Study on the toxicity of the repeated administration of protective polypeptides in rats

Abstract:
ObjectiveTo evaluate the toxic reaction of human body protection peptide(HuBPP) in rat, and determine non-toxic reaction dose for clinical application and providing the reference information in clinical safe medication and monitoring of adverse reactions.MethodsThe dosage of administration in 80 SD rats were confirmed by single intravenous and intramuscular injection.The dose of 0 mg/kg, 0.2 mg/kg, 1 mg/kg and 4 mg/kg were intramuscularly injected into the control group, low, medium high dose groups for 1 month, respectively(30 rats each group).The body weight and food intake in four groups were recorded during the test, the biochemical index at the end of drug administration and recovery period were detected, and the toxicity change of main organs was detected using histopathology.ResultsThe acute toxicity test showed that there was no significant change in the observed indicators between control group and experiment group.The long-term toxicity test showed that there was no statistical significance in toxicity reaction of body weight, food intake and main organ indicators between control group and experiment group(P>0.05).After the long-term poison test, the levels of MCHC and MPV in male rats of high-dose group were higher and lower than that in control group, respectively(P < 0.01 and P < 0.05);the levels of TG and Na+ in female rats of middle-dose group were higher and lower than that in control group, respectively(P < 0.05);the levels of APTT in male rats of high- and middle-dose group were significantly lower than that in control group(P < 0.05);the Cl- concentration in high-dose group was significantly higher than that in control group(P < 0.05).At the end of the recovery period, the values of RDW in male rats of high-dose group were lower than those in control group(P < 0.05), the levels of ALT, GLU, and TG in female rats of low-dose group were significantly higher than those in control group(P < 0.05 to P < 0.01).However, the parameters were within the normal range as reported in literature, and the significant difference of which was not found.ConclusionsUnder the condition of the test, the 4 mg/kg or less than 4 mg/kg of HuBPP is safe dose in rats.
Study of protective effect of ursolic acid on myocardial injury of type 2 diabetic rats

Abstract:
ObjectiveTo observe the effects of ursolic acid on cardiomyopathy of type 2 diabetic rats.MethodsThe SD rat model with type 2 diabetes mellitus was randomly divided into the control group, model group, low dose of ursolic acid group, middle dose of ursolic acid group, high dose of ursolic acid group and metformin group.Each group was treated with intragastric administration using corresponding drugs, and the mental state, diet quantity and urine volume of SD rat in each group were observed during administration.The blood of abdominal aorta was collected after final administration and anesthesia, the heart was harvested and weighed, and the heart weight index was calculated.The activities of creatine kinase(CK) and creatine kinase isoenzyme(CK-MB)were determined by automatic biochemical analyzer.The serum levels of AGEs, TNF-α, SOD and MDA in myocardium tissue were detected using ELISA.The heart tissue was embedded in paraffin, stained using HE, and the change of issue morphology was observed under the light microscope.ResultsThe activity of serum CK in high dose of ursolic acid group was lower than that in control group(P < 0.05).The high dose of ursolic acid could decrease the levels of TNF-α, MDA and AGEs, and increase the level of SOD in serum and heart tissues in type 2 diabetic rats, the effect of which was similar to that of metformin, and the difference of the effect between high dose of ursolic acid and metformin was not statistically significant(P>0.05).ConclusionsUrsolic acid has protective effect on myocardial injury in rats with type 2 diabetes mellitus, and the mechanism of which may be related to reduce the levels of AGEs and TNF-α, and improve the oxidative stress of rats.
Study on the effect of Xuebijing on the early intervention of acute closed liver injury induced by underwater explosion

Abstract:
ObjectiveTo explore the effects of Xuebijing on the early intervention of acute closed liver injury induced by underwater explosion.MethodsFifteen healthy New Zealand rabbits were randomly divided into the control group, injury group and intervention group(treated with Xuebijing), and the acute closed hepatic injury in three groups was established by underwater explosion.The serum levels of ALT, IL-6 and MMP-9 in three groups after 0, 4, 12 and 24 h of explosion were detected using enzyme-linked immunoassay.ResultsThe differences of the ALT levels among three groups after 4 h of explosion were statistically significant(P < 0.05), and the level of ALT in injury group was higher than that in control group(P < 0.05).The differences of the IL-6 levels among three groups after 12 h of explosion were statistically significant(P < 0.01), and the level of IL-6 in injury group was higher than that in control group and intervention group(P < 0.05 to P < 0.01).The differences of the MMP-9 levels among three groups after 24 h of explosion were statistically significant(P < 0.05), and the level of MMP-9 in injury group was higher than that in control group and intervention group(P < 0.05).The differences of the MMP-9 levels in injury group between after 4 h and 12 h, 24 h of explosion were statistically significant(P < 0.05), and the difference of the MMP-9 level in intervention group between after 4 h and 12 h of explosion was statistically significant(P < 0.05).ConclusionsXuebijing plays a protective effect on the acute closed liver injury caused by underwater explosion in rabbits.
The relationship between CYP2C19 gene polymorphisms and different types of acute coronary syndromes

Abstract:
ObjectiveTo investigate the relationship of CYP2C19 gene polymorphisms in cytochrome P450 oxidase(CYP450) and two types of acute coronary syndromes(ACS).MethodsThe 12-lead ECG monitoring in 203 cases with ACS on admisssion were examined, and the myocardial enzyme indictors in all cases were examined on the second day after admission.The CYP2C19 genotypes of all patients were detected by PCR and gene sequencing technique.The coronary involvement in all patients were identified using coronary angiography(CAG).ResultsThe difference of the CYP2C19 gene polymorphisms between ST-segment elevation ACS group and non-ST-segment elevation ACS group was not statistically significant(P>0.05), and the differences of the levels of blood glucose, C-reactive protein(CRP) and serum homocysteine, and prevalence rate of hypertension between two groups were statistically significant(P < 0.05 to P < 0.01).The results of logistic regression analysis showed that there was no statistical significance in CYP2C19 gene polymorphisms between two groups(OR=0.71, P>0.05), and the differences of the levels of blood glucose, CRP, HCY and HDL-C and prevalence rate of hypertension between two groups were statistically significant(P < 0.05).ConclusionsThere is no association between CYP2C19 polymorphism and different types of ACS, and the levels of blood glucose, HCY and CRP, and HDL-C level in patients with ST-segment elevation ACS may be higher and lower, respectively.
Clinical application of lateral vascular chain of foot in the prevention of cutaneous necrosis in lateral L-shaped incision of calcaneus

