Study on the correlation between the plasma level of glutathione peroxidase 4 and clinical characteristics, risk stratification and prognosis of patients with acute coronary syndrome

Abstract:
ObjectiveTo explore the correlation between the plasma level of glutathione peroxidase 4(GPX4) and clinical characteristics, risk stratification and prognosis of patients with acute coronary syndrome(ACS).MethodsA total of 404 patients scheduled by coronary angiography were divided into the ACS group(n=316) and control group(n=88) according to the medical history, biochemical examination, results of imaging examination and coronary angiography.The ACS group was subdivided into the unstable angina pectoris group(UAP group, n=261) and acute myocardial infarction group(AMI group, n=55).According to TIMI risk stratification, the ACS patients were divided into the high-risk group(n=36), medium-risk group(n=227) and low-risk group(n=53).The plasma levels of GPX4 in all cases were determined by enzyme linked immunosorbent assay.The ACS group was followed up for an average of 21 months after discharge for major adverse cardiovascular events(MACEs), 4 cases were lost to follow-up, and the other patients were divided into the MACEs group(n=30) and non-MACEs group(n=282).The ACS group were divided into the GPX4≥101.90 ng/mL group(n=158) and GPX4 < 101.90 ng/mL group(n=158) according to the median GPX4 level of pateints.ResultsThe serum levels of GPX4 in ACS group were lower than that in control group(P < 0.05).The results of ROC curve showed that serum level of GPX4 could assist the diagnosis of ACS.The area under curve(AUC) was 0.828(0.778-0.878), the optimal cut-off value was 128.78 ng/mL, the specificity was 64.8%, and the sensitivity was 90.0%.In TIMI risk stratification, the serum level of GPX4 in low-risk group was higher than that in high-risk and medium-risk groups(P < 0.05).The results of ROC curve showed that the serum level of GPX4 could assist in determining whether ACS patients were in low-risk state in TIMI risk stratification, and the AUC was 0.665(0.577-0.754).The optimal cut-off value was 127.06 ng/mL, the sensitivity was 37.7%, and the specificity was 95.0%(P < 0.05).The level of GPX4 in MACEs group was lower than that in non-MACEs group(P < 0.05).There was no statistical significance in the survival time between GPX4 < 101.90 ng/mL group and GPX4≥101.90 ng/mL group in Kaplan-Meier curve.The serum level of GPX4 increasing was a protective factor for MACEs(P < 0.05).ConclusionsThe serum level of GPX4 has reference value in the clinical diagnosis, TIMI risk stratification and long-term prognosis of ACS patients.
Value of prealbumin in the prognostic evaluation of patients with chronic heart failure

Abstract:
ObjectiveTo explore the clinical value of prealbumin(PA) in the prognostic evaluation of patients with chronic heart failure(CHF).MethodsThe CHF patients were followed up for 180 days after discharge.According to whether major adverse cardiovascular events(MACE) occurred, the patients were divided into the MACE group and non-MACE group.The general data were compared between two groups, and the influencing factors of MACE were analyzed by multivariate logistic regression.A model was built to verify the trend change of MACE with the change of PA, and the receiver operating characteristic curve was drawn to determine the optimal critical value of influencing factors.ResultsA total of 213 patients were enrolled, which included 113 cases in the non-MACE group and 100 cases in the MACE group.The results of multivariate analysis showed that the serum level of PA decreasing and plasma N-terminal pro-B-type natriuretic peptide(NT-proBNP) level increasing were the independent risk factors of MACE(P < 0.01).According to the results of model 1(adjusting the age and body weight) and model 2(adjusting the age, body weight and NT-probNP) analysis, there was a trend in the risk of MACE at PA level(P < 0.01).The results of Kaplan-Meier curve showed that the lower the PA level was, the higher the incidence of MACE was(P < 0.01).When the PA was 191.95 mg/L, the area under the curve predicting MACE incidence was 0.810(95%CI: 0.753-0.868).ConclusionsThe serum level of PA decreasing is closely related to the poor prognosis of CHF patients.
Study on the effects of modified Millard′s method in the reconstruction of unilateral degree Ⅲ cleft lip evaluated by geometric principles

Abstract:
ObjectiveTo evaluate the improvement and stability of the modified Millard′s method in the reconstruction of lip-nasal deformity after unilateral degree Ⅲ cleft lip surgery.Methods Forty children with unilateral degree Ⅲ cleft lip treated with the modified Millard′s method from October 2018 to July 2020 were investigated.According to the same standard, the photos were collected before surgery, after 1 week and 1 year of operation.The coordinate system was constructed according to the same standard for the photos collected, the alar point(A), alar base point(B), lip peak point(C) and front cut point(D) of the healthy and affected sides were measured, and the coordinate area and symmetry rate of each measuring point were calculated.The difference analysis was made by comparing the area of healthy and affected sides at each time point, the overall change of symmetry rate over time was evaluated and the changes of symmetry rate between two time points were analyzed to assess the surgical improvement and stability.ResultsThe differences of the coordinate area at A, B, C and D points between the healthy side and affected side before operation were statistically significant(P < 0.01).After the modified Millard′s method surgery, the differences of the A, C and D points between the healthy side and affected side after 1 week of operation were statistically significant(P < 0.01), and the difference of which in B point was not statistically significant(P>0.05).After 1 year of operation, the difference of the coordinate area at A point between the healthy side and affected side was statistically significant(P < 0.01), but the differences of which at B, C and D points were not statistically significant(P>0.05).The symmetry rate of each measurement point after 1 week and 1 year of operation was higher than that before surgery(P < 0.01), and the symmetry rates of A and D points after 1 year of operation were higher than that after 1 week of operation(P < 0.01).ConclusionsThe modified Millard′s method in the reconstruction of unilateral degree Ⅲ cleft lip can significantly improve the lip-nasal deformity, and make the symmetry and stability better.
Significance of PNI and NLR for predicting initial efficacy and prognosis in the metastatic castration-resistant prostate cancer patients treated

Abstract:
ObjectiveTo explore the significance of prognostic nutritional index(PNI) and neutrophil to lymphocyte ratio(NLR) in the prediction of initial efficacy and prognosis of metastatic castration-resistant prostate cancer(mCRPC) patients treated with abiraterone acetate(AA) and prednisone.MethodsThe data of 40 mCRPC patients treated with AA combined with prednisone were collected using the cohort study method.The patients were followed up in outpatient clinics before and 1 month after treatment.Serum albumin, neutrophil count, and peripheral blood lymphocyte count of patients were detected and the corresponding PNI and NLR were further calculated.The influencing factors of initial efficacy of AA treatment were analyzed, and the factors associated with prostate-specific antigen(PSA), progression-free survival(PSA-PFS), radiological progression-free survival(rPFS), and overall survival(OS) were also screened.ResultsThe initial efficacy of AA treatment was significantly different between the median baseline PSA and baseline PNI values(P < 0.05).The initial efficacy of AA treatment was significantly different among the median PSA-PFS, median rPFS, and median OS survival indicators(P < 0.05).There were significant differences in body mass index of different grades between groups with different levels of PNI(P < 0.05).Gleason scores were significantly different between groups in PNI and NLR indexes(P < 0.05).Patients with median baseline PSA level and low baseline PNI value were more likely to develop PSA flare phenomenon during AA treatment(P < 0.05).Baseline PSA value, baseline PNI value, and baseline NLR value may be the influencing factors of initial efficacy of AA treatment(P < 0.05 to P < 0.01).ConclusionsPNI and NLR can be used as independent indicators to predict the prognosis of mCRPC patients treated with AA.Moreover, they may be important influencing factors of the initial efficacy of AA in mCRPC patients.
Analysis of safety and efficacy of perioperative esketamine in elderly patients undergoing hip arthroplasty

