The inhibiting effect of melatonin combined with cisplatin on the proliferation of Eca109 cell line

Abstract:
Objective: To explore the effects of melatonin combined with cisplatin on the proliferation and apoptosis of Eca109 cells. Methods: Eca109 cells were co-cultured with different concentrations of melatonin combined with or without cisplatin 2. 5 mg /L. The inhibiting effects on the proliferation of Eca109 Cell were examined using MTT assay,and the inhibitory rate was analyzed. The cloning cell colony formation was observed using colony clone forming experiment,and the cell apoptosis rate was detected using flow cytometry after treatment. Results: The inhibitory rates of Eca109 cells induced with different concentrations of melatonin combined with cisplatin 2. 5 mg / L were significantly higher than that in Eca109 cells induced with cisplatin or melatonin alone( P 0. 01). Melatonin combined with cisplatin could obviously enhanced the inhibitory effects on colony formation of Eca109 cells. The apoptosis rate of Eca109 cells induced with 10-5mol / L of Melatonin for 24 h was 7. 8%. The apoptosis rate of Eca109 cells induced with 10-5mol / L of Melatonin combined with 2. 5 mg / L of cisplatin for 24 h was 32. 0%,which was higher than that in cells induced with only cisplatin( 9. 7%)( P 0. 01). Conclusions: Melatonin can enhance the anti-proliferative and apoptosis-inducing effects of cisplatin on Eca109 cells, which provide the basis of clinical treatment for esophageal cancer.
The effect of repeated inhaling of 2.3% sevoflurane on learning and memory and the activity of acetylchalinesterase in young rats

Abstract:
Objective: To investigate the effects of repeated inhaling of 2. 3% sevoflurane on learning and memory in young rats. Methods: Sixty SD male rats were randomly divided into Con1,Con3,Con5,Sev1,Sev3 and Sev5 group( 10 rats each group). The group Con1,Con3 and Con5 inhaled oxygen( 0. 5 L / min) 1 hour every day for 1,3 and 5 days,respectively. The group Sev1,Sev3 and Sev5 inhaled the mixed gas of 2. 3% sevoflurane and oxygen( 0. 5 L / min) 1 hour every day for 1,3 and 5 days,respectively. Water maze test in all groups were implemented 7 days later after the last administration. The training number of rats with requiring standard,escaping latency period,the number of entering blind end and activity of acetylcholinesterase after behavioral experiment were observed. The effects of repeated inhaling of 2. 3% sevoflurane on learning and memory in young rats were observed after 7 days. Results: The differences of the training number of rats with requiring standard,escaping latency period,the number of entering blind end and activity of acetylcholinesterase in all groups were not statistical significance( P 0. 05). Conclusions: The repeated inhaling 2. 3% sevoflurane 1 hour every day for 5 days in young rats have no effects on their learning and memory founction after 7 days.
The expressions of focal adhension kinase and phosphatase and tensin homology deleted on chromosome ten in human glioma and their correlations

Abstract:
Objective: To investigate the expressions of focal adhension kinase( FAK) and phosphatase and tensin homology deleted on chromosome ten( PTEN) in human glioma and their relationships with the occurance,invasive growth and clinical pathological features of tumor. Methods: The expression levels of FAK and PTEN in 6 cases of normal brain tissue and 54 cases of human glioma samples were detected by EliVision immunohistochemical method. Results: The total positive rate of FAK expression in glioma was 66. 67% including stageⅠto Ⅱ( 35. 00%),stage Ⅲ( 81. 25%) and stage Ⅳ( 88. 89%),the positive rates of FAK expression in stage Ⅰto Ⅱwere lower than that in stage Ⅲ and Ⅳ( P 0. 01) and its expressions in grade Ⅲ and Ⅳ were not statistical significance( P 0. 05). The total positive rate of PTEN expression in glioma was 46. 30% including stageⅠto Ⅱ( 75. 00%),stage Ⅲ( 31. 25%) and stage Ⅳ( 27. 78%),the positive rates of FAK expression in stageⅠto Ⅱwere higher than that in stage Ⅲ and Ⅳ( P 0. 01) and its expressions in stage Ⅲ and Ⅳ were not statistical significance( P 0. 05). The FAK and PTEN expression in glioma was negative correlation( P 0. 01). Conclusions: The positive rates of FAK and PTEN expression increase and decrease with the increase of glioma pathological grade,respectively. These tumor marker expressions can help determine the pathological stage and prognosis of glioma,and further guide clinical treatment.
The effect of cetuximab combined with gemcitabine on the proliferation and apoptosis of triple negative breast cancer cells

Abstract:
Objective: To investigate the antitumor effects of cetuximab of the epidermal growth factor receptor monoclonal antibody combined with gemcitabine on triple negative breast cancer MDA-MB-231 cells. Methods: The growth inhibitory and apoptosis of MDAMB-231 cells treated with different concentration drugs were detected by MTT and flow cytometry PI staining,respectively. The bcl-2 expression in MDA-MB-231 cells were detected by western blot. Results: The survival rate of MDA-MB-231 cells treated with 75 g / ml of cetuximab combined with 10 mol / L gemcitabine for 48 h was 48. 53%. The apoptosis rate of MDA-MB-231 cells treated with 75 g / ml of cetuximab combined with 2 mol / L of gemcitabine( 45. 7%) was significantly higher than that in these cells treated with cetuximab alone( 16. 5%)( P 0. 01). The bcl-2 expression in MDA-MB-231 cells treated with gemcitabine was downregulated,the downregulation of which was more obvious after combination with other drugs. Conclusions: The cetuximab combined with gemcitabine in the treatment of triple negative breast cancer MDA-MB-231 cells can inhibit proliferation and promote apoptosis,a combination of two drugs show additive or synergistic effect.
The significance of the RRM1,-Tubb3 and ERCC1 expressions in the individual treatment of stage Ⅲb/Ⅳ non-small cell lung cancer