Abstract:
ObjectiveTo explore the relationship between the blood supply of the lateral L-shaped incision flap of calcaneus and blood vessel chain, and probe the safe operative incision.MethodsFive cadaver samples were dissected to understand the blood vessel chain of L-shaped incision flap.According to the autopsy results, 10 cases with calcaneus fractures were treated with enlarging lateral L-shaped incision of calcaneal combined with internal fixation of bone plate, and the incision healing of patients was observed.ResultsThe lateral blood vessel chain formed by peroneal and anterior tibial arteries was the main source of blood supply of L-shaped incision flap.The survival of enlarging lateral L-shaped incision of calcaneus was good in all cases, and no necrosis and infection were found.ConclusionsProtecting the lateral vessel chain of foot can avoid the damage of blood supply of L-shaped incision flap, and reduce the poor healing of lateral L-shaped incision of calcaneal.
Effect of the preoperative indomethacin suppository intrarectal application on the serum levels of PGE2, IL-17 and TNF-α after ERCP, and its clinical significance

Abstract:
ObjectiveTo investigate the molecular mechanism of indometacin preventing pancreatitis after endoscopic retrograde cholangiopancreatography(ERCP).MethodsSeventy patients scheduled by ERCP were randomly divided into the control group and observation group(35 cases each group).The observation group and control group were treated with 100 mg of indomethacin suppository and placebo suppository intrarectal application before operation, respectively.The incidence rates of postoperative hyperamylasemia and pancreatitis in two groups were observed, and the serum levels of prostaglanding E2(PGE2), interleukin-17(IL-17) and tumor necrosis factor-α(TNF-α) in two groups were detected before operation and after 3 h and 24 h of operation.ResultsThe postoperative pancreatitis in observation group(1 case)was lower than that in control group(8 cases)(P < 0.05).There was not statistical significance in the incidence rate of the total complications between two groups(P>0.05).There was not statistical significance in the levels of PGE2, IL-17 and TNF-α between two groups before operation(P>0.05).The levels of PGE2, IL-17 and TNF-α in observation group after 3 h of operation were lower than those in control group(P < 0.05).After 24 h of operation, the levels of IL-17 and TNF-α in observation group were lower than those in control group(P < 0.05 and P < 0.01), and the difference of the level of PGE2 between two groups was not statistically significant(P>0.05).The differences of the levels of IL-17 and TNF-α in control group between before and after operation were statistically significant(P < 0.01 and P < 0.05), but which in observation group was not statistically significant(P>0.05).ConclusionsIndomethacin suppository preoperative intrarectal application can effectively reduce the incidence rate of pancreatitis after ERCP, which may be related to the decreasing levels of PGE2, IL-17 and TNF-α.
Study on the influencing factor of sharp instrument injury of medical worker in a grade A tertiary hospital

Abstract:
ObjectiveTo investigate the incidence of sharp instrument injury of medical workers in a grade A tertiary hospital, analyze its influencing factors, and provide the scientific basis for hospital formulating effective prevention and control measures.MethodsThe incidence rates of sharp instrument injury of 1 156 medical workers in a grade A tertiary hospital in 2015 were retrospectively analyzed using self-designed questionnaire.The correlations of the gender, nature of work, department, length of service, professional title, education and training with sharp instrument injuries in 1 071 valid questionnaires were analyzed.ResultsThe sharp instrument injury in 284 cases were found in 2015, and the incidence rate of which was 26.5%.Except for gender, the differences of the incidence rates of sharp instrument injury among different nature of work, department, length of service, professional title, education and training medical workers were statistically significant(P < 0.01).The incidence rate of sharp instrument injury was set as the dependent variable, the statistically significant factors in the univariate analysis were set as the independent variable, and the results of univariate analysis showed that the department, length of service, professional title and training were the independent risk factors of sharp instrument injury(P < 0.05 to P < 0.01).ConclusionsAccording to the influencing factors of sharp instrument injury, the knowledge training should be carried out, which can reduce the risk of sharp instrument injury.
Study on the influencing factor of early renal damage in patients with primary hypertension

Abstract:
ObjectiveTo study the risk factors of early renal damage in patients with primary hypertension.MethodsOne hundred and eighty-seven patients with primary hypertension were divided into the hypertension group(116 cases) and hypertensive renal damage group(71 cases) according to the urine albumin/creatinine ratio(UACR), the clinical data, 24 h systolic blood pressure(SBP) and diastolic blood pressure(DBP) between two groups were compared, and the risk factors of early renal damage in patients with primary hypertension were analyzed.ResultsThe differences of gender, age, body mass index, smoking and drinking between two groups were not statistically significant(P>0.05), the disease duration in hypertension group was significantly shorter than that in hypertensive renal damage group(P < 0.01), and the differences of dSBP, dDBP, 24hSSD, 24hDSD, dSSD and dDSD between two groups were statistically significant(P < 0.05 to P < 0.01).The UACR was set as the dependent variable, the disease duration, dSBP, dDBP, 24hSSD, 24hDSD, dSSD and dDSD were set as the independent variable, and the multiple logistic regression analysis showed that the disease duration, dSBP, 24hSSD, 24hDSD, dSSD and dDSD were the independent influencing factors of UACR(P < 0.01).ConclusionsThe disease duration, dSBP, 24hSSD, 24hDSD, dSSD and dDSD are the independent risk factors of renal function damage in patients with primary hypertension.
Investigation of the community rehabilitation mode for hemiplegic patients relied on rehabilitation medicine of general hospital

