Study on the role of miRNA-200c/SOX2 axis in alveolar bone resorption in experimental periodontitis

Abstract:
ObjectiveTo investigate the role of miRNA-200c/sex determining region Y-box 2 (SOX2) axis in alveolar bone resorption in experimental periodontitis.MethodsC57BL/6 mice were randomly divided into blank group, model group, miRNA-200c group and combination group.Mice in the model group, miRNA-200c group and combination group were given lentiviral empty vector, miRNA-200c lentiviral overexpression vector, miRNA-200c plus SOX2 lentiviral overexpression vector, respectively.Chronic mice periodontitis models were established by smearing Porphyromonas gingivalis in the oral cavity of mice in model group, miRNA-200c group and combination group.The expression levels of miRNA-200c and SOX2, gingival index, and distance from the cementoenamel junction (CEJ) to the alveolar bone crest (ABC) were determined in the periodontal tissues of mice in all groups.HE staining was used to analyze the pathomorphological changes of periodontal tissues of mice.The expression levels of miRNA-200c and SOX2 in periodontal tissues of mice were detected by qPCR.The protein expression levels of osteoprotegerin (OPG), receptor activator of nuclear factor-κB ligand (RANKL), and receptor activator of nuclear factor-κB (RANK) in periodontal tissues of mice were detected by Western blotting.ResultsCompared with the blank group, the expression level of miRNA-200c in the periodontal tissue of the model group decreased (P < 0.05), and the expression level of SOX2 increased (P < 0.05).The expression level of miRNA-200c in the periodontal tissue of the miRNA-200c group was higher than that of the blank group and model group, and the expression level of SOX2 was lower than that of the model group (P < 0.05).The expression levels of miRNA-200c and SOX2 in the periodontal tissue of the combination group were higher than those of the blank group and model group (P < 0.05).The gingival index and the distance from CEJ to ABC in the model group, miRNA-200c group and combination group were higher than those in the blank group (P < 0.05), which in the miRNA-200c group were lower than those in the model group (P < 0.05), and which in the combination group were higher than those in the miRNA-200c group (P < 0.05).The results of Western blotting showed that the expression levels of RANKL and RANK, and the ratio of RANK/OPG in the model group, miRNA-200c group and combination group were higher than those in the blank group (P < 0.05), which in the miRNA-200c group were lower than those in the model group (P < 0.05), and which in the combination group were higher than those in the miRNA-200c group (P < 0.05).The results of HE staining showed that a large number of inflammatory cells infiltrated in the periodontal tissue of mice in the model group, and collagen fibers were disordered, denatured and broken; compared with the model group, these symptoms in the miRNA-200c group were significantly alleviated; compared with the miRNA-200c group, mice in the combination group also showed significant inflammatory cell infiltration, and collagen fibers were disordered, denatured and broken.The detection of luciferase activity showed that miRNA-200c mimic could significantly reduce the activity of wild-type SOX2 luciferase (P < 0.01), but there was no significant difference in the activity of mutant SOX2 luciferase (P>0.05).ConclusionsMiRNA-200c can improve the inflammatory response and alveolar bone resorption in mice with periodontitis, which may be related to the RANKL/RANK/OPG axis regulated by SOX2.
Down-regulation of MTHFD2 inhibits invasion and metastasis in prostate cancer PC3 cells

Abstract:
ObjectiveTo investigate the effect of mitochondrial methylenetetrahydrofolate dehydrogenase 2 (MTHFD2) on cell invasion and metastasis in prostate cancer PC3 cells.MethodsThe prostate cancer PC3 cells were divided into 3 groups: blank control group (ctrl group), negative control group (si-NC group) and transfected siRNA-MTHFD2 group (si-MTHFD2 group).The mRNA expression of MTHFD2 in human normal prostate epithelial cells RWPE-1 and prostate cancer PC3 cells was detected by qRT-PCR.The apoptosis rate in different groups was detected by flow cytometry, the changes of cell migration ability were detected by scratch test, the changes of cell invasion ability were detected by Transwell test, and the changes of E-cadherin, Vimentin and N-cadherin protein expressions were detected by Western blotting.ResultsThe expression level of MTHFD2 in prostate cancer PC3 cells was significantly higher than that in normal prostate epithelial cells(P < 0.05), and the mRNA expression level of MTHFD2 in si-MTHFD2 group was significantly lower than that in ctrl and si-NC groups(P < 0.05).The results of cell scratch and invasion assay showed that the migration and invasion abilities in si-MTHFD2 group was significantly lower than in ctrl and si-NC groups(P < 0.05).E-candherin protein expression level in si-MTHFD2 group was significantly higher than that in ctrl and si-NC groups(P < 0.05), and the protein expression levels of vimentin and N-cadherin were significantly lower than those in ctrl and si-NC groups.ConclusionsDown-regulation of MTHFD2 expression can increase the apoptosis of prostate cancer cells and inhibit the migration and invasion of prostate cancer cells.
FGD5-AS1/miR-519d-3p axis regulates high glucose-induced inflammatory factor expression and apoptosis in renal tubular epithelial HK-2 cells

Abstract:
ObjectiveTo investigate the effect of FGD5-AS1 on high glucose-induced tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) expression and apoptosis in renal tubular epithelial HK-2 cells and its mechanism.MethodsAfter HK-2 cells were induced with 25 mmol/L glucose for 24 h, the expressions of FGD5-AS1 and miR-519d-3p in the cells were detected by RT-qPCR.After HK-2 cells were transfected with FGD5-AS1 overexpression vector(pcDNA-FGD5-AS1) and miR-519d-3p inhibitor, or co-transfected with pcDNA-FGD5-AS1 and miR-519d-3p mimic, respectively, they were induced with 25 mmol/L glucose for 24 h, and then the levels of TNF-α and IL-6 in the cell culture supernatant were detected by ELISA, and the apoptosis rate and the expression of apoptosis-related proteins cleaved-caspase3 and cleaved-caspase9 were detected by flow cytometry and Western blotting, respectively.The regulatory relationship between FGD5-AS1 and miR-519d-3p was verified by the dual-luciferase reporter gene assay.ResultsCompared with the control group, the expression of FGD5-AS1 in HK-2 cells induced by high glucose significantly decreased (P < 0.01), and the expression of miR-519d-3p significantly increased (P < 0.01).After FGD5-AS1 was up-regulated or miR-519d-3p was down-regulated in HK-2 cells, and the cells were induced by high glucose, the levels of inflammatory factors TNF-α and IL-6, apoptosis rate, expression of apoptosis-related proteins cleaved-caspase3 and cleaved-caspase9 were decreased (P < 0.05 to P < 0.01).FGD5-AS1 could targetedly bind to miR-519d-3p, and the expression of miR-519d-3p was significantly decreased in HK-2 cells with up-regulated FGD5-AS1 (P < 0.01).Overexpression of miR-519d-3p reversed the effect of up-regulating FGD5-AS1 expression on the expression of inflammatory factors TNF-α and IL-6, and apoptosis in HK-2 cells induced by high glucose.ConclusionsUp-regulation of FGD5-AS1 may inhibit high glucose-induced expression of inflammatory factors and apoptosis in HK-2 cells through targeted down-regulation of miR-519d-3p.
Nrf2/HO-1 pathway improves anoxia/reoxygenation-induced injury in murine cardiomyocytes via regulating endoplasmic reticulum stress