Abstract:
ObjectiveTo observe the safety and efficacy of perioperative esketamine in elderly patients with hip arthroplasty.MethodsNinety elderly patients scheduled for hip arthroplasty were randomly divided into group S and group E, 45 cases in each group.The patients in group S received ultrasonic-guided anterior lumbar and sacral plexus nerve block, and midazolam and sufentanil injection were slowly injected intravenously 5 minutes before skin cutting, and sufentanil injection was used as patient-controlled intravenous(PCIA) pump for postoperative analgesia.The patients in group E received ultrasonic-guided anterior lumbar and sacral plexus nerve block, and midazolam and esketamine were slowly injected intravenously 5 minutes before skin cutting, and esketamine was used as a PCIA pump for analgesia.The number of cases of body movement reaction during skin cutting(T1), the change value of MAP 5 min utes after skin cutting(T2)(△MAP) and the incidence of intraoperative respiratory depression were observed and compared between the two groups.The change of interleukin-6(IL-6) and cortisol(Cor) before operation(T0) and 6, 12, 24 h after operation(T3, T4, T5) were also tested.Visual analogue scale(VAS) and Ramsay sedation score at 6, 12, 24 h after surgery(T3, T4, T5), the changes of Mini-mentul State Examination(MMSE), the total number of PCIA compressions and the incidence of nausea and vomiting at 1 and 3 days after surgery(T5 and T6) were recorded.ResultsThe △MAP at T2 in group E was lower than that in group S(P < 0.05).There was no statistical significance in the incidence of the number of cases of body movement reaction at T1 and intraoperative respiratory depression between the two groups(P>0.05).There was no statistical significance in the concentration of IL-6 and Cor between the two groups before operation(P>0.05), however, both the concentration in group E was significantly lower than that of group S at each time point after operation(P < 0.01).There was no significant difference in VAS score between the two groups at T3(P>0.05), but the VAS score in group E was significantly lower than that in group S at T4 and T5(P < 0.01).Ramsay score in group E was significantly lower than that in group S at each time point(P < 0.01), and MMSE score of cognitive state at T5 and T6 was significantly higher than that in group S(P < 0.01).The total number of PCIA compressions and the incidence of nausea and vomiting in group E were lower than those in group S(P < 0.05).ConclusionsEsketamine used in elderly patients with hip arthroplasty can significantly reduce perioperative complications, effectively inhibit inflammatory response, provide good analgesic effect, and also have slight effect on postoperative cognitive function of patients.
Levels of BDNF, MIF and EGF in peripheral blood of patients with schizophrenia and their relationship with cognitive function

Abstract:
ObjectiveTo investigate the serum levels of brain-derived neurotrophic factor(BDNF), macrophage migration inhibitory factor(MIF) and epidermal growth factor(EGF) in patients with schizophrenia and their relationship with cognitive function.MethodsA total of 54 patients with schizophrenia were selected as the observation group, and 58 healthy people in the same period were selected as the control group.The levels of BDNF, MIF and EGF were detected by enzyme-linked immunosorbent assay, the patient′s clinical symptoms were evaluated by positive and negative syndrome scale(PANSS), the cognitive function was assessed by MATRICS consensus cognitive battery(MCCB), and the relationship between serum BDNF, MIF, EGF levels and cognitive function was evaluated by Pearson linear correlation analysis.ResultsThe level of BDNF in observation group was significantly lower than that in control group(P < 0.01), and the levels of MIF and EGF were significantly higher than those in control group(P < 0.01).BDNF level was negatively correlated with PNASS score(P < 0.05), MIF level was positively correlated with PNASS score(P < 0.05), and EGF level was not significantly correlated with PANSS score(P>0.05).The scores of connection test in observation group were significantly higher than those in control group(P < 0.01), and the scores of symbol coding, visual memory, verbal memory and semantic fluency in observation group were significantly lower than those in control group(P < 0.01).The serum levels of MIF and EGF in observation group were positively correlated with visual spatial memory(P < 0.05).ConclusionsThe serum levels of MIF and EGF are closely related to the clinical manifestations and some cognitive impairments in patients with schizophrenia.
Application effect of Bobath ball combined with VR somatosensory game on rehabilitation of children with cerebral palsy

Abstract:
ObjectiveTo explore the effect of Bobath ball combined with virtual reality(VR) somatosensory game on the rehabilitation of children with cerebral palsy(CP) and its effects on gross movement, balance function and three-dimensional gait space-time indexes.MethodsA total of 80 children with CP were randomly divided into control group and observation group according to the order of admission, with 40 cases in each group.The control group was given routine rehabilitation training, and the observation group was given Bobath ball+VR somatosensory game on the basis of the control group for 3 months.The gross movement, balance function(track length, track length per unit area, peripheral area), three-dimensional gait spatio-temporal indexes(stride, step length, dual support phase), surface electromyographic parameters of gastrocnemius muscle under passive activity and maximum isometric contraction of flexion and extension[integral electromyogram(iEMG), median frequency(MF)], and ability of daily living(ADL) were compared before intervention and 1 and 3 months after intervention.The satisfaction of family members of the two groups were compared.ResultsAfter 3 months of intervention, the gross movement scores of D and E areas in the observation group were significantly higher than those in the control group(P < 0.01);the scores of ADL in the observation group were significantly higher than those in the control group(P < 0.01).After 1 and 3 months of intervention, the track of unit area in the observation group were significantly higher than those in the control group(P < 0.01);the track length and peripheral area in the observation group were significantly less than those in the control group(P < 0.01);the stride and step length in the observation group were greater than those in the control group(P < 0.05 to P < 0.01), and the double support phase were significantly less than those in the control group(P < 0.01);the iEMG in the passive activity state was significantly lower than that in the control group(P < 0.01), and the iEMG and MF in the maximum isometric contraction state of flexion and extension were significantly greater than those in the control group(P < 0.01).The total satisfaction of family members of children in the observation group was higher than that in the control group(P < 0.05).ConclusionsBobath ball combined with VR somatosensory game can improve the gross movement and balance function of children with CP, reduce the muscle tension in passive state, increase the step length, reduce the time of bipedal support phase, and play a positive role in improving the ADL and family members′ satisfaction of children with CP.
Effect of complete arthroscopic debridement of calcified foci surgery on postoperative ASES, VAS and Constant-Murley scores in patients with rotator cuff calcific tendinitis of the shoulder

