Methodology for detection of CDR3 polymorphism of human TCRδ2 gene repertoire by gene scan technique

Abstract:
Objective:To establish a method for detecting the CDR3 length polymorphism of TCR δ2 gene repertoire of γδ T cells in human peripheral blood by gene scan technique.Methods:otal RNA was extracted from fresh isolated peripheral blood mononuclear cells (PBMCs) or PBMCs stimulated and expanded with IL-2 cultivated alone (IL-2 group) or with Mtb-HAg plus IL-2 (Mtb-HAg group) cultivated for 7 days.The expression of CDR3 of δ2 gene in the three groups was detected by reverse transcriptase polymerase (RT-PCR),and the CDR3 length and polymorphism of TCR δ2 gene was analyzed by native polyacrylamide gel electrophoresis (native-PAGE) and gene scan technique.Results:The PCR product of the CDR3 of δ2 gene was showed wide and diffused bands by native-PAGE in the three groups,and there were no significant difference.While through gene scan analysis,all PCR products in the three groups were displayed more clear fragments (peaks),and the length of the predominant fragments (peaks) in Mtb-HAg group was significantly increased compared to PBMCs group and IL-2 group (P<0.01),but no difference between PBMCs group and IL2 group.Conclusions:The results suggested that gene scan technique is a convenient and rapid method for detection and analysis of the CDR3 length polymorphism of TCR δ2 gene of γδT cells in human peripheral blood.
Effects of ubiquitin carboxy-terminal hydrolase L1 activity on tau phosphorylation in primary cortical neuron

Abstract:
Objective:To investigate the effects of ubiquitin carboxy-terminal hydrolase L1 (UCH-L1) activity on tau phosphorylation in primary cortical neuron cultures.Methods:The primary cortical neuron cultures were treated with LDN-57444,the specific inhibitor of UCH-L1.The activity of UCH-L1 in the neuron was determined,and the level of tau phosphorylation was detected by Western blot.Results:With the evident inhibition of UCH-L1 activity,the levels of tau phosphorylation at the sites of Ser199,Ser202 and Ser396 were significantly increased (P<0.01),while the level of total tau was not changed (P>0.05).Conclusions:Inhibition of UCH-L1 could induce the accumulation of hyperphosphorylated tau in the primary cultured neurons.UCH-L1 dysfunction might be involved in the pathogenesis of Alzheimer's disease.
Influnnce of Bu Shen Prescription on local angiotensin Ⅱin myocardial tissues of rats with pressure overload

Abstract:
Objective:To observe the influence of Bu Shen Prescription on the level of local angiotensin Ⅱ (AngⅡ) in the left ventricular myocardial tissue of rats with pressure overload,and to study the possible mechanism of regressing the left ventricular remodeling.Methods:Sixty SD male rats were randomly divided into five groups:shan operatien group,untreated group,captopril group,Bushen prescription high dosage group and the low dosage group,12 rats in each group.Left venticular mass index (LVMI) was measured,and pathological morphology was observed.Radioimmunoassay was used to detect the local AngⅡlevel of the left ventricular myocardial tissue.Results:The AngⅡ level of the left ventricular myocardial tissue in model group was higher than that of sham operation group (P<0.01).In captopril group,Bu Shen Prescription high dosage group and low dosage group,the AngⅡ level was lower than that in model group (P<0.01).The left ventricular myocardial pathological morphology was similar except untreated group.Conclusions:Bu Shen Prescription and captopril have similar effects in regressing the left ventricular remodeling in rats with pressure overload,which may be concerned with the decrease of local AngⅡ level of the left ventricular myocardial tissue.
Study on the relationship between Helicobacter pylori L-form infection and the genesis and development of gastric carcinoma