Abstract:
ObjectiveTo discuss the effect of Nrf2/HO-1 up-regulation on improving anoxia/reoxygenation-induced injury in murine cardiomyocytes and its related mechanism.MethodsThe primary murine cardiomyocytes were divided into normal culture treatment and anoxia/reoxygenation treatment, and the anoxia/reoxygenation treated cardiomyocytes were divided into model group, Nrf2 overexpression group, HO-1 overexpression group, HO-1 silence+Nrf2 overexpression group.Cell viability was detected by MTT assay, apoptosis was analyzed by TUNEL staining, cardiac troponin Ⅰ(cTnⅠ) and creatine kinase isoenzyme MB(CK-MB) contents in cell culture supernatant were determined by ELISA, and levels of endoplasmic reticulum stress-related molecules were measured by fluorescence quantitative PCR.ResultsAfter anoxia/reoxygenation treatment, the activity decreased, apoptosis increased, and the contents of cTnⅠ and CK-MB in cell culture supernatant of murine cardiomyocytes increased(P < 0.05).Overexpression of Nrf2 or HO-1 could effectively alleviate anoxia/reoxygenation-induced injury in murine cardiomyocytes(P < 0.05).After pretreatment with HO-1 gene silencing, the protection effect of Nrf2 overexpression was significantly weakened(P < 0.05).In addition, after anoxia/reoxygenation treatment, the mRNA expressions of GRP78, ATF6, CHOP and Caspase-3 in cardiomyocytes increased(P < 0.05).Overexpression of Nrf2 or HO-1 resulted in down-regulation of the expression levels of GRP78, ATF6, CHOP and Caspase-3(P < 0.05), while down-regulation of HO-1 level reduced the inhibitory effect of Nrf2 overexpression on endoplasmic reticulum stress signal transduction(P < 0.05).ConclusionsThe up-regulation of Nrf2/HO-1 pathway can effectively improve anoxia/reoxygenation-induced injury in cardiomyocytes, which may be related to the suppression of endoplasmic reticulum stress and its downstream apoptosis signaling transduction.
Cisplatin increases nuclear translocation of Nrf2 and promotes autophagy and apoptosis in HeLa human cervical cancer cells

Abstract:
ObjectiveTo investigate the effect of cisplatin on the protein expression and nuclear translocation of Nrf2, and on the changes of autophagy and apoptosis in HeLa human cervical cancer cells.MethodsAfter the HeLa human cervical cancer cells were treated with different concentrations of cisplatin(0, 2.5, 5 and 10 μmol/L), MTT assay was used to detect the cell activity, Transwell assay was used to observe the cell migration ability, mCherry-GFP-LC3B fusion protein adenovirus transfected HeLa cells were applied to observe the autophagy changes, flow cytometry was employed to detect the apoptosis, Western blotting was used to detect the expression of Nrf2 in the cytoplasm and nucleus, and immunofluorescence staining was used to observe the nuclear translocation of Nrf2.ResultsCompared with the control group, 2.5, 5 and 10 μmol/L cisplatin could inhibit the activity and migration ability of HeLa cells(P < 0.05), and the inhibitory effect increased with the increase of drug concentration(P < 0.05).Compared with the control group, 2.5, 5 and 10 μmol/L cisplatin could promote the expression of LC3B, apoptosis and nuclear translocation of Nrf2 in HeLa cells(P < 0.05), reduce the protein expression level of Nrf2 in the cytoplasm(P < 0.05), and increase the protein expression level of Nrf2 in the nucleus(P < 0.05).ConclusionsCisplatin promotes autophagy and apoptosis by increasing nuclear translocation of Nrf2 in HeLa human cervical cancer cells, and inhibits cell viability and migration ability.
Anti-β2GPⅠ antibody promotes uptake of oxLDL by macrophages to accelerate the progression of atherosclerosis

Abstract:
ObjectiveTo investigate the role of anti-β2-glycoprotein Ⅰ(β2GPⅠ)antibody in the relationship between macrophage uptake and atherosclerosis in apoE knockout (apoE-/-) mice.MethodsThe male apoE-/- mice were randomly divided into the anti-β2GPⅠ antibody group and model group, with 8 mice in each group, and fed with high-fat diet for 8 weeks.The mice in the anti-β2GPⅠ antibody group were intraperitoneally injected with anti-β2GPⅠantibody, and the mice in the model group were intraperitoneally injected with the same dose of homologous control IgG antibody diluted with 0.9% sodium chloride solution.After 8 weeks, the mice were dissected, and the aortic root tissues were isolated for Movat staining and immunohistochemical staining to observe the histopathological characteristics of atherosclerotic plaques in the aortic root; Western blotting was used to detect the expression of marker proteins of endoplasmic reticulum stress and macrophage in aortic plaques; the peritoneal macrophages were cultured in vitro, the anti-β2GPⅠ antibody group and model group were stimulated with oxidized low-density lipoprotein (oxLDL) plus anti-β2GPⅠ antibody and oxLDL for 24 hours, respectively, to observe the uptake of oxLDL by macrophages.ResultsMovat staining showed that the aortic plaque increased, the plaque area was larger, and the contents of collagen fibers and smooth muscle fibers were decreased in the anti-β2GPⅠ antibody group compared with the model group (P < 0.05 to P < 0.01).Immunohistochemical staining indicated that the area of macrophages in the aortic root plaque in the anti-β2GPⅠ antibody group was larger than that in the model group (P < 0.05).The results of Western blotting displayed that the expression levels of marker proteins of endoplasmic reticulum stress pathway such as IRE1-α, p-IRE1-α, ATF6, GRP-94 and macrophage CD36 protein were higher than those in the model group (P < 0.05 to P < 0.01).The results of fluorescence staining showed that the green fluorescence intensity in the anti-β2GPⅠ antibody group was significantly stronger than that in model group (P < 0.01).The results of oil red O staining of foam cells showed that foam cells existed in the anti-β2GPⅠ antibody group and the number of foam cells was significantly higher than that in the model group (P < 0.01).ConclusionsThe anti-β2GPⅠ antibody promotes the uptake of oxLDL by macrophages in apoE-/- mice, accelerates the formation of foam cells, and increases the expression of CD36 in macrophages through endoplasmic reticulum stress to accelerate the uptake of oxLDL, thus promoting the progression of atherosclerosis.
Clinical, genetic and follow-up analysis of newly diagnosed diabetic ketoacidosis in children and adolescents

Abstract:
ObjectiveTo analyze the clinical features, genetics and follow-up outcomes of newly diagnosed diabetic ketoacidosis(DKA) in children and adolescents.MethodsThe clinical and follow-up data of 23 newly diagnosed DKA patients were retrospectively analyzed.Those who were willing to undergo gene examination were given second-generation sequencing.ResultsAmong the 23 patients, there were 6 males and 17 females with a median age of 10.1 years.There were 22 cases of type 1 diabetes mellitus(T1DM) and 1 case of type 2 diabetes mellitus(T2DM).There were 22 cases of polydipsia, 16 cases of prolonged breathing, 12 cases of abdominal pain, nausea and vomiting, 5 cases of fever, 12 cases of disturbance of consciousness and 15 cases of shock.The median random blood glucose was 25.0 mmol/L(11.48-34.72 mmol/L), and the mean HbA1c was 13.62%(9.80%-17.40%).The C peptide decreased in 22 cases, the level of which was 0.18 ng/mL(0.05-0.37 ng/mL).The mean of venous pH was 7.06(6.80-7.23), and the diabetes autoantibody in 10 patients were positive.All children were treated with fluid rehydration and continuous intravenous pumping of low-dose insulin.One case was transferred to hospital, and 22 cases were successfully rescued.After DKA correction, T1DM patients were treated with insulin, and T2DM patients were treated with metformin orally.Eighteen patients were followed up for an average of 26.5 months(5-57 months).The HbA1c was 6.8% in 1 patient with T2DM, and 8.79%(5.5%-14.4%) in 17 patients with T1DM.The median C-peptide was 0.09 ng/mL(0.05-0.50 ng/mL).The diabetes autoantibody in 3 T1DM children were positive.The second-generation sequencing in two children with T1DM were performed, the c.20_21delGA heterozygous deletion mutation in exon 2 of PDE11A in 1 cases was found, which leaded to the reading frame changes and protein truncation(p.Arg7ThrfsTer30).The father was wild-type, and the mother was heterozygous carrier.ConclusionsThe newly diagnosed DKA in children and adolescents is mostly found in T1DM, with various clinical manifestations and low HbA1c compliance rate.The PDE11A gene mutation may be related to T1DM.
Analysis of relationship between plasma NT-proBNP level and disease severity and prognosis in patients with early traumatic craniocerebral injury