Abstract:
Objective: To investigate the relationship between the expressions of human tubulin beta-3 Chain( -Tubb3), ribonucleotide reductase 1( RRM1) and excision repair cross compementation 1( ERCC1) in stage Ⅲb / Ⅳ non-small cell lung cancer( NSCLC) and treatment effects with gemcitabine,taxanes and platinum. Methods: Four hundred and eighty patients with stage Ⅲb/Ⅳ NSCLC were randomly divided into the test group and control group( 240 cases each group). The mRNA expressions of RRM1,-Tubb3 and ERCC1 in paraffin-embedded tissue sample from the test group were detected using fluorescence quantitative-HRM method,the patients from the test group were treated with GP,NP,TP or cis-platinum combined with Pemetrexed chemotherapy regimens according to their RRM1 and -Tubb3 expressions levels,respectively. The control group were treated with a random chemotherapy regimen. The chemotherapeutic effects and overall survival of two groups were observed. Results: The objective response rates in test group and control group were 66. 1% and 31. 7%,respectively. The disease control rates in two groups were 79. 7% and 46. 3%,the difference of which was statistical significance( P 0. 01). The overall survival in two groups were 11. 2 months and 9. 8 months,the difference of which had no statistical significance( P 0. 05). The objective response rate in test group treated with GP program( 70. 7%) was significantly higher than that in the control group( 32. 6%)( P 0. 01),the objective response rate in test group treated with NP and TP program( 66. 7%) was significantly higher than that in the control group( 29. 5%)( P 0. 01). The objective response rates in test group with low and high ERCC1 expressions were78. 7% and 50. 9%,respectively,the difference of which had statistical significance( P 0. 01). The overall survival in test group with low and high ERCC1 expressions were 11. 4 months and 10. 9 months,respectively,the difference of which had no statistical significance( P 0. 05). Conclusions: The expression levels of RRM1 and -Tubb3 in patients with stage Ⅲb/Ⅳ NSCLC can guide the selecting of chemotherapy regimen,which can significantly increase the response rate.
Risk factors analysis of intra abdominal hypertension in severe acute panreatitis patients

Abstract:
Objective: To investigate the risk factors of intra abdominal hypertension( IAH) in severe acute panreatitis( SAP) patients and assess the prognosis of SAP patients with IAH. Methods: Fifty-eight SAP patients was monitored intra-abdominal pressure indirectly via urinary bladder pressure,repeated or continuous intra-abdominal pressure 12 mmHg was set as IAH. Multivariate logistic regression analysis was applied to analyze the effects of parameters including age,gender,acute physiology and chronic health evaluation Ⅱ( APACHEⅡ) scores,24 h fluid balance,hematocrit,serum calcium level on IAH. The influence of IAH on rates of operation, infection,multiple organ dysfunction syndrome,use of vasoactive agent,mechanical ventilation,acute kidney injury,positive blood culture,hospital duration,length of intensive care unit stay and mortality were also investigated. Results: The logistic regression analysis displayed,the first 24 h fluid balance,APACHE Ⅱ scores and serum albumin level were the independent risk factors for IAH in patients( P 0. 05). Moreover,patients with IAH had significantly longer average length of hospital and intensive care unit duration,higher rates of systemic and local complications and more invasive treatments compared with those without IAH( P 0. 05 to P 0. 01). Conclusions: The first 24 h fluid balance,APACHE Ⅱ score and serum albumin level were found to be independent risk factors for IAH in patients. It played the important role to relieve the possible risk factors of IAH promptly when treatment of SAP.
The effect of percutaneous biliary metal stent placement in the treatment of malignant obstructive jaundice in 46 cases

Abstract:
Objective: To evaluate the clinical effects of percutaneous transhepatic cholangial drainage( PTCD) combined with biliary metal stent placement in the treatment of elderly patients with malignant obstructive jaundice. Methods: Fourty-six patients over 70 years old with malignant obstructive jaundice were treatedwith PTCD under the guidance of ultrasound. The biliary metal stent was implanted into PTCD sinus under digital subtraction arteriography in a week after operation,external drainage converted to internal drainage by the way. Results: The serum total bilirubin in 46 cases post-PTCD decreased significantly( P 0. 01),and liver function was improved( P 0. 01). The mean survival priod was( 10. 53 2. 35) months after operation. Conclusions: PTCD combined with biliary metal stent placement for treating elderly patients with malignant obstructive jaundice is an effective way. The way can improve quality of life,and is implicity,safety,good efficacy and minimal trauma,which can be the preferred approach to treat elderly patients with malignant obstructive jaundice.
The expression and relevance of PTEN and p53 in hepatocellular carcinoma

Abstract:
Objective: To investigate the expression and clinical significance of gene of phosphate and tension homology deleted on chromsome ten( PTEN) and p53 in hepatocellular carcinoma( HCC) sample. Methods: The protein levels of PTEN and p53 were examined by EliVision immunohistochemistry in 70 samples of HCC tissue and 40 samples of adjacent hepatic tissue,while the correlation between the expression and clinicopathological features was analyzed. Results: The positive expression rate of PTEN in HCC tissue was 57. 1%,significantly lower than that in adjacent hepatic tissue,which the positive expression rate was 95. 0%( P 0. 01). Patients with advanced stage of tumor or lymph node metastasis had a lower tendency to express PTEN in HCC tissue( P 0. 01). The positive expression rate of p53 was 60. 0% in HCC tissue,significantly higher than that in adjacent hepatic tissue,which the positive expression rate was 15. 0%( P 0. 01). There were no statistical significance for the p53 expression in HCC tissue in clinicopathological parameters such as tumor size,pathologic grade,TNM stage and lymphnode metastasis( P 0. 05). The expression of PTEN was negatively related with that of p53 in HCC tissue( P 0. 05). Conclusions: The expression of PTEN may be associated with the progression of HCC. It may serve as an index for prognosis of HCC.
The effect of letrozole on patients with polycystic ovary syndrome