Abstract:
Objective:To investigate the role of Helicobacter pylori L-form (Hp-L) in the genesis and development of gastric carcinoma.Methods:The specimens of chronic atrophic gastritis (CAG),intestinal metaplasia (IM),gastric epithelial dysplasia (GED) and gastric carcinoma (GCa) were collected from each 40 cases,and 40 cases of chronic superficial gastritis (CSG) (mild) acted as control.Hp-L was examined in each group by Gram's staining and immunohistochemical stain (SP method);The expression of skp2 was detected by Immunohistochemistry (IHC) and ISH,and the expression of Ki-67 and p53 were also examined by IHC.Results:The positive rates of Hp-L in CAG,IM,GED and GCa were 22.5%,40.0%,65.0% and 55.0%,respectively,demonstrating an increasing trend except for GCa group.The positive rate of mRNA and protein of Skp2 in CAG,IM,GED and GCa in the positive Hp-L groups of GCa were higher than that in the negative groups.The difference had statistical significance (P<0.05-P<0.01).Conclusions:Hp-L may play an important role in the evolution course of CAG,IM,GED and GCa,and correlates to the development of gastric carcinoma.Its mechanism of promoting the occurrence and development of gastric carcinoma may up-regulate the expressions of skp2,Ki-67 and mutant p53.
Expression and clinical significance of E-cadherin in esophageal squamous cell carcinoma

Abstract:
Objective:To investigate the expression of E-cadherin as well (E-cad) as its correlation with the invasion and metastasis of esophageal squamous cell carcinoma (ESCC).Methods:The expression of E-cad in 76 cases of ESCC and 30 cases of normal esophageal squamous epithelia was detected by SP method of immunohistochemical staining,and its clinicopathological features were analyzed.Results:The abnormal expression rate of E-cad in ESCC and normal esophageal squamous epithelia was 59.21% and 6.67%,respectively (P<0.01).The abnormal expression of E-cad was related to the depth of invasion,the degree of cell differentiation and lymph node metastasis (P<0.05-P<0.01).Conclusions:The expression of E-cad in ESCC is abnormal.And the abnormal expression rate is related to the depth of invasion,the degree of cell differentiation and lymph node metastasis.
Effect of intrapleural injection of urokinase on tuberculous pleurisy

Abstract:
Objective:To investigate the effect of intrapleural injection of urokinase in treatment of tuberculous pleural effusion.Methods:Ninety-six cases of exudative tuberculous pleurisy were randomly divided into treatment group and control group.The cases in treatment group were treated with 100 000 IU urokinase (diluted by 20 ml normal saline) after effusion drainage,while the cases in control group were given normal saline only.The other therapies in both groups were the same.Results:In the treatment group,the time of pleural effusion absorption,the frequency of stetho-prick,the incidence of pleural adhesion,and the mean thickness of pleura all decreased significantly as compared with those of the control group (P<0.01).The lung function indexes FEV1% and FEV1.0/FVC% in the treatment group were significantly superior to those of the control group (P<0.01).The changes of thrombin time,prothrombin time and activated partial thromboplastin time after administration had no significant difference in the treatment group.Conclusions:The therapy of thoracic injection of urokinase can promote the absorption of pleural effussion,lessen the pachynsis pleurae and adhesion,and improve the lung function.
Diagnosis and treatment of nonurothelial bladder cancer:a report of 15 cases

Abstract:
Objective:To improve the diagnosis and treatment of nonurothelial cancer of the bladder.Methods:Fifteen cases of nonurothelial cancer of the bladder were reviewed.The diagnostic methods included ultrasound,pelvic CT scan,cystoscopy and biopsies.Partial cystectomy was performed in 14 cases,and radical cystectomy in 1 case.Results:Adenocarcinoma was observed in 11 cases,and squamous cell carcinoma in 4 cases.The mean survival time was 15.6 months.Conclusions:The nonurothelial bladder cancer is rarely seen.It's highly malignant and has a poor prognosis.Early diagnosis and surgical treatment are essential.Radical cystectomy is recommended as the first choice for treatment of the disease.
Correlation between serum soluble E-selectin,interleukin-10 and the degree of coronary artery lesions in acute coronary syndrome

Abstract:
Objective:To explore the correlation between the levels of soluble E-selectin (SES),interleukin-10 (IL-10) and the degree of coronary artery stenosis in acute coronary syndrome (ACS).Methods:Ninety-four patients receiving coronery artery angiography were divided into ACS group (39 cases),stable angina pectoris (SAP) group (23 cases) and control group (32 cases).The serum levels of SES and IL-10 were measured by ELISA,and the high-sensitive creactive protein (hs-CRP) was dertermined by immunoturbidimetry.Results:The serum levels of SES and hs-CRP in ACS group were significantly higher than those in SAP group and control group (P<0.01);the serum levels of SES and hs-CRP in SAP group had no significant difference compared with those of the control (P<0.05).SES and IL-10 had no obvious correlation with the count of coronary athey stenosis or the degree of coronary artery stenosis (P>0.05).Conclusions:Increased serum levels of SES and IL-10 may reflect the instability of atherosclerotic plaques but not the degree of coronary artery stenosis in acute coronary syndrome.
Efficasy of irinotecan plus fuorouracil/leucovorin in treatment of patients with advanced colorectal cancer

