Study on the LncRNA SNHG15 promoting the epithelial mesenchymal transformation of breast cancer cells by regulating miRNA-451a

Abstract:
ObjectiveTo study the effects of leng noncoding RAN small nucleolar RNA host gene 15(LncRNA SNHG15) and microRNA-451a(miRNA-451a) on the biological activity of breast cancer cells.MethodsThe expression levels of SNHG15, miRNA-451a and chemokine receptor 4(CXCR4) in drug-resistant breast cancer SKBR3-PR cells were detected using qRT-PCR.The effects of the SNHG15 and miRNA-451a inhibitors on SKBR3-PR cells migration were determined by cell scratch assay.Flow cytometry was used to detect the apoptosis, and the protein expression levels of CXCR4, epithelial mesenchymal transformation(EMT)-related proteins Vimentin and Snail were detected by Western blotting.ResultsCompared with the parent cells, the expression level of miRNA-451a decreased(P<0.05), the expression level of CXCR4 was up-regulated in drug-resistant breast cancer cell lines(P<0.01).The results of cell scratch assay showed that the SNHG15 promoted cell migration(P<0.01), and the miRNA-451a inhibited cell migration(P<0.01).The results of apoptosis assay showed that the SNHG15 inhibited apoptosis(P<0.01) and the miRNA-451a promoted apoptosis(P<0.01).After interfering SNHG15, the results of Western blotting showed that the expression levels of EMT-related proteins Snail and CXCR4 decreased, the expression levels of Snail and CXCR4 increased in miRNA-451a inhibitor group(P<0.01), and the differences of the Vimentin expression level was not statistically significant(P>0.05).ConclusionsLncRNA SNHG15 may promote the migration and apoptosis of breast cancer cells by regulating miRNA-451a, and potentially regulate CXCR4 to induce EMT of breast cancer cells.
Inhibitory effect and mechanism of novel recombinant myricetin on triple negative breast cancer

Abstract:
ObjectiveTo investigate the inhibitory effect and mechanism of novel recombinant myricetin on 4T1 cells.MethodsA new myricetin derivative was synthesized by modifying the chemical formula of myricetin.CCK-8 cell proliferation inhibition assay, and half inhibitory concentration(IC50) was calculated.The effects of novel myricetin on the migration and invasion of 4T1 cells were observed by scratch and Transwell assay.Flow cytometry was used to observe the changes of the cycle and apoptosis of 4T1 cells after the intervention of synthetic myricetin.The inhibitory effect of the novel myricetin on triple negative breast cancer(TNBC) was evaluated in the mouse tumor transplantation model in vivo.HE and TUNEL staining were used to observe the cell necrosis and apoptosis in transplanted tumor tissue sections of mice.Finally, the expression of related proteins(P53, Bcl2, Bax, Caspase 3) in 4T1 cells after intervention was detected by Western bloting.ResultsThe IC50 was 5.5 μmol/mL.Compared with DMSO group, the cell count and migration rate of 4T1 cells were decreased after treated with 5.5 μmol/mL novel myricetin for 24 h(P<0.01).Flow cytometry and TUNEL staining of tumor tissue sections showed that compared with DMSO group, the number of apoptotic cells was increased, the proportion of G1 phase cells was lower, the proportion of G2 phase cells was higher, the tumor growth volume and tumor weight were smaller, p53 and Bcl2 proteins were down-regulated, Caspase 3 was up-regulated in 5.5 μmol/mL recombinant myricetin group(P<0.05 to P<0.01).ConclusionsThe novel recombinant myricetin can induce the apoptosis and inhibit proliferation of 4T1 cells in vitro and in vivo, which may inhibit the proliferation of 4T1 cells by down-regulating p53 and induce the apoptosis of 4T1 cells through the Bcl2 mitochondrial apoptosis pathway.
Effect of vitamin C on the expression of miR126a-5p and miR155-5p in the lung tissue of sepsis mice

Abstract:
ObjectiveTo observe the inflammatory cytokines expression of lung tissue and the changes of miR126a-5p and miR155-5p in septic mice after vitamin C(Vit C) intervention, and to analyze the potential protective mechanism of Vit C in the acute lung injury in sepsis.MethodsA total of 18 male mice(4 to 6 weeks) were selected and randomly divided into the control group(CON), sepsis+NS group(SEP) and sepsis+Vit C intervention group(SEP+IR), before the injection of Vit C 1 000 mg/kg by tail vein for three consecutive days in the cecum puncture ligation model.Wet weight/dry weight(W/D) of lung tissue was measured 12 h after operation.HE staining was used to observe the pathological appearance of lung and the diameter of alveoli was measured.The changes of miR126a-5p and miR155-5p were detected by RT-qPCR, the protein expressions of Caspase 1 and Caspase 3 were detected by Western blot, and the expressions of IL-6 and TNF-α were detected by ELISA.ResultsCompared with CON group, the W/D of lung in SEP group was increased and the alveolar aperture was decreased significantly in SEP group.The expressions of miR126a-5p, miR155-5p, Caspase 1 and Caspase 3 were increased in SEP group.Meanwhile, the inflammatory cytokines IL-6 and TNF-α were further increased(P<0.05).Compared with SEP group, SEP+IR group had lower W/D, higher alveolar pore size, lower expression in miR126a-5p, miR155-5p, Caspase 1 and Caspase 3.Vit C further inhibited the increase of inflammatory cytokines IL-6 and TNF-α(P<0.05).ConclusionsVit C can reduce pulmonary edema and stabilize alveolar volume, which may be related to the decreased expressions of Caspase 1 and Caspase 3 induced by miR126a-5p and miR155-5p.
Genotype identification of Trib3 knockout rats using CRISPR/Cas9 technology

Abstract:
ObjectiveTo construct a Trib3 knockout rat model and identify the related genotype.MethodsAfter targeted knockout of Trib3 gene in rat model, the deletion of exon 3 of Trib3 gene was detected, and the genotype was identified by gene sequencing and PCR methods.ResultsThe positive F0 generation of Trib3-/+ rats was obtained, the F1 and F2 generations were obtained by self-crossing, and the F2 generation of Trib3-/- rats was selected.ConclusionsTrib3 knockout rat models are successfully prepared, and Trib3-/-, Trib3-/+ and Trib3+/+ rats are successively identified and can be stably inherited, it provides a theoretical basis for subsequent experiments.
Effect of Inhibition of LncRNA LRRC75A-AS1 on the development of lung cancer cells

