2009, 34(8): 656-658.
Abstract:
Objective: To explore the role and significance of abnormal expressions of phosphotase and tensin homolog deleted from chromsome 10(PTEN) and retinoblastoma(Rb) gene in the development of breast cancer. Methods: Immunohistochemical technique(S-P method) was used for detection of PTEN and Rb gene proteins in 150 cases of breast cancer and 30 cases of mammary benign proliferative lesions. The results were analyzed and compared with some clinicopathological parameters. Results: The protein expression levels of PTEN and Rb in breast cancer were significantly lower than those in benign proliferative lesions(P<0.005).Loss of expression of PTEN protein was obviously related to lymph nodes metastasis and the loss of expression of estrogen receptor(P<0.05).Loss of expression of Rb protein was not related to the clinicopathological parameters except the loss rate of expression was higher in grade Ⅲ.The positive association was found between the expressions of PTEN and Rb proteins(P<0.05).Conclusions: Expression rates of anti-oncogenes PTEN and Rb proteins are higher in benign proliferative lesions and decrease in breast cancer. The loss of inhibiting function of anti-oncogenes PTEN and Rb results in neoplastic transformation.Expression of PTEN relates to lymph nodes metastasis and the loss of expression of estrogen receptor, which shows a certein reference value to judge the prognosis of breast cancer.

2009, 34(8): 659-662.
Abstract:
Objective: To investigate the expressions and clinical significance of human telomerase reverse transcriptase(hTERT) and survivin in gallbladder carcinoma. Methods: The expressions of hTERT and survivin in 60 cases of primary gallbladder carcinoma,15 cases of gallbladder adenoma and 15 cases of chronic cholecystitis were determined using streptavidin-peroxidase immunohistochemical method. Results: The positive rates of hTERT and survivin were significantly higher in gallbladder carcinoma group than in gallbladder adenoma group and chronic cholecystitis group(P<0.01).The positive expression of hTERT in gallbladder carcinoma had no significant correlation with the gender, age, histologic type, gallbladder stone, pathological grade,Nevin stage, metastasis or survival time after operation(P>0.05).The positive expression was significantly higher in Nevin staging Ⅲ-Ⅴ group than in Nevin staging Ⅰ-Ⅱ group(P<0.05).The positive expression of survivin was higher in lymph node or distant metastasis group than in non-metastasis group(P<0.05).In gallbladder carcinoma, the expression of hTERT had a positive correlation with that of survivin(P<0.05).Conclusions: Abnormal high expressions of hTERT and survivin protein are observed in gallbladder carcinoma, and there is a positive correlation between them. Both hTERT and survivin play an important role in the pathogenesis and progression of gallbladder carcinoma, and may have a cooperative effect on the carcinogenesis.High expression of hTERT is an early event in the pathogenesis of gallbladder carcinoma. The overexpression of survivin indicates malignant biological behavior of gallbladder carcinoma to some extent.

2009, 34(8): 662-665.
Abstract:
Objective: To investigate the mechanism of the effect of diethylstilbestrol on the proliferation and differentiation of hippocampal neural stem cells(NSCs) in fetal rats. Methods: Hippocampal neural stem cells from the embryo(gestational day 16) of fetal rats were prepared and divided into five groups:control, diethylstilbestrol 1×10-7 mol/L,diethylstilbestrol 1×10-8 mol/L,diethylstilbestrol 1×10-9 mol/L and BDNF 5×10-2 mg/L.The expressions of Nestin, neurone specific enolase(NSE) and glial fibrillary acidic protein(GFAP) were determined by immunohistochemistry, and the level of brain-derived neurotrophic factor(BDNF) mRNA was detected by RT-PCR.Results: Different doses of diethylstilbestrol could increase the area of neural spheres, quantity of NSE,and GFAP-positive cells. At a concentration of 10-8 mol/L,diethylstilbestrol facilitated the proliferation and differentiation of hippocampal NSCs of fetal rats the most, and the level of BDNA mRNA was the highest(P<0.01).Conclusions: Diethylstilbestrol enhances the proliferation of hippocampal NSCs in fetal rats, which is probably related to the up-regulation of the BDNF mRNAs' expression.

