Effect of TR3 receptor agonist 6-mercaptopurine on NF-κB p65/CylinD1 pathway in diabetic ApoE-/- mice, and its relationship with anti-arteriosclerosis

Abstract:
ObjectiveTo observe the effects of TR3 receptor agonist 6-mercaptopurine(6-MP) on the glucose and lipid metabolism and NF-κB p65/CylinD1 pathway in streptozotocin (STZ)-induced diabetic apolipoprotein E knockout(ApoE-/-) mice, and explore the possible mechanism of TR3 receptors in improving diabetic atherosclerosis.MethodsForty ApoE-/- mice were randomly divided into the ApoE-/- group, STZ-ApoE-/- group, STZ-ApoE-/- +6-MP5(5 mg·kg-1·d-1) and STZ-ApoE-/- +6-MP10(10 mg·kg-1·d-1) group(10 rats each group).The 6-MP intraperitoneal injection was implemented, once a day, for 8 weeks.The diabetic animal model was established using intraperitoneal injection of STZ.The levels of blood glucose and blood lipid were determined using the blood glucose test strip and the enzymic method or selective homogeneous method, respectively.The Western-blotting was used to detect the protein levels of NF-κB p65/CylinD1 in thoracic aorta tissue, the lipid deposition in the thoracic aortic intima was observed by oil red O staining, and the patch area in the thoracic aorta was measured using HE staining.ResultsCompared with the ApoE-/- mice group, the levels of blood glucose, triglyceride(TG), total cholesterol(TCHO) and low-density lipoprotein-cholesterol(LDL-C) significantly increased in STZ-ApoE-/- group(P < 0.05), and the level of TR3 receptor agonist 6-MP decreased the levels of plasma glucose and lipid with dose-dependently.Comapred the STZ-ApoE-/- group, the levels of the blood glucose and TG, LDL-A and TC significantly decreased in the STZ-ApoE-/- +6-MP5 group(P < 0.05).Compared with the ApoE-/- group, the levels of NF-κB p65 and CylinD1 remarkably increased in the STZ-ApoE-/- group(P < 0.05).The 6-MP reduced the expression level of NF-κB p65/CylinD1 protein in thoracic aorta of diabetic ApoE-/- mice in a dose-dependent manner, and the protein level of NF-κB p65/CylinD1 in STZ-ApoE-/- +6-MP5 group were significantly lower than that in STZ-ApoE-/- group(P < 0.05).The differences of the levels of NF-κB p65/CylinD1 in thoracic aorta between STZ-ApoE-/- +6-MP10 group and STZ-ApoE-/- +6-MP5 group were statistically significnat(P < 0.05).The results of oil red O staining showed that 6-MP inhibited the intima lipid deposition in the thoracic aorta of STZ-ApoE-/- mice in a dose-dependent manner.The results of HE staining showed that 6-MP reduced the area of intima plaque and formation of vacuoles(lipids) inside plaque, and inhibited the proliferation of smooth muscle layer around plaque and plaque base in the thoracic aorta of STZ-ApoE-/- mice in a dose-dependent manner.Compared with the STZ-ApoE-/- group, the plaque area in the STZ-ApoE-/- +6-MP5 group significantly reduced(P < 0.05), and further reduced in the STZ-ApoE-/- 6-MP10 group(P < 0.01).ConclusionsThe TR3 receptor agonist 6-MP can significantly decrease the formation of atherosclerosis in diabetic ApoE-/- mice, which may be related to the improvement of glycolipid metabolism and inhibition of NF-κB p65/CylinD1 signaling pathway.
Preparation of paclitaxel long-circulating nanomicelles and their multidrug resistance to breast cancer

Abstract:
ObjectiveTo prepare paclitaxel long-circulating micelle TPGS-PEG2000-DSPE-PTX and investigate the effects of paclitaxel micelles on proliferation, apoptosis, migration and invasion of paclitaxel-resistant cells (MCF-7/PR) in breast cancer.MethodsThe paclitaxel-loaded TPGS-PEG2000-DSPE-PTX micelles were prepared by membrane dispersion method.The morphology, particle size and potential of the drug-loaded micelles were characterized..The sulfonyl rhodamine B method, flow cytometry and migration were performed.Sulfonylrhodamine B staining, Annexin V/PI staining, scratch and transwell assays were used to detect the effects of paclitaxel micelles on proliferation, apoptosis, migration, and invasion of paclitaxel resistant cell lines.ResultsPaclitaxel-loaded micelles were successfully constructed.The results of transmission electron microscopy showed that the drug-loaded micelles became spherical, with a particle size of 30-40 nm and a zeta potential of -5.4 mV.The UV spectrophotometer detected and calculated the drug loading amount to 25.37%.Compared with the control group and the paclitaxel group, the TPGS-PEG2000-DSPE-PTX group effectively inhibited the proliferation, migration and invasion of drug-resistant cells, induced apoptosis, up-regulated the protein expression levels of caspase3 and Bax(P < 0.01), and down-regulated Bcl-2, P-gp and MRP protein expression levels(P < 0.01).ConclusionsThe successful construction of TPGS modified paclitaxel long-circulating micelle delivery system can effectively exert anti-tumor effect and reverse multi-drug resistance in vitro, and provide a reference for the development of a new anti-cancer drug delivery system.
Role of glucose-regulated protein 78 in the invasion and metastasis of ovarian cancer

Abstract:
ObjectiveTo investigate the role of glucose-regulated protein 78(GRP78) in the invasion and metastasis of ovarian cancer.MethodsThe expression levels of GRP78 in 60 ovarian serous carcinoma and 40 ovarian serous cystadenoma tissues were detected using the immunohistochemistry(IHC) and Western blotting.The relationship between the clinical indicators of ovarian serous carcinoma and GRP78 expression was analyzed.The invasion and migration ability of HO-8910 cells and HO-8910PM cells were detected using Transwell assay.The invasion and migration ability of HO-8910 cells and HO-8910PM cells treated with shRNA-GRP78 interference and pcDNA-GRP78 overexpression were detected, respectively.ResultsThe results of IHC showed that the positive expression rate of GRP78 in ovarian serous carcinoma tissues(90%) was significantly higher than that in ovarian serous cystadenoma tissues(25%)(P < 0.01).The results of Western blot showed that the expression level of GRP78 in ovarian serous carcinoma tissue was significantly higher than that in ovarian serous cystadenoma tissue.Among the ovarian serous carcinoma patients with high GRP78 expression level, the clinicopathological grade and stage and carbohydrate antigen 125 level were higher, and the proportion of patients with lymph node metastasis, pelvic metastasis, and external pelvic metastasis was higher(P < 0.01).The results of in vitro studies showed that the expression level of GRP78 in HO-8910PM cells with high metastatic potential was significantly higher than that in HO-8910 cells with low metastatic potential(P < 0.01).The overexpression of GRP78 in HO-8910 cells could promote the invasion and migration ability of the cells, while silencing GRP78 expression in HO-8910PM cells could significantly reduce the invasion and migration potential of the cells(P < 0.01).ConclusionsGRP78 can promote the invasion and migration potential of ovarian cancer cells.
Study on the effect of different porosity of 3D printing porous titanium implants on bone ingrowth