Abstract:
ObjectiveTo explore the effect of LncRNA LRRC75A-AS1/miR-22-3p on the proliferation, apoptosis and migration in lung cancer cells.MethodsThe qRT-PCR method was used to detect the expressions of LRRC75A-AS1 and miR-22-3p in lung cancer tissues and adjacent tissues.Human lung cancer cells A549 were cultured in vitrol, si-NC, si-LRRC75A-AS1, si-LRRC75A-AS1 and anti-miR-NC, si-LRRC75A-AS1 and miR-22-3p inhibitor were transfected into A549 cells.The qRT-PCR method was used to detect the expressions of LRRC75A-AS1 and miR-22-3p in A549 cells.MTT assay and plate clone formation experiment were used to detect cell proliferation and clone formation ability, respectively.Flow cytometry was used to detect the apoptosis rate.Scratch test was used to detect cell migration ability.The dual luciferase reporter experiment was used to detect the targeting relationship between LRRC75A-AS1 and miR-22-3p.Western blotting was used to detect the expression of cleaved-caspase3 protein.ResultsCompared with adjacent tissues, the expression of LRRC75A-AS1 in lung cancer tissues was increased [(1.01±0.13) vs(4.59±0.34)](P < 0.01), and the expression level of miR-22-3p was decreased [(1.00±0.12)vs(0.28±0.04)](P < 0.01).Compared with the si-NC group, the cell survival rate of the si-LRRC75A-AS1 group was reduced[100%vs(54.45±2.21)%](P < 0.01), the number of clone formation was reduced[(121.00±5.10) vs(58.67±2.49)](P < 0.01), the apoptosis rate was increased[(8.42±0.34)%vs(25.59±0.80)%](P < 0.01), the protein level of cleaved-caspase3 was increased [(0.20±0.01)vs(0.68±0.05)](P < 0.01), and the rate of scratch healing was decreased [(66.71±1.43)%vs(32.56±0.95)%](P < 0.01).The dual luciferase report experiment confirmed that LRRC75A-AS1 could act as a competitive endogenous RNA for miR-22-3p.Compared with the si-LRRC75A-AS1+anti-miR-NC group, the cell survival rate of the si-LRRC75A-AS1+miR-22-3p inhibitor group was increased[(54.61±2.28)%vs(88.75±3.22)%](P < 0.01), the number of clones was increased [(57.67±2.49)vs(106.67±3.68)](P < 0.01), the apoptosis rate was decreased [(25.63±0.99)%vs(13.11±0.55)%](P < 0.01), the scratch healing rate was increased [(32.52±0.98)%vs(55.30±1.18)%](P < 0.01), and the protein level of cleaved-caspase3 was decreased[(0.67±0.06)vs(0.30±0.03)](P < 0.01).ConclusionsInhibiting the expression of LRRC75A-AS1 may reduce the proliferation, migration and clone formation ability of lung cancer cells by up-regulating miR-22-3p, which can induce cell apoptosis.
Expression of RIP1-RIP3-MLKL signaling pathway in peripheral blood monocytes of patients with acute coronary syndrome

Abstract:
ObjectiveTo investigate the expression changes of RIP1-RIP3-MLKL signaling pathway in peripheral blood mononuclear cells(PBMCs) of patients with acute coronary syndrome(ACS) and analyze the mechanism.MethodsSixty patients with ACS were enrolled, including 31 patients with unstable angina(UAP) and 20 patients with acute myocardial infarction(AMI).The control group received 19 healthy cases.Peripheral venous blood was collected from all subjects, plasma and monocytes were extracted, and interleukin(IL)-1 and IL-18 were detected by enzyme-linked immunosorbent assay(ELISA) and turbidimetry.The level of MLKL was detected by real-time PCR.The expression of RIP1, RIP3 and MLKL in PBMCs were detected by Western blotting.ResultsThe levels of IL-1, IL-18 and MLKL in plasma of 3 groups were significantly different(P < 0.01), RIP1 and RIP3 protein expression increased in PBMCs(P < 0.05), mRNA and protein expression of MLKL were also increased(P < 0.05).Compared with the UAP group, IL-1, IL-18 and MLKL were increased in peripheral blood of patients with AMI, and the protein expression of RIP1 and RIP3 were increased(P < 0.05).The expressions of MLKL mRNA and protein expression were also increased.ConclusionsThere are different degrees of inflammatory activation in UAP and AMI patients.The expression of RIP1-RIP3-MLKL signaling pathway in PBMCs gradually increases with the severity of the disease, suggesting that RIP1-RIP3-MLKL signaling pathway may play an important role in ACS.
Expression of NLRP3 in secondary hyperparathyroidism and its clinical significance

Abstract:
ObjectiveTo investigate the expression and clinical significance of NOD-like receptor family pyrin domain containing 3(NLRP3) in secondary hyperparathyroidism(SHPT).MethodsHE staining was used to observe the difference of parathyroid gland in normal people and SHPT patients.ELISA was used to detect the serum level of NLRP3 in normal people and SHPT patients.Immunohistochemistry of SP method was used to detect the expression of NLRP3 in normal and hyperplastic parathyroid tissue, and the relationship between NLRP3 expression and SHPT clinical-pathological characteristics was analyzed.ResultsHyperplastic parathyroid glands in SHPT patients manifested as diffuse and adenoma-like hyperplasia, which was obviously different from that of normal, and the serum level of NLRP3 in SHPT patients was significantly higher than that of normal people(P < 0.01).The positive rate of NLRP3 in abnormal hyperplastic parathyroid tissue in SHPT patients was significantly higher than that in normal parathyroid tissue(P < 0.01).The expression of NLRP3 in SHPT patients had no correlation with gender, age, dialysis method or dialysis age(P>0.05).ConclusionsNLRP3 may be involved in the occurrence and development of SHPT, and its high expression is expected to become an important indicator for the early diagnosis and prognostic evaluation of SHPT.
Effect of low-dose ketamine combined with dexmedetomidine on the postoperative fatigue, pain mediators and immune function in patients treated with radical cervical cancer surgery

Abstract:
ObjectiveTo observe the effects of low-dose ketamine combined with dexmedetomidine on the postoperative fatigue, pain mediators and immune function in patients treated with radical cervical cancer surgery.MethodsOne hundred and fifty patients treated with radical cervical cancer surgery were selected, and divided into the observation group(n=75) and control group(n=75) by simple randomization method.Two groups were given general anesthesia.In the control group, the 0.5 μg/kg dexmedetomidine was intravenously injected before induction of anesthesia, and then the 0.5 μg·kg-1·h-1 was infused until before 0.5 h of operation.The observation group was injected intravenously with 0.5 mg/kg ketamin before induction of anesthesia.The general conditions during the operation, hemodynamic indexes at different time points of operation, serum levels of pain mediator and immune function indexes before and after operation, visual analog scale(VAS) score after operation, fatigue scale-14(FS-14) and mini-mental state examination(MMSE) scores before and after operation and adverse reactions of anesthesia were compared between two groups.ResultsThe differences of the operation time, intraoperative blood loss, and intraoperative fluid supplement between two groups were not statistically significant(P>0.05).The average arterial pressure and heart rate in observation group at 1 hour during operation and at the end of operation were lower than those in control group(P < 0.05 to P < 0.01).The serum levels of serotonin, substance P, and serotonin E2 in observation group were lower than those in control group at the end of the operation, and after 24 h and 48 h of operation(P < 0.01).The serum levels of CD3+, CD4+ and CD4+/CD8+ in observation group at the end of the operation, and after 24 h and 48 h of operation were higher than those in the control group(P < 0.01).The VAS scale scores in observation group were lower than those in control group after 24 h and 48 h of operation(P < 0.01).After 3 d and 7 d of operation, the FS-14 scores in observation group were lower than those in control group, and the cognitive function score in observation group was higher than that in control group(P < 0.01).The difference of the incidence rate of adverse reactions of anesthesia between two groups was not statistically significant(P>0.05).ConclusionsThe application of low-dose ketamine combined with dexmedetomidine in patients treated with radical cervical cancer surgery can stabilize the hemodynamics, reduce serum pain mediator levels, reduce postoperative pain and fatigue, and improve immune function and cognitive function.
Effect of butorphanol tartrate combined with flurbiprofen ester on the postoperative analgesia in patients treated with laparoscopic hysterectomy