Abstract:
ObjectiveTo explore the community rehabilitation mode for hemiplegic patients relied on rehabilitation medicine of general hospital.MethodsSixty-two hemiplegic patients from Chongzhou City community medical institutions and 42 cases from Chongzhou People's Hospital were divided into the observation group and control group, respectively.The control group was treated with systematic rehabilitation method in rehabilitation center of hospital, and the observation group was treated with the community rehabilitation mode relied on rehabilitation medicine of general hospital.After six months, the treatment effects in two groups were evaluated using the simple Fugl-Meyer(FMA) scale, improved Barthel index(BI), and Hamilton Anxiety Scale(HAMA), and the rehabilitation costs between two groups were compared.ResultsThere was no statistical significance in the scores of FMA, BI and HAMA between two groups before rehabilitation(P>0.05).After 6 months of rehabilitation, the scores of FMA and BI in two groups were significantly higher than those before treatment(P < 0.01), and the HAMA score in two groups were significantly lower than that before treatment(P < 0.05).The differences of the scores of FMA and BI between two groups were not statistically significant(P>0.05), and the score of HAMA in observation group was significantly higher than that in control group(P < 0.01).The average total cost of rehabilitation treatment in observation group[(24 674.8±3 861.2)yuan] was significantly lower than that in control group[(38 672.5±6 167.4)yuan](P < 0.01).ConclusionsUnder the guidance and co-management of general hospital, the physical rehabilitation treatment of hemiplegic patients in community hospitals is economic and effective, which can promote the construction of tertiary rehabilitation medical system.
Bioinformatics analysis of the differential expression of miRNA-181b in peripheral blood of schizophrenic patients

Abstract:
ObjectiveTo analyze the target gene, transcription factor and signal pathway of the differential expression of miRNA-181b in the peripheral blood of schizophrenic patients.MethodsThe miRNA-181b target gene was analyzed using the DIANA-TarBase database, the GO function and KEGG pathway enrichment of miRNA-181b gene were analyzed using DIANA-miPath V.3 software, and the transcription factors of miRNA-181b were analyzed using the DIANA-miRGen v3.0 database.ResultsThere were 1 344 target genes of miRNA-181b, and the target genes ARHGAP11A, PPA1, YY1 and ZC3H12B significantly expressed.The target genes of miRNA-181b were widely involved in the regulation of biological processes(including molecular function, RNA binding and cell composition).The results of KEGG bioinformatics analysis showed that the target gene of miRNA-181b enriched 12 signal pathways, which included the hippo signal pathway, p53 signal pathway, HIF-1 signal pathway and lysine degradation.Seven transcription factors were significantly associated with miRNA-181b, and JUN, EGR1 and MEF2C were significantly associated with schizophrenia.ConclusionsThe multiple target genes, signal pathways and transcription factors are closely related to the onset of schizophrenia using the bioinformatics analysis of the differential expression of miRNA-181b in the peripheral blood of schizophrenic patients.
Comparative analysis of the clinical effect between inflation-free and inflation single-hole laparoscopic totally extraperitoneal inguinal hernia repair

Abstract:
ObjectiveTo compare the clinical effects between inflation-free and inflation single-hole laparoscopic totally extraperitoneal inguinal hernia repair(TEP).MethodsEighty adult patients with inguinal hernia were randomly divided into the observation group and control group(40 cases each group).The observation group was treated with inflation-free single-hole TEP, and the control group was treated with inflation single-hole TEP.The clinical effects between two groups were compared.ResultsThe differences of the operation time, intraoperative blood loss, hospitalization cost and hospitalization time between two groups were not statistically significant(P>0.05).The pain score in observation group was significantly lower than that in control group(P < 0.01).The incision satisfaction in observation group(95.00%) was higher than that in control group(75.00%)(P < 0.05).The postoperative recurrence in two groups was not found.ConclusionsThe inflation-free single-hole TEP in treating inguinal hernia is safe, feasible, and has high degree of acceptance, which has certain clinical application value.
Clinical effect of Tanreqing injection combined with cefoperazone sulbactam in the treatment of elderly community acquired pneumonia in 50 cases

Abstract:
ObjectiveTo observe the clinical effects of Tanreqing injection combined with cefoperazone sulbactam in the treatment of elderly community acquired pneumonia.MethodsFifty elderly community acquired pneumonia cases, with wind warm lung heat disease phlegm heat obstructie pulmonary syndrome, were randomly divided into the treatment group and control group(25 cases each group).The control group were intravenously treated with cefoperazone sulbactam, and the treatment group was additionally treated with Tanreqing injection based on the control group for 14 days.The main clinical symptoms, signs and X-ray of chest in two groups before and after treatment were observed.ResultsThe difference of treatment effect between two groups was not statistically significant(P>0.05).The recovery time of the clinical symptoms and signs in treatment group was shorter than that in control group(P < 0.05 to P < 0.01), and the result of chest X-ray showed that the absorption rate of inflammatory lesion in treatment group was higher than that in control group(P < 0.05).ConclusionsThe treatment of elderly community acquired pneumonia with Tanreqing injection combined with cefoperazone sulbactam has good effect, short course, and no obvious side reaction.
Comparison of the clinical efficacy and prognosis between early enteral nutrition and total parenteral nutrition in the treatment of severe pancreatitis

Abstract:
ObjectiveTo compare the clinical efficacy and prognosis between early enteral nutrition and total parenteral nutrition in the treatment of severe acute pancreatitis.MethodsEighty patients with severe pancreatitis were randomly divided into the control group and observation group(40 cases each group).The control group was treated with total parenteral nutrition, and the observation group was treated with early enteral nutrition.The recovery time of hemodiastase and urine amylase levels, hospital stay, levels of total serum protein, albumin and urea nitrogen, percentage of blood lymphocyte, inflammatory factor level, cellular immune function and APACHEⅡ score between two groups were compared.ResultsThe recovery time of hemodiastase, recovery time of urine amylase and hospital stay in observation group after treatment were significantly shorter than those in control group(P < 0.01).The levels of total serum protein, albumin, urea nitrogen and percentage of blood lymphocyte in observation group after treatment were significantly higher than those in control group(P < 0.01).After treatment, the levels of TNF-α, IL-1β, IL-6, IL-8 and endotoxin in two groups significantly decreased compared with before treatment(P < 0.05 to P < 0.01), and the levels of TNF-α, IL-1β, IL-6, IL-8 and endotoxin in observation group were significantly lower than those in control group(P < 0.01).Compared with before treatment, the levels of CD3+, CD4+ and CD4+/CD8+ significantly increased(P < 0.01), and the level of CD8+ significantly decreased in two groups after treatment(P < 0.05).After treatment, the levels of CD3+, CD4+ and CD4+/CD8+ in observation group were significantly higher than those in control group(P < 0.01), and the level of CD8+ in observation group was significantly lower than that in control group(P < 0.01).The APACHEⅡ score in observation group after 24h, 48h and 72h of treatment was significantly lower than that before treatment(P < 0.01), the APACHEⅡ score in observation group after 48 h and 72h of treatment was significantly lower than that after 24h of treatment(P < 0.05 and P < 0.01), and the APACHEⅡ score in observation group at each time-point was significantly lower than that in control group(P < 0.05 to P < 0.01).ConclusionsEarly enteral nutrition in the treatment of severe pancreatitis can effectively reduce the inflammatory factor level, enhance cellular immune function, and improve prognosis of patients.
Effect of continued vacuum sealing drainage on nerve conduction and oxidative stress in diabetic foot patients