Abstract:
Objective: To investigate the effects of letrozole( LE) on the ovulation in patients with polycystic ovary syndrome( PCOS). Methods: The patients with good follicular growth and nomal ovulation after taking ovulation stimulator for 44 cycles were divided into letrozole group and clomifene( CC) group. The LE group with 20 cycles took 5 mg of LE per day for 5 days at the fifth day of menstrual cycle or withdrawal bleeding by oral. The CC group with 24 cycles took 100 mg of CC per day for 5 days at the fifth day of menstrual cycle or withdrawal bleeding by oral. The clinical pregnancy rate and early pregnancy abortion rate of two groups were compared. Results: Compared with the CC group,the endometrial thickness and clinical pregnancy rate of LE group with human chorionic gonadotropin period was higher,its serum estradiol level was lower( P 0. 05 to P 0. 01). There were not statistical differences in dominant follicle number,serum luteinizing hormone level,serum progesterone,three line rate of endometrium and early pregnancy abortion rate between two groups( P 0. 05). Conclusions: The effect of LE on promoting ovulation is good,the development of follicle and endometrium is synchronous. The clinical pregnancy rate is high after promoting ovulation. The LE may be a promising promoting ovulation drug.
The CD133 expression in breast cancer and its relationship with microvessel density

Abstract:
Objective: To investigate the expression of CD133,a cancer stem cell marker,in infiltrating ductal carcinoma(IDC) and its relationship with microvessel density( MVD) and clinical significance. Methods: The CD133 expressions in 105 specimens of IDC and 20 specimens of normal breast tissue and MVD count were detected by immunohistochemical ElivisionTM plus method. Results: The positive rates of CD133 expression in IDC and normal breast tissue were 5. 0% and 60. 0%,respectively,the difference of which had statistical significance( P 0. 01). The CD133 expression level differences in tumor differentiation,lymph node metastasis and clinical stage had statistical significance( P 0. 01). The MVD count differences in lymph node metastasis and clinical stage had statistical significance( P 0. 01 and P 0. 05),the difference of MVD count in the positive and negative specimens of CD133 expression had statistical significance( P 0. 01). Conclusions: The CD133 expression and MVD count are related to tumor lymph node metastasis and clinical stage. The joint detection of CD133 and MVD have important significance in predicting the progress and prognosis of IDC.
Clinical significance of combination detection of serum retinol-binding protein, pre-albumin,albumin and cholinesterase for patients with liver cirrhosis

Abstract:
Objective: To study the clinical value of detecting serum retinol-binding protein( RBP),pre-albumin( PA),albumin( ALB) and cholinesterase( CHE) for patients with liver cirrhosis. Methods: Immunotransmission turbidity,end-point method, Bromocreosol creosol green method and Butyrylthiocholine / Potassium ferricyanide method were used to detect the serum RBP,PA,ALB and CHE levels in 81 cases of liver cirrhosis of various degrees( according to the Child-pugh grading); and 35 healthy persons acted as control. Results: Compared with the control group,the levels of serum RBP,PA,ALB and CHE in the liver cirrhosis group were decreased significantly( P 0. 01). In patients with liver cirrhosis,the levels of serum RBP,PA,ALB and CHE were decreased according to the Child-pugh grading with marked differences( P 0. 01). Conclusions: Combination detection of serum RBP,PA,ALB and CHE could effectively monitor the degree of liver damage,which has great reference value in evaluating the pathogenetic condition, curative effect and prognosis of patients with liver cirrhosis.
Changes of brainstem auditory evoked potential in adult patients with migraine and its mechanism

Abstract:
Objective: To investigate the changes of brainstem auditory evoked potential( BAEP) and its mechanisms in adult patients with migraine during attack. Methods: Sixty ineurs( migraine group) and fifty normal subjects( control group) were performed BAEP and transcranial Doppler examination. Results: The right Ⅲ and bilateral Ⅴ wave latencies,the left Ⅲ- Ⅴ,right Ⅰ- Ⅲ and bilateral Ⅰ- Ⅴwave interpeak latency of BAEP in the migraine group were significantly longer than those in the control group( P 0. 05- P 0. 01). The peak velocity and mean blood flow velocity of the bilateral vertebral artery,basilar artery and posterior cerebral artery of the migraine group were significantly slower than those of the control group( P 0. 01). Conclusions: Neurophysiological changes are observed in the right areas of the brainstem of migraine patients during attack; slow vertebrobasilar system blood flow might be relevant to the mechanism of the change.
The analysis of anastomotic obstruction caused by improper use of stapler in gastrointestinal surgery

Abstract:
Objective: To explore the reasons of anastomotic obstruction caused by improper use of stapler in gastrointestinal surgery and control strategy.Methods: The clinical data of 9 cases with anastomotic obstruction caused by improper use of stapler were retrospectively analyzed. Nine cases were operated by end-to-side anastomosis,8 cases were sutured by hand after anastomotic obstruction during operation. Results: Nine patients with anastomotic obstruction were caused by the intestinal mucosa of mesentery edge nailing into the anastomotic stoma during operation. The anastomotic stoma in 1 case was sutured by hand again when the intestinal obstruction symptom was found by enteroscopy after operation. Conclusions: The details should be paid attention to during the using of stapler,which can avoid the anastomotic obstruction and reduce the suffering of patients.
The value of colposcope biopsy combined with loop electrosurgical excision procedure in the diagnosis and treatment of cervical high-grade intraepithelial neoplasia

