Influence of genistein on spike encoding of cerebellar Purkenje cells after ischemia

Abstract:
Objective:To observe the influence of genistein on spike encoding of cerebellar Purkenje cells after ischemia.Methods:The microelectrode and whole cell voltage clamp technique were used in the experiment.The changes of spike encoding of cerebellar Purkenje cells were detected during the ischemia after the cells were incubated with genistein at 200 mol/L for 10 min.Results:After ischemia,the values of inter-spike interval of sequential spikes were shortened and the values of standard deviation of spike timing were increased,the cells were overexcited,the accuracy of spike encoding was decreased,which were prevented by genistein.Conclusions:Genistein can prevent ischemia-induced over-excitation and stabilize the spike encoding of cerebellar Purkinje cells after ischemia.
The experimental study of the transfection recombinant lentiviral vector with FasL gene into rat kidney

Abstract:
Objective:To explore the target gene expression of SD rat kidney transfected recombinant lentiviral vector with FasL and provide a basis of transplantation study.Methods:The recombinant lentiviral vector and empty vector(control) were injected into SD rat kidney through renal artery.The organization form and gray value of kidney tissues were detected by HE and immunohistochemistry(IHC) at days 3,7,15,25 and 40 after transfection and statistic analyzed.Results:No morphologic changes and inflammatory cells infiltration were found in kidney by HE staining..The peak of FasL gene expression was on the 15 day,which had significant difference compared with the control group(P0.05).Conclusions:The recombinant lentiviral vector with FasL gene can be successfully transfected into SD rat kidney,which can express target gene.
Pharmacodynamics effects of stachys floridana schuttl.ex benth polysaccharide on blood sugar in streptozotocin-induced diabetic mice

Abstract:
Objective:To observe the hypoglycemic effects of stachys floridana schuttl.ex benth polysaccharide(SFSBP)in hyperglycemic mice induced by streptozotocin.Methods:Forty-eight mice were randomly divided into six groups,in addition to the blank control group,intraperitoneal injection of an equal volume of saline,the other groups were intraperitoneal injection of streptozotocin 150 mg/kg to built diabetic mice model,SFSBP was given by 10 d.The blood sugar level of mice were determined by hexose kinase method.Results:SFSBP 50,100 and 200 mg/kg could significantly decrease the blood sugar level in streptozotocin-induced diabetic mice(P0.01).Conclusions:SFSBP has the hypoglycemic action on streptozotocin-induced diabetic mice.
Cloning and sequencing of GP50 gene from cysticercus cellulosae

Abstract:
Objective:To set up a way of detecting cysticercosis quickly and accurately by use of recombinant protein,the GP50 gene was amplified from cysticercus cellulosae and cloned into pGEM-T vector.Methods:The specific primers were designed and synthesized.The DNA fragment encoding GP50 gene was amplified by PCR from cDNA of C.cellulosae.The PCR products were cloned into pGEM-T vector and recombinants were screened by PCR,enzyme digestion and sequencing.Results:The size of pGEM-T-GP50 digested with BamH Ⅰ and Xho Ⅰ was identical to the length of the PCR generated products.DNA sequencing indicated that the GP50 gene had an open reading frame of 897 bp,which encodes 298 amino acids with an approximate molecular weight of 33 000 and had high homology with the GP50 gene submitted to GenBank(AY212945).Conclusions:The findings suggested that GP50 gene had been successfully cloned.
The failure causes and countermeasures of ureteroscope pneumatic lithotripsy

Abstract:
Objective:To explore the failure causes and countermeasures of ureteroscope pneumatic lithotripsy(URSL).Methods:The failure causes of 12 cases with ureteral calculi treated with URSL were analyzed retrospectively.Results:5 cases with tortuous and narrow ureter and 1 case with ureteral polypus were open surgery.There were 6 cases with stone going up to pelvis or kidney in operation process,and 3 cases with upper stone and 3 cases with middle stone.Six cases with stone going up were treated with placing double J tube and extracorporeal shock-wave lithotripsy.Conclusions:Stone migrations,tortuous or narrow ureters,ureteral perforation and polypus wrapping are the main failure causes of URSL.To raise operating technique andcareful handling can reduce failure.