Abstract:
Objective:To observe the effects and side effects of irinotecan plus fuorouracil/leucovorin in treatment of patients with advanced colorectal cancer.Methods:Irinotecan 150-180 mg/m2 intravenous drip (90 min),fuorouracil 500 mg bolus and leucovorin 500 mg intravenous drip (2 h) were administered on day 1;1 250-1 500 mg/m2 was injected intravenously for 44 h,at least 2 cycles.Results:All the 20 patients received 73 cycles of the chemotherapy,and 19 patients were assessable.Complete remission was observed in 1 case,partial remission in 6 cases,disease stable in 5 cases and disease progress in 7 cases.The main adverse reactions were leucopenia,diarrhea,nausea and vomiting.No chemotherapy related death was noted.Conclusions:The combination therapy of irinotecan plus fuorouracil/leucovorin is effective for treatment of patients with advanced colorectal cancer,and the adverse reaction can be obviated or controlled.This therapy can be widely used in clinical application.
Comparison of local recurrence rate and cosmetic results after two types of radical operations for mammary cancer

Abstract:
Objective:To discuss the local recurrence rate and cosmetics results in breast cancer after breast conserving radica1 mastectomy and radical mastectomy conserving ammilla,skin combining simultaneous breast augmentationwith prosthesis.Methods:Eighty patients were divided into two groups.There were 45 patients in breast conserving radica1 mastectomy (group A) and 35 in radical mastectomy conserving ammilla,skin combining simultaneous breast augmentationwith prosthesis (group B).The loacal recurrence rate and cosmetics results were evaluated after surgery.Results:The loacal recurrence rate in groups A and B was 2.2% and 0,respectively,the ipsilateral breast reoperation rate in groups A and B was 31.1% and 0,respectively.The cosmetic results of group A were better than that of group B (P<0.05).Conclusions:Group B patients are also suitable for group A patients for operation.Radical mastectomy conserving ammilla,skin combining simultaneous breast augmentationwith prosthesis is the recommendatory for those who are suitable for two radical mastectomies,for it can reduce the ipsilateral breast reoperation rate.
Weekly docetaxel for treatment of advanced gastric cardia cancer in the elderly:a report of 28 cases

Abstract:
Objective:To observe the efficacy of weekly administration of domestic docetaxel on advanced cardia cancer in the elderly.Methods:Twenty-eight elderly patients with advanced cardia cancer receiving chemotherapy were treated with domestic docetaxel in the dose of 35 mg/m2 on day 1,8 and 15 every 28 days.Evaluation was performed after two cycles,and the patients who did not respond went through another two cycles.Results:All the 28 cases were evaluated.CR was observed in 0 cases,PR in 10 cases,SD in 6 cases and PD in 12 cases,with a response rate of 35.7%.Clinical benefit was achieved in 57.1% of the cases.The common toxicities were Ⅰ to Ⅱ degree neutropenia,aneamia and shed-hair.Conclusions:Weekly domestic docetaxel shows good efficacy in treatment of advanced cardia cancer in the elderly,and the patients tolerate well.
Treatment of the lumbar spinal canal stenosis coupled with disc protrusion of the elderly

Abstract:
Objective:To investigate the clinical characteristics and therapies of the lumbar spinal canal stenosis coupled with lumbar disc protrusion the elderly.Methods:Eighteen cases were operated on with Spinal decompression and spinal cord nucleus extraction,the other 18 cases were operated on with spinal cord nucleus extraction and internal fixation.Results:Following-up from 1.5 to 5 years,the efficient race of the patients using the spinal cord nucle us extraxtion and spinal clecompression is 83.3%,the efficient of these cases added to internal fixation is 94.4%.Conclusions:The elderly lumbar spinal canal stenosis coupled with lumbar disc protrusion should be operated on as soon as possible.The articular facet hyperplasia should be fixed and the anti-osteoporosis therapy should be carried out pre/post-operation.
Influencing factors for pathological diagnosis of lung mass