Abstract:
Objective: To investigate the value of colposcope biopsy combined with loop electrosurgical excision procedure( LEEP) in the diagnosis and treatment of cervical high-grade intraepithelial neoplasia( CIN). Methods: One hundred and three patients with grade 2 and 3 CIN diagnosed by colposcope biopsy were treated with LEEP. The postoperative pathological findings were compared with the preoperative pathologic results. Results: The operations of 103 cases were successful. The postoperative pathology of 80 cases was still CIN 2 and 3,the coincidence rate of which was 77. 67%. The postoperative pathological levels of 21 cases( 20. 39%)( including chronic cervicitis in 16 cases and CIN 1 in 5 cases) declined. The postoperative pathological levels of 2 cases( 1. 94%)( cervical invasive carcinoma) increased. Conclusions: The colposcope cannot obtain the tissue of cervical canal lesions,which has certain limitation in the diagnosis of cervical lesions. LEEP resection can arrive at the enough scope and depth of lesion. The colposcope combined with LEEP can improve the diagnosis and treatment of grade 2 and 3 CIN.
Influence of esmolol on myocardial enzymes in children with severe hand-foot-mouth disease

Abstract:
Objective: To explore the influence of esmolol on the myocardial enzymes in children with severe hand-foot-mouth disease( HFMD). Methods: One hundred and twenty cases of severe HFMD were randomly divided into routine therapy group( 65 cases) and esmolol group( 55 cases),and 45 healthy children were selected as normal control. The children in the routine therapy group were given antiviral to reduce the intracranial pressure and creatine phosphate sodium to protect myocardial cells. The children in the esmolol group were administered esmolol in addition to the routine therapy. The lactic dehydrogenase( LDH),creatine kinase( CK),phosphate kinase isozyme( CK-MB) and norepinephrine( NE) were detected at admission and checked again three days after treatment. The changes of the myocardial enzyme and NE were compared before and after the therapy. Results: The myocardial enzyme and NE were significantly higher in children with severe HFMD than that in the normal control( P 0. 01). In the routine therapy group and the esmolol group,the LDH,CK,CK-MB and NE decreased significantly after treatment( P 0. 01); the above indicators in the two groups had no significant differences before treatment( P 0. 05),but decreased significantly in the esmolol group after treatment( P 0. 01). Conclusions: The esmolol therapy can effectively reduce the myocardial injury in children with severe HFMD.
Correlation between peripheral blood endothelial progenitor cells and prognostic outcome of patients with acute ischemic stroke

Abstract:
Objective: To investigate the influence of peripheral blood endothelial progenitor cells( EPCs) on the prognostic outcome of patients with acute ischemic stroke( IS). Methods: The peripheral blood EPCs( double-stained markers: CD133 / CD34) of 108 patients who had suffered from IS within 48 hours( stroke group),98 risk patients for cerebrovascular diseases( risk factor group) and 40 healthy volunteers( control group) were determined by flow cytometry. Meanwhile,the National Institutes of Health stroke scale( NIHSS) was used to evaluate the physical function and neurological impairment of the patients at the acute( 48 hours),convalescence( 21 days) and chronic phases of the stroke( 90 days). Results: The circulating EPCs level in the stroke group and the risk factor group was remarkably lower than that of the control( P 0. 01); patients with NIHSS12 had significantly lower level of EPCs than those with NIHSS 12( P 0. 01). High number of peripheral blood EPCs was closely associated with the improvement of NIHSS( 4) on day 21 after IS( P 0. 01) and independently associated with the good outcome on day 90 after IS( P 0. 01). Conclusions: The number of peripheral blood EPCs may act as an independently predictive maker for the prognosis of patients after IS.
The causes and treatment strategies of the complications in 121 patients implanted by permanent cardiac pacemaker

Abstract:
Objective: To explore the causes,treatment strategies and prevention measures of the complications in permanent cardiac pacemaker implantation. Methods: The data of 121 patients with permanent cardiac pacemaker implantation were retrospectively analyzed. One hundred and twenty-one patients included 53 cases of athological sinoatrial node syndrome,23 cases of Ⅲ degree atrioventricular block,16 cases of Ⅱ degree atrioventricular block,15 cases of pathological sinoatrial node syndrome complicated with atrioventricular block and 14 cases of persistent atrial fibrillation complicated with slow ventricular rate or more than 3 second long interval. Results: The complications occurred in 15 cases,including 1 case of pneumothorax,5 cases of capsule bag hemorrhage and hematoma,5 cases of capsule bag infection( 1 cases complicated with capsule bag hematoma),2 cases of electrode dislocation,1 case of wire electrode connection fail,1 case of pacemaker syndrome and 1 case of upper extremity deep vein thrombosis. The complications of 15 cases cured or alleviated after the corresponding treatment. Conclusions: Thoroughly understanding of the common complications, paying attention to preoperative prevention,standardizing operation procedure,strengthening following up and the education of pacemaker knowledge and early discovering and handling complication can reduce the complications.
Clinical analysis of ureteroscopic holmium laser lithotripsy in the treatment of ureteral calculi in 60 cases

Objective: To evaluate the clinical effects of ureteroscopic holmium laser in the treatment of ureteral calculic. Methods: Sixty cases with ureteral calculi complicated with infection,narrow ureter openings,large stone,ureteral injury during lithotripsy and ureteroscope return difficulty were treated with ureteroscopic holmium laser lithotripsy. Results: The successful rate of lithotripsy was 93. 3%( 56 cases),the stone-free rate was 96. 4%( 54 cases). Ureteral peforation in 2 cass and stone moving into kidney in 2 cases were found,and no serious complications such as ureteral mucosa avulsion and septicemia occurred. Conclusions: Ureteroscopic holmium laser in the treatment of ureteral calculic is safe,effective and less invasion,but operative technique and experience accumulation are needed.

