The morphology and stereotaxy of bifurcation of the cerebral collateral sulcus

Abstract:
Objective:To explore the morphology of cerebral collateral sulcus(COS) on the MRI cross section and coronal plane image of brain.Methods:In the eFilm2.1 workstation, the continuously cross section and coronal plane MRI scanning data of 40 health adult volunteer brain were selected.The morphological characteristics of COS on the continuously cross section and coronal plane MRI were identified, observed and analyzed using the structure continuous tracking method and 3D-Cursor technology.Results:The collateral sulcus could be divided into the sulcus olfactorius nasi, middle collateral sulcus and occipital collateral sulcus.With the stratification plane rearward shift, the collateral sulcus on the coronal plane image moved upper.The 72.5% of occipital collateral sulcus was divided into the medial and lateral branches.The mapping length of collateral sulcus on the coronal plane and cross section plane equaled to the number of plies of cross section times 2 mm and number of plies of coronal plane times 2 mm, respectively.Conclusions:The morphological pattern of COS in the MRI cross and coronal-sectional images can provide the anatomical information for identifying and locating the collateral sulcus and its adjacent brain lesion.
Influence of acupuncture pretreatment on ischemia-induced functional impairment of cerebellar Purkenje cells in rats

Abstract:
Objective:To investigate the influence of acupuncture pretreatment Baihui-point and bilateral Zusanli-point on ischemia-induced impairment of cerebellar Purkenje cells.Methods:Thirty male SD rats were randomly divided into three groups:control group, acupuncture pretreatment in Baihui-point and bilateral Zusanli-point group and pretreatment in non acupuncture point group, 10 rats in each group.The oxygen-glucose deprivation model was established, the survival rate of Purkinje cells, malondiadehyde(MDA) content and superoxide dismutase(SOD) activity were detected during the ischemia period after the rats were acupunctured.Results:Compared with the control, the survival rate of Purkinje cells was lower, MDA content was increased and SOD activity was decreased significantly in non-acupuncture pretreatment group(P<0.01).Compared with the non-acupuncture pretreatment group, the survival rate of Purkinje cells was increased, MDA content was decreased, and SOD activity was increased in the acupuncture pretreatment in Baihui-point and bilateral Zusanli-point group(P<0.01).Conclusions:Acupuncture pretreatment to Baihui-point and bilateral Zusanli-point prevented ischemia-induced functional impairment of cerebellar Purkenje cells, the protective mechanism may be related to reduce the production of reactive oxygen species and promote the clearance in cerebellar Purkenje cells.
Study of the relationship between the antinociception of ketamine mediated by nitric oxide pathway and inhibiting substance P receptors in the spinal cord

Abstract:
Objective:To investigate the relationship between the antinociception of ketamine(Ket) mediated by nitric oxide(NO) pathway and inhibiting substance P(SP) receptor in the spinal cord, and its possible mechanism.Methods:The SP of intrathecal injection and/or Ket of intraperitoneal injection were recorded using behavioral experiment, immunohistochemical staining and spectrophotometry.The incubation period of licking foot reaction in mice and licking time in phase Ⅰ and phase Ⅱ mice were detected using hot-plate test and using formaldehyde experiment, respectively.The changes of the number of Fos-like immunoreactive(FLI) positive cells, nitric oxide synthase(NOS) activity and NO content were measured.Results:In hot-plate test, the 20 mg/kg and 30 mg/kg of Ket of intraperitoneal injection could increase the pain threshold in the hot-plate test(HPPT) of mice(P<0.05 to P<0.01), the 0.5 ng of SP of intrathecal injection after 5 min and 20 min could decrease the HPPT of ketamine-treated mice(P<0.01 and P<0.05).In formalin test, the 30 mg/kg of Ket of intraperitoneal injection could decrease the licking time of mice(P<0.01), the 0.5 ng of SP of intrathecal injection could increase the licking time of mice in two stages(P<0.05 to P<0.01).In control group, the few FLI positive cells were symmetrical distribution on both sides of spinal dorsal horn, the FLI positive cells on both sides of spinal dorsal horn injected by formalin were significantly higher than that in control group(P<0.01).The FLI positive cells on both sides of spinal dorsal horn injected by intraperitoneal Ket significantly decreased(P<0.01), the 0.5 ng of SP of intrathecal injection could significantly inhibit FLI positive cells on both sides of spinal dorsal horn injected by formalin(P<0.01).Ket could inhibit the copy paste in spinal cord(P<0.05), and the SP of intrathecal injection can counter copy paste caused by Ket(P<0.05).Conclusions:The antinociception of Ket is correlation with the inhibiting of SP receptor, which may be mediated by NO pathway.
The protective effect of curcumin on the ischemia-reperfusion injury in rats