Abstract:
Objective:To explore the factors affecting the pathological diagnosis of lung mass.Methods:A retrospective analysis of 148 cases of mass distribution in lung was performed.The cases were divided into tumor group and non-tumor group according to the results of pathological diagnosis.Results:The incidence of lung cancer was higher in patients above the age of 60 than that under 60 (P<0.01);but the positive rate had not significant difference in gender or distribution of lung mass.Conclusions:The highest incidence of lung cancer is observed in patients with lung mass above the age of 60.
Pathological study of malignant fibrous histiocytoma of the heart

Abstract:
Objective:To investigate the pathological diagnosis and pathological characteristics of primary heart malignant fibrous histiocytoma.Methods:One case of primary cardiac malignant fibrous histiocytoma was observed by light microscopy and immunohistochemistry,and the literature was reviewed.Results:The tumor which was divided into three sub-leaves was located on the posterior wall of the left atrium,with a maximal diameter of 2.0 to 4.5 cm;microscopically,the tumor cells were mostly in the shape of spindle,showing fascicular arrangement;myxo-matrix,rich blood vessels and interstitial chronic inflammatory cell infiltration could be observed.Immunophenotype showed tumor cells of vimentin strongly positive,CD68,Ki-67 (>50%),SMA (focal) and S-100 protein positive,CD34 and desmin negative.Conclusions:Cardiac malignant fibrous histiocytoma is frequently misdiagnosed as atrial myxoma;correct diagnosis should be based on pathological examination and immunohistochemistry.This tumor is highly malignant,easy to relapse and has a poor prognosis.
Locking plate for treatment of extremities fracture:a report of 45 cases

Abstract:
Objective:To investigate the application of locking plate in treatment of the extremities fracture.Methods:Forty-five cases (51 fractures) of the extremities fracture were treated with less invasive stabilization system (LISS) or locking compress plate (LCP) by indirect reestablishment and minimally invasive techniques.Twenty-two patients with distal femoral or proximal tibial fractures were treated with LISS,and the others were treated with LCP.Results:All the cases were followed up for 1-17 months.No wound infection,delayed union,bone nonunion,loss of height of articular surface,displaced or angulationfracture,screw extraction or plate fragmentation occurred.The healing time of the fracture was 8-16 weeks.The patients were able to walk without support or grip after 12-16 weeks.The function of all the extremities restored well.Conclusions:Treatment of extremities fracture using locking plate with minimally invasive plate osteosynthesis techniques has the advantages of less soft tissue trauma,less damage to the blood vessel of the bone or periosteum,lower infection rate,rapid union and satisfactory function recovery.
Laser therapy combined with high ligation of the great saphenous vein for treatment of varicose veins in the lower extremities:analysis of 6 cases

Abstract:
Objective:To evaluate the short-term therapeutic effect of laser therapy combined with high ligation of the great saphenous vein on the treatment of varicose veins in the lower extremities.Methods:Endovenous Semiconductor laser therapy combined with high ligation of the great saphenous vein was adopted for treatment of varicose veins in the lower extremities.Results:The operation time was 45-70 min for each limb;The number of incisions was 2-5 in each limb;The hospital stay after operation was 2-4 day.Postoperatively,subcutaneous bleeding occurred in 2 limbs,skin burn in 1 limb,subcutaneous sclerosis in 2 limbs and saphenous nerve injury in 1 limb.Among the patients,6 (7 limbs) were followed up for 3-6 months,during which time no recurrence was found.Conclusions:Laser therapy combined with high ligation of the great saphenous vein is effective for treatment of varicose veins in the lower extremities.
Non-invasive biphasic positive airway pressure ventilation for post extubation respiratory support in the patients with palatopharyngoplasty