Abstract:
ObjectiveTo analyze the effects of complete arthroscopic debridement of calcified foci surgery on postoperative American shoulder and elbow scoring system(ASES), visual analogue scale(VAS) and Constant-Murley scores in patients with rotator cuff calcific tendinitis of the shoulder.MethodsThe clinical data of 63 patients with rotator cuff calcific tendinitis of the shoulder were collected.Among them, 31 patients undergoing complete arthroscopic debridement of calcified foci surgery were set as the observation group, and 32 patients undergoing partial arthroscopic debridement of calcified foci surgery were set as the control group.The operation time, hospitalization time, active maximum activity time, passive maximum activity time, and ASES, VAS and Constant-Murley scores before operation and at 1, 2, 6, and 12 months after operation were compared between the two groups.ResultsThe hospitalization time, active maximum activity time and passive maximum activity time in observation group were significantly longer than those in control group(P < 0.01).There was no significant difference in operation time between the two groups(P>0.05).The ASES scores in the two groups at 1, 2, 6, and 12 months after operation were higher than those before operation(P < 0.05), and the ASES scores in observation group at 1 month and 2 months after operation were significantly lower than those in control group(P < 0.01), but there was no significant difference in ASES scores between the two groups at 6 months and 12 months after operation(P>0.05).The VAS scores in the two groups at 1, 2, 6, and 12 months after operation were lower than those before operation(P < 0.05), and the VAS scores in observation group at 1, 2, and 6 months after operation were significantly higher than those in control group(P < 0.01), but there was no significant difference in VAS scores between the two groups at 12 months after operation(P>0.05).The Constant-Murley scores in the two groups at 1, 2, 6, and 12 months after operation were higher than those before operation(P < 0.05), and the Constant-Murley scores in observation group at 1 month and 2 months after operation were significantly lower than those in control group(P < 0.01), but there was no significant difference in Constant-Murley scores between the two groups at 6 months and 12 months after operation(P>0.05).ConclusionsThe application of complete arthroscopic debridement of calcified foci surgery in rotator cuff calcific tendinitis of the shoulder has the same effect in the long-term after operation on the improvement of ASES, VAS and Constant-Murley scores with partial arthroscopic debridement of calcified foci surgery, but the short-term effect is weaker than that of partial arthroscopic debridement of calcified foci surgery.The surgery method can be flexibly selected according to the specific conditions of patients in clinic.
Relationship between nutrition condition and quality of life in maintenance hemodialysis patients

Abstract:
ObjectiveTo investigate the correlation between nutrition condition and quality of life in maintenance hemodialysis(MHD) patients.MethodsA total of 102 patients with MHD were selected as the study subjects.The nutrition condition of patients was evaluated by the modified quantitative subjective global assessment(MQSGA).Kidney disease quality of life-36(KDQOL-36) questionnaires were used to evaluate the quality of life of patients, and the correlation between nutrition condition and quality of life was analyzed.ResultsAccording to MQSGA, there were 49 patients in normal nutrition group(48.04%) and 53 patients in malnutrition group(51.96%).No significant difference between the two groups were found in biochemical indexes(P>0.05).In terms of physical function, social function, physiological function, burden of kidney disease, influence of symptoms and influence of kidney disease, the score of malnutrition group was lower than that of normal nutrition group(P < 0.05).In addition, the score of social role dimension in normal nutrition group was lower than that in malnutrition group, and the score of overall health and other dimensions in normal nutrition group was significantly higher than that in malnutrition group(P < 0.01).Pearson correlation analysis showed that nutrition condition scores were negatively correlated with total quality of life scores(r=-0.378, P < 0.01) and their dimensions, such as general health, physical function, social function, the body pain, physiological function, kidney disease burden, symptoms, kidney disease affects(P < 0.05 to P < 0.01).ConclusionsThe nutrition condition of MHD patients is closely related to the quality of life.Therefore, the nutritional intervention should be considered as soon as possible to improve the quality of life of patients.
Comparative analysis of toxin clearance between hemodiafiltration and high-flux hemodialysis in uremia patients with chronic renal failure

Abstract:
ObjectiveTo compare the effects of toxin clearance between high-flux hemodialysis(HFHD) and hemodiafiltration(HDF) in uremia patients with chronic renal failure.MethodsA total of uremia 122 patients with chronic renal failure were investigated.The patients were divided into the control group and observation group according to the time of the first dialysis.The observation group and control group were treated with HDF and HFHD, respectively.The toxin clearance effects were compared between two groups, and the application effects of two methods were analyzed.ResultsThere was no statistical significance in the clearance rate of small molecule toxins between two groups(P>0.05).The large and medium molecule toxins clearance in two groups were improved after dialysis(P < 0.01), and the improvement in observation group was significantly better than that in control group(P < 0.01).There was no statistical significance in urea clearance time and standard protein catabolic rate index between two groups(P>0.05).The incidence rate of post-dialysis complications in observation group was significantly lower than those in control group(P < 0.01).ConclusionsHFHD on clearance of large molecule toxins in uremia patients with chronic renal failure has better effects, can reduce the incidence rate of complications, and is worthy of clinical application.
Effect of isotretinoin soft capsule combined with Yupingfeng capsule on the treatment of facia verruca plane with recombinant human interferon α-2b cream

Abstract:
ObjectiveTo explore the effects of the recombinant human interferon α-2b cream, isotretinoin capsules combined with Yupingfeng capsule in the treatment of facial verruca plana.MethodsThe control group was treated with recombinant human interferon α-2b cream three times a day, and the observation group was additionally treated with isotretinoin capsules and Yupingfeng capsules on the basis of the control group.In two groups, the treatment course was 8 weeks, and the curative effects were observed.ResultsAfter treatment, the scores of verruca plana count change in two groups were lower than that before treatment, the decreasing degree in the observation group was more obvious, and the difference of which was statistically significant(P < 0.01).The curative effect in observation group was better than that in control group(P < 0.01).The total effective rate in observation group was higher than that in control group(P < 0.05).There were not obvious adverse reactions in two groups.ConclusionsThe isotretinoin soft capsule combined with Yupingfeng capsule and external recombinant human interferon α-2b cream is an ideal and efficient treatment for facial verruca plana.
Application of GH/IGFs axis function detection in the etiological diagnosis of short stature children and its clinical significance

Abstract:
ObjectiveTo explore the clinical application value of growth hormone-insulin-like growth factors(GH/IGFs) axis function detection in the etiological diagnosis of short stature children.MethodsNinety children with short stature in the pre-puberty stage(P1 by Tanner stage) and 50 healthy children were divided into the observation group and control group, respectively.The levels of serum insulin-like growth factor-1(IGF-1) and insulin-like growth factor binding protein-3(IGFBP-3) in two groups were measured.The pathogenesis in the observation group were diagnosed using GH challenge test, and the observation group was divided into the complete GH deficiency(CGHD) group, partial GH deficiency(PGHD) group and idiopathic short stature(ISS) group.The sensitivity, specificity and accuracy of serum IGF-1 and IGFBP-3 in the diagnosis of short stature were analyzed.ResultsAccording to the results of GH challenge test, there were 22 patients in CGHD group, 36 patients in PGHD group and 32 patients in ISS group.The serum levels of IGF-1 and IGFBP-3 in CGHD group, PGHD group and ISS group were significantly lower than those in control group(P < 0.01), and the serum levels of IGF-1 and IGFBP-3 in CGHD group were significantly lower than those in PGHD group and ISS group(P < 0.01).With the serum level of IGF-1 < 52.8 ng/mL as the judgment value, the sensitivity, specificity and accuracy of CGHD diagnosis were 81.82%, 100.00% and 95.56%, respectively.With the serum level of IGFBP-3 < 3.12 μg/mL as the judgment value, the sensitivity, specificity and accuracy of CGHD prognosis were 100.00%, 75.00% and 81.11%, respectively.ConclusionsThe GH/IGF axis function detection has high clinical value in the etiological diagnosis of children with short stature.The detection of serum IGF-1 and IGFBP-3 may contribute to the primary screening of CGHD, and which combining with GH challenge test may be more helpful to the diagnosis of PGHD and ISS.

