The effects of endomorphin-1 postconditioning in rat with myocardial ischemia reperfusion injury

Abstract:
Objective: To investigate the effects of endomorphin-1(EM-1) postconditioning in rat with myocardial ischemia-reperfusion injury (IRI) in vivo and to assess its mechanism.Methods: twenty-four male Sprague Dawley rats were randomly divided into 4 groups:sham group(S group),ischemia-reperfusion group(I/R group), ischemia postconditioning group (IPO group),EM-1 postconditioning group(EM group).S group was established through threading under the left anterior descending branch of coronary artery and no occluding for 150 min;I/R group was established through occluding the left anterior descending branch of coronary artery for 30 min and reperfusion for 120 min;IPO group and EM group were respectively intravenous injection 0.9% NaCl 0.5ml and EM-1 20 μg/kg on 5 min before reperfusion,then reperfusion for 120 min.Dynamic index of blood flow was recorded and analyzed. At the end of reperfusion, arterial blood sample was obtained to measure plasma contents of malondialdehyde(MDA) and the activities of superoxide dismutase(SOD);the rats were sacrificed for assessment cell morphology of light microscopy. Results: Compared to S group,the heart rate,mean arterial pressure and rate-pressure product were decreased in I/R group(P<0.05~P<0.01);the contents of MDA was significantly increased and the activities of SOD was significantly decreased in I/R group(P<0.01);in cell morphology of light microscopy,the damage of myocardial structure was significantly increased in I/R group.The heart rate,mean arterial pressure and rate-pressure product in IPO and EM group were increased compared to I/R group except of MAP at 120 min following reperfusion in EM group(P<0.05~P<0.01),the contents of MDA in IPO and EM group was decreased and the activities of SOD was increased compared to I/R group(P<0.01),in cell morphology of light microscopy,the damage of myocardial structure was significantly reduced in IPO and EM group.Conclusions: EM-1 postconditioning can relieve ischemia reperfusion injury in myocardial protection,EM-1 postconditioning produces the protective effect by reducing the MDA and increasing the SOD.
Expression of amplified in breast cancer 1 gene in gastric carcinoma and its clinical significance

Abstract:
Objective: To investigate the expression of amplified in breast cancer 1(AIB1) in gastric carcinoma and its clinical significance.Methods: The expressions of AIB1 in gastric carcinoma tissue(n=88) and Para-cancerous tissue(n=85) were detected by immunohistochemical method.The relationship between AIB1 expression and clinicopathologic features was analyzed.Results: The positive rates of AIB1 expression in 88 gastric carcinoma samples and 85 Para-cancerous samples were 75.0% and 7.1%,respectively,the difference of which was statistically significant(P<0.01).The differences of positive AIB1 expression in lymph node metastasis and clinical stages were statistically significant(P<0.01),but the differences of AIB1 expression in sex,age,size of tumor,degree of differentiation and depth of tumor infiltration were not statistically significant(P>0.05).Conclusions: The high expression of AIB1 protein in gastric carcinoma tissue may be associated with the occurrence,development and metastasis of gastric cancer.
Cloning and sequential analysis of ESAT6 gene of Mycobacterium tuberculosis H37Ra

Abstract:
Objective: To clone and sequence the ESAT6 gene from Mycobacterium tuberculosis(MTB) H37Ra.Methods: The encoding gene of ESAT6 was amplified from MTB H37Ra genomic DNA using PCR technique.The amplified PCR product was subcloned into pTG19-T vector.The target gene was identified by PCR,single and double enzyme digestion,sequencing and sequence alignment analysis with BLAST software.Results: The ESAT6 gene was successfully cloned.The sequencing showed the fragment length was 288 bp,which was homology with the MTB H37Ra gene reported by GenBank.Conclusions: The coding gene of ESAT6 from MTB H37Ra is successfully cloned.
The Survivin expression and its significance in cervical cancer and precancerous lesions

Abstract:
Objective: To investigate the application value of Survivin expression in the diagnosis of cervical cancer and precancerous lesions by in situ hybridization.Methods: The Survivin expressions in 45 tissue samples of cervical cancer(observation group),45 tissue samples of cervical intraepithelial neoplasia(control group 1) and 45 normal cervical tissue samples(control group 2) were detected by in situ hybridization.Results: The positive rates of Survivin expression in control group 2,control 1 group and observation group were 2.2%,48.9% and 84.4%,respectively,the differences of which were statistically significant(P<0.01).The differences of positive rate of Survivin expression in the patients with stageⅠ,Ⅱ and Ⅲ cervical cancer,high,moderate and low differentiation cervical cancer and cervical cancer complicated with and without lymph node metastasis were not statistically significant(P>0.05).Conclusions: Survivin plays certain role in the occurrence and development of cervical cancer and precancerous lesions,which can be used as a sign of poor prognosis.
The diagnostic value of fibrobronchoscopy in pulmonary mycosis