Abstract:
ObjectiveTo explore the effects of continued vacuum sealing drainage on nerve conduction and oxidative stress in diabetic foot patients.MethodsSeventy patients with diabetic foot were randomly divided into the control group and observation group(35 cases each group).The control group was treated with conventional method combined with defibrase,and the observation group was treated with continued vacuum sealing drainage based on the conventional treatment.The total effective rate,adverse reaction rate,motor nerve conduction velocity(MCV),sensory nerve conduction velocity(SCV),artery blood flow velocity,antioxidant superoxide anion free radical(Anti-O2-)activity,activity of superoxide dismutase(SOD),nitric oxide(NO)content,level of malondialdehyde(MDA),total content of nitric oxide synthase(TNOS) and inducible nitric oxide synthase(iNOS) level between two groups were compared before and after treatment.ResultsThe clinical total effective rate in observation group(94.29%) was significantly higher than that in control group(77.14%)(P<0.05).The MCV and SCV in two groups increased(P<0.05 to P<0.01),and which in observation group was higher than that in control group(P<0.05).After treatment,the plasma levels of iNOS and MDA increased(P<0.05 to P<0.01),the levels of SOD,Anti-O2-,NO and TNOS decreased(P<0.05 to P<0.01),and the differences of the increasing and decreasing degrees of those between two groups were not statistically significant(P>0.05).After treatment,the blood flow velocity in observation group was higher than that in control group(P<0.01).The incidence rates of adverse reaction in observation group and control group were 14.29% and 8.57%,respectively,and the difference of which was not statistically significant(P>0.05).ConclusionsContinued vacuum sealing drainage in the treatment of diabetic foot patients can improve the nerve conduction velocity,promote the healing of foot ulcers,and do not significantly aggravate the oxidative stress reaction of patients,which is worthy of clinical application.
Analysis of the risk factor of the ventilator-associated pneumonia in ICU

Abstract:
ObjectiveTo analyze the risk factors of ventilator-associated pneumonia(VAP) in intensive care unit(ICU), and provide scientific evidence for prevention and treatment of VAP.MethodsA total of 112 patients treated with mechanical ventilation > 48 h were divided into the VAP group(42 cases) and non-VAP group(70 cases) according to clinical diagnosis.The age, sex, state of consciousness, underlying disease, hospitalization time, and mechanical ventilation between two groups were compared.ResultsThe differences of the tracheostomy, enteral nutrition, aspiration frequence, air pressure therapy, early rehabilitation, hospitalization time, mechanical ventilation time and antimicrobial drug use between two groups were statistically significant(P < 0.01).Multivariate logistic regression analysis showed that the tracheotomy, long hospitalization time, long time of mechanical ventilation, antimicrobial drug combination were the independent risk factor of VAP.ConclusionsVAP can be caused by several high-risk factors.Taking the targeted preventive measures, strengthening the VAP risk factor monitoring and strict implementing the sterile operation system can reduce the incidence of VAP.
Study on the risk factors of early-onset coronary heart disease in female

Abstract:
ObjectiveTo study the risk factors of early-onset coronary heart disease in female and characteristics of coronary artery lesion.MethodsA total of 66 women(age of onset ≤ 65 years) with early-onset coronary heart disease diagnosed by coronary angiography, 42 healthy women, and 70 late-onset coronary heart disease women(age of onset > 65 years) from June 2015 to June 2018 were divided into the early-onset group, control group and late-onset group, respectively.The general information, disease history, and biochemical indicators among three groups were compared.ResultsThe positive rate of smoking history and hypertension history in early-onset group(21.2% and 45.5%) were higher than those in control group(4.8% and 9.5%) and late-onset group(11.4% and 35.7%)(P < 0.05 to P < 0.01).The levels of urea nitrogen, total cholesterol, low density lipoprotein, lipoprotein, triglyceride, lactate dehydrogenase and creatine kinase in early-onset group were significantly higher than those in the healthy control group(P < 0.05 to P < 0.01).Compared with the late-onset group, the level of creatinine was lower(P < 0.01), while the levels of urea nitrogen, lipoprotein, low-density lipoprotein, lactate dehydrogenase and creatine kinase were higher in arly-onset group(P < 0.05 to P < 0.01).ConclusionsThe smoking, hypertension history, and abnormal levels of blood urea nitrogen, lipoprotein, low density lipoprotein, lactate dehydrogenase and creatine kinase may be the risk factors for early-onset of coronary heart disease in women.The abnormal increasing of total cholesterol and triglyceride levels may be the risk factors of coronary heart disease in middle-aged and young women.

Abstract:
ObjectiveTo investigate the correlation between cystatin C, (Cys C)and hypertensive intracerebral hemorrhage and carotid atherosclerotic plaque.MethodsOne hundred and fifteen patients with hypertensive cerebral hemorrhage were set as the observation group, and 71 healthy examination people were as the control group.The general information and laboratory inspection between two groups were compared, and the risk factors of cerebral hemorrhage were analyzed using multivariate logistic regression.The observation group was subdivided into the plaque group(57 cases) and nonplaque group(58 cases) according the results of carotid artery ultrasonography, the general information and laboratory inspection between two subgroups were compared, and the risk factors of plaque formation were analyzed using multivariate logistic regression.ResultsThe levels of serum Cys C and C-reactive protein(CRP) in observation group were significantly higher than those in control group(P < 0.01), and the level of high-density lipoprotein in observation group was significantly lower than that in control group(P < 0.01).Multivariate logistic regression analysis showed that the Cys C and level were the independent risk factors of cerebral hemorrhage.The age, incidence rate of hyperlipidemia and Cys C level in plaque group were significantly higher than those in nonplaque group(P < 0.05 to P < 0.01).Multivariate logistic regression analysis showed that the age and Cys C were the independent risk factors of plaque formation in cerebral hemorrhage patients.ConclusionsThe serum level of Cys C is closely related to the hypertensive intracerebral hemorrhage and carotid plaque formation, and the early detection of serum Cys C level has certain guiding significance in evaluating the hypertensive intracerebral hemorrhage and carotid plaque.