Abstract:
Objective:To investigate the effects and its mechanism of curcumin Cur on ischemia-reperfusion(I/R) injury in rats.Methods:Sixty rats were randomly divided into the sham operation group(group A), I/R group(group B), curcumin+I/R group(group C) and Cur+I/R+Zn-protoporphyrin group(group D)(15 rats each group).group C and D were intraperitoneally injected with 100 mg/kg of curcumin before 5 days of operation, and group C were intraperitoneally injected with 20μmol/kg of Zn-protoporphyrin before 60 mins of operation.Group A and B were injected with equivalent 0.9% of sodium chloride.Except for group A, the bilateral renal blood vessels in other 3 groups were blocked, and poured into after 60 mins.The levels of serum creatinine(SCr), blood urea nitrogen(BUN), tumor necrosis factor-α(TNF-α), heme oxygenase-1(HO-1), intercellular adhesion molecule-1(ICAM-1) in 4 groups were measured after 4 h, 12 h and 24 h of reperfusion.The kidney tissue was excised, and the levels of myeloperoxidase(MPO) activity in excised kidney tissue was measured.Results:The expression levels of SCr, BUN, TNF-α, ICAM-1 and MPO in group C and group D were higher and lower than those in group A and group B, respectively(P<0.05 to P<0.01).The expression levels of HO-1 in group C and group D were lower and higher than that in group A and group B, respectively(P<0.05 to P<0.01).The expression levels of TNF-αand ICAM-1 at each time-points in group D were higher than that in group C(P<0.05 to P<0.01).The expression levels of SCr and BUN in serum, and MPO in kidney tissue in group D at 12 h and 24 h were higher than that in group C(P<0.05 to P<0.01), the differences of whose at 4 h were not statistically significant(P>0.05), the level of HO-1 at each time-points in group D was lower than that in group C(P<0.05 to P<0.01).Conclusions:Curcumin can protect the kidneys against I/R injury, and effectively relieve the inflammatory reaction and oxidative stress of kidney tissue.HO-1 may be involved in the renal protective mechanism of curcumin.
The expressions of Fas and FasL in rabbit retina with ischemia-reperfusion injury, and the effect of recombinant human basic fibroblast growth factor on its expression

Abstract:
Objective:To investigate the protective effects of recombinant human basic fibroblast growth factor(rh-bFGF) on retinal ischemia-reperfusion injury(RIRI) in rabbit.Methods:The models of RIRI in rabbit were established by elevating intraocular pressure.Sixty-six healthy rabbits with normal external ocular and fundus were randomly divided into the sham-operation group(6 rabbits), retinal ischemia-reperfusion injury group(RIRI group, 30 rabbits) and rh-bFGF treatment group(RIRI+rh-bFGF group, 30 rabbits).The right eye was selected as the experimental eye.RIRI group and RIRI+rh-bFGF group were treated with 0.9% sodium chloride injection and rh-bFGF, respectively.Morphological changes of retinal cells and the expressions of Fas and FasL of retinal tissue in sham-operation group, and at 6 h, 12 h, 24 h, 48 h and 72 h time-points of RIRI and RIRI+rh-bFGF groups were detected by HE staining and immunohistochemical method, respectively.Results:No apoptotic cells was found in sham-operation group.Apoptotic cells in RIRI group located mainly in the inner nuclear layer and the ganglion cell layer, and which arrived at peak at 24 h after reperfusion.No obvious expression of Fas in retinal tissue of the sham-operation group was found.After RIRI, little Fas expressing at 6 h, arriving at peak at 24 h, then begaining to decrease at 48 h and significant weakening at 72 h were found.No obvious expression of FasL in retinal tissue of the sham-operation group was found.After RIRI, FasL expression beginning to increase at 12 h, arriving at peak at 24 h, then beginning to decrease and still maintaining a high level at 72 h were found.Compared with the RIRI group, the positive expression trend of apoptotic factors at each time-point in RIRI+rh-bFGF group was similar, the expressions of Fas and FasL in RIRI group and RIRI+rh-bFGF group obviously decreased(P<0.05).The differences of the expression of Fas and FasL at 6 h, 12 h, 24 h, 48 h and 72 h between RIRI and RIRI+rh-bFGF group were statistical significance(P<0.01).Conclusions:The protective mechanism of rh-bFGF on RIRI in rabbit may be related to inhibit the expressions of Fas and FasL in retina.

Abstract:
Objective:To investigate the career cognition of medical student under the influence of the doctor-patient relationship, and its influence factors.Methods:One thousand students from freshman to grade five of clinical medicine of Bengbu medical college were investigated using self-made questionnaire.Results:Under the influence of the doctor-patient relationship, the level of the career cognition of clinical medicine student was low, the confidence level of employment in male student was significantly higher than in female student(P<0.01).The levels of communication ability, employment confidence, career moral and professional confidence in junior medical students were higher than those in the senior medical students(P<0.01).The employment confidence level in single child medical student was higher than that in non-single child medical students(P<0.05).The levels of the professional knowledge, communication ability, employment confidence and professional quality in class leader were higher than those in non-class leader(P<0.05 to P<0.01).Conclusions:The adverse doctor-patient relationship has negative effects on the professional cognition of medical student through a variety of ways, which can be settled by the joint of efforts of government, hospital, school, society and medical students.

Abstract:
Objective:To investigate the protective effects of remote ischemic preconditioning(RIPC) on the brain injury of patients treated with cardiac valve replacement under cardiopulmonary bypass(CPB).Methods:Forty patients treated with cardiac valve replacement under CPB were randomly divided into the RIPC group and control group(CON group)(20 cases each group).After induction of anesthesia and before cardiopulmonary bypass, the pressure of left upper arm in RIPC group was increased by cerclage with orthopedic inflatable pressure tourniquet, loop 3 times, 5 min each time, and an interval of 5 min.The treatment of the CON group was the same as the RIPC group, but the tourniquet was not inflated.The levels of S-100β protein and NSE in internal carotid artery bulb were detected after 5 min of the induction of anaesthesia stability(T0), at the end of operation(T1), and at postoperative 1 h(T2), 6 h(T3) and 24 h(T4).The neuropsychology in all operation patients were tested and assessed before 1 day of operation and after 3 and 8 days of operation.Results:The levels of S-100β and NSE in two groups at T2 to T4 were significantly higher than those at T0(P<0.01), but the levels of S-100β at T2 and T3, and the levels of NSE at T2 to T4 in RIPC group were significantly lower than those in CON group(P<0.01).The MMSE and MoCA scores in RIPC group after 3 days of operation were lower than those in CON group(P<0.05 to P<0.01).Conclusions:The RIPC can decrease the levels of S-100β and NSE in the patients treated with cardiac valve replacement under CPB, and the incidence of cognitive impairment of patients, which can play a role on protecting brain.
The clinical application of the noninvasive positive pressure ventilation in acute heart failure

