The study of chemokine receptors CXCR4 expression regulation by N-peptide of vMIP-Ⅱ

Abstract:
Objective: The aim of this study is to investigate the effect of viral macrophage inflammatory protein II (VMIP-II) on the expression of CXCR4 in the human breast cancer and the molecular mechanism.Methods: To designed and synthesized the sequence of CXCR4 core promoter were designed and synthesized,and established fireflyluciferase reported carrier;By detecting the fireflyluciferase activity of reported gene to reflect the effect of NT21MP on CXCR4 promotor; Western blotting and qRT-PCR were used to assess the expression of the related gene and miRNAs after depletion or overexpression of CXCR4,depletion of HER-2 and transfected with miRNAs inhibitor and mimic;SRB assay was used to measure the cell viability,while PI staining and AnnexinV/PI stainning was performed to analyze the cell cycle and apoptosis.Results: NT21MP down-regulated CXCR4 and up-regulated miR-125b,miR-135a and miR-146a(P < 0.05-P < 0.01);NT21MP inhibited the activity of CXCR4 core promoter;SP1 is a transcription factors,which connected with CXCR4 core promoter frequently,miR-135a mimic down-regulated SP1(P < 0.01);overexpression of CXCR4 up-regulated HER-2(P < 0.01),while depletion of CXCR4 down-regulated HER-2(P < 0.01);miR-125b down-regelated HER-2(P < 0.01);the downexpression of HER-2 can induce the expression of miR-146a;miR-146a inhibited CXCR4(P < 0.01);compared to the blank group,miR-146a and miR-135a all can inhibit cell proliferation,arrest cell cycle and induce apoptosis.Conclusions: NT21MP down-regulated the expression of CXCR4 by inducing miR-125b,miR-135a and miR-146a,inhibited the proliferation ability of breast cancer cell,and promoted theoretical basis to NT21MP applied to breast cancer treatment.
Relationship between delayed cardioprotection induced by monophosphoryl lipid A nitric oxide synthase combinded with protein kinase C

Abstract:
Objective: To observe the delayed protection effects of monophosphoryl lipid A(MLA) on ischemia-reperfusion(I/R) injury in rabbits,and investigate the protect effects of inducible nitric oxide synthase(iNOS) combined with protein kinase C(PKC) on cardioprotection induced by MLA.Methods: Twenty four newsland rabbits were randomly divided into the control group(intravenous injection of 0.9 sodium chloride solution),MLA group(intravenous injection of MLA) and inhibition group(Polymyxins combined with intravenous injection of MLA).Three groups were treated with I/R( 30 min of ischemia and 60 min of reperfusion),the PKC inhibitor/Polymyxin B(PMB) was intravenously injected into the inhibition group before 30 min of I/R.The creatine kinase(CK) releasing,arrhythmia happening and expression of iNOS were measured in rabbits induced by MLA.Results: The level of CK in MLA group was significantly lower than that in control group and inhibition group(P < 0.01).The arrhythmia in MLA group was not found,and the expression integral of iNOS in MLA group was significantly more than that in control group and inhibition group(P < 0.01).The differences of the expression integral of iNOS in cardiac muscular tissue between ischemic region and non-ischemic region in 3 groups were not statistically significant(P > 0.05).Conclusions: The MLA pretreatment can effectively alleviate the I/R myocardium injury after 24 h,namely mediating the delayed protection.PKC maybe involve in the delayed protection by regulating iNOS expression.
Protective effects of soybean isoflavones combined with fasudil on cerebral ischemia reperfusion injury in rats

Abstract:
Objective: To investigate the effects of soybean isoflavones(SI) combined with fasudil(Fas) on the expressions of high mobility group box 1(HMGB1),vascular cell adhesion molecule-1(VCAM-1),nuclear factor κB1(NF-κB1) in rats with cerebral ischemia reperfusion.Methods: The weight 240 to 260 g male SD rats were randomly divided into the sham group(S group),ischemia reperfusion(IR group),IR and Fas group,IR and SI group,and IR,Fas and SI group.The cerebral pathological change was observed by HE staining.The qualitative and quantitative expressions of NF-κB1(p50) and HMGB1 in cerebral tissue were measured by immunohistochemistry and western blotting,respectively.The expressions of HMGB1,NF-κB1 and VCAM-1 mRNA were examined by qRT-PCR.Results: Compared with the single drug group,the decreasing of the positive cells with NF-κB1 and HMGB1 expressions in IR,Fas and SI group were the more significant.Compared with the S group,the protein expression of HMGB1,and mRNA expression of HMGB1,NF-κB1and VCAM-1 in IR group increased significantly(P < 0.01).Compared with the IR group,the protein expression of HMGB1,and mRNA expressions of HMGB1,NF-κB1and VCAM-1 in other drug groups decreased significantly(P < 0.01).Compared with the single drug group,the decreasing of the protein expression of HMGB1,and mRNA expressions of HMGB1,NF-κB1 and VCAM-1 in IR,Fas and SI group were the more significant(P < 0.01).Conclusions: The protective effects of SI combined with Fas on cerebral ischemia reperfusion injury in rats maybe be implemented by inhibiting the expressions of HMGB1,NF-κB1 and VCAM-1,which can attenuate the signal channel of NF-κB,and decrease the cerebral tissue injury casued by inflammatory response.The protective effects of drug combination is significantly better than that of single drug.
Effect of Xuebijing on the expressions of HSP 90 in liver and lung injury of the septic mice