Abstract:
ObjectiveTo explore the feasibility and clinical effects of using vaginoplasty partial labia flap mesh graft in the treatment of congenital complete vaginal atresia or congenital absence of vagina.MethodsFrom May 2011 to September 2017, nine cases with congenital atresia of vagina complicated with congenital abnormal cervix or congenital absence of vagina were treated with vaginoplasty or cervicoplasty using partial labia flap mesh graft, and the operation results were evaluated.ResultsAll operations were successful, and the depth of vagina arrived at 7 to 9 cm before discharge.Long term follow-up:nine cases were satisfied with the width and depth of the vagina.Five cases rserved uterine had normal menstral cycle, who had no dysmenorrhe.Seven cases were already married with satisfied sex.ConclusionsCervicovaginal reconstruction with part of the labia minora flap retiform transplantation is an effective alternative for treating congenital complete vaginal atresia combined with cervical atresia or congenital absence of vagina.
Effect of Juanbi Jianxi recipe combined with arthroscopic surgery in the treatment of knee osteoarthritis

Abstract:
ObjectiveTo investigate the effects of Juanbi Jianxi recipe combined with arthroscopic surgery in the treatment of knee osteoarthritis.MethodsEighty patients with knee osteoarthritis were randomly divided into the observation group and control group(40 cases each group).Two groups were treated with arthroscopic surgery, and then the control group and observation group were treated with functional training and Juanbi Jianxi recipe combined with functional training, respectively.The knee joint function score and pain in two groups were evaluated using Lysholm knee joint function scale and visual analogue scale(VAS), and the levels of IL-1 and TNF-α in two groups between before and after treatment were compared.ResultsAfter treatment, the scores of Lysholm and VAS in two groups were significantly improved compared with before with treatment(P < 0.01), and which in observation group was significantly better than that in control group(P < 0.01).After treatment, the levels of IL-1 and TNF-α in two groups significantly decreased compared before treatment(P < 0.01), and which in observation group was significantly lower than that in control group(P < 0.01).The total effective rates in observation group and control group were 95.00%(38/40) and 72.50%(29/40), respectively.The treatment effect in observation group was better than that in control group(P < 0.05).ConclusionsThe effects of Juanbi Jianxi recipe combined with arthroscopic surgery in the treatment of knee osteoarthritis are good, which can effectively decrease the level of inflammatory factor, and is worthy of promotion in clinic.
Effect of the mixture of 0.3% lidocaine and propofol on propofol injection pain

Abstract:
ObjectiveTo investigate the optimal concentration of lidocaine in inhibiting the propofol injection pain, and evaluate its efficacy and safety.MethodsThe optimal concentration of lidocaine in painless artifical abortion patients(ASA Ⅰ to Ⅱ)was detected using step by step increasing or decreasing mode.The patients injected with sufentanil were randomly divided into the blank group(group A injected with 1% propofol), control group(group B injected with 1% propofol after 40 mg of lidocain preinjection) and experimental group(group C injected with the mixture of 0.3% lidocaine and 0.83% propofol).The pain scores in three groups were evaluated during injection period.The blood pressure before and after injection, heart rate(HR) and SpO2, waking time, respiratory or circulatory inhibition, nausea or vomiting and satisfaction degree in three groups were analyzed.ResultsBefore medication, the differences of diastolic pressure among three groups were not statistically significant(P > 0.05), and the levels of systolic pressure in group B and group C were higher than that in group C(P < 0.01).After 1 min and 3 min of medication, the levels of diastolic pressure and systolic pressure in three groups significantly decreased compared with before medication(P < 0.01), and the differences of those among three groups were not statistically significant(P > 0.05).There was no difference in HR among three groups before medication, the HR in three groups after 1 min of medication significantly decreased(P < 0.05 to P < 0.01).After 3 min of medication, the HR in group A was lower than before medication(P < 0.05).After 3 min of medication, the HR in group B and group C were lower than before medication, and the difference of which was not statistically significant(P > 0.05).After medication, the HR in group C was significantly higher than that in group B(P < 0.05).After medication, the SpO2 levels in three groups significantly decreased(P < 0.01), but the differences of which among three groups were not statistically significant(P > 0.05).After 3 min of medication, the SpO2 levels in three groups significantly increased compared with after 1 min of medication(P < 0.01), and which in group B and group C were higher than that in group A(P < 0.01).The incidence rates of severe pain and satisfaction degree in group C, group B and group A gradually increased in turn, respectively(P < 0.05).The differences of the incidence rates of low SpO2, blood pressure and heart rate, nausea, vomiting and movement among three groups were not statistically significant(P > 0.05).ConclusionsThe mixture of 0.3% lidocaine and 0.83% propofol can safely and effectively eliminate the injection pain of propofol.

Abstract:
ObjectiveTo explore the clinical effects of compound Xueshuantong capsule combined with pancreatic kininogenase enteric-coated tablets in the treatment of retinopathy.MethodsSixty patients diagnosed by non-proliferative retinopathy were randomly divided into the control group and observation group(30 cases each group).The control group was treated with pancreatic kininogenase enteric-coated tablets, and the observation group was treated with compound Xueshuantong capsule combined with pancreatic kininogenase intestine.The therapeutic effects, hemorheology and hemodynamics indexes between two groups were compared after treatment.ResultsThe total effective rate in observation group(96.67%) was slightly higher than that in control group(80.00%), but the difference was not statistially significant(P > 0.05).After treatment, the hematocrit, erythrocyte aggregation index, and whole blood high tangential viscosity in two groups were significantly improved compared with before treatment, and all indexes in observation group were significantly better than those in control group(P < 0.01).After treatment, the systolic peak velocity, diastolic velocity and resistance index of ophthalmic artery, central retinal artery and posterior ciliary artery in observation group were significantly improved compared with before treatment(P < 0.01), and all indexes in observation group were better than those in control group(P < 0.05 to P < 0.01).ConclusionsThe compound Xueshuantong capsule combined with pancreatic kininogenase enteric-coated tablet in the treatment of retinopathy can improve the clinical effect, which is worthy of clinical application.