ObjectiveTo explore the effect of levonorgestrel-releasing intrauterine system(LNG-IUS) on serum estrogen, progesterone, inflammatory factors, and on endometrial tissue insulin-like growth factor Ⅰ(IGF-Ⅰ) and the related receptor(IGF-IR) in patients undergoing endometrial polyp(EP) electrotomy.MethodsA total of 100 EP patients were selected as the research objects, a random number table was used to divide them into observation group and control group, 50 cases in each group.The patients in control group were treated with conventional oral progesterone drugs after EP resection, and the patients in observation group were combined with LNG-IUS treatment after EP resection.The expression levels of serum estrogen receptor(ER), progesterone receptor(PR), C reactive protein(CRP), interleukin-6(IL-6), interleukin-10(IL-10) were examined, IGF-Ⅰ and IGF-ⅠR in endometrial tissue were detected and compared between the groups.ResultsWith the prolongation of treatment, the endometrial thickness and pictorial blood loss assessment chart score in the two groups were significantly reduced, and there were statistically significant differences between the groups at different time points(P < 0.01).After 6 months, the levels of ER and PR in the uterine gland cortex and interstitium in the observation group were lower than those in the control group at the same point(P < 0.05 to P < 0.01).After treatment, the relative expressions of various inflammatory indexes, IGF-Ⅰ and IGF-ⅠR mRNA in the two groups were significantly reduced, and they were significantly lower in the observation group than those in the control group(P < 0.05 to P < 0.01).After 6 months of follow-up, there was no significant difference in the complication occurrence rate between the two groups(P>0.05).ConclusionsAfter the resection of endometrial polyps, electrotomy, the LNG-IUS implantation can effectively reduce the levels of estrogen and progesterone in patients, relieve postoperative inflammation, and reduce IGF-Ⅰ level.It is worthy of clinical promotion.

Abstract:
ObjectiveTo study the relationship between platelet to lymphocyte ratio(PLR), neutrophil to lymphocyte ratio(NLR) and the prognosis of patients with biliogenic acute pancreatitis, and to compare their value in predicting the prognosis of biliogenic acute pancreatitis.MethodsThe clinical data of 338 patients with biliogenic acute pancreatitis diagnosed first time from January 2015 to January 2020 were retrospectively reviewed.Receiver operating curve(ROC) was used to determine the best critical value of PLR and NLR.The clinical features of patients with different levels of PLR and NLR were compared and the prognostic risk factors of biliogenic acute pancreatitis were explored.ResultsThe best critical values of PLR and NLR were 344.7 and 17.8, respectively according to the survival status of patients.The area under ROC curve(AUC) of PLR was bigger than NLR(Z=2.03, P < 0.05).Multivariate Cox regression model showed that the death risk of patients with low PLR was 3.551 times higher than that in patients with high PLR(95%CI: 1.161-10.868), and the risk of death in patients with high NLR was 2.769 times higher than that in patients with low NLR(95%CI: 1.039-7.397).ConclusionsPLR and NLR may be related to the prognosis of biliogenic acute pancreatitis.Compared with NLR, PLR is more accurate in predicting the prognosis of biliogenic acute pancreatitis.
Analysis of TCM syndrome types and related risk factors of diabetic nephropathy

Abstract:
ObjectiveTo explore the risk factors of the main traditional Chinese medicine(TCM) syndrome types in patients with diabetic nephropathy.MethodsA total of 130 patients with diabetic nephropathy were selected as the research subjects.The age, gender, body mass and other general information of the patients were collected through a questionnaire, and the information of the four diagnostic methods(inspection, auscultation and olfaction, inquiry, pulse-taking and palpation) was collected.The frequency and frequency distribution of TCM syndrome types of diabetic nephropathy were analyzed by expert differentiation of syndromes, and the risk factors of the main TCM syndrome types of diabetic nephropathy patients were analyzed.ResultsThe TCM syndrome types of diabetic nephropathy were mainly Qi-Yin deficiency syndrome(29.23%), spleen-kidney Yang deficiency syndrome(20.77%), Yin deficiency and dry heat syndrome(16.92%), and liver-kidney Yin deficiency syndrome(13.85%).The distribution frequency of each TCM syndrome type was in the following order: Qi-Yin deficiency syndrome >spleen-kidney Yang deficiency syndrome >Yin deficiency and dry heat syndrome >liver-kidney Yin deficiency syndrome > spleen-kidney Qi deficiency syndrome >blood stasis and toxin syndrome >Yin-Yang deficiency syndrome.There was no significant difference in the distribution of TCM syndrome types among diabetic nephropathy patients with different gender, sedentariness or not, and family history or not(P>0.05).There was significant difference in the distribution of TCM syndrome types among patients with different age, body mass index, smoking history and drinking history(P < 0.05).The results of logistic regression analysis showed that body mass index, smoking history and drinking history were the risk factors of Qi-Yin deficiency syndrome(P < 0.05 to P < 0.01), age and drinking history were the risk factors of spleen-kidney Yang deficiency syndrome(P < 0.01), body mass index was the risk factor of Yin deficiency and dry heat syndrome(P < 0.05), and smoking history was the risk factor of liver-kidney Yin deficiency syndrome in patients with diabetic nephropathy(P < 0.05).ConclusionsThe TCM syndrome types of diabetic nephropathy are mainly Qi-Yin deficiency syndrome, spleen-kidney Yang deficiency syndrome, Yin deficiency and dry heat syndrome, and liver-kidney Yin deficiency syndrome.The distribution of TCM syndrome types of diabetic nephropathy is related to body mass index, age, smoking history and drinking history.
Application of ESWL and F-URS in pediatric nephrolithiasis patients with different body mass

Abstract:
ObjectiveTo investigate the application value of extracorporeal shock wave lithotripsy(ESWL) and flexible ureteroscope(F-URS) lithotripsy in pediatric nephrolithiasis patients with different body mass.MethodsA total of 41 cases with single pediatric nephrolithiasis were included according to stone size body mass index(BMI) and personal intention.According to BMI, the patients were divided into overweight group(BMI ≥24.0 kg/m2, n=20) and non-overweight group(BMI < 24.0 kg/m2, n=21).The patients in the two groups were further divided into two subgroups including ESWL group(treated with ESWL) and F-URS group(treated with F-URS combined with holmium laser lithotripsy) according to the treatment method.The common postoperative complications(transient hematuria and postoperative fever), the success rate of one-time lithotripsy and the placement of double-J catheter after operation were compared between subgroups with different treatment method in overweight group and non-overweight group, respectively.ResultsIn the non-overweight group, the incidence of transient hematuria and postoperative fever, and the number of patients with double-J catheter in the ESWL group were lower than those in the F-URS group(P < 0.05), and there was no significant difference in the success rate of one-time lithotripsy between the two groups(P>0.05).In the overweight group, the success rate of one-time lithotripsy in the F-URS group was higher than that in the ESWL group(P < 0.05), and there was no significant difference in the incidence of transient hematuria and postoperative fever, and the number of patients with double-J catheter between the two groups(P>0.05).ConclusionsIn non-overweight patient with pediatric nephrolithiasis, ESWL and F-URS have similar therapeutic effects, which can be selected according to the patient's condition or hospital preference.In overweight patient with pediatric nephrolithiasis, the treatment effect of ESWL decreases, while F-URS has a stable lithotripsy effect and is more suitable for overweight patient.