Abstract:
Objective: To detect the expression of heat shock protein 90(HSP90) in septic mice,and investigate the effects of Xuebijing on sepsis.Methods: Eighty-four mice were randomly divided into the sepsis model group(model group) and Xuebijing inhibition group(inhibition group).The serum levels of ALT,AST and HSP90,and HSP90 expressions and pathological changes in lung and liver tissues were detected after 0 h,1 h,2 h,4 h,8 h,12 h and 18 h of molding.Results: The serum levels of AST in inhibition group were higher than that in model group after 2 h,4 h and 8 h of administration(P < 0.05 to P < 0.01),but the serum levels of ALT in inhibition group were higher than that in model group after 1 h,4 h and 8 h of administration(P < 0.05 to P < 0.01).The HSP90 levels in serum,and liver and lung tissues in model group were significantly higher than that in inhibition group after 2 h to 18 h of administration(P < 0.01).Histopathology showed that the inflammation of lung and liver tissues gradually aggravated,the alveolar wall destructed,the alveolar septum widened,the interstitium proliferated,part of liver cells ballooning degenerated,spotty Leaflet necrosed,and hepatic portal bridging necrosis occurred in model group.The inflammation reaction in inhibition group significantly decreased.Conclusions: The expression of HSP90 in sepsis mice upregulates,which indicates that the HSP90 involves in the immune response and regulation.The Xuebijing can decrease the levels of HSP90 in liver and lung tissues,alleviate the inflammation reaction and pathological lesion of liver and lung tissues in sepsis mice.
The change of protein lysine acetylation in carcinoma tissue of the gallbladder and its clinical significance

Abstract:
Objective: To investigate the change of protein lysine acetylation in carcinoma tissue of the gallbladder and its clinical significance.Methods: Western blot analysis was used to determine the change of protein lysine acetylation in tissues of the gallbladders from 30 patients with carcinoma of the gallbladder and 20 patients with chronic cholecystitis.The quantitative results of some bands in western blot were analyzed.Results: The protein lysine acetylation was significantly increased in gallbladder cancer tissue,compared to the chronic cholecystitis.The quantitative analysis indicated that the significant increase of protein lysine acetylation took place in proteins with molecular weight (Mw) from 10 000 to 36 000.Conclusions: The protein lysine acetylation of gallbladder tissue could be examined by the highly sensitive and specific antibody against lysine acetylation.the lysine acetylation was evidently increased in gallbladder cancer protein,especially in a subset of proteins of Mw 10000 to 36 000.
Analysis of acute pancreatitis complicated with diabetes mellitus:a report of 18 cases

Abstract:
Objective: To investigate the clinical characters of the patients with acute pancreatitis(AP) complicated with diabetes mellitus,and improve the understanding of the disease diagnosis and treatment.Methods: The clinical data of 41 patients with mild acute pancreatitis(MAP) and 17 patients with severe acute pancreatitis(SAP) were retrospectively analyzed.Forty patients with simple AP and 18 patients with AP complicated with diabetes were divided into the control group and observation group,respectively.The levels of triglyceride(TG),hemoglobin A1c(HbA1c),serum calcium and C-reactive protein(CRP) in all patients were measured,and the scores of Acute Physiology and Chronic Health Evaluation(APACHEⅡ) in SAP patients were evaluated.Biochemical Indexes,acute physiology and chronic health evaluation(APACHEⅡ) score between two groups were compared.Results: The levels of the blood-glucose,fasting blood-glucose,HbA1c and serum calcium,the incidence of SAP and the scores of APECHEⅡ in observation group were significantly higher than those in control group(P < 0.01).Conclusions: Compared with the simple AP patient,the disease condition in AP patients complicated with diabetes are more serious.The high level of serum glucose maybe an important aggravation factor in AP patients.
Effect of the anterolateral and posterolateral approaches in hip replacement surgery on the postoperative recovery

Abstract:
Objective: To investigate the effects of the anterolateral and posterolateral approaches in hip replacement surgery on the postoperative recovery.Methods: Sixty patients treated with hip replacement surgery were randomly divided into the anterolateral group and posterolateral group(30 cases each group).The assessment before and after treatment,and comprehensive recovery training in two groups were implemented.The ability of daily movement,VAS score and perimeter measurement in two groups were observed before and after hip replacement surgery,and after 3 and 10 days of rehabilitation treatment.Results: The differences of the activities of daily living(ADL) between two groups treated with hip replacement surgery after 3 and 10 days of rehabilitation treatment were statistically significant(P < 0.01).The difference of the VAS score between two groups treated with hip replacement surgery after 3 days of rehabilitation treatment was statistically significant(P < 0.01).The differences of the circumference measurement between two groups treated with hip replacement surgery after 3 and 10 days of rehabilitation treatment were not statistically significant(P > 0.01).Conclusions: The effect of different surgery approaches on the ability of daily movement of postoperative patients treated with hip replacement surgery is small.Compared with the posterolateral group,the improvement time of the pain and swelling in anterolateral group after surgery shorten significantly.
Clinical value of the dynamic detection of vascular endothelial growth factor and brain specific isoenzyme of creatine kinase in neonates with hypoxic ischemic encephalopathy