Abstract:
ObjectiveTo explore the relationship between prognosis nutritional index(PNI), prognosis and clinicopathology of breast cancer.MethodsThe clinical data and following-up data of breast cancer patients from January 2014 to January 2017 were retrospectively analyzed.The relationships between PNI, and pathology and postoperative complications were analyzed.ResultsThe incidence rate of postoperative complications in patients with high PNI score was lower(P < 0.01), and PNI was related to Her-2 and Ki-67.The positive rates of Her-2 and Ki-67 in patients with PNI more than or equal to 45 were less than those in patients with PNI less than 45(P < 0.05).The tumor size in patients with PNI more than or equal to 45 was less than that in patients with PNI less than 45(P < 0.01).The local recurrence rates of breast cancer in patients with PNI more than or equal to 45 and PNI less than 45 were 3.33% and 13.3%, respectively, and the difference of which was not statistically significant(P > 0.05), and the systemic metastasis rate in patients with PNI more than or equal to 45(5%) was lower than that in patients with PNI less than 45(20%)(P < 0.05).ConclusionsPNI is a prognostic index of breast cancer, which is simple and easy to perform.
Analysis of the influencing factors of the operation time in urgent cesarean section

Abstract:
ObjectiveTo explore the influencing factors of the operation time in urgent cesarean section.MethodsThe cesarean section type, surgical indicators, total operation time and operation time from the beginning to delivery(DDI) in 366 cases treated with urgent caesarean section were investigated, and the influencing factors of the operation time were analyzed.ResultsThe total operation time and operation time from the beginning to delivery in class Ⅰ cesarean section were shorter than those in class Ⅱ cesarean section(P < 0.01).The total operation time and operation time from the beginning to delivery in pregnant women with scar uterus were shorter than those in pregnant women without scar uterus(P < 0.05).The total operation time in patients with fetal distress, relative cephalopelvic disproportion, malposition and more hemorrhage were shorter than that in patients without above indicators(P < 0.05 to P < 0.01).The operation time from the beginning to delivery in patients with fetal distress, placental abruption and relative cephalopelvic disproportion were shorter than that in patients without above indicators(P < 0.05 to P < 0.01).The results of multilevel univariate analysis showed that fetal distress, relative cephalopelvic disproportion, scar uterus, placental deviation, blood loss and cesarean section type were related to the operation time of cesarean section(P < 0.05 to P < 0.01), and the fetal distress, placental abruption, relative cephalopelvic disproportion, scar uterus and cesarean section type were related to the DDI(P < 0.05 to P < 0.01).ConclusionsThe indicators of urgent cesarean section and amount of blood loss are related to the operation time.Shortening the total operation time may reduce the operative blood loss and incidence rate of maternal and neonatal complication.

Abstract:
ObjectiveTo evaluate the clinical significance of the combined detection of serum prealbumin(PA) and neuron-specific enolase(NSE) in the diagnosis of neonatal hypoxic ischemic encephalopathy(HIE).MethodsA total of 45 neonates with HIE(including mild HIE 21 cases, moderate HIE 19 cases and severe HIE 5 cases) and 24 normal neonates were divided into the observation group and control group, respectively.The serum level of PA and NSE in observation group were detected, and the differences of which among three degrees HIE neonates were compared.ResultsAfter 1 and 3 d of birth, the differences of the serum level of PA in neonates with HIE were statistically significant(P < 0.01), with the increasing of HIE degree, the level of PA gradually decreased(P < 0.05 to P < 0.01), and the level of PA in three degrees HIE patients were lower than that in control group(P < 0.01).After 10 d of birth, the level of PA in three degrees HIE patients were lower than that in control group(P < 0.01), and the differences of the PA level between severe HIE patients and mild HIE, moderate HIE patients were not statistically significant(P > 0.05).After 1, 3 and 10 d of birth, the difference of the serum level of NSE in neonates with HIE was significant(P < 0.01), the difference of the serum level of NSE between mild HIE patient and control group was not statistically significant(P > 0.05), the serum level of NSE in mild and moderate HIE patients were higher than that in control group(P < 0.01), and the serum level of NSE in severe HIE patients was higher than that in moderate group(P < 0.01).ConclusionsWith the increasing of HIE degree of neonates, the serum level of PA gradually decreases, and the serum level of NSE gradually increases.The serum level of PA and NSE can be used for predicting and diagnosing HIE, and judging the prognosis and severity of disease.
Comparative study of the effect of NIPPV and oxygen hood on the transition tripper in preterm infants after mechanical ventilation

Abstract:
ObjectiveTo compare the effects between nasal synchronized intermittent positive pressure ventilation(SNIPPV) and oxygen hood on the transition tripper in preterm infants after mechanical ventilation.MethodsForty preterm infants with pediatric respiratory distress syndrome were randomly divided into the observation group and control group(20 cases each group).The observation group and control group were treated with SNIPPV and oxygen hood, respectively.The levels of PaCO2 and PaO2 between two groups at different time-points after weaning were compared.The oxygen therapy time, hospitalization time, incidence of apnea, success rate of weaning, and incidence rate of complications between two groups were compared.ResultsThe levels of PaO2 in observation group after 12 and 24 h after weaning were higher than that in control group(P < 0.05 and P < 0.01), and the levels of PaCO2 in observation group were lower than that in control group(P < 0.05).The differences of the levels of PaO2 and PaCO2 between two groups after 48 h of weaning were not statistically significant(P > 0.05).The oxygen therapy time and hospitalization time in observation group were significantly shorter than those in control group(P < 0.01).The success rate of weaning in observation group(85.0%) was higher than that in control group(P < 0.05).The incidence rate of apnea in observation group(20.0%) was lower than that in control group(50.0%)(P < 0.05).The differences of the incidence rate of complications between two groups was not statistically significant(P > 0.05).ConclusionsThe application of SNIPPV in transition tripper in preterm infants after mechanical ventilation can decrease the incidence rate of apnea, effectively shorten the oxygen therapy time and hospitalization time, and improve the oxygenation and success rate of weaning.