Abstract:
Objective:To evaluate the effect of the treatment vascular malformation with endovascular embolization through percutaneous guided by digital subtraction angiography(DSA).Methods:Twenty cases with vascular malformation on surface were confirmed according to clinical and imaging features(including artery malformation 7 cases,vein malformation 9 cases,arteriovenous malformation 4 cases).Eleven cases of artery and arteriovenous malformation were treated with bleomycin and polyvingl alcohd,the other 9 cases were injected with bleomycin and ethanol guided by DSA,each course interval was 3 weeks.Results:The anomalies in 12 cases were cured,favorable improvement in 8 cases.No cases appeared tissue necrosis and skin burst.Conclusions:The treatment mothod for vascular malformation is easy,safe and effective,and can reduce or avoid complications,worth populararizing in clinical practice.
Clinical analysis of 92 cases of undifferentiated connective tissue diseases

Abstract:
Objective:To explore the clinical features and serological characteristics of patients with undifferentiated connective tissue diseases(UCTD) so as to elevate the cognition of UCTD.Methods:The clinical features,laboratory data and management of 92 patients with UCTD were analyzed retrospectively.Results:The most common clinical manifestations were the joint disease(67 cases),fever(30 cases),Raynauds phenomenon(25 cases),decreased white blood cell count(13 cases),interstitial lung disease(8 cases),and dry mouth and/or dry eye(7 cases).The positive rate of serum anti-nuclear antibody reached 100%,among which anti u1RNP antibodies accounted for 29 cases,anti-SSA/RO60 21 cases,anti-SSA/RO52 16 cases,elevated immunoglobulin 23 cases and decreased complement C3 17 cases.Conclusions:The clinical and serological manifestations of UCTD are moderate simplified connective tissue diseases.Excepting for the interstitial lung disease,the symptoms of other diseases are generally mild and the prognosis is good.The conversion of the patients needs further follow-up.
The research on the relativity between plasma visfatin,nitric oxide and type 2 diabetes and diabetic nephropathy

Abstract:
Objective:To explore the relationships of visfatin,nitric oxide(NO) and the metabolic markers and observe their roles in type 2 diabetes and the development in diabetic nephropathy(DN).Methods:Seventy-two patients with diabetes,in which 40 patients with diabetic nephropathy and 32 patients without complications were selected,and 24 healthy control cases whose age and body mass index(BMI) matched were also selected.The plasma level of visfatin was measured by enzyme-linked immunosorbent assay,and the level of plasma NO was measured by nitrate reductase method.While blood glucose,blood lipids,insulin,C peptide,proinsulin,glucagon,BMI,waist-hip ratio(WHR),high sensitivity C-reative protein and other related indicators were also measured.Insulin resistance was evaluated by insulin resistance index the homeostasis model assessment-IR(HOMA-IR),and insulin secretion index the HOMA- was used to evaluate -cell insulin secretion.The correlation between each index and microvascular complications of diabetes and diabetic nephropathy were analyzed.Results:The levels of plasma visfatin were 47.463.94,50.244.93,and 27.224.39 in diabetes cases without DN(nDN),diabetic cases with DN and control group respectively,and there had statistical differences among the three groups(P0.01);the plasma NO levels were 52.056.14,44.056.43,61.014.76 in diabetic patients nDN,diabetes with DN and control group,which had statistical differences(P0.01).The correlation analysis showed that there had positively correlation with the plasma visfatin level and the concentrations of NO,HOMA-IR,fasting insulin,waist circum ference,WHR,triglyceride in type 2 diabetes.Furtherly,HOMA-IR,WHR were independently associated with visfatin.Conclusions:Plasma visfatin and NO were related with type 2 diabetes and DN.
Clinicopathologic analysis of uterine fibromatosis of one case and review of the literature