Abstract:
Objective: To investigate the clinical value of the dynamic detection of vascular endothelial growth factor(VEGF) and brain specific isoenzyme of creatine kinase(CK-BB) in neonates with hypoxic ischemic encephalopathy(HIE).Methods: Eighty-four neonates with HIE(HIE group),which included 33 mild cases,27 moderate cases and 24 severe cases,were intravenously injected with citicoline and 1,6 fructose diphosphate.Fifty-two normal newborns were set as the control group.The levels of VEGF and CK-BB in two groups were detected after 24 h,72 h and 7 d of birth.Results: After 24 h and 72 h of birth,the levels of VEGF in severe HIE,moderate HIE and mild HIE were gradual decreasing,which was significantly higher than that in control group.After 7 d of birth,the differences of the levels of VEGF between two groups were not statistically significant(P > 0.05).After 24 h,72 h and 7 d of birth,the levels of CK-BB in severe HIE,moderate HIE and mild HIE were gradual decreasing,the levels of CK-BB in HIE group were significantly higher than that in control group after 24 h and 72 h of birth(P < 0.01).After 7 d of birth,the levels of CK-BB in moderate and severe HIE patients were significantly higher than that in control group(P < 0.01),and the difference of the level of CK-BB between the mild patients and control group was not statistically significant(P > 0.05).Conclusions: The levels of VEGF and CK-BB in neonates with HIE increase significantly,the dynamic detection of which can help to determine the degree of brain injury in neonates with HIE,and has positive value in the diagnosis,treatment and prognosis of HIE.
Effect of ticagrelor and clopidogrel on the platelet aggregation in patients with acute coronary syndrome

Abstract:
Objective: To explore the therapeutic effects of ticagrelor on the patients with acute coronary syndrome(ACS).Methods: One hundred and seven ACS patients were randomly divided into the ticagrelor group(group A,50 cases) and clopidogrel group(group B,57 cases).The clinical data of two groups were collected.The platelet aggregation rates in two groups were detected before and after 2 h,4 h,24 h,7 d and 30 d of medicine.The major adverse reaction and adverse cardiac event within 30 days were observed.Results: The platelet aggregation rates in group A after 2 h,4 h,24 h,7 d and 30 d of medicine were significantly lower than that in group B(P < 0.05 to P < 0.01),and the platelet aggregation rates in two groups before medicine were significantly lower than that in different time points after medicine(P < 0.01).The differences of the incidences of major adverse reaction and adverse cardiac event between two groups were not statistically significant(P > 0.05).Conclusions: The effect of the ticagrelor in inhibiting platelet in patients with ACS is better than that of clopidogrel,and the securities of two are similar.
Observation on the effect of active participation of the laryngeal mask in the anesthesia quality control during laparoscopic surgery

Abstract:
Objective: To explore the role of the laryngeal mask in the anesthesia quality control during laparoscopic surgery.Methods: Fifty patients treated with laparoscopic gallbladder excision,50 patients treated with laparoscopic uterine fibroids eliminate and 50 patients treated with laparoscopic assisted vaginal hysterectomy were divided into A,B and C group,respectively.Three groups were subdivided into the observation group(A1,B1 and C1 group,probe group) and control group(A2,B2 and C2 group,non-probe group),respectively.The PACU observation time,vomiting rate,postoperative gastrointestinal exhaust time,first time eating time and degree of satisfaction in all patients were observed.Results: The PACU observation time,gastrointestinal exhaust time and first time eating time in observation group were significantly shorter than those in control group(P < 0.01).The degree of satisfaction in A1 group was higher than that in A2 group(P < 0.05).Conclusions: The intraoperative surgical exploration of flatulence and timely handling can improve the PACU observation time,gastrointestinal exhaust time,first time eating time and degree of satisfaction.
The clinical, electrophysiological and pathological features of myotonic dystrophy in 8 patients

Abstract:
Objective: To explore the clinical,electrophysiological and pathological characteristics of myotonic dystrophy(DM).Methods: The clinical,electrophysiological and pathological data of 8 patients with DM were retrospectively analyzed.Results: The different degree of muscle weakness,myotonia or amyotrophy symptoms in 8 cases were found.The alopecia,intelligence disorder,menstrual disorder of no skeletal muscle symptoms were found in part of the patients.No family history in 3 cases and myotonic potential diagnosed by electromyogram examination in 7 cases were identified.Under the light microscopy,the unequal muscle fibers in size,nucleus moving inward,muscle membrane nuclear increasing,nucleus gathering and nuclear chain forming were found.The sarcoplasmic masses in 1 case and muscular dystrophy diagnosed by NADH-TR staining in 7 cases,which included that the major muscle fiber atrophy in 5 cases were type Ⅰ fiber,were identified.Conclusions: Electromyography and skeletal muscle biopsy have important value in diagnosing myotonic dystrophy.The illness onset,and skeletal muscle and multi-system involvement features can benefit for the DM typing.The illusion of "no family history of genetic" caused by genetic anticipation of myotonic dystrophy should be paid attention to.
Effects of nalmefene on the anesthesia resuscitation in patients treated with vocal polyp extirpation