Abstract:
Objective: To investigate the morphological manifestation and the diagnostic value of fibrobronchoscopy in pulmonary mycosis.Methods: The clinical data and the fibrobronchoscopy results of 21 cases with pulmonary mycosis were analyzed retrospectively.Results: Twenty-one pulmonary mycosis cases were enrolled,cough or expectoration was the main symptom,followed by hemoptysis,febricity or chest distress rare.Chest imaging results were shown:3 cases of atelectasis,3 cases of nodules,12 cases of tumor,4 cases of lamellar and patchy shadow and one of endobronchial nodule.The morphological manifestation of fibrobronchoscope were shown that 14 cases with neoplasm(including 12 neoplasms' surface covered with gray necrosis),5 with sphacelus in tracheal cavity,one with tube wall mucosal nodular eminence,and one patient was no obvious abnormality.Twenty-one cases were confirmed with the diagnosis of pulmonary mycosis by pathology through fibrobronchoscopy biopsy,including 7 cases with lung cancer.Conclusions: There were different morphological manifestations of fibrobronchoscopy in pulmonary mycosis,and no specificity.Obtaining the pathological specimens of lung by fibrobronchoscopy played an important role in the diagnosis of pulmonary mycosis.
The expression of vascuoar endothelial growth factor and its relationship with molecular subtype in breast cancer

Abstract:
Objective: To detect the expression of vascuoar endothelial growth factor(VEGF) and investigate its relationship between clinicopathological features and molecular subtypes in breast cancer.Methods: The expressions of VEGF,estrogen receptor,progesterone receptor,human epidermal growth factor receptor 2(Her-2) and nuclear associated antigen-67 in breast cancer were detected by immunohistochemical method.The Her-2 gene level was measured by fluorescence in situ hybridization.The breast cancers were divided into four molecular subtypes according to the expression levels of estrogen receptor,progesterone receptor,Her-2 and nuclear associated antigen-67,the expression of VEGF in different subtypes was measured.Results: The positive rate of VEGF expression in breast cancer tissue was significantly higher than that in benign lesion tissue and para-carcinoma tissue(P<0.01).The positive rate of VEGF expression in breast cancer tissue with lymph node metastasis was significantly higher than that in breast cancer tissue without lymph node metastasis(P<0.05).The positive rate of VEGF expression increased with the increasing of histological grade and clinical stage(P<0.05 to P<0.01).The positive rate of VEGF expression in type Basal-like was higher than that in type Luminal A(P<0.05).Conclusions: VEGF involves the process of occurrence,development and metastasis of breast cancer,which maybe an important index in guiding clinical treatment.Combining the expression of VEGF with molecular subtype of breast cancer can contribute to judge the malignant degree and prognosis of tumor,which maybe another important index in guiding molecular subtype.
The application of Narcotrend in the monitoring of general anesthesia

Abstract:
Objective: To evaluate the accuracy and practicability of Narcotrend(NT) in the monitoring of general anesthesia.Methods: Forty patients with grade Ⅰ-ⅡASA were randomly divided into the observation group and control group(20 cases each group).The drug in observation group was adjusted according to the anesthetic depth detected by NT,the drug in control group was adjusted according to the clinical experience.The levels of MAP,ECG and HR were observed,the drug use and awakening time were recorded.Results: The differences of MAP and HR on the time-point of skin incision(T4),operating(T5),end of surgery(T6) and going out the operation room(T7) between observation group and control group were statistically significant(P<0.05 to P<0.01).The differences of NT index on the time-point of T4,T6 and T7 between observation group and control group were statistically significant(P<0.05 to P<0.01),the differences of NT index between T6 & T7 time-point and T1 in control group were statistically significant(P<0.05 and P<0.01).The differences of the dosage of propofol,sufentanil and sevoflurane between two groups were statistically significant(P<0.01),for the dosage of midazolam,it was not statistically significant(P>0.05).The awakening and extubation time in observation group were shorter than those in control group(P<0.05).No awarenss case in observation group and 1 awareness case with in control group during operation were observed.Conclusions: NT can well reflect the sedation depth of general anesthesia,which can help to judge the depth of anesthesia,guide anesthesia drug use and improve the quality of anesthesia.
The levothyroxine dose in the treatment of patients with subclinical hypothyroidism during pregnancy

Abstract:
Objective: To investigate the levothyroxine(L-T4) dose in the treatment of patients with subclinical hypothyroidism(SCH) during pregnancy.Methods: The thyroid functions of 42 patients with SCH during pregnancy were detected every 4 weeks,the L-T4 dose was adjusted according to the level of thyroid stimulating hormone(TSH) to make the TSH arrive at the standard scope.The patients weight,TSH value and the level of thyroid peroxidase antibodies(TPO-Ab) at initial diagnosis,and neonatal weight and prenatal patients weight were recorded.The prenatal L-T4 dose was analyzed by multiple linear regression with SPSS17.0.Results: There were not significant relationships between the prenatal L-T4 dose and patients weight,pregnant woman increasing weight,TSH value and the level of TPO-Ab at initial diagnosis,and neonatal weight(P>0.05).Conclusions: The patient weight and TSH value,pregnant woman increasing weight,the level of TPO-Ab at initial diagnosis,and neonatal weight can not predict the prenatal L-T4 dose.Controlling the TSH value depends on the regular monitoring and following-up.
Comparsion of the risk factors of coronary heart disease complicated with depression and post stroke depression