Abstract:
Objective:To observe the clinicopathological features,immunohistochemical type and differential diagnosis of uterine fibromatosis.Methods:One case of uterine fibromatosis was studied with histopathology,immunohistochemistry examination and the related literatures were reviewed.Results:Grossly,the mass was nodulated and indistinct margin,with a white-grey cut surface.Microscopy revealed that the neoplasm was composed of spindle cells and collagen fibers,arrayed bunchy and striation,extended to the surrounding tissue.Immunohistochemical findings showed that the tumor cells were positive for vimentin,partly positive for SMA,but negative for Desmin,H-caldesnen and Calponin.Conclusions:The diagnosis of uterine fibromatosis mainly relies on pathological examination.Morphologically,it should be differentiated from other spindle cell lesions in the uterus,such as hysteromyoma,adenomyosis,fibrosarcoma,perivascular epithelioid cell tumor,leiomyosarcoma and endometrial stromal sarcoma.
Expression of S-phase kinase associated protein 2 in bladder transitional cell cancinoma and their clinical significance

Abstract:
Objective:To investigate the expression of S-phase kinase associated protein 2(Skp2) and their association with clinical pathological characteristics in bladder transitional cell cancinoma(BTCC).Methods:Skp2 protein was examined by immunohistochemical S-P staining method in 63 cases of BTCC and 15 cases of normal bladder tissue.Results:The positive rate of Skp2 in BTCC and normal bladder urothelial tissue were 47.6% and 6.7%,respectively,there was significant difference(P0.01).The expression level of Skp2 in BTCC was positively correlated with tumor grade and clinical stage(P0.01).Conclusions:Skp2 expression might play an important role in the development and progression of BTCC,and the examination of Skp2 expression might be a guide for diagnosis and a determinant of malignancy and prognosis of BTCC.
The effects of different dosages of remifentanil on time and dose with propofol target controlled infusion on general anesthesia patients

Abstract:
Objective:To investigate the effects of different dosages of remifentanil on dose and time in propofol target controlled infusion(TCI) induction.Methods:Forty patients,classified as ASAⅠ-Ⅱ,undergoing general anesthesia were randomly divided into four groups:group Ⅰ,group Ⅱ,group Ⅲ and group Ⅳ.Anesthesia was induced with propofol target controlled infusion,the target plasma concentration of propofol was started with 1.5 mg/L,and increased 0.5 mg/L every four minutes.When patients lose response,the patients received remifentanil infusion,the dosage was set at 0.1,0.2,and 0.4 gkg-1min-1 in group Ⅱ,group Ⅲ and group Ⅳ,respectively.The group Ⅰ as a controll received normal saline.When modified obsever is assessment of alertness/sedation scale is zero,0.6 mg/kg tracrium was given to facilitated tracheal intubation after 3 min.The infusion time of remifentanil and total dose of propofol were recorded.The adverse effects such as hypotension,bradycardia and respiratory depression were recorded during induction.Results:The propofol dosages in group Ⅰ were higher than those in group Ⅱ,group Ⅲ and group Ⅳ(P0.05-P0.01).The propofol dosages in groupⅡ were higher than those in group Ⅲ and group Ⅳ(P0.01 and P0.05);There was no significant difference between groupⅢ and group Ⅳ(P0.05).The infusion time of groupⅠ was not significant different with group Ⅱ(P0.05),but there were significant differences with group Ⅲ and group Ⅳ(P0.01).In contrast to Group Ⅱ,there were differences with group Ⅲ and group Ⅳ(P0.01);and differences between group Ⅲ and group Ⅳ(P0.05).The accidence of adverse effects had significant differences(P0.01).Conclusions:Different doses of remifentanil can reduce the infusion time of propofol TCI,reduce the dose of propofol;propofol TCI induction combined with 0.2 gkg-1min-1 remifentanil infusion can meet the clinical needs.
Significance and expressions of HIF-1 and Bmi-1 in diffuse large B cell lymphoma