Abstract:
Objective: To evaluate the clinical effects of nalmefene on anesthesia resuscitation in patients treated with vocal cord polypectomy.Methods: Ninety patients scheduled by vocal cord polypectomy were randomly divided into the control group(C group) and nalmefene group,and the nalmefene group were subdivided into the N1 and N2 group according to digital number table method(30 cases each group).The 0.9% of sodium chloride injection,0.25 μg/kg and 0.5 μg/kg of nalmefene were injected into the C,N1 and N2 group at end of the surgery.The changes of vital signs after 0,2 min and 10 min of administration,and after 5 min of extubation,and the breath recovery at T2 in 3 groups were recorded.The time of eye opening,extubation and orientation recovery,and the PaO2 and PaCO2 after 15 min of extubation were recorded.The scores of Ramsay scale,VAS scale and postoperative adverse reaction were recorded after 5,30,60 and 120 min of extubation.Results: Compared with the C group,the incidence of breathing recovery in N1 and N2 group significantly increased after 10 min of administration,and the time of eye opening,extubation and orientation recovery significantly shortened.The PaO2,PaCO2 and incidence of postoperative nausea and vomiting in 3 groups significantly decreased after 15 min of extubation(P < 0.05 to P < 0.01),and the differences of whose between N1 and N2 group were not statistically significant(P > 0.05).The consciousness in N1 and N2 group after 5,30,60 and 120 min of extubation were better than that in C group(P < 0.05),and the difference of which between N1 and N2 group was not statistically significant(P > 0.05).The differences of vital signs and VAS scale at each time-point between 3 groups were not statistically significant(P > 0.05).The differences of the incidences of nausea and vomiting,and respiratory depression after extubation between three groups were statistically significant(P > 0.05).Conclusions: The treatment of vocal cord polypectomy with nalmefene has better anesthesia recovery effects and low incidence of side action,which is worthy of promotion.
The application effect of wire-reinforced epidural catheter during the anesthesia period of cesarean section

Abstract:
Objective: To observe the application effects of wire-reinforced epidural catheter during the anesthesia period of cesarean section.Methods: Four hundred pregnancy women treated with cesarean section under combined spinal and epidural anesthesia were randomly divided into the ordinary epidural catheter group(group A) and wire-reinforced epidural catheter group(group B) according to the digital table method(200 cases each group).The two groups were treated with combined spinal-epidural anesthesia(CSEA),and intubated the corresponding epidural catheter.Results: The incidences of the catheter difficulty,paresthesia,bleeding withdraw,prolonging catheter and bleeding after extubating in group B were lower than those in group A(P < 0.05 to P < 0.01).The incidence of the discomfort in puncture location during postoperative analgesia in group B was significantly higher than that in group A(P < 0.01).The catheter discount,straying the subarachnoid or epidural hematoma were found in two groups.Conclusions: The safety of wire-reinforced epidural catheter during the cesarean section period is better than that in ordinary epidural catheter,but which can lead to pain discomfort in catheter reserved location.
Effect of pulmonary surfactant combined with nasal continuous positive airway pressure in the treatment of neonatal respiratory distress syndrome

Abstract:
Objective: To investigated the effects of pulmonary surfactant(PS) combined with nasal continuous positive airway pressure(NCPAP) in the treatment of neonatal respiratory distress syndrome(NRDS).Methods: Thirty-six cases with NRDS were treated with NCPAP combined with PS.The changes of clinical manifestation and blood gas index in 36 patients were observed before and after treatment.Results: After 4 hours of treatment of the 36 NRDS cases treated with NCPAP combined with PS,the cyanosis,shortness of breath,groan and three depressions sign significantly relieved or disappeared,the blood gas analysis showed that the PaO2 increased obviously,the PaCO2 decreased significantly and the acidosis were corrected in 33 cases.The success rate was 91.67%.The other 3 cases with invalid treatment were changed to the mechanical ventilation treatment.No chronic lung disease occurred in all patients.Conclusions: The clinical effect of NRDS treated with NCPAP combined with PS is good.The treatment method is noninvasive ventilation,simple and quick,which can prevent the development of NRDS,avoid the mechanical ventilation use and decrease the occurrence of chronic lung disease.
Analysis of the correlation between ischemic white matter lesions, and cognitive function and plasma level of heat shock protein 70

Abstract:
Objective: To investigate the effects of the different extent white matter ischemic lesions(WMIL) on the cognitive function,and correlation of plasma heat shock protein 70(HSP70) level with WMIL.Methods: According to the results of head MRI,75 patients with WMIL were selected.The patients were divided into the mild group,moderate group and severe group according to the Fazekas rating scale,and 40 cases normal people were set as the control group.The cognitive function were evaluated using the Montreal Cognitive Assessment(MoCA) scale,and the levels of HSP70 were measured using the enzyme-linked immunosorbent assay in four groups.Results: The scores of total MoCA,visual space,executive function and delayed memory ability in mild,moderate and severe group were significantly lower than those in control group(P < 0.01),the scores of attention and calculation in severe group were also lower than those in control group(P < 0.05).The score of Fazekas was significantly negative correlation with the score of MoCA(P < 0.01).The plasma levels of HSP70 in mild,moderate and severe group were significantly higher than that in control group(P < 0.01).The score of Fazekas was significantly positive correlation with the score of HSP70(P < 0.01).Conclusions: With the WMIL severity aggravating,the cognitive impairment is more severe,and the plasma HSP70 level is more high.The HSP70 may play certain clinical significance in evaluating the severity of WMIL.