Abstract:
ObjectiveTo observe the effects of butorphanol tartrate combined with flurbiprofen ester on the postoperative analgesia in patients treated with laparoscopic hysterectomy.MethodsTwo hundred female patients treated with laparoscopic hysterectomy were divided into the experimental group(group PB) and control group(group F) according to the random table method.Before 30 min of the end of surgery, the group PB was intravenously injected with 20 μg/kg of butorphanol, and connected with PCIA pump(formula: 12 mg butorphanol+6 mg granisetron injection), and the group F were intravenously injected with 1 μg/kg of fentanyl, and connected with PCIA pump(formula: 0.6 mg fentanyl + 6 mg granisetron injection).The volume of analgesia pump was diluted to 100 mL in two groups.Two groups were intravenously injected with 50 mg flurbiprofen ester before surgical incision.The intraoperative hemodynamic changes in two groups were recorded.The incision pain score, visceral pain score and sedation score in two groups after 1 h(T1), 6 h(T2), 24 h(T3) and 48 h(T4) of operation were recorded.The times of adding analgesics and pressing PCIA were recorded after operation.The postoperative nausea, vomiting, skin pruritus, respiratory depression and bradycardia were also recorded in two groups.ResultsThere was no statistical significance in hemodynamic changes between two groups(P>0.05).The visceral pain scores in group PB at T1, T2 and T3 were significantly lower than those in group F(P < 0.01), and the Ramsay scores in group PB at T1, T2, T3 and T4 were higher than those in group F(P < 0.05 to P < 0.01).The times of pressing PCIA and postoperative adverse reactions in PB group were lower than those in group F(P < 0.01).ConclusionsButorphanol tartrate combined with flurbiprofen ester can significantly relieve visceral pain in patients treated with laparoscopic hysterectomy, and has a good sedative effect with a low incidence of adverse reactions.
A randomized controlled clinical study of onlay and full crown restorations for early vital cracked teeth

Abstract:
ObjectiveTo investigate the 2-year curative effect of full ceramic onlay in the treatment of early vital cracked teeth.MethodsA randomized controlled study was conducted to evaluate the 2-year effects of all-ceramic onlay and full crown on the restoration of early vital cracked teeth.One hundred and twenty-eight affected teeth from 128 patients were selected and stratified based on the clinical symptoms, layer one: occlusal discomfort; layer two: occlusal discomfort accompanied by cold and hot irritation and pain.The affected teeth in each layer were divided into the onlay group and full crown group according to the random number table method.The difference of 24 h postoperative pain was compared between two groups.All cases were followed-up for 2 years.The curative effects of the two restoration methods and clinical symptom stratification in the treatment of early vital cracked teeth with living pulp were analyzed.ResultsThe postoperative pain scores in onlay group of layer one and two were lower than that in full crown group(P < 0.01).The differences of the effective rate between two groups in layer one and two were not statistically significant(P>0.05).The effective rate of two groups in layer one was higher than that in layer two(P < 0.05).ConclusionsThe full ceramic onlay is an effective method in the restoration of early vital cracked teeth, and has little postoperative pain.Early treatment can obviously improve the effective rate of treatment.
Analysis of the lymph node metastasis and prognostic risk factors of stage T1b esophageal squamous cell carcinoma

Abstract:
ObjectiveTo explore the risk factors and prognostic independent risk factors of lymph node metastasis in stage T1b esophageal squamous cell carcinoma patients.MethodsThe clinical data of 140 stage T1b esophageal squamous cell carcinoma patients treated with radical surgery combined with lymph node dissection were retrospectively analyzed.The patients were divided into the lymph node positive group and lymph node negative group according to the postoperative pathological results, and the lymph node metastasis and prognostic factors in two groups were analyzed.ResultsAmong 140 patients, there were 20 cases in the lymph node positive group and 120 cases in the lymph node negative group, and the lymph node metastasis rate was 14.3%.The results of univariate analysis showed that the differences of the age, tumor differentiation and tumor diameter between two groups were statistically significant(P < 0.05).The results of multivariate analysis showed that the age ≥55 years, medium and low differentiation and tumor diameter ≥5 cm were the independent risk factors of lymph node metastasis(P < 0.05 to P < 0.01).During 58(3-127) months of following-up, 9 cases were lost to follow-up and 15 cases died, and the survival rate was 98.2%.The 3-year and 5-year survival rates in the stage T1b lymph node positive group and negative group were 84.7% & 73.3% and 98.2% & 96.9%, respectively.The survival rate in lymph node positive group was significantly lower than that in lymph node negative group(P < 0.01).The results of COX multivariate analysis showed that the lymph node metastasis and female were the independent risk factors of prognosis of stage T1b esophageal squamous cell carcinoma patients(P < 0.05 and P < 0.01).ConclusionsThe age≥55 years, moderate and low differentiation and tumor diameter ≥5 cm are the high-risk independent factors of lymph node metastasis in patients with stage T1b esophageal squamous cell carcinoma.Lymph node metastasis and female are the independent risk factors of prognosis of patients with stage T1b esophageal squamous cell carcinoma.
Application value of ultrasound-guided whole-process visualization of radial artery puncture and catheterization in anesthesia of obese patients

Abstract:
ObjectiveTo evaluate the clinical effectiveness of ultrasound-guided whole-process visualization of radial artery puncture catheterization in anesthesia of obese patients.MethodsOne hundred patients with body mass index ≥28 kg/m2 through arterial puncture catheterization were randomly divided into the group L(n=50, radial arterial cannulation with whole-process visible ultrasound-guided long-axis in-plane technique) and group S(n=50, radial arterial cannulation with ultrasound-guided short-axis out-of-plane technique).The success rate at first-time attempt, puncture time, puncture times and puncture complications were compared between two groups.ResultsThe success rate at first-time attempt in group L was significantly higher than that in group S(P < 0.05).In group S, 4 patients with hematoma at the puncture site were found, and the contralateral radial artery puncture were successful in all patients, no hematoma at the puncture site in group L was found, and the difference of which between two groups was statistically significant(P < 0.05).The differences of the puncture time, number of puncture and number of puncture needles between two groups were not statistically significant(P>0.05).ConclusionsThe success rate of ultrasound-guided long-axis whole-process visualization of radial artery puncture and catheterizationis higher than that of ultrasound-guided short-axis out-of-plane technique.