Abstract:
Objective:To evaluated the clinical effects of the noninvasive positive pressure ventilation(NIPPV) in the treatment of acute heart failure(AHF).Methods:Sixty patients with AHF were randomly divided into the control group and treatment group.The control group were treated with routine anti-heart failure therapy, and the treatment group were treated with NIPPV combined with routine therapy.The changes of clinical symptoms, heart rate(HR), respiratory rate(RR), mean arterial pressure(MAP), central venous pressure(CVP), arterial blood gas [partial pressure of oxygen(PaO2), partial pressure of oxygen(PaCO2) and oxygen saturation(SaO2)] in two groups were observed before treatment and after 2 and 8 hours of treatment.The rehospitalization conditions of patients caused by heart failure were followed up after 6 months of discharge.Results:The cardiac function in treatment group treated with NIPPV was significantly improved, the effect of which was better than that in control group(P<0.05).After 2 and 8 hours of therapy, the levels of HR, RR, PaO2 and SaO2 in treatment group were better than those in control group(P<0.01).The difference of the PaCO2 level in treatment group between before treatment and after 2 and 8 hours of therapy was not statistically significant(P>0.05).Compared with the control group, the levels of PaO2, SaO2 and CVP in treatment group increased(P<0.05 to P<0.01), and the levels of HR, RR and MAP in treatment group decreased(P<0.05 to P<0.01).The difference of the rehospitalization rate between two groups caused by heart failure was not statistically significant during the six months of following-up(P>0.05).Conclusions:NIPPV can obviously improve the hypoxemia and cardiac function in patients with AHF, which is a kind of safe and effective method in the treatment of AHF.
The clinical significance of serum progastrin releasing peptide in the diagnosis and staging of small cell lung cancer

Abstract:
Objective:To explore the clinical value of the levels of serum progastrin releasing peptide(proGRP) in the differential diagnosis and clinical staging of small cell lung cancer(SCLC) and non-small cell lung cancer(NSCLC).Methods:The serum levels of proGRP in 62 patients with SCLC(including 25 cases of limited stage and 37 cases of extensive stage) and 38 patients with NSCLC were measured by chemiluminescent immunoassay, which was compared with neuron specific enolase(NSE).The sensitivity, specificity and tumor staging in all patients were analyzed.Results:The sensitivities of the single detection of proGRP, NSE and combination detection of proGRP and NSE in diagnosing SCLC were 82.93%, 64.15% and 71.58%, respectively.The positive rate of the combination detection of proGRP and NSE in diagnosing SCLC was significantly higher than that of the single detection of NSE, the difference of the positive rate between combination detection proGRP and NSE and single detection of proGRP was not statistically significant(P>0.05).The serum levels of proGRP and NSE in patients with SCLC were obviously higher than those in NSCLC patients(P<0.01).Among the SCLC patients, the serum levels of proGRP and NSE in extensive stage patients were significantly higher than those in limited stage patients(P<0.01).Conclusions:The ProGRP is a tumor marker of SCLC, which is high sensitivity and specificity, and can reflect the staging.The combination detection of proGRP and NSE can improve the accurate rate of SCLC diganosis.
Clinical effect of the rosuvastatin combined with nicorandil in the treatment of coronary slow flow

Abstract:
Objective:To observe the effects of the rosuvastatin combined with nicorandil in the treatment of coronary slow flow(CSF).Methods:Sixty-four patients with CSF diagnosed by coronary arteriography were randomly divided into the control group and treatment group(32 cases each group).The control group were treated with aspirin combined with rosuvastatin, the treatment group were additionally treated with nicorandil based on the control group.The improvement of chest pain and the levels of APN, TNF-α and hs-CRP in two groups after 4 and 12 weeks of treatment were measured and recorded.Results:After 4 weeks of treatment, the chest pain symptoms and levels of APN in two groups were significantly improved.The levels of APN in two groups increased significantly after 12 weeks of treatment, and the increasing degree of APN in treatment group was higher than that in control group(P<0.01).After 4 weeks of treatment, the levels of hs-CRP and TNF-α in two groups decreased.The levels of hs-CRP and TNF-α in two groups decreased significantly(P<0.01) after 12 weeks of treatment, and the decreasing degree of hs-CRP and TNF-α in treatment group was higher than those in control group(P<0.01).Conclusions:The treatment of coronary slow flow with rosuvastatin combined with nicorandil can ease the chest pain, significantly improve the level of APN and decreased the levels of hs-CRP and TNF-α of patients.
The expressions and significance of kinase domain receptor,melanocortin-2 receptor and proliferating cell nuclear antigen in aldosterone-producing adenoma and adrenal cortical hyperplasia

Abstract:
Objective:To explore the expressions and significance of kinase domain receptor(KDR), melanocortin-2 receptor(MC2R) and proliferating cell nuclear antigen(PCNA) in aldosterone-producing adenoma(APA) and adrenal cortical hyperplasia(IHA).Methods:The expressions of KDR, MC2R and PCNA in 40 cases APA and 40 cases IHA lesions were detected by immunohistochemistry.Results:The positive expression rates of KDR, MC2R and PCNA in APA lesions were 40.0%, 32.5% and 85.0%, respectively.The expression rates of KDR, MC2R and PCNA in IHA lesions were 65.0%, 62.5% and 77.5%, respectively.The differences of KDR, MC2R and PCNA expressions between APA and IHA were statistically significant(P<0.05 to P<0.01).Conclusions:The KDR, MC2R and PCNA play an important role in the occurrence and development of IHA, which have important reference meaning in the differentiation of APA and IHA lesions.
Analysis of 25 cases with Severe complication following gynecological laparoscopic operations