Objective: To explore the diagnosis and treatment strategy of the acute obstruction of ureteral calculus complicated with infection in patients with solitary kidney for improving the diagnosis and treatment level.Methods: The treatment processes of the acute obstruction of ureteral calculus complicated with infection in two patients with solitary kidney were retrospectively analyzed,and the relevant literatures were reviewed.Results: Two patients with solitary kidney were treated with Holmium laser lithotripsy in emergency.One case with 0.7 cm calculus,postoperative sepsis,infectious shock and acute renal failure was treated with anti-infection and blood purification,and the patient cured.One case with 0.5 cm calculus and successful operation was treated with anti-infection and rehydration,and the patient recovered.Conclusions: The progression of ureteral calculus complicated with infection in patient with solitary kidney is fast and severe renal impairment.The individualized treatment should be used,the ureteral obstruction should be early relieved,and the renal function should be maximum protected.
Clinical analysis of the tracheobronchial foreign body complicated with tracheal granulation in children

Abstract:
Objective: To detect the cause of tracheal granulation and its correlation with tracheobronchial foreign body in children,and find the means to improve the prognosis of tracheobronchial foreign body.Methods: The data of the gender,course of disease,imaging at discharge in 968 children with trachebronchial foreign body(including 76 children complicated with tracheal granulation) were retrospectively analyzed.Results: The incidence rates of tracheal granuloma in male and female were 7.1% and 8.8%,respectively,the difference of which was not statistically significant(P > 0.05).The foreign body history was identified in 64.9% of the patients with tracheal granuloma and 84.7% of patients without tracheal granuloma.The incidence rate of tracheal granuloma in patients with foreign body history(6.0%) was significantly lower than that in patients without foreign body history(16.0%) (P < 0.01).The imaging performance of the pneumonia in 31.1% of patients with granuloma at discharge was found,which was significantly higher than that in 17.1% of patients without granuloma(P < 0.01).Conclusions: The formation of granuloma is not correlation with gender,and is correlation with foreign body history.The removal of granulation during the operation can improve the prognosis of tracheobronchial foreign body.
Application of the ultrasound combined with hysteroscopy examination in the diagnosis of uterine incision scar diverticulum

Abstract:
Objective: To explore the application value of the hysteroscopy combined with ultrasound in the diagnosis of uterine incision scar diverticulum.Methods: The clinical data of 35 patients with uterine incision scar diverticulum were retrospectively analyzed.Results: The average length,width and depth of uterine incision scar diverticulum detected by hysteroscopy combined with ultrasound were (19.6±5.2)mm,(17.6±4.3)mm and (11.5±3.9)mm,respectively,and the diagnostic accuracy rate was 100.0%.The difference of the diagnosis rate between hysteroscopy combined with ultrasound and transvaginal ultrasound was statistically significant(P < 0.01).Conclusions: Hysteroscopy combined with ultrasound examination in comprehensive assessing and diagnosing the uterine incision scar diverticulum is effective method,and which is worthy of clinical application.

Abstract:
Objective: To explore the clinicopathological features,immunophenotypes,pathological diagnosis and differential diagnosis of primary pulmonary myxoid sarcoma(PPMS).Methods: The clinical features,histopathological structure and immunophenotypes of a patient treated with right lower lobectomy were analyzed,and the related literature was reviewed.Results: The tumor showed multinodular architecture under microscope,the tumor cells arranged as reticular and cords,and rich myxoid stroma could be found in matrix.Immunohistochemically,the diffusely strong positive vimentin,and locally positive S-100 and epithelial membrane antigen could be used to diagnose the PPMS.Conclusions: PPMS is a very rare lung malignant soft tissue tumor,and its diagnosis mainly depends on the location and histopathologic features of tumor cells,and the detection of Ewing sarcoma breakpoint region 1 gene rearrangement can help to diagnosis.

Abstract:
Objective: To observe the clinical effects of fasudil in the treatment of interstitial lung disease associated with connective tissue disease.Methods: Sixty-two patients with interstitial lung disease associated with connective tissue disease were randomly divided into the control group(32 cases) and treatment group(30 cases).The control group were treated with conventional measures,and the treatment group were additionally treated with 30 mg fasudil based on the control group for 14 days.The 6-min walk test,pulmonary arterial pressure measured by echocardiography,high chest resolution CT and blood gas analysis and erythrocyte sedimentation rate and C reactive protein before and after treatment were compared.Results: After treatment,the 6-min walk distance increased,pulmonary artery pressure dropped,high resolution chest CT score decreased and activity reduced.The differences of whose between treatment groupbeforeand after treatment,and control group were statistically significant(P < 0.01).Conclusions: The treatment of interstitial lung disease associated with connective tissue disease with fasudilis effective.

Abstract:
Objective: To observe the effects of oxycodone hydrochloride injection on early pain in patients with Wilson's disease(WD) after splenectomy.Methods: Sixty WD patients were randomly divided into the fentanyl hydrochloride injection group(group F) and oxycodone hydrochloride injection group(group O).In postanesthesia cura unit(PACU),the groups F and O were intravenously injected with 20 μg(10 μg/mL) of fentanyl and 2 mg (1 mg/mL) of oxycodone hydrochloride to relieve pain at extubation after operation,repectively,the treatment did not stop until the VAS score of patients was less than or equal to 4 points,the total number of drug delivery,incidence of nausea and vomiting,and the scores of heart rate,blood pressure,respiratory rate,oxygen saturation and sedation in all patients were recorded.After discharged from PACU,the VAS scores after 1,3 and 6 hours and adverse reactions after 6 hours were recorded,the postoperative analgesia satisfactory degree,time of the first administration and adverse reactions after 48 hours in two groups were recorded.Results: In PACU,compared with group F,the incidence of the respiratory rate and blood oxygen desaturation decreasing in group O decreased(P < 0.05 and P < 0.01),the Ramsay sedation score in group O was lower than that in group F (P < 0.01),the number of titration administration frequency and cases of remedial administration,and the first remedy analgesia interval time in group O significantly decreased and prolonged,respectively(P < 0.01).At 6 h discharged from PACU,compared with group F,the incidence of the respiratory rate slowing and blood oxygen desaturation decreasing and sedation score in group O decreased(P < 0.05 and P < 0.01).At 48 h discharged from the PACU,compared with group F,the analgesia satisfaction degree in group O increased(P < 0.05).Conclusions: The treatment of the early pain in WD patients after splenectomy with oxycodone hydrochloride injection is good effects and less adverse reactions.