Abstract:
Objective:To study the expressions and relationships of hypoxia-inducible factor-1(HIF-1) and B-cell-specific moloney leukemia virus insert site 1(Bmi-1)in diffuse large B cell lymphoma(DLBCL) tissue,and explore their roles and significance in the progression of carcinogenesis.Methods:The expressions of HIF-1 and Bmi-1 in 64 paraffin wax samples of DLBCL and 40 lymph node reactive hyperplasia tissue samples were detected by immunohistochemistry.The relationships and clinical features of HIF-1 and Bmi-1 in DLBCL were analyzed.Results:The postive expression rates of HIF-1 and Bmi-1 in DLBCL were 67.19% and 64.06% respectively,there were significantly statitical differences(P0.01)compared with the control.The significantly different expressions of HIF-1 and Bmi-1 were related with clinical stage and IPI(international prognostic index)(P0.01),and there were no obvious correlation with sex,age,out of lymph nodenode invasion and systemic symptoms of patient(P0.05).Conclusions:HIF-1 and Bmi-1 play a synergistic role in the progression of DLBCL,and can be used as an efficacious and prognostic reference indication of DLBCL.
Cost-effectiveness analysis of transvaginal hysterectomy vs minilaporotomy hysterectomy

Abstract:
Objective:To compare the cost and effectiveness of transvaginal hysterectomy versus minilaporotomy hysterectomy.Methods:From Oct.2008 to Oct.2009,35 patients were undergone transvaginal hysterectomy(TVH),while 43 cases were submitted to minilaporotomy hysterectomy(MiniLPT).The off-work day,hospital charge,total cost(including direct and indirect cost) were analyzed and compared.Results:The direct cost and indirect cost of TVH were significantly lower than those of MiniLPT(P0.01).The return-work and off-work days were significantly shorter in TVH group than in MiniLPT group(P0.01).But the hospitalization time and drug cost in the two groups had no statistical difference(P0.05).Conclusions:Compared to MiniLPT,TVH can recover quickly and improve cost-effectiveness utilization and decreasing operation cost through decreasing direct and indirect cost.
Synergistic effect of aripiprazole on the treatment of treatment-resistant depression

Abstract:
Objective:To explore the efficacy and safety of small-dose aripiprazole combined with mirtazapine in the treatment of treatment-resistant depression.Methods:Forty-six patients with treatment-resistant depression were randomly divided into combined group(treated with aripiprazole plus mirtazapine)and single group(treated with mirtazapine alone),23 cases in each group,period of treatment was 6 weeks.Hamilton depression scale and treatment emergent symptom scale were used to evaluate the clinical efficacy and adverse effects before treatment and at the end of 1st,2nd,4th and 6th week treatment.Results:The therapeutic efficacy of aripiprazole combined with mirtazapine was significantly better than that of mirtazapine group after 4 weeks treatment(P0.05).There was no significant difference in adverse events between the two groups.Conclusions:Small-dose aripiprazole combined with mirtazapine can augment the efficacy in the treatment of treatment-resistant depression and has a better safety.
Effects of naloxone on treatment of type Ⅱ respiratory failure of chronic obstructive pulmonary disease