Abstract:
Objective: To investigate the difference of the risk factors of coronary heart disease(CHD) complicated with depression and post stroke depression(PSD),and provide the reference for preventing and treating CHD and PSD etiology.Methods: The general data and clinical data of patients with CHD and stroke were collected,the risk factors of which were analyzed using univariate and multivariate Logistic regression analysis.Results: The differences of gender,age,occupation,economic status,smoking and alcohol consumption between two groups were statistically significant using univariate analysis(P<0.05).Logistic regression analysis showed that the effects of male and hypertension on PSD patients were greater than that on CHD patients with depression(P<0.05 and P<0.01),and the effects of older and occupation on CHD patients with depression were greater than that on PSD patients(P<0.05).Conclusions: The risk factors of CHD depression and PSD are different.
Clinical application of serum procalitonin and prealbumin levels in patients with acute exacerbations of chronic obstructive pulmonary disease

Abstract:
Objective: To investigate the clinical application value of serum procalcitionin(PCT) and prealbumin(PA) level in patients with acute exacerbations of chronic obstructive pulmonary disease(AECOPD).Methods: The changes of serum PCT,PA and white blood cell(WBC) levels,blood gas analysis and sputum culture of 72 patients with AECOPD before and after treatment were detected and compared.Results: There was no statistical differences of serum PCT and PA levels before treatment between sputum culture positive and negative groups(P>0.05),white blood cell count didn't change before and after treatment(P>0.05),however,there had statistical differences of serum PCT and PA levels before and after treatment(P<0.01).Conclusions: The changes of serum PCT and PA levels in AECOPD patients were more obvious,which were good diagnostic indicators.Combined with PCT and PA had more important reference value for AECOPD bacterial infection diagnosis and disease monitoring.

Abstract:
Objective: To investigate the clinical characteristics,diagnosis and treatment of ankylosing spondylitis complicated with primary hyperparathyroidism.Methods: The clinical characteristics of a patient with ankylosing spondylitis complicated with primary hyperparathyroidism were analyzed,and the relevant literatures were reviewed.Results: The severe osteoporosis,short course of disease,and kidney stone,hypercalcemia and hyperparathyroidism were the mainly clinical characteristics of patient with ankylosing spondylitis.Conclusions: Ankylosing spondylitis and hyperparathyroidism have similar or overlapping clinical characteristics,but their pathogenesis and treatment are different,which need to be differentiated.
The application of end-tidal carbon dioxide partial pressure monitoring in the mechanical ventilation of neonatal respiratory distress syndrome

Abstract:
Objective: To explore the application value of end-tidal carbon dioxide partial pressure(PETCO2) monitoring in mechanical ventilation of neonatal respiratory distress syndrome(NRDS).Methods: The PETCO2 and 122 arterial blood gas of 30 cases with NRDS treated with mechanical ventilation were monitored.The relation of PETCO2 and arterial carbon dioxide partial pressure(PaCO2) were analyzed.Results: The average value of PETCO2 and PaCO2 were(31.7±5.3) mmHg and(37.6±4.5) mmHg,respectively,which was a positive correlation(P<0.05).With the increasing of oxygenation index(OI) value,the r values of PETCO2 and PaCO2 were decreased.When the OI value was more than 25,there was no correlation between PaCO2 and PETCO2.PaCO2 and PETCO2 was positive correlation after mechanical ventilation,the difference of which were decreased.Twenty-seven cases survived,and three cases died.Conclusions: When the OI value is less than 25 or children with NRDS get better,the PETCO2 and PaCO2 has a good correlation during the period of mechanical ventilation.
The mucin 1 expression in renal clear cell carcinoma and its clinical significance

Abstract:
Objective: To investigate the mucin 1(MUC1) expression in renal clear cell carcinoma and its clinical significance.Methods: The MUC1 expressions of renal clear cell carcinoma(RCCC) tissue in 57 cases(RCCC group) and paracancerous tissue in 21 cases(paracancerous group) were detected by immunohistochemical method.The average optical density(OD) value in positive staining area was measured by Image-Pro Plus 6.0 analysis software.Results: The MUC1 expressions in cytoplasm of renal cancer cells and paracancerous tissue were found.The average OD value in RCCC group and paracancerous group were 0.429 5±0.093 9 and 0.337 1±0.082 2,respectively,the difference of which was statistical significance(P<0.01).The MUC1 expression was correlated with the clinical stage and metastasis of RCCC(P<0.01 and P<0.05).Conclusions: The high level expression of MUC1 may play important roles on the occurrence,progression,invasion and metastasis in RCCC,which may be a target antigen in the immunotherapy of renal carcinoma.
The correlation of the lesion and size of cerebral hemorrhage and electroencephalogram wave