Abstract:
ObjectiveTo investigate the relationship between the homocysteine(Hcy) level, ambulatory blood pressure variability(BPV) and left ventricular ejection fraction(LVEF) in elderly patients with hypertension.MethodsA total of 100 elderly patients with hypertension and 100 elderly patients without hypertension were divided into the observation group and control group, respectively.The serum Hcy level, ambulatory BPV index and LVEF between two groups were compared, and the correlation between Hcy, BPV index and LVEF were analyzed.ResultsThe serum Hcy level in observation group was significantly higher than that in control group(P < 0.01), the standard deviation of 24 h systolic blood pressure, day systolic blood pressure and night systolic blood pressure in observation group were significantly higher than those in control group(P < 0.01), there was no statistical significance in the diastolic blood pressure index between two groups(P>0.05), and the LVEF in observation group was significantly lower than that in control group(P < 0.01).The results of Pearson correlation analysis showed that the serum Hcy level was negatively correlated with LVEF(P < 0.05), and the standard deviation of 24 h systolic blood pressure, day systolic blood pressure and night systolic blood pressure were positively correlated with LVEF in elderly patients with hypertension(P < 0.05).ConclusionsCompared with the elderly group with normal blood pressure, the level of standard deviation of serum Hcy and ambulatory systolic blood pressure are significantly high, the level of LVEF is significantly low, and there is a correlation among the three.

ObjectiveTo investigate the gene locus of antihypertensive drug metabolizing enzymes CYP2C9*3(1075A>C), CYP2D6*10(100C>T), CYP3A5*3(6986A>G) and genotype distribution frequency of the drug target gene loci AGTR1(1166 A>C), ACE(I/D), ADRB1(1165G>C) and NPPA(2238T>C), to provide a theoretical basis for the personalized medication of five types of antihypertensive drugs including β-blocker, angiotensin receptor antagonist, angiotensin-converting enzyme inhibitor, calcium ion antagonist, and diuretics in hypertensive patients in Bengbu area of Anhui.MethodsThe PCR melting curve method was used to detect the gene locus polymorphism of drug in 879 hypertensive patients hospitalized from January 2020 to December 2020, and the typing frequency of each gene locus was analyzed.ResultsThe frequency of wild type, heterozygous mutant and homozygous mutant of AGTR1(1166A>C) gene locus in 879 hypertensive patients was 88.74%, 10.92% and 0.34%, respectively, which was 5.23%, 35.38% and 59.39% for ADRB1(1165G>C) gene locus, 40.73%, 43.12% and 16.15% for ACE(I/D) gene locus, 98.63 %, 1.37% and 0% for NPPA(2238T>C) gene locus, 91.81%, 7.96% and 0.23% for CYP2C9*3(1075A>C) gene locus, 25.94%, 45.73% and 28.33% for CYP2D6*10(100C>T) gene locus, and 7.17%, 39.70% and 53.13% for CYP3A5*3(6986A>G) gene locus.Among them, the distribution difference of genotype at CYP2C9*3(1075A>C) gene locus was statistically significant between male and female patients(P < 0.05), and the distribution difference of allele gene polymorphism of CYP2C9*3 and CYP2D6*10 was statistically significant between male and female patients(P < 0.05).ConclusionsThe data on the distribution frequency of hypertensive drug-related gene polymorphisms in the primary hypertension population in Bengbu area of Anhui have been obtained, which provide a basis for the establishment of a patient gene pool, and facilitate further study on the polymorphisms of related gene locus in future to guide rational drug use in clinic.
Comparison of the value of the Goodsall rule and biplane transrectal ultrasonography in positioning the internal opening of anal fistula

Abstract:
ObjectiveTo evaluate the value of the Goodsall rule in predicting internal opening of anal fistula and the value in locating the internal opening of anal fistula using biplane transrectal ultrasonog raphy.MethodsThe clinical data of 52 patients(70 anal fistulas) who underwent surgical treatment were retrospectively analyzed.The internal opening of anal fistula of each patient was predicted according to the Goodsall rule before surgery, and the internal fistula was simultaneously explored by biplane transrectal ultrasonography.Both results were compared with surgical exploration results via a consistency test.ResultsThe consistency rate of the Goodsall rule and operation diagnosis was 87.1%, and the Kappa value in the consistency test was 0.657, showing strong consistency(P < 0.01).The consistency rate of ultrasonic exploration and surgical diagnosis was 88.6%, and the Kappa value in the consistency test was 0.676, showing strong consistency(P < 0.01).Results of ultrasonic exploration of 70 anal fistulas were listed as follows: 12 superficial subcutaneous fistulas, 43 intersphincteric fistulas, 10 transsphincteric fistulas, 2 superior sphincteric fistulas and 3 horseshoe fistulas.The Goodsall rule had a high coincidence with ultrasonic exploration for superficial subcutaneous, intersphincteric anal fistulas and ultrasonic probe(90.1%).However, the prediction rate of the Goodsall rule for the internal opening of transsphincter type, horseshoe type, and suprasphincter type was not high(46.9%), and the Kappa value of consistency test was 0.486, showing moderate consistency(P < 0.05).ConclusionsThe value of the Goodsall rule in predicting the internal opening is highly consistent with surgery or biplane transrectal ultrasonography, showing a certain clinical value, especially for superficial subcutaneous fistula and intersphincteric anal fistulas, whereas there are some limitations in predicting the location of fistulas in front of the transverse line and the internal opening of horseshoe, transsphincter, and suprachincter fistulas.In ultrasound examination and clinical treatment, the Goodsall rule can be effectively combined with biplane transrectal ultrasonography to improve the diagnosis and treatment effect.
Clinical value of transvaginal sonography and hysteroscopy in the diagnosis of uterine abnormalities in infertility patients

Abstract:
ObjectiveTo explore the clinical value of transvaginal sonography(TVS) and hysteroscopy in the diagnosis of uterine abnormalities in infertiity patients.MethodsNinety infertility patients were examined using TVS and hysteroscopy.The hysteroscopic results were used as the gold standard to evaluate the efficacy of TVS in the diagnosis of uterine cavity abnormalities in infertility patients, the disease types of patients with uterine cavity abnormalities were counted, and the results of TVS in the diagnosis of different types of diseases were analyzed.ResultsAmong 90 infertility patients, 86 patients with uterine anomalies were found.The sensitivity, specificity, positive predictive value and negative predictive value of the effectiveness of TVS in diagnosing uterine abnormalities were 95.35%, 75.00%, 98.80% and 57.14%, respectively.The accuracy of TVS in diagnosing uterine cavity abnormalities was 94.44%, and there was no statistical significance between TVS and hysteroscopy(P>0.05).Among the patients with abnormal uterine cavity, there were 35 cases of submucosal fibroids, 29 cases of intrauterine polyps, 21 cases of intrauterine adhesions and 1 case of uterine malformation.The sensitivity, specificity, positive predictive value and negative predictive value of TVS in the diagnosis of submucosal myoma were 85.71%, 90.91%, 85.71% and 90.91%, respectively.The sensitivity, specificity, positive predictive value and negative predictive value of TVS in the diagnosis of uterine polys were 82.76%, 95.08%, 88.89% and 92.06%, respectively.The sensitivity, specificity, positive predictive value and negative predictive value of TVS in the diagnosis of intrauterine adhesions were 76.19%, 89.86%, 69.57% and 92.54%, respectively.The TVS diagnosis of uterine malformation was consistent with the hysteroscopic results.ConclusionsTVS has a high accuracy in diagnosing uterine abnormalities in infertility patients, which can provide a reference for doctors to judge the types of uterine abnormalities, and has important clinical application value.
Construction of evaluation index system for perioperative nursing quality of gastric cancer