Abstract:
ObjectiveTo investigate the effects of different porosity of 3D printing porous titanium implants on bone ingrowth through animal experiments.MethodsEighteen healthy New Zealand rabbits were randomly divided into A, B and C groups.Three kinds of porosity porous titanium plates in A, B and C groups prepared by 3D printing technology were 35%, 55% and 75%, respectively.The titanium plate was implanted into the femoral shaft plate area of rabbits.After 4 weeks and 16 weeks of operation, the femoral shaft specimens were examined using X-ray, and the gross and histological observation in bone tissue section were implemented.The rates of new bone formation in three groups were compared.ResultsAfter 4 weeks of operation, the fibrous tissue was found around the titanium plate and between titanium plate and bone surface, and no obvious callus formation was observed in each group.The results of X-ray films showed that some gaps were identified between titanium plate and the bone surface.After 16 weeks of operation, there were new bone connections around titanium plate and between titanium plate and bone surface in each group, a few titanium plates were wrapped by bone scabs, and the results of X-ray films showed that the titanium plate was closely attached to the bone surface.The results of histological observation showed that some new bone formation could be seen at the edge of titanium plate, but only a small amount of bone tissue grew into the middle pore in group A; In group B, some bone tissue grew into the pores, but there was a certain gap between pores and titanium plate; In group C, the density of new bone between pores of titanium plate was higher, and it was closely bound to titanium plate.The differences of the rates of new bone formation among three groups were statistically significant(P < 0.01), the rate of new bone formation in group A was significantly lower that in B and C groups(P < 0.01), and the rate of new bone formation in group B was significantly lower that in group C(P < 0.01).ConclusionsThe pore structure of 3D printing porous titanium plate can allow new bone to grow in.Different porosities affect the effect of new bone ingrowth of 3D printed titanium implants The 75% porosity is more beneficial to bone ingrowth compared with 35% and 55% porosity.The printing porous titanium plates can achieve good bone fusion with proper treating cortical bone surface.
Study on the mechanism of miR-125b targeting HK2 expression to enhance radiosensitivity of breast cancer MCF-7 cells

Abstract:
ObjectiveTo investigate the effects of miR-125b targeting HK2 expression on the radiosensitivity of breast cancer MCF-7 cells.MethodsThe miR-125b mimics and its negative control were transfected into breast cancer MCF-7 cells, and divided into the miR-125b group and miR-NC group, respectively.The expression levels of miR-125b and HK2 mRNA in MCF-7 cells were detected using RT-PCR, and the expression of HK2 protein was detected by Western blotting.The targeting relationship between miR-125b and HK2 was detected using double luciferase reporter gene assay.The mimic of miR-125b and pcDNA3.1-HK2 plasmid were co-transfected into MCF-7 cells(miR-125b+HK2 group), and the survival fraction and apoptotic rate of cells in three groups were detected by clone formation assay and flow cytometry.ResultsAfter transfecting miR-125b mimics, compared with the miR-NC group, the expression level of miR-125b in MCF-7 cells increased(P < 0.01), and the expression levels of HK2 mRNA and protein decreased(P < 0.01).The results of double luciferase reporter gene assay confirmed that the HK2 was a potential target gene of miR-125b.Compared with the miR-NC group, the survival fraction and apoptotic rate of cells in miR-125b group decreased and increased, respectively(P < 0.01).Compared with the miR-125b group, the survival fraction and apoptotic rate in miR-125b + HK2 group significantly increased and decreased, respectively(P < 0.01).ConclusionsMiR-125b can enhance the radiosensitivity of breast cancer MCF-7 cells by targeting HK2 expression.
Comparison of the efficacy between LAPG and LATG in the treatment of Siewert Ⅱ and Ⅲ AEG

Abstract:
ObjectiveTo compare the clinical efficacy between laparoscopic-assisted total gastrectomy(LATG) and laparoscopic-assisted proximal gastrectomy(LAPG) in the treatment of Siewert Ⅱ and Ⅲ esophagogastric adenocarcinoma(AEG).MethodsThe clinical data of 62 patients with type Ⅱ and type Ⅲ AEG were retrospectively analyzed.Thirty-two cases treated with LAPG and 30 cases treated with LATG were divided into the LAPG group and LATG group, respectively.The operation time, number of lymph nodes dissected, blood loss during operation, recovery time of gastrointestinal function, postoperative hospital stay, perioperative and postoperative complications were compared between two groups.The survival, quality of life and CEA situation in two groups were recorded.ResultsThe differences of the intraoperative bleeding volume, TNM staging, recent complications and level of serum CEA after 12 months of operation between two groups were not statistically significant(P>0.05).The number of lymph nodes in LATG group[(22.9±6.7)] was more than that in LAPG group[(19.7±4.6)](P < 0.01), the operation time in LATG group was significant longer than that in LAPG group(P < 0.01), and the incidence rate of reflux esophagitis in LAPG group(40.0%) was more than that in LAPG group(3%)(P < 0.05).ConclusionsThe LAPG and LATG in treating Siewert type Ⅱ and Ⅲ AEG are safety.The lymph node dissection of LATG is more, and the long-term complications of LATG is fewer.The progressive phase AGE should be treated with LATG.
Predictive value of NT-proBNP and heart rate variability in readmission rate of patients with chronic heart failure

Abstract:
ObjectiveTo investigate the predictive value of NT-proBNP and heart rate variability(HRV) in readmission rate of patients with chronic heart failure(CHF).MethodsA total of 179 patients with CHF were grouped according to the cardiac functional grading.The levels of NT-proBNP and ambulatory electrocardiograph in all cases were detected, the readmission rates were recorded after 1, 3 and 6 months of discharge, and the predictive value of the HRV indexes combined with NT-proBNP level in readmission rate of each group were analyzed.ResultsThe differences of SDNN, RMSSD, PNN50 and HRV triangular index among patients with different cardiac functional grades were statistically significant(P < 0.01).The levels of NT-proBNP and HRV indexes in patients with grade Ⅲ and Ⅳ CHF were significantly higher than those in patients with grade Ⅱ CHF(P < 0.01), and which in patients with grade Ⅳ CHF was significantly higher than that in patients with grade Ⅲ CHF(P < 0.01).The readmission rate in patients with grades Ⅱ, Ⅲ, and Ⅳ CHF increased significantly with the time increasing after 1, 3 and 6 months of discharge(P < 0.01), and the readmission rate at the same time increased with the increasing of heart failure grade(P < 0.01).ConclusionsThe HRV indexes combined with NT-proBNP level have important value in predicting the readmission rate of patients with CHF.
Clinical study on contrast agent extravasation in acute ischemic stroke patients after machinery thrombectomy