Abstract:
ObjectiveTo investigate the relationship between early plasma N-terminal pro-B-type natriuretic peptide(NT-proBNP) level and disease severity and prognosis in patients with traumatic craniocerebral injury.MethodsA retrospective research method was used to select 100 patients with traumatic craniocerebral injury as observation group.The plasma NT-proBNP level was recorded at 2, 12 and 24 hours of the onset, and the change was analyzed.According to Glasgow coma scale(GCS) score of patients at admission, patients with traumatic craniocerebral injury were divided into mild group, middle group and severe group.The plasma NT-proBNP level of the three groups was compared at indicated times, and the correlation between plasma NT-proBNP level and disease severity was explored.At 30 d after the onset, the patients with traumatic craniocerebral injury were divided into poor prognosis group and good prognosis group according to the Glasgow outcome scale(GOS) score, and the plasma NT-proBNP level of patients with different prognosis was compared.ResultsAt 2, 12 and 24 hours, there were significant differences in plasma NT-proBNP levels among the three groups, the mild group was lower than the other two groups, and the middle group was lower than the severe group(P < 0.01).There were significant differences in plasma NT-proBNP levels at 2, 12 and 24 hours of onset in the three groups(P < 0.01).The levels at 12 and 24 hours of onset in the mild group were significantly higher than those at 2 hours of onset(P < 0.01), the levels at 24 hours of onset in the middle group were significantly higher than those at 2 and 12 hours of onset(P < 0.01), the levels at 12 and 24 hours of onset in the severe group were significantly higher than those at 2 hours of onset(P < 0.01), and the levels at 24 hours of onset were significantly higher than those at 12 hours of onset(P < 0.01).At 2, 12 and 24 hours of onset, the NT-proBNP level in the good prognosis group was significantly lower than that in the poor prognosis group(P < 0.01).There were significant differences in NT-proBNP levels between the two groups at different times of onset(P < 0.01).At 12 and 24 hours of onset, both groups were higher than 2 hours of onset(P < 0.01), and the 24 hours of onset in the good prognosis group was higher than 12 hours of onset(P < 0.01).Plasma NT-proBNP was significantly negatively correlated with GCS and GOS scores(P < 0.01).ConclusionsThe level of plasma NT-proBNP in patients with traumatic craniocerebral injury is significantly increased in the early stage, and its expression level is negatively correlated with GCS and GOS score.The above-mentioned scores can be used to predict the changes of the disease in clinical practice.
Effect of 3D printing implant guide on accuracy and aesthetic effect of patients with single tooth loss

Abstract:
ObjectiveTo study the effect of 3D printing implant guide on the accuracy and aesthetic effect of patients with single tooth loss.MethodsSixty patients with single tooth defect were selected, and were randomly divided into observation group and control group(30 cases in each group).The patients in the control group were treated with traditional implant guide, and the patients in the observation group were treated with 3D printing implant guide.The deviation distances of implant top and middle in mesial, distal, buccolingual and vertical directions were calculated in 2D plane, and the deviation angles of mesial, distal and buccolingual directions were calculated in 3D plane.The tooth color, shape, edge coloring and the degree of fit in the two groups were observed.The incidence of the adverse events within 2 weeks after dental implantation was observed and recorded.ResultsCompared with the control group, in the observation group, the deviation distances of the top and middle of the implant in the mesial and distal, buccolingual and vertical directions were smaller(P < 0.01);the deviation angles in the mesial, distal and buccolingual directions were smaller(P < 0.01);compared with the control group, in the observation group, there had more patients with grade A in the color, shape, edge coloring and tightness of implant teeth(P < 0.05), and the total incidence of adverse events was lower(P < 0.05).ConclusionsCompared with the traditional implant guide, the 3D printing implant guide can reduce the deviation distance and angle of the implant teeth, and has higher evaluation in the color, shape and edge coloring of the implant teeth.
Analysis of correlation between thyroid hormone and prevalence of chronic complications in diabetes mellitus

Abstract:
ObjectiveTo identify the correlation between normal thyroid hormones and the prevalence of lower-extremity arterial disease(LEAD) and diabetes peripheral neuropathy(DPN) in type 2 diabetes mellitus(T2DM) patients.MethodsA total of 1 342 T2DM patients were enrolled from January 2014 to October 2020 at the First Affiliated Hospital of Xi'an Jiaotong University and stratified into T2DM group(348 cases), LEAD group(704 cases) and DPN group(290 cases) depending on their disease's diagnosis.The differences in basic data and biochemical indexes between the three groups were compared.At the same time, the correlation between the incidence of LEAD, DPN and thyroid hormone was analyzed.ResultsAge, free thyroxine(FT4), total protein(TP), albumin(ALB), apolipoprotein E(apo E) were significantly different among the three groups(P < 0.05 to P < 0.01).Stratified by the quartile of thyroid hormone, it was found that there were statistically significant differences in the prevalence of LEAD and DPN among patients with different FT4 levels(P < 0.05).Multivariate logistic regression analysis showed that FT4 was an independent factor affecting the occurrence of lead and DPN(P < 0.05 to P < 0.01).ConclusionsLow level of FT4 is a risk factor for LEAD and DPN in patients with T2DN.

ObjectiveTo explore the reference value of serum thyroid stimulating hormone(TSH) and free thyroxine(FT4) levels in different gestational weeks of normal pregnancy.MethodsA total of 1 901 pregnant women with screening thyroid during pregnancy were retrospectively analyzed, and divided into the first trimester(T1, ≤11+6 weeks), second trimester(T2, 12-27+6 weeks) and third trimester(T3, ≥28 weeks) according to the gestational age at the time of detection.According to the specific gestational weeks, the puerpera were divided into the 0-11+6 weeks gestation group, 12-19+6 weeks gestation group, 20-27+6 weeks gestation group, 28~35+6 weeks gestation group and ≥36 weeks gestation group.The serological reference range of thyroid function indexes in different gestational weeks and periods of pregnancy were established.ResultsThe differences of the levels of TSH and FT4 among different stages of pregnancy were statistically significant(P < 0.01), the TSH in T3 stage was (2.12±1.32) μU/mL, which was significantly higher than that in T1 and T2 stages[(1.78±1.01)μU/mL and (1.83±0.95)μU/mL] (P < 0.01).The upper limits of TSH reference range were 3.76, 3.79 and 4.71 μU/mL in T1, T2 and T3 stages, respectively.The FT4 of T3 stage was (0.52±0.09)ng/dL, which was significantly lower than that of T1 and T2 stages[(0.84±0.18)ng/dL and (0.60±0.10]ng/dL(P < 0.01).The FT4 of T2 stage was also significantly lower than that of T1 (P < 0.01).The differences of the TSH and FT4 levels among different gestational weeks were statistically significant(P < 0.01).The TSH level was the lowest at 12-19+6 weeks of gestation(P < 0.05), then increased with the increase of gestational age(P < 0.05), and the TSH level was the highest at ≥36 weeks of gestation(P < 0.05).The level of FT4 was the highest at 0-11+6 weeks of gestation(P < 0.05), lower than that at 0-11+6 weeks of gestation(P < 0.05), and tended to be stable with the increase of gestational age(P>0.05).ConclusionsThe reference ranges of TSH and FT4 are different in different stages of pregnancy.The establishment of gestational age specific and method specific reference values in different stages of pregnancy and specific gestational age is helpful to guide the screening and management of thyroid diseases during pregnancy.
The value of ALBI and PALBI grades in the prognostic assessment of hepatitis B virus associated HCC patients