Abstract:
ObjectiveTo construct an evaluation index system for perioperative nursing quality of gastric cancer based on enhanced recovery after surgery(ERAS), so as to provide reference for evaluating the perioperative nursing quality of gastric cancer.MethodsBased on the three-dimensional quality structure model, the first draft of the indicator system was drawn up on the basis of literature review and semi-structured interviews.The Delphi method was used to conduct two rounds of expert letter inquiries.ResultsThe effective recovery rates of the two rounds of questionnaires were 92% and 100%, the expert authority coefficients(Cr) were 0.866 and 0.874 respectively, and the Kendall coordination coefficient were 0.367 and 0.416 respectively(P < 0.01).The consistency test of each index weight showed that the coefficient of variation(CV) was < 25%, indicating good consistency.The final index system formed included 3 first-level indicators, 16 second-level indicators, and 33 three-level indicators.ConclusionsThe constructed evaluation index system for perioperative nursing quality of gastric cancer based on the concept of ERAS is scientific and reasonable, close to the clinic, and has special characteristics.It can effectively standardize the behavior of nurses and evaluate the perioperative nursing quality of gastric cancer.
Comparison of predictive effect of two neonatal early warning scores on transfer rate of newborns to neonatal intensive care unit

Abstract:
ObjectiveTo investigate the predictive effects of modified newborn early warning score(MNEWS) and newborn early warning score(NEWS) on the transfer of newborns to neonatal intensive care unit(NICU).MethodsMNEWS and NEWS were used to evaluate 328 newborns.According to the evaluation scores, the predictive effects of the two early warning scores on the transfer rate of newborns to NICU were analyzed.According to whether the children were admitted to NICU, they were divided into NICU group and general ward group.The scoring time of MNEWS and NEWS in the two groups were compared.ResultsThe total MNEWS score≥6 points, number of cases with single score ≥3 points and total score of newborns in NICU group were significantly higher than those in general ward group(P < 0.01), and the prediction rate of single score(87.50%) was significantly higher than that of total score(70.24%)(P < 0.01).The total NEWS score ≥2 points and total score of newborns in NICU group were significantly higher than those in general ward group(P < 0.01).The area under ROC curve(AUC), 95%IC, sensitivity and specificity of MNEWS for transfer of newborns to NICU were 0.899, 0.889-0.934, 89.28% and 81.13%, respectively.The AUC, 95% IC, sensitivity and specificity of NEWS for transfer of newborns to NICU were 0.712, 0.687-0.765, 69.12% and 76.65%, respectively.The scoring time of MNEWS to evaluate the scores of children on admission was(54.12±3.51) s, which was significantly longer than(49.15±3.25) s of NEWS(t=18.81, P < 0.01).ConclusionsBoth MNEWS and NEWS can predict the transfer of newborns to NICU.The index range of MNEWS is more detailed and reasonable, and MNEWS has a higher predictive effect on the transfer of newborns to NICU, which is worthy of clinical application.
Application of diabetes specialized nurse-led nursing group model in improving standardized management of diabetic hypoglycemia in non-endocrine wards

Abstract:
ObjectiveTo explore the application effect of diabetes specialized nurse-led nursing group model on the standardized management of diabetic hypoglycemia in non-endocrine wards.MethodsA hospital-level diabetes nursing team was established, consisting of 33 nurses(including 4 diabetes specialized nurses and 29 non-endocrine specialized nurses selected from the non-endocrine wards), of which the 4 diabetes specialized nurses were the core members.The management of 595 diabetic hypoglycemia cases in 29 non-endocrine wards from January to June 2020 were reviewed through the Jiahe electronic medical record system, and the existing problems were analyzed.The diabetes specialized nurse-led nursing group model was used to carry out homogenization training on the ability of nurses in non-endocrine wards to deal with hypoglycemia in a standardized manner.The standardized management ability of hypoglycemia(qualified rate of glucose rising food selection, qualified rate of 15-20 g glucose amount, implementation rate of hypoglycemia symptom evaluation, implementation rate of 15-minute retest of blood glucose), standardized rate of writing nursing records after hypoglycemia management, system reporting rate of hypoglycemia events and qualified rate of hypoglycemia knowledge were compared between January to June 2020(pre-implementation group) and August to December 2020(post-implementation group).ResultsAfter receiving the training and management of diabetes nursing group model in non-endocrine wards, the qualified rate of hypoglycemia knowledge and the ability of standardized management of hypoglycemia and so on were improved(P < 0.05 to P < 0.01), and the system reporting rate of hypoglycemic events was increased(P < 0.01).The incidence of hypoglycemia at all levels in the post-implementation group was significantly lower than that in the pre-implementation group(P < 0.01).ConclusionsThe diabetes specialized nurse-led nursing group model can improve the management ability of hypoglycemia in non-endocrine wards, improve the knowledge and management level of diabetic hypoglycemia of nurses in non-endocrine wards, and realize the safe management of blood glucose in the hospital to reduce the injury of diabetic hypoglycemia to patients.
Construction and validation of a prediction model for the risk of new-onset depression in stroke patients after admission

Abstract:
ObjectiveTo construct and validate a prediction model for the risk of new-onset depression in stroke patients after admission.MethodsA total of 916 stroke patients who without depression before admission were selected.These patients were divided into the new-onset depression group and the non-depression group according to the occurrence of depression in the first week after admission.Single-factor and multi-factor logistic regression models were used to analyze the influencing factors of new-onset depression and establish the prediction model.Then the nomogram was draw.Meanwhile, the area under the ROC curve and Hosmer-Lemeshow test were used to evaluate the prediction effect of the model.A total of 298 hospitalized patients from April 2020 to April 2021 were selected as the model validation group.ResultsThe incidence of new-onset depression in the model building group and validation group was 31.8% and 32.9%, respectively.Age, body mass index, education level, per capita annual household income, sleep quality, sleep duration, care after admission, Barthel index and NIHSS score were the influencing factors of new-onset depression in stroke patients after admission(P < 0.05).The validation results of the prediction model showed that the discrimination was good.The area under ROC curve of the risk of new-onset depression(P < 0.05) after admission was 0.868 in the model construction group and 0.813 in the validation group, respectively(P < 0.01).The calibration degree was also high, and the P values of Hosmer-Lemeshow test were 0.817 and 0.389, respectively.ConclusionsThe prediction model constructed can effectively predict the occurrence of new-onset depression in stroke patients after admission, providing a reference for timely psychological intervention in clinical nursing work for stroke patients.

ObjectiveTo investigate the effect of cross-theoretical model combined with empowerment education on self-management behavior and negative emotions in young and middle-aged patients with coronary heart disease after percutaneous coronary intervention(PCI).MethodsA total of 68 young and middle-aged patients with coronary heart disease undergoing PCI were selected and divided into the observation group(n=33) and control group(n=35) according to the random number table method.The control group was given routine health education, and the observation group was given cross-theoretical model combined empowerment education on the base of control group.The coronary heart disease self-management behavior scale and anxiety and depression rating scale were used to conduct a questionnaire survey, and the scores of each dimension of self-management behavior, anxiety and depression scores of patients in the two groups were compared.ResultsThere was no significant difference in the scores of each dimension of self-management between the two groups at discharge(P>0.05).At 3 months after discharge, there was no significant difference in the scores of emotional cognitive management dimension of self-management between the two groups(P>0.05), but the scores of other dimensions of self-management in observation group were higher than those in control group(P < 0.05 to P < 0.01).At 6 months after discharge, the scores of each dimension of self-management in observation group were significantly higher than those in control group(P < 0.01).There was no significant difference in the scores of anxiety and depression between the two groups at discharge(P>0.05).The scores of anxiety in observation group were lower than those in control group at 3 and 6 months after discharge(P < 0.05 to P < 0.01).The scores of anxiety depression in the two groups at 3 and 6 months after discharge were lower than those at discharge(P < 0.05), and the scores of anxiety at 6 months after discharge were lower than those at 3 months after discharge in observation group(P < 0.05).ConclusionsThe application of cross-theoretical model combined empowerment education in the health education of young and middle-aged patients with coronary heart disease after PCI can improve the patient's self-management ability, improve the patient's self-management behavior, and reduce the patient's negative emotions such as anxiety and depression, which is worthy of clinical promotion.
Mediating effect of mental resilience on self-care ability and discharge readiness of patients with early gastric cancer undergoing ESD