Abstract:
Objective: To explore the wave characteristics of continuous electroencephalogram(CEEG) in different lesions and bleeding of cerebral hemorrhage.Methods: The 24 hours CEEGs wave of 60 patients with cerebral hemorrhage were recorded.Results: The abnormal CEEGs in 60 patients with cerebral hemorrhage were obvious,which showed extensive changes.The bilateral or unilateral diffuse θ and paroxysmal δ were found.The abnormal rate of CEEG in shallow layer was higher than that in deep lesions,the abnormal rate of CEEG in patient with 20 to 40 ml or more than 40 ml bleeding was higher than that in patient with less than 20 ml bleeding(P<0.05 and P<0.01).Conclusions: CEEG has diagnostic significance in the evaluation of treatment effects and prognosis of cerebral hemorrhage.
The expression of miaroRNA-143 in non-small cell lung cancer tissue

Abstract:
Objective: To explore the expression of micro RNA(miRNA)-143 in non-small cell lung cancer(NSCLC) tissue and relationship with clinicopathological features of tissue.Methods: The expression levels of miRNA-143 in NSCLC and adjacent cancerous tissue of 40 cases were detected by real-time PCR.The relationship of miRNA-143 expression with clinicopathological features of tissue was analyzed.Results: The expression of miRNA-143 in NSCLC tissue was significantly lower than that in adjacent cancerous tissue(P<0.05).The expression differences of miRNA-143 in different ages,gender,histologic types,differentiation degrees and TNM stages were not statistical significance(P>0.05).The expression level of miRNA-143 in NSCLC tissue with lymph node metastasis was lower than that in NSCLC tissue without lymph node metastasis(P<0.05).Conclusions: The expression level of miRNA-143 in NSCLC tissue decreases,which may be correlated with the cell metastasis of lung cancer.
Clinical significance of c-reactive protein in the early diagnosis of neonatal septicemia

Abstract:
Objective: To observe the early changes of C-reactive protein(CRP) in neonatal septicemia and its value in the early diagnosis of neonatal septicemia.Methods: Twenty-six cases of neonatal septicemia and 19 non-infected cases were divided into the septicemia group and control group,respectively.The level of CRP detected by immunoturbidimetry and white blood cell counts between two groups were analyzed,the sensitivity,specificity and Youden index of CRP in the early diagnosis of neonatal septicemia were observed.Results: The positive rate of CRP in septicemia group(88.46%) was significantly higher than that in control group(5.26%)(P<0.01),and the CRP level of the septicemia group was also high(P<0.01).The positive rate of CRP was significantly higher than that of white blood cell in septicemia group(P<0.01).Conclusions: CRP can provide the basis in the early diagnosis of neonatal septicemia.
The clinical analysis of vascular endothelial cell function in the patients with pre-hypertension

Abstract:
Objective: To study the changes of plasma nitric oxide(NO),calcitonin gene-related peptide(CGRP),endothelin(ET),resistin and monocyte chemoattractant protein-1(MCP-1) levels in the patients with pre-hypertension.Methods: A total 60 patients which had continuous blood pressure records were divided into hypertension group 30 cases and prehypertension group 30 cases,the other 30 healthy people were as control group according to blood pressure level.The changes of serum NO,CGRP,ET,resistin and MCP-1 levels among the three groups were evaluated.Results: The serum NO and CGRP levels in the hypertension and pre-hypertension groups were significantly reduced compared with those in normal control group(P<0.01),ET,MCP-1 and resistin levels were higher than those in normal control group(P<0.05 to P<0.01).Serum NO and CGRP levels in hypertension group were significantly lower than those in pre-hypertension group(P<0.01),while ET,MCP-1 and resistin levels were significantly higher than those in pre-hypertension group(P<0.01).Conclusions: The endothelial cell function is significantly damaged in high blood pressure patients,representing with metabolic imbalances of serum NO,CGRP,ET,resistin and MCP-1;endothelial cell dysfunction is also emerged in the patients with pre-hypertension.
The experience of perioperative treatment of thoracic operation in aged patients

Abstract:
Objective: To explore the perioperative clinical treatment of thoracic operation in aged patients.Methods: The clinical data of 28 patients treated with thoracic operation were retrospectively analyzed.Results: Postoperative complications were found in 15 cases(53.57%),which included low oxygen saturation in 3 cases,atelectasis and pulmonary infection in 7 cases,arrhythmia in 4 cases and anastomotic fistula in 1 case.The preoperative patient age,lung and cardiovascular disease,diabetes,smoking and cerebral infarction did not have significant effects on the postoperative complications(P>0.05).Conclusions: The incidence of postoperative complications in aged patients is high,but old age is not the contraindication of chest surgery.Active treating preoperative diseases,intraoperative safe anesthesia,reducing the damage to the heart and lung as far as possible and strengthening the postoperative respiratory tract management can prevent the occurrence of complications.
Effect comparison of two kinds of adhesive system in repairing class Ⅳ cavity