Abstract:
Objective:To analyze the related factors of the severe complications following gynecological laparoscopic operations and prevent the complications.Methods:The data of 3210 patients treated with laparoscopic surgery were retrospectively analyzed from June.2003 to May.2013.Results:Compared with the uterus adnexal surgery, the incidence of complications and transfering operation rate of hysterectomy were significantly increased(P<0.01).The transfering operation rate of hysteromyomectomy was significantly higher than that in the uterus adnexal surgery(P<0.01).The number of patients treated with gynecological laparoscopic operations increased from June.2003 to May.2008, and the incidence of severe complications of hysterectomy decreased significantly(P<0.01).Conclusions:The incidence of severe complications following gynecological laparoscopic operations and transfering operation rate are related to the operation type, operation difficulty and performer level.
Effect of magnesium valprote in the treatment of migraine

Abstract:
Objective:To explore the effects and safety of magnesium valproate in the treatment of migraine.Methods:Eighty-one patients with migraine(including miss follow-up 6 cases) were randomly divided into the flunarizine group(37 cases treated with flunarizine) and magnesium valprote group(38 cases treated with magnesium).Compared with before treatment, the frequency of headache attack and severity of headache in two groups after 3 months of treatment were analyzed.Results:Compared with before treatment, the frequency of headache attack in two groups decreased significantly(P<0.01), and the difference of which between two groups was not statistically significant after treatment(P>0.05).Compared with before treatment, the scores of headache severity in two groups decreased significantly(P<0.01), and the difference of which between two groups was statistically significant after treatment(P>0.05).The incidences of adverse reactions in flunarizine group and magnesium valprote group were 18.92% and 23.68%, respectively, the difference of which was not statistically significant(P>0.05).Conclusions:Magnesium valproate treating migraine is secure and effective.

Abstract:
Objective:To investigate the application of the synchronization imaging of electroencephalogram-functional MRI(EEG-fMRI) in the locating the epilepsy lesion of temporal lobe.Methods:Seventeen patients with temporal lobe epilepsy were detected using EEG-fMRI.The true location of epileptogenic focus and scope was identified by cortical electrode and deep electrode, which was compared with the results of EEG-fMRI.Results:The location and scope of disease detected by two methods were exactly same in 7 cases.The centre of lesions in 10 cases were unanimous, but the concordant scope was big.Conclusions:EEG-fMRI is a noninvasive method to locate the time and space, which can intuitively know the location of epileptogenic focus and scope in fMRI imaging.
Effect comparison of vitamin B6 through different administration routes in the treatment of pregnancy hyperemesis

Abstract:
Objective:To explore the clinical effects of vitamin B6 through oral, intravenous infusion and acupoint injection in the treatment of pregnancy hyperemesis.Methods:Eighty-four patients with pregnancy hyperemesis were randomly divided into the group A, B and C(28 cases each group).The A, B and group C were treated with vitamin B6 by oral, intravenous infusion and acupoint injection, respectively.The motilin levels and treatment effects in 3 groups were observed before and after treatment.Results:The total efficiency in group A(100.00%) was significantly higher than that in group B(71.43%) and group C(82.14%) after 1 week of treatment(P<0.01).The level of motilin in group C(133.53±33.16) ng/L was significantly higher than that in other two groups after treatment(P<0.01).One case with subcutaneous hemorrhage caused by short pressing time in group C was improved by himself.Conclusions:Acupoint injection of vitamin B6 can effectively improve the motilin secretion, promote gastrointestinal motility and prevent hyperemesis gravidarum.The curative effect of vitamin B6 is obviously superior to the oral and intravenous drip, and which is worthy of the clinical promotion.

Objective:To investigate the clinical effects of Tanreqing injection in the treatment of pneumonia children.Methods:One hundred and seventy-six pneumonia children were randomly divided into the observation group and control group(88 cases each group).The control group were treated with antiviral, antibacterial, relieving cough and reducing sputum, the observation group were additionally treated with 0.5 ml/kg of Tanreqing injection by intravenous drip based on conventional therapy, once a day, 7 days for a course.The improvement of signs and symptoms and adverse reactions in two groups were compared after 1 course.Results:The total effective rate in the observation group(90.9%) was significantly higher than that in control group(79.5%)(P<0.01).The time of defervescence, disappearing of cough and wheezing, dyspnea remission, pulmonary rales disappearance and hospitalization in observation group were shorter than those in control group(P<0.01).The control group were small children with facial rash, other patients were no obovious adverse reactious.Conclusions:Tanreqing injection can effectively improve the clinical symptoms, significantly improve the therapeutic effect and shorten the course of disease of pneumonia children, which is safe, reliable and worthy of popularization and application.

Abstract:
Objective:To observe the clinical efficacy of levocarnitine combined with salvianolate in the treatment of stable angina in elderly patients(≥65 years old).Methods:One hundred and twenty elderly patients with coronary heart disease complicated with stable angina were randomly divided into the observation group and control group(60 cases each group).Both groups were treated with conventional treatment(including aspirin, nitrates, calcium channel blockers and β-R blockers).The observation group were additionally treated with levocarnitine combined with salvianolate.The clinical symptoms and electrocardiogram in two groups were observed before and after treatment.Results:The total effective rates of clinical symptoms improvement in the observation group(90.0%) was statistically higher than control group(63.3%)(P<0.01).The total effective rates of the electrocardiogram improvement in the observation group(86.7%) was statistically higher than control group(53.3%)(P<0.01).No significant adverse reaction in two groups was found.Conclusions:The treatment of stable angina in elderly patients with coronary heart disease with levocarnitine combined with salvianolate is safe and effective, which can significantly alleviate the clinical symptoms, improve the ischemic electrocardiogram, and is worthy of further promotion in clinic.