Abstract:
Objective: To investigate the clinical significance of serum cytoplasmic thymidine kinase-1( TK1) in the diagnosis of digestive system carcinoma. Methods: The serum levels of TK1 in 144 patients with digestive system carcinoma and 60 healthy people were detected by enhanced chemi-luminescence assay. The data were statistically analyzed. Results: The TK1 positive rates in the patients with digestive system carcinoma( 59. 0%) were significantly higher than that in healthy people( 11. 7%)( P 0. 01). The TK1 positive rates in adenocarcinoma( 72. 7%) and hepatocytic carcinoma( 68. 6%) were higher than that in squamous cell carcinoma( 20. 0%)( P 0. 05). The TK1 positive rates in gastric,hepatic,pancreatic and colonic carcinoma were higher than that in esophagal squamous cell carcinoma( P 0. 05). Conclusions: The TK1 positive rate is high in digestive system carcinoma,especially in the adenocarcinoma of stomach,colon and pancreas and hepatocytic carcinoma,which surgest that the serum TK1 level is specific to the digestive gland carcinoma. The high expression of serum TK1 has certain applicative value in the screening and diagnosis of gastrointestinal adenocarcinoma and hepatocytic carcinoma.

Abstract:
Objective: To investigate the effects of valsartan and amlodipine tablets on the improvement of arterial elasticity and carotid intima-media thickness( C-IMT) in patients with hypertension. Methods: Eighty patients with hypertension were divided into the treatment group and control group( 40 cases each group). The treatment group and control group were treated with 80 mg / d of valsatan combined with 5 mg / d of amlodipine and 12. 5 mg / d of hydrochlorothiazide combined with 5 mg / d of amlodipine by daily oral for 1 year,respectively. The carotid-femoral pulse wave velocity( CF-PWV),large and small arterial elastic indexes( C1 and C2), augumentation index and C-IMT in two groups were measured before and after treantment. Results: Compared with the control group after treatment,the CF-PWV was slow down,the indexes of C1 and C2 and pressurised reflected wave were increased,and the C-IMT were decreased in treatment group( P 0. 01). The brachial artery blood pressure in 2 groups were decreased significantly after treatment( P 0. 01). The difference of which was not satistically significant( P 0. 05). Conclusions: The valsartan combined with amlodipine can significantly reduce the hypertension and C-IMT,and improve the arterial elasticity in patients with hypertension.
Application of T-tube fistula in ureteral stenting insertion in patients with advanced cervical cancer

Abstract:
Objective: To evaluate the application value of T-tube fistula in ureteral stenting insertion in patients with advanced cervical cancer under the condition of unclear ureter openings through cystoscopy. Methods: The ureter openings of 18 sides in 12 patients with advanced cervical cancer complicated with hydronephrosis could not be found by cystoscopy. The ureter openings were explored by T-tube fistula under the straight incision of back,perfusing methylene blue,wire search under X-ray and anterograde venography,ureteral stent was retrogradely implanted into ureter. Results: The ureteral stents were successfully implanted in 17 sides. T tube in 1 case with lower ureter atresia was placed for external drainage,which was replaced after 1 month. Conclusions: The effects of internal drainage in the patients with advanced cervical cancer complicated with unclear ureter openings through cystoscopy maybe good,external drainage is not preferred. T-tube fistula under the straight incision of back is less trauma and high success rate. Increasing the success rate of internal drainage can improve the life quality of the patient,which is worthy to spread.

Abstract:
Objective: To investigate the effects of continuous blood purification( CBP) in the treatment of refractory heart failure. Methods: One hundred and fourteen patients with refractory heart failure were divided into CBP group treated with CBP therapy( 73 cases) and conventional group treated with conventional heart failure drugs( 41 cases). The biochemical index,heart function and clinical outcome in two groups were compared after treatment. Results: The levels of serum sodium,potassium,creatinine,urea nitrogen and B-type brain natriuretic peptide in CBP group were significantly better than those in conventional group( P 0. 01). Compared with the conventional group,the levels of heart rate,systolic blood pressure,diastolic blood pressure,left ventricular ejection fraction,cardiac index,left ventricular end-diastolic diameter and 24 h urine in CBP group were significantly improved( P 0. 01). The improvement of cardiac functional grading in CBP group was significantly better than that in conventional group( P 0. 01). The mortality of CBP group was significantly lower than that in conventional group during treatment( P 0. 01). Conclusions: Compared with the conventional heart failure drugs treatment,the CBP treatment can significantly improve the biochemical markers and prognosis,enhance the heart function of patients with refractory heart failure.

Objective: To investigate the glucolipid metabolic features of type 2 diabetes mellitus( T2DM) patients with or without central obesity,and discuss the relationship between central obesity and large vessel complication. Methods: Sixty-six patients with T2DM were divided into obesity group and non-obesity group according to their waist circumference. The glucolipid metabolic features and large vessel complication in two groups were analyzed. Results: The insulin resistance index and the occurrence rate of large vessel complication in obesity group were higher than those in non-obesity group( P 0. 01). Conclusions: The severe insulin resistance and high occurrence rate of metabolic syndrome in T2DM patients with central obesity are found,which can lead to the high occurrence rate of large vessel complications.

Results: The gestational age in mild group was significantly higher than that in severe group( P 0. 05). The abnormal fetal heart rate,B ultrasound detection rate,uterine B-Lynch suture rate and postpartum hemorrhage rate in severe group were significantly higher than those in mild group( P 0. 05 to P 0. 01). Conclusions: The clinical manifestations of placental abruption are complex. Early detecting placental abruption and timely terminating pregnancy can improve the prognosis of mother and child.