Abstract:
ObjectiveTo explore the reasons of contrast agent extravasation in acute ischemic stroke patients after mechanical thrombectomy, and analyze its correlation with prognosis.MethodsForty acute large vessel occlusion patients with successful re-opened of offending vessels after thrombectomy were divided into the non-contrast agent extravasation group, exudative contrast agent extravasation group and hemorrhagic contrast agent extravasation group.The from onset to revascularization time(OTR), from puncture to revascularization time(PTR), number of intraoperative pulling bolt, volume of extravasated contrast agent, time of absorption, postoperative 24 h score of national institute of health stroke scale(NIHSS) and postoperative 3 months score of modified Rankin scale(mRS) among three groups were compared.ResultsThe differences of the OTR, PTR, 24 h NIHSS score, ASPECT score and 3-month mRS score were statistically significant among three groups(P < 0.01).The volume of contrast agent exudation in exudation group was lower than that in bleeding group(P < 0.01), while the difference of the total absorption time of contrast agent, number of successful pulling bolt recanalization between two groups were not statistically significant(P>0.05).ConclusionsShortening the length of hospital stay and outpatient of patients with ischemic stroke, knowing surgical technique and reducing the number of pulling plug can reduce the probability of postoperative contrast agent extravasation and cerebral hemorrhage transformation, and improve the prognosis.
The clinical analysis of 210 cases of triple-negative breast cancer

Abstract:
ObjectiveTo investigate the clinicopathologic characteristics, prognosis and its risk factors of triple-negative breast cancer(TNBC).MethodsThe clinical data of 210 patients with TNBC(observation group) and 70 non-TNBC patients(control group) from September 2010 to September 2015 were retrospectively analyzed.ResultsThe lymph node metastasis rate was related to the tumor size, and the larger the tumor size, the higher the lymph node metastasis rate was(P < 0.01).Compared with the coincidence rate of pathological diagnosis of ultrasound or mammography, the coincidence rate of combined diagnosis was the highest(97.46%)(P < 0.01), and the coincidence rates of ultrasound and mammography were 84.76% and 70.0%, respectively(P < 0.01).The pathological histological classification in observation group was higher than that in control group(P < 0.01).Prognostic analysis showed that the recurrence rate and survival rate in observation group(42.38%, 74.29%) were higher and lower than those in control group(28.57%, 85.71%) respectively(P < 0.05).The larger the tumor size, the higher the lymph node metastasis rate was(P < 0.01).The survival rate in patients with negative lymph node(86.00%) was higher than that in patients with positive lymph node(65.45%)(P < 0.01).ConclusionsCompared with the non-TNBC, the tumor size is large, the age of patient is young, the pathological histological classification is high, the relapse and metastasis are ease, and the prognosis is poor in TNBC.Ultrasonography and mammography are the effective methods in auxiliary diagnosis of TNBC, and the correct rate of combination of both methods is higher.Larger tumor size and lymph node metastasis are the risk factors of TNBC death.
Analysis of influencing factors of early post operative urine control after laparoscopic radical prostatectomy

Abstract:
ObjectiveTo detect the influencing factors of early postoperative urine control after laparoscopic radical prostatectomy.MethodsA total of 65 prostate cancer patients treated with radical prostatectomy were selected.The general features, tumor, operation and recovery of early urinary control in all patients were retrospectively analyzed, and the influencing factors of early postoperative urinary control were investigated.ResultsThe rate of early urine control in all patients was 49.23%.The results of univariate analysis showed that the age, body mass index, neoadjuvant therapy, prostate volume, tumor staging, blood loss, retaining complete bladder neck and postoperative leakage were correlated with the recovery of early postoperative urinary control, and the differences of those were statistically significant(P < 0.05 to P < 0.01).The results of logistic regression analysis showed that the age, neoadjuvant therapy, amount of bleeding and retaining complete bladder neck were the independent influencing factors of early postoperative urine control, and the differences of those were statistically significant(P < 0.05 to P < 0.01).ConclusionsMultiple factors can affect the early postoperative urine control after laparoscopic radical prostatectomy.Early urinary control can benefit from the appropriate operation timing, preoperative neoadjuvant endocrine therapy, intraoperative precise operation to reduce blood loss and side injury of hemostatic operation, retaining complete bladder neck and urethra as long as possible.
Application study of apical preservation combined with guided bone regeneration in jaw cyst surgery

Abstract:
ObjectiveTo explore the clinical effects of periapical curettage preservation combined with Bio-oss bone meal/Haiao artificial biomembrane in jaw cyst surgery.MethodsFifty-five patients with jaw cyst were divided into the group A(15 cases) and group B(40 cases).The group A was treated with the periapical curettage preserving the root tip, bone meal covering the lack of part of the root exposed to jaw and biofilm covering.The group B was treated with simple cyst excision combined with apicoectomy.All patients were followed up for more than 12 months.The recovery of tooth function, bone regeneration and reconstruction of jaw defects between two groups before and after operation were compared.ResultsAfter operation, the normal function recovery in cyst involved teeth in group A was faster than that in group B(P < 0.01).The osteogenic effect in bone graft area in group A was better than that in autogenous healing area.ConclusionsThe efficacy of apical preservation combined with guiding bone regeneration in jaw cyst excision is good and reliable.
Effect of biliary stent implantation combined with intrabile duct perfusion chemotherapy in the treatment of advanced extrahepatic cholangiocarcinoma

Abstract:
ObjectiveTo explore the effects of biliary stent implantation combined with intrabile duct perfusion chemotherapy in the treatment of advanced extrahepatic cholangiocarcinoma(EHCC).MethodsThe clinical data of 86 advanced EHCC patients treated with percutaneous extrahepatic biliary stenting(PTBS group) or PTBS combined with intraductal perfusion chemotherapy(PTBS combined with intraductal perfusion chemotherapy group) were retrospectively analyzed.The improvement of liver function, complications and postoperative survival were analyzed.The influencing factors of overall survival(OS) were analyzed using Kaplan-Meier curve and Cox regression model.ResultsCompared with before operation, the serum levels of ALT, AST and TBIL significantly reduced after 1, 3 and 6 months of operation(P < 0.05), the decreasing degree of which in PTBS combined with intraductal perfusion chemotherapy group was more obviously than that in PTBS group, and the increasing degree of ALB level in PTBS combined with intraductal perfusion chemotherapy group was more obviously than that in PTBS group.The difference of the incidence rate of postoperative complication between two groups was not statistically significant(P>0.05).The median survival time and 1-year survival rate in PTBS group were 7 months and 11.3%, respectively.The median survival time and 1-year survival rate in PTBS combined with intraductal perfusion chemotherapy group were 10 months and 37.5%, respectively.Log-rank test suggested that the PTBS combined with intraductal perfusion chemotherapy could improve the prognosis, and prolong the survival of EHCC patients.Cox regression analysis showed that the PTBS combined with intraductal perfusion chemotherapy was an independent protective factor for prognosis(HR=0.46, 95%CI:0.23-0.92, P < 0.05).ConclusionsCompared with the simple PTBS, the PTBS combined with intraductal perfusion chemotherapy can improve the prognosis, and prolong the survival of EHCC patients.The PTBS combined with intraductal perfusion chemotherapy has good treatment effect, which is worthy of further clinical exploration and research.