Abstract:
Objective: To compare the effects of two kinds of adhesive system in repairing class IV cavity.Methods: One hundred and sixty-two teeth with Class IV cavity were randomly divided into A group and B group(81 cases each group).Group A and B were repaired by two-step and one-step adhesive system combined with composite resin,respectively.Results: There was not significant difference in the repair success rate between two groups in 6 months and 1 year after treatment(P>0.05).The success rate of A group was higher than that in B group in 2 years after treatment(P<0.05).Conclusions: The long-term(2 years) effect of two-step adhesive system in repairing class IV cavity is good.
The prognostic factors analysis in patients with progressive ischemic stroke

Abstract:
Objective: To identify the predictors correlated with the prognosis 90 days after acute progressive ischemic stroke.Methods: One hundred and fifty-five acute progressive ischemic stroke patients within 24 hours after onset were enrolled into the study during Feb.2011 to Dec.2012.The demographic data,past history,laboratory examination and imaging studies were recorded.The neurologic deficit scores at admission to hospital,symptomatic progression period and 90 days'period were assessed using U.S National Institutes of Health stroke scale(NIHSS),Glasgow coma scale(GCS) and modified Rankin scale(mRS).Results: There were statistical significant differences about C reactive protein within 24 hours,blood glucose level,fibrinogen,erythrocyte sedimentation rate,baseline GCS and NIHSS scores and hospitalization infection in patients at 90 days mRS clinical outcome(P<0.05 to P<0.01).Conclusions: The factors including C reactive protein,blood glucose level,fibrinogen,erythrocyte sedimentation rate,GCS and NIHSS scores,infection were correlated with 90 days mRS clinical outcome.In clinical treatment,control these factors can improve the outcome.
The treatment experience of tetralogy of Fallot radical operation in 77 infants

Abstract:
Objective: To summarize the experience of tetralogy of Fallot radical operation in infants,improve surgical technique and reduce mortality.Methods: The clinical data of 77 cases treated with tetralogy of Fallot radical operation were retrospectively analyzed.Pure right ventricular outflow tract patch in 15 cases and pulmonary valve ring patch in 62 cases were implentmented through median incision under the condition of middle and low temperature/profound hypothermia low flow.Results: One case died after the operation,which was mainly caused by low cardiac output syndrome,other cases with ventilator-associated pneumonia,arrhythmia,cardiac tamponade and pleural effusions were cured.None died during the period of following-up for 3 to 24 months.Conclusions: The effects of radical operation in the treatment of tetralogy of Fallot in infants are good.Early correcting cardiac malformation and strengthening postoperative management can effectively reduce the mortality rate of infants.

Objective: To evaluate effect and safety of edaravone on patients with brain stem hemorrhage.Methods: Forty patients with brain stem hemorrhage were devided into edaravone group(n=20) and control group(n=20).Cases of edaravone group were infused at a dose 30 mg twice a day for 14 days.The measures of osmotic diuretics,antihypertensives,antidiabetics and prevention complication were executed in the control group.The European stroke scale(ESS) and activities of daily living(ADL) were evaluated before and after treatment for judging effect and side effect.Results: There were significant differences between edaravone group and control group in ESS and ADL(P<0.01).The efficiency in treatment group was significantly higher than that in control group(P<0.01)and without complications.Conclusions: Edarvone is effective and safe drug in treating patients with acute brain stem hamorrage.
Characteristics and correlation between ingested and inhaled allergens during infancy

Abstract:
Objective: To determine the characteristics of ingested allergens and inhaled allergens in infancy and the relationship between them.Methods: 370 infants with allergic diseases were included in this study.Serum-specific IgE to milk,egg and pyroglyphidae were measured in all the patients with UniCAP System;the allergic rate of the three kinds of allergens in different ages,the cross allergy phenomenon between ingested allergens and inhaled allergens were all analyzed.Results: The prevalence of milk allergy and egg allergy went down with the age increasing,and the prevalence decreased obviously after the age of 3;The prevalence of pyroglyphidae allergy went down with the age increasing,and the prevalence increased obviously after the age of 2.There was no cross allergic phenomenon between the milk and pyroglyphidae except the >2-3 age group.Conclusions: Allergic phenomena in infancy and early childhood are mainly caused by ingested allergens,and will gradually develop tolerance with the age increasing and turn to inhalant allergens.So when ingested allergy symptoms are relieved,attention still should be paid to inhaled allergen-induced allergic phenomena.
The effect of preemptive analgesia of parecoxib sodium on preventing agitation in recovery period of general anaesthesia

Abstract:
Objective: To observe the effects of preemptive analgesia of parecoxib sodium on preventing agitation in recovery period of general anaesthesia.Methods: Sixty patients with ASA Ⅰ-Ⅱ scheduled with operation were randomly divided into the parecoxib sodium group(P group) and control group(C group)(30 cases each group).Ten minutes before general anesthesinduction,the group P and group C were injected with 40 mg of parecoxib sodium(4 ml) and 4 ml of normal saline,respectively.The time of spontaneous breathing recovery and extubation,the incidence and degree of agitation in anesthesia recovery period,the pain score at 10 minutes after extubation of all patients were observed.The incidence of adverse reactions,such as nausea and vomiting in recovery period,drowsiness and respiratory depression,of all patients were recorded.Results: There were not statistically significant in the time of spontaneous breathing recovery and extubation between two groups(P>0.05).Compared with group C,the incidence and degree of agitation and pain score at 10 minutes after extubation in group P were significantly lower(P<0.01).One case with nausea in group C in recovery period was found,no vomiting,drowsiness and respiratory depression were found in two groups.Conclusions: The injection with 40 mg of parecoxib sodium 10 minutes before general anesthesia induction can play analgesic effect,reduce and prevent the agitation in anesthesia recovery period and significantly improve the safety of the recovery period,but it doesn't prolong the time of spontaneous breathing recovery and extubation.