Abstract:
Objective: To explore the influence factors of social anxiety in senior pupils of elementary school.Methods: One thousand and twenty pupils from grade four to six in northern Anhui rural were investigated using the stratified cluster random sampling method.The questionnaire including the general condition,emotion management ability,parent-child attachment and social anxiety.Results: In the school environment,the differences of the total scores of different classroom environment,relationship between classmate and teacher,classmate relation,number of good friend,social anxiety,fear of negation, social avoidance and distress between students were statistically significant(P < 0.01).In the family environment,the scores of social anxiety and social avoidance and distress in students with father going out to work were higher than those in students without father going out to work(P < 0.05 and P < 0.01),and the scores of social avoidance and distress in students with mother going out to work were higher than those in students without mother going out to work(P < 0.05).The scores of social anxiety and binomial factors in students with "satisfaction " father live performance were lower than those in students without "satisfaction " father live performance(P < 0.05 to P < 0.01),the scores of social anxiety and binomial factors in students with "don't know" and "satisfaction " mother live performance were significantly lower than those in students without "satisfaction" mother live performance(P < 0.01),and the score of fear of negation in students with "general" and "good" economic condition were significantly lower than that in students with "poor" economic condition(P < 0.01).Correlation analysis showed that the emotion management and parent-child attachment were significantly negative correlation with the scores of social anxiety and each factor(P < 0.01).Multiple linear regression analysis indicated that the emotion management ability,mother-child attachment and classmate relation were the influence factors of the total scores of social anxiety and each factor(P < 0.01). Conclusions: The emotion management ability,mother-child attachment and classmate relation are the influence factors of social anxiety.Regulating the parent-child relationship and classmate relation,and improving the emotion management ability should be emphasized in the prevention of social anxiety of children.

Abstract:
Objective: To explore the characters of the sexual assault cases by analyzing the relevant forensic data of female victim with mental retardation.Methods: The forensic data of psychiatric identification in 83 female victims with mental retardation were retrospectively analyzed using the self-compiled questionnaire.Results: Most abilities loss of self-defence in the sexual abuse of female victims with mental retardation were identified(97.6%).The countryside area was the most crime scene(77.1%).The monitoring effort was negatively correlated with the violation degree of victim(P < 0.05).Conclusions: The sexual assault cases of female victim with mental retardation has its characteristics.The guardians and community should strengthen the sex education in female victim with mental retardation to improve their defense capability.

Abstract:
Objective: To evaluate the value of the real-time three-dimensional echocardiograph(RT-3DE) in the diagnosis of atrial septal defect(ASD) and guidance of transcatheter closure.Methods: Twenty-one patients with ASD were diagnosed by two-dimensional ultrasound and RT-3DE,the defect panorama was showed by three-dimensional imaging,the area of which was measured.The whole process of operation were guided by RT-3DE and fluoroscopy,and the blockage effects were evaluated after operation.The correlation between the maximum diameter of waist occluder and defect area measured by RT-3DE were studied.Results: All operations were successfully completed.Transthoracic RT-3DE could clearly display the shape,size,position of ASD and occluder releasing process.The differences of the ASD areas between RT-3DE measuring and maximum diameter of the waist minus 2 mm calculating were not statistically significant(P > 0.05).This differences between RT-3DE and X-ray in identifying the position and status of sheath,and occluder releasing were not statistically significant(P > 0.05).Conclusions: The method of transthoracic RT-3DE in guiding the closure of ASD is feasible,which can avoid the radiation of X-ray and has application value.

Abstract:
Objective: To investigate the application value of two-dimensional ultrasound quantification indexes combined with hepatic vein waveform classification in the evaluation of liver fibrosis degree of patients with chronic hepatitis B.Methods: The hepatic morphology,essence,echo and blood flow dynamics in 106 patients with chronic hepatitis B were observed using two-dimensional ultrasound and Doppler ultrasound.The liver capsula and papernchyma,hepatic vein wall and hepatic vein spectrum waveform were recorded,which were compared with the diagnosis of liver pathology.Results: With the increasing of fibrosis degree,the comprehensive scores of liver ultrasonography increased,and the difference of which in each group was statistically significant(P < 0.01).The proportions of HV1 and HV2 type wave in phase S0,S1 and S2 patients were lower than those in S4 phase patients(P < 0.05 to P < 0.01).Conclusions: Two dimensional ultrasound quantification indexes and hepatic vein waveform classification in evaluating the liver fibrosis degree of patients with chronic hepatitis B has good guidance significance,which is worthy of promotion and use.