ObjectiveTo investigate the effects of gestational weight gain and pre-pregnancy body mass index(BMI) on obstetric outcome, and guide reasonable weight gain during pregnancy.MethodsThe clinical data of 3 190 term singleton live births pregnant women were retrospectively analyzed.The patients were divided into the low weight group, normal weight group and overweight group based on the pre-pregnancy BMI, and the below standard group, reach standard group and exceed standard group according to the weight increasing during pregnancy.The clinical parameters among different groups were compared to analyze the influence of the pregestational body mass index and pregnancy body gain on obstetric outcome.ResultsThe pregnant women with standard(BMI) accounted for 68.5%, and the cesarean section rate, forceps midwifery rate, macrosomia birth rate, neonatal asphyxia rate, the first and second stages of labor in overweight group were significantly higher than those in low weight group and normal weight group.The cesarean section rate, forceps midwifery rate, macrosomia birth rate and neonatal asphyxia rate in overstandard group were significantly higher than those in reach standard group and exceed standard group.The incidence rate of low body mass children in low standard group was significantly higher than that in reach standard group and exceed standard group(P < 0.05).ConclusionsExcessive BMI before pregnancy and excessive pregnancy weight gain can negatively affect on obstetric outcomes.Women of child-bearing age should be guided to maintain normal pre-pregnancy weight and reasonable pregnancy weight gain so as to improve obstetric outcomes.

Abstract:
ObjectiveTo investigate the effects of raltitrexed, and raltitrexed combined with oxaliplatin or irinotecan on transaminase in patients with advanced colorectal cancer.MethodsForty-eight patients with advanced colorectal cancer were divided into group A(13 case) group B(23 cases) and group C(12 cases) according to the chemotherapy regimen.The A group, B group and C group were treated with raltitrexed monotherapy, chemotherapy of raltitrexed combined with oxaliplatin and raltitrexed combined with irinotecan, respectively.The effects of three regimens on the levels of AST and ALT in patients with advanced colorectal cancer were analyzed.ResultsAfter three groups were treated with three or more cycles of chemotherapy, the proportions of AST in normal range in group A, B and C were 53.85%, 86.96% and 83.33%, respectively, the proportions of ALT in normal range in group A, B and C were 92.31%, 91.30% and 100%, respectively, and the differences of the AST and ALT grading among three groups were not statistically significant(P>0.05).The differences of the ALT in patients with liver metastasis among three groups were not statistically significant(P>0.05), and the level of AST in group C were higher than that in group A and group B(P < 0.01 and P < 0.05).The levels of AST and ALT in patients without liver metastasis of group B and group C were lower than those of group A(P < 0.05 to P < 0.01).The differences of the ALT and AST in patients with different primary lesions among three groups were not statistically significant(P>0.05).ConclusionsThe raltitrexed-based second-line treatment can be used as an effective palliative treatment in patients with advanced colorectal cancer, which is worthy of further promotion.
Correlation analysis of preoperative outdoor activity status and prognosis of lumbar spinal stenosis patients treated with decompression surgery

Abstract:
ObjectiveTo investigate the correlation between the level of outdoor activity before operation and efficacy after 3 and 12 months of decompression in patients with lumbar spinal stenosis(LSS).MethodsThe clinical data of 99 LSS patients treated with surgical decompression were retrospectively analyzed.According to the outdoor activities before operation, the patients were divided into the outdoor activity group(44 cases with regular participating in high intensity or moderate level of outdoor activities) and non-outdoor activity group(55 cases with mild outdoor activity or inactive sedentary state).The demographic data and prognosis after 3 and 12 months of operation in two groups were analyzed, and the correlation between two groups was studied.ResultsCompared with the outdoor activity group, the daily smoking rate and non-insulin dependent type 2 diabetes mellitus in non-outdoor activity group were higher(P < 0.05).The postoperative effects in two groups were better(P < 0.01), the improvement of VAS score and ODI index of leg pain in non-outdoor activity group were significantly lower than those in outdoor activity group(P < 0.05), and the difference of the treatment effect after 12 months of operation was not statistically significant(P>0.05).The results of multivariate analysis showed that the preoperative low back pain in patients with depression, BMI for 25.0-29.9, BMI for 30.0-34.9 and BMI for ≥ 35.0 kg/m2 were more severe(P < 0.05).The preoperative dysfunction and quality of life in patients with outdoor activities were lighter and higher, respectively(P < 0.05 to P < 0.01).The ODI index in LSS patients with outdoor activities were significantly improved after 12 months of decompression surgery(P < 0.05).ConclusionsThe early recovery time in LSS patients without outdoor activities before lumbar decompression surgery is long, but the desired outcome can be achieved after 12 months of surgery.
Clinical value and prognostic factors of PTBS in the treatment of malignant obstructive jaundice

Abstract:
ObjectiveTo compare the clinical value and prognostic factors between ultrasound-guided percutaneous transhepatic cholangial drainage(PTCD) and percutaneous transhepatic biliary stent(PTBS) in the treatment of malignant obstructive jaundice(MOJ).MethodsThe clinical data of the MOJ patients treated with palliative care in The First Affiliated Hospital of Bengbu Medical College from May 2014 to March 2018 were retrospectively analyzed, and divided into PTCD group and PTBS group according to the different treatment methods.The improvement of liver function, changes of neutrophil-lymphocyteratio(NLR), platelet-lymphocyte ratio(PLR) and monocyte-lymphocyte ratio(MLR), complications and postoperative survival situation were analyzed in two groups.The influencing factors of postoperative overall survival(OS) in two groups were analyzed using Kaplan-Meier curves and Cox proportional hazard model.ResultsA total of 69 patients were investigated, which included 30 cases in PTCD group and 39 cases in PTBS group, 36 males and 33 females, and aged(70.7±5.6) years.The levels of ALT, AST, TBil and DBil in two groups after 1 month of operation significantly decreased compared before operation(P < 0.05).Compared with the PTCD group, the levels of DBIL, NLR, MLR and PLR in PTBS group after 1 month of operation significantly decreased(P < 0.01), and the better postoperative survival in the PTBS group was found(P < 0.05).There was no statistical significance in the postoperative complications between two groups(P>0.05).The results of Cox multivariate proportional hazard model showed that the higher preoperative TBiL, MLR and CA19-9 were the independent risk factors of influencing the prognosis of patients, and the PTBS treatment was an independent protective factor.ConclusionsCompared with PTCD alone, PTBS can improve the prognosis and prolong the survival time of MOJ patients.Preoperative higher TBiL, MLR, and CA19-9 are the independent risk factors of influencing the prognosis of patients.