Abstract:
ObjectiveTo explore the value of albumin-bilirubin (ALBI) and platelet-albumin-bilirubin (PALBI) grades in the prognostic evaluation of Child-Pugh grade A HCC patients.MethodsClinical data of 134 Child-Pugh grade A hepatitis B virus associated HCC patients undergoing radical resection were retrospectively analyzed.ALBI and PALBI grades were calculated based on preoperative serologic examination.Overall survival (OS) and recurrence-free survival (RFS) were estimated using Kaplan-Meier curves and Cox regression.ResultsAfter follow-up, 37 patients died and 35 patients relapsed.Kaplan-Meier analysis showed that ALBI and PALBI grades were significantly correlated with OS and RFS (P < 0.05).Multivariate analysis indicated that both ALBI and PALBI grades were independent prognostic models for patients.Among different scoring systems, PLABI + tumor diameter, ALBI + tumor diameter, and ALBI + Italian liver cancer group score (CLIP) were more significant in predicting prognosis than CLIP, PLABI, ALBI, end-stage liver cancer model score, and Barcelona staging alone.ConclusionsALBI and PALBI grades are valuable models for evaluating the prognosis of Child-Pugh grade A HCC patients, if when they combine with tumor diameter, PALBI grade has better prognostic value for HCC patients than ALBI grade.

Abstract:
ObjectiveTo investigate the effects of different ventilation modes + Autoflow technique on respiratory mechanical indexes of patients undergoing thoracic surgery.MethodsA total of 96 patients with lung cancer treated by thoracic surgery from December 2018 to June 2020 were randomly divided into 6 groups, with 16 cases in each group.Group A: volume-controlled ventilation(VCV) [tidal volume(Vt) 5 mL/kg]+PEEP 4 mmHg+Autoflow mode; group B: VCV(Vt 5 mL/kg) + PEEP 4 mmHg+Constflow mode; group C: VCV(Vt 5 mL/kg)+PEEP 8 mmHg+Autoflow mode; group D: VCV(Vt 5 mL/kg) +PEEP 8 mmHg+Constflow mode; group E: VCV(Vt 8 mL/kg) +Autoflow mode; group F: VCV(Vt 8 mL/kg)+Constflow mode.The changes of peak inspiratory pressure(APIP), airway resistance(Raw), chest compliance(C), heart rate(HR), mean arterial pressure(MAP) and arterial partial oxygen pressure(PaO2) were compared in each group at bilateral lung ventilation, ventilation for 1 hour and for 2 hours.ResultsThere were no statistically significant differences in the levels of APIP, Raw, C, HR, MAP and PaO2 in each group during bilateral lung ventilation(P>0.05).After 1 hour of ventilation, APIP and Raw in group B, E and F were higher than those in group A and C(P < 0.05), the C value in group C was lower than that in other groups(P < 0.05), the PaO2 in group C was lower than that in group B(P < 0.05), there was no significant difference among other groups(P>0.05).After 2 hours of ventilation, APIP and Raw in group B, E and F were higher than those in group A and C(P < 0.05), the C value in group B was lower than that in group A(P < 0.05), the PaO2 value of group A and group B was lower than that in group C(P < 0.05), there was no significant difference among other groups(P>0.05).After ventilation for 1 hour and 2 hours, there was no significant difference in HR and MAP among the groups(P>0.05).ConclusionsVCV(Vt 5 mL / kg) + PEEP 8 mmHg + Autoflow mode can reduce airway peak pressure and resistance, improve arterial oxygen partial pressure, and effectively prevent injury caused by mechanical ventilation in elderly patients treated by thoracic surgery.
Efficacy and safety of intravenous thrombolysis with alteplase for the treatment of wake-up ischemic stroke under the guidance of multimode CT

Abstract:
ObjectiveTo study the efficacy and safety of intravenous thrombolysis with alteplase for the treatment of wake-up ischemic stroke(WUIS) under the guidance of multimode CT.MethodsA total of 28 patients with WUIS who were admitted to the neurology department and given intravenous thrombolysis with alteplase under the guidance of emergency multimode CT(observation group) were prospectively included.At the same time, 30 patients(control group) who were not with WUIS at the same period with time window of contemporaneous onset < 4.5 hours were collected.The coagulation function[prothrombin time(PT), activated partial thromboplastin time(APTT), fibrinogen(Fib)], platelet(PLT), high-sensitivity C-reactive protein(hs-CRP), National Institutes of Health stroke scale(NIHSS) score, living ability score(Barthel), clinical efficacy and outcome were compared between the two groups before treatment and 24, 7 and 14 days after treatment.ResultsThe clinical total effective rate and clinical good outcome rate in the observation group were 78.57% and 75.00%, respectively, and those in the control group were 83.33% and 80.00%, there was no statistical significance between the two groups(P>0.05).After treatment, the levels of Fib, PLT and hs-CRP in the observation group were gradually lower than those before treatment, and the levels of PT and APTT were gradually higher than those before treatment, with statistical significance before and after treatment(P < 0.05).The change trend of the control group was consistent with that of the observation group, but there was no statistical significance at each time point between the two groups(P>0.05).After treatment, the NIHSS score in the observation group decreased gradually and Barthel score increased gradually, which was statistically significant compared with that before treatment(P < 0.05).The change trend of the control group was consistent with that of the observation group, but there was no statistical significance at each time point between the two groups(P>0.05).ConclusionsEmergency multimode CT can be used as a reliable imaging basis for intravenous thrombolysis in WUIS patients with high efficacy and safety.

Abstract:
ObjectiveTo investigate the effect of dexmedetomidine combined with ultrasound-guided transversus abdominis plane(TAP) block anesthesia on the serum level of β-42 amyloid(Aβ-42) and Tau protein in patients undergoing laparoscopic inguinal hernia repair.MethodsA total of 100 patients undergoing laparoscopic inguinal hernia repair were selected and randomly divided into control group and observation group(50 cases in each group).The control group received laryngeal mask general anesthesia, and the observation group received ultrasound-guided TAP block combined with dexmedetomidine anesthesia.The mean arterial pressure, heart rate(HR) and blood oxygen saturation at the time of skin incision(T1), 1 h at the beginning of operation(T2) and immediate time after operation(T3) were recorded.The pain visual analogue scale(VAS) score, mini-mental state examination(MMSE) score, incidence of postoperative cognitive dysfunction(POCD) and expression levels of Aβ-42 and Tau protein were compared between the two groups.ResultsThe VAS scores in the two groups decreased gradually at immediate time after operation, 6 h, 24 h and 48 h after operation(P < 0.05), and the VAS scores at 6 h, 24 h and 48 h after operation in observation group were significantly lower than those in control group(P < 0.01).The HR in observation group at T2 and T3 was significantly lower than that in control group(P < 0.01), and HR at T2 and T3 was lower than that at T1(P < 0.05).The incidence of POCD in observation group was 6.00%(3/50), which was lower than 22.00%(11/50) in control group(P < 0.05).The MMSE scores in the two groups at 1 d, 7 d after operation were lower than those before anesthesia(P < 0.05), and which in observation group were significantly higher than those in control group(P < 0.01). The Tau protein level and Tau/Aβ-42 value in the two groups at 7 d after operation were higher than those before anesthesia(P < 0.05), the Aβ-42 protein level was lower than that before anesthesia(P < 0.05), and the Tau protein level and Tau/Aβ-42 value in observation group were lower than that in control group(P < 0.05 and P < 0.01), the Aβ-42 protein level was significantly higher than that in control group(P < 0.01).ConclusionsUltrasound-guided TAP block anesthesia combined with dexmedetomidine has good analgesic effects in laparoscopic inguinal hernia repair, which can effectively improve the patient's cognitive function, regulate the postoperative serum level of Aβ-42 and Tau protein, and decrease the incidence of POCD.
Effect of round ligament combined with uterosacral ligament suspension in preventing pelvic floor dysfunction after hysterectomy