ObjectiveTo evaluate the diagnostic value of non-contrast-enhanced ECG-gated MR angiography(NCE-MRA) in lower extremity artery disease.MethodsThe lower extremity artery disease in 30 patients was detected using NCE-MRA and contrast-enhanced MR angiography(CE-MRA), respectively.The detection time between two examination methods was compared.The image quality was scored using 5-point scale according to the arterial display, venous infection and soft tissue artifacts.CE-MRA was set as the standard, the stenosis degree of each segment of blood vessel of NCE-MRA was assessed.The sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of severe stenosis artery diagnosed by NCE-MRA were calculated.The consistency of NCE-MRA and CE-MRA in diagnosis of severe stenosis was analyzed.ResultsThe examination time of CE-MRA was significantly shorter than that of NCE-MRA(P < 0.01).Four hundred and eighty segmental arteries in 30 patients were identified, and 477 segmental arteries of CE-MRA(99.4%) and 466 segmental arteries of NCE-MRA(92.9%) were accord with diagnostic requirements.The image quality scores of CE-MRA in abdomen-pelvis, thigh and calf vessels were better than those of NCE-MRA(P < 0.05 to P < 0.01).CE-MRA was set as the standard, 37 and 13 severe stenosis arteries were over and under estimated by NCE-MRA, respectively.The sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of NCE-MRA were 93.97%, 92.21%, 90.78%, 94.94% and 93.00%, respectively.The consistency of two methods in the diagnosis of severe arterial stenosis of the lower extremity was good(Kappa=0.859, P < 0.05).ConclusionsNCE-MRA is a safe and effective technique.The accuracy of NCE-MRA in showing severe stenosis of lower limb artery is high, which can be used as an alternative method for arteriography of lower extremity in patients with renal insufficiency or not suitable for contrast agents.

ObjectiveTo explore the morphological characteristics, bilateral and gender differences of the inferior frontal sulcus(IFS) in right-handed healthy adults, and provide the anatomical reference for the function study of Broca's area.MethodsThe transverse, coronal and sagittal planes of MRI images of 80 unilateral IFS in 40 right-handed healthy adults brain were statistically analyzed, and which was used to investigate the bilateral and gender differences.ResultsThe difference of the transverse layer number of IFS between two genders was not statistically significant(P>0.05).The Y value of right IFS was greater than that of left IFS in female, and the differences of the right Y value among other layers except for Z=18 mm layer were not statistically significant(P>0.05).The X value of left and right IFS in male was greater than that in female, and the differences of the X value at Z=2-10 mm, 14-16 mm and 20-26 mm layers of right IFS, and Z=2 mm, 10-16 mm and 24-30 mm layers of left IFS between male and female were statistically significant(P < 0.05 to P < 0.01).At Z=18-20 mm and Z=24 mm layers of left IFS, the Y value in male was greater than that in female(P < 0.01), and the differences of the Y value of other layers of left IFS between female and male were not statistically significant(P>0.05).ConclusionsThe closer to the brain middle axis the IFS, the smaller the acquired developmental variability of IFS is, and the larger the variabilities of IFS at the direction of forward-backward and left-right are.
Study on the risk factors of undiagnosed diabetes among young and middle-aged people in Bengbu city

Abstract:
ObjectiveTo investigate the incidence of undiagnosed diabetes, and its risk factors among young and middle-aged people in community of Bengbu city for providing the valuable information in the early detection, diagnosis and intervention of undiagnosed diabetes.MethodsOne thousand four hundred and twenty-four people without diabetes history from seven communities in Longzihu district of Bengbu were investigated.The information of somatometry and laboratory biochemical test was obtained by questionnaire.The fasting plasma glucose ≥ 7.0 mmol/L or HbA1c ≥ 6.5% were defined as diabetes.ResultsAmong 1 424 participants without diagnosed diabetes, 142 people were with undiagnosed diabetes.The results of multiple logistic regression analysis showed that the age increasing, hypertension, family history of diabetes, waist circumference increasing, obesity, eating less than 400 g of vegetables a day and no intaking fruit everyday were the independent risk factors of undiagnosed diabetes(P < 0.05 to P < 0.01).ConclusionsThe proportion of undiagnosed diabetes is high among young and middle-aged people in community of Bengbu.The controllable factors(including the waist circumference, obesity, smoking, daily consumption of vegetable and fruit are closely related to the incidence of undiagnosed diabetes.

ObjectiveTo investigate the cognitive function and its influencing factor in community cardiovascular and cerebrovascular disease(CCVD) patients(over 60 years old) during recovery period.MethodsThe chronic disease patients over 60 years old from 4 communities in Dragon Lake District in Bengbu City were investigated using Montreal Cognitive Assessment(MoCA) (Beijing edition).Seventy-nine questionnaires were collected, which included hypertension, coronary heart disease, dyslipidemia and stroke patients, and healthy people.According to the disease, the patients were divided into the CCVD group and control group.The difference of the cognitive function between two groups was compared, and the influencing factor of which was analyzed.ResultsThe difference of the MoCA score between two groups was not statistically significant (P>0.05).The analysis of the influencing factor of MoCA score showed that the score of MoCA was high in patients with high education level, and low drinking frequency(P < 0.01 and P < 0.05).ConclusionsThe education level and drinking frequency are the main risk factors of cognitive dysfunction.The drinking should be limited or prohibited, and the balanced diet and the healthy lifestyle should be advocated in patients with CCVD, which can achieve the primary prevention effect.

Abstract:
ObjectiveTo explore the effects of integrated management mode of "medical service-rehabilitation-nursing-elderly care" in improving the life quality and living ability of patients with stroke sequela.MethodsOne hundred and sixty-four patients with stroke sequelae in three pension agencies of Futian district of Shenzhen from the year 2015 to 2016 were randomly divided into the observation group and the control group(82 cases each group).The control group was nursed with the second level prevention and management, and the observation group was nursed with the integrated management mode of "medical service-rehabilitation-nursing-elderly care".The ability of daily living, quality of life and sport function between two groups before intervention and after 3 and 6 months were assessed and compared.ResultsAfter intervention, the scores of daily life ability and quality of life index in observation group were significantly improved compared with before intervention, and control group after intervention(P < 0.01).There was not statistical significance in the scores of daily life ability and quality of life index in control group between before and after intervention(P>0.05).The scores of Fugl-Meyer in two groups after intervention were better than that before intervention, and the improvement of which in observation group was significantly higher than that in control group(P < 0.01).ConclusionsThe integrated management mode of "medical service-rehabilitation-nursing-elderly care" can significantly improve the ability of daily living, quality of life and sport function of patients with stroke sequelae, which is helpful to the rehabilitation of patients, and worthy of further promotion.