Abstract:
ObjectiveTo compared the effects between pre-pregnancy laparoscopic cervix cerclage and McDonald cervical cerclage in the treatment of cervical incompetence.MethodsFifty patients with cervical incompetence were divided into the pre-pregnancy group(24 cases treatment with laparoscopic cervix cerclage) and pregnancy group(26 cases treatment with McDonald's cervical cerclage).The operation, and maternal and fetal outcomes between two groups were compared.ResultsNo conversion to open laparotomyànd operative complication occurred in two groups.The hospital stays and intraoperative blood loss in pre-pregnancy group were lower than those in pregnancy group(P < 0.05), and the difference of operation time between two groups was not statistically significant(P>0.05).The prenatal pregnancy and preterm birth rates, and full-term delivery rate in pregnancy group were lower and higher than those in pregnancy group, respectively(P < 0.05), while the difference of the late abortion rate between two groups was not statistically significant(P>0.05).There were 23 live births of neonates, and all were cesarean section in pre-pregnancy group.There were 22 live births of neonates, 11 cases were cesarean section, and 11 cases were natural labour in pregnancy group.The average gestational week and neonatal body mass in pregnancy group were lower than those in pre-pregnancy group(P < 0.05 to P < 0.01), and the rates of neonatal asphyxia and transfer NICU in pregnancy group were higher than those in pre-pregnancy group(P < 0.05).ConclusionsPre-pregnancy laparoscopic cervix cerclage in the treatment of cervical incompetence is short hospital stay and less bleeding during operation.It can reduce pregnancy spuc, and improve maternal and infant outcomes.It has good clinical effect, and is worth popularizing and applying.

ObjectiveTo investigate the effect of different renal vein management methods on right renal carcinoma radical operation under retroperitoneoscope.MethodsRetrospective analysis of the clinical data of 35 patients with right renal cancer who underwent retroperitoneal laparoscopic radical resection of right renal carcinoma:the renal artery was severed near the lip of the kidney(group A) and the renal artery was severed near the right lateral wall of the inferior vena cava (group B).ResultsThe operative time of group A was better than that of group B in terms of intraoperative blood loss and drainage tube placement time, with statistically significant differences(P < 0.01).There was no statistically significant difference in postoperative bed time and postoperative hospital stay between the two groups(P>0.05).ConclusionsIn the radical surgery of right renal carcinoma under retroperitoneal laparoscope, the separation of the right renal artery near the lip of the kidney is more convenient than that near the right wall of the inferior vena cava, which saves the operation time, reduces the surgical trauma and improves the surgical safety.This method is worthy of reference.
Clinical observation of NPWT combined with silver ion hydrogel dressing in the treatment of diabetic foot ulcer

Abstract:
ObjectiveTo explore the effects of negative pressure wound therapy(NPWT) combined with silver ion hydrogel dressing in the treatment of diabetic foot ulcer.MethodsOne hundred and one patients with diabetic foot ulcer were randomly divided into the silver ion hydrogel dressing group(group A), NPWT group(group B) and NPWT combined with silver ion hydrogel dressing(group C).The wound granulation growth integral, wound healing time, antibiotic use time, and time and cost of each change were compared among three groups.ResultsThe wound healing time in group A, B and C gradually shortened(P < 0.05 to P < 0.01).The duration of antibiotic use in group C was shorter than that in group A and group B(P < 0.05 to P < 0.01), and the difference of which between group A and group B was not statistically significant(P>0.05).The differences of granulation growth integral among three groups at the first week were not statistically significant(P>0.05), the differences of granulation growth integral between group A and group B, and between group B and group C at the second and third week were not statistically significant(P>0.05), and the granulation growth integral in group C was lower than that in group A(P < 0.05 to P < 0.01).The time and number of dressing change in group B and group C were significantly lower than those in group A(P < 0.01), and the difference of which between group B and group C was not statistically significant(P>0.05).The differences of the cost of dressing change between group A and group B, and between group B and group C were not statistically significant(P>0.05), and the cost of dressing change in group A was higher than that in group C(P < 0.05).ConclusionsNPWT combined with silver ion hydrogel dressing can promote the growth of granulation tissue, shorten the healing time of diabetic foot ulcer wound, reduce the use time of antibiotics, and do not increase the workload of medical staff and treatment cost.

ObjectiveTo analyze the collagen type Ⅳ α 5 chain(COL4A5) gene, provide the basis for molecular diagnosis and genetic counseling of a family with X-linked hereditary Alport syndrome, and investigate the COL4A5 gene shift mutation spectrum and clinical phenotype of Alport syndrome.MethodsThe whole exome sequencing was used for gene detection, and Sanger sequencing was used to validate candidate pathogenic variants.ResultsThe renal damage of patient progressed gradually, the gross hematuria and nephrotic syndrome appeared.The proteinuria and gross hematuria could be relieved after anti-infection treatment.The pathological biopsy of the kidney of the proband showed that the thickness of the basement membrane varied, the thickness of the dense layer increased, and parts were torn or cobweb.The results of high-throughput sequencing and first-generation sequencing indicated the exon 49 of COL4A5 gene, c.4518_c.4519 heterozygous variants of insC(p.gln1507profs *14) in patient was found, and neither parent carried the mutation.ConclusionsIt is the first report that the new variation of COL4A5 gene at this locus is the pathogenesis of patient, which enrichs the spectrum of COL4A5 gene variation, and provides the basis for the molecular diagnosis and prenatal diagnosis of this family of X-linked hereditary nephritis.It is of great significance to guide the reproduction of mother and daughter in the family, and prevent the disease through prenatal genetic diagnosis or preimplantation genetic diagnosis.