Abstract:
Objective:To explore the therapeutic effect of naloxone on the treatment of chronic obstructive pulmonary disease(COPD) complicated with type Ⅱ respiratory failure.Methods:Sixty-nine consecutive patients with COPD complicated with type Ⅱ respiratory failure were randomly divided into two groups:control group and therapy group.Thirty-four patients were in control group,16 of them had pulmonary encephalopathy.They were received conventional therapy.While 35 patients were in therapy group,18 of them had pulmonary encephalopathy.In therapy group,1.0 mg of naloxone and 20 ml normal saline were intravenously infused twice one day in addition to conventional treatment,6 days for a course of treatment.The recovery time of pulmonary encephalopathy and the changes of blood gas analysis in the two groups were observed.Results:There was significant improvement in therapy group about the recovery time of respiratory failure and pulmonary encephalopathy and the improvement of blood gas analysis compared to control group(P0.05-P0.01).Conclusions:Naloxone has the significant therapeutic effect in treatment of COPD complicated with type Ⅱ respiratory failure.
Surgery of acute angle-closure glaucoma with continuous high intraocular pressure

Abstract:
Objective:To explore the method and clinical effect of trabeculectomy for acute angle-closure glaucoma(ACG) with continuous high intraocular pressure(IOP).Methods:Fifty-six patients(56 eyes) were accepted trabeculectomy whose IOP were continuously over 40 mmHg after use of repressive pressure medicine more than 3 days,including dropping IOP before surgery,releasing aqueous humor slowly and repetitiously,using releasable suture and injecting blind source separation to renew the anterior chamber depth.Results:There were no serious complications such as suprachoroidal space bleeding and malignant glaucoma.The vision was between 0.01-0.1 in 16 eyes,0.1-0.8 in 40 eyes.Most of the cases had better vision than before(P0.01).The post-operative IOP was 21 mmHg in 48 eyes,which was significantly lower than before surgery(P0.01).Choroidal detachment was occurred in 3 eyes,delayed anterior chamber reformation for strong filteration in 2 eyes,and high IOP for low filteration in 2 eyes.Conclusions:It is necessary and safety to do trabeculectomy for ACG with continuous high state of IOP.
The clinical application of high-sensitivity C reactive protein level in maintenance hemodialysis patients

Abstract:
Objective:To study the clinical significance of serum high-sensitivity C reactive protein(hs-CRP) measurement,and explore the relationships between hs-CRP and immunoglobulin(IgG,IgA and IgM),complement(C3 and C4)in maintenance hemodialysis(MHD) patients.Methods:The level of serum hs-CRP was detected in 93 MHD patients and 40 healthy controls by an automated latex-enhanced immune transmission turbidity method.At the same time,the levels of IgG,IgA,IgM and C3,C4 were determined.According to the normal value(0-10mg/L) of hs-CRP,93 patients were divided into 2 groups:the normal hs-CRP group(63 patients) and the abnormal hs-CRP group(30 patients),and the changes of serum immunoglobulin and complement among the 3 groups were analyzed.Results:The level of hs-CRP in MHD patients was significantly higher than that in healthy controls(P0.01),but C3 was remarkably lower than that in healthy controls(P0.01).IgG in the abnormal hs-CRP group of MHD patients was significantly higher than that in the normal hs-CRP group(P0.01).Conclusions:The increasing of hs-CRP and IgG levels is beneficial to judge the chronic infection of MHD patients,and the decrease of C3 level is the important reason for the decline of humoral immunity in MHD patients.
The clinical observation of dexmedetomidine for preventing agitation in anesthesia recovery period

Abstract:
Objective:To observe the effect and safety of dexmedetomidine(Dex) for preventing agitation in anesthesia recovery period.Methods:Sixty patients(ASA Ⅰor Ⅱ) were randomly divided into two groups(n=30).In Dex group(group D),Dex was injected by micro pump 10 minutes before surgery and continued until wound was sutured.In fentanyl group(group F),1 g/kg fentanyl was injected when the wound was sutured.The spontaneous breathing recovery time,extubation time,the incidence and degree of anesthesia recovery period in all patients were observed.The recovery period with nausea and vomiting,drowsiness and respiratory depression and other adverse events were recorded.Results:There were no statistical differences about spontaneous breathing recovery time and extubation time in all patients of the two groups(P0.05).Compared with group F,the incidence and degree of agitation in group D were significantly lower(P0.01).Nausea,vomiting,drowsiness and respiratory depression were also significantly reduced(P0.01).Conclusions:Dex applied in general anesthesia can reduce the agitation of anesthesia recovery period and no respiratory depression.It is safe and effective for preventing agitation in anesthesia recovery period
Clinical analysis of 370 old patients with diabetes mellitus