Abstract:
Objective:To evaluate the effect of non-invasive biphasic positive airway pressure (BIPAP) ventilation on post extubation respiration support the in patients with palatopharyngoplasty (PPP).Methods:Thirty patients with obstructive sleep apnea syndrome (OSAS) undergoing PPP post extubation in the post anaesthetic care unit (PACU) by non-invasive BIPAP ventilation.Systolic blood pressure (SBP),diastolic blood pressure (DBP),heart rate (HR),pulse blood oxygen saturation (SpO2) and end expiratory carbon dioxide partial pressure (PetCO2) before or after non-invasive BIPAP were recorded.Results:Thirty patients with PPP post extubation wered received non-invasive BIPAP ventilation because of hypoxia and hypercapnia.SpO2 increased and PetCO2 decreased significantly after non-invasive BIPAP ventilation 30 minutes and one hour,compared with before non-invasive BIPAP ventilation,however,SBP,DBP,and HR had no difference before and after the non-invasive BIPAP ventilation.Conclusions:Non-invasive BIPAP ventilation is suitable for post extubation respiratory support in patients with OSAS post PPP because of hypoxia and hypercapnia in the PACU.
Surgical management of acetabular fractures:a report of 32 cases

Abstract:
Objective:To explore an effective surgical technique for management of acetabular fractures.Methods:Thirty-two cases of acetabular fractures were treated by surgical procedures.Nineteen cases were operated through Kocher-lengenbeck approach,8 cases ilioinguinal approach and 5 cases combined anterior and posterior approaches.All the cases were internally fixed with the reconstructive plate and screws.Results:All the cases were followed up for an average of 18.5 months (ranging from 1 to 48 months).The function of the hip was evaluated by the mordified Merled' Aubigne and Postel score system.The results were excellent in 15 cases,good in 10 cases,fair in 5 and poor in 2.The total excellent and good rates were 78.1%.Avascular necrosis occurred in 6 cases,traumatic arthritis in 5 cases and sciatic nerve injury in 2 cases.Conclusions:Early operation,proper surgical approach and stable internal fixation are essential to the outcome of the hip function.
Relations between body fluid balance and cardio-pulmonary complications in elderly patients after thoracotomy

Abstract:
Objective:To explore the significance of body fluid balance in prevention of cardio-pulmonary complications in elderly patients after thoracotomy.Methods:One hundred and thirty-five elderly patients undergoing thoracotomy were grouped according to the volume of liquid in 1-3 days after thoracic surgery.The cases with positive balance of body fluid (≥ 800 ml) were enrolled in groupⅠ,cases maintaining negative balance in groupⅡ and the others in group Ⅲ.A retrospective analysis and comparison about the incidence of cardio-pulmonary complications were made among the three groups.Results:In group Ⅰ,9 cases presented postoperative cardio-pulmonary complications,including 5 cases of cardiovascular complications and 8 cases of respiratory complications,and 2 patients died;in group Ⅱ,5 cases presented postoperative cardio-pulmonary complications,including 2 cases of cardiovascular complications and 4 cases of respiratory complications,and no one died;in group Ⅲ,15 cases developed postoperative cardio-pulmonary complications,including 5 cases of cardiovascular complications and 13 cases of respiratory complications,and 1 patient died.The incidences of arrhythmia,heart failure,lung infections,asthma,hypoxemia,pulmonary edema and respiratory failure complications differed significantly among the three groups (P<0.05-P<0.01).In group Ⅱ,the incidence of hypoxemia had a significant reduction and no pulmonary edema or respiratory failure occurred.Conclusions:Positive fluid balance should be avoided in the elderly patients early after thoracotomy.Positive fluid balance may be turned to negative fluid balance by controlling the liquid input,which would contribute to the reduction of morbidity and mortality due to postoperative cardiopulmonary complications.
Clinical analysis of 35 cases of transvaginal hysterectomy for non-prolapsed uterus

Abstract:
Objective:To appraise the technique and effect of transvaginal hysterectomy (TVH) for non-prolapsed uterus.Methods:Thirty-five cases undergoing TVH were included in the observation group,and 35 cases receiving total abdominal hysterectomy (TAH) acted as control.The average operation time,hemorrhage,time of exhausting,time of out-of-bed activity and postoperative hospital stay were compared and analyzed between the two groups.Results:The operation time had no significant difference between the two groups (P>0.05),but the hemorrhage during operation,time of exhausting,time of out-of-bed activity and posthospital stay in the observation group were obviously less than that in the control (P<0.01).Conclusions:TVH is highly effective for non-prolapsed uterus.Compared with TAH,TVH has the advantages of lower damage,less hemorrhage,reduced abdominal cavity sufferings and interference,higher recovery,shorter postoperative hospital stay and no scar left on the abdominal wall.Therefore,TVH is worthy of popularization in clinical application.
Role of neutrophils in early progressive injury of deep partial-thickness burn wound

Abstract:
Objective:To observe the role of neutrophils in early progressive injury of deep partial-thickness burn wound.Methods:The neutrophils in 360 cases of deep partial-thickness burn wound were counted,and the recovery time was observed.Results:The count of neutrophils in all the patients was high at the time of admission.The faster the count of neutrophils descended,the longer the recovery time was.Conclusions:The tissue injury induced by neutrophils occurs before hospitalization.Anti oxidant agent and proteinase inhibitor should be administered immediately after the patients are hospitalized.