ObjectiveTo analyze the clinical effects of SGTB appliance in the correction of growth peak Angle class Ⅱ division 1 skeletal malocclusion using X-ray cephalometric measurement.MethodsForty cases with Angle class Ⅱ division 1 malocclusion deformity complicated with bone retraction of mandible were randomly divided into the group S and group F.The group S and group F were treated with stage Ⅰ SGTB appliance combined with stage Ⅱ fixed mandible and stage Ⅰ fixed appliance combined with stage Ⅱ Forsus appliance, respectively.The lateral head X-ray was taken before and after treatment, the Pancherz measurement analysis was used for analysis, and the related indicators in two groups were compared between before and after treatment.ResultsBefore treatment, there was no statistical significance in the measurement data of each index between the two groups(P>0.05).After treatment, the L1-MP in group F was higher than that in group S(P < 0.05).Compared with before treatment, the SNB, B/OLP, Pg/OLP, Pg/OLP+Co/OLP, ii/OLP and mi/OLP increased, and the ANB, is/OLP, U1-NSL, is/OLP-ii/OLP, ms/OLP-mi/OLP and is/OLP-A/OLP decreased in two groups after treatment(P < 0.05 to P < 0.01).Compared with before treatment, the L1-MP in group F increased, the ii/OLP-PG/OLP in group S decreased after treatment, and the differences of which were statistically significant(P < 0.05 to P < 0.01).There was no statistical significance in other indexes between before and after treatment(P>0.05).In the correction of anterior overlaying, the proportions of dental and osseous factors in group S was 43% and 57%, repectively.In group F, 56% and 44%, repectively, respectively.In the correction of molar relations, the proportions of dental and osseous factors in group S was 39% and 61%, respectively.In group F, 49% and 51%, respectively.There was no statistical significance in the constituent ratios of above indexes between the two groups(P>0.05).ConclusionsThe SGTB appliance appliance has a good effect on the correction of Angle class Ⅱ division 1 skeletal malocclusion, and is better than Forsus appliance in the control of lip tilt of lower incisor teeth.
Relationship between CD40L, CD4+/CD8+, IL-4 and the classic inflammation biomarkers for bacterial pediatric community acquired pneumonia and the predictive performance in antibacterial therapy

Abstract:
ObjectiveTo investigate the relationship between tumor necrosis factor-related activator protein(CD40L), CD4+/CD8+, interleukin-4(IL-4) and classic inflammatory biomarkers of bacterial pediatric community acquired pneumonia(CAP) and the predictive efficacy in antibacterial therapy.MethodsA total of 300 children with bacterial CAP were selected for treatment of phlegm removing, cough relieving, asthma reducing and anti-infection.The non-response cases were set as observation group(n=43) and the improved cases were set as control group(n=257).The white blood cell count(WBC), C-reactive protein(CRP), procalcitonin(PCT), CD40L, CD4+/CD8+ and IL-4 were detected before treatment.Pearson correlation analysis was used to analyze the relationship between CD40L, CD4+/CD8+, IL-4 and classic inflammation markers and the relationship between CD40L, CD4+/CD8+, IL-4.The logistic regression equation was used to analyze the relationship between CD40L, CD4+/CD8+, IL-4 and non-response treatment.The receiver operating characteristic(ROC) curve and the area under the ROC cure(AUC) were used to analyze the predictive efficacy of CD40L, CD4+/CD8+ and IL-4 of antibacterial treatment.ResultsWBC, CRP, PCT, CD40L and IL-4 in the observation group were higher than those in the control group(P < 0.01) and CD4+/CD8+ was lower than that in the control group(P < 0.01).CD40L was positively correlated with WBC, CRP and PCT(r=0.720, 0.433, 0.832, P < 0.01), CD4+/CD8+ was negatively correlated with WBC, CRP and PCT(r=-0.709, -0.449, -0.698, P < 0.01), IL-4 was positively correlated with WBC, CRP and PCT(r=0.889, 0.760, 0.723, P < 0.01).CD40L, CD4+/CD8+, IL-4 were all significantly correlated with treatment failure(P < 0.01).CD40L was negatively correlated with CD4+/CD8+, and positively correlated with IL-4(r=-0.776, 0.554, P < 0.01).CD4+/CD8+ was negatively correlated with IL-4(r=-0.538, P < 0.01).Among the single indicators, the maximum AUC of IL-4 predicting the effect of antibacterial treatment was 0.805, the AUC of each indicator combined to predict the effect of antibacterial treatment was 0.867, and the sensitivity was 79.07%, which was greater than any single indicator(P < 0.05).ConclusionsThere is a certain correlation between CD40L, IL-4, CD4+/CD8+ and classic inflammatory biomarkers, which can reflect the severity of disease in bacterial pediatric CAP to a certain extent.Dynamic monitoring will provide timely predictive value and better reference for the follow-up treatment of bacterial pediatric CAP.

Abstract:
ObjectiveTo study the correlation between the expression levels of recombinant human interferon inducible protein-10(IP-10) and CXC subfamily chemokine receptor 3(CXCR3) in diffuse large B-cell lymphoma(DLBCL) tissue, and cell proliferation, invasion and drug resistance proteins expression.MethodsSixty DLBCL patients and 30 patients with reactive lymph node hyperplasia were divided into the DLBCL group and control group, respectively.The levels of IP-10, CXCR3, cell proliferation indicator Ki67, proliferating cell nuclear antigen(PCNA), leukocyte differentiation antigen 44V6(CD44V6), matrix metalloproteinase-9(MMP-9), P-glycoprotein(P-gp) and multidrug resistance associated protein 1(MRP1) were compared between two groups.ResultsThe expression levels of IP-10, CXCR3, Ki67, PCNA, CD44V6, MMP-9, P-gp and MRP1 in DLBCL group were significantly higher than those in control group(P < 0.01).In DLBCL group, the expression levels of IP-10 and CXCR3 were correlated with the expression of cell proliferation-related proteins(Ki67 and PCNA) and tumor invasion-related proteins(PCNA and CD44V6)(P < 0.05 to P < 0.01), and not correlated with the expression of cell drug-resistance-related proteins(P-gp and MRP1)(P>0.05).ConclusionsIn DLBCL, the expression levels of IP-10 and CXCR3 increase significantly.The expression of IP-10 and CXCR3 may increase the proliferation and invasion of tumor cells, which may not be related to the drug resistance of tumor cells.