Objective: To explore the preparation of Gefitnib(PLGA) delayed-release microspheres based on Gefitnib for medol drug and PLGA for carrier.Methods: The microspheres were prepared using the O/W emulsification-solvents diffusion technique,the preparation technology was optimized by orthogonal design based on single factor experiment.The appearance of Gefitnib microsphere was observed by optical microscope and scanning electron microscope,the formation of PLGA microsphere was indentified by DSC,the release of Gefitnib PLGA microsphere was observed in vitro.Results: The results of DSC showed that the intermolecular force of Gefitnib and PLGA changed,Gefitnib evenly dispersed in carrier in the form of molecular.Gefitnib PLGA microsphere was white and smooth sphere,the mean particle size,encapsulate efficiency and drug loading of which were(10.35±0.32)μm,(88.44±1.26)% and(10.00±0.23)%,respectively.The release behavior of Gefitnib PLGA microsphere in vitro followed the zero order equation,Q=0.769t-1.800 9,r2=0.980 8.Conclusions: The preparation technique of Gefitnib PLGA microsphere is stable,the release of drug in vitro of which is more than 5 d,and it has obvious sustained release effect.

Abstract:
Objective: To investigate the awareness of the risk factors for chronic diseases and the current behavior of the residents in Jinjiazhuang community,Maanshan city so as to provide scientific evidence for the prevention of chronic diseases.Methods: Four thousand six hundred and seventy six residents aged over 18 were enrolled by stratified sampling.Each resident was scheduled to complete a questionnaire regarding to knowledge about the risk factors for chronic diseases and their current behavior.Results: The residents' overall awareness of the risk factors for hypertension was 38.37%,and the residents with hypertension had a higher awareness than the non-hypertensive population(P<0.05).The residents' awareness of the risk factors for diabetes was 33.00%,and the residents with diabetes were more conscious of the risk factors than the non-diabetic population(P<0.05).The residents' awareness of the risk factors for stroke was 38.54%;the stroke patients and the non-stroke population had no statistically significant difference in knowledge about the risk factors(P>0.05).Residents with hypertension,diabetes and stroke had a high smoking rate,which was 72.34%,73.21% and 73.24%,respectively.The drinking rate and active- exercise rate in patients with hypertension were 12.40% and 20.57%,respectively.The rates of obesity and health education in patients with hypertension and diabetes were 43.19% and 41.52%,respectively.Conclusions: Behavioral risk factors such as smoking,over-drinking,obesity and lack of active exercises are widely existent in residents with chronic disease in Jinjiazhuang District.The residents' knowledge about the risk factors for chronic disease is insufficient,so it is necessary to provide health education about prevention and treatment of the chronic diseases to them to improve their awareness and health behavior.

Objective: To analyze the influence factors of patient satisfaction for providing the suggestions regarding the improvement of medical service.Methods: The patient satisfactions of medical service were investigated by questionnaire.Results: The differences of the general satisfaction and overall satisfaction of patient,and correlations of the first visit to hospital and hospital stays with overall satisfaction were statistical significance(P<0.05).The difference of the correlation of eight dimensions of medical service satisfaction with overall satisfaction was statistical significance(P<0.01).Conclusions: The overall evaluation of patient on hospital is high.The evaluations of the medical treatment cost and procedure,and diet and lifestyle of patient on hospital are low.The service quality and medical treatment technique should be improved to increase the patient satisfaction.
Analysis of MSCT imaging of the solid pseudopapillary tumor of pancreas in 9 cases

Abstract:
Objective: To analyze the CT imaging of the solid pseudopapillary tumor of pancreas(SPTP) so as to improve its diagnosis and differential diagnosis.Methods: The CT and MSCT imaging characteristics of 9 cases with SPTP diagnosed by pathology were retrospectively analyzed.Results: Among the 9 cases with SPTP,5 cases in pancreatic head(55.6%),2 cases in pancreatic neck(22.2%),1 case in pancreatic body(11.1%) and 1 case in pancreatic tail(11.1%) were found.The CT imaging showed that the tumor was round or oval low density solid or solid-cystic mass with clear boundary,calcification in 2 cases,strip calcification under amicula in 1 case and punctate calcification in tumor centre in 1 case were found.The tumors in all cases were wrapped by fiber amicula.The delayed reinforcement with persistently slight enhanced features(floating cloud symptom) in solid part of tumor was showed by enhanced CT scan,no obvious enhancement in the cystic part of tumor and obvious enhancement in fiber amicula were found.The lymph nodes enlarging or metastasis in all cases were not be found.Conclusions: The CT imaging of SPTP has certain characteristics,and multi-phase enhanced scan can improve its diagnosis and differential diagnosis.