Abstract:
ObjectiveTo investigate the effect of round ligament combined with uterosacral ligament suspension in preventing pelvic floor dysfunction after hysterectomy, and the risk factors of pelvic floor dysfunction after hysterectomy.MethodsOne hundred patients undergoing total hysterectomy due to uterine fibroids and adenomyosis were selected as the research subjects, and randomly divided into experimental group and control group, with 50 cases in each group.The patients in the control group underwent conventional laparoscopic total hysterectomy, and the patients in the experimental group underwent round ligament combined with uterosacral ligament suspension on the basis of the control group.The operation conditions(including operation time and blood loss), pelvic floor dysfunction-related conditions [including pelvic organ prolapse quantification(POP-Q) score, pelvic floor impact questionnaire-7(PFIQ-7) score, and pelvic organ prolapse/urinary incontinence sexual questionnaire-12(PISQ-12) score] were compared between the two groups.The risk factors and protective factors of pelvic floor dysfunction after hysterectomy were analyzed.ResultsThere was no significant difference in operative blood loss and operative complications between the two groups(P>0.05), and the operation time in the experimental group was longer than that in the control group(P < 0.05).There was no significant difference in POPQ scores between the two groups at 6 months after operation(P>0.05).The POPQ scores at point Bb(the farthest point from the anterior vaginal wall to the hymen), point C(the point in the anterior fornix of vagina or vaginal stump), and point Bp(the point in the posterior vaginal prolapse, farthest from hymen) in the experimental group at 12 months after operation were significantly higher than those in the control group(P < 0.01).There was no significant difference in PFIQ-7 scores between the two groups before operation(P>0.05), and the PFIQ-7 scores in the experimental group at 6 months and 12 months after operation were significantly lower than those in the control group(P < 0.01).There was no significant difference in PISQ-12 scores in the two groups between before and after treatment(P>0.05).Pelvic floor dysfunction was found in 6 cases(12%) in the experimental group and in 15 cases(30%) in the control group, and the incidence of pelvic floor dysfunction in the experimental group was lower than that in the control group(χ2=4.88, P < 0.05).Univariate analysis showed that the proportion of patients with age >50 years, parity >2 times, without round ligament suspension and without early pelvic floor exercise in patients with pelvic floor dysfunction was higher than that of patients without pelvic floor function(P < 0.05 to P < 0.01).Multivariate analysis showed that age, parity and no early pelvic floor exercise were the risk factors for postoperative pelvic floor dysfunction(P < 0.05), and round ligament suspension was a protective factor(P < 0.01).ConclusionsRound ligament combined with uterosacral ligament suspension can reduce the incidence of pelvic floor dysfunction after hysterectomy, and age >50 years old, parity >2 times and no early pelvic floor exercise are the risk factors of pelvic floor dysfunction after hysterectomy.
Effect of HOXB7 and VEGF on the prognosis of patients with HCC

Abstract:
ObjectiveTo investigate the effect of homeobox B7 (HOXB7) and vascular endothelial growth factor(VEGF) expressions on the prognosis of patients with hepatocellular carcinoma(HCC) after operation.MethodsSeventy-four patients with HCC following surgical resection were enrolled.Clinicopathological and survival data were analyzed, and immunohistochemical staining method for HOXB7 and VEGF expressions measurement was performed on tissue microarray sections.ResultsThe 3-and 5-year overall survival rates in the 74 patients were 67.6% and 48.6%, respectively.Multivariate analysis revealed that HOXB7(P < 0.01, HR=1.139) and VEGF(P < 0.01, HR=1.192) expressions, Child-Pugh class(P < 0.01, HR=4.100), T stage(P < 0.01, HR=4.317), Tumor number(P < 0.01, HR=2.639) were the independent prognostic factors for overall survival rate.Immunohistochemical staining results showed that the expressions of HOXB7 and VEGF influenced the 5-year survival rate.ConclusionsHOXB7 and VEGF overexpression affect the survival and prognosis of HCC patients, which suggesting a poor overall survival rate.

Abstract:
ObjectiveTo observe the frequency of sister-chromatid exchange(SCE)in peripheral blood lymphocytes of women with spontaneous abortion, and analyze the relationship between SCE and spontaneous abortion.MethodsOne hundred and seventeen women with spontaneous abortion and fifty normal reproductive healthy women were divided into the observation group and control group, respectively.The peripheral blood lymphocytes were detected by SCE method to count the frequency of SCE.ResultsThe SCE frequency in observation group and control group were(8.04±2.30)and(6.43±1.50), respectively.The difference of SCE frequency between two groups was statistically significant(P < 0.01).ConclusionsThe female spontaneous abortion may be related to the abnormal frequency of SCE.

Abstract:
ObjectiveTo explore the clinical efficacy of trigger point acupuncture with fine silver needle combined with ozone in the treatment of female chronic pelvic pain.MethodsA total of 132 patients with chronic pelvic pain were randomly divided into three groups, with 44 patients in each group.Group A was treated with trigger point acupuncture with fine silver needle combined with ozone, group B was treated with simple pudendal nerve block combined with ozone, and group C was treated with electrical stimulation-biofeedback.The VAS score, clinical total effective rate, and the improvement effect of pelvic floor type Ⅰ muscle strength and type Ⅱ muscle strength were compared among the three groups before treatment, after treatment, 1 week after treatment, and 2 weeks after treatment.In addition, SDS, SAS and SF-36 were used to evaluate the psychological score and quality of life score of patients before and 2 weeks after treatment.ResultsThere was no statistically significant difference in each index of patients in the three groups before treatment (P>0.05).At different time points after treatment, the VAS score of patients in the three groups decreased compared with that before treatment (P < 0.01), and the VAS score of patients in group A was lower than that of the other two groups (P < 0.01).After treatment, the clinical total effective rate in group A was higher than that in group C, the total effective rate of pelvic floor type Ⅰ and type Ⅱ muscle strength, SF-36 scores were higher than those in group B and group C, and the SDS and SAS scores were lower than those in group B and group C (P < 0.05 to P < 0.01).ConclusionsTrigger point acupuncture combined with ozone has good clinical effects on female chronic pelvic pain, and the pelvic floor muscle strength is obviously enhanced, which is conducive to the recovery of pelvic floor function.
Relationship between the serum vitamin A and E levels, and onset and severity of preeclampsia