Abstract:
ObjectiveTo explore the correlation between the level of high density lipoprotein(HDL-C) in peripheral blood and inflammatory index, renal function and insulin resistance in diabetic nephropathy.MethodsAccording to the 24-hour urinary microalbuminuria excretion rate(UAER), 246 patients with type 2 diabetes mellitus were divided into the normal albuminuria group(98 cases with UAER < 30 mg/24 h), microalbuminuria group(60 cases with UAER < 300 mg/24 h) and massive albuminuria group(88 cases with UAER≥300 mg/24 h), and 92 healthy subjects were set as the control group.The differences of the expression levels of inflammatory factors, renal function index and insulin resistance index were compared among four groups, and the correlation between HDL-C and each index was analyzed.ResultsThere was no statistical significance in the levels of cholesterol among four groups(P>0.05).The levels of the fasting blood glucose(FBG), triglyceride(TG), glycosylated hemoglobin(HbA1c), interleukin-6(IL-6), tumor necrosis factor α(TNF-α), hypersensitive C-reactive protein(hs-CRP), blood urea nitrogen(BUN), blood creatinine(Scr), fasting insulin(FINS) and insulin resistance index(HOM) in control group were lower than those in other three groups, while the levels of HDL-C, glomerular filtration rate(eGFR), insulin sensitivity index(ISI) and insulin secretion index(HOMA-β) in control group were significantly higher than those in other three groups(P < 0.05 to P < 0.01).In the normoalbuminuria group, microalbuminuria group and macroalbuminuria group, the levels of the FBG, TG, HbA1c, IL-6, TNF-α, hs-CRP, BUN, Scr, FINS, HOMA-IR showed an increasing trend, and the levels of HDL-C, eGFR, ISI and HOMA-β showed a decreasing trend(P < 0.05 to P < 0.01).The levels of HDL-C in diabetic nephropathy patients were negatively correlated with the course of disease, FBG, IL-6, hs-CRP, Scr and FINS, but positively correlated with eGFR and HOMA-β(P < 0.05 to P < 0.01).ConclusionsThe HDL-C is correlated with the inflammatory response, renal function and islet function in diabetic nephropathy patients, which has high clinical guiding significance.
Effect of the FGF-21 and BMP-2 on vascular calcification of autologous arteriovenous fistula in maintenance hemodialysis patients

Abstract:
ObjectiveTo investigate the effects of fibroblast growth factor-21(FGF-21) and bone morphogenetic protein-2(BMP-2) on vascular calcification of autologous arteriovenous fistula(AVF) in maintenance hemodialysis patients.MethodsTwenty maintenance hemodialysis patients treated with internal fistula repair and 20 patients treated with primary internal fistula operation at stage 4-5 of chronic kidney disease in the Department of Nephrology of The First Affiliated Hospital of Bengbu Medical College were selected as the observation group, and 20 healthy people matched with age and gender in the health examination center were selected as the control group.The internal fistula blood vessels from establishing autogenous arteriovenous fistula operation(cephalic vein tissue of early fistula patients or arterialized venous cephalic vein of repairing fistula patients) were stained with alizarin red S to detect the presence of calcification, and calculate the calcification score for calcified stage(including severe calcification group, light and moderate calcification group and noncalcification group).The clinical data, serum biochemical parameters, and brachial and ankle pulse wave conduction velocity(baPWV) in all cases were collected.The serum concentrations of FGF-21 and BMP-2 in all cases were determined by ELISA, the immunohistochemistry was used to detect the expression levels of FGF-21 and BMP-2 in the vascular wall of internal fistula in the observation group.ResultsThe serum levels of FGF-21 and BMP-2 in observation group were significantly higher than those in control group(P < 0.01).With the aggravation of vascular calcification, the serum levels of FGF-21 and BMP-2 increased, and the expression levels of FGF-21 and BMP-2 in the immune tissue of the vascular wall with higher calcification degree also increased(P < 0.05).The levels of FGF-21, BMP-2, serum phosphorus, creatinine, parathyroid hormone and baPWV in the observation group were positively correlated with the vascular calcification of internal fistula(rs=0.850, 0.769, 0.718, 0.433, 0.441 and 0.457, respectively)(P < 0.01).ConclusionsThe levels of FGF-21 and BMP-2 are correlated with the vascular calcification of autologous arteriovenous fistula in patients with maintenance hemodialysis, which can be used as the reference for monitoring the vascular calcification of internal fistula.

Abstract:
ObjectiveTo explore the value of the variation rate of carotid peak flow in the evaluation of volumetric responsiveness in elderly patients scheduled by laparoscopic surgery.MethodsSixty-three elderly patients scheduled by laparoscopic surgery were investigated.According to the positive liquid reaction, the patients with the increase of cardiac index ≥15% was set as the reaction group(n=38), and the patients with increase of cardiac index < 15% was set as the non-reaction group(n=25).The changes of heart rate, mean arterial pressure, stroke volume variability, central venous pressure, peak velocity variability of carotid artery, peak airway pressure and heart index in two groups were analyzed at anesthesia induction 5 min, pneumoperitoneum pressure stabilization 5 min and fluid replacement 5 min.The relationship between the variation rate of carotid peak flow at pneumoperitoneum pressure stabilization 5 min, stroke volume variation, central venous pressure and cardiac index were analyzed, the receiver operating characteristic(ROC) curve of subjects was drawn, and the predictive efficacy of the index for liquid positive reaction was judged.ResultsThe heart rate and heart index in reaction group were lower than those in non-reaction group(P < 0.05 to P < 0.01).The variation of stroke volume and peak velocity of carotid artery in reaction group were higher than those in non-reaction group(P < 0.01).The variation rate of peak velocity of carotid artery at pneumoperitoneum pressure stabilization 5 min and stroke volume variability were negatively correlated with heart index(r=-0.638, -0.517, P < 0.05).The sensitivity, specificity and area under the curve of carotid peak velocity variability at pneumoperitoneum pressure stabilization 5 min were 86.96%, 87.50% and 0.877(0.758-0.958), respectively.The results of Delong test showed that the predictive efficacy of the variability rate of peak velocity of carotid artery at pneumoperitoneum pressure stabilization 5 min was higher than that of the variability of stroke volume and central venous pressure(P < 0.05).ConclusionsThe variability of peak velocity of carotid artery can effectively evaluate the volumetric responsiveness of elderly patients scheduled by laparoscopic surgery.

Abstract:
ObjectiveTo investigate the diagnostic value of eosinophil percentage(EOS%) and interferon-γ(IFN-γ) in bronchiectasis with asthma.MethodsA total of 165 patients with asthma or bronchiectasis were selected, including 48 patients with bronchiectasis combined with asthma in group A, 60 patients with asthma alone in group B, and 57 patients with bronchiectasis alone in group C.The clinical data of patients were collected, WBC, CRP, PCT, PO2, pCO2, EOS% and IFN-γ levels were measured and analyzed.Logistic regression was used to fit the IFN-γ and EOS% joint diagnosis model and draw the ROC curve to evaluate the diagnostic value.The area under the ROC curve was compared by Hanley-McNeil method.ResultsEOS% in group A and group B was significantly higher than that in group C(P < 0.01), and there was no significant difference in EOS% between group A and group B(P>0.05).IFN-γ in group A was significantly lower than that in group B and group C(P < 0.01).The sensitivity of EOS% in the diagnosis of bronchiectasis with asthma was 0.684, the specificity was 0.621;the sensitivity of IFN-γ in the diagnosis of bronchiectasis with asthma was 0.614, the specificity was 0.661.The sensitivity and specificity of IFN-γ combined with EOS% in the diagnosis of bronchiectasis with asthma were 0.754 and 0.601, respectively.ConclusionsThe sensitivity and specificity of EOS% combined with IFN-γ in the diagnosis of bronchiectasis combined with asthma are high, which has high diagnosis value.