Abstract:
Objective: To analyze the MRI features of brain pleomorphic xanthoastrocytomas(PXA) in 8 cases for improving its clinical,pathological and MRI features understanding.Methods: The MRI plain scans,enhanced scans and DWI were explored in 8 cases with brain PXA diagnosed by pathology,and the MRS combined with PWI were examined in 2 cases.Results: Three patients were male and 5 patients were female,the age of all patients ranged from 14 to 56 years old(including 6 patients less than 25 years old).Interstitial change in 2 cases were found.Single lesion in 7 cases and single lesion complicated with round multiple lesions in 1 case with interstitial change were found.Five cases located or involved in temporal lobe,6 cases located in the cortex and subcortical region.Solid-cystic tumors in 5 cases and solid tumors in 3 cases(including 1 case with partial pia mater invasion,1 case with bleeding and ependymal invasion and 1 case with lateral ventricle spread) were identified by MRI.The tumor solid area showed slightly hypointense on T1-weighted images,slightly hyperintense on T2-weighted images and marked enhancement.The difference between the apparent diffusion coefficient value and subcortex alba was not statistical significance(P>0.05).The solid-cystic capsule wall enhanced in 2 cases.The light to moderate degree edema around the tumor were detected in all tumors.The significant increasing Cho peak and decreasing N-acetyl aspartic acid peak and Lip peak were found in tumor solid area with high perfusion.Conclusions: PXA occurres usually in adolescent,which locates the brain shallow position of the temporal lobe.The tumor pathologic and genetic characteristics are different from other astrocytomas,complete surgical excision is important for a good prognosis.The conventional magnetic resonance imaging,magnetic resonance perfusion weighted imaging and magnetic resonance spectroscopy of PXA have certain characteristics.

Abstract:
Objective: To explore the value of the intraperitoneal free gas distribution of multislice CT(MSCT) in the positioning diagnosis of upper and lower gastrointestinal perforation.Methods: The distribution characteristics of intraperitoneal free gas in MSCT of 54 patients with gastrointestinal perforation were retrospectively analyzed,which was compared with surgical results.Results: The distribution proportions of MSCT intraperitoneal free gas in hepatic portal,falciform ligament,ligamentum teres hepatis and stomach and duodenum of the patients with upper gastrointestinal perforation were significantly higher than those of the patients with lower gastrointestinal perforation(P<0.05 to P<0.01).The distribution proportions of MSCT intraperitoneal free gas in jejunum and ileum,mesentery,appendix and pelvic cavity of the patients with upper gastrointestinal perforation were significantly lower than those of the patients with lower gastrointestinal perforation(P<0.05 to P<0.01).Conclusions: The distribution of the free gas in MSCT is conducive to identify the upper and lower gastrointestinal perforation,and can improve the diagnostic accuracy before operation.
Study of left cardiac structure and function on uremia patients treated with arteriovenous fistula

Abstract:
Objective: To assess the left cardiac structure and function after 6 months on uremia patients undergoing arteriovenous fistula(AVF).Methods: The indexes of left cardiac structure and systolic function were examined and analyzed by echocardiography from 40 uremia patients before and six months after AVF.The cardiac structural indexes were including left ventricular diameter of diastole(LVDd),left atrium diameter(LAD),interventricular septum(IVS),left ventricular posterior wall(LVPW),left ventricular mass index(LVMI) and relative wall thickness(RWT);the systolic functional indexes were including ejection fraction(EF),fraction shorten(FS),strove volume(SV),cardiac output(CO) and cardiac index(CI).Results: Compared with echocardiographic indexes before AVF,after treatment for 6 months,IVS,LVPW,LVMI and RWT were decreased(P<0.05);EF,SV,CO and CI were increased(P<0.01).There were no significant differences on LVDd,LAD and FS(P>0.05).Conclusions: Part of uremia patients with left ventricular hypertrophy can be improved in the early period of AVF.Echocardiography is an important method for detection the left cardiac structure and function.

Abstract:
Objective: To investigate the health literacy of college students for providing the basis of implementing health education and intervention measures.Methods: One thousand two hundred and forty-three college students from two schools were screened by stratified cluster random sampling method,and were investigated using health literacy questionnaire.Results: The proportion of college student with health literacy rate was 4.2%,the proportions of college student with health knowledge and concept,healthy lifestyle and behavior,and health skills were 28.8%,14.8% and 79.0%,respectively.The scores of health literacy,health knowledge,concept,healthy lifestyle and behavior in schoolgirls were significantly higher than those in schoolboys(P<0.01),the score of health literacy in medical students was higher than that in non-medical students(P<0.01),the score of health literacy in the first year college students was lower than that in the second college students(P<0.05).Conclusions: The level of health literacy of college student is low,especially in the healthy lifestyle and behavior.The correct guidance of lifestyle should be strengthened to promote the formation of health behavior,and improve the level of mental health.