Abstract:
ObjectiveTo understand the discharge readiness of patients with early gastric cancer undergoing endoscopic submucosal dissection(ESD), and analyze the mediating effect of mental resilience on self-care ability and discharge readiness.MethodsA total of 210 patients with early gastric cancer undergoing ESD were selected as the research subjects, and surveyed with the general situation questionnaire, psychological resilience scale, self-care ability implementation scale and readiness for hospital discharge scale.The correlations between discharge readiness and self-care ability, mental resilience of patients with early gastric cancer undergoing ESD were analyzed, and the structural equation modeling was constructed to test the mediating effect of mental resilience on the self-care ability and discharge readiness of patients with early gastric cancer undergoing ESD.ResultsGender, age, education level, with caregiver or not, and social support were the influencing factors of discharge readiness of patients with early gastric cancer undergoing ESD(P < 0.01).The total score of discharge readiness of patients with early gastric cancer undergoing ESD was(88.55±8.76) points, the total score of self-care ability was(97.45±11.83) points, and the total score of mental resilience was(51.98±7.87) points.Except that there was no significant correlation between adaptive ability and self-concept(P>0.05), the self-care ability of patients with early gastric cancer undergoing ESD was positively correlated with the total score of discharge readiness and the score of its each dimension(P < 0.05 to P < 0.01);there was a positive correlation between self-care ability and the total score of mental resilience and the score of its each dimension(P < 0.05 to P < 0.01);there was a positive correlation between discharge readiness and the total score of mental resilience and the score of its each dimension(P < 0.05 to P < 0.01).Mental resilience had a 42.37% mediating effect on discharge readiness and self-care ability.ConclusionsSelf-care ability can not only directly and positively predict discharge readiness, but also indirectly predict discharge readiness through mental resilience.Clinical medical staff should pay attention to the importance of communication, promote the patient to maintain a positive and optimistic psychological state, and help to improve the patient's discharge readiness level.
Effects of CCT and MSST combined with staged intervention on cognitive function in adolescent patients with schizophrenia

Abstract:
ObjectiveTo explore the effects of compensatory cognitive training(CCT) and medication self-management skills training(MSST) combined with staged intervention on cognitive function in adolescent patients with schizophrenia.MethodsA total of 59 cases of adolescent schizophrenia patients were selected as research objects, who were randomly divided into observation group(n=29) and control group(n=30).The control group took routine treatment intervention, and the observation group took MSST+CCT combined staged intervention.The changes of cognitive function, mental symptoms and medication compliance before and after intervention were compared between the two groups.ResultsIn the observation group, 7 patients(24.1%) had poor compliance with cognitive strategy exercise, and 7 patients(24.1%) had poor compliance with drug management.The length of hospital stay was positively correlated with the total number of days of taking medication(r=0.493, P < 0.01).There was a positive correlation between the total number of days of taking medication and the total number of days of CCT practice measurement(r=0.379, P < 0.05).The total number of CCT practice strategies was positively correlated with the total number of practice measurement days(r=0.450, P < 0.05).Before intervention, there were no significant differences in the total score of positive and negative symptom scale(PANSS), total score of each subscale, total score of min-mental state examination(MMSE), and total score of medication compliance between the two groups(P < 0.05).After the course, the total score of PANSS in the observation group was lower than that in the control group(P < 0.05), there were no significant differences in PANSS subscales scores, MMSE total scores and medication compliance total scores(P < 0.05).Three months after intervention, the total scores of PANSS and its subscales, MMSE and medication compliance in the observation group were better than those in the control group(P < 0.05 to P < 0.01).There were statistically significant differences between the two groups at each time point before and after intervention(P < 0.05 to P < 0.01), the PANSS total positive symptom scores of the observation group at 3 months after intervention were lower than those before intervention(P < 0.05 to P < 0.01), PANSS total positive symptom scores of the control group at 3 months of intervention were lower than those before intervention and after the end of the course(P < 0.01);the negative symptoms in the observation group were lower 3 months after intervention than before and after the course(P < 0.01), the negative symptoms in the control group were lower than those before intervention 3 months after intervention(P < 0.01);general psychiatric symptoms in both groups were lower 3 months after intervention than before and after the course(P < 0.05 to P < 0.01), MMSE total scores increased compared with those before and after intervention(P < 0.01);the scores of medication compliance in both groups at 3 months after intervention were lower than those before intervention(P < 0.01), 3 months after intervention, the compliance score of the observation group continued to decrease compared with that after the course(P < 0.01), and the amplitude of decrease and increase in observation group was higher than that in control group(P < 0.05 to P < 0.01).ConclusionsThe combined intervention of CCT and MSST can better improve the cognitive function, psychotic symptoms and medication compliance of adolescent schizophrenia, which is worthy of clinical application.
Construction and effect evaluation of the intervention model of psychosocial adaptation in patients with coronary heart disease

Abstract:
ObjectiveTo establish a psychosocial adaptation intervention model in patients with coronary heart disease and angina pectoris, and evaluate its application effects.MethodsA total of 78 patients with angina pectoris were selected as the research objects, and randomly divided into the study group(40 cases) and control group(38 cases).The control group was treated with conventional intervention, and the study group was treated with psychosocial adaptation intervention based on the control group.The psychosocial adaptation was divided into three stages: confusion reflection period, adaptation and reconstruction period, acceptance integration period, and the targeted intervention programs were provided.ResultsAfter 6 months, the scores of self-report psychosocial adjustment to illness scale in two groups decreased significantly(P < 0.01), and which in study group was significantly lower than that in control group(P < 0.01).After 6 months, the scores of health care, sexual life, communication, entertainment and psychological status in study group were lower than those in control group(P < 0.05 to P < 0.01).After 6 months, the brief illness perception questionnaire scores in two groups decreased significantly(P < 0.01), the social support scale scores increased significantly in two groups(P < 0.01), and the brief illness perception questionnaire score and social support scale score in study group were significantly lower than those in control group(P < 0.01).After 6 months, the retest results showed that the positive coping score and general self-efficacy scale score in study group were higher than those in control group(P < 0.05), while the negative coping score in study group was lower than that in control group(P < 0.01).The positive coping score and general self-efficacy scale score were higher than that of the baseline(P < 0.05 to P < 0.01), and the negative coping score was lower than that of the baseline in two groups(P < 0.05 to P < 0.01).ConclusionsThe psychosocial adaptation intervention model can improve the disease coping style, disease cognition and self-efficacy of patients with angina pectoris, and promote the psychosocial adaptation of patients.