Abstract:
ObjectiveTo investigate the clinical value of closed reduction interlocking intramedullary nails for internal fixation(IMN) in the treatment of the middle and lower tibial fractures, and its effects on the improvement of patient function, inflammatory stress, serum alkaline phosphatase(ALP), osteocalcin N-terminal middle molecule fragment(N-MID), type Ⅰ collagen carboxy-terminal peptide fragment(β-CTX) expression.MethodsEighty patients with middle and lower tibial fractures were divided into the IMN group(n=40) and control group(n=40) according to the random number table.The control group was treated with open reduction and limited-access steel plate internal fixation, and the IMN group was treated with IMN.The operative conditions, complications, excellent and good rates of ankle joint and knee joint function recovery after 6 months of surgery, serum inflammatory response indexes[tumor necrosis factor-α(TNF-α), interleukin-8(IL-8) and interleukin-6(IL-6)] before surgery, after 1 d and 3 d of surgery, stress response indicators[norepinephrine(NE), angiotensin-Ⅱ(Ang-Ⅱ) and cortisol(Cor)], and levels of ALP, N-MID and β-CTX before, after 1 week and 1 month of opertaion were compared between two groups.ResultsThe incision length, postoperative time to ground and fracture healing time in IMN group were shorter than those in control group, and the intraoperative blood loss, postoperative drainage and ratio of midsection diameter of the affected side after 24 h of operation to preoperative diameter in IMN group were lower than those in control group(P < 0.01).The incidence rate of complication in IMN group was 5.00%, which was lower than that in control group(22.50%)(P < 0.05).The serum levels of TNF-α, IL-8, IL-6, NE, Ang-Ⅱ and Cor in two groups after 1 day and 3 days of surgery increased compared with before operation, and which in IMN group were lower than those in control group(P < 0.01).After 1 week and 1 month of surgery, the levels of ALP and N-MID in two groups increased compared with before surgery, and the levels of ALP and N-MID in IMN group were higher than those in control group(P < 0.01).After 1 week and 1 month of surgery, the levels of β-CTX in two groups decreased compared with before surgery, and the level of β-CTX in IMN group was lower than that in control group(P < 0.01).The excellent and good recovery rates of ankle joint and knee joint in IMN group afetr 6 months of operation were 94.87% and 97.44%, respectively, which were higher than those in control group(74.36% and 79.49%)(P < 0.05).ConclusionsThe application of IMN in the treatment of middle and lower tibia fractures has the advantages of less bleeding, rapid postoperative recovery, fewer complications, good functional recovery, light body's inflammatory stress response and fast recovery of the levels of ALP, N-MID, β-CTX.
Value of neutrophil to lymphocyte ratio and interleukin-6 level in peripheral blood in predicting the recurrence of chronic rhino sinusitis after endoscopic sinus surgery

Abstract:
ObjectiveTo explore the clinical value of neutrophil to lymphocyte ratio(NLR)and interleukin(IL)-6 level in peripheral blood in predicting the recurrence of chronic rhino sinusitis(CRS)after endoscopic sinus surgery.MethodsEighty-seven patients with CRS treated with endoscopic sinus surgery were divided into the postoperative recurrence group(30 cases)and non-recurrence group(57 cases)according to the disease recurrence after surgery, and 40 healthy people were set as the control group at the same period.The levels of neutrophils and lymphocytes in peripheral blood in two groups were detected by automatic blood biochemical analyzer to calculate the NLR.Meanwhile, the levels of IL-6 in peripheral blood of each group were detected by enzyme-linked immunosorbent assay.The neutrophil count, lymphocyte count, NLR and IL-6 levels were compared between two groups.The receiver operating characteristic curve was used to analyze the value of single and combined peripheral blood NLR and IL-6 detection in predicting the recurrence of CRS after endoscopic sinus surgery.ResultsThe neutrophil count, NLR and IL-6 levels in CRS group were significantly higher than those in control group(P < 0.01), while the lymphocyte count in CRS group was significantly lower than that in control group(P < 0.01).The neutrophil count, NLR and IL-6 levels in recurrence group were significantly higher than those in non-recurrence group(P < 0.01), while the lymphocyte count in recurrence group was significantly lower than that in non-recurrence group(P < 0.01).The sensitivity, specificity and area under the curve of the combined detection of peripheral blood NLR and IL-6 were significantly better than those of single detection of peripheral blood NLR and IL-6(P < 0.01).ConclusionsThe combined detection of NLR and IL-6 in peripheral blood has good clinical values to predict the recurrence of CRS after endoscopic sinus surgery.
Application value of multi-parameter MRI in the diagnosis of anal fistula

Abstract:
ObjectiveTo investigate the value of different MRI sequences in the diagnosis of anal fistula.MethodsThe clinical data of 43 patients with anal fistula confirmed by surgery and pathology were retrospectively analyzed.The MRI examination in all patients were implemented before surgery.Using the surgical pathological results as the gold standard, the signs and diagnostic value of three different MRI sequences in anal fistula were compared and analyzed.ResultsIn the diagnosis of main fistula(including abscess), the accuracy of T2WI, T2WI-FS and T1WI-FS was 96.7%, 95.1% and 96.7%, respectively, and the differences of which were not statistically significant(P>0.05).In the diagnosis of anal fistula, the accuracy of T2WI and T1WI-FS enhanced sequences was 98.0% and 86.3%, respectively, and the differences of which were statistically significant(P < 0.05).In the diagnosis of secondary/branch/small fistula, the accuracy of T2WI and T2WI-FS was 76.7%, and the T1WI-FS enhancement was 95.3%, and the differences of which were statistically significant(P < 0.01).In addition, there was no statistical significance in the articulation score of three scanning sequences in the internal anal fistula and fistula(P>0.05).ConclusionsThe MRI can clearly show the main signs of anal fistula.The T2WI and T1WI-FS enhanced sequences are better for the overall anatomical structure of the lesion, and the T1WI-FS enhanced sequences can display the more subtle lesions.

ObjectiveTo investigate the diagnostic efficacy of fetal umbilical artery(UA) combined with middle cerebral artery(MCA) hemodynamics in single pregnancy complicated with vela placenta.MethodsFifty cases of single pregnancy women from November 2017 to November 2020 were set as the control group, and 50 pregnancy women with single pregnancy complicated with vela placenta were set as the observation group.The UA[including peak systolic velocity/end-diastolic velocity(S/D), resistance index(RI) and pulsation index(PI) of umbilical artery], MCA hemodynamics(RI and PI) in two groups were measured using color Doppler ultrasound.The ROC curve was drawn to analyze the diagnostic efficacy(sensitivity and specificity) of UA and MCA hemodynamics in single pregnancy complicated with vela placenta.ResultsThe levels of S/D, RI and PI in UA hemodynamics in observation group were higher than those in control group(P < 0.01).The levels of RI and PI in MCA hemodynamics in observation group were lower than those in control group(P < 0.01).The results of ROC curve showed that the diagnostic efficacy of UA combined with MCA hemodynamics in single pregnancy complicated with vela placenta was higher than that of UA or MCA hemodynamics alone(P < 0.01).The sensitivities of single UA or MCA and the combination of UA and MCA were 79.43%, 84.17% and 93.51%, respectively.The specificities of single UA or MCA and two combination of UA and MCA were 67.81%, 65.67% and 61.42%, respectively.ConclusionsIn UA hemodynamics, the expression levels of S/D, RI and PI in patients with single pregnancy complicated with vela placenta are high, while the level of MCA hemodynamics is low.Strengthening the combined determination of fetal UA and MCA can obtain the high diagnostic efficacy, which can guide the clinical treatment.