Abstract:
Objective: To explore the relationship between serum levels of CA15-3,CA125,CEA,PRL and breast cancer with bone metastasis.Methods: The levels of the serum CA15-3,CA125,CEA and PRL in 60 healthy people,67 patients with breast benign hyperplasia and 95 patients with breast cancer were measured with ECLIA,whose were compared in breast cancer patients before and after surgery and before and after bone metastasis.Results: The levels of serum CA15-3,CA125,CEA and PRL in patients with breast cancer were significantly higher than those in patients with breast benign hyperplasia and healthy people(P<0.01).The levels of serum CA15-3,CA125,CEA and PRL in patients with breast cancer significantly decreased after operation(P<0.01).The levels of serum CA15-3,CA125,CEA and PRL in patients with bond metastasis were significantly higher than those in patients without bond metastasis.Conclusions: Detecting the levels of serum CA15-3,CA125,CEA and PRL may have important value in diagnosing and judging prognosis of breast cancer(P<0.01).

Abstract:
Objective: To investigate the clinical value of multi-tumor markers protein biochip detective system(C-12 system).Methods: The serum of 280 malignant tumor patients and 106 healthy people were detected by C-12 system.The sensitivity and specificity of the system in all cases were analyzed.Results: Two hundred and fourty-two postive cases in 280 malignant tumor patients and 7 postive cases in 106 healthy people were detected by C-12 system.The sensitivity of C-12 system in 280 malignant tumor patients and 106 healthy people were 86.43% and 93.40%,respectively.Conclusions: C-12 system is available for the clinical laboratory diagnosis of tumor,especially for health examination and tumor screening.Detecting multiple indicators at the same time can enhance the detection rate and accuracy,which has certain supplementary meaning in judging the tumor source.

Abstract:
Objective: To investigate the continuing medical education status of a Class Ⅱ Grade A hospital,and analyze its influencing factors and countermeasures.Methods: The basic information,motivation and obstacle factors of attending the continuing medical education of 509 medical staff were investigated and analyzed using a self-designed questionnaire.Results: The main influencing factors included professional development motivation(89.2%),unreasonable time arrangement(84.0%),long distance of training places(62.9%),social factors(51.1%) and individual interest(50.1%).Academic lectures(74.3%) and training courses(68.3%) were the main forms of continuing medical education,and the professional new theory and technology were the main study content(77.0%).The difference of the continuing medical education form in different titles and staffing levels medical staff were statistically significant(P<0.05 to P<0.01).The differences of the motivation and obstacle factors of attending the continuing medical education in different titles and posts medical staff were statistically significant(P<0.05 to P<0.01).Conclusions: The according continuing medical education plan is formulated according to the title and post of medical staff,which can improve the popularity and accessibility of continuing education and overall quality of medical staff.
Effects of high-flux dialysis on resistant hypertension in patients with uremia

Abstract:
Objective: To investigate the influence of high-flux dialysis(HFD) on resistant hypertension in patients with uremia.Methods: Thirty cases with resistant hypertension in uremia patients were randomly assigned to observation group and control group.Low flux dialyzers were used in control group and high flux dialyzers were used in observation group.The dialysis time was 12 hours every week and the observation duration was 12 weeks.The changes of systolic blood pressure,diastolic blood pressure and mean arterial pressure in the two groups before and after treatment were observed.Results: The decrease degrees of systolic blood pressure,diastolic blood pressure and mean arterial pressure in observation group were superior than in control group after 12 weeks,which had statistical significances(P<0.01).Conclusions: HFD mode can effectively improve resistant hypertension in uremia patients.

Abstract:
Objective: To investigate the application of psycho-cardiology medical mode in patients with coronary artery disease(CAD) and hypertension complicated with psychological disorder.Methods: One hundred and twenty patients with CAD and hypertension complicated with depression and anxiety were randomly divided into the observation group and control group.The observation group and control group were treated with the intervention of psycho-cardiology medical mode based on routine medication and routine medication,respectively.The clinical symptoms,electrocardiogram,blood pressure,scores of Zung self-rating depression/anxiety scale(SDS/SAS),B-type natriuretic peptide(BNP),left ventricular ejection fraction(LVEF) and levels of plasma cortisol,IL-10 and TNFα,and ratio of TNFα to IL-10 in two groups were observed before therapy and after 4 weeks of therapy.Results: The clinical symptoms,electrocardiogram,blood pressure,scores of SDS and SAS,BNP JNFα and LVEF in two groups were improved after therapy(P<0.05 to P<0.01).The levels of IL-10 in two groups increased after therapy,the increasing degree of which in observation group were more than which in control group(P<0.05).The levels of cortisol in two groups decreased after therapy,the decreasing degrees of whose in observation group were more than whose in control group(P<0.05).Conclusions: The treatment of patients with CAD and hypertension complicated with psychological disorder with psycho-cardiology medical mode can improve the clinical effects,the related mechanisms of which are associated with the role of plasma cortisol and imbalance between inflammation and anti-inflammation.