ObjectiveTo explore the application effect of early exercise rehabilitation program based on process index of nursing sensitive index in the early motor function and neurological rehabilitation of stroke patients.MethodsSixty stroke patients were selected and randomly divided into the observation group and control group, with 30 cases in each group.The control group implemented routine nursing, and the observation group implemented the nursing process index monitoring of "nursing program for early motor rehabilitation of stroke patients" on the base of the control group.The scores of National Institutes of Health stroke scale(NIHSS) and Fugl-Meyer motor function assessment(FMA) were compared between the two groups before and after intervention.ResultsThere was no significant difference in NIHSS score between the two groups before intervention(P>0.05);the NIHSS score in the two groups after intervention was significantly lower than that before intervention(P < 0.01), and the NIHSS score in the observation group after intervention was significantly lower than that in the control group(P < 0.01).There was no significant difference in FMA score between the two groups before intervention(P>0.05);the FMA score in the two groups after intervention was significantly higher than that before intervention(P < 0.01), and the FMA score in the observation group after intervention was significantly higher than that in the control group(P < 0.01).ConclusionsWith the help of the process index of nursing quality-sensitive index and under the guidance of the early exercise nursing program of stroke patients, it is helpful to restore the neurological and motor function of stroke patients and improve the clinical outcome of patients.
Effect of graded walking exercise program on cancer-related fatigue in patients with lung cancer undergoing chemotherapy

Abstract:
ObjectiveTo study the effect of graded walking exercise program on cancer-related fatigue(CRF) in patients with lung cancer undergoing chemotherapy.MethodsA total of 90 patients with lung cancer undergoing chemotherapy were selected as the research subjects and randomly divided into the intervention group and control group, with 45 cases in each group.The control group was treated with routine chemotherapy guidance, and the intervention group was treated with graded walking exercise program for a total of 8 weeks of intervention.The amount of walking exercise, CRF degree distribution, cancer-related fatigue scale(CFS) score and walking distance of 6-minute walking test(6MWT) were compared between the two groups.ResultsThe amount of walking exercise in the intervention group was(8 295±912) steps/day, which in the control group was(3 102±1 113) steps/day, and the difference of which was statistically significant(P < 0.01).There was no significant difference in CRF degree distribution between the two groups before chemotherapy and 4 weeks after intervention(P>0.05), and the CRF degree distribution in the intervention group was significantly better than that in the control group at 8 weeks after intervention and 3 months after intervention(P < 0.01).There was no significant difference in CFS scores between the two groups before chemotherapy and 4 weeks after intervention(P>0.05), and the CFS score in the intervention group was significantly lower than that in the control group at 8 weeks after intervention and 3 months after intervention(P < 0.01).The CFS score in the intervention group gradually decreased after the intervention(P < 0.01), and which in the control group gradually increased after the intervention(P < 0.01).There was no significant difference in 6MWT walking distance between the two groups before chemotherapy and 4 weeks after intervention(P>0.05), and the 6MWT walking distance in the intervention group at 8 weeks after intervention and 3 months after intervention was greater than that in the control group(P < 0.05 and P < 0.01).The 6MWT walking distance in the intervention group did not change significantly from pre-chemotherapy to the eighth week of intervention(P>0.05), but the 6MWT walking distance at 3 months after the intervention was higher than that before the intervention, 4 weeks after the intervention and 8 weeks after the intervention(P < 0.05).The change of 6MWT walking distance in the control group showed a downward trend during the intervention(P < 0.01).ConclusionsThe graded walking exercise program can reduce CRF in patients with lung cancer undergoing chemotherapy, effectively increase the 6MWT walking distance after intervention, and the effect is obvious at the beginning of 8 weeks of intervention.
Mediating effect of self-perceived burden on family support and health behavior in maintenance hemodialysis patients

Abstract:
ObjectiveTo explore the mediating effect of self-perceived burden on family support and health behavior in hemodialysis patients.MethodsA total of 96 hemodialysis patients were selected.The general information questionnaire, self-perceived burden scale, family adaptability and cohesion evaluation scale Ⅱ, health promoting lifestyle profile scale -Ⅱ were distributed.Sequential regression test and classical ratio method were used to investigate the mediating effect of self-perceived burden on family health behavior and support function.ResultsFamily support of hemodialysis patients had significant predictive value for health behavior(β=0.542, t=6.14, P < 0.01).Self-perceived burden score showed significant reverse predictive value for family support(β=-0.582, t=-7.21, P < 0.01).Taking health behavior as the dependent variable, the inclusion of self-perceived burden made the predictive value of family support to health behavior decline, but it was still significant(β=0.322, t=3.99, P < 0.01), and self-perceived burden predicted the health behavior of patients(β=-0.268, t=-4.05, P < 0.01).ConclusionsSelf-perceived burden has a mediating effect on health behavior and family support.Intervening family support can improve self-perceived burden in patients.

Abstract:
ObjectiveTo explore the teaching effects of a novel online teaching mode based on mind mapping in the clinical medicine undergraduates.MethodsA total of 147 students from 2 classes of clinical medicine major who received clinical medicine course theory teaching were selected as the research objects.Among them, 91 students from clinical medicine class and 56 students from clinical medicine experimental class were divided into the traditional teaching class and experimental teaching class, respectively.After the implementation of teaching, questionnaire surveys were conducted, which included the cognitive feelings of current teaching mode, acceptance of new teaching methods and concepts and evaluation of the effects of teaching mode.ResultsCompared with the traditional teaching class, the satisfaction of current teaching mode was lower(P < 0.05), the interactive teaching method combined with explanation was more(P < 0.05), and the awareness rates of "learning pyramid" and learning group or learning community were higher in the experimental teaching class(P < 0.01).The awareness of the new tools to improve learning efficiency in two classes were relatively high, and the expectations for the mastery of professional knowledge, clinical skill improvement, cooperation awareness training and logical thinking enhancement in teaching effects in two classes were high, but the differences of which between two classes were not statistically significant(P>0.05).The classroom thinking enthusiasm, concentration and mastery in experimental class were better than those in traditional class(P < 0.05), the proportion of teaching helpful to clinical practice in experimental teaching teaching class was significantly higher than that in traditional teaching teaching class(P < 0.01), and the proportion of intention to try to construct knowledge system with mind mapping in experimental teaching class was higher than that in traditional teaching class(P < 0.05).ConclusionsThe new online teaching mode based on mind mapping is helpful to improve the students' enthusiasm in class and teaching effects, which is worthy of further improvement and promotion.

ObjectiveTo explore the teaching effects of integrated medical teaching mode in the standardized training of emergency medical residents.MethodsFifty trainees from the standardized training base for emergency medical residents in The First Affiliated Hospital of Bengbu Medical College were selected and randomly divided into the observation group and control group, with 25 trainees in each group.The traditional teaching method was used in the control group and the integrated teaching method was used in the observation group.The comprehensive evaluation was carried out by the combination of questionnaire survey and practical skill assessment.The clinical skill examination results and satisfaction evaluation of teaching effect in the two groups were compared.ResultsThe scores of debridement and suture, gastric lavage, endotracheal intubation, nasogastric tube intubation, cardiopulmonary cerebral resuscitation and case analysis ability of the trainees in the observation group were significantly higher than those in the control group(P < 0.01).The satisfaction scores of teaching effect in health awareness, first aid skill, scientific research and innovation ability, communication ability, problem solving and analyzing ability, knowledge mastery, stimulating learning interest and improving clinical thinking ability of the trainees in the observation group were significantly higher than those in the control group(P < 0.01).ConclusionsThe application of integrated medical teaching mode in the standardized training of emergency medical residents is helpful to improve the trainees' scientific research and innovation ability, first aid skill, knowledge mastery, communication ability and clinical thinking ability, and the teaching effect is significantly improved.