ObjectiveTo measure the angle between superior mesenteric artery(SMA) and abdominal aorta(AA)(SMA-AA angle) to calculate its 95% reference value range, and explore its correlation with fetal gestational age(GA), head circumference(HC), abdominal circumference(AC) and femoral length(FL).MethodsThe single pregnancy pregnant women diagnosed by obstetric ultrasound examination in The Fourth Affiliated Hospital of Anhui Medical University from January to April 2021 were selected according to the inclusion criteria.The SMA-AA angle in each fetus was measured, and the correlation of the angle with gestational age, head circumference, abdominal circumference and femur length were investigated.ResultsA total of 260 fetuses were included in the study.The mean value of SMA-AA angle of normal fetuses at 21-41 weeks of gestation was(57.75±7.27)°(range from 40.90° to 73.60°), and the 95% reference range was 43.50°-72.00°.There was no significant correlation between the SMA-AA angle and fetal GA, HC, AC and FL(r=0.007, 0.025, 0.022 and 0.025, respectively, P>0.05).The reproducibility of ultrasound measurement of the SMA-AA angle of normal fetuses at 12-41 weeks of gestation was good(the number of data points outside the interval of 95% agreement of inter-observers was 2.86%, and the number of data points outside the interval of 95% agreement of intraobserver was 0.04%).ConclusionsThe SMA-AA angle in normal fetus at 21-41 weeks of gestation remains relatively stable, and is not significantly correlated with fetal GA, HC, AC and FL.

Abstract:
ObjectiveTo detect the relationships between the genotype of hepatitis C virus(HCV) and severity of the disease, expression of immunocompetent cells in patients with chronic hepatitis C(CHC).MethodsFrom June 2017 to July 2018, 169 Tibetan CHC patients were selected as the study objects(CHC group), and 170 Tibetan healthly examinees in Qinghai were selected as the control group.The levels of HCV-RNA in all cases were detected using PCR fluorescence probe, the PCR reverse hybridization was used to detect the HCV genotyping, and the levels of serum interferon-γ(INF-γ) and interleukin-4(IL-4) were measured by enzyme-linked immunosorbent assay(ELISA).The relationships between the genotype distribution, and disease severity, serum levels of INF-γ and IL-4;and between disease severity, and levels of INF-γ and IL-4 were analyzed.ResultsCompared with the control group, the positive rate of HCV-RNA and serum level of IL-4 increased, and the serum level of INF-γ decreased in CHC group(P < 0.01).The positive rates of HCV-RNA in severe chronic hepatitis group and acute hepatitis group were higher than that in mild and moderate chronic hepatitis group(P<0.05).The differences of the positive rates of HCV-RNA among different genotypes and different severity of CHC were statistically significant(P < 0.01).Compared with the HCV genotype 1a, the serum levels of INF-γ in patients with genotype 2a and 3b increased(P < 0.01).Compared with the HCV genotype 1b, the serum level of INF-γ increased(P < 0.01), and the serum level of IL-4 level decreased in genotype 2a(P < 0.01).The serum level of INF-γ in genotype 3b decreased compared with the genotype 2a(P<0.05).The differences of the serum levels of INF-γ and IL-4 among different severity of CHC were statistically significant(P < 0.01).There was no statistical significance in the levels of INF-γ between mild and moderate group, and between moderate and severe group(P>0.05).The serum level of INF-γ in the mild group was higher than that in severe group, it decreased gradually in moderate group, severe group and acute group, and the differences of which were statistically significant(P < 0.01).The serum level of IL-4 increased gradually in the mild group, moderate group, severe group and acute group, and the differences of which were statistically significant(P < 0.01).ConclusionsFour subtypes of HCV(1a, 1b, 2a and 3b) are detected in Tibetan patients with hepatitis C in Qinghai area, the proportions of 1b and 2a are high.The genotype 1b is closely related to the severity of the disease, the level of INF-γ decreases, and the level of IL-4 increases.
Analysis on the current situation and influencing factors of health literacy of undergraduate in a medical college

Abstract:
ObjectiveTo understand the level and influencing factors of health literacy of undergraduate in a medical college, and provide the reference for the targeted intervention of health literacy of college students.MethodsTaking the undergraduates of a medical college as the research object, the stratified cluster sampling method was adopted, and the Chinese Citizens' Health Literacy Questionnaire in 2009 was used to investigate.ResultsThe overall health literacy rate of the medical college students was 16.5%, and the rates of knowledge and concept of health literacy, healthy lifestyle and behavior, health skills were 29.3%, 5.4% and 63.0%, respectively.The results of the analysis of influencing factors on health literacy score showed that the total score and three dimensions score of health literacy in clinical students were higher than those in nursing and general practice students(P < 0.01).With the increasing of grade, the total score of students' health literacy, and scores of knowledge and concept of health literacy, healthy lifestyle and behavior and health skills showed an increasing trend(P < 0.01).The overall score of health literacy, healthy lifestyle and behavior score in female students were higher than those in male students(P < 0.05 and P < 0.01).The total level of health literacy, scores of knowledge and concept of health literacy, scores of healthy lifestyle and behavior in Han students were higher than those in Hui and Man students(P < 0.01).With the increasing of the average monthly income of students' families, the overall level of health literacy and score of health literacy knowledge concept showed an increasing trend(P < 0.05).The differences of the effects of only child, place of origin, parents' education level and monthly living expenses on the health literacy score of college students were not statistically significant(P>0.05).ConclusionsThe level of the health literacy of medical college students needs to be further improved, the school should strengthen the cultivation of health literacy to improve the level of health literacy of medical students.
Study of Morisky scale on medication compliance and influencing factors of elderly patients with hypertension in community

Abstract:
ObjectiveTo investigate the medication compliance of elderly hypertension patients in the community and put forward feasible suggestions for the health management of hypertension patients.MethodsA total of 120 elderly hypertension patients over 60 years old in Yijishan community from July to September 2020 were selected.Information was collected through questionnaires and interviews.One hundred and nineteen cases were effectively surveyed, but 1case was invalid.The Morisky scale questionnaire was used to evaluate patients' medication compliance, and the influencing factors of medication compliance were analyzed.ResultsAmong the 119 patients, 52 patients(43.7%) had good medication compliance, 67 patients(56.3%) had poor medication compliance, among which 23 cases(34.33%)had poor compliance due to forgetting to take medication, followed by 21 cases(31.34%)who stopped medication without taking medication because of symptom improvement, and 23 cases(34.33%)who stopped medication and did not pay attention to taking medication because of conscious symptom worse.Age, times of medication, residence status and blood pressure monitoring were the main influencing factors for low compliance(P < 0.05).Logistic regression analysis showed that living alone and having no blood pressure monitoring increased the risk of decreased medication compliance by 16.9% and 10.4%.ConclusionsMedical staffs and pharmacists should strengthen the publicity of the relevant knowledge of hypertension patients, carry out systematic health education for patients, improve their compliance, and improve the standardization of hypertension treatment and blood pressure control level.