Abstract:
ObjectiveTo investigate the relationship between the serum vitamin A and E levels, and onset and severity of preeclampsia.MethodsA total of 257 pregnant women with preeclampsia were divided into the mild group(n=104) and severe group(n=153) according to the severity of preeclampsia, and 12 822 normal pregnant women were set as the control group.The serum levels of vitamin A and E, diastolic blood pressure(DBP), systolic blood pressure(SBP), serum creatinine(Scr) and 24-hour urinary protein quantification(24 hpro) were compared among three groups.The correlation of vitamin A and E with DBP, SBP, SCr and 24 hpro were analyzed, and the predictive value of vitamin A and E to preeclampsia was analyzed.ResultsThe vitamin A and E levels in severe group were significantly lower than those in mild group and control group, and those in mild group were also significantly lower than those in control group(P < 0.01).The DBP, SBP, Scr and 24 hpro in severe group were significantly lower than those in mild group and control group, and those in mild group were significantly lower than those in control group(P < 0.01).The results of Pearson correlation analysis showed that the serum levels of vitamin A and E were negatively correlated with DBP, SBP, SCr, and 24 hpro(P < 0.05).When the cut-off point was 0.35 mg/dL, the sensitivity and specificity of vitamin A to predict the preeclampsia were 63.3% and 60.9%, respectively(P < 0.05).When the cut-off point was 16.85 mg/dL, the sensitivity and specificity of vitamin E to predict the preeclampsia were 72.1% and 69.7%, respectively(P < 0.05).ConclusionsThe levels of serum vitamin A and E levels are abnormal in pregnant women with preeclampsia, which has a certain correlation with the severity of disease.The levels of serum vitamin A and E should be strictly monitored and controlled.
Analysis of prognosis and influencing factors of middle and advanced laryngeal carcinoma

Abstract:
ObjectiveTo investigate the prognosis and its influencing factors of patients with middle and advanced laryngeal carcinoma.MethodsThe clinical data of 67 patients with middle and advanced laryngeal carcinoma were retrospectively analyzed.Kaplan-Meier method was used for survival analysis, Log-rank method was applied for univariate analysis, and COX regression model was employed to analyze the independent influencing factors of prognosis.ResultsThe 3-year overall survival rate and 5-year overall survival rate of the patients at the median follow-up time were 74.6% and 64.2%, respectively.Univariate analysis showed that N stage and age were the related factors affecting the survival of patients with middle and advanced laryngeal carcinoma(P < 0.05).COX regression model analysis showed that age, T stage, N stage and treatment method were the independent factors affecting the prognosis of patients with middle and advanced laryngeal carcinoma(P < 0.05).ConclusionsThe prognosis of patients with middle and advanced laryngeal carcinoma is relatively poor.N stage and age are important factors affecting the prognosis.Early detection and early treatment are the keys to improve the prognosis of patients with laryngeal cancer.
Effect of the second stage duration in the new standard of labor on the maternal and infant outcomes of single full-term primipara with first presentation

Abstract:
ObjectiveTo study the effects of the labor duration of the second stage on maternal and infant outcomes of single full-term first-born primipara in the new standard of labor.MethodsThe clinical data of 269 cases of single full-term primipara with first appearance were retrospectively analyzed.The cases were divided into the control group(n=113, duration of the second stage of labor < 2 h) and observation group(n=156, duration of the second stage of labor>2 h) according to the length of the second stage of labor.The observation group subdivided into the group A(72 cases, duration of the second stage of labor for 2-2.5 h), group B(44 cases, duration of the second stage of labor for 2.5-3 h) and group C(40 cases, duration of the second stage of labor for 3 h) according to the duration of the second stage of labor, and the mode of delivery, pregnancy outcome and neonatal outcome were analyzed.ResultsThe differences of the perineal lateral resection rate, maternal adverse outcomes and neonatal adverse outcomes between the observation group and control group were not statistically significant(P>0.05), and the maternal forceps assisted delivery rate and cesarean section rate in observation group were significantly higher than those in control group(P < 0.01).There was no statistical significances in the maternal delivery mode, maternal adverse outcomes and neonatal adverse outcomes between group A and control group(P>0.05).The episiotomy and forceps-assisted delivery rates, and incidence rates of postpartum fever, postpartum hemorrhage, puerperal infection and urinary retention in group B and group C were higher than those in control group and group A(P < 0.05), and the cesarean section rates in group B and group C were higher than that in control group(P < 0.05).The incidence rates of the neonatal asphyxia, macrosomia, shoulder dystocia and neonatal acidemia in group C were higher than those in control group and group A(P < 0.05).The incidence rate of shoulder dystocia in group B was higher than that in group A(P < 0.05), and the neonatal acidemia rate in group B was lower than that in group C(P < 0.05).ConclusionsCompared with the duration of the second stage of labor under the traditional standard of labor, the prolongation of the second stage of labor under the new standard of labor has adverse effects on the maternal and infant outcomes.In order to reduce the rate of cesarean section and occurrence of adverse outcomes, it is necessary to closely monitor maternal vital signs.

ObjectiveTo explore the self-influencing factors of curative effects of posterior cervical expansive single open-door laminoplasty multlevel cervical spondylotic myelopathy(CSM).MethodsA total of 120 patients with multlevel CSM from December 2016 to December 2018 were treated with expansive laminoplasty, and followed up for 1 to 3 years.At the last follow-up, the patients were divided into the excellent group and poor group according to the the clinical efficacy evaluated by Japanese Orthopaedic Association(JOA) score.The general data such as gender, age and disease course of patients were collected, and the maximum diameter and average compression rate of pressure objects of the image data were calculated.The independent influencing factors of the curative effects between groups were investigated using the single factor and multi-factor analysis.ResultsAmong 120 patients, the results of the last follow-up showed that the excellent in 34 cases, good in 61 cases, fair in 20 cases and poor in 5 cases were found, and the excellent and good rate of which was 79.17%.The differences of the disease course, age, maximum compression diameter, mean compression rate, spinal drift distance, preoperative Cobb angle and preoperative JOA score between two groups were statistically significant(P < 0.05 to P < 0.01).The differences of the sex and number of disease segments between two groups were not statistcially significant(P>0.05).The results of multivariate logistic regression analysis showed that the course of disease more than 2 years, maximum diameter of the pressure object more than 5 mm, spinal cord drift distance less than 2.6 mm, average compression rate more than 30% and preoperative JOA score less than 10 were the independent risk factor affecting the efficacy of expansive laminoplasty for multlevel CSM.ConclusionsThe spinal enlargement is an effective method for treating multilevel CSM.The clinical efficacy is affected by its own factors such as longer course of disease, high degree of spinal cord compression and poor neurological function before surgery.
Analysis on the status and risk factors of cardiovascular events in elderly patients with hypertension in convalescent center

Abstract:
ObjectiveTo explore the status quo and risk factors of cardiovascular events in elderly hypertensive patients in convalescent center.MethodsA total of 825 elderly patients with hypertension were investigated, and the patients were divided into the cardiovascular event group and non-cardiovascular event group according to whether cardiovascular events occurred.The clinical data of all cases were collected to analyze the risk factors of cardiovascular events.ResultsThe incidence rate of cardiovascular events in 825 elderly patients with hypertension was 10.42%(86/825), the ratio of male to female was 1.53∶1, the majority age of patients was 70 years or above(46/86, 53.49%), and the patients included 41 cases of angina pectoris, 24 cases of acute myocardial infarction, 13 cases of chronic heart failure and 8 cases of cardiogenic death.The differences of the levels of coronary heart disease history, smoking, alcohol consumption, heart rate, pulse pressure, SBP and urinary protein, TC, TG, HDL, LDL, FIB, BUN, UA, Hcy, ApoA and Cr between two groups were statistically significant(P < 0.05 to P < 0.01).The results of logistic regression analysis showed that the history of coronary heart disease, smoking, alcohol consumption, and abnormalities of SBP, TC, TG, LDL, FIB, UA, Hcy and ApoA were the independent risk factors of cardiovascular events in elderly patients with hypertension(P < 0.05).ConclusionsThe incidence rate of cardiovascular events in elderly patients with hypertension in convalescent center is low, and the majority of them are male, and 70 years old and over.Taking the intervention measures for the independent risk factors such as history of coronary heart disease, smoking, alcohol consumption, SBP, TC, TG, LDL, FIB, UA, Hcy and ApoA abnormalities can reduce the risk of cardiovascular events.
Clinical study on the evaluation of hepatic space-occupying lesions related to androgen treatment for aplastic anemia by multi-modality image fusion