Objective: To investigate the surveillance data of bacterial resistance in Lu'an city hospitals in 2008.Methods: Bacterial susceptibility experiments were performed on 102 samples clinically collected from 3 hospitals in Lu'an city, using routine custom dilution MIC method according to the CLSI(2008) guidelines;the resistance, intermediate and susceptible rates of the bacteria to antimicrobial agents were measured. Results: Among the 102 bacteria samples,Escherichia coli accounted for 27,Acinetobacter baumannii 25,Pseudomonas aeruginosa 23,Klebsiella 18,Stenotrophomonas maltophilia 7 and others 2.Carbapenems were the most effective antimicrobial agents against Gram-negative bacilli, and other effective ones were piperacillin-tazobactam(73.5%),amikacin(67.6%) and fosfomycin(62.7%).Gram-negative bacilli showed high resistance to ampicillin(96.1%),cefoxitin(83.3%) and aztreonam(81.4%),and were also noticeably resistant to cefoperazone/sulbactam(51.0%) and cefepime(39.4%).Conclusions: We should administer antimicrobial drugs according to the "Guiding principles for the clinical use of antibacterial drugs",and rationally choose antimicrobial agents in treatment of bacterial infections to control the bacterial resistance rates.

Abstract:
Objective: To investigate the changes and the clinical value of biochemical markers of myocardial damage in patients undergoing operation of heart valve replacement. Methods: The creatine kinase(CK),creatine kinase-MB(CK-MB),cardiac treponin I(cTnI),C reactive protein(CRP),lactate dehydrogenase(LDH) and hydroxybutyrate dehydrogenase(HBDH) in 40 patients receiving heart valve replacement were measured before operation, instantly after extracorporeal circulation, and at 6,24,48,72 hour and 6 days after the operation, and the changes were analyzed. Results: All the biochemical markers of myocardial damage were within normal limits except CRP before operation. The figures of cTnI and CK-MB increased significantly at 6 hour after operation(P<0.01).Aspartate aminotrans ferase(AST),LDH and HBDH rose significantly at 24 hour after operation.CRP reached the peak at 48 hour after operation. The two variables of cTnI and CK-MB in serum showed a positive linear correlation and had a higher correlativity comparable to other markers. Conclusions: All the biochemical markers of myocardial damage present abnormal changes successively during the perioperation of cardiac valve replacement.Monitoring the myocardial biochemical markers can decrease sudden death and incidence of other cardiac events.

Abstract:
Objective: To analyze the anxiety in patients with total hip replacement during perioperation and the related psychosocial factors. Methods: The anxious status of 47 patients receiving total hip replacement were evaluated by Hamilton depression rating scale(HAMD),and the influencing factors were assessed by self-designed questionnaire and operation questionnaire. Results: Anxiety symptoms were noted in 31 cases before operation and in 17 cases after operation. The preoperative scores on HAMD were significantly higher than that in the postoperative ones.Postoperative anxiety was positively correlated to the preoperative anxiety symptoms(P<0.01).The anxiety was related to the operation, postoperative complication, joint functional recovery, cost of treatment, acknowledge to the disease and postoperative support from relatives and friends(P<0.05 to P<0.01).Conclusions: The incidence of postoperation anxiety in patients receiving total hip replacement is higher than that after operation.Patients with pre-operation anxiety symptoms have a higher risk of postoperative anxiety. The anxiety in patients with total hip replacement is correlated to the factors such as the condition of the operation, social support and so on.

Abstract:
Objective: To investigate the effect of informational support on restlessness of patients with retained urethral catheterization in the stage of recovery from general anesthesia. Methods: Sixty esophageal cancer patients were randomly divided into two groups:The intervention group(n=30) were interviewed by the operating room nurses before operation and inducement of general anesthesia, and were given informational support about urethral catheterization besides the routine health guidance;the control group(n=30) received only the routine health guidance. The restlessness in the stage of recovery from general anesthesia was compared between the two groups. Results: The restlessness in the intervention group was much relieved compared with that in the control group(P<0.01).Conclusions: Preoperative informational support to patients with retained urethral catheterization could obviously relieve their restlessness in the recovery stage from general anesthesia.