Abstract:
Objective:To investigate the characteristics of diabetes mellitus in the old patients.Methods:The clinical data of 370 old patients with diabetes mellitus were collected and analyzed.Results:Compared with the male patients,the female patients were more easily affected by the peripheral arterial disease and asymptomatic bacteriuria(P0.05).Compared with the patients under the age of 70,the patients over 70 had a higher level of cystatin C and the incidence of asymptomatic bacteriuria,peripheral arterial disease and hypertension increased significantly(P0.05).Conclusions:The old patients with diabetes mellitus often suffer from many basal diseases and chronic complications,so their blood sugar should be controlled properly to avoid hypoglycemia;asymptomatic bacteriuria is one part of the chronic complications.
The therapeutic effect observation of zinc preparation on rotavirus enteritis

Abstract:
Objective:To observe the therapeutic effect of zinc preparation on rotavirus enteritis.Methods:A total of 72 cases of rotavirus enteritis were divided randomly into treatment group(34 cases) and control group(38 cases).The patients in treatment group were treated with zinc preparation plus conventional treatment,whereas the patients in control group were given conventional treatment only.The therapeutic effect of two groups was compared.Results:The efficiency was 88.2% and 68.4%,the hospitalization days were(4.621.78) and(5.631.99) respectively in treatment group and control group,there were a significant differences between the two groups in efficiency and hospitalization days(P0.05).Conclusions:Zinc preparation has the obvious curative effect on rotavirus enteritis,and it can reduce the hospitalization days and side effects,it is worthy for being widely used in clinic.
Expressions of villin,cytokeratin 7,and cytokeratin 20 in primary and intestinal tract metastatic liver tumors

Abstract:
Objective:To investigate the expressions of villin,cytokeratin 7(CK7) and cytokeratin 20(CK20) in differentiating primary and intestinal tract metastatic liver tumors and evaluate the diagnostic value of combined markers including alpha-fetoprotein(AFP),hepatocyte paraffin 1(Hep-1),CK19 and carcinoembryonic antigen(CEA).Methods:The expressions of CK7,CK20 and villin were detected immunohistochemically with S-P method in 40 cases of hepatocellular carcinoma,35 cases of cholangiocarcinoma,40 cases of metastatic bowel cancer and 20 cases of normal liver tissue.Tumor cases were also marked with Hep-1,CK19,CEA and AFP.Results:Primary and metastatic liver tumors had the specific histological structure and morphology relatively.Combined these markers including CK7,CK20,villin,AFP,Hep-1,CK19 and can CEA could differentiate hepatocellular carcinoma and metastatic colorectal cancer,there was statistical difference(P0.05).CK7 and CK19 could differentiate primary intrahepatic bile duct cell carcinoma and metastatic colorectal cancer,there was statistical difference(P0.05).Conclusions:Better diagnosis and differential diagnosis can be made in primary and intestinal tract metastatic liver tumors based on the histological observation and comprehensive clinical analysis,with reference to the combined immune makers panel.
Effect of travoprost on patients with angle-closure glaucoma after the surgery of phacoemulsification and goniosynechialysis

Objective:To investigate the efficiency of travoprost in lowering patients intraocular pressure who contained angle-closure glaucoma even after phacoemulsification and goniosynechialysis.Methods:Eleven angle-closure glaucoma patients whose intraocular pressures were not ideal after phacoemulsification and goniosynechialysis were selected.Four weeks after surgery,the intraocular pressure was measured,and the eyes were treated with 0.004% travoprost once every night.Intraocular pressure at 9am and 16pm,visual acuity,and visual field were checked at the first,second,forth week after travoprost treatment.Finally,the intraocular pressure values after treatment were compared with before treatment.Results:In contrast to before treatment of travoprost,at the first,second,forth weeks after treatment,the average intraocular pressure was reduced obviously(P0.01).Conclusions:Travoprost was safe and effective in treatment of angle-closure glaucoma after phacoemulsification and goniosynechialysis.