Abstract:
Objective: To investigate the effects of bronchoalveolar lavage on the expression of Toll-like receptor 4( TLR4) in peripheral blood of patients with chronic obstructive pulmonary disease( COPD) and its clinical significance. Methods: Among 70 patients with mechanical ventilation of COPD,36 cases treated with bronchoalveolar lavage and 34 cases treated with conventional mechanical ventilation were divided into the observation group and control group,respectively. The expression of TLR4 in peripheral blood were detected by Real-time PCR,and the levels of tumor necrosis factor-( TNF-),interleukin( IL)-6 and IL-8 were measured using enzyme-linked immunosorbent assay in two groups. Results: The differences of TNF-,IL-6,IL-8 and TLR4 levels in two groups on the first day after treatment with mechanical ventilation had no statistical significance( P 0. 05). The levels of TLR4,IL-6 and IL-8 in observation group on the fifth day after treatment and the level of TNF- in observation group on the third and fifth day after treatment were significantly higher than those in control group( P 0. 01). The differences of TNF-,IL-6,IL-8 and TLR4 levels in observation group on the first,third and fifth day after treatment with mechanical ventilation combined with BAI had statistical significance( P 0. 01),the pairwise comparisons of which had statistical significance( P 0. 05 to P 0. 01). The differences of TNF-,IL-6,IL-8 and TLR4 levels in control group on the first,third and fifth day after treatment with mechanical ventilation had no statistical significance( P 0. 05). The levels of TLR4 expression and TNF-,IL-6 and IL-8 in observation group were positive correlation( P 0. 05). The differences of pulmonary infection control window time,total time of mechanical ventilation,ICU time,the rate of first offline and online again in two groups had statistical significance( P 0. 05). Conclusions: The bronchoalveolar lavage can raise the TLR4 expression in peripheral blood of patients with mechanical ventilation of COPD and increase the success rate of offline, which hints that the COPD treatment is associated with the body's natural immune.

Objective: To compare the treatment effects of the improved transvaginal hysterectomy( TVH) and conventional vaginal hysterectomy. Methods: Thirty-two patients with uterine size for 12 to 16 gestational age and uterine quality for 280 for 330 g were treated with the improved TVH( the improved group),37 patients with uterine size for 12 to 18 gestational age and uterine quality for 270 to 430 g were treated with the conventional TVH( the conventional group). The operative time,blood loss,postoperative exhaust time and side injury in two groups were compared. Results: The operative time,blood loss and postoperative exhaust time in improved group were significantly shorter than those in conventional group( P 0. 01). Conclusions: The improved TVH is very simple and do not need special equipment,which is minimal invasion,less bleeding,fast recovery and shorter hospital stays for large hysterectomy.

Abstract:
Objective: To compared the clinical efficacy of itraconazole combined with mosapride and nystatin in the treatment of fungal esophagitis. Methods: One hundred and eighty-eight patients with fungal esophagitis were randomly divided into the control group and observation group( 94 cases each group). The control group and observation group were treated with nystatin by oral and itraconazole combined with mosapride,respectively. The clinical efficacy and adverse reactions in two groups were compared. Results: The total effective rate in observation group( 100. 00%) were higher than those in control group( 93. 62%)( P 0. 05). Two cases with adverse reaction in control group and no adverse reaction in observation group were found,the difference of the occurrence rate of adverse reaction in two groups was not statically significant( P = 0. 497). Conclusions: The effects of itraconazole combined with mosapride in the treatment of fungal esophagitis are notable,which can effectively relieve clinical symptoms and eliminate mold spots,it is a ideal medication for fungal esophagitis.

Methods: The phenomenology method of the qualitative study was employed to interview 12 patients receiving maintenance hemodialysis,and the data were analyzed by means of Colaizzi content analysis. Results: The main pressure sources of the young and middle-aged patients came primarily from worries of the disease and uncertain prognosis,alteration of life patterns,lack of the knowledge and confidence for self-management,negative self-concept and financial burden. Conclusions: The young and middle-aged patients receiving maintenance hemodialysis undertake great physical,spiritual and economic pressures,so effective measures should be taken to alleviate their pressure to relieve pressure reactions of physical and mental.

Objective: To investigate the vaccination and re-inoculation of children measles vaccine in 2 streets of Bengbu. Methods: The data of vaccination of measles vaccine were retrospectively analyzed from October 2011 to May 2012. The reasons of 100 children with no-vaccinated measles vaccine were investigated by their partents telephone,and these children were re-inoculation in March 2013. Results: The re-inoculate and total vaccination rates of measles vaccine in May 2012 and March 2013 were higher than those in October 2011( P 0. 05). The mainly reasons of measles vaccine omission were children in sick or medication( 47 cases),followed by going out or migrant children( 11 cases) and others in 31 cases by telephone follow-up. Abstract:
Objective: To explore the MRI manifestations and clinicopathologic characteristics of the primary lymphoma in the soft tissue. Methods: The clinicopathological data and MRI manifestations of two cases of primary tissue lymphoma were analyzed retrospectively,and the literature was reviewed. Results: The 2 cases of primary soft tissue lymphoma were both located in the lower extremity. One was well demarcated and the other was ill-defined. The dimension of the tumors ranged from 7 to 12 cm,with intense or slightly low signal intensity on T1-weighted images and high signal intensity on T2-weighted images. The tumors were mild to moderate enhanced after contrast-enhanced scans. Conclusions: Primary lymphoma in the soft tissue is rarely seen. The MRI manifest even signals,and cystis degeneration and necrosis are seldom observed; the existence of fatty clearance between muscle fascias is one of its characteristics.

Abstract:
Objective: To investigate the value of multi-slice CT in the diagosis of pulmonary laceration. Methods: The CT data of the first examination and reexamination after treatment in 25 cases with pulmonary laceration were analyzed retrospectively. Results: Fortyseven lesions in 25 patients were found,which included 7 lesions of traumatic gas cyst,35 lesions of gas-fluid cyst and 5 lesions of fluid cyst complicated with various degree pulmonary contusion around the lung laceration in CT finding. The CT dynamic observation in 21 patients showed that the gas cyst lesions could change to gas-fluid cyst or fluid cyst,and the gas-fluid cyst lesions could change to fluid cyst,no reverse evolution lesion was found. Conclusions: Multi-slice CT examination is the best way to diagnose pulmonary laceration and dynamic observation has important value for clinical treatment.