Abstract:
ObjectiveTo explore the clinical value of multi-modality image fusion in evaluating hepatic space-occupying lesions related to androgen treatment for aplastic anemia (AA).MethodsTotal 49 patients with AA who had undergone androgen treatment and had hepatic space-occupying lesions detected by ultrasound were selected as the observation group.Meanwhile, 78 patients with the other hepatic space-occupying lesions were selected as the control group.All patients were subjected to spiral CT and MRI scanning.CT and MRI dual-phase images were fused, and the value of multi-modality image fusion in evaluating hepatic space-occupying lesions related to androgen treatment for AA was analyzed.ResultsSerum biological examination, puncture, postoperative histopathological examination or more than 6 months of clinical and imaging follow-up found that there were 45 cases with 68 hepatic space-occupying lesions in the observation group and 75 cases with 92 hepatic space-occupying lesions in the control group.The proportion of benign lesions in the observation group was higher than that in the control group (P < 0.05).The proportion of lesions detected by fusion images was higher than that by CT in the observation group (P < 0.05), and the proportion of lesions detected by fusion images was higher than that by CT or MRI in the control group (P < 0.05), but the proportion of lesions detected by fusion images in the observation group was not significantly different from that in MRI examination (P>0.05).Taking surgical results, pathological results or follow-up results as the golden standard, the sensitivity, specificity and accuracy of spiral CT, MRI and fusion images to diagnose hepatic space-occupying lesions related to androgen treatment for AA were (71.43%, 87.50%, 80.00%), (85.00%, 88.00%, 86.67%) and (100.00%, 94.74%, 97.78%), respectively.The diagnostic value of fusion images was higher than that of CT or MRI (P < 0.05).ConclusionsMulti-modality image fusion has good diagnostic value in evaluating the hepatic space-occupying lesions related to androgen therapy in aplastic anemia.It can improve the detection rate of hepatic space-occupying lesions, and its diagnostic efficiency is better than that of CT or MRI.It is worthy of clinical promotion and application.
Predictive value of preoperative contrast-enhanced ultrasound in sentinel lymph node metastasis of breast cancer

Abstract:
ObjectiveTo analyze the predictive value of preoperative contrast-enhanced ultrasound in sentinel lymph node metastasis of breast cancer.MethodsThe contrast-enhanced ultrasound in 276 patients with breast cancer were performed to find sentinel lymph nodes.The results between pathological examination and contrast-enhanced ultrasound were compared.The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of contrast-enhanced ultrasound in all cases were calculated.ResultsOf the 276 patients, 124 tumors were located in the left breast and 152 tumors were located in the right breast.There were 100 cases in the outer upper quadrant, 47 cases in the outer lower quadrant, 60 cases in the inner upper quadrant and 69 cases in the inner lower quadrant.All patients were pathologically confirmed as primary breast cancer, which included 183 cases of invasive ductal carcinoma, 65 cases of intraductal carcinoma in situ, 16 cases of mucinous carcinoma and 12 cases of other cancers.The diameter of the primary tumor was 0.6-4.9 cm, and the diameter of the sentinel lymph node was 0.3-3.2 cm.A total of 497 sentinel lymph nodes were detected by preoperative contrast-enhanced ultrasound, and a total of 518 sentinel lymph nodes were removed by operation.The detection rate of preoperative contrast-enhanced ultrasound of sentinel lymph nodes was 95.95%(497/518).The results of pathological examination confirmed that 120 nodes were metastatic sentinel nodes.Seventy-three patients with positive and 203 patients with negative sentinel lymph nodes were predicted using the preoperative contrast-enhanced ultrasound.The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of predicting sentinel lymph nodes were 76.12%, 89.47%, 69.86%, 92.12% and 86.23%, respectively.ConclusionsIn order to get the best predicting results in the sentinel lymph node metastasis of breast cancer, the contrast-enhanced ultrasound should be combined with other tracing methods.

Abstract:
ObjectiveTo explore the expression and clinical significance of soluble CD86(sCD86) and high mobility group protein B1(HMGB1) in peripheral blood of children with allergic rhinitis.MethodsA total of 150 children with allergic rhinitis were selected as the observation group.They were divided into mild group(n=112) and moderate to severe group(n=38) according to the severity of the disease, and 50 normal children who underwent physical examination in the same period were selected as the control group.The levels of immunoglobulin E(IgE), interleukin-4(IL-4), interleukin-9(IL-9), interferon-γ(IFN-γ), sCD86 and HMGB1 in each group were compared.In the observation group, the sCD86, HMGB1 levels and IgE, IL-4, IL-9, IFN-γ levels were analyzed by univariate, logistic multivariate and correlation analysis.ResultsIgE, IL-4, IL-9, sCD86 and HMGB1 levels in the observation group were significantly higher than that in the control group; IFN-γ levels were significantly lower than that in the controls group(P < 0.01).Moreover, IgE, IL-4, IL-9, sCD86 and HMGB1 levels in the moderate to severe group were significantly higher than those in the mild group; IFN-γ levels were significantly lower than that in the mild group(P < 0.01).IgE, IFN-γ, IL-4, IL-9, sCD86 and HMGB1 were the independent influencing factors for the severity of allergic rhinitis(P < 0.05 to P < 0.01).In sera from children with allergic rhinitis, sCD86 and HMGB1 levels were positively correlated with IgE, IL-4 and IL-9(P < 0.05), and negatively correlated with IFN-γ(P < 0.05).ConclusionsThe high expression of sCD86 and HMGB1 in serum of children with allergic rhinitis may be related to IgE mediated type Ⅰ allergy and Th1/Th2 cell imbalance.

Abstract:
ObjectiveTo detect the levels of four-item urinary proteins and blood indexes in patients with early kidney injury of hypertension, and to explore their correlations with early kidney injury of hypertension and its clinical value in the treatment of patients with early kidney injury of hypertension.MethodsSixty patients with hypertension were selected as the study subjects.They were divided into hypertension microalbuminuria group (n=30) and simple hypertension group (n=30) according to urinary microalbumin (mAlb) to urinary creatinine (Ucr) ratio (ACR).Meanwhile, 30 healthy adults were selected as control group.And the corresponding urinary mAlb, β2-microglobulin (β2-MG), transferrin (TRF), immunoglobulin G (IgG), serum free triiodothyronine (FT3), free thyroxine (FT4), thyroid stimulating hormone (TSH), uric acid (UA), serum urea (UREA) and creatinine (Scr) were detected, respectively.ResultsThe urinary mAlb, β2-MG, TRF, IgG, serum concentrations of UA, UREA and Scr in the hypertension microalbuminuria group were high than those in both the control group and simple hypertension group (P < 0.01).There was no significant difference in the urinary mAlb, β2-MG, TRF, IgG, serum concentrations of UA, UREA, Scr between the simple hypertension group and the control group (P>0.05).Compared with the control group, the levels of FT3 in both simple hypertension group and hypertension microalbuminuria group were decreased (P < 0.01).Pearson correlation analysis showed that mAlb, β2-MG, TRF, IgG and UA in hypertension microalbuminuria group were positively correlated with ACR, UREA and Scr (P < 0.05), and FT3 was negatively correlated with ACR, UREA and Scr (P < 0.01).ConclusionsUrinary mAlb, β2-MG, TRF, IgG and serum concentrations of UA, FT3 can serve as effective indexes for the early kidney injury of hypertension.