Abstract:
ObjectiveTo observe the levels of norepinephrine(NE) and interleukin-6(IL-6) during the stage Ⅱ, Ⅲ and Ⅳ hand foot mouth disease(HFMD) children infected by enterovirus 71(EV71), and investigate the correlation of levels of NE and IL-6 with EV71 infection.MethodsThe differences of the clinical presentation and laboratory examination among stage Ⅱ, Ⅲ and Ⅳ HFMD children infected by EV71 and healthy children were compared.The levels of NE and IL-6 in different period were detected, and the correlation of levels of NE and IL-6 with disease progression were analyzed.ResultsWith the progression of HFMD, the proportions of EV71 infection children with duration 72 h of high fever, poor spirit, vomiting, irritability, dysphasia, rapid heart rate, capillary refill time >2 s, white blood cell >15×109/L and glucose >8.3 mmol/L gradually increased(P < 0.05 to P < 0.01).The levels of NE and IL-6 in EV71 infection stage Ⅱ, Ⅲ and Ⅳ children were significantly higher than those in healthy children, and which gradually increased with the increasing of EV71 infection staging(P < 0.01).The results of Pearson correlation analysis showed that the level of IL-6 was positively correlated with NE level in EV71 infection children(P < 0.01).ConclusionsThe levels of NE and IL-6 in HFMD increasing with the progression of EV71 infection staging may be an important factor for the disease progression after EV71 infection, which hints that clinicians can evaluate the children's condition by analyzing the levels of NE and IL-6.

ObjectiveTo investigate the value of B type natriuretic peptide(BNP) combined with C reactive protein(CRP) in the evaluation of chronic obstructive pulmonary disease(COPD) complicated with cardiac insufficiency.MethodsSixty COPD patients complicated with cardiac insufficiency were divided into group A(left ventricular dysfunction) and group B(right cardiac insufficiency), and 53 COPD patients without cardiac insufficiency were set as the control group.The levels of BNP and CRP were measured in three groups.ResultsThe level of LVEF in group A was lower than that in control group(P < 0.01), and the left ventricular diameter in group A was higher than that in control group(P < 0.01).The pulmonary artery pressure and right ventricular diameter in group B were higher than those in control group(P < 0.01).The levels of BNP and CRP in group A and group B were significantly higher than those in control group(P < 0.01), and the level of BNP in group A was higher than that in group B(P < 0.01).The levels of BNP and CRP in group A and group B after treatment were significantly lower than those before treatment(P < 0.05 to P < 0.01).The levels of BNP and CRP in group A were negatively correlated with LVEF level(P < 0.05), and the levels of BNP and CRP in group B were positively correlated with pulmonary artery pressure(P < 0.05).ConclusionsThe levels of BNP and CRP can help to identify the cardiac insufficiency in COPD patients.

Abstract:
ObjectiveTo compare the clinical effects between retroperitoneal laparoscopic modified single-layer adrenalectomy and traditional three-layer adrenalectomy in adrenal mass resection.MethodsForty-three patients with adrenal masses were randomly divided into the observation group(22 cases) and control group(21 cases).The observation group was treated with retroperitoneal laparoscopic modified single-layer adrenalectomy.The adrenal central vein was found from the loose and no blood vessel plane of the dorsal psoas major, and the adrenal gland was quickly removed after the central vein was treated.The control group was treated with retroperitoneal laparoscopic conventional three-layer adrenalectomy.The total operation time, time of finding central adrenal vein, amount of intraoperative blood loss, time of removing drainage tube and length of hospital stay were compared between two groups.ResultsThe retroperitoneal adrenalectomy in 43 cases with adrenal mass were successful, and no open surgery was performed.The total operation time, time of finding central adrenal vein, amount of intraoperative blood loss and length of hospital stay in observation group were less than those in control group(P < 0.05 to P < 0.01).The difference of the time of removing drainage tube between two groups was not statistically significant(P>0.05), and no serious complications occurred in two groups.ConclusionsBoth surgical methods are safe and effective.The retroperitoneal laparoscopic modified single-layer adrenalectomy can quickly find and treat the adrenal central vein in the early stage, and the anatomical layer is clear.

ObjectiveTo investigate the efficacy and safety of the local injection of methotrexate guided by ultrasound combined with oral mifepristone in the treatment of cesarean scar pregnancy.MethodsFifty-four patients with cesarean scar pregnancy were randomly divided into the control group and observation group.The control group was treated with oral mifepristone, and the observation group was treated with the local injection of methotrexate guided by ultrasound combined with oral mifepristone based on the control group.At the same time, two groups were treated with cesarean curettage.The clinical therapeutic effect, adverse reactions and effective rate of treatment between two groups were statistically analyzed.ResultsThe time of hospitalization, time of hCG turning negative, time of disappearance of internal lesions and time of menstrual recover in observation group were better than those in control group(P < 0.01), and the intraoperative blood loss in observation group was less than that in control group(P < 0.01).The incidence rate of total adverse reactions, such as palpitation, nausea and abdominal pain, in observation group was lower than that in control group(P < 0.01), and the effective rate of treatment in observation group was higher than that in control group(P < 0.05).ConclusionsThe local injection of methotrexate guided by ultrasound combined with oral mifepristone in the treatment of cesarean scar pregnancy can effectively improve the clinical efficacy, and reduce the incidence rate of adverse reactions, which is worthy of promotion in clinical practice.
Application value of the anterolateral tibial approach in the treatment of Pilon fracture

ObjectiveTo evaluate the clinical effects of the anterolateral tibial approach in the treatment of Pilon fracture.MethodsA total of 28 patients with Pilon fracture were randomly divided into the anterior lateral approach group(group A) and traditional medial approach group(group B)(14 cases each group).The incision length, operation time, intraoperative blood loss, postoperative drainage, fracture healing time, fracture reduction, postoperative complications and postoperative ankle function between two groups were compared.ResultsThe differences of the anatomical reduction rate, operation time and fracture healing time between two groups were not statistically significant(P>0.05).The incision length in group A was significantly shorter than that in group B, and the intraoperative bleeding and postoperative drainage volume in group A were significantly lower than those in group B(P < 0.01).The differences of the excellent rate of joint function and incidence rate of complications between two groups were not statistically significant(P>0.05).The Barid-Jackson score in group A was significantly higher than that in group B(P < 0.01).ConclusionsThe anterolateral tibial approach in the treatment of Pilon fracture except to severe crushing and collapse of the inner side of the articular surface can achieve satisfactory efficacy and promote the patients to recover as soon as possible, which is superior to the traditional medial and lateral approach.

ObjectiveTo investigate the effects of ultrasound-guided percutaneous microwave ablation in the treatment of thyroid benign nodules on the levels of thyroid hormones and safety of patients.MethodsOne hundred and seventy-five patients with benign thyroid nodules were divided into the observation and control group.The observation group was treated with ultrasound-guided percutaneous microwave ablation, and the control group was treated with traditional surgery.The levels of thyroid hormone before and after operation and postoperative complications between two groups were compared, and the effects of different treatments on the thyroid function and incidence rate of complications were analyzed.ResultsBefore treatment, the differences of the age, course of disease, number of nodules, and levels of TSH, FT3 and FT4 between two groups were not statistically significant(P>0.05).After treatment, the level of TSH, and levels of FT3 and FT4 in control group were higher and lower than those before treatment, respectively(P < 0.01), and the level of TSH, and levels of FT3 and FT4 in control group were significantly higher and lower than those in observation group, respectively(P < 0.01).The total incidence rate of postoperative complications in observation group and control group were 6.1% and 31.2%, respectively, and the difference of which was statistically significant(P < 0.01).ConclusionsThe effect of ultrasound-guided percutaneous microwave ablation in the treatment of thyroid benign nodules is better than that of traditional surgery, and its postoperative complication is less.It is an effective alternative to traditional surgical treatment.