Abstract:
Objective:To observe the clinical efficacy of dezocine combined with remifentanil in the sedation and analgesia of Alzheimer's disease patients treated with phacoemulsification.Methods:Sixty Alzheimer's disease patients with grade ASA Ⅰ to Ⅱ treated with phacoemulsification were randomly divided into the dezocine group(30 cases, group D) and control group(30 cases, group R).The group D were treated with 0.25μg·kg-1·min-1 of remifentanil for 2 min by vein, then the dosage of remifentanil was adjusted for 0.08μg·kg-1·min-1, and the patients were additionally injected using 0.04 mg/kg of dezocine combined with 0.02 to 0.06 mg/kg of midazolam by vein.The group R were treated with 0.25μg·kg-1·min-1 of remifentanil for 2 min by wein, then the dosage of remifentanil was adjusted for 0.08μg·kg-1·min-1, and the patients were additionally injected using 0.02 to 0.06 mg/kg of midazolam by vein.The midazolam dosage and intraoperative remifentanil dosage were adjusted to maintain the modified observer assessment of sedation ≤2, which was stopped until the end of operation.The mean arterial pressure, heart rate, respiratory rate, oxyhemoglobin saturation, time of operation duration and patients awakening, dosage of remifentanil and midazolam and intraoperative adverse reaction were recorded before anaesthesia, after anesthesia, at the start of operation and at the end of operation.Results:Compared with the group R, the changes of oxyhemoglobin saturation and respiratory frequency in group D were less after anesthesia(P <0.05 to P<0.01), the time of operation duration and patients awakening in group D were shorter(P<0.01), the dosage of remifentanil and midazolam in group D were less(P<0.01), and the incidence of respiratory depression was lower(P<0.05).Conclusions:The clinical effect of dezocine combined with remifentanil in the sedation and analgesia of Alzheimer's disease patients treated with phacoemulsification is good, which is a safe method of anesthesia.

Abstract:
Objective:To analyze risk factors of lumbar infection caused by sacral canal blocking, and find corresponding preventive measures.Methods:Review cases from Jan.2006 to Jan.2014, according to analysis of rate, single factor analysis, sites, types and bacteria species, screen risk factors for lumbar infection.Results:Infection rate was 8.6%(18 in 210), among the related risk factors, the cases that over the age of 40, with diabetes mellitus, with spina bifida occulta, with cerebrospinal fluid leakage caused by puncture, and combined with more than 4 kinds of drugs injection, whose infection rates were higher than cases that below the age of 40, without diabetes mellitus, without spina bifida occulta, without cerebrospinal fluid leakage caused by puncture, and combined with less than 4 kinds of drugs injection(P<0.05 to P<0.01).The infection site located mainly in L5~S1 and posterior lumbar soft tissue, the infection types mainly were intervertebral infection and posterior lumbar soft tissue infection, bacteria species mainly were gram-positive cocci.Conclusions:According to analysis of risk factors of infection caused by sacral canal blocking, we can take preventive measures, to avoid infection.

Abstract:
Objective:To investigate the clinical effects of the hemostasis with predictive perforating vessels during the eschar operation of burn wound of limbs.Methods:Seventy patients with Ⅱ degree and above burn treated with paring scab were randomly divided into the observation group and control group(35 cases/group), the observation group given predictive perforating vessels hemostasis, the control group given conventionalelectric coagulation hemostasis.The 1% overall surface area(TBSA), operative time, blood loss, blood transfusion and graft survival and postoperative complications in two groups were compared.Results:The operative time of 1% TBSA wound in observation group was shorter than that in control group(P<0.01).The amount of blood loss and blood transfusion in observation group were significantly less than those in control group(P<0.01).The graft survival rates in treatment group and control group were 97%, the 2 cases with subcutaneous hematoma in treatment group and 3 cases with subcutaneous hematoma in control group were found, the difference of which was not statistically significant(P>0.05).Conclusions:The hemostasis with predictive perforating vessels can effectively shorten the skin grafting time, control intraoperative blood loss and reduce the amount of blood transfusion.
Comparison of therapeutic effect of the replantation and non-replantation on the amputated distal finger

Abstract:
Objective:To compare the therapeutic effect of the replantation and non-replantation of amputated finger on the amputated finger in the distal finger.Methods:Among the 286 patients treated with distal finger complete disconnection operation, the 118 fingers(112 cases) in observation group were treated with replantation, the 183 fingers(174 cases) in control group were treated with non-replantation(including skin flap repair, suture in situ and stump dressing).The figure appearance, nail length, two-point discrimination, distal interphalangeal joint motion and finger sense between two groups were compared.Results:The survival rate in the observation group the and control group were 97.46% and 94.54%, respectively, the difference of which was not statistically significant(P>0.05).The nail length, two-point discrimination, DIPJ activity and finger sense in the observation group were better than those in the control group after 6 months of operation(P<0.01).The excellent rates of finger joints activity in the observation group and the control group were 97.39% and 77.86%, respectively, the difference of which was statistically significant(P<0.05).Conclusions:Strictly following the indications of different operations and technical requirements are the key of the success of the clinical treatment.