Abstract:
Objective: To evaluate the clinical value of uterine artery chemoembolization( UACE) in the treatment of hemorrhagic caesarean scar pregnancy( CSP). Methods: Thirteen patients with hemorrhagic CSP were treated with UACE combined with transarterial perfusion with 50 to 75 mg methotrexate. After treatment,all patients were asked to rest in bed for 12- 24 h. The level of human chorionic gonadotropin was detected per 3 days,B ultrasound was examined every week. The success rate of operation,therapeutic effects and adverse reactions in all cases were recorded. Results: The UACE operations of 13 cases were success,the effect of hemostasis was good. All patients were followed up for 6 months and no re-bleeding. In addition to tabdomen severe pain in 4 cases,no severe complications were found. Conclusions: UACE for treating hemorrhagic CSP is safe and reliable,which is worth for clinical use.

Abstract:
Objective: To explore the correlation between abnormal CT perfusion( CTP) in the early period of cerebral infarction in patients with transient ischemic attack( TIA) and angiostenosis,and analyse the change of permeability surface( PS). Methods: The CTP and CT angiography( CTA) in 56 patients with TIA were implemented using 128 layers spiral CT machine when cerebral hemorrhage and acute cerebral infarction were ruled out by CT and MRI scanning. The patients with abnormal CT P were divided into stage Ⅰand Ⅱ according to perfusion stage,and mild and moderately severe angiostenosis group according to CTA. Results: Amony 56 patients with abnormal cerebral perfusion,stage Ⅰ in 39 cases and stage Ⅱ in 11 cases were found. According to CTA,mild angiostenosis in 11 cases and moderately severe angiostenosis in 26 cases were found. The difference of perfusion stage between mild and moderately severe angiostenosis group was statistically significant( P 0. 01). The value of PS in lesion side was longer than that in contralateral side in patients with stage Ⅰand Ⅱ( P 0. 01),there was not significant difference between lesions and contralateral side of brain PS in patients with stage Ⅰand Ⅱ( P 0. 05). Conclusions: Abnormal cerebral perfusion in patients with TIA is related with the degree of angiostenosis. The patient prognosis can be evaluated by CTP,CTA combined with clinical sign,which can guide the preventive treatmen.

Abstract:
Objective: To explore the value of color Doppler ultrasound in the predicting of the viability after testicular torsion. Methods: The clinical data of color Doppler ultrasound flow imaging in 16 cases with testicular torsion were retrospectively analyzed. The testicular torsion were divided into type a,b,c and d according to the color Doppler ultrasound flow imaging before operation. According to intraoperative testicular torsion,testicular viability was divided into level. A,B,C and D The testis with level A to C viability could be saved,the testis with level D viability could not be saved. The testicular viability during the period of following-up could be divided into grade Ⅰ to Ⅲ,the testis with grade Ⅰ and Ⅱ survived,and grade Ⅲ atrophied. Results: Among 16 cases with testicular torsion,the testis in 10 cases and 6 cases were restored and resected,respectively. Nine cases with restoration survived,their preoperative ultrasonography included type a in 3 cases,type b in 2 cases and type c in 4 cases,their intraoperative testicular activity included level A in 1 case,level B in 3 cases and level C in 5 cases,their forward testicular viability included grade Ⅰin 2 cases and Ⅱ grade in 7 cases. One testis with restoration atrophied,its preoperative ultrasonography,intraoperative testicular activity and forward testicular viability were type d,level D and grade Ⅲ,respectively. Amony 6 cases with testicular resection,the type c and d of preoperative ultrasonography were 1 case and 5 cases,respectively,their intraoperative testicular activity were level D. Conclusions: Color Doppler ultrasound of testicular torsion can predict the testis viability,which provide an important reference for clinical surgery program,and is the key to rescue testicular torsion and improve its viability.

Objective: To study the changes of serum thyroglobulin antibody( TGAb),thyroid peroxidase antibody( TpoAb) and thyroid hormone receptor antibody( TRAb) levels in Hashimoto's thyroiditis( HT) and Graves disease( GD),and evaluate their clinical values. Methods: Two hundred and forty cases of hyperthyroidism were selected,including 112 cases of HT in group Ⅰ,66 cases of GD in group Ⅱ,62 cases of non autoimmune hyperthyroidism in group Ⅲ,and 80 healthy persons in group Ⅳ. Serum TGAb,TpoAb and TRAb levels were detected by electrochemiluminescence immunoassay in four groups. Results: The serum TGAb,TpoAb and TRAb levels from the patients in Group Ⅰ were significantly higher than in group Ⅱ,Ⅲ and Ⅳ( P 0. 01); the serum TGAb,TpoAb and TRAb levels in group Ⅱ were also significantly higher than those in group Ⅲ and Ⅳ( P 0. 01). In group Ⅰ,the positive rate of TGAb was 90. 17%,the positive rate of TpoAb was 100. 00%,the positive rate of TRAb was 6. 25%; in group Ⅱ,the positive rate of TGAb was 30. 30%,the positive rate of TpoAb was 70. 21%,the positive rate of TRAb was 90. 90%. Conclusions: Detection of serum TGAb, TpoAb,TRAb level plays an important role in the diagnosis and differential diagnosis of HT and GD in autoimmune thyroid disease,the elevation of TRAb plays special diagnostic value for GD.

Objective: To investigate the effects of blood sample type and storage time on the assay of neuron-specific enolase( NSE) at room temperature. Methods: Blood samples of 30 healthy people were stored in tube containing gel / coagulant and heparin, respectively. The NSE levels of all samples were determined within 1 hour,the NSE levels of all samples were determined again when they were placed for 2 hours,3 hours and 4 hours at room temperature( 25. 0 0. 5) ℃. Results: Compared with the results of sample storing in gel / coagulant within 1 hour,the differences of NSE levels at different storage time had no statistical significance( P 0. 05), the differences of sample storing in heparin had statistical significance( P 0. 01). The NSE level of sample storing in heparin was significantly higher than that in sample storing in gel / coagulant for 1 hour to 4 hours( P 0. 01). Conclusions: The results of blood sample storing in gel / coagulant for 4 h were good and reliable,which can provide the scientific basis in detecting,re-checking and analysing sample.