Abstract:
ObjectiveTo analyze the effects of the cognitive interaction model of family member-patient on daily living recovery ability and cognitive level in patients with cerebral infarction.MethodsOne hundred and two patients with cerebral infarction were randomly divided into the observation group and control group according to the random number table(51 cases each group).The control group was nursed with traditional nursing care, and the observation group was nursed with the cognitive interaction model of family member-patient based on the control group.The activities of daily living, cognitive level and nursing satisfaction between two groups were compared before and after intervention.ResultsAfter intervention, the item score of daily living abilities and its total score in observation group were higher than those in control group(P < 0.05 to P < 0.01), the knowledge, belief and behavior scores in observation group were significantly higher than those in control group(P < 0.01), and the nursing satisfaction in observation group was higher than that in control group(P < 0.05).ConclusionsThe cognitive interaction model of family member-patient can improve the daily living ability, cognitive level and nursing satisfaction of patients with cerebral infarction, which is worthy of further promotion in clinical practice.
Application of path management based on multidisciplinary team collaboration in the transit of patients treated with anesthesia operation

Abstract:
ObjectiveTo investigate the application value of path management based on multidisciplinary team collaboration in the transit of patients treated with anesthesia operation, and provide the reference in optimizing the transfer process of surgical patients.MethodsTwo thousand and forty-six patients scheduled by operation from February to August 2016 and 1 965 patients from April to October 2015 were divided into the observation group and control group, respectively.The observation group and control group were treated with path management based on multidisciplinary team collaboration and conventional way in the transit of hospital, respectively.The transit time, incidence of nursing deficiency and satisfaction degree of department and families were compared between two groups.ResultsThe transit time in observation group was significantly lower than that in control group(P < 0.01).During the transportation, the rate of nursing deficiency in observation group(22.58%) was significantly lower than that in control group(27.99%)(P < 0.01).The receiving department satisfaction rate(including advance notice for ready, handover by transfer orders, introducing the operation and medication) in observation group was higher than that in control group(P < 0.05 to P < 0.01).There was no statistical significance in the unobstructed and fixed pipeline and assist resettlement between two groups(P>0.05).The satisfaction degree of the family members in observation group(92.99%) was significantly higher than that in control group(88.96%)(P < 0.01).ConclusionsThe path management based on multidisciplinary team collaboration can significantly shorten the transit time of patients treated with anesthesia operation, reduce the incidence of nursing deficiency, and improve the quality of nursing care.
Effect of homogeneous medical health education model on life style of patients with coronary heart disease after PCI

Abstract:
ObjectiveTo investigate the effects of homogeneous medical health education model on clinical outcome and quitting smoking in patients with coronary heart disease(CHD) after percutaneous coronary intervention(PCI).MethodsA total of 140 CHD patients with smoking after PCI were randomly divided into the observation group(70 cases) and control group(70 cases).The control group was nursed with routine nursing intervention, and the observation group was nursed with homogeneous medical health education model.The quitting smoking effect, diet compliance rate, physical examination indexes[triglyceride(TG), low density lipoprotein cholesterol(LDL-C) and target rate of movement] were compared between two groups.ResultsAfter intervention, the quitting smoking rate, target rate of quitting smoking and diet compliance rate in observation group were significantly higher than those in control group(P < 0.01).The improvement of systolic blood pressure, TG, abdominal circumference, body mass index and LDL-C in observation group were better than those in control group(P < 0.05 to P < 0.01).ConclusionsHomogeneous medical health education model after PCI is helpful to improve the bad living habits, quitting smoking rate and clinical prognosis of patients.
Analysis of the related risk factor of no-reflow in patients with acute inferior myocardial infarction during PCI

Abstract:
ObjectiveTo investigate the risk factors of no-reflow in patients with acute inferior myocardial infarction during percutaneous coronary intervention(PCI).MethodsThe data of 126 acute inferior myocardial infarction patients treated with PCI were investigated, and the related risk factors of no-reflow were analyzed.ResultsOne hundred and twenty-six patients with acute inferior myocardial infarction were successfully treated with PCI, no-reflow in 16 cases were found, and the incidence rate of which was 12.70%.Logistic regression analysis showed that the diabetes(OR=1.047, 95%CI:1.019 to 1.233, P=0.036), reperfusion time(OR=5.281, 95%CI:2.782 to 10.018, P=0.002), TIMI flow grading before PCI(OR=1.735, 95%CI:1.040 to 3.062, P=0.020) and low systolic blood pressure before PCI(OR=1.844, 95%CI:1.153 to 3.638, P=0.015) were the risk factors of no-reflow.ConclusionsThe incidence of no-reflow in acute inferior myocardial infarction patients is high.The effectively preventive measures against the related risk factor of no-reflow should be implemented, which can reduce the no-reflow risk of PCI.
Study on the relationship between insulin resistance and cognition impairment in middle-aged hypertensive patients

Abstract:
ObjectiveTo explore the relationship between insulin resistance and cognition impairment in middle-aged hypertensive patients.MethodsNinety-six middle-aged hypertensive patients were divided into the cognition impairment group(40 cases) and normal cognitive function group(56 cases) according to the score of mini-mental state examination scale.The general data of two groups were collected, the blood pressure and biochemical indicators between two groups were compared, and the influencing factors of cognition impairment were analyzed in middle-aged hypertensive patients.ResultsThe differences of the cultural level, insulin resistance index and diastolic blood pressure between two groups were statistically significant(P < 0.05).The results of multiple factors regression analysis showed that the cultural level and insulin resistance index were the independent influencing factors of cognition impairment in middle-aged hypertensive patients(P < 0.01 and P < 0.05).ConclusionsThe insulin resistance is the independent risk factor of cognition impairment in middle-aged hypertensive patients, which should be paid attention to.