ObjectiveTo study the occurrence and risk factors of dislocation after total hip arthroplasty(THA) for osteoarthritis.MethodsA total of 266 patients with osteoarthritis were treated with THA, and followed up for 6 months.The occurrence of dislocation of the hip joint was recorded.The patients with postoperative dislocation of the hip joint were set as the dislocation group, and the patients without dislocation were set as undislocation group.The surgical indicators and medical record in two groups were analyzed using univariate and logistic regression analysis to explore the independent risk factors of postoperative dislocation in patients treated with THA.ResultsAmong 266 patients with osteoarthritis, 25 cases with dislocation after THA were found, and the dislocation rate was 9.40%.The differences of the age, artificial femoral head diameter, history of hip surgery, postoperative blood loss, anterior angle of femoral prosthesis and postoperative femoral eccentricity between two groups were statistically significant(P < 0.05 to P < 0.01), and the differences of the gender, body mass index, surgical approach, combined diabetes mellitus, operative time, postoperative transfusion rate, postoperative drainage volume and operative side between two groups were not statistically significant(P>0.05).The results of multivariate logistic regression analysis showed that age >75 years, female, diameter of artificial femoral head < 30 mm, history of hip surgery, posterolateral approach and anteversion of femoral prosthesis >10° were the independent risk factors of dislocation after aosteoarthritis treated with THA(P < 0.05).ConclusionsThe hip joint with osteoarthritis is prone to dislocation after THA, the age >75 years, female, artificial femoral head diameter < 30 mm, hip surgery history and posterior lateral approach can increase the risk of dislocation.

Abstract:
ObjectiveTo observe the effects of sufentanil combined with ropivacaine on epidural anesthesia in the second cesarean section, and its influence on fetus.MethodsForty puerperas with the second cesarean section were randomly divided into the sufentanil-ropivacaine group(group S) and ropivacaine group(group R)(20 cases each group).The mixture of 10 mL of 0.75% ropivacaine with 10 μg of sufentanil was injected into the epidural space in group S, and the 10 mL of 0.75% ropivacaine was injected into the epidural space in group R.The onset time of anesthesia, duration of analgesia, adverse reactions of anesthesia, and Apgar scores of neonates after 1 and 5 min of birth between two groups were compared.The blood gas analysis of umbilical vein and artery was performed at newborn birth, and the oxygen saturation(SpO2) of right radial artery was compared among at 1, 2, 3, 4 and 5 min after birth.ResultsThe onset time of anesthesia, duration of analgesia and anesthetic effect in group S were significantly shorter, longer and better than those in group R, respectively(P < 0.01 or P < 0.05).The difference of the adverse reaction of anaesthesia between two groups was not statistically significant(P>0.05).The differences of the Apgar scores of neonates after 1 and 5 min of birth, and SpO2 of right radial artery at 1, 2, 3, 4 and 5 min after birth between two groups were not statistically significant(P>0.05).The differences of the indexes of blood gas analysis in umbilical vein and artery between two groups were not statistically significant(P>0.05).ConclusionsThe application of sufentanil combined with ropivacaine in epidural anesthesia of puerpera with the second cesarean section can enhance the effects of anesthesia, improve the quality of anesthesia, and has no obvious effect on fetal respiratory oxygenation.

Abstract:
ObjectiveTo compare the clinical efficacy of simple posterior reduction and decompression internal fixation with bone graft fusion in the treatment of lumbar spondylolisthesis among different age groups.MethodsForty patients with lumbar spondylolisthesis were divided into the group A(aged 26 to 55 years old) and group B(aged 55 to 77 years old).Two groups were treated with simple posterior reduction and decompression internal fixation with bone graft fusion.ResultsForty cases were followed up for 6 to 30 months.The slip reduction rate in group A was higher than that in group B(P < 0.05).The differences of the operation time, blood loss, postoperative VAS score, postoperative JOP score, false joint formation rate, dural sac injury rate, intervertebral infection rate and incision hematoma rate between two groups were not statistically significant(P>0.05).The differences of VAS score and JOP score between two groups at the last follow-up were not statistically significant(P>0.05).ConclusionsFor patients aged ≤ 55 years old, the complete reduction of the spondylolisthesis should be achieved.The patients aged >55 years old may not be completely reduced, but the nerve root relaxation and no compression must be guaranteed.
Analysis of clinical application of the precision attachment denturein in free end defects cases

Abstract:
ObjectiveTo analyze the prosthese effect of the precision attachment denture in the free end defects cases.MethodsThirty-eight patients with molar missing were selected.They were randomly divided into two groups.The patients who used the precision attachment denture were selected as observation group.While the patients choosed the clasp attachment denture were in the control group.After 12 months, the clinical situations and satisfaction of the patients were compared.ResultsThe excellent and good rates of masticatory efficiency and food impaction in the observation group were higher than in the group(P < 0.05).The satisfaction rate of observation group was 94.74%, and which of the control group was 68.42%.The satisfaction rate of observation group was higher than that in control group(P < 0.05).ConclusionsThe precision attachment dentures in the molar missing cases has high satisfaction and good clinical effect.

Abstract:
ObjectiveTo investigate the clinical analysis of extraperitoneal laparoscopic radical prostatectomy on local prostate cancer by different surgical approaches.MethodsThe clinical data of 87 patients with local prostate cancer undergoing extraperitoneal laparoscopic radical prostatectomy were retrospectively analyzed.Among them, 28 cases underwent three-port ELRP and 59 cases underwent four-port ELRP.The clinical factors as ages, tPSA, prostate volume, clinical stage, hypertension, operative time, estimated blood loss, pathological scores, positive surgical margin rates, biochemical recurrence and drainage tube keeping days.ResultsAll the patients underwent successful surgery without turn to open surgery.During the three-port group and four-port group, operative time were (90.56±17.87) min and(118.66±22.45) min, the estimated blood loss were(121.45±45.76)mL and (189.87±75.43) mL, prostate volume were (36.75±12.26) mL and (67.47±22.65) mL respectively, with significant statistical difference(P < 0.05).Positive surgical margin were 2 cases and 4 cases, of which underwent the endocrine therapy, with no statistical difference(P>0.05).These was no statistical difference in the other factors(P>0.05).Follow-up 3 to 17 months after surgery, biochemical recurrence in 6 cases, to endocrine control satisfaction.ConclusionsCompared with the four-port ELRP, the three-port ELRP not only has not increased complicaiton, but also is more minimally invasive.It is worthy of clinical application for small and localized prostate cancer.