Abstract:
Objective:To investigate the effects of dehydroepiandrosterone(DHEA) on patients with diminished ovarian reserve(DOR).Methods:Thirty patients with DOR were treated with DHEA for 3 months.The ovarian reserve indexes before and after DHEA treatment were compared, and the induced ovulation rate, clinical pregnancy rate and abortion rate after treatment were observed.Results:Compared with before treatment, the levels of follicle-stimulating hormone(FSH), FSH/lutenizing hormone and estradiol on the 3rd day of menstrual cycle obviously decreased(P<0.01), and antral follicle count significantly increased after 3 months of treatment with DHEA(P<0.01).The induced ovulation in 24 cases were successful, the ovulation rate was 80.00%.The pregnancy in 11 cases were successful, the cumulative pregnancy rate was 45.83%.Two pregnancy cases for 2 months aborted, the abortion rate was 18.18%.Conclusions:DHEA can effectively improve the ovarian reserve function, sensitivity of fertility drugs, success rate of induced ovulation and egg quality, obtain the good clinical pregnancy rate, and decrease the adverse reaction.
The application value of percutaneous lung biopsy guided by CT in the diagnosis of pulmonary occupying lesions

Abstract:
Objective:To explore the application value of the percutaneous lung biopsy guided by CT in the diagnosis of pulmonary occupying lesions.Methods:The pathological data of different space.occupying lesions of lung in 110 patients were retrospectively analyzed.The postoperative adverse reactions and complications were recorded.The advantages and disadvantages of the percutaneous lung biopsy guided by CT technology were analyzed.Results:The effective specimens were successfully obtained from 110 patients with different space-occupying lesions, one-time.Eighty-three cases were identified by histologic examination, The diagnosis rate, specificity, false positive rate, false negative rate and sensibility were 93.6%(103/110), 74.1%(20/27), 25.9%(7/27), 0.0% and 100.0%(83/83), respectively.The incidence of complications caused by lung puncture biopsy in 110 cases was 27.3%, serious pneumothorax in 3 cases(accounting for 2.7%) were found.Conclusions:The percutaneous lung biopsy guided by CT in patients with different pulmonary occupying lesions is feasible, safe and no adverse reaction, which is worthy of further clinical application.

Abstract:
Objective:To observe the clinical effects of ambroxol in the treatment of secretory otitis media, and its influence on the inflammatory mediators in serum and ear fluid.Methods:Seventy-eight patients with secretory otitis media were randomly divided into the control group(treatment with routine method) and observation group(treatment with routine method combined with ambroxol)(39 cases each group).The total effective rates at 1 and 2 weeks after treatment and the levels of inflammatory mediators before and after treatment between two groups were compared.Results:The total effective rates in observation group at 1 and 2 weeks after treatment were higher than those in control group(P<0.05).The levels of inflammatory mediators in observation group after treatment were lower than those in control group(P<0.01).Conclusions:The clinical effect of ambroxol in the treatment of secretory otitis media is good, and its improvement for the inflammatory mediators serum and ear fluid is not obvious, the application value of which in treating secretory otitis media is high.
The observation of curative effect of Chenlanhua particles in the treatment of diabetic foot ulcers

Abstract:
Objective:To observe clinical effects of Chenlanhua particles in the treatment of diabetic foot(DF) ulcers.Methods:One hundred and fifteen cases of DF patients were randomly divided into treatment group(58 cases), control group(57 cases).The ourse of treatment was 28 days in both of the two groups.The efficacy score, erythrocyte sedimentation rate and neutrophil proportion after one course of treatment were observed.Results:The total effective rate in the treatment group was 89.66%, which was better than that in the control group by 71.93%(P<0.01).The wounds in the treatment group were significantly improved, the efficacy score, erythrocyte sedimentation rate and the proportion of neutrophils were significantly lower than those in the control group(P<0.01).Conclusions:Chenlanhua particles could gain good clinical curative effect in the treatment of DF ulcers.

Abstract:
Objective:To develop the competency evaluation index system of the rural general practitioners.Methods:Thirty general practitioners from Bengbu and Chuzhou, 10 administrators from the health and Family Planning Commission in city/county and 10 presidents from township hospitals were interviewed using the critical incident interview method, the results of which were analyzed by NVivo8.0.software.Thirty experts were consulted with Delphy expert consultation for two rounds.The 1458 rural general practitioners from Henan, Hunan, Shanxi, Hubei, Anhui and Jiangxi provinces were investigated by questionnaire.Results:Nine competency quality dimensions(including personal trait, occupation moral ability, psychological adaptability, basic public health service ability, basic clinical skill, information management capability, system analysis capability, communication capability, cooperation capability and nonmedical professional knowledge) were extracted according to the meaning of the subject and opinions of experts.The results of Delphy expert consultation showed that the enthusiasm and authority degree of experts were 100% and Cr=0.89, respectively.The exploratory factor analysis results showed that the kaiser-meyer-olkin was 0.976, the P value was 0.000, 9 common factors were extracted, the cumulative variance was 75.128%, the Cronbach's α of the total questionnaire and various dimensions of the questionnaire was from 0.857 to 0.975.The results of confirmatory factor analysis showed that the χ2/df was 2.649, the root mean square error of approximation was 0.065, the goodness of fit index was 0.94, the incremental fit index was 0.98, the non-normed fit index was 0.98 and the comparative fit index was 0.98.The score of each dimension in the performance excellence general practitioners was significantly higher than that in qualifier(P<0.01).Conclusions:The development of the rural general practitioner competency assessment system can be used as the evaluating basis of the competency of rural general practitioner.