Objective:To evaluate the security and efficacy of topical anesthesia in small incision non-phacoemulsification cataract extraction.Methods:Small incision non-phacoemulsification cataract extraction and intraocular lens (IOL) implantation were performed in 53 eyes of 45 patients under topical anesthesia with Benoxil;the anesthetic effects and surgical complications were observed.Results:The operation was carried out successfully in 52 eyes,with no additional anesthesia;only 1 eye was applied sub-tenon's capsule anesthesia for suture fixation of IOL.Conclusions:Topical anesthesia with Benoxil is a safe and reliable method for cataract surgery.

Abstract:
Objective:To assess the value of endoscopic decompression with memorized Nitinol alloy network stent in treatment of malignant stricture of esophagus and cardia.Methods:Thirty patients with unrespectable malignant stricture of esophagus and cardia were emplaced memorized Nitinol alloy network stent through gastroscope.For patients with serious stricture,the stents were inserted after Savary's dilation.Results:Successful stent placement was achieved in all the 30 cases,with an effective rate of 100%;28 patients were emplaced one stent and 2 patients two stents.Conclusions:The emplacement of Nitinol alloy network stent is effective for treatment of malignant stricture of esophagus and cardia (operation useless or unrespectable).It may extend the life span of the patients and improve their quality of life.The operation is safe and applicable.

Abstract:
Objective:To analyze the X-ray plain film and CT image of symptomatic accessory navicular bone.Methods:Twenty-eight cases (32 feet) of symptomatic accessory navicular bone which had been confirmed by clinical presentation,X-ray plain film and CT were taken routine orthotopic and oblique X-ray plain film.The pictures of both feet of 12 cases were compared and 20 cases were performed coronal CT routine scan.Results:Twenty-four feet presented soft tissue swelling,19 feet sclerosis and hyperplasia of accessory navicular bone,9 feet cystic degeneration of accessory navicular bone and 3 feet fragmentation of accessory navicular bone;Sclerosis and hyperplasia of navicular bone were observed in 13 feet,integration of navicular bone and accessory navicular bone in 4 feet.Three feet showed widened distance between navicular bone and accessory navicular bone,and 2 feet navicular bone hypertrophy.Conclusions:The symptomatic accessory navicular bone can be diagnosed by X-ray plain film and typical clinical presentation.CT routine scan image has great supplementary value.

Abstract:
Objective:To explore the clinical significance of serum marker at the metaphase of pregnancy in screening fetus Down's syndrome (DS).Methods:The concentration of alpha-fetoprotein (AFP) and free β-subunit of human chorionic gonadotrophin (β-hCG) in the serum of women at the metaphase of pregnancy were tested by time-resolved fluoroimmunoassay.Multiples of the median (MOM) and the risk probability were calculated concerning the age,gestation and weight of the women by homemade DS screening software.Results:Three thousand and one hundred and seventeen women at the metaphase of pregnancy were screened;177 of them were in high risk of DS,and the high-risk rate was 5.68%,which was lower than that calculated by imported analysis software.Eighty-two of the 177 high risk cases received chromosome test by amniotic fluid puncturation and 3 cases were confirmed.Two of the remaining 95 cases which failed to take amniotic fluid test resulted in spontaneous abortion,and no DS newborns were delivered in women with negative screening results.The group over the age of 35 had a higher positive rate than that below 35.The difference was significant (P<0.01).Conclusions:The double-marker test with AFP and free β-hCG in serum of women at the metaphase of pregnancy using homemade analysis software is a feasible prenatal screening technology easily accepted by pregnant women.The MOM output by homemade software is more suitable to the Chinese,which may reduce the false positive rate of screening and diminish the waste of medical resources.
Comparison of cefoxitin and moxalactam disc diffusion methods for detection of methicillin resistance in coagulase-negative staphylococci