Abstract:
Objective:To observe the sedation and analgesic effects and safety of midazolam with propofol in endoscopic retrograde cholangiopancreatography(ERCP) for elder patients.Methods:Fifty-two elder patients were received midazolam plus propofol for sedation during diagnostic and therapeutic ERCP emergency procedure.The infusion scheme for a target blood propofol concentration of about 3 g/ml,which was according to 10-8-6 programme,consisting of a loading dose of 1 mg/kg of propofol combined with 0.02 mg/kg of midazolam,loading dose over 20 seconds followed immediately by an infusion of 10 mgkg-1h-1 of propofol for 10 minutes;8 mgkg-1h-1 for the next 10 minutes and 6 mgkg-1h-1 thereafter.The observing parameters including systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR),SPO2 were recorded on five time points:before infusion,1 minute after infusion,esophagoscope introducing,stop infusion and recovery period.The lasting time on every stage as well as side effects,satisfaction of the procedure were assessed.Results:SBP had no difference on each time point after infusion than before infusion(P0.05).Compared with before infusion,DBP was decreased significantly on the other time points before recovery(P0.05-P0.01).HR and SPO2 were not changed on each time point(P0.05).Anterograde amnesia was appeared in all cases and the total rate of amnesia was 100%.The total rate of patients satisfaction was 98.08%.Conclusions:Midazolam combined with propofol represents a safe and effective sedation technique for elder patients in emergency case of ERCP.

Abstract:
Objective:To investigate the clinical and pathological characteristics of the mammary hamartoma.Methods:The pathological sections of 25 cases of mammary hamartoma were stained ordinarily and immunohistochemically.Their clinical and pathological features were analyzed retrospectively.Results:All the patients were female and their ages ranged from 18 to 51.Fourteen tumors were located in the left breast and 11 in the right.Most of the tumors were aponia local masses with well-defined borders.The tumor histologically consisted of ducts,lobules,fibrous stroma and adipose tissues.Conclusions:The mammary hamartoma is a special type benign tumor which is rarely seen and difficult to diagnose before and after surgery for the absence of specific clinical manifestations.The prognosis is usually satisfactory.

Abstract:
Objective:To improve the life quality of early breast cancer patients treated with breast conserving therapy and postoperative radiotherapy.Methods:Forty-nine early breast patients patients who received breast conserving therapy and postoperative radiotherapy were treated and followed up in our department.Local recurrence rate,breast cosmetic outcome and survival rate were reported.Results:No local recurrence was found.Developed metastasis was happened in three cases.Disease-free survival rate for 3 years was 93.88%.Cosmetic result scored as good by patient and doctor were 91.83% and 85.71%,respectively.Acute skin reaction was the main complication.Conclusions:Satisfactory life quality and local control can be obtained in early breast cancer patients with breast conserving therapy and postoperative radiotherapy,but the doctors must master the conserving therapy indications and the comprehensive therapy technique strictly.

