,and(155.7±18.6) min,(305.2±44.7) ml,(46.6±5.8) min,(118.9±8.5) min,12.00% in the tradition group,respectively.There was a significant difference between the two groups(P<0.01).The total effective rates in the reconstruction group and tradition group were 97.78% and 76.00%,respectively.The difference was significant between the two groups(P<0.01).Conclusions: Pelvic flour reconstruction surgery plus transobturator suburethral tape ventrofixation is effective for treatment of PFD,with the advantages of shorter operation time,lower volume of blood loss and less recurrence rate.The technique is easy to master and worth popularizing.

Objective: To evaluate the efficacy of surgical management of obstructive sleep apnea hypopnea sysndrome(OSAHS) in children.Methods: The clinical data of 31 children with OSAHS were reviewed retrospectively.All the 31 cases underwent adenotonsillectomy and the clinical outcome was observed.Results: The follow-up lasted for 6 months,and 28 cases demonstrated significant improvement in clinical symptoms after adenotonsillectomy.Conclusions: Adenotonsillectomy is a preferential treatment for children with OSAHS.

Objective: To compare the results by screening tests and confirmatory tests,and to evaluate the methods of detecting Treponema pallidum in the elderly.Methods: The positive samples detected by screening tests with enzyme-linked immunosorbent assay and syphilis toluidine red untreated serum test were respectively confirmed by treponema pallidum particle agglutination(TPPA),fluorescent treponemal antibody absorption test(FTA-ABS) and Western blotting(WB),and then the results were analyzed statistically.Results: Seventy-two of the 76 samples were detected positive by TPPA in screening tests(94.7%,72/76).Of the 72 positive samples by TPPA,71 were simultaneously confirmed by FTA-ABS and WB assays(98.6%,71/72).Conclusions: The positive samples of Treponema pallidum detected by screening tests in the elderly must be confirmed by TPPA assay and even by FTA-ABS and WB assays if needed.

Objective: To establish standardized measuring method for blood lead.Methods: Tungsren-ship atomic absorption spectrophotometry(GFAAS) was used to measuring blood lead level.According to the standard and regulation for blood lead detection from NCCLS and bureau of health,following performances were evaluated:inprecision,inaccuracy,method checking line,quantitative checking line,reportable range,cross contamination etc.Results: The blood lead level was highly consistent with adsorption value,the coefficient r=0.999 9(t=103.06,P<0.01);the inprecision,inaccuracy,methods test line and quantitative test line all meet the clinical need.The quantitative data could be provided when the blood lead level below 298 μg/L.Average cross contamination rates were 0.378%-0.394%.Experiment result fulfilled all demand for performance evaluation.Conclusions: Successfully establish measurement protocol with stable,repeatable and significant clinical development value.

Objective: To study the relationship between antibiotic resistance and extended-spectrum β-lactamases(ESBLs) genes in clinical isolates of E.coli.Methods: Antibiotic susceptibility was tested by Kirby-Bauer disk diffusion method and ESBLs production was detected by CLSI phenotypic confirmarory test in E.coli isolates.The β-lactamases genes were identified by PCR and sequencing.Results: Fourty-nine of 117 strains collected were ESBLs-producing E.coli and all of them were resistant to ampicillin,piperacillin,cefazolin,cefuroxime.The resistance rates to cefotaxime,ceftriaxone,cefpodoxime,and cefoperazone were more than 95%,but only 41.8% to ceftazidime.The resistance rates to piperacillin-tazobactam and cefoperazone-sulbactam were 8.1% and 18.4%,respectively,to imipenem was 0.0%.ESBLs gene was found in all ESBLs-producing isolates,and CTX-M-14 genotype was dominated(87.8%).Conclusions: ESBLs-producing E.coli are prevelance in our hospital and most of them are multidrug resistant strains.CTX-M-14 is the main genotype of ESBLs,and the second is CTX-M-3 genotype.No other genotype of ESBLs was detected.