Objective:To explore the relationship between the perceived academic self-efficacy and academic adaptability in vocational college students.Methods:Nine hundred and twenty-five vocational students were investigated using the perceived academic self-efficacy scale and academic adaptability questionnaire.The variable of social demography was descriptively analyzed, the related factor of self-efficacy was investigated, and the relationship between the academic self-efficacy and learning adaptability was analyzed.Results:The differences of the total scores of self-efficacy of academic behavior and learning ability, and perceived academic self-efficacy scale in vocational college students with different gender, grade, age, whether the only child, parents' cultural level, family residence and family economy were not statistically significant(P>0.05).The total scores of self-efficacy of academic behavior and learning ability and perceived academic self-efficacy scale in the upper and middle achievement vocational college students were significantly higher than those in the lower achievement students(P<0.01).The scores of students' academic self-efficacy, learning ability and learning behavior self-efficacy scores were significantly positive correlated with the scores of learning adaptability, physical and mental health, learning interest, learning strategies, learning environment and teaching model(P<0.01).Correlation analysis showed that the total scores of learning adaptability and five components of the study were significantly positive correlation with the total scores of the learning self-efficacy and components(P<0.01).Conclusions:The learning adaptability of higher vocational college students is an important factor of influencing the learning self-efficacy.

Abstract:
Objective:To investigate the nosocomial infection prevalence rates and the risk factors of nosocomial infection, and to provide the scientific basis for the control of nosocomial infection.Methods:A survey was performed through medical record checking and bedside visiting in July 10, 2013 in Fuyang Second People's Hospital.Results:A total of 1030 cases were enrolled, 1026 cases were received in the survey, and the actual investigation rate was 99.61%.Twenty-two cases were subjected to nosocomial infection, the nosocomial infection rate was 2.14%.The two major infection positions were surgical site infection(31.82%) and lower respiratory tract infection(22.73%).Infectious ICU was the department which had the highest prevalence rate(28.57%).The total application rate of antibiotics was 44.64%.Fourteen strains of bacteria were isolated, and gram-negative bacilli were the major pathogens.The main blood borne diseases were HBV(16.61%), HIV(2.98%) and HCV(4.13%).Conclusions:There were high prevalence of blood borne diseases among patients in the infectious disease hospital.So the protection of medical personnel occupation should be strengthen.Infectious ICU has the highest prevalence rate of nosocomial infection, surgical site and the lower respiratory tract were the key positions of nosocomial infection.We should pay attention to the key departments and key infection site management.

Abstract:
Objective:To investigate the clinical value of two-dimensional ultrasound combined with four-dimensional ultrasound in the diagnosis of fetal malformation.Methods:Five thousand and fifty pregnant women detected by two-dimensional ultrasound combined with four-dimensional ultrasound, pregnancy 20 to 28 weeks were investigated.The fetus was examined using two-dimensional ultrasonography, the fetus suspected with malformation was additionally examined using four-dimensional ultrasound.The data of the induced abortion, delivery and further examination results from superior hospital were analyzed to evaluate the diagnosis coincidence rate.Results:Five thousand and sixty-four fetuses(including 14 cases twin) born by five thousand and fifty pregnant women included 87 cases malformation detected by two-dimensional ultrasonography and 117 cases malformation detected by two-dimensional ultrasound combined with four-dimensional ultrasound.The diagnosis coincidence rate of two-dimensional ultrasound combined with four-dimensional ultrasound(94.4%) was significantly higher than that of two-dimensional ultrasound(70.2%)(P<0.01).Conclusions:The two-dimensional ultrasound combined with four-dimensional ultrasound can effectively improve the accurate rate of diagnosis, and reduce the birth rate of defect child, which has high clinical value.

Objective:To investigate the application value of single nasolabial coronal section and conventional three section combined with special cross section in the screening of cheilopalatoschisis.Methods:Among 2530 foetuses, the nasolabial coronal section in 1000 cases(group A) and conventional three section combined with special cross section in 1530 cases(group B) were detected using ultrasound.The results of whose were compared with neonates.Results:The rates of misdiagnosis and consistent in group A and group B were 40.00% & 60.00%, and 4.76% & 95.24%, respectively.Prenatal sonographic manifestations showed that the incidence of upper lip skin discontinuity was significantly higher than that of the alveolar cleft, palate discontinuity echo and nose collapse(P<0.01).Conclusions:The conventional three section combined with special cross section can accurately diagnose the fetal cheilopalatoschisis.

Objective:To explore the levels of serum immunoglobulin IgG subclass, complement C3 and C4, and C reactive protein(CRP) in children with hand-foot-mouth disease(HFMD) and its clinical significance. Methods:One hundred and three severe HFMD children and 301 common HFMD children were divided into the severe and common groups, 50 healthy children were set as the control group. The serum levels of immunoglobulin IgG subclass, complement C3 and C4, and CRP in 3 groups were detected. Results:The levels of immunoglobulin IgG1 and IgG3, complement C3 and C4 in severe group were significantly lower than those in control group and common group(P<0.01), the level of CRP in severe group were significantly higher than that in the control group and common group(P<0.01). The differences of the levels of immunoglobulin IgG1, IgG2, IgG3 and IgG4, complement C3 and C4, and CRP between the common group and control group were not statistically significant(P>0.05). Conclusions:The humoral immune response in severe HFMD children is abnormal, children can be secondarily bacteria infected on the basis of virus infection. The combined detections of immunoglobulin IgG subclass, complement C3 and C4, and CRP has certain clinical significance in understanding the progress of the HFMD.

Abstract:
Objective:To investigate the effects of the nursing intervention on the moderate and advanced ovarian carcinoma patients treated with abdominal hyperthermia combined with TP program. Methods:Two hundred and thirty-four moderate and advanced ovarian carcinoma patients treated with abdominal hyperthermia combined with TP program were divided randomly into the control group and intervention group(117 cases each group). The control group were conventionally nursed. In intervention group before, during and after chemotherapy and hyperthermia, the considerations were prompted, the vital signs were paid close attention to, and the relevant nursing was implemented. The short-term effects, adverse reactions and long-term survival rate between two groups were analyzed. Results:The short-term total effective rate in the intervention group(69.2%) was significantly higher than that in the control group(51.2%)(P<0.01). The differences of the incidence rates of leucopenia and anaphylaxis between two groups were not statistically significant(P>0.05). The incidence rates of nausea and hepatic injury in the intervention group were significantly lesser than those in the control group(P<0.05). The one-year and three-year survival rates in the intervention group(86.3% and 54.7%) were significantly higher than those in the control group(72.6% and 41.9%), respectively(P<0.05). Conclusions:The reasonable nursing intervention in moderate and advanced ovarian carcinoma patients treated with abdominal hyperthermia combined with TP program can improve the short-term effects and long-term survival rate, and decrease the complications.