Abstract:
Objective: To explore the value of echocardiography in the diagnosis of senile calcified valvular heart disease(CVHD).Methods: The clinical data of echocardiography in 368 elderly patients were retrospectively analyzed.The incidences of CVHD between 60 to 69,70 to 79 and 80 to 89 years old patients were compared,the differences of the incidence in different parts and ages CVHD patients were analyzed.The involvement of the heart valve,hemodynamic change,cardiac enlargement,dysfunction and characteristics of echocardiography in CVHD patients were analyzed.Results: Ninty-four CVHD cases in 368 senile patients were detected by echocardiography,the total incidence of which was 25.54%.With the increasing of age,the incidence of CVHD increased(P < 0.05).The difference of the incidence of simple aortic valve CVHD in different ages was statistically significant(P < 0.01).The hemodynamic change,hypertension,coronary heart disease,diabetes and hyperlipidemia in some patients were found.Conclusions: The incidence of CVHD in elderly patients increases with the increasing of age,the major disease location is the simple aortic valve,and the aortic and mitral stenosis,reflux,heart cavity expansion,hypertrophy and cardiac dysfunction are found in more than half of patients.Echocardiography can clearly show the valve calcification and location,monitor hemodynamic change,accurately assess the heart valve,which is no injury and worthy of clinical application.
Comparison of the value of ultrasound and molybdenum-target X-ray photography in the diagnosis of breast cancer

Abstract:
Objective: To compare the value of ultrasound and molybdenum-target X-ray photography in the diagnosis of breast cancer.Methods: Each quadrant of breast in 78 patients suspected with breast cancer was detected by two-dimensional ultrasound.The morphology and distribution of blood flow of the internal and peripheral location of mass of patients,which was used to measure the resistance index(RI) value.The foot and 45° oblique positions of two papillas in the patients were detected using molybdenum-target X-ray photography,and if necessary,the side or local point could be detected using the mammography X-ray.The results between examination and clinicopathology were compared.Results: Among 78 patients suspected with breast cancer,the diagnostic accuracy rate of mammography X-ray(78.21%) was lower than that of ultrasound(89.74%) (P < 0.05).The diagnostic accuracy rate of the necrosis and extent of breast cancer of ultrasound were significantly higher than those of mammography X-ray(P < 0.01),the difference of the diagnostic accuracy rate in axillary lymph node metastasis between mammography X-ray and ultrasound was not statistically significant(P > 0.05).The diagnostic accuracy rate of calcification of ultrasound was significantly lower than that of mammography X-ray(P < 0.01).Conclusions: Ultrasound and mammography X-ray have its certain advantage in the diagnosing of breast cancer.The diagnostic methods can be selected according to actual situation,which can improve the clinical diagnosis accuracy.
The clinical significance of fetal electrocardiogram in the diagnosis of high-risk pregnancy

Abstract:
Objective: To investigate the clinical significance of fetal electrocardiogram in the diagnosis of high-risk pregnancy.Methods: The fetal electrocardiogram in 130 high-risk pregnant women with 32 to 42 gestational weeks and 60 normal pregnant women with the same gestational age were examined,the result of which was analyzed.Results: The incidence of abnormal fetal electrocardiogram in high-risk pregnant women(49.23%) was significantly higher than that in normal pregnant women(11.67%)(P < 0.01).The main manifestations of abnormal fetal electrocardiogram were tachycardia(42.19%),ST segment change(18.75%),arrhythmia(17.19%) and bradycardia(15.63%),the composition difference of abnormal fetal electrocardiogram in all cases was not statistically significant(P > 0.05).Conclusions: The periodic testing of fetal electrocardiogram in high-risk pregnant women can early find the fetal distress and hypoxia,reduce the incidence of neonatal asphyxia,and reduce the birth rate of disabled children.

Abstract:
Objective: To explore the levels IL-12,IFN-γ,IL-10 and IL-4 in peripheral blood of normal pregnant women and preeclampsia patients and its clinical significance,and investigate the role of Th1/Th2 cytokines of helper T cell in pathogenesis of preeclampsia.Methods: Thirty pregnant women with preeclampsia and 30 normal pregnant women without complications were divided into the observation group and control group,respectively.The serum levels of IL-12,IFN-γ,IL-10 and IL-4 in two groups were detected and analyzed.Results: The serum protein levels and mRNA expressions of IL-12 and IFN-γ in observation group were higher than that in control group(P < 0.01).The serum protein levels and mRNA expressions of IL-4 and IL-10 in observation group were lower than control group(P < 0.01).Conclusions: The type Th1cytokine expression increasing,the Th2 expression decreasing and proportion imbalance of Th1/Th2 may be the important causes of preeclampsia.
Identification capability and application value of automatic microorganism identification drug sensitivity analyzer for clinically relevant bacteria

Abstract:
Objective: To investigate the identification capability and application value of VITEK 2 system automatic microorganism identification drug sensitivity analyzer for clinical bacteria.Methods: One hundred and twenty-six strains of pathogenic bacteria isolated from respiratory tract,urogenital tract,pus and blood sample were selected,and 60 reference strains were served as control.The drug sensitivity of pathogenic bacteria was identified using VITEK 2 system automatic microorganism identification drug sensitivity analyzer,the correct rate,identification time and drug sensitivity of whose were analyzed.Results: The four kinds of germplasm control strains was tested for 10 times,a total of 40 times,the repetition rate of identification was 100%.Among 90 strains of gram negative bacteria,87 strains were identified,and the correct identification rate of which was 96.67%.Among 96 strains of gram positive bacteria,93 strains were identified,and the correct identification rate of which was 96.88%.The identifications of enterobacter sp,non fermentation bacteria,streptococcus sp.and micrococcus were completed within 9 h,the 85% identification of enterobacter sp.and micrococcus were completed within 6h.The coincidence rate of drug susceptibility in gram positive bacteria and gram negative bacteria were 93.94% and 93.40%,respectively,and the total coincidence rate was 93.66%.Conclusions: The correct identification rate of VITEK 2 automatic microorganism identification drug sensitivity analyzer is high,which can quickly identify and guide the rational use of antimicrobial drugs,and has important clinical value in disease diagnosis.