Objective:To observe the influence of different dosages of naloxone on morphines postoperation analgesia efficiency in elder patients.Methods:One hundred and twenty elder patients who received intravenous-inhalation combined anesthesia for abdominal operation were randomly divided into 4 groups,30 in each group.Group A:morphines dosage was 10 gkg-1h-1,which was diluted by sodium chloride injection to 100 ml.Group B:0.1gkg-1h-1 dosage of naloxone was added on the base of group A.Group C:0.2 gkg-1h-1 dosage of naloxone was added on the base of group A.Group D:0.3gkg-1h-1 dosage of naloxone was added on the base of group A.Analgesia mode:background infusion(2 ml/h) with patient controlled analgesia(1 ml),lock out time 15 min.Pain visual analogue scale(VAS),nausea and vomiting,pruritus and breath depression were recorded at the time of 2,6,12,24 and 48 hours after analgesia was begun.Results:Analgesia efficiency:VAS was no statistical significances at the same time point between group A,B and C.VAS was raisen at the time of 2,6,12,24 and 48 hours after analgesia was begun in Group D compared with group A,B and C.There were significant differences(P0.01).The scores of nausea and vomiting,pruritus at the time of 6,12,24,48 hours in group B,C,D and at the time of 2 hours in group D were decreased compared with group A,there were significant differences(P0.01).No breath depression was recorded in group A,B,C and D.Conclusions:Naloxone combined with morphine in postoperation analgesia for elder patients can relief the nausea and vomiting,pruritus effectively.Naloxone at 0.3 gkg-1h-1 will resist the analgesia efficiency of morphine.

Abstract:
Objective:To explore the role of magnetic resonance cholangiopancreatography(MRCP) in diagnosis of biliary obstructive diseases and the imaging manifestations of benign and malignant lesions.Methods:The MRCP and clinical data of 45 patients with biliary obstructive diseases were analyzed retrospectively.Results:The diagnostic accuracy rate of MRCP was 100.0% for location and 88.9% for qualitation.The imaging manifestation of biliary stones was filling defects in the bile duct which was surrounded by bile or obstruction in the shape of a cup,cholangitis displayed a segmental dilation and stenosis of the bile duct,malignant lesions showed an abrupt occlusion or asymmetric narrowing in the bile duct.Conclusions:MRCP is of great value in diagnosis of biliary obstructive diseases and should be used as a routine examination means.

Abstract:
Objective:To assess the value of echocardiograghy in diagnosing pediatric cardiac rhabdomyoma by detecting the tumor location,size,motion,morphometric,hemodynamic features and cardiac function changes.Methods:Cases were identified by searching the pediatric cardiac surgery records.The data of echocardiograghy information and diagnostic features of 13 children were reviewed.Results:Hyperecho or homogenous echo(single in 5 or multiple in 8) in different cavity of the heart were found in all cases.Echocardiograghy showed the right ventricular(RV) masses in 7 cases,the left ventricular(LV) masses in 3 casses,both RV and LV masses in 2 cases,and 1 case involving LV,RV,left atrium.Among the cases,1 case with ventricular septal defect,3 cases with patent foramen ovale,2 cases with right ventricular out troct obstruction and 1 case with right ventriculav inflow tract obstruction were detected.There were 3 cases with cardiac arrhythmias and other 3 cases with heart failure.Conclusions:Echocardiograghy has many advantages including non-invasive,repetitive,high accuracy and its diagnosis sensitivity is higher than MRI.As thus,cardiac ultrasound is preferred for clinical diagnosing cardiac rhabdomyoma,especially for children patients,but echocardiograghy can not give the final diagnosis results,it must relay on pathological histology diagnosis.

Abstract:
Objective:To assess the availability and feasibility of the Problem-based learning(PBL) in the teaching of child and adolescent health.Methods:All students were divided into two groups.The control group was taught using traditional teaching methods,and the experimental group was taught using PBL teaching methods.The teaching effects of PBL were analysed by evaluation system and test scores.Results:10.7%,60.8% and 28.6% of students behavior were assessed as excellent,good and normal grade respectively.The evaluation results revealed that most students accepted the PBL method.80% students can accomplish the PBL case,and 42% students hoped to continue the PBL study in the future,but the test scores had not statistically significant compared with the control group(P0.05).Conclusions:PBL can be used in the teaching of child and adolescent health.The test scores were not the only criterion to judge the students ability,and multi-index parameters can be used to access the quality of teaching and comprehensive abilities of students.must be emphasized.