ObjectiveTo optimize the extraction process of ginkgolic acid from the seedcoat of Ginkgo biloba L.MethodsThe ginkgolic acid was extracted from the seedcoat of Ginkgo biloba using alkaline extraction and acid precipitation.The single factor test and orthogonal experiment were applied.The content of ginkgolic acid was set as parameter, and determined using high performance liquid chromatography.The effects of solid-liquid ratio, alkali concentration, alkali extraction time, acid precipitation pH and acid precipitation time on extraction process were investigated.ResultsThe ratio of solid to liquid for 1:5, alkali concentration for 3%, alkali extraction time for 1 h were the optimum extraction conditions.ConclusionsThe optimized extraction process of ginkgolic acid using alkaline extraction and acid precipitation is simple, stable, easy to operate, and has good extraction repeatability, and suitable to extract ginkgolic acid from the seedcoat of Ginkgo biloba.

Abstract:
ObjectiveTo construct a health communication platform, and investigate its application value in patients with coronary heart disease(CHD) complicated with diabetes mellitus after percutaneous coronary intervention(PCI).MethodsFifty-four patients with CHD complicated with diabetes mellitus were divided the control group(26 cases from January 2017 to February 2018) and observation group(28 cases from March 2018 to June 2019).The control group and observation group were nursed with routine means and health communication platform, respectively.The biochemical indicators, Seattle angina scale(SAQ) score and incidence of adverse cardiac events and result of a six-minute walk were analyzed in two groups.ResultsThe SAQ score in observation group was higher than that in control group(P < 0.01), the incidence rate of adverse cardiac events in observation group was lower than that in control group(P < 0.05), and the result of a six-minute walk in observation group was better than that in control group(P < 0.01).ConclusionsThe application value of health communication platform in patients with CHD complicated with diabetes after PCI is significant, and it is worthy of clinical promotion.

Abstract:
ObjectiveTo investigate the effects of time-based nursing strategy on the improvement of motor function and quality of life in cerebral aneurysm patients with interventional therapy.MethodsSixty cerebral aneurysms patients with interventional therapy were randomly divided into the control group and observation group(30 cases each group).The control group was given the routine clinical nursing intervention, while the observation group was given the time-based nursing strategy on the basis of the control group.The motor function and quality of life before and after nursing intervention were compared between two groups.ResultsBefore intervention, the difference of the Fugl-Meyer Assessment of Motor Function(FMA) score between two groups was not statistically significant(P>0.05).After intervention, the FMA scores in two groups were improved, and the FMA score in observation group was significantly better than that in control group(P < 0.01).After 6 months of intervention, the score of each item of Short Form 36 Questionnaire in observation group was significantly better than that in control group(P < 0.01).ConclusionsThe time-based nursing strategy can actively improve the motor function and quality of life in cerebral aneurysm patients with interventional therapy.

Abstract:
ObjectiveTo study the making and using methods of modified restraint band, and explore its effects on reducing the occurrence of unplanned extubation and adverse events in severe patients.MethodsEighty eligible hospitalized patients were conveniently selected, and randomly divided into the observation group and control group(40 cases each group).The patients were evaluated using the "ICU Patient Physical Constraint Assessment Scale", and the severe patients were binded with physical restraints.The improved breathable restraint band and conventional sponge restraint band were used in the observation group and control group, respectively.After the restraint ended, the restraint effects, occurrence rate of adverse events, and nurses' satisfaction with constraint were compared between two groups.ResultsA case with unplanned tubing in observation group and 5 cases with unplanned tubing in control group were found, and the difference of which between two groups was not statistically significant(P>0.05).The incidence rate of adverse events in observation group was less than that in control group(P < 0.01), and the nurses' satisfaction in observation group was higher than that in control group(P < 0.01).ConclusionsThe improved breathable restraint band can effectively reduce the incidence rate of unplanned tubing, occurrence rate of adverse events and workload of nurses, and improve the satisfaction of nurses.
Effect of Roy's adaptive nursing model on the perioperative psychological resilience, coping style and quality of life in patients with angle-closure glaucoma

Abstract:
ObjectiveTo observe the effects of Roy's adaptive nursing model on perioperative psychological resilience, coping style and quality of life of patients with angle closure glaucoma.MethodsSixty patients with angle closure glaucoma were randomly divided into the control group and observation group(30 cases each group).The observation group were treated with Roy's adaptive nursing intervention until discharge, and the control group were treated with routine ophthalmic nursing.The Self-rating Anxiety Scale(SAS), Self-rating Depression Scale(SDS), Connor-Davidson Resilience Scale, Medical Coping Style Questionnaire(MCMQ) and MOS item short from health survey(SF-36) were used to evaluate the degree of anxiety and depression, psychological resilience, coping style and quality of life of all patients.The differences of the SAS score, SDS score, psychological resilience, coping style and SF-36 were compared between two groups before and after intervention.ResultsThe scores of SAS and SDS in observation group after intervention were lower than those in control group(P < 0.05), the scores of resilience, self-improvement, optimism and total score of psychological toughness in observation group were higher than those in control group(P < 0.01), the face score in observation group was higher than that in control group(P < 0.05), the avoidance and yield scores in observation group were lower than those in control group(P < 0.05), and the score of each item of SF-36 in observation group were higher than those in control group(P < 0.01).ConclusionsRoy's adaptive nursing model can reduce the intensity of perioperative stimulus source, promote adaptive response, and improve patients'psychological resilience, coping style and quality of life in patients with angle closure glaucoma.
Observation on the effect of continuous nursing based on humanistic care on home recuperation of patients undergoing arteriovenous internal fistula

ObjectiveTo analyze the intervention effect of humanistic care-based continuous nursing on patients with arteriovenous internal fistula at home.MethodsThirty patients in the control group were given routine nursing intervention, and 30 patients in the observation group were given humanistic care-based continuous nursing on the basis of routine nursing intervention.After 2 months of intervention, the self-management ability, internal fistula related knowledge acquisition status, negative emotion and the loss of function in the initial stage of internal fistula were compared between the two groups.ResultsThe score of self-rating anxiety scale and self-rating depression scale in the observation group were significantly lower than control group(P < 0.01).The incidence rate of weak internal fistula tremor and hematoma/bleeding in the observation group were lower than those in the control group(P < 0.05).Complications management, disease surveillance, diet management, the score of medication compliance and operation technique in the observation group were higher than those in the control group(P < 0.05 to P < 0.01).ConclusionsOn the basis of routine nursing, the implementation of continuous nursing based on humanistic care can significantly improve the internal fistula related knowledge acquisition and self-management ability at home for patients with arteriovenous internal fistula, relieve their negative emotion, reduce the occurrence of loss of function in the initial stage, which is worthy of clinical attention.