ObjectiveTo investigate the effects of find organize clarify understand select plan do check act(FOCUS-PDCA) on improving basic operation level of junior nurses in emergency department.MethodsWith the help of simulation equipment, 26 junior nurses in emergency department were trained and assessed, and the basic operation skills of nurses between before and after the implementation of FOCUS-PDCA were compared.ResultsAfter the implementation of FOCAL-PDCA, the passing rates of defibrillation, cardiopulmonary resuscitation, balloon, sputum aspiration, ECG monitoring and gastric lavage in junior nurses of emergency department were significantly higher than those before implementation(P < 0.01).ConclusionsThe implementation of FOCUS-PDCA is helpful to improve the passing rate of basic operation skills of junior nurses in emergency department, and which should be carried out timely to promote the continuous improvement of nursing quality.

Abstract:
ObjectiveTo observe the incidence rate and related risk factors of poor prognosis in hypertensive intracerebral hemorrhage(HICH) patients treated with neuroendoscopic hematoma removal.MethodsThe clinical data of 152 HICH patients treated with neuroendoscopic hematoma removal were retrospectively analyzed, and the patients were followed up for more than 12 months.According to the Barthel index(BI) at 1 year after surgery, the patients were divided into the good prognosis group(98 cases, BI>60 points) and bad prognosis group(54 cases, BI≤60 points).The age, gender, Glasgow coma score(GCS), midline deviation, preoperative blood loss, bleeding site, operation timing, operation time, cerebrospinal fluid circulation patency time, postoperative intracranial pressure, hematoma clearance rate, and preoperative complicated with chronic diseases were compared between two groups, and the logistic regression analysis was used to explore the risk factors of poor prognosis after neuroendoscopic surgery.ResultsThe results of single factor analysis showed that the preoperative blood loss, midline deviation rate, proportion of operation time in late stage and time of cerebrospinal fluid circulation patency in good prognosis group were lower than those in poor prognosis group(P < 0.05 to P < 0.01), and the GCS score and hematoma clearance rate in good prognosis group were higher than those in poor prognosis group(P < 0.05 and P < 0.01).The results of logistic regression analysis showed that the high preoperative blood loss(OR=1.638), low GCS score(OR=0.612), late operation time(OR=2.750) and midline deviation(OR=3.677) were the independent risk factors of poor prognosis in HICH patients after neuroendoscopic surgery(P < 0.05 to P < 0.01).ConclusionsThe preoperative neuroendoscopy in HICH patients can improve the outcome through shortening the waiting time and reducing the amount of blood loss.

Abstract:
ObjectiveTo explore the effects of the mindfulness training combined with biofeedback electrical stimulation on the improvement of pelvic floor function after delivery.MethodsThe convenience sampling method was used to select 85 parturients in outpatient department, and the cases were randomly divided into the control group(42 cases) and observation group(43 cases).The control group were treated with the biofeedback electrical stimulation, and the observation group were treated with the mindfulness training combined with biofeedback electrical stimulation.The pelvic floor surface muscle function score, urinary incontinence and pelvic organ prolapse were compared between two groups before intervention and after 1 week and 3 months of intervention.ResultsAt 1 week and 3 months after intervention, the type Ⅰ and Ⅱ muscle fibers scores and muscular endurance scores in the observation group were significantly higher than those before intervention(P < 0.01), and which were significantly higher than those in control group(P < 0.01).After 3 months of intervention, the incidence rates of uterine prolapse and vaginal anterior wall prolapse in the observation group were improved compared with that in the control group(P < 0.05 and P < 0.01).The situation of vaginal prolapse and urinary incontinence in the observation group after 3 months of intervention were also improved compared with that before intervention(P < 0.05), but the difference of which between two groups was not statistically significant(P>0.05).ConclusionsThe mindfulness training combined with biofeedback electrical stimulation can significantly improve the physiological function of the postpartum pelvic floor and relieve the symptoms of pelvic floor dysfunction.
Study on the application of two intracavity electrocardiogram localization techniques in Dacron-cuffed double lumen hemodialysis catheter

Abstract:
ObjectiveTo explore the application of two intracavity electrocardiogram localization techniques in Dacron-cuffed double lumen hemodialysis catheter.MethodsSixty patients implanted with Dacron-cuffed double lumen hemodialysis catheter were selected and randomly divided into 0.9% sodium chloride solution-guided lumen electrocardiogram group and wire-guided lumen electrocardiogram group(30 cases in each group).The characteristic P-wave stability, catheter tip in place rate, medical staffs′ satisfaction, operation time and puncture point infection rate of the two methods were compared.ResultsStable intracavity electrocardiogram during the implantation of Dacron-cuffed double lumen hemodialysis catheter was exported in all patients, with characteristic P-wave, and the catheter tip was located at the level of right atrial entrance.The stability ratio of characteristic P-wave in wire-guided lumen electrocardiogram group was higher than that in 0.9% sodium chloride solution-guided lumen electrocardiogram group, but the difference was not statistically significant(P>0.05).The operation time in 0.9% sodium chloride solution-guided lumen electrocardiogram group was significantly higher than that in wire-guided lumen electrocardiogram group(P < 0.01).The satisfaction of doctors and nurses in 0.9% sodium chloride solution-guided lumen electrocardiogram group was significantly lower than that in wire-guided lumen electrocardiogram group(P < 0.01).There was no infection in the two groups, the difference of which was not statistically significant(P>0.05).ConclusionsBoth methods can guide the intracavity electrocardiogram.The wire-guided intracavity electrocardiogram can obtain characteristic P-wave more stably, the process is more convenient, and the satisfaction degree of medical staffs is higher.

ObjectiveTo explore the application effects of the management process of elderly patients with colonoscopy treatment based on PISETI management model in the prevention of falls.MethodsA total of 130 patients ≥60 years old scheduled by colonoscopy were selected, and randomly divided into the PISETI management model group(observation group) and routine care group(control group).The corresponding measures in formulating the fall management process based on PISETI management model in the observation group were implemented according to the patient report, information access, signage, environment, teamwork, involving patient or family and other aspects, and the control group was nursed with routine measures to prevent falls.The differences in the timely rate of fall risk assessment, knowledge of fall prevention of patients and their families, rate of fall prevention markers, rate of environmental safety, and incidence rate of fall between two groups were compared.ResultsThe timely rate of fall risk assessment, knowledge of fall prevention of patients and their families, rate of fall prevention markers and rate of environmental safety in observation group were significantly better than those in control group(P < 0.05 to P < 0.01).No patient fell in two groups.ConclusionsThe fall prevention management process based on PISETI management model can promote the implementation of relevant fall prevention measures for elderly patients treated with colonoscopy, which is worthy of clinical application.
Effect of intracavitary ECG and P-wave ratio on neonatal PICC localization

Abstract:
ObjectiveTo investigate the relationship between the P-wave size of neonatal PICC intracavitary electrocardiogram (ECG) tip and the position of PICC catheter tip in superior vena cava and its clinical effect.MethodsA total of 184 patients with PICC catheterization were enrolled in the study.They were randomly divided into observation group and control group according to the random number table method, 92 cases in each group.The control group took the regular body surface and the chest X-ray positioning after operation, while the observation group was located by intracavitary ECG P-wave ratio and chest X-ray.The disposable catheter success rate and time of catheterization were compared between the two groups, and the position of catheter tip corresponding to different P waves was analyzed.ResultsThe PICC disposable catheter success rate of the newborns in the observation group was 95.65% (88/92), which was significantly higher than that of the control group (81.52%, 75/92)(P < 0.01).The catheterization time and the puncture site bleeding volume in the observation group were significantly lower than the control group (P < 0.01).In the observation group, the tip of the catheter of 1/2-2/3 R wave was mainly recommended by the guideline, the 1/3-1/2 R wave was mainly 2/3 of the superior vena cava, and the 1/2-2/3 R wave was mainly recommended by the guideline or right atrium, the difference was statistically significant (P < 0.05).ConclusionsThe P-wave ratio of intracavitary ECG is helpful to locate the tip of neonatal PICC.