Objective: To explore and practice the protein and enzyme engineering three-dimensional teaching according to the cultivation of consciousness of innovation talents.Methods: The protein and enzyme engineering three-dimensional teaching model was carried out in class 2013 of biological sciences of Bengbu Medical College,the teaching effect was compared with the traditional teaching method used in class 2012.Results: The ability of independent learning,consciousness of innovation,spirit of teamwork,teaching effects and assessment of learning quality in the student of class 2013 executed three-dimensional teaching model were significantly better than those in class 2012(P < 0.01).Conclusions: The effects of three-dimensional teaching model on the effect of teaching effects,especially on autonomous learning ability,application ability and consciousness of innovation talents are better than those whose of traditional teaching,which is beneficial to the cultivation of innovative and practical medical talents.

Abstract:
Objective: To explore the teaching effects of multified teaching method guided by post competency in the teaching of ultrasonic diagnostics.Methods: One hundred and eighty-five students major in medical imaging were randomly divided into the experimental group(86 students) and control group(99 students).The experimental group and control group were taught using multified teaching method and lecture based learning,respectively.The final examination and questionnaire were used to evaluate the teaching effects of multified teaching method.Results: The scores of theory test,experiment skill and the final course grade in experimental group were significantly higher than those in control group(P < 0.01).The learning interest,and the abilities of autonomous learning,analyzing problem,solving problem,language expression,communication,clinical thinking,comprehensive quality,understanding of theoretical knowledge,classroom atmosphere and student-teacher communication in experimental group were better than those in control group(P < 0.01).Conclusions: The application of multified teaching method can improve the teaching quality of ultrasonic diagnostics,which can play an important role in cultivating the high-quality medical imaging personnel guided by post competency.

Objective: To evaluate the use of bedside blind placement of nasal-intestinal tube in enteral nutrition for critical patients.Methods: 40 critical patients were selected and divided into the blind placement of nasal-intestinal tube group(n=20) and nasogastric tube group(n=20).The occurrence of abdominal distension,diarrhea,vomiting,aspiration and hiccup complications was observed.Results: The incidence rates of abdominal distension,diarrhea,vomiting complications in nasal-intestinal tube group were lower than those of nasogastric tube group(P < 0.05).The incidence rates of aspiration and hiccup had no significant difference in both groups(P > 0.05).Conclusions: Compared with the traditional nasogastric tube,bedside blind placement of nasal-intestinal tube in enteral nutrition for critical patients can reduce the incidence of complications.

Objective: To explore the effects of the self-made Yoga intervention on the related-cancer fatigue and anxiety in breast cancer patients with chemotherapy.Methods: Sixty breast cancer patients treated with modified radical mastectomy during the 6 courses of chemotherapy were divided into the intervention group and control group using Non-simultaneous grouping method(30 cases each group).The control group were nursed with conventional method,the intervention group were additionally nursed with self-made Yoga intervention based on the conventional method.The scores of the related-cancer fatigue and anxiety between two groups were evaluated using the Zung self-rating anxiety scale(SAS) and cancer fatigue scale(CFS) at the start of the second chemotherapy and the end of the sixth chemotherapy.Results: At the start of the second chemotherapy,the differences of the CFS and SAS scores between two groups were not statistical significance(P > 0.05).At the end of the sixth chemotherapy,the decrease of CFS and SAS scores in the intervention group was significantly higher than that in control group(P < 0.01).Conclusions: The self-made Yoga intervention can effectively alleviate the related-cancer fatigue and improve anxiety in breast cancer patients with chemotherapy.
The effect of humanistic care nursing combined with functional exercise on the anxiety and bone density of patients with osteoporosis

Abstract:
Objective: To investigate the effects of humanistic care nursing combined with functional exercise on the anxiety and bone density of patients with osteoporosis.Methods: Ninety-eight osteoporosis patients treated with conventional treatment were randomly divided into the control group and observation group(49 cases each group).The control group were nursed with conventional method,the observation group were nursed with humanistic care nursing and functional exercise.The total efficiency of treatment,Hamilton anxiety score,Hamilton depression score,bone density and life quality between two groups were compared.Results: The total efficiency of treatment in observation group was higher than that in control group(P < 0.05).After treatment,the Hamilton anxiety score and Hamilton depression score in observation group were notably lower than those in control group(P < 0.01).The bone densities of femoral neck and lumbar L1 to L4,and each dimension score of life quality in observation group were significantly higher than those in control group(P < 0.01).Conclusions: The effects of humanistic care nursing combined with functional exercise on osteoporosis are good,which can significantly alleviate the anxiety and depression,increase the bone density and improve the life quality in patients with osteoporosis.
Effect of continuous nursing intervention on the negative emotion and dyssomnia in elderly patients with stroke

Abstract:
Objective: To explore the effects of continuous nursing intervention on the negative emotion and dyssomnia in elderly patients with stroke.Methods: One hundred and twelve elderly patients with stroke were randomly divided into the observation and control group(56 cases each group).The control group were nursed with routine intervention,and the observation group were nursed with continuous nursing intervention based on routine intervention.The scores of SAS and SDS before and after intervention,and the improvement of dyssomnia after intervention between two groups were compared.Results: The scores of SAS and SDS in observation group after intervention were significantly lower than before intervention and those in control group(P < 0.01).The decreasing degree of the score of PSQI in observation group after nursing intervention was significantly better than that in control group(P < 0.01).Conclusions: Continuous nursing intervention for elderly patients with stroke can effectively relieve the negative emotion and improve the sleep quality.