Abstract:
ObjectiveTo summarize the overall project establishment and topic selection in various directions and fields of the innovation and entrepreneurship training program among medical college students("innovation program"), and to comprehensively explore the implementation and topic selection.MethodsTwo hundred and fourty-three national innovation programs of Shanghai Jiao Tong University School of Medicine from 2008 to 2021 were classified.The number of projects in different years and students' characteristics were compared.The topics were grouped into categories and subcategories, and the percentage of topics in each category and its change with years were analyzed.ResultsStudents from five-year education system, Chinese class and majoring in clinical medicine had the largest number in participating in the innovation program.The percentage of basic research projects was much higher than that of clinical research and translational research.And the percentage of basic research projects increased year by year.According to more detailed classification, the percentage of both pathogenesis research and modeling/diagnosis research increased.Conclusions"Innovation program" for medical students is conducive to help students choose their majors and better promote the development of medical science.

Abstract:
ObjectiveTo explore the effects of psychological intervention based on timing theory on the psychological state and self-efficacy of patients with chronic periodontitis.MethodsEighty patients with chronic periodontitis were selected and divided into two groups according to the time order of admission.Forty patients from May 2018 to May 2019 and 40 patients from June 2019 to May 2020 were divided into the control group and observation group, respectively.The control group was given the routine nursing intervention, and the observation group was given psychological intervention based on timing theory.The disease knowledge mastery, psychological state[state-trait anxiety questionnaire(STAI) and center for epidemiologic studies depression scale(CES-D)], periodontal index[gingival bleeding index(BI), periodontal pocket depth(PD), plaque index(PLI) and attachment level(CAL)], self-efficacy, periodontal health(OHIP-14) points and nursing satisfaction were compared between the two groups.ResultsAfter intervention, the levels of BI, PD, PLI and CAL in observation group were lower than those in control group(P < 0.01), and the mastery of health knowledge in observation group was significantly higher than that in control group(P < 0.01).After intervention, the scores of STAI and CES-D in observation group were lower than those in control group(P < 0.01), the self-efficacy in observation group was higher than that in control group(P < 0.01), and the OHP-14 scores of physiological disorders, psychological disorders and physical pain in observation group were lower than those in control group(P < 0.01).After intervention, the nursing satisfaction in observation group was better than that incontrol group(P < 0.01).ConclusionsThe targeted psychological guidance based on timing theory can inhibit the negative psychological feelings of patients with chronic periodontitis, improve the level of health knowledge, enhance the sense of self-efficacy, help to improve the periodontal index of patients, promote the oral health and improve the nursing satisfaction.
Application of frontier management intervention flow chart in rigid endoscope management

ObjectiveTo explore the application effect of frontier management intervention flow chart in rigid endoscope management.MethodsThrough the analysis of the current situation, the frontier management intervention flow chart for the cleaning and disinfection of rigid endoscope was formulated.A total of 1 100 rigid endoscopes before the implementation of the frontier management intervention flow chart from June to August 2020 were selected as the control group, and 800 rigid endoscopes after the implementation of the frontier management intervention flow chart from October to December 2020 were set as the intervention group.The qualification status of rigid endoscope management, nursing management status and doctor's satisfaction in the two groups were compared.ResultsThe qualification rate of cleaning, sterilization, packaging, storage and distribution of endoscopes in intervention group was significantly higher than that in control group(P<0.01).The scores and total scores of nurse's endoscope knowledge and hand hygiene knowledge after the implementation of the frontier management intervention flow chart were significantly higher than those before the implementation(P<0.01).The hand hygiene compliance rate and standardization rate after the implementation of the frontier management intervention flow chart were significantly higher than those before the implementation(P<0.01).After the implementation of the frontier management intervention flow chart, the doctor's satisfaction with distribution, packaging, cleaning and disinfection was better than that before the implementation(P<0.05 to P<0.01).ConclusionsThe implementation of the frontier management intervention flow chart effectively improves the management effect of rigid endoscope.

Abstract:
ObjectiveTo analyze the effects of process decentralization strategy combined with secondary triage mode on the service quality of emergency patient triage in waiting management.MethodsFrom May 2018 to June 2019, 860 patients were randomly selected from the emergency department of Beijing Luhe Hospital Affiliated, and divided into the control group and observation group(430 cases in each group).The control group was given the routine triage mode, and the observation group was given the process decentralization strategy combined with secondary triage mode.The waiting time and accuracy of triage in two groups were observed.The patients' waiting anxiety was evaluated using the HAMA score.The self-designed satisfaction questionnaire was used to calculate the patient satisfaction, and the adverse events were recorded in the two groups.ResultsThe waiting time in observation group was shorter than that in control group, the triage accuracy in observation group was higher than that in control group, the HAMA score and incidence rates of adverse events in observation group were lower than those in control group, and the total satisfaction rate in observation group was higher than that in control group(P<0.01).ConclusionsThe process decentralization strategy combined with secondary triage mode in the application of emergency patient triage in waiting management can significantly improve the patients' anxiety, shorten the waiting time, reduce the incidence rate of adverse events and improve the quality of triage services, and it has higher application value.
Application effects of the perioperative comprehensive thermal insulation management in patients treated with posterior spinal surgery

Abstract:
ObjectiveTo analyze the effects of comprehensive thermal insulation management on the perioperative hypothermia, stress response and complications of patients treated with posterior spinal surgery.MethodsSixty patients treated with posterior spinal surgery under general anesthesia were divided into the conventional group and comprehensive group(30 cases in each group)according to theorder of operation time.In the conventional group, the operating room temperature was maintained at 23 ℃, and the non-operating area was covered with a segmented surgical blanket.In the comprehensive group, the periodic room temperature, skin disinfectant heating, intravenous infusion heating and inflatable heating blanket were adopted.The intraoperative blood loss, intraoperative infusion volume, waking up time, leaving the operating room time, core temperature at the time of entering the operating room, endotracheal intubation, after 1 h of skin resection and end of the operation, postoperative levels of blood glucose(GLU) and C reactive protein(CRP), and occurrence of shivering, hypothermia, pressure injury and wound infection in two groups were recorded.ResultsThe intraoperative blood loss and infusion volume of patients in comprehensive group were significantly less than those in conventional group(P<0.01), and the time of awakening and leaving the operating room in comprehensive group were significantly shorter than those in conventional group(P<0.01).There was no statistical significance in the core temperature between two groups at entering the operating room and after endotracheal intubation(P>0.05).The core temperature in conventional group at 1 h after skin excision and the end of operation were significantly lower than that in comprehensive group(P<0.01).There was no statistical significance in the levels of GLU and CRP between two groups before operation(P>0.05).The levels of GLU and CRP in two groups significantly increased after operation, but which in the comprehensive group were significantly lower than those in the conventional group(P<0.01).The total incidence rate of postoperative complications in the comprehensive group was 3.33%, which was significantly lower than that in conventional group 30.00%(P<0.01).ConclusionsThe perioperative comprehensive thermal insulation management for patients treated with posterior spinal surgery can effectively stabilize their intraoperative core body temperature, reduce the intraoperative blood loss and transfusion volume, shorten the time of patients waking up and leaving operating room, relieve the surgical stress response, and reduce the incidence of postoperative complications.