Abstract:
Objective:To explore the effects of the Roy adaptation model on the physical and mental status in patients with pregnancy-hypertension disease(PH). Methods:Eighty patients with PH were randomly divided into the control group(routine nursing group) and intervention group(Roy adaptation model nursing group)(40 cases each group). The quality of life and psychological situation in two groups were evaluated using the SF-36, and Hamilton depression scale(HAMD) & Hamilton anxiety scale(HAMA), respectively. Results:Before the nursing intervention, the difference of the scores of SF-36 between two groups was not statistically significant(P>0.05). After the nursing intervention, the scores of each dimension of SF-36 in intervention group were significantly higher than that in control group(P<0.01). Before the nursing intervention, the differences of the scores of HAMD and HAMA factors between two groups were not statistically significant(P>0.05). After the nursing intervention, the scores of HAMD and HAMA factors were lower those in control group(P<0.01). Conclusions: The PH patients intervened with Roy adaptation model can significantly improve the psychological state and quality of life of patients.

Objective:To investigate the effects of massaging head acupoint on the behavior ability and brainstem auditory evoked potential(BAEP) of high-risk neonates. Methods:One hundred and fifty-six full-term high-risk neonates were randomly divided into the massaging head acupoint group(80 cases) and control group(76 cases). The massaging head acupoint group and control group were treated with comprehensive massaging head acupoint combined with visual stimulation and simple visual interference, respectively. The total scores of neonatal behavioral neurological assessment(NBNA), behavior ability score and normal rate of BAEP between two groups were compared after treatment. Results:The total scores of NBNA, behavior ability score and normal rate of BAEP in massaging head acupoint group were significantly higher than those in control group(P<0.05 to P<0.01). Conclusions:The massaging head acupoint is helpful to promote the repair of brain damage, and improve early behavioral responses in high risk neonates, which lays the foundation for the further developing intelligent.
Application of whole seamless nursing model in quality nursing of operation room

Abstract:
Objective:To explore the clinical effects of the application of whole seamless nursing model in operation room. Methods: Seventy-four patients scheduled by operation were randomly divided into the control group and intervention group(37 cases each group). The control group and intervention group were nursed with the quality nursing and whole seamless nursing model based on the quality nursing, respectively. The mental comfort, surgical stress and nursing satisfaction in two groups were observed before and after nursing. Results:Compared with before nursing, the SAS and SDS scores in two groups were significantly decreased after nursing(P<0.01), but for intervention group, the SAS and SDS scores decreased more significantly(P<0.01). After nursing, the systolic and diastolic pressure and heart rate in intervention group were significantly lower than those in the control group(P<0.01). The difference of the satisfaction in the two groups was not statistically significant(P>0.05). Conclusions:The application of whole seamless nursing model in operation room can effectively improve the patient's mental comfort, reduce the occurrence of surgical stress and make the patients maintain a good attitude.

Abstract:
Objective:To explore the effects of the cardiopulmonary resuscitation(CPR) skill training combined with supervision evaluation on the skill of emergency department nurses. Methods:Forty emergency department nurses were randomly divided into the training group and control group(20 cases each group). The control group and training group were trained using the conventional mode and CPR skill training combined with supervision evaluation, respectively. The degrees of mastery of the recovery position, opening the airway, connecting breath apparatus and CPR skills, and the incidence of error during nursing and number of testimonials from patients between two groups were statistically analyzed after training. Results:The examination achievements of the related CPR skills in training group were significantly higher than those in control group(P<0.01). The incidence of error during nursing in training group was significantly lower than that in control group(P<0.01). The difference of the number of testimonials from patients between two groups was not statistically significant(P>0.05). Conclusions:The CPR skill training combined with supervision evaluation can effectively improve the CPR skill of emergency department nurses, and decrease the incidence of error.

Abstract:
Objective:To explore the effect of foot soak with traditional Chinese medicine and foot massage on diabetic foot(DF) and changes of transcutaneous oxygen pressure(TcPO2 ). Methods:Ninety DF patients were divided into control group(n=30), foot soak group(n=30.and foot soak-massage group(n=30). The control group were given conventional treatment, the foot soak group received foot soak with Chinese medicinal herb in addition to the conventional therapy and the foot soak-massage group were applied foot massage in addition to foot soak. The therapy lasted for a total of 14 days. The patients' foot symptom scores and foot TcPO2 were recorded and measured the day before intervention and the first day after intervention. The efficacy was evaluated after the therapy. Results:After intervention, the score of the foot symptoms decreased significantly in all the three groups(P<0.01). The foot TcPO2 of the foot soak group and foot soak plus massage group improved significantly compared to that before intervention(P<0.01). The foot TcPO2 of the control group had no statistically significant difference before and after intervention(P>0.05). The foot symptoms, curative effect and foot TcPO2 of the three groups had statistic significance after intervention(P<0.05 to P<0.01). Conclusions:Either Foot soak with traditional Chinese medicine alone or combined with foot massage can improve the symptoms and TcPO2 of DF. The combination therapy is more effective than foot soak alone in improving the symptoms and TcPO2 of DF.