Abstract:
Objective: To investigate the apoptosis of macrophage induced by different flora Candida albicans,and provide a new insight for the prevention and treatment of disease. Methods: The THP-1 cells were induced to differentiate into macrophage by phorbol myristate acetate. The macrophage was infected by yeast form,hyphal form of Candida albicans and Candida albicans pre-treated with prepstatin( PEP) for 1 hour,2,4 and 8 hours,the blank control group was also established. The apoptosis rate of macrophage was detected by flow cytometer. Results: Compared with the blank control group,the apoptosis rates of macrophage infected by the yeast form,hyphal form of Candida albicans and Candida albicans pre-treated by PEP for 1 hour,2,4 and 8 hours significantly increased( P 0. 05 to P 0. 01),the apoptosis rate of macrophage increased with the extension of infection time( P 0. 01). Compared with macrophage infected by Candida albicans pre-treated by PEP,the apoptosis rates of macrophage infected by the yeast form and hyphal form of Candida albicans significantly increased in different time-point( P 0. 01). The apoptosis rate of macrophage infected by hyphal form of Candida albicans was significantly higher than that in macrophage infected by yeast form( P 0. 01). Conclusions: Candida albicans can induce the apoptosis of macrophage,the effect of which is related to the form of Candida albicans and secreted aspartic proteinase.

Abstract:
Objective: To explore the effects of concentrated cleaning and disinfection of oxygen humidification bottles. Methods: Thirty oxygen humidification bottles randomly selected from the supply center were included in the experimental group and 30 oxygen humidification bottles from the clinical department acted as control. The cleaning and disinfection effects were evaluated by ocular estimate and sampling detection; and the results were compared between the 2 groups. Results: In the control group,33. 3% of the bottles were up to the standard by ocular estimate,and the qualified rate of germiculture was 43. 3%; in the experimental group,the qualified rates were 100% by both ocular estimate and in germiculture. The difference was statistically significant( P 0. 01). Conclusions: The outcome of concentrated cleaning and disinfection of oxygen humidification bottle is satisfactory,which may effectively help to prevent hospital infections.
The application effect of optimization calm on the patients with cerebral hemorrhage

Abstract:
Objective: To explore the application effects of optimization calm on the patients with cerebral hemorrhage. Methods: Thirty-nine patients with cerebral hemorrhage were randomly divided into the observation group( 20 cases) and control group( 19 cases). The control group were calmed by simple daily waking up plan. The observation group were calmed by procedural sedation combined with daily waking up. The application time of mechanical ventilation,the complication rates of mechanical ventilation,the man-machine confrontation,the incidence of the tube accidental taking off,and the change of MAP and consciousness recovery of two groups were compared. Results: There were not statistical significance in the application time of mechanical rentilation and complication rates of mechanical ventilation between two groups( P 0. 05). Compared with the control group,the patients' MAP and the consciousness recovery were good in the observation group. Conclusions: Both the optimization calm and simple daily waking up plan can prevent unconscious behavior interference treatment. The optimization calm can reduce the occurrence of postoperative complications in patients with cerebral hemorrhage,and is more conducive to the recovery of consciousness.
The influence of different analgesic methods on postoperative care of esophageal cancer

Abstract:
Objective: To compare the influence of patient-controlled epidural analgesia( PCEA) and patient-controlled intravenous analgesia( PCIA) on relieving the postoperative pain of patient with thoracic operation. Methods: Eighty-six patients with esophageal surgery were randomly divided into PCEA group( E group,43 cases) and PCIA group( I group,43 cases),and treated with PCEA and PCIA analgesia,respectively. Their effects were observed. Results: The differences of the visual analogue scale at 5 d after operation,the dosage of analgesic auxiliary drug,the hospital stays in ICU,the time of postoperative walking and hospital stays between two groups had statistical significance( P 0. 01). The occurrence of postoperative pulmonary complications,stomal leak and delirium in group E and group I were 3 cases 14 cases,2 cases 6 cases and 2 6 cases,respectively,the differences of which had no statistical significance( P 0. 05). Conclusions: The effects of PCEA on relieving the postoperative pain of patients with thoracic operation are better than that of PCIA. The PCEA can improve the postoperative care of esophageal cancer,reduce hospital stays and promote the patients rehabilitation.

Abstract:
Objective: To investigate the mental health status of emergency nurse,the causes of psychological problems and seek proper measures. Methods: Sixty ordinary nurses and 60 emergency nurses from three tertiary general hospitals of Bengbu were investigated using Symptom Check List 90( SCL-90). Results: The total scores of SCL-90 and inquietude,depression,hostility,stubborn behvior and sensitively interpersonal relationship of emergency nurses were higher than those in domestic norm and ordinary medical and surgical nurses( P 0. 01 to P 0. 05). Conclusions: Attaching great importance to the emergency nurses mental health status and psychological problem causes and adopting effective means to alleviate psychological pressure of emergency nurses can maintain and improve their mental and physical health.

Abstract:
Objective: To explore the clinical nursing effects of magnesium sulfate wet compress on postpartum perineal edema. Methods: Three hundred puerpera with postpartum perineal edema were randomly divided into the control group and treatment group( 150 cases each group). The control group and treatment group were treated with metronidazole wet compress combined with infrared irradiation and magnesium sulfate wet compress combined with infrared irradiation,respectively. The clinical effects in two groups were observed at 72 hours after treatment. Results: One hundred and thirty in observation group and 90 cases in control group were cured,the difference of which was statistically significant( P 0. 01). The recovery time in observation group( 3. 5 1. 5) days was significantly less than that in control group( 4. 5 2. 5) days( P 0. 01). The difference of infection rate in two groups was not statistically significant( P 0. 05). Conclusions: The effects of magnesium sulfate wet compress combined with infrared irradiation treatment on postpartum perineal edema are good,which can reduce pain,promote wound healing,and shorten the treatment time.