Abstract:
Objective:To compare cefoxitin and moxalactam 30 μg disc diffusion (DD) methods in detecting methicillin-resistance in coagulase-negative staphylococci (MRCoNS).Methods:Two hundred and fourteen strains of clinically isolated coagulase-negative staphylococci (CoNS) were tested by cefoxiti and moxalactam 30 μg DD methods,and the result was compared with that by penicillin-binding protein (PBP2a) and polymerase chain reaction (PCR) detection.Results:One hundred and eighty-six strains of MRCoNS were identificated by PCR and PBP2a) latex agglutination test.The sensitivity/specificity of cefoxitin and moxalactam were 100.0%/100.0% and 90.3%/87.1%,respectively.The results of two tests were consistent in 99.4% of the isolates.Conclusions:Moxalactam 30 μg DD method has the same sensitivity and similar specificity with cefoxitin DD method in detecting CoNS.Two DD methods show a similar intrinsic performance in detecting methicillin resistance in CoNS.

Objective:To explore the clinical significance of the increase of heterotypic lymphocyte in peripheral blood.Methods:A total of 76 peripheral blood smears which contained more than 0.05 heterotypic lymphocyte affirmed by microscopy after Wright's staining were collected.Results:The heterotypic lymphocyte exceeding 0.1 were detected in thirty-nine of the samples (51%).The ratio of heterotypic lymphocyte infected by Epstein-barr virus and cytomegalovirus increased obviously.Conclusions:There is an intimate relation between the ratio increase of heterotypic lymphocyte in peripheral blood and some diseases.Different diseases present significant differences in the number of heterotypic lymphocyte in peripheral blood.

Abstract:
Objective:To investigate the capacity of tobacco control among medical students,and to provide the basis for promoting tobacco control model among medical students.Methods:In a cross-sectional survey,self-administered questionnaires were completed by 864 medical students.Results:The current smoker rate among respondents was 20.3% (175/864),and was higher among male medical students than among female students (31.5% vs 4.2%).There was no significant difference in the current smoker rate of male students among different specialties.But rural respondents were more likely to be current smokers (34.6%) than urban respondents (25.3%).The respondents students were lack of knowledge about smoking hazards:87.8% of medical students knew that smoking increased the risks of lung cancer,but only 30% -40% of respondents knew that smoking increased the risks of heart disease and osteoporosis,and about 50% of respondents agreed with these three myth conceptions:Nicotine is the chemical substance which increases the risks of multiple cancers,smoking low-tar or low-nicotine cigarettes is less harm,the filter can reduce the hazards of smoking.With respect to attitudes towards tobacco control in public places,most medical students did not feel positive:1/3 of respondents supported a total smoking ban in hospitals,schools,workplaces and meeting-rooms,and less than 20% of respondents supported a total smoking ban in waiting rooms,restaurants,KTV rooms,and 12.3% (106/864) of respondents supported a total smoking ban in different public places.Conclusions:The study showed that medical students had deficiencies in their knowledge of smoking-associated adverse effects,the attitude for tobacoo control was not positive,and the capacity of tobacco control among medical students was weak.Since medical students are future health promoters,misconceptions in this regard probably will become a major professional problem.In general,this indicates it is needed for medical schools to put more priorities on developing health education programm and effective intervention plan to enhance the tobacco control ability.
Nursing intervention for relieving difficult urination after removal of the catheter

Abstract:
Objective:To observe the role of nursing intervention in preventing difficult urination or acute urinary retention after the urinary catheter was removed.Methods:Sixty patients detaining an indwelling urinary catheter after the orthopedic operation were randomly divided into test group (30 cases) and control group (30 cases).The test group were applied urethral external aperture fomentation after the urinary catheter was removed,while the control group adopted lower abdominal fomentation.The two groups were released of the urinary catheter one day after the operation and removed of the catheters when the patients had a filled bladder and micturition desire.Results:The first uresis time of the test group was (28.6±4.4) minutes after the urinary catheters were removed,and the successful rate of urination was 93.3%;While it was (68.3±4.6) minutes in the control group,and the successful rate of urination was 63.3%.The difference was significant (P<0.01).Conclusions:Timing opening of the urinary catheter and urethral external aperture fomentation after the urinary catheter is removed may effectively relieve the difficulty of the patients in urination and avoid reinsertion of the urethral catheterization.
